Background Prospective studies are needed to understand the safety and feasibility of laparoscopic pancreatectomy. The aim of the present study was to describe laparoscopic pancreatectomy currently undertaken in Japan, using a prospective registration system. Methods Patient characteristics and planned operations were registered preoperatively, and then the performed operation and outcomes were reported using an online system. Collected data were also compared between institutions based on their level of experience. This study was registered with UMIN000022836. Results Available data were obtained from 1,429 patients at 100 Japanese institutions, including 1,197 laparoscopic distal pancreatectomies (LDPs) and 232 laparoscopic pancreatoduodenectomies (LPDs). The rates of completion for planned operations were 92% for LDP and 91% for LPD. Postoperative complication rates after LDP and LPD were 17% and 30%, and 90-day mortality rates were 0.3% and 0.4%, respectively. Shorter operation time, less blood loss, and lower incidence of pancreatic fistula were observed in institutions experienced in LDP. A higher rate of pure laparoscopic procedure and shorter operation time were noted in institutions experienced with LPD. Conclusion LDPs and LPDs are performed safely in Japan, especially in experienced institutions. Our data could support the next challenges in the field of laparoscopic pancreatectomy.

右副腎領域に発生し，FDG-PETにて悪性腫瘍が疑われた後腹膜神経鞘腫の1例を経験したので報告する．症例は35歳，女性．心窩部痛と背部痛を認め，前医を受診．腹部CTにて，右副腎領域，下大静脈背側に6cm大の境界明瞭な充実性腫瘤像を認めた．MRI検査では，T1強調像にて低信号，T2強調像にて高信号の腫瘤であった．FDG-PET検査で，同部に高集積を認め，遅延像で集積の増加を認めた．右副腎癌を含めた悪性腫瘍の疑いで開腹手術を行った．腫瘍は表面平滑で境界明瞭であり，周囲組織への浸潤を認めなかった．病理組織学的所見はAntoni type A主体の良性神経鞘腫であった．術後11カ月，再発所見は認めていない．

症例は77歳，男性．1992年（64歳時）に胆嚢摘出術を受けている．2006年3月近医で黄疸を指摘され当院紹介．画像検査で上部胆管に楔状の狭窄を認めたが，胆管壁の不整は無かった．拡張した上流胆管（肝内胆管）内に多数の小結石を認めた．胆汁細胞診はClassIIであった．良性胆管狭窄および胆道結石症の診断で内視鏡的治療を試みたが不成功のため開腹手術を行った．胆管狭窄部を切除し結石を除去した後，肝管空腸吻合を施行した．狭窄部には肉眼的にも腫瘍を認めず，慢性胆管炎による胆管狭窄であった．なお，術後胆管狭窄の原因で最も多いとされる初回手術中の胆管損傷の有無は本症例では不明である．

症例は67歳の女性で, 近医に高血圧, 糖尿病で通院中であった. 血糖コントロールの悪化と, 腹部超音波検査で膵腫瘍を指摘され紹介入院となる. 血液検査所見ではエラスターゼ1が900ng/dlと高値であった. 腹部超音波検査所見では主膵管の閉塞と末梢膵管の拡張を認めた. MRCP所見では膵体部にて膵管は途絶し, 末梢膵管は拡張していた. ERCP所見でも, 膵体部にて膵管は途絶していた. 腹部CT所見では明らかな腫瘤は同定できなかった. その他, 上部消化管内視鏡検査, 下部消化管造影検査にて, 異常所見はなかった. 以上より, 通常の膵管癌を疑い, 平成17年2月, 膵体尾部切除術を施行した. 術後の病理組織学的検査にて, 同腫瘍細胞は卵円形細胞が充実性結節状, 胞巣状に増殖し, chromogranin染色陽性であった. また, 各種ホルモンの染色ではserotoninのみ陽性であり, 膵内分泌腫瘍と診断した.

