Purpose To develop and validate a health-related quality of life (HRQoL) questionnaire for patients with current or previous coronavirus disease (COVID-19) in an international setting. Methods This multicenter international methodology study followed standardized guidelines for a four-phase questionnaire development. Here, we report on the pretesting and validation of our international questionnaire. Adults with current or previous COVID-19, in institutions or at home were eligible. In the pretesting, 54 participants completed the questionnaire followed by interviews to identify administration problems and evaluate content validity. Thereafter, 371 participants completed the revised questionnaire and a debriefing form to allow preliminary psychometric analysis. Validity and reliability were assessed (correlation-based methods, Cronbach’s α, and intra-class correlation coefficient). Results Eleven countries within and outside Europe enrolled patients. From the pretesting, 71 of the 80 original items fulfilled the criteria for item-retention. Most participants (80%) completed the revised 71-item questionnaire within 15 min, on paper (n = 175) or digitally (n = 196). The final questionnaire included 61 items that fulfilled criteria for item retention or were important to subgroups. Item-scale correlations were > 0.7 for all but nine items. Internal consistency (range 0.68–0.92) and test–retest results (all but one scale > 0.7) were acceptable. The instrument consists of 15 multi-item scales and six single items. Conclusion The Oslo COVID-19 QLQ-W61© is an international, stand-alone, multidimensional HRQoL questionnaire that can assess the symptoms, functioning, and overall quality of life in COVID-19 patients. It is available for use in research and clinical practice. Further psychometric validation in larger patient samples will be performed.

At the end of the last century, there was a debate about whether cognitive psychology had superseded behavioral psychology and psychoanalysis, and the question was raised of whether the latter two might even be dead. The aim of this study was to investigate how these sub-disciplines have developed since then. The citation count, Journal Impact Factor (JIF), and Immediacy Index of four leading journals from each sub-discipline were abstracted for the years 1998, 2008, and 2018. Trends were analyzed using joinpoint regression analysis. The average number of times each sub-discipline’s journals were cited increased between 1988 and 2018. The cognitive journals’ JIF increased slightly, the behavioral journals’ doubled, and the psychoanalytic journals’ remained at the same level. The average annual percentage change for citations to the International Journal of Psychoanalysis showed that the number of citations statistically significantly increased over the years. This was also the case for the Journal of Applied Behavior Analysis. Its JIF trend changed at one point: it decreased up through 2006, after which it increased. Citations to Cognitive Psychology also increased over time, while there was no evidence that its JIF changed. This shows that all three sub-disciplines are still alive and relevant.

Zusammenfassung Hintergrund Es existieren keine Studien zur Inanspruchnahme psychoonkologischer Angebote durch Weichteilsarkompatienten in Deutschland. Ziel war es deshalb, die Häufigkeit der Inanspruchnahme psychoonkologischer Angebote im Krankenhaus in dieser Gruppe zu ermitteln und damit assoziierte Faktoren zu untersuchen. Methode Die Kohortenstudie PROSa (Krankheitslast und Versorgungssituation bei Sarkomen) wurde zwischen 2017 und 2020 in 39 deutschen Studienzentren durchgeführt. Für die vorliegende Analyse wurden Querschnittsdaten von erwachsenen Weichteilsarkompatienten ausgewertet. Faktoren auf Patienten- wie auf Einrichtungsebene wurden als mögliche Prädiktoren der Inanspruchnahme psychoonkologischer Beratung mittels logistischer Regression in einem verallgemeinerten linearen gemischten Modell exploriert. Resultate Bei 910 teilnehmenden Patienten lagen von 576 (63,3 %) Angaben zur Inanspruchnahme vor. 212 Patienten (unter Einbeziehung der fehlenden Angaben 23,3 %, ohne diese 36,7 %) nahmen psychoonkologische Angebote in Anspruch. Negativ mit der Inanspruchnahme assoziiert waren männliches (vs. weibliches) Geschlecht (Odds Ratio [OR] 0,62) und höheres Alter (18– Konklusion Frauen, jüngere Personen, Patienten mit höherer Bildung und fortgeschrittenem Krankheitsstadium nehmen häufiger psychoonkologische Versorgung in Anspruch. Ein struktureller Faktor für eine erhöhte Inanspruchnahme ist die Beteiligung der Psychoonkologie am Tumorboard.

