Treadmill walking has been used as a surrogate for overground walking to examine how load carriage affects gait. The validity of using treadmill walking to investigate load carriage's effects on stability has not been established. Thirty young adults were randomized into 3 front-loaded groups (group 1: 0%, 2: 10%, or 3: 20% of bodyweight). Participants carried their load during overground and treadmill walking. Dynamic gait stability (primary outcome) was determined for 2 gait events (touchdown and liftoff). Secondary variables included step length, gait speed, and trunk angle. Groups 1 and 2 demonstrated similar stability between walking surfaces. Group 3 was less stable during treadmill walking than overground (P ≤.005). Besides trunk angle, all secondary outcomes were similar between groups (P >.272) but different between surfaces (P ≤.001). The trunk angle at both events showed significant group- and surface-related differences (P ≤.046). Results suggested that walking with an anterior load of up to 10% bodyweight causes comparable stability between surfaces. A 20% bodyweight front load could render participants less stable on the treadmill than overground. Existing online gait event detection algorithms mostly rely on add-on sensors, limiting their practicality. It successfully recognized the majority of cross-steps, with a detection success rate of 94%. However, some misclassifications or missed events occurred, mainly due to the complexity of cross-step events. AME for heel strikes (HS) during native gait and cross-step events averaged at 78 ms and 64 ms respectively, while toe off (TO) AME were 126 ms and 111 ms respectively. A statistically significant difference in the algorithm's success rate score in detecting gait events during cross-step intervals was observed across various walking speeds in a sample of 12 healthy participants, while there was no significant difference among groups. Conclusion: The proposed algorithm represents an advancement in gait event detection on instrumented treadmills. By leveraging the CoP signal, it successfully identifies gait events and cross-steps in the simulated real-time processing operation, providing valuable insights into human locomotion. A symmetric gait pattern in humans reflects near-identical movement in bilateral limbs during walking. However, little is known about how gait symmetry changes on different inclines. This study aimed to address this knowledge gap using the central pattern generator and internal model hypotheses. Eighteen healthy young adults underwent five 2-minute walking trials (inclines of +15%, +8%, 0%, −8%, and −15%). Electromyography (EMG) is the process of acquiring electrical signals generated through muscle activity (contraction/relaxation). Surface EMG deliberates the amount of electrical activity in the musculoskeletal system in a non-invasive way. Under specific conditions and during certain motor activities, this signal is substantially associated with muscle strength. These Signals are used as Control Inputs by assistive devices. The study aimed to investigate the EMG parameters of lower limb muscles (rectus femoris and biceps femoris) in healthy individuals and transtibial amputees walking on a treadmill at different speeds (0.55 m/s, 0.83 m/s, and 1.11 m/s). Ten non-amputee and two amputee subjects participated. Findings reveal significant reductions in EMG signals at slower speeds, emphasizing foot stability. The right biceps femoris exhibits the highest signals average, while the right rectus femoris has the lowest for amputees. The male participants’ right biceps femoris muscle showed the greatest signals of average treadmill walking activity at 0,55 m/s (0.0014 V) compared to the amputee individuals’ (0.001 V). At (0,83 m/s), male participants (0.0015 V) outperformed amputee subjects (0.0004 V). At (1,11 m/s), male participants (0.0024 V) outperformed amputee subjects (0.001 V). Male participants consistently outperform amputees across speeds. The study suggests the potential application of findings in rehabilitating transtibial amputees on a treadmill, considering distance and maximum speed with a prosthesis. Abstract Background The effect of listening to audiobooks, podcasts, and other audio files while walking on gait performance has not been well studied. Although the number of audio users is growing annually. Evidence suggests that a posture-first strategy contributes to gait stability in healthy individuals during dual-task conditions, but this effect may be diminished when the cognitive task is consciously prioritized. Objectives: To study the effect of listening to an audiobook while walking, as a daily life-like dual-task, on spatiotemporal gait parameters. Methods Forty young healthy (24.05 ± 3.66) subjects participated in the study. Spatiotemporal gait parameters were measured for 5 min on a treadmill once without (single-task) and once while listening to an audiobook through over-ear headphones (dual-task). Measured parameters included spatiotemporal parameters, gait phases, maximum pressure, and dual-task cost. Data were statistically analyzed using SPSS software. Results There were no significant differences in any of the studied parameters between the single- and dual-task conditions, even though the subjective cognitive load of listening to audiobooks while walking was high. However, participants with different habits had significant differences in gait phases and maximum pressure. Rare listeners had a shorter stance phase, a longer swing phase, and a higher maximum pressure on the dominant heel. They also had significant differences in dual-task costs. Conclusion No differences in the spatiotemporal gait parameters for walking with and without listening to audiobooks, as a daily life-like dual-task, were observed. However, the difference between participants who listened rarely and participants who listened often may confirm the “posture first” strategy in young healthy people. Background: Previous work suggests that proprioceptive information from ankle and hip are crucial in maintaining balance during upright standing; however, the contribution of these proprioceptive information during stepping balance recovery in not clear. The goal of the current study was to assess the role of ankle and hip proprioceptive information on balance recovery performance by manipulating type 1a afferent in muscle spindles using vibratory stimulation. Methods: Twenty healthy young participants were recruited (age = 22.2 ± 2.7 years) and were randomly assigned to balance recovery sessions with either ankle or hip stimulation. Trip-like perturbations were imposed using a modified treadmill setup with a protecting harness. Vibratory stimulation was imposed bilaterally on ankle and hip muscles to expose participants to three condition of no-vibration, 40Hz vibration, and 80Hz vibration. Kinematics of the trunk and lower-extremities were measured using wearable sensors to characterize balance recovery performance. Using the non-affected leg as stable frame of reference for the affected leg in gait assessment in knee osteoarthritis (KO) fails due to compensatory mechanisms. Assessing the cyclical movements of the