Berichtet wird uber den Fall eines 37 Jahre alten Mannes, der an einem heisen Tag im Sommer durch die Symptomatik eines akuten Delirs auffallig geworden war. Der auf den Vorfallszeitpunkt zuruckgerechnete Blutalkohol von maximal 1,11 ‰ bot keine ausreichende Erklarung fur das psychopathologische Erscheinungsbild. Eine Intoxikation durch gangige Betaubungsmittel oder Drogenersatzstoffe konnte ausgeschlossen werden. Die psychiatrische Anamnese war leer, der klinische Befund unauffallig. Der Verlauf mit einer raschen Normalisierung nach Verbringung in eine kuhle Umgebung und Aufnahme einer Flussigkeitsmenge von zwei Litern lasst auf ein durch Dehydratation verursachtes Delir ruckschliesen, vermutlich in Verbindung mit der eher als geringgradig bis masig einzustufenden Alkoholisierung.

The consumption of garlic reduces the risk of cardiovascular disease and cancer, S-allylcysteine sulfoxide (alliin), allicin (DATi), diallyl disulfide (DADS), S-allylcysteine (SAC) and several storage dipeptides are the organo-sulphur compounds (OSC) involved in the protective mechanism of garlic against cardiovascular disorders and carcinogenesis. Thus it is very interesting to quantify simultaneously all these compounds in different garlic powders obtained in several cultural conditions. The quantification of OSC by a new ion-pair HPLC method allowed showing the general sulphur-dependence positive effect of garlic on cardiovascular disorder and carcinogenesis and the variable specific activity of each implicated OSC. The screening of 11 garlic tablets proposed on the market showed the variability and particularly the differential instability of each OSC. From these results, a new garlic tablet was realised and each step was controlled by this method. This analytical method proved to be a very powerful tool for the understanding of the garlic protective mechanism against cancer and cardiovascular diseases and the development and quality control of garlic tablets.

Garlic is proposed to have immunomodulatory and anti-inflammatory properties. This paper shows that garlic powder extracts (GPE) and single garlic metabolites modulate lipopolysaccharide (LPS)-induced cytokine levels in human whole blood. GPE-altered cytokine levels in human blood sample supernatants reduced nuclear factor (NF)-kappaB activity in human cells exposed to these samples. Pretreatment with GPE (100 mg/L) reduced LPS-induced production of proinflammatory cytokines interleukin (IL)-1beta from 15.7 +/- 5.1 to 6.2 +/- 1.2 micro g/L and tumor necrosis factor (TNF)-alpha from 8.8 +/- 2.4 to 3.9 +/- 0.8 micro g/L, respectively, whereas the expression of the anti-inflammatory cytokine IL-10 was unchanged. The garlic metabolite diallydisulfide (1-100 micro mol/L) also significantly reduced IL-1beta and TNF-alpha. Interestingly, exposure of human embryonic kidney cell line (HEK293) cells to GPE-treated blood sample supernatants (10 or 100 mg/L) reduced NF-kappaB activity compared with cells exposed to untreated blood supernatants as measured by a NF-kappaB-driven luciferase reporter gene assay. Blood samples treated with extract obtained from unfertilized garlic (100 mg/L) reduced NF-kappaB activity by 25%, whereas blood samples treated with sulfur-fertilized garlic extracts (100 mg/L) lowered NF-kappaB activity by 41%. In summary, garlic may indeed promote an anti-inflammatory environment by cytokine modulation in human blood that leads to an overall inhibition of NF-kappaB activity in the surrounding tissue.

After the enzyme systems responsible for methanol oxidation were blocked by ethanol, five test persons were given methanol at a dose of approximately 10 mg/kg weight, once orally and once parenterally. Taking into account the endogenous blood methanol levels detectable before the administration of methanol, C0 concentrations of 11.1-15.9 mg/kg were reached. This corresponds to a distribution volume of approximately 0.77 +/- 0.07 l/kg, which is comparable to the 0.78 +/- 0.09 l/kg obtained for ethanol. After parenterally administering methanol as a bolus, the distribution half-life was on average 8 min (range: 3.8-13.8 min). After oral administration of methanol diluted in 100 ml water on an empty stomach, invasion took place with a half-life of approximately 5 min (3.8-6.9 min). In one case, however, due to vegetative disturbances the invasion half-life was 23.1 min.