Soft tissue sarcomas (STS) and gastrointestinal stromal tumours (GIST) are a group of rare malignant tumours with a high and heterogenous disease burden. As evidence is scarce, we analysed the prevalence of increased emotional distress and identified distress-associated factors in these patients.The PROSa-study (Burden and medical care of sarcoma) was conducted between 2017 and 2020 in 39 study centres. Cross-sectional data from adult STS and GIST patients were analysed. Distress was measured with the Patient Health Questionnaire (PHQ-4). The relation of socioeconomic and clinical factors with distress was explored in adjusted logistic regression models.Among 897 patients, 17% reported elevated anxiety and 19% reported depression. Unemployed patients (odds ratio [OR] 6.6; 95% CI 2.9-15.0), and those with a disability pension (OR 3.1; 95% CI 1.9-5.0) were more likely to experience distress compared to employed patients. Also, patients with a disability pass had higher odds of increased distress than those without (OR 1.8; 95% CI 1.2-2.7). Lowest distress was observed in patients 2 to5 years and ≥5 years after diagnosis (comparison:6 months) (OR 0.4; 95% CI 0.2-0.6) and (0.3; 95% CI 0.2-0.6). Patients with thoracic STS (vs. lower limbs) had twice the odds to experience distress (OR 2.0; 95% CI 1.1-3.6). Distress was seen almost twice as often in patients with progressive disease (vs. complete remission) (OR 1.7; 95% CI 1.1-2.8).The prevalence of elevated distress in STS and GIST patients is high. In unemployed patients, in those with a disability pension and in newly diagnosed patients a noticeable increase was observed. Clinicians should be aware of these factors and consider the social aspects of the disease.

Purpose This study examined the pattern of psychosocial care in breast cancer survivors. Methods In a prospective study with measurements before surgery, 1 month, 8 months, and 5 years thereafter, we examined the proportion of breast cancer survivors who were aware about, had been offered and received various types of psychosocial services from psychologists, social workers, doctors, self-help groups etc. The degree of helpfulness per service among users was ascertained with Likert scales. Determinants of awareness, offer and use were investigated using binary logistic regression analyses. How the services are inter-related was tested with principal component analyses. Results Among 456 breast cancer survivors who participated until 5 years, psychological services were known by 91%, offered to 68%, and used by 55% of patients. Social services were known by 86%, offered to 65%, and used by 51%. Women ≥ 65 years were less likely to be informed about (odds ratio (OR) 0.2) and get offers for psychosocial services (OR 0.4 for social and 0.5 for psychological services) than women Conclusion These findings underline the importance of psychosocial support by physicians in addition to the "professional" mental health and social care providers. They also show that elderly women in need for support might be in danger of not being well-informed about the services available.