upper extremities in a frequency-controlled repetitive punching task may provide an alternative frame of reference in gait assessment in patients with KO. Eleven participants with unilateral KO and eleven healthy controls were asked to perform treadmill walking and repetitive punching. The KO group showed more predictable (p = 0.020) and less automatized (p = 0.007) movement behavior than controls during treadmill walking. During repetitive punching, the KO group showed a similar degree of predictability (p = 0.784) but relative more automatized movement behavior (p = 0.013). Thus, the predictability of the movement behavior of the upper extremities during repetitive punching seems unaffected by KO and could provide an alternative frame of reference in gait assessment in patients with KO. This study examined heart rate (HR) and stride frequency (SF) values of 30 college-aged males and females during dry-land (DL) and aquatic walking (AW). Aquatic walking trials were completed in an underwater treadmill with the water depth at waist level; the water temperature (31°C ± 0.1°C) and room temperature (26.6°C ± 0.1°C) were maintained at thermoneutral levels throughout the study. During each walking condition, HR and SF were recorded at treadmill speeds of 1 mph,2 mph, and 3 mph. Participants were instructed to walk with their hands at their sides swinging as they would when walking on dry-land unless they felt the need to use the handrails to steady themselves. Heart rate monitors were used to record cardiovascular changes, and strides were measured from consecutive left and right toe strikes. Results of the study indicated HR was significantly higher during DL than AW at 1 mph (p < .001) and 3 mph (p < .001) but was not significantly different (p = .64) at 2 mph. The SF of the participants was significantly lower (p < .001) during AW than DL at all speeds. When comparing sex, females had significantly higher HR at 1 mph (p = .012), 2 mph (p = .007), and 3 mph (p < .001) than males for DL conditions. No differences in HR were found during the AW conditions (F = 0.66, p = 0.44, ƞp2. = 0.02). No differences in SF were observed between males and females in both DL (F = 2.96, p = 0.06, ƞp2 = 0.09) and AW (F = 1.03, p = 0.32, ƞp2 = 0.036) conditions. As a result, AW increased HR values similar to those of DL, but without the added stress to the lower extremities due to the buoyancy of the water. Thus, AW provided an exercise medium capable of meeting the ACSM intensity guidelines for PA and allowed adults to be physically active presumably with less stress on the lower body. Nowadays, adding weights to different locations while walking is popular in general fitness and rehabilitation programs, which again increases energy expenditure as well as oxygen consumption in a shorter quantity of time. Aims: The aim of this study was to compare the effect of adding handheld and ankle weights during treadmill walking on blood pressure and rate of perceived exertion in normal healthy adults. Settings and Design: The experimental study took place in the laboratory and was designed to be carried on for three sessions to measure the changes in blood pressure and ratings of perceived exertion (RPE) while walking with and without weight. Subjects and Methods: Thirty-three participants who satisfied the inclusion criteria were recruited to perform the exercise session with handheld and ankle and without weights and it consisted of 8 min of exercise testing with and without weights for two sessions. Statistical Analysis Used: The statistical test data were screened for normal distribution by Shapiro--Wilk test. Data were normally distributed for the systolic blood pressure (SBP), diastolic blood pressure (DBP), and RPE. Hence, parametric tests were applied for SBP, DBP, and RPE within-session and between-session analysis. Within-session analysis was done by paired t-test and between-session analysis was done by unpaired t-test. Results: Intra-session analysis by paired t-test in both sessions shows statistically significant changes in SBP and RPE (P < 0.05). Moreover, DBP was found significant while using handheld weights compared to no weights. While inter-session analysis by unpaired t-test showed significant changes in DBP compared to that of SBP, RPE using handheld weights during treadmill walking. Since self-paced treadmills enable more natural gait patterns compared to fixed-speed treadmills we examined the use of a self-paced treadmill as a alternative for overground gait analysis in persons after stroke. Twenty-five persons after stroke (10 males/15 females; 53 ± 12.05 years; 40.72 ± 42.94 months post-stroke) walked at self-selected speed overground (GAITRite, CIR Systems) and on a self-paced treadmill (GRAIL, Motek) in randomized order. Spatiotemporal parameters, variability and symmetry measures were compared using paired-sample t-tests or Wilcoxon Signed Rank tests. Concurrent validity was assessed using intraclass correlation coefficients and Bland-Altman plots. A regression model determined the contribution of the walking velocity to the changes in spatiotemporal parameters. The velocity on the treadmill was significant lower compared to overground (p < 0.001). This difference predicted the significant changes in other spatiotemporal parameters to varying degrees (27.7%–83.8%). Bland-Altman plots showed large percentage of bias and limits of agreement. Variability and symmetry measures were similar between conditions. When considering gait analysis in persons after stroke a self-paced treadmill may be a valuable alternative for overground analysis. Although a slower walking velocity, and accompanying changes in other spatiotemporal parameters, should be taken into account compared to overground walking. Considering the advantages regarding space and time, instrumented treadmills provide opportunities for gait assessment and training in a stroke population. When using self-paced treadmills for clinical gait analysis in persons after stroke, the slower walking velocity and accompanying changes in other spatiotemporal parameters need to be taken into account. Ultrasonography with motion analysis enables dynamic imaging of medial gastrocnemius (MG) muscles and tendons during gait. This revealed pathological muscle-tendon dynamics in children with spastic cerebral palsy (CP) compared to typically developing (TD) children. However, wearing an ultrasound probe on the lower leg could interfere with gait and bias muscle length changes observed with ultrasound. Does wearing an ultrasound probe on the MG influence gait in children with CP and TD children? Eighteen children with spastic CP and 16 age-matched TD children walked at comfortable walking speed on an instrumented treadmill. One baseline gait condition (BASE) and two conditions with an ultrasound probe and custom-made probe holder were measured: on the mid-muscle fascicles (FAS) and on the muscle-tendon junction (MTJ). The effect of condition and group on spatiotemporal parameters, hip, knee and ankle kinematics, ankle moment, ankle power, and modeled MG muscle-tendon unit (MTU) length was assessed using two-way repeated measures ANOVA's. Statistical non-parametric