症例は77歳の女性で, 心窩部不快感を主訴に近医受診し, 上部消化管内視鏡検査にて胃体中部前壁に壁外からの圧迫所見を認め当院紹介受診となった. 腹部超音波検査にて, 肝左葉外側区域(S2)に39×30mmの等から高エコー腫瘤を認めた. 腹部造影CTでは, 同部の辺縁部から内部に向けて, 不均一な造影効果を認めた. 腹部MRIではT1強調像で低信号, T2強調像で高信号, dynamic studyにて内部が不均一に造影された. その他, 消化管精査で異常はなかった.第1に海綿状血管腫を疑ったが, 画像所見上, 肝内胆管癌, 転移性肝癌などとの鑑別は困難であった. 平成16年7月, 腹腔鏡補助下肝部分切除術を施行した. 病理組織学的検査では線維性結合組織と海綿状血管腫が混在し, 肝硬化性血管腫と診断した. 肝硬化性血管腫の報告はまれであり, 検索しえたかぎり9例を認めるのみであった.

症例は69歳の女性で主訴は黄疸,肝機能障害であった.精査の結果,下部胆管癌による閉塞性黄疸と診断し,胆道ドレナージによる減黄後,膵頭十二指腸切除術 (PD-II) を施行した.術後,膵空腸吻合不全を生じ保存的に経過観察していたが, 34病日目に腹腔ドレーンより突然の動脈性出血を認めショック状態となった.緊急腹腔動脈造影を施行し,総肝動脈の仮性動脈瘤破裂と診断した.治療としてauto perfusion balloon catheterを用いて肝血流を保存しつつ破裂部位を圧迫止血した.翌日ショック状態を離脱した後,総肝動脈塞栓術を施行した.その後肝機能障害も軽度で,再出血を認めず術後49病日目に退院となった.本法は仮性肝動脈瘤破裂に対する肝不全を予防する有用な治療手段である.

肝細胞癌に対して2回の切除術と異時性に出現した鼠径部と傍大動脈リンパ節転移を切除しえた, まれな進展形式を呈した肝細胞癌切除例を経験したので報告する. 症例は69歳の男性で, 1980年胃癌手術時の輸血によりC型肝炎を発症した. 1995年肝細胞癌に対して肝S6切除術を施行し, 以後再発肝細胞癌に対して2度TAEを施行した. 2000年1月右鼠径部腫瘤に対して摘出術を施行した. 組織学的に肝細胞癌のリンパ節転移と診断された. 他に肝外転移巣なく同年3月右葉切除術を施行した. 腫瘍マーカーは正常化し経過観察していた. 3年後の2003年6月超音波検査で傍大動脈にリンパ節腫脹を指摘され, 腫瘍マーカーも上昇しており, 同年8月摘出術を施行した. 組織学的に肝細胞癌のリンパ節転移と診断された. 術後, 腫瘍マーカーは正常化し, 現在, 初回手術より9年, 無再発生存中である.

症例は62歳,男性,数カ月前から続く下痢と体重減少で近医受診し,当院紹介となる. 48歳時,穿孔性十二指腸潰瘍に対し幽門側胃切除術, Billroth II法再建の既往あり.来院時,極度のるい瘻を認め,低栄養状態であった.血液検査成績では,ヘマトクリット31.0%,血色素10.4g/dlと軽度の貧血を呈し,血清総蛋白4.1g/dl,血清アルブミン2.2g/dlと低蛋白,低アルブミン血症を認めた.腫瘍マーカーはCEA7.8ng/ml, CA19-948U/mlと軽度上昇を認めた.悪性腫瘍を疑い,下部消化管内視鏡検査を施行したところ,横行結腸に狭窄部を認め,その口側が双口となり,小腸と瘻孔を形成していた.そこで上部消化管内視鏡検査を施行したところ,胃空腸吻合部に多発潰瘍を認め,一部横行結腸に穿通していた.胃空腸吻合部潰瘍の横行結腸穿通と診断し,残胃全摘,瘻孔を含む結腸右半切除術を施行した.胃切除後の吻合部潰瘍は比較的多く認められるが,横行結腸に穿通するのは稀である.さらに主訴が便通異常で診断に苦慮したため,興味深い症例と考え報告する.