Zusammenfassung Hintergrund und Zielsetzung Die Abschaffung und Ersetzung des gegenwärtigen Antrags- und Gutachterverfahren wurden 2019 vom Gesetzgeber beschlossen. Die vorliegende Studie untersuchte, was Psychotherapeut*innen über dieses Vorhaben denken. Methoden Niedergelassene Psychotherapeut*innen wurden in leitfadengestützten Interviews hinsichtlich ihrer Erfahrungen und Einstellungen zur psychotherapeutischen Versorgungssituation in Deutschland befragt. Aussagen zum Gutachterverfahren wurden mittels qualitativer Inhaltsanalyse ausgewertet. Meinungen zum Gutachterverfahren wurden klassifiziert und mit dem Therapieverfahren sowie dem Altersschwerpunkt verglichen. Ergebnisse Von 41 Befragten äußerten sich 27 hinsichtlich des Gutachterverfahrens. Aus deren Aussagen ergaben sich folgende Themen: Wertschätzung aber auch Bemängelung der Qualitätssicherung durch das Gutachterverfahren, Sorge vor einem künftigen Verlust an wirtschaftlicher und behandlungsbezogener Sicherheit, Wahrnehmung der Berichtspflicht als Infragestellung der Kompetenz, Missbilligung der schlechten Vergütung, Zusammenhang zwischen persönlicher Einstellung zum Gutachterverfahren und Sorgfalt bei der Berichtanfertigung sowie Kritik an der Intransparenz der Gesetzesinitiative. Psychodynamische Psychotherapeut*innen wollten das Gutachterverfahren häufiger beibehalten als Verhaltenstherapeut*innen, Kinder- und Jugendlichenpsychotherapeut*innen häufiger als Psychologische Psychotherapeut*innen. Diskussion Psychotherapeut*innen erleben die Reformierung als Vorgang mit großem Einfluss auf ihre Behandlungspraxis und psychotherapeutische Identität. Um sowohl die Akzeptanz als auch die Qualität künftiger Qualitätssicherungsverfahren zu verbessern, sollten Psychotherapeut*innen aktiv an deren Entwicklung teilhaben.

Purpose We examined how migration background is associated with awareness and usage of psycho-oncology services. Methods Oncologists in community-based practices and outpatient clinics asked their patients and their relatives to complete a questionnaire. Migrants were purposely over-sampled. The questionnaire was provided in Arabic, English, Farsi, French, German, Hindi, Kurdish, Pashto, Russian, Somali, Turkish, Urdu, and Vietnamese. Results From 9 collaborators, 177 participants were enrolled (130 with and 47 without migration background). The existence of outpatient cancer counselling centres was known to 38% of the participants without and 32% with migration background, self-help groups to 32 vs. 12%, and psychotherapy to 43 vs. 25%. Respondents from the Near and Middle East were less likely to know about psychotherapy (odds ratio (OR) 0.1, p = 0.01); those from the Commonwealth of the Independent States or former Yugoslavia were less often informed about self-help groups (OR 0.1, p = 0.06). Migrants retrieved information less frequently from the internet than non-migrants (10 vs. 25%). At least one service had been used by 27% of migrants and 42% of non-migrants (OR 0.5, p = 0.06). After adjusting for gender, age, education, and patient-relative status, there was no evidence for an association between migration background and service use. Conclusions Migrants should be better informed about psychotherapy and self-help groups, in particular the ones coming from the Near or Middle East and the Commonwealth of the Independent States or former Yugoslavia. The under-use of psychosocial services can largely be explained by confounding factors. Therefore, these factors must always be taken into account when analysing the use of psychosocial services in the aforementioned populations.

Background This study examined the relationship between social service counseling (SSC) and financial and role functioning problems in primary breast cancer (BC) patients over a 5-year observation period. Methods In the multicenter prospective study, patients were approached before surgery (t1), before initiation of adjuvant treatment (t2), after therapy completion (t3), and 5 years after surgery (t4). We examined the proportion of BC survivors who had financial and role functioning problems and the proportion who were employed at t4. We examined how frequently patients were informed about, offered, or used SSC, and we used multivariate logistic regression analyses to examine the relationship between this and financial and role functioning problem prevalence. Results Of the 456 BC survivors, 33% had financial problems and 22% reported role functioning problems at t4. There was no evidence that women with increased financial problems were informed about SSC more often than those without (OR 1.1, p = 0.84) or that they used SSC more often (OR 1.3, p = 0.25). However, women with role functioning problems were informed about SSC significantly more often (OR 1.7, p = 0.02) and attended counseling significantly more often (OR 1.6, p = 0.03). Among participants aged n = 255), 70% were employed. Patients who had received SSC were more likely to be employed at t4 than patients who did not (OR 1.9, p = 0.04). Conclusion These findings underline the importance of SSC for BC patients with role functioning issues. They indicate that individuals who use SSC are more likely to be employed later on than individuals who do not.