mapping was applied for time-series. Post-hoc paired-samples t-tests were conducted, and the root mean square difference was calculated for significant parts. Children took wider steps during FAS (CP, TD) and MTJ (TD) compared to BASE, and during FAS compared to MTJ (CP). Hip extension was lower (2.7°) during terminal stance for MTJ compared to FAS for TD only. There was less swing knee flexion (FAS 4.9°; MTJ 4.0°) and ankle plantarflexion around toe-off (FAS 3.0°; MTJ 2.4°) for both ultrasound placements, with no group effect. Power absorption during loading response was slightly increased for both ultrasound placements (0.12 W/kg), with no group effect. MTU shortened less in swing for both ultrasound placements (FAS 3.6 mm; MTJ 3.7 mm), with no group effect. Wearing an ultrasound probe causes minimal lower-limb gait alterations and MTU length changes that are mostly similar in CP and TD. • Children take wider steps when wearing an ultrasound probe during gait. • Ultrasound imaging interferes minimally with children's kinematics and kinetics. • The MTU shortened slightly less in swing when an ultrasound probe was worn. • We recommend using a light-weight probe with flexible cable for dynamic imaging. This study aimed to investigate the effect of varying the hardness of prefabricated orthopedic insoles on plantar pressure and muscle fatigue during uphill walking with a heavy backpack. Fifteen healthy male recreational athletes (age: 20.4 ± 1.0 years, height: 176.9 ± 5.7 cm, weight: 76.5 ± 9.0 kg) wore prefabricated orthopedic insoles with foot arch support; a heel cup with medium (MI), hard (HI), and soft (SI) relative hardnesses; and flat insoles (FI). They performed treadmill walking on uphill gradients with 25 kg backpacks. The plantar pressure and surface electromyographic activity were recorded separately, in 30 s and 6 min uphill treadmill walking trials, respectively. The HI, MI, and SI significantly decreased peak plantar pressure in the lateral heel compared to FI. The MI and SI significantly decreased the peak plantar pressure in the fifth metatarsal compared to FI. The MI significantly reduced the pressure–time integral in the lateral heel compared to FI. The HI significantly increased the peak plantar pressure and pressure–time integral in the toes compared to other insoles, and decreased the contact area in the metatarsal compared to SI. In conclusion, a prefabricated orthopedic insole made of soft material at the fore- and rearfoot, with midfoot arch support and a heel cup, may augment the advantages of plantar pressure distribution during uphill weighted walking. Background: This study investigated the cortical activation mechanism underlying locomotor control during healthy and hemiplegic walking. Methods: A total of eight healthy individuals with right leg dominance (male patients, 75%; mean age, 40.06 ± 4.53 years) and six post-stroke patients with right hemiplegia (male patients, 86%; mean age, 44.41 ± 7.23 years; disease course, 5.21 ± 2.63 months) completed a walking task at a treadmill speed of 2 km/h and a functional electrical stimulation (FES)-assisted walking task, respectively. Functional near-infrared spectroscopy (fNIRS) was used to detect hemodynamic changes in neuronal activity in the bilateral sensorimotor cortex (SMC), supplementary motor area (SMA), and premotor cortex (PMC). Results: fNIRS cortical mapping showed more SMC-PMC-SMA locomotor network activation during hemiplegic walking than during healthy gait. Intake of the catechin epigallocatechin gallate and caffeine has been shown to enhance exercise-induced fat oxidation. Matcha green tea powder contains catechins and caffeine and is consumed as a drink. We examined the effect of Matcha green tea drinks on metabolic, physiological, and perceived intensity responses during brisk walking. A total of 13 females (age: 27 ± 8 years, body mass: 65 ± 7 kg, height: 166 ± 6 cm) volunteered to participate in the study. Resting metabolic equivalent (1-MET) was measured using Douglas bags (1-MET: 3.4 ± 0.3 ml·kg−1·min−1). Participants completed an incremental walking protocol to establish the relationship between walking speed and oxygen uptake and individualize the walking speed at 5- or 6-MET. A randomized, crossover design was used with participants tested between Days 9 and 11 of the menstrual cycle (follicular phase). Participants consumed three drinks (each drink made with 1 g of Matcha premium grade; OMGTea Ltd., Brighton, UK) the day before and one drink 2 hr before the 30-min walk at 5- (n = 10) or 6-MET (walking speed: 5.8 ± 0.4 km/hr) with responses measured at 8–10, 18–20, and 28–30 min. Matcha had no effect on physiological and perceived intensity responses. Matcha resulted in lower respiratory exchange ratio (control: 0.84 ± 0.04; Matcha: 0.82 ± 0.04; p <.01) and enhanced fat oxidation during a 30-min brisk walk (control: 0.31 ± 0.10; Matcha: 0.35 ± 0.11 g/min; p <.01). Matcha green tea drinking can enhance exercise-induced fat oxidation in females. The current study aimed to assess walking stability by applying principal component analysis (PCA) to decompose three-dimensional (3D) whole-body kinematic data of 104 healthy young adults (21.9 ± 3.5 years, 54 females) derived from a depth-sensing camera into a set of movement components/synergies called "principal movements" (PMs), forming together to achieve the task goal. The effect of sex as the focus area was tested on three PCA-based variables computed for each PM: the relative explained variance (rVAR) as a measure of the composition of movement structures; the largest Lyapunov exponent (LyE) as a measure of variability; and the number of zero-crossings (N) as a measure of the tightness of neuromuscular control. The results show that the sex effects appear in the specific PMs. Specifically, in PM1, resembling the swing-phase movement, females have greater LyE (p = 0.013) and N (p = 0.017) values than males. Moreover, in PM3, representing the mid-stance-phase movement, females have smaller rVAR (p = 0.020) but greater N (p = 0.008) values than males. Background: Despite prosthetic technology advancements, individuals with transfemoral amputation have compromised temporal-spatial gait parameters and high metabolic requirements for ambulation. It is unclear how adding mass at different locations on a transfemoral prosthesis might affect these outcomes. Research question Does walking with mass added at different locations on a transfemoral prosthesis affect temporal-spatial gait parameters and metabolic requirements compared to walking with no additional mass?Methods: Fourteen participants with unilateral transfemoral amputations took part. A 1.8 kg mass was added to their prostheses in three locations: Knee, just proximal to the prosthetic knee; Shank, mid-shank on the prosthesis; or Ankle, just proximal to the prosthetic foot. Temporal-spatial gait parameters were collected as participants walked over a GAITRite® walkway and metabolic data were collected during treadmill walking for each of these conditions and with no mass added, the None condition. Separate linear mixed effects models were created and post-hoc tests to compare with the control