Aim: For several years now, medical students have also been taught general practice at academic medical teaching practices. Specialty practices have not yet been included in the curricular education. Since 1998, we have conducted a block seminar in hematology twice per semester for eighth-semester medical students. This block seminar was offered from 1998-2001 to students at the Philipps University in Marburg and since 2001 to students at the Johannes Gutenberg University in Mainz. Since 2010 our block seminar has been part of the curriculum at the Johannes Gutenberg University.Method: Standardized course evaluation by students who had attended our block seminar between January 2010 and March 2022. Courses that were held virtually due to corona were not included in the analysis. The questionnaire used to evaluate courses in the medical degree program at the Johannes Gutenberg University served as the evaluation instrument.Results: Since 1998 more than 1,000 students have attended our seminar. The systematic evaluation of the course by 500 students who participated in the curricular, classroom-based seminar sessions since 2010 shows that the highest ratings possible are given for practical relevance, learning atmosphere, teaching and effectiveness.Conclusion: High quality in teaching curricular courses to medical students at a specialty practice is possible. Insights into the possibilities connected with working in the outpatient setting at a medical practice broadens students' experience. This teaching format facilitates external university instructors in terms of teaching and, at the same time, relieves the university in terms of staff and financial budget., Zielsetzung: Die Ausbildung von Studierenden der Humanmedizin im Fach Allgemeinmedizin geschieht seit einigen Jahren auch in akademischen Lehrpraxen. Facharztpraxen sind bisher nicht an der curricularen Lehre beteiligt. Seit 1998 führen wir in unserer Gemeinschaftspraxis zweimal pro Semester ein Blockpraktikum Hämatologie für Studierende der Humanmedizin im 8. Fachsemester durch. Das Blockpraktikum wurde von 1998-2001 für Studierende der Philipps-Universität Marburg und seit 2001 für Studierende der Johannes-Gutenberg-Universität Mainz angeboten. Durch dieses Lehrmodell wird externen Hochschuldozenten der Unterricht erleichtert, gleichzeitig kann die Universität personell und finanziell entlastet werden.

Background: Addition of immune checkpoint inhibitors to neoadjuvant chemotherapy (NACT) is a promising strategy in early breast cancer, but the optimal duration of therapy is currently unknown. In the GeparNuevo (NCT02685059) trial, addition of durvalumab to NACT as previously reported led to a moderate increase in pathological complete response (pCR) rate by an absolute 9% (P = 0.287). Patients and methods: Patients with cT1b-cT4a-d triple-negative breast cancer (TNBC) received durvalumab 1.5 g or placebo every 4 weeks added to nab-paclitaxel 125 mg/m2 weekly for 12 weeks, followed by durvalumab/placebo every 4 weeks plus epirubicin/cyclophosphamide every 2 weeks followed by surgery. Durvalumab was not continued after surgery. The primary objective was pCR. Secondary endpoints included invasive disease-free survival (iDFS), distant disease-free survival (DDFS) and overall survival (OS). Results: A total of 174 patients were randomised between June 2016 and October 2017. After a median follow-up of 43.7 months, 34 events had occurred. Despite a non-significant increase in the pCR rate, significant differences were observed for 3-year iDFS, DDFS and OS: iDFS was 85.6% with durvalumab versus 77.2% with placebo [hazard ratio (HR) 0.48, 95% confidence interval (CI) 0.24-0.97, stratified log-rank P = 0.036]; DDFS 91.7% versus 78.4% (HR 0.31, 95% CI 0.13-0.74, P = 0.005); OS 95.2% versus 83.5% (HR 0.24, 95% CI 0.08-0.72, P = 0.006). pCR patients had 3 -year iDFS of 95.5% with durvalumab and 86.1% without (HR 0.22, 95% CI 0.05-1.06). In the non-pCR cohort 3-year iDFS was 76.3% versus 69.7% (HR 0.67, 95% CI 0.29-1.54). Multivariable analysis confirmed a durvalumab effect independent of the pCR effect. No new safety signals occurred. Conclusions: Durvalumab added to NACT in TNBC significantly improved survival despite a modest pCR increase and no adjuvant component of durvalumab. Background: The education of medical students to become qualified medical doctors should be improved in Germany. Reasons for this are medical teachers not properly trained in education, a lack of quality control and low esteem of medical education, too large groups, no intensive relationship between students and academic teachers and the triad for medical teachers at university hospitals to qualify in their discipline, to perform science and lecture medical students. At the same time experienced associate professors who left university are not integrated optimally in medical education programs. Pilotproject: Since 1998 we are performing weekend seminars for advanced medical students. 3 students are educated by 1 teacher. The topics of the training are differential diagnosis of anaemia, polyglobulinaemia, leukocytopenia, leukocytosis, thrombocytopenia, thrombocytosis, training in morphology, diagnosis and treatment of leukaemia and lymphoma and principles of communication skills. Patient cases are presented in a problem oriented learning manner using patient files, a video microscope and a computer based learning programme for interactive purposes.Results: So far 320 students have participated in our seminars. Systematic quality evaluation of the teaching reveals excellent marks for practical relevance, learning atmosphere, didactics and effectiveness. Impressions of the routine work in an oncology group practice widens the experience of the students.Conclusion: Academical training for medical students is feasable and successful in an oncology group practice. This teaching model enables external associate professors to give their education in the environment where they are working and may relieve the University personally and financially., Hintergrund: Die Ausbildung von Medizinstudenten zu guten Ärzten ist in Deutschland verbesserungsbedürftig. Die Gründe für ein suboptimales Studium sind die häufig fehlende Vernetzung von Theorie und Praxis, die mangelnde didaktische Ausbildung der akademischen Lehrer, die fehlende Wertschätzung einer guten Lehre, zu große Gruppen, fehlende fachliche und persönliche Beziehungen zwischen Lehrern und Studierenden, und immer noch die Triade, die von den Ärzten der Unikliniken verlangt ihre Facharztweiterbildung, Grundlagenforschung und den Studentenunterricht simultan zu leisten. Gleichzeitig wird die Erfahrung und Kompetenz der großen Zahl von erfahrenen, habilitierten Hochschuldozenten, die die Universität verlassen haben, nicht optimal genutzt.Pilotprojekt: Seit 1998 führen wir in unserer Gemeinschaftspraxis zweimal pro Semester ein Blockseminar für Studierende der klinischen Semester durch. Die maximale Gruppenstärke beträgt 3 Studenten pro Dozent. Die Unterrichtsinhalte umfassen die Anämiediagnostik, Differentialdiagnose der Leukozytopenie, Leukozytose, Thrombozytose, Thrombozytopenie, Diagnostik und Systematik der Leukämien und Lymphome, Morphologieunterricht sowie die Grundprinzipien der ärztlichen Gesprächsführung. Methodisch steht das problemorientierte Lernen unter Verwendung von Patientenakten, einem Videomikroskop sowie eine von uns entwickelte Differentialdiagnose CD im Mittelpunkt.Ergebnisse: Bisher haben 320 Studenten an unseren Seminaren teilgenommen. Die systematische Evaluation des Unterrichtes durch die Studenten ergibt Bestnoten für Praxisrelevanz, Unterrichtsatmosphäre, Didaktik und Effektivität.