Zusammenfassung Hintergrund und Fragestellung Mit der Psychotherapiestrukturreform von 2017 sollte u.a. der Zugang zu ambulanter Psychotherapie erleichtert werden. Ein Aspekt der Evaluation der Reform ist, die Perspektive der Therapiesuchenden abzubilden. Der Fokus der vorliegenden Studie liegt dabei auf der Frage, welche Präferenzen Therapiesuchende äußern und inwiefern sie diese in den Suchprozess einbringen können. Methoden Mit Hilfe von qualitativen, leitfadengestützten Interviews wurden Therapiesuchende zu Beginn ihrer Suche (erster Erhebungszeitpunkt = t1) zu ihren Präferenzen und drei Monate später (zweiter Erhebungszeitpunkt = t2) nach ihren Erfahrungen befragt. Im Rahmen einer strukturierten Inhaltsanalyse wurden die Transkripte zunächst kategorisiert, bevor Generalisierungen, welche die Kernaussagen der Interviews abbilden, abgeleitet wurden. Ergebnisse Es wurden 83 Interviews mit 46 Therapiesuchenden geführt. Mehr als die Hälfte gaben an, dass sie anfängliche Präferenzen im Verlauf der Suche nicht länger priorisierten, um so die Chance auf einen Therapieplatz zu erhöhen. Vor der Suche lag der Fokus darauf, dass „die Chemie“ zu dem Therapeuten/der Therapeutin stimmt; nach bzw. während der Suche war z. B. die Vereinbarkeit mit dem Alltag entscheidender. Diskussion Die Studie zeigt, dass es weiter nötig ist, den Zugang zu Psychotherapie zu vereinfachen und die grundlegende Versorgungssituation zu verbessern. Um gute Voraussetzungen für den Therapieprozess und die Behandlungsergebnisse zu schaffen, wäre es erforderlich, den Therapiesuchenden die Berücksichtigung ihrer Präferenzen zu ermöglichen.

ZusammenfassungVersorgungsengpässe in der ambulanten Psychotherapie haben in den letzten Jahren zu diversen Gesetzesinitiativen geführt. Eine davon war die Einführung von diagnosespezifischen Behandlungskontingenten. Ausgehend von diesem gesundheitspolitischen Vorhaben fragten wir uns, inwiefern empirische Daten für oder gegen eine solche Praxis sprechen, das heißt, wie „homogen“ Patient:innen mit derselben ICD-10-Diagnose aus psychodynamischer Sicht sind. Mithilfe der qualitativen Inhaltsanalyse wurden 13 Berichte von Patient:innen mit der Erstdiagnose F33.1 (Rezidivierende depressive Störung, gegenwärtig mittelgradige Episode) ausgewertet, die im Rahmen des Gutachtenverfahrens geprüft wurden. Zusätzlich schätzten wir das generelle Funktionsniveau ein und verglichen die Reife der Abwehr mit dem Vorliegen komorbider psychischer Diagnosen. Es wurden 56 Seiten Textmaterial analysiert. Wir identifizierten 4 Konflikttypen (Selbstwert, Nähe-Distanz, Unterwerfung, Schuld) sowie strukturelle Beeinträchtigungen in 4 Bereichen (Selbst- und Objektwahrnehmung, Selbstregulierung, Bindung und Identität), bei Vorliegen verschiedener Therapieziele und Interaktionsformen. Die Abwehr bewegte sich auf unreifem bis neurotischen Niveau, was nicht im Zusammenhang mit dem Vorliegen komorbider Diagnosen stand. Das Funktionsniveau ließ auf ein breites Spektrum genereller Beeinträchtigung schließen. Unsere Ergebnisse legen nahe, dass Depressionen vielfältige psychodynamische Hintergründe haben. Dies steht im Widerspruch zu der Idee einer Zuordnung von ICD-10-Diagnose und Behandlungskontingent.