condition of None were performed with a significance level of 0.05.Results: Overground self-selected walking speed for Ankle was significantly slower than for None (p < 0.05) (None: 1.16 ± 0.24; Knee: 1.15 ± 0.19; Shank: 1.14 ± 0.24; Ankle 0.99 ± 0.20 m/s). Compared to None, Ankle showed significantly increased oxygen consumption during treadmill walking (p < 0.05) (None: 13.82 ± 2.98; Knee: 13.83 ± 2.82; Shank: 14.30 ± 2.89; Ankle 14.56 ± 2.99 ml O2/kg/min). Other metabolic outcomes (power, cost of transport, oxygen cost) showed similar trends. Knee and Shank did not have significant negative effects on any metabolic or temporal-spatial parameters, as compared to None (p > 0.05). Significance Results suggest that additional mass located mid-shank or further proximal on a transfemoral prosthesis may not have negative temporal-spatial or metabolic consequences. Clinicians, researchers, and designers may be able to utilize heavier components, as long as the center of mass is not further distal than mid-shank, without adversely affecting gait parameters or metabolic requirements. A symmetric gait pattern in humans reflects near-identical movement in bilateral limbs during walking. However, little is known about how gait symmetry changes on different inclines. This study aimed to address this knowledge gap using the central pattern generator and internal model hypotheses. Eighteen healthy young adults underwent five 2-minute walking trials (inclines of +15%, +8%, 0%, -8%, and -15%). Dependent variables included step time, step length, step width, maximum heel clearance, time to peaks of maximum heel clearance, their corresponding coefficients of variation (CV), and respective symmetry indices (SI). Significant differences were observed in SI of step length ( p = .022), step length variability ( p < .001), step width variability ( p =.001), maximum heel clearance ( p < .001), and maximum heel clearance variability ( p = .049). Compared to level walking, walking at -8% and -15% inclines increased SI of step length ( p = .011, p = .039 respectively) but decreased SI of maximum heel clearance ( p = .025, p = .019 respectively). These observations suggested that incline walking affected gait symmetry differently, possibly due to varied internal models used in locomotion. Downhill walking improved vertical gait symmetry but reduced anterior-posterior symmetry compared to level walking. Downhill walking may be a preferable rehabilitation protocol for enhancing gait symmetry, as it activates internal model controls. Aims: The aim of the present study was to investigate whether the predictability of fronto-parallel trunk rotations (lateral body sway) could serve as a frame of reference to monitor recovery after total knee arthroplasty (TKA)., Methods: Before surgery, 11 TKA patients were asked to perform a treadmill walking task at three different speeds. In addition, their gait abilities were scored on three standard clinical walking tests. The treadmill walking task was repeated at three different timepoints following surgery, i.e., at 3, 6 and 12 months post-TKA. The movements of the trunk were digitized with an inertial sensor to capture the amplitude and the sample entropy (SEn) of the lateral body sway that were evaluated in separate ANOVAs., Results: Before surgery the TKA group showed larger body sway (P = 0.025) with smaller SEn values (P = 0.038), which both restored to levels of healthy adults in the 12 months following surgery. Treadmills are widely used in rehabilitation and gait analysis. However, previous studies have reported differences in terms of kinematics and kinetics between treadmill and overground walking due to physical and psychological factors. The aim of this study was to analyze gait differences due to only the physical factors of treadmill walking. Walking motions of a male participant were captured at 0.63, 1.05, 1.33, and 3.91 m/s. A gait controller of a virtual subject (63 kg) was trained for ground walking at each walking speed via a reinforcement learning method. Additionally, the gait controllers of virtual subjects with different body masses of 47, 79, and 94 kg were trained for ground walking at 1.05 m/s. The gait controllers and virtual subjects were tested for treadmill walking, and their lower-limb joint kinematics were compared with those for ground walking. Treadmill conditions of maximum allowable belt force and feedback control frequency of belt speed were set between 100 and 500 N and between 10 and 50 Hz, respectively. The lower-limb kinematics were identical between the two conditions regardless of the body mass and walking speed when the belt could provide a constant speed regardless of external perturbation in the ideal treadmill. However, kinematic differences were observed when simulation was performed on a non-ideal treadmill with a relatively low belt force and control frequency of belt speed. The root-mean-square differences of the hip, knee, and ankle flexion angles between treadmill and overground running at 3.91 m/s increased by 3.76°, 3.73°, and 4.91°, respectively, when the maximum belt force and control frequency decreased from infinity to 100 N and 10 Hz, respectively. At a maximum belt force exceeding 400 N or a control frequency exceeding 25 Hz, the root-mean-square difference of the joint kinematics was less than 3° for all body masses and walking speeds. Virtual subjects walking on non-ideal treadmills showed different joint kinematics from ground walking. The study identified physical factors that differentiate treadmill walking from overground walking, and suggested the belt forces and control frequencies of a treadmill to achieve the desired limit of kinematic differences.

To review and quantify the effects of walking training for the improvement of various aspects of physical function of people with chronic stroke. We conducted a systematic search and meta-analysis of randomized controlled trials (RCTs) of chronic stroke rehabilitation interventions published from 2008 to 2020 in English or French. Of the 6476-screened articles collated from four databases, 15 RCTs were included and analyzed. We performed a meta-regression with the total training time as dependent variable in order to have a better understanding of how did the training dosage affect the effect sizes. Treadmill walking training was more effective on balance and motor functions (standardized mean difference (SMD)=0.70[0.02, 1.37], p = 0.04) and 0.56[0.15, 0.96], p = 0.007 respectively). Overground walking training improved significantly walking endurance (SMD = 0.38[0.16, 0.59], p < 0.001), walking speed (MD = 0.12[0.05, 0.18], p < 0.001), participation (SMD = 0.35[0.02, 0.68], p = 0.04) and quality of life (SMD = 0.46[0.12, 0.80], p = 0.008). Aquatic training improved balance (SMD = 2.41[1.20, 3.62], p < 0.001). The Meta-regression analysis did not show significant effect of total training time on the effect sizes. Treadmill and overground walking protocols consisting of ≥30 min sessions conducted at least 3 days per week for about 8 weeks are beneficial for improving motor impairments, activity limitations, participation, and quality of life in people with chronic