(all-Z)-(9,10,12,13,15,16-2H6)Octadeca-9,12,15-trienoic acid ( = α-linolenic acid; D6-4) was synthesized to investigate the biochemical formation of linolenic-acid-derived aroma compounds in cultures of the yeast Sporobolomyces odorus, using an established gas chromatographic/mass spectrometric (GC/MS) method. Three compounds were identified as labeled: (Z)-dec-7-eno-5-lactone (δ-jasmin lactone), (Z,Z)-dodeca-6,9-dieno-4-lactone, and (2E,4Z)-hepta-2,4-dienoic acid. Both lactones were biosynthesized mostly under conservation of the initial configuration from their corresponding oxygenated linolenic-acid intermediates. The application of (13S,9Z,11E,15Z)-13-hydroxy(9,10,12,13,15, 16-2H6)octadeca-9,11,15-trienoic acid (D6-7) as a OH-functionalized precursor of δ-jasmin lactone allowed to gain insight into the stereochemical course of the biosynthesis to both enantiomers of this lactone. In this experiment, 88.3% of the metabolized labeled precursor was transformed under retention of the original configuration of the (R)-enantiomer. This investigation is also a contribution to a better understanding of the CC bond isomerization steps which took place during the β-oxidative degradation of the substrate.

Addition of [9,10-2H2]oleic acid to cultures of the yeast Sporobolomyces odorus led to the formation of labeled (R)-γ-decanolactone and both enantiomers of (Z)-6-γ-dodecenolactone and γ-dodecanolactone. The labeling patterns of these lactones were estimated by a quantitative gas chromatography/mass spectrometry method, suitable for ring-labeled lactones. For the first time, there is strong evidence of oleic acid being a genuine precursor of the important aroma compound (R)-γ-decanolactone. On the basis of the presented and former results, a new biosynthetic pathway of this compound is proposed. Starting with a strict enantioselective (R)-12-hydroxylation of oleic acid as an initial step, the following β-oxidation led to the lactone. In addition, biosynthetic aspects of the formation of (Z)-6-γ-dodecenolactone and γ-dodecanolactone in S. odorus are discussed. Four male subjects aged between 20 and 29 years were given intravenous injections of methanol at a dosage of 10 mg per kg body weight, once without prior administration of ethanol, and once after oral ingestion of 0.3 g ethanol per kg body weight. The serum methanol concentration was monitored over the next 5 h (after methanol administration alone) and 6-7 h (after methanol administration following ethanol ingestion). The elimination of methanol administered alone was found to follow first-order kinetics with a rate constant for the elimination phase of 0.475-0.259 h-1, corresponding to an elimination half-life of 1.8-3.0 h. When ethanol was also administered methanol oxidation was found to be completely blocked until the blood ethanol concentration had fallen to 0.2 g/kg. During screening for biosurfactants among marine, n-alkane-utilizing bacteria, low- and high-molecular surface-active substances were detected. The marine bacterial strain MM1 was found to synthesize a novel glycolipid that has not so far been cited in the literature. Both 1H, 13C-nuclear magnetic resonance spectroscopic and positive ion fast atom bombardment mass spectrometer studies led to the identification of a glucose lipid consisting of four 3-OH-decanoic acids, which are linked together by ester bonds. The lipophilic moiety is coupled glycosidically with C-1 of glucose. The glucolipid reduced the surface tension from 72 mN/m to 30 mN/m while the minimum interfacial tension towards n-hexadecane was lowered to values smaller than 5 mN/m.

Drink tests involving 14 women were carried out to determine the effects of the menstrual cycle phases on the pharmacokinetics of ethanol. One experiment was carried out in the follicular phase of the cycle and another in the luteal phase, with the estradiol, progesterone, and testosterone levels being determined in both cases. The target concentration was a final blood alcohol concentration (BAC) of approximately 0.08g%. After drinking was completed, concurrent BAC and breath alcohol concentration (BrAC) measurements were carried out at intervals of 10-20min. The ethanol elimination rate was determined by calculating a linear function in the part of the slope that was clearly linear. In addition, the c(0) and Widmark factors r were calculated. In 10 of the volunteers, who had a normal increase in progesterone in the luteal phase, the average hourly elimination rate ss(60) in the follicular phase amounted to 0.0194+/-0.0020g%/h (BAC) and 0.0975+/-0.0068mg/L/h (BrAC), and in the luteal phase to 0.0193+/-0.0031g%/h (BAC) and 0.1026+/-0.0101mg/L/h (BrAC). There was no significant difference. Other pharmacokinetic parameters (c(0) concentrations, Widmark factors r, distribution volumes, maximal BAC, mean absorption rate, time until the peak concentrations were reached) also revealed no significant differences between the blood and breath alcohol levels of the luteal and follicular phases. In addition, no significant correlations were observed between the absolute progesterone level and the respective elimination rates ss(60).

The problems concerning an unstable data basis with regard to lethal Doxepin concentrations have been manifested based on a case about a 39-year-old man, who was found dead in his apartment with strangulation marks on his neck, for which a lethal Doxepin intoxication entered the differential diagnosis discussion. For a long time it has been known that postmortem redistribution leads to a falsely inflated concentration as measured in cardiac blood, while the concentrations in peripheral postmortem blood change comparatively little. Despite this, most of the current literature relies on published case report, which fails to mention the location of blood sampling, whereby it is fairly safe to assume that a central sample is intended. Only 9 cases of an isolated lethal Doxepin intoxication have been found, in which the concentrations in blood samples from peripheral vessels had been measured. These values lie between 1.5 and 7.0 mg/L, which is in the lowest quarter of the span of lethal concentrations mentioned in literature without specific mention of the location of the blood sample.