BackgroundThis study examined 5-year overall, recurrence and distant metastasis-free survival (OS, RFS, MFS) of high- and intermediate-risk breast cancer (BC) patients who declined guideline-recommended adjuvant chemotherapy (CHT).MethodsIn the prospective multicenter cohort study BRENDA II, patients with primary BC were sampled over a period of four years (2009–2012). A multi-professional team (tumorboard) discussed recommendation for adjuvant CHT according to the German guideline. Potential differences in 5 year survival were analyzed using Kaplan–Meier curves and Cox regression. The hazard ratios (HR) were adjusted for age, Charlson Comorbidity Score, American Society of Anesthesiologist (ASA) physical status classification, and endocrine therapy.ResultsA total of 759 patients were enrolled of which 688 could receive CHT according to the guidelines (n = 219 had a clear indication, inn = 304 it was possible). For 360 patients, the tumorboard advised to perform CHT, for 304 it advised against and in 24 cases, no decision was documented. Of those with a positive suggestion, 83% received CHT. Until 5 years after diagnosis, 57 patients were deceased, 41 had at least one distant metastasis and 29 a recurrence. There was no evidence for differences in OS and MFS in patients who declined CHT despite tumorboard recommendation (HR 3.5, 95% CI 0.8–15.1 for OS, HR 1.9, 95% 0.6–6.6 for MFS). Patients who received CHT had significantly better 5-year RFS compared to those who declined (HR 0.3, 95% CI 0.1–0.9,p = 0.03). There was no evidence for different survival in those who had no CHT because of comorbidity and those who declined actively, neither for OS, MFS nor RFS.ConclusionThe prospective BRENDA II study demonstrates benefit in RFS by guideline adherence in adjuvant breast cancer treatment, indicating prospectively the value of internationally validated guidelines in breast cancer care.

Zusammenfassung Ziel der Studie Ziel der Studie war herauszufinden, welche Erwartungen KrebspatientInnen an professionelle psychosoziale Unterstützung haben. Methodik Im Rahmen einer multizentrischen prospektiven Studie wurden KrebspatientInnen zwei Jahre nach ihrer Diagnose befragt, was sie von professioneller psychosozialer Unterstützung erwarten. Aus den Freitextantworten wurden gemäß qualitativer Methoden Kategorien gebildet und alle Antworten kodiert. Ergebnisse 447 PatientInnen wurden kontaktiert, für die Auswertung lagen 285 Antworten vor. PatientInnen war der Erhalt von Informationen sowie Austausch mit genug Zeit für Zuhören wichtig. Als Resultat der Unterstützung wünschten sie sich vor allem die Förderung psychischer Stärke. Diskussion Neben den Inhalten sind Aspekte des „wie“ der Unterstützung sehr wichtig, vor allem Aufmerksamkeit und fachliche Kompetenz. Es sollte genug Zeit für das Eingehen auf individuelle Bedürfnisse zur Verfügung stehen. Schlussfolgerung Zusätzlich zu Erhalt von Informationen und Kompetenz sind Zuhören und persönlicher Austausch besonders wichtig für Betroffene.