stroke. Treadmill walking training is effective for improving balance and motor functions. Overground walking training improved significantly walking endurance, walking speed, participation and quality of life. Background: Sensorimotor training using visual perturbations has been widely applied to astronauts for rapidly handling and adapting to unpredictable environments. However, these visual perturbations might not be strong enough to trigger long-term effects. Therefore, this study aimed to develop a novel sensorimotor training paradigm using pseudo-random visual perturbations and to determine the demands and patterns of active control under different types of visual perturbations. Method: Thirty healthy young adults participated in this study. Four walking conditions were randomly assigned to these participants: 1) walking without optic flow (NoOptic), 2) walking with the optic flow (Optic), 3) walking under reduced visual capability (Vre), and 4) walking under perturbed optic flow (Vpe). The dependent variables were the step length variability, the step width variability, the 95% confidence interval ellipse area, the long axis of the ellipse, and the short axis of the ellipse. Results: The results indicated that 1) the step length variability and the ellipse area were greater in Vre compared to Optic (p < 0.001, p < 0.001). Moreover, the step width variability and ellipse area were greater in Vpe than Optic (p < 0.001, p = 0.002). Conclusion: The abovementioned results demonstrated that 1) walking in both Vre and Vpe conditions required greater demands and different patterns in active controls compared to the Optic condition, suggesting both Vre and Vpe conditions could be applied for sensorimotor training; 2) the Vre condition would be the first choice if there were no concerns in potential trips on the treadmill. Background: Anteriorly-loaded walking is common in many occupations and may increase fall risk. Dynamic gait stability, defined by the Feasible Stability Region (FSR) theory, quantifies the kinematic relationship between the body's center of mass (COM) and base of support (BOS). FSR-based dynamic gait stability has been used to evaluate the fall risk.Research Question: How does front load carriage affect dynamic gait stability, step length, and trunk angle among young adults during treadmill walking?Methods: In this between-subject design study, 30 healthy young adults were evenly randomized into three load groups (0%, 10%, or 20% of body weight). Participants carried their assigned load while walking on a treadmill at a speed of 1.2 m/s. Body kinematics were collected during treadmill walking. Dynamic gait stability (the primary variable) was calculated for two gait events: touchdown and liftoff. Step length and trunk angle were measured as secondary variables. One-way analysis of variance was conducted to detect any group-related differences for all variables. Post-hoc analysis with Bonferroni correction was performed when main group differences were found.Results: No significant differences but medium to large effect sizes were found between groups for dynamic gait stability at touchdown (p = 0.194, η2 = 0.114) and liftoff (p = 0.122, η2 = 0.139). Trunk angle significantly increased (indicating backward lean) with the front load at touchdown (p < 0.001, η2 = 0.648) and liftoff (p < 0.001, η2 = 0.543). No significant between-group difference was found related to the step length (p = 0.344, η2 = 0.076).Significance: Carrying a front load during walking significantly alters the trunk orientation and may change the COM-BOS kinematic relationship and, therefore, fall risk. Abstract Background Altered knee muscle activity in children with asymptomatic Generalized Joint Hypermobility (GJH) is reported during isometric contraction, static and dynamic balance tasks and jumping, but has not been studied during gait. Therefore, the aim was to investigate group differences in knee muscle activity simultaneously with knee joint kinematics during treadmill walking between children with and without GJH. Methods Girls 14–15 years of age with GJH (inclusion criteria: Beighton score ≥6 of 9 and positive hyperextension ≥10° (one/both knees)) and a matched control group without GJH (inclusion criteria: Beighton score ≤5 and no knee hyperextension ≥10° ) were recruited. In total 16 participants with GJH and 10 non-GJH participants were included in the study. Surface electromyography (sEMG) was measured from the quadriceps, hamstrings and gastrocnemius muscles of the dominant leg during treadmill walking. Maximal voluntary isometric contractions while sitting were used for normalisation of sEMG to % of Maximum Voluntary EMG (%MVE). Knee joint angles during treadmill walking were measured by electrogoniometer. Furthermore, co-contraction index (CCI) was calculated, and presented for muscle groups of hamstrings-quadriceps (HQ) and gastrocnemius-quadriceps (GQ). CCI of medial and lateral sides of the knee, including ratio of the medial and lateral CCI for HQ and GQ were calculated. Results No group differences were found in demographics, muscle activation level, nor CCI and CCI ratios. However, participants with GJH displayed significantly decreased knee joint angle, mean (153º vs. 156º; p =0.03) and minimum (105º vs. 111º; p=0.01), during treadmill walking compared with controls. Conclusion Muscle activity during gait was not different between participants with GJH and non-GJH participants. However, participants with GJH displayed minor but statistically significant increased knee flexion during gait. Since the clinical consequences of increased knee joint flexion during gait are unknown, future studies should follow a larger cohort longitudinally during overground walking for development of clinical complications in this group.

This study aimed to investigate the effect of varying the hardness of prefabricated orthopedic insoles on plantar pressure and muscle fatigue during uphill walking with a heavy backpack. Fifteen healthy male recreational athletes (age: 20.4 ± 1.0 years, height: 176.9 ± 5.7 cm, weight: 76.5 ± 9.0 kg) wore prefabricated orthopedic insoles with foot arch support; a heel cup with medium (MI), hard (HI), and soft (SI) relative hardnesses; and flat insoles (FI). They performed treadmill walking on uphill gradients with 25 kg backpacks. The plantar pressure and surface electromyographic activity were recorded separately, in 30 s and 6 min uphill treadmill walking trials, respectively. The HI, MI, and SI significantly decreased peak plantar pressure in the lateral heel compared to FI. The MI and SI significantly decreased the peak plantar pressure in the fifth metatarsal compared to FI. The MI significantly reduced the pressure–time integral in the lateral heel compared to FI. The HI significantly increased the peak plantar pressure and pressure–time integral in the toes compared to other insoles, and decreased the contact area in the metatarsal compared to SI. In conclusion, a prefabricated orthopedic insole made of soft material at the fore- and rearfoot, with midfoot arch support and a heel cup, may augment the advantages of plantar pressure distribution during uphill weighted walking.