The purpose of the study was to examine gender differences on the pharmacokinetics of ethanol. Sixty-eight healthy men and 64 healthy women with normal body mass indexes received between 0.79 and 0.95 g ethanol/kg body weight in the form of their choice after they had eaten a "typical" breakfast. The aimed concentration for both genders was a blood alcohol concentration C(0) of 0.104 g/dl. Blood samples in the elimination phase were taken in 10- to 20-min intervals beginning after completion of absorption. The maximum blood ethanol concentration was 0.0819+/-0.0184 g/dl for women and 0.0841+/-0.0155 g/dl for men. Background: Epidemiologic studies suggest that garlic may have beneficial effects on risk factors associated with cardiovascular disease (CVD). However, these findings are not unambiguously supported by randomized placebo-controlled clinical trials. Objective: We sought to investigate the effects of a chemically well-characterized garlic preparation on biomarkers for inflammation, endothelial function, and lipid metabolism in subjects with risk factors for CVD. Design: This was a double-blind, randomized, placebo-controlled trial in 90 overweight [body mass index (in kg/m2) > 24.5] subjects aged 40-75 y who smoked >10 cigarettes/d. The subjects were randomly assigned to 3 parallel treatment groups: garlic powder (2.1 g/d), atorvastatin (40 mg/d), or placebo. Duplicate measurements were performed at baseline and after 1 and 3 mo of treatment. Treatments were compared with analysis of covariance with baseline as the covariate, and differences between the treatments were reported as mean percentage difference and corresponding 97.5% CI. Results: None of the variables showed significant differences between the garlic-treated and the placebo groups. In contrast, compared with the placebo group, atorvastatin treatment resulted in significantly lower plasma concentrations of C-reactive protein (20.2%; 1.7%, 35.3%), total cholesterol (37.2%; 33.1%, 41.1%), LDL cholesterol (52.7%; 47.9%, 57.1%), triacylglycerols (31.9%; 20.8%, 41.5%), and tumor necrosis factor α (TNF-α; 41.9%; 19.0%, 58.3%) and increased the ratio of ex vivo whole blood lipopolysaccharide-stimulated to nonstimulated TNF-α concentrations (109.7%; 37.9%, 218.9%). Conclusion: We conclude that a chemically well-characterized garlic preparation has no significant effect on inflammatory biomarkers, endothelial function, or lipid profile in normolipidemic subjects with risk factors for CVD. © 2006 American Society for Nutrition. Cerebral hemorrhages are rare complications that occur after the consumption of amphetamine; the mortality rate is estimated at 50 %. It is assumed that cerebral hemorrhages are caused by amphetamine-induced hypertensive crises coinciding with pre-existing vascular alterations (congenital vascular malformations, vasculitis). In the present case report, a 40-year-old man, who is said to have regularly consumed hashish, heroin and speed, died of a massive cerebral hemorrhage located in the region of the basal ganglia shortly after the intravenous administration of amphetamine and heroin. In the course of the post-mortem investigation, neither vascular malformations nor vasculitis could be detected in the brain. Even if the evidence for the existence of such alterations was missing, this does not exclude their presence for certain. Other potential amphetamine-induced vascular alterations are discussed.

An 87-year-old woman, who was largely immobilized in a wheelchair, suffered rib fractures and an unstable fracture of the pelvic ring in a fall. Approximately 2 1/2 hours later she developed marked clinical symptoms of pulmonary thromboembolism and died 5 hours after the accident. The question to be discussed with regard to the causality was the unusually short interval of only 2 1/2 to 5 hours between the accident and the clinical signs of embolism leading to her death. Current literature gives only rough outlines stating that thromboembolism can occur as early as the first day after the trauma. An alternative theory explaining the pulmonary thromboembolism may be the breaking off of a pre-existing thrombus due to manipulation during transport or diagnostic measures in connection with the unstable fracture of the pelvic ring.

The quality of garlic and garlic products is usually related to their alliin content and allicin release potential. Until now no analytical method was able to quantify simultaneously allicin, its direct precursor alliin (S-allyl-L-cysteine sulfoxide), SAC (S-allyl-L-cysteine) as well as various dipeptides that apparently serve as storage compounds in garlic. It is well known that all these intermediates are involved in the allicin biosynthetic pathway. A simple and rapid HPLC method suitable for routine analysis was developed using eluents containing an ion-pairing reagent. Particularly, heptanesulfonate as ion-pairing reagent guarantees a sufficient separation between alliin and the more retained dipeptides at very low pH. Allicin was eluted after 18 min on a 150 x 3 mm column. Synthetic reference compounds were characterized by the same chromatographic method using a diode-array UV detector and an ion trap mass spectrometer (electrospray ionization) in the multiple MS mode. In routine analysis of garlic bulbs, powders and other products, the diode-array detector is sufficient for a relevant quantification. Our method has been used in studies to improve the quality of garlic and its derived products.