This study aims to identify pathways patients and their relatives take to outpatient psychosocial cancer counselling centres. We had a special interest in how access for men can be eased.Cancer patients and relatives were purposively sampled in two regions in Germany. Participants were either outpatient cancer counselling centres (OCCCs) users or non-users and participated in qualitative face-to-face interviews. We used different guidelines for users and non-users. The interviews were analysed using content analysis.One hundred and three people participated in the study. Important pathways to outpatient psychosocial cancer counselling centres for both men and women were: information about the service and its content, easy access (obtaining appointments quickly and without bureaucracy, close to home), and recommendations from another person, in particular from their treating physician. Pathways especially important for men are positive and repeated recommendations from their treating physician and other people they trust, organisation by others on the men's behalf, the Internet, the possibility to talk to a male counsellor, making it a routine in the hospital to refer distressed patients to the counselling services, and the emphasis on information sharing. Women reported more often than men that they discovered and accessed OCCCs via information material.Men in particular need recommendations from others, especially from their treating physician, in order to make use of psychosocial cancer counselling. In addition, stressing the provision of information instead of exploring and expressing emotions can ease access for men to cancer counselling.

Zusammenfassung Hintergrund Knochensarkome sind eine Gruppe sehr seltener maligner Tumoren. Es existieren nur wenige Studien zur psychischen Belastung der betroffenen Patienten. Ziel war es, die Prävalenz erhöhter psychischer Belastung in dieser Gruppe zu ermitteln, damit assoziierte Faktoren zu untersuchen und die Inanspruchnahme psychoonkologischer Angebote zu erfassen. Methode Die Kohortenstudie PROSa (Krankheitslast und Versorgungssituation bei Sarkomen) wurde zwischen 2017 und 2020 in 39 deutschen Studienzentren durchgeführt. Für die vorliegende Analyse wurden Baseline-Querschnittsdaten von erwachsenen Knochensarkompatienten ausgewertet. Die psychische Belastung wurde mit dem Patient Health Questionnaire (PHQ-4) evaluiert. Sozioökonomische und klinische Faktoren wurden als mögliche Prädiktoren erhöhter psychischer Belastung mit multivariablen logistischen Regressionsmodellen exploriert. Resultate Bei den 194 eingeschlossenen Patienten betrug die Prävalenz von Ängsten 18 %, die von Depressivität 22 %. Insgesamt waren 29 % der Patienten überschwellig psychisch belastet. 23 % hatten eine psychoonkologische Betreuung in Anspruch genommen. Im vollen Modell waren arbeitslose Patienten (Odds Ratio [OR] 5,7; 95 %-Konfidenzintervall [CI] 1,6–20,0) und Patienten mit Erwerbsminderungsrente (OR 3,6; 95 %-CI 1,03–12,9) im Vergleich zu solchen in Beschäftigung häufiger belastet, Patienten mit Altersrente, in Vorruhestand oder in Altersteilzeit dagegen weniger häufig (OR 0,2; 95 %-CI 0,05–0,9). Die Häufigkeit psychischer Belastung war bei Patienten 5 Jahre nach Diagnose (Vergleich Diagnose Konklusion Die Prävalenz erhöhter psychischer Belastung bei Knochensarkompatienten ist hoch. Arbeitslose Patienten, solche mit Erwerbsminderungsrente sowie neu diagnostizierte Patienten sind besonders vulnerabel. Das Behandlungsteam sollte sich dieser Faktoren bewusst sein und auch diese sozialen Aspekte der Erkrankung berücksichtigen.

Background Sarcomas are rare cancers of high heterogeneity. Health-Related Quality of Life (HRQoL) has been shown to be a prognostic factor for survival in other cancer entities but it is unclear whether this applies to sarcoma patients. Patients and methods HRQoL was prospectively assessed in adult sarcoma patients from 2017 to 2020 in 39 German recruiting sites using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30). Vital status was ascertained over the course of 1 year. HRQoL domains were analysed by multivariable cox-regressions including clinical and socio-economic risk factors. Results Of 1102 patients, 126 (11.4%) died during follow-up. The hazard ratio (HR) for global health was 0.73 per 10-point increase (95% confidence interval (CI) 0.64–0.85). HR for the HRQoL-summary score was 0.74 (CI 0.64–0.85) and for physical functioning 0.82 (CI 0.74–0.89). There was also evidence that fatigue (HR 1.17, CI 1.10–1.25), appetite loss (HR 1.15, CI 1.09–1.21) and pain (HR 1.14, CI 1.08–1.20) are prognostic factors for survival. Conclusion Our study adds sarcoma-specific evidence to the existing data about cancer survival in general. Clinicians and care-givers should be aware of the relations between HRQoL and survival probability and include HRQoL in routine assessment.