症例は65歳女性.下血のため近医を受診し,大腸内視鏡検査で下行結腸に隆起性病変を指摘され,当院に紹介となった.術前の腹部CT検査にて,膵管は著明に拡張し,膵尾部に胃大彎を圧排する径7 cmの内部不均一な嚢胞性病変を認めた.大腸内視鏡検査では,脾彎曲部に粘液が付着した手拳大の隆起性病変が指摘された.内視鏡的逆行性膵管造影では,主乳頭開口部の開大および主膵管の著明な拡張と粘液の充満を認め粘液産生膵腫瘍と診断した.以上より,粘液産生膵腫瘍の胃・結腸直接浸潤と診断し,膵体尾部切除術,脾摘出術,結腸部分切除術,胃部分切除術,リンパ節郭清術を施行した.最終組織診断はinvasive ductal carcinoma, mucinous carcinoma (colloid carcinoma)であり,臨床的には, intraductal papillary mucinous tumor (IPMT)から進展した膵粘液癌とした. IPMTは,浸潤性に乏しく比較的予後が良好とされているが,本例のように周囲臓器へ浸潤する症例があり,早期診断治療の重要性が示唆された.

近年, 超音波検査・CT検査・MRI検査などの臨床画像診断法の進歩は著しい. コーン・ビームCTとは, 円錐X線ビームを360度1回転させて投影で得られる2次元データから3次元画像を再構成する方法である. 今回我々は, 優れた空間分解能を有するコーン・ビームCTを用いた空気造影検査にて胃病変の診断を試みたので報告する. 症例1(早期胃癌)においては, 癌の描出が可能であった. 症例2 (進行胃癌) においては, 腹腔内から観察した画像に構成することで狭窄部を立体的に捉えることができた. 症例3 (胃粘膜下腫瘤) においては, 形状・大きさ・表面の性状・食道胃接合部との位置関係を正確に判断することが可能であった. 臨床応用するためには, 診断の恒常性や質的診断を向上など問題が山積しており, 今後症例を重ね検討を行う必要がある.

Selection of patients for hepatectomy for hepatocellular carcinoma conventionally has been based upon Child-Pugh grading. However, postoperative liver failure after hepatectomy is a major cause of hospital mortality. A new predictor of postoperative liver failure is required. The objective of this study was to identify risk factors for postoperative liver failure after hepatectomy.Perioperative risk factors for liver failure after hepatectomy were analyzed in 112 patients with hepatocellular carcinoma Eight of these patients died of liver failure. Stepwise multivariate logistic regression was performed to investigate significant independent factors among 17 variables, including the serum alkaline phosphatase ratio (ALPR) on the first day after hepatectomy. ALPR was calculated as the postoperative ALP level divided by the ALP level before surgery.Significant risk factors of postoperative liver failure were ALPR on postoperative day 1 (ALPR1), sex, operative blood loss, and operative procedure. As an indicator of liver failure, the diagnostic accuracy of the ALPR1 was 93.7% when the ALPR was less than 0.4 on the first postoperative day. The ALPR and the serum total bilirubin concentration after hepatectomy were uncorrelated.ALPR1 is a useful predictor of liver failure after hepatectomy.

Since there was no precedent in the management of radiation control in Japan from the standpoint of commercial power station, we were obliged to grope in the dark in establishing a policy at our station at first. Not being expert by nature it was necessary for us to deal with problems open-heartedly as unique matters which would crop up one after another in the course of practical business at site.Anyhow we were able to set our business in the right direction at present by conducting to suit the circumstances of our station though we were forced to make rules in various respects in attaining normal operation.An interesting experience gained at Tokai station has been compiled in this manuscript on the actual conditions of radiation control and the problems we had in mind during the period from the initial stage of operation to normal operation of the station based on the article introduced at the third research presentation meeting and we would like to present it to you all for a new trial.