Modern technology has enabled researchers to analyze gait with great accuracy and in various conditions based on the needs of the trainees. The purpose of the study was to investigate the agreement between systems equipped with optical and capacitive sensors in the analysis of treadmill-based level and sloping gait. The spatiotemporal parameters of gait were measured in 30 healthy college-level students during barefoot walking on 0% (level), −10% and −20% (downhill) and +10% and +20% (uphill) slopes at hiking-related speeds using an optoelectric cell system and an instrumented treadmill. Inter-system agreement was assessed using the Intraclass Correlation Coefficients (ICCs) and the 95% limits of agreement. Our findings revealed excellent ICCs for the temporal and between moderate to excellent ICCs for the spatial parameters of gait. Walking downhill and on a 10% slope demonstrated better inter-system agreement compared to walking uphill and on a 20% slope. Inter-system agreement regarding the duration of gait phases was increased by increasing the number of LEDs used by the optoelectric cell system to detect the contact event. The present study suggests that systems equipped with optical and capacitive sensors can be used interchangeably in the treadmill-based spatiotemporal analysis of level and sloping gait. Use of VR-games is considered a promising treatment approach in stroke rehabilitation. However, there is little knowledge on the use and expectations of patients and health professionals regarding the use of treadmill walking in a fully immersive virtual environment as a rehabilitation tool for gait training for stroke survivors. The objectives of the current study were to determine whether stroke survivors can use fully immersive VR utilizing modern HMDs while walking on a treadmill without adverse effects, and to investigate the experiences of stroke survivors and clinicians after testing with focus on acceptability and potential utilization in rehabilitation. A qualitative research design with semi-structured interviews was used to collect data. Five stroke survivors and five clinicians participated in the study and tested a custom-made VR-game on the treadmill before participating in individual semi-structured interview. Data were analyzed through thematic analysis. The analysis of the interview data identified two main categories: (1) experiencing acceptability through safety and motivation , and (2) implementing fully immersive VR in rehabilitation. Both stroke survivors' and clinicians enjoyed the treadmill-based VR-game and felt safe when using it. The stroke survivors experienced motivation for exercising and achievement by fulfilling tasks during the gaming session as the VR-game was engaging. The clinicians found additional motivation by competing in the game. Both groups saw a potential for use in gait rehabilitation after stroke, on the premise of individual adaptation to each patient's needs, and the technology being easy to use. The findings from this qualitative study suggest that a fully immersive treadmill-based VR-game is acceptable and potentially useful as part of gait rehabilitation after stroke, as it was positively received by both stroke survivors and clinicians working within stroke rehabilitation. The participants reported that they experienced motivation in the game through safety, engagement and achievement. They also saw the potential of implementing such a setup in their own rehabilitation setting. Artificial green tea extracts may enhance exercise-induced fat oxidation. Natural Matcha green tea consumption involves the ingestion of the powdered green tea leaves. We examined the effects of three weeks daily intake of Matcha green tea powder on substrate oxidation during moderate-intensity exercise in females. Females with a regular menstrual cycle (n = 12, age: 28 ± 10 yr, body mass: 69 ± 17 kg, height: 163 ± 6 cm) volunteered to complete an incremental walking test to determine the individual moderate exercise intensity (four metabolic equivalent) for the subsequent 30-min treadmill walk. The study had a randomized placebo-controlled cross-over design with participants tested between day 9 and 11 of the menstrual cycle (follicular phase). Participants consumed 3x1 gram capsules of Matcha premium grade, (OMGTea Ltd, UK) per day for three weeks, with the final dose (1 gram) two hours before the 30-min walk (walking speed: 5.8 ± 0.4 km·h−1). Matcha had no effect on physiological responses (e.g. heart rate, placebo: 127 ± 14; Matcha: 124 ± 14 beats·min−1, p = 0.154), but resulted in lower respiratory exchange ratio (placebo: 0.872 ± 0.040; Matcha: 0.839 ± 0.035) (p = 0.033), higher fat oxidation by 35 ± 47% (placebo: 0.21 ± 0.08; Matcha: 0.26 ± 0.06 g·min−1) (p = 0.034), and lower carbohydrate oxidation (placebo: 0.75 ± 0.21; Matcha: 0.60 ± 0.18 g·min−1) (p = 0.048) during the 30-min moderate-intensity walk. Energy expenditure was similar for both conditions. There was no significant correlation between body fat % and the absolute or relative change in Matcha-induced fat oxidation during exercise. Continuous intake of Matcha green tea effects exercise-induced metabolic responses by enhancing fat oxidation during moderate-intensity exercise in adult females, seemingly independent of body composition. A significant decline in gait, visuomotor, and cognitive game performance measures was observed during DT compared to ST conditions, and in the PD-3 compared to PD-2 groups.ConclusionThe high to moderate ICC values along with the lack of systematic errors in the measures indicate that this tool has the ability to repeatedly record reliable DT interference (DTI) effects over time. The use of interactive digital media provides a flexible method to produce and evaluate DTI for a wide range of executive cognitive activities. A significant decline in gait, visuomotor, and cognitive game performance measures was observed during DT compared to ST conditions, and in the PD-3 compared to PD-2 groups. Conclusion: The high to moderate ICC values along with the lack of systematic errors in the measures indicate that this tool has the ability to repeatedly record reliable DT interference (DTI) effects over time. The use of interactive digital media provides a flexible method to produce and evaluate DTI for a wide range of executive cognitive activities. Background. Post-traumatic joint contracture (PTJC), characterized by loss of motion and permanent stiffness, affects up to 50% of patients following elbow joint dislocation or fracture. Mechanisms governing successful conservative treatment methods aimed at preventing elbow PTJC and avoiding operative treatments (e.g., physical therapy) are poorly understood. Using a previously established rat model of elbow PTJC, the purpose of this study was to explore the effect of varying timing, intensity and duration of active, functional exercise on joint motion outcomes. Methods. Following a surgically-induced unilateral elbow dislocation in rats, injured limbs were immobilized in bandages for 42 days followed by free mobilization for 42 additional days producing long-term PTJC. This work summarizes several studies (Phases I-III) that investigated the effects of early versus delayed therapy (timing), free mobilization versus forced treadmill walking (intensity), and limited-time versus unlimited use (duration) on elbow PTJC. Results. Joint motion outcomes in therapy groups showed no improvements compared to non-treated injured animals when therapy began day 14 post-injury or later regardless of timing, intensity or duration. Improved joint range-of-motion was only achieved when bandages were permanently removed at day 3 post-injury, regardless of whether added treadmill walking was performed. Conclusions. Background: Depth sensors could be a portable, affordable, marker-less alternative to three-dimension motion capture systems for gait analysis, but the effects of camera viewing angles on their joint angle tracking performance have not been fully investigated.Research Questions: This study evaluated the accuracies of three depth sensors [Azure Kinect (AK); Kinect v2 (K2); Orbbec Astra (OA)] for tracking kinematic gait patterns during treadmill walking at five camera viewing angles (0°/22.5°/45°/67.5°/90°).Methods: Ten healthy subjects performed fifteen treadmill walking trials (3 speeds × 5 viewing angles) using the three depth sensors to measure joint angles in sagittal hip, frontal hip, sagittal knee, and sagittal ankle. Ten walking steps were recorded and averaged for each walking trial. Range of motion in terms of maximum and minimum joint angles measured by the depth sensors were compared with the Vicon motion capture system as the gold standard. Depth sensors tracking accuracies were compared against the Vicon reference using root-mean-square error (RMSE) on the joint angle time series. Effects of different walking speeds, viewing angles, and depth sensors on the tracking accuracy were observed using three-way repeated-measure analysis of variance (ANOVA).Results: ANOVA results on RMSE showed significant interaction effects between viewing angles and depth sensors for sagittal hip [F(8,72) = 4.404, p = 0.005] and for sagittal knee [F(8,72)=13.211, p < 0.001] joint angles. AK had better tracking performance when subjects walked at non-frontal camera viewing angles (22.5°/45°/67.5°/90°); while K2 performed better at frontal viewing angle (0°). The superior tracking performance of AK compared with K2/OA might be attributed to the improved depth sensor resolution and body tracking algorithm.Significance: Researchers should be cautious about camera viewing angle when using depth sensors for kinematic gait measurements. Background: Altered knee muscle activity in children with asymptomatic Generalized Joint Hypermobility (GJH) is reported during isometric contraction, static and dynamic balance tasks and jumping, but has not been studied during gait. Therefore, the aim was to investigate group differences in knee muscle activity simultaneously with knee joint kinematics during treadmill walking between children with and without GJH.Methods: Girls 14-15 years of age with GJH (inclusion criteria: Beighton score ≥6 of 9 and positive hyperextension ≥10° (one/both knees)) and a matched control group without GJH (inclusion criteria: Beighton score ≤5 and no knee hyperextension ≥10° ) were recruited. In total 16 participants with GJH and 10 non-GJH participants were included in the study. Surface electromyography (sEMG) was measured from the quadriceps, hamstrings and gastrocnemius muscles of the dominant leg during treadmill walking. Maximal voluntary isometric contractions while sitting were used for normalisation of sEMG to % of Maximum Voluntary EMG (%MVE). Knee joint angles during treadmill walking were measured by electrogoniometer. Furthermore, co-contraction index (CCI) was calculated, and presented for muscle groups of hamstrings-quadriceps (HQ) and gastrocnemius-quadriceps (GQ). CCI of medial and lateral sides of the knee, including ratio of the medial and lateral CCI for HQ and GQ were calculated.Results: No group differences were found in demographics, muscle activation level, nor CCI and CCI ratios. However, participants with GJH displayed significantly decreased knee joint angle, mean (153º vs. 156º; p =0.03) and minimum (105º vs. 111º; p=0.01), during treadmill walking compared with controls.Conclusion: Muscle activity during gait was not different between participants with GJH and non-GJH participants. However, participants with GJH displayed minor but statistically significant increased knee flexion during gait. Since the clinical consequences of increased knee joint flexion during gait are unknown, future studies should follow a larger cohort longitudinally during overground walking for development of clinical complications in this group. Walking patterns of persons affected by cerebellar ataxia (CA) are characterized by wide stride-to-stride variability ascribable to: the background pathology-related sensory-motor noise; the motor redundancy, i.e., an excess of elemental degrees of freedom that overcomes the number of variables underlying a specific task performance. In this study, we first tested the hypothesis that healthy and, especially, CA subjects can effectively exploit solutions in the domain of segmental angles to stabilize the position of either the foot or the pelvis (task performance) across heel strikes, in accordance with the uncontrolled manifold (UCM) theory. Next, we verified whether a specific perturbation-based training allows CA subjects to further take advantage of this coordination mechanism to better cope with their inherent pathology-related variability. Results always rejected the hypothesis of pelvis stabilization whereas supported the idea that the foot position is stabilized across heel strikes by a synergic covariation of elevation and azimuth angles of lower limb segments in CA subjects only. In addition, it was observed that the perturbation-based training involves a decreasing trend in the variance component orthogonal to the UCM in both groups, reflecting an improved accuracy of the foot control. Concluding, CA subjects can effectively structure the wide amount of pathology-related sensory-motor noise to stabilize specific task performance, such as the foot position across heel strikes. Moreover, the promising effects of the proposed perturbation-based training paradigm are expected to improve the coordinative strategy underlying the stabilization of the foot position across strides, thus ameliorating balance control during treadmill locomotion. Modern technology has enabled researchers to analyze gait with great accuracy and in various conditions based on the needs of the trainees. The purpose of the study was to investigate the agreement between systems equipped with optical and capacitive sensors in the analysis of treadmill-based level and sloping gait. The spatiotemporal parameters of gait were measured in 30 healthy college-level students during barefoot walking on 0% (level), −10% and −20% (downhill) and +10% and +20% (uphill) slopes at hiking-related speeds using an optoelectric cell system and an instrumented treadmill. Inter-system agreement was assessed using the Intraclass Correlation Coefficients (ICCs) and the 95% limits of agreement. Our findings revealed excellent ICCs for the temporal and between moderate to excellent ICCs for the spatial parameters of gait. Walking downhill and on a 10% slope demonstrated better inter-system agreement compared to walking uphill and on a 20% slope. Inter-system agreement regarding the duration of gait phases was increased by increasing the number of LEDs used by the optoelectric cell system to detect the contact event. The present study suggests that systems equipped with optical and capacitive sensors can be used interchangeably in the treadmill-based spatiotemporal analysis of level and sloping gait.

Background: Previous work suggests that proprioceptive information from ankle and hip are crucial in maintaining balance during upright standing; however, the contribution of these proprioceptive information during stepping balance recovery in not clear. The goal of the current study was to assess the role of ankle and hip proprioceptive information on balance recovery performance by manipulating type 1a afferent in muscle spindles using vibratory stimulation., Methods: Twenty healthy young participants were recruited (age = 22.2 ± 2.7 years) and were randomly assigned to balance recovery sessions with either ankle or hip stimulation. Trip-like perturbations were imposed using a modified treadmill setup with a protecting harness. Vibratory stimulation was imposed bilaterally on ankle and hip muscles to expose participants to three condition of no-vibration, 40Hz vibration, and 80Hz vibration. Kinematics of the trunk and lower-extremities were measured using wearable sensors to characterize balance recovery performance. Outcomes were response time, recovery step length, trunk angle during toe-off and heel-strike of recovery stepping, and required time for full recovery., Findings: Ankle vibratory stimulation elicited main effects on reaction time and recovery step length ( p  < 0.002); reaction time and recovery step length increased by 23.0% and 21.2%, respectively, on average across the conditions. Objectives: Improving ankle joint contracture is important because stiffness in ankle dorsiflexion can lead to pain, especially when weight-bearing during walking, which tends to concentrate on the forefoot. We hypothesized that the contraction of the gastrocnemius muscle in ankle dorsiflexion would increase the Achilles tendon length and improve the dorsiflexion range of motion. We evaluated the effects of walking with and without a gradient on Achilles tendon length., Methods: This study included 23 men who underwent ultrasound imaging to measure the Achilles tendon length while they stood on an inclined table adjusted according to the dorsiflexion angle. Treadmill walking was performed for 10 min with a 10° incline (gradient condition) or without gradient (level condition). The measurements were compared using a paired t -test., Results: In the gradient condition, the range of motion for ankle dorsiflexion was significantly increased after the intervention. In the gradient condition, the Achilles tendon length while standing on an inclined surface was significantly increased after the intervention., Conclusions: Walking under gradient conditions led to the extension of the Achilles tendon in the ankle dorsiflexion position. This was accompanied by contraction of the gastrocnemius muscle, resulting in lengthening of the Achilles tendon. This study investigated changes in gait symmetry with trunk displacement during phase-shifted auditory paced treadmill walking for effective training with auditory cueing provided in conventional gait training. Eighteen able-bodied participants walked at a comfortable speed on a treadmill and the measured cadence was set at 100% (baseline). The phase-shifted auditory cue was set to both phase advance and delay of 20% at 5% intervals based on the baseline with respect to matching foot contact to the auditory cue. Trunk displacement increased with the phase-shifted auditory cue, and the largest value was found in the 120% condition compared to baseline (P < 0. 0 5). Step length, step time, and swing phase time symmetry ratio gradually increased with increasing phase delay and gradually decreased with increasing phase advance on the linear model. However, single support time and stance phase time symmetry ratio showed contrasting characteristics compared to above parameters. The results indicate that the phase-shifted auditory cue significantly changes gait symmetry and trunk displacement. Particularly, the 20% phase advance and delay cues yielded about a 5% change in the step length symmetry ratio. Continuous motor tasks like walking have the potential to allow a dynamic allocation of processing resources when interrupted by intermittent cognitive tasks. The degree to which a successful interleaving of processing streams of both tasks is possible may depend on the temporal regularity of events. Fifteen subjects participated in an experiment where we systematically manipulated the regularity of stimulus onsets in a 2-back task relative to the step cycle. We tested three conditions where stimulus onset was always synchronous to a defined event in the stride (right heel strike, left heel strike, and midway between two heel strikes) and two conditions where the temporal location of the stimulus shifted from stride to stride. In order to test for potential effects of task difficulty, we also manipulated walking speed. We measured reaction times, accuracy of the reactions and several measures describing motor performance. There was no sign of task interference in these measures when stimuli always appeared at the same relative location within the step cycle. However, we observed prolonged reaction times when the stimulus came up earlier than expected. Surprisingly, in the other non-regular regime, where the stimulus appeared later than expected, reaction times were fastest. We interpret this result in the light of a prescheduled allocation of processing resources that is linked to the cyclic profile of processing requirements of the motor task.

We examined how the application of a forward horizontal force applied at the waist alters the metabolic cost, kinematics, and external work of gait. Horizontal assist forces of 4%, 8% and 12% of a subject's body weight were applied via our testing apparatus while subjects walked at comfortable walking speed on a level treadmill. Kinematic and metabolic parameters were measured using motion capture and ergospirometry respectively on a group of 10 healthy male subjects. Changes in kinematic and metabolic parameters were quantified and found similar to walking downhill at varying grades. A horizontal assist force of 8% resulted in the greatest reduction of metabolic cost. Changes in recovery factor, external work, and center of mass (COM) movement did not correlate with changes in metabolic rate and therefore were not driving the observed reductions in cost. The assist force may have performed external work by providing propulsion as well as raising the COM as it pivots over the stance leg. Assist forces may decrease metabolic cost by reducing the concentric work required for propulsion while increasing the eccentric work of braking. These findings on the effects of assist forces suggest novel mobility aids for individuals with gait disorders and training strategies for athletes. [ABSTRACT FROM AUTHOR]

Although lower limb strength becomes asymmetrical with age, past studies of aging effects on gait biomechanics have usually analyzed only one limb. This experiment measured how aging and treadmill surface influenced both dominant and nondominant step parameters in older (mean 74.0 y) and young participants (mean 21.9 y). Step-cycle parameters were obtained from 3-dimensional position/time data during preferred-speed walking for 40 trials along a 10 m walkway and for 10 minutes of treadmill walking. Walking speed (young 1.23 m/s, older 1.24 m/s) and step velocity for the two age groups were similar in overground walking but older adults showed significantly slower walking speed (young 1.26 m/s, older 1.05 m/s) and step velocity on the treadmill due to reduced step length and prolonged step time. Older adults had shorter step length than young adults and both groups reduced step length on the treadmill. Step velocity and length of older adults' dominant limb was asymmetrically larger. Older adults increased the proportion of double support in step time when treadmill walking. This adaptation combined with reduced step velocity and length may preserve balance. The results suggest that bilateral analyses should be employed to accurately describe asymmetric features of gait especially for older adults. [ABSTRACT FROM AUTHOR]

Background: Restoring one's walking ability and balance is a major goal of gait rehabilitation in stroke survivors. The demands of the treadmill and over ground walking training are different in its impact on gait parameters, and treadmill training with or without body weight support may offer a favorable effect on gait parameters. Objective: The objective of this review was to summarize the current best evidence for the effectiveness of treadmill training in improving walking speed, walking distance, and balance of stroke survivors. Methods: A comprehensive search of literature published between 2000 and April 2019 was conducted using the following six electronic databases; CINAHL, MEDLINE, AMED, EMBASE, PUBMED, and PEDro. Only randomized controlled trials evaluating the effect of treadmill training on walking balance, speed and distance post-stroke were included. Physiotherapy Evidence Database (PEDro) was used to assess the risk of bias of included trials. Qualitative synthesis of evidences was prepared and meta-analysis was implemented to draw pooled effects of treadmill training on the selected walking parameters. Results: Eight trials involving 695 participants met the inclusion criteria. Meta-analysis showed no statistically significant difference between treadmill and over ground walking groups for walking speed and balance. However, treadmill training improved walking distance during immediate post-intervention period (MD 13.03 m, 95% CI 1.21, 24.85, I2 = 0%) but the effects were lost during follow up periods (MD 10.6 m, 95% CI −4.0, 25.3, I2 = 0%). Conclusion: Treadmill training following stroke offers improvement in walking distance. However, it has no significant advantage in improving walking speed and balance over over-ground walking training. A large systematic review and meta-analysis of randomized controlled trials could provide more definitive evidence.