Background Outpatient cancer counselling centres (OCCC; in German Krebsberatungsstellen [KBS]) offer psychological, social and sociolegal support for people with cancer and their relatives, who are confronted not only with physical but also with psychosocial stress as a result of the disease. Research question How do people become aware of outpatient cancer counselling centres? Methods In a nationwide study, guided qualitative interviews were conducted with people seeking advice (cancer patients and relatives) and referrers (health care providers who refer to OCCC). The interviews with those seeking advice were conducted face-to-face and those with referrers by telephone. The data were analysed using content analysis according to Mayring. Results In all, 43 people seeking advice and 30 referrers were interviewed. A total of 31 h of audio material was recorded. From the respondents' point of view, flyers, information brochures and internet platforms are important sources for making patients and their relatives aware of OCCC. In addition to public relations, the personal approach in the clinical and non-clinical care system and in the social setting is of central importance. Some referrers address OCCC routinely, while others only do so when patients or relatives explicitly express a need for counselling. Conclusions In the medical sector, more attention could and should be drawn to OCCC as a helpful outpatient counselling setting. Closer cooperation between OCCC and general practitioners, oncologists and psycho-oncology services should be sought. Targeted and needs-oriented information can draw attention to the support offered by OCCC and thus improve the care of cancer patients., Hintergrund Ambulante Krebsberatungsstellen (KBS) bieten psychologische, soziale und sozialrechtliche Unterstützung für an Krebs erkrankte Menschen und Angehörige an, die infolge der Erkrankung nicht nur mit körperlichen, sondern auch mit psychosozialen Belastungen konfrontiert werden. Fragestellung Wie werden Menschen auf ambulante Krebsberatungsstellen aufmerksam? Methoden In einer deutschlandweiten Studie wurden leitfadengestützte qualitative Interviews mit Ratsuchenden (Krebspatient/innen und Angehörige) und Zuweisenden (Gesundheitsversorger/innen, die an KBS verweisen) geführt. Die Befragungen der Ratsuchenden erfolgten im persönlichen Gespräch, die der Zuweisenden telefonisch. Ausgewertet wurde inhaltsanalytisch nach Mayring. Ergebnisse Es wurden 43 Ratsuchende und 30 Zuweisende befragt. Insgesamt entstanden 31 h Audiomaterial. Aus Sicht der Befragten sind Flyer, Infobroschüren und Internetplattformen wichtige Quellen, um Patient/innen und deren Angehörige auf KBS aufmerksam zu machen. Neben der Öffentlichkeitsarbeit ist die persönliche Ansprache im klinischen wie nicht-klinischen Versorgungssystem und im sozialen Umfeld von zentraler Bedeutung. Manche Zuweisenden sprechen KBS routinemäßig an, andere erst bei explizit von Patient/innen oder Angehörigen geäußertem Beratungsbedarf. Schlussfolgerungen Im medizinischen Sektor könnte und sollte noch mehr auf KBS als hilfreiches ambulantes Beratungssetting hingewiesen werden. Eine engere Zusammenarbeit der KBS mit Hausärzt/innen, Onkolog/innen und psychoonkologischen Diensten ist anzustreben. Gezielte und bedarfsgerechte Informationen können auf das Unterstützungsangebot durch eine KBS hinweisen und so die Versorgung von Krebspatient/innen verbessern.