Alkohol stellt in unserer Gesellschaft ein weit verbreitetes, sozial akzeptiertes Genussmittel mit dosisabhangig berauschender Wirkung dar. Es ist eine logische Folge, dass dies auch in Delinquenz und Strafverfolgung seinen Niederschlag findet. Zahlenmasig steht die Alkoholintoxikation unter allen Ursachen, die eine Erorterung der Schuldfahigkeit eines Angeklagten notwendig machen, mit Abstand an der Spitze. Bei einem weiteren grosen Anteil ist Alkoholeinfluss zumindest als moglicher Kofaktor zu diskutieren. Dass Alkohol in derartigen Fallen keine Rolle spielt, ist eher selten. Dabei ist die Frage der Schuldfahigkeit anlassbedingt keineswegs nur bei schwerwiegenden Delikten zu prufen, in denen in der Regel der Begutachtung eine eingehende ambulante oder stationare Untersuchung des Beschuldigten vorausgeht. Die Mehrzahl der Falle betrifft minder schwere Delikte, die haufig sogar ohne vorhergehende Kenntnisnahme der Ermittlungsakte im Verfahren selbst ad hoc zur Beurteilung anstehen. Vom Sachverstandigen wird erwartet, dass er die Blutalkoholkonzentration (BAK) zum Zeitpunkt der Tat nach vorgegebenen Regeln berechnet und vor dem Hintergrund des numerisch benannten BAK-Wertes zur Frage der moglichen rauschbedingten Beeintrachtigung der Schuldfahigkeit Stellung nimmt. Die Wertigkeit der BAK unterliegt dabei fast schon traditionell etwas unterschiedlicher Gewichtung. Wahrend von juristischer Seite haufig ein gewisser Hang zu einer schematisierenden Sichtweise mit hoher Wertigkeit der BAK zu beobachten ist, wird dies von medizinischer Seite uberwiegend als dem Einzelfall nicht gerecht werdend abgelehnt.

The metabolic course of four isomeric epoxyfatty acids derived from oleic-, elaidic-, (Z)-, and (E)-vaccenic acids in the lactone-producing yeast, Sporidiobolus salmonicolor, was studied by using the deuterium-labeled precursors. Dihydroxy-, hydroxyoxo-, and hydroxy fatty acids as well as gamma-lactones were identified as metabolic intermediates. Quantitative analysis of the label content and estimation of the enantiomeric composition of the lactones established that, in the first step, the racemic epoxyfatty acids were enantiospecifically hydrolyzed by an epoxide hydrolase. During the subsequent metabolism, the stereochemical orientation of the hydroxy groups of the dihydroxyfatty acids were modified by an oxidation/reduction step.

Eine wesentliche Aufgabe der Kriminologie liegt in der Erfassung und Analyse des Verbrechens in seiner Gesamtstruktur. Dabei darf die Tat nicht isoliert gesehen werden. Tater und Opfer mit ihren Personlichkeitsbildern, ihrem sozialen Umfeld und ihren Wechselwirkungen aufeinander sind einzubeziehen. Bedeutsam ist nicht allein die auf den Tatzeitpunkt begrenzte Querschnittsbeurteilung, sondern insbesondere die Langsschnittbeurteilung unter Einschlus aller pradisponierenden Faktoren. Nur so lassen sich Ansatzpunkte fur eine fruhe, schon im Vorfeld einer moglichen Tat wirksame Pravention gewinnen. Auch ist dies der einzige Weg, eine bereits begangene Tat vollumfanglich und angemessen zu wurdigen.

In 33 patients with a depressive syndrome the effects of partial SD restricted to the 2 hours between 3:00 and 5:00 in the morning was compared to that of no SD (= undisturbed sleep, n = 17) and total SD (n = 16) in a balanced crossover design. 2. 2. Partial SD had a moderate antidepressant effect which is, however, clearly inferior to that of total SD. 3. 3. Reasons for the scarceness of the effect may be an inappropriate timing of this 2-hr SD period (which seems improbable), too short a duration of SD, or the resumption of sleep at 5 o'clock. 4. 4. “Intermediate” SD in the form it was used here represents no therapeutic advantage as we had expected. Its moderate effects lend support to the hypothesis that sleep disturbance is a sign of (mostly insufficient) restorative or self-healing efforts of the depressive organism.