Treatment concepts in oncology are becoming increasingly personalized and diverse. Successively, changes in standards of care mandate continuous monitoring of patient pathways and clinical outcomes based on large, representative real-world data. The German Cancer Consortium’s (DKTK) Clinical Communication Platform (CCP) provides such opportunity. Connecting fourteen university hospital-based cancer centers, the CCP relies on a federated IT-infrastructure sourcing data from facility-based cancer registry units and biobanks. Federated analyses resulted in a cohort of 600,915 patients, out of which 232,991 were incident since 2013 and for which a comprehensive documentation is available. Next to demographic data (i.e., age at diagnosis: 2.0% 0–20 years, 8.3% 21–40 years, 30.9% 41–60 years, 50.1% 61–80 years, 8.8% 81 + years; and gender: 45.2% female, 54.7% male, 0.1% other) and diagnoses (five most frequent tumor origins: 22,523 prostate, 18,409 breast, 15,575 lung, 13,964 skin/malignant melanoma, 9,005 brain), the cohort dataset contains information about therapeutic interventions and response assessments and is connected to 287,883 liquid and tissue biosamples. Focusing on diagnoses and therapy-sequences, showcase analyses of diagnosis-specific sub-cohorts (pancreas, larynx, kidney, thyroid gland) demonstrate the analytical opportunities offered by the cohort’s data. Due to its data granularity and size, the cohort is a potential catalyst for translational cancer research. It provides rapid access to comprehensive patient groups and may improve the understanding of the clinical course of various (even rare) malignancies. Therefore, the cohort may serve as a decisions-making tool for clinical trial design and contributes to the evaluation of scientific findings under real-world conditions.

Introduction: Early guidelines for minimally important differences (MID) for the EORTC QLQ-C30 proposed ≥10 points change as clinically meaningful for all scales. Increasing evidence that MIDs can vary by scale, direction of change, cancer type and estimation method has raised doubt about a single global standard. This paper identifies MID patterns for interpreting group-level change in EORTC QLQ-C30 scores across nine cancer types. Methods: Data were obtained from 21 published EORTC Phase III trials that enrolled 13,015 patients across nine cancer types (brain, colorectal, advanced breast, head/neck, lung, mesothelioma, melanoma, ovarian, and prostate). Anchor-based MIDs for within-group change and between-group differences in change over time were obtained via mean change method and linear regression respectively. Separate MIDs were estimated for improvements and deteriorations. Distribution-based estimates were derived and compared with anchor-based MIDs. Results: Anchor-based MIDs mostly ranged from 5 to 10 points. Differences in MIDs for improvement vs deterioration, for both within-group and between-group, were mostly within a 2-points range. Larger differences between within-group and between-group MIDs were observed for several scales in ovarian, lung and head/neck cancer. Most anchor-based MIDs ranged between 0.3 SD and 0.5 SD distribution-based estimates. Conclusions: Our results reinforce recent claims that no single MID can be applied to all EORTC QLQ-C30 scales and disease settings. MIDs varied by scale, improvement/deterioration, within/between comparisons and by cancer type. Researchers applying commonly used rules of thumb must be aware of the risk of dismissing changes that are clinically meaningful or underpowering analyses when smaller MIDs apply.

Zusammenfassung Hintergrund und Fragestellung Die Psychotherapiestrukturreform im Jahr 2017 verfolgte u. a. das Ziel, die Wartezeiten auf Psychotherapien zu verringern. Diese Studie ging der Frage nach, ob dieses Ziel erreicht wurde. Methoden Psychotherapiepraxen aus 7 Bundesländern entnahmen ihren Akten die Daten für Erstkontakt, Erstgespräch und Behandlungsbeginn. Der Vergleich der Wartezeiten auf ein Erstgespräch und auf den Beginn der Behandlung vor vs. nach der Reform erfolgte mithilfe von Kaplan-Meier-Kurven und stratifizierten Log-Rank-Tests. Ergebnis Akten von 1548 Patienten aus 9 Praxen wurden ausgewertet. Die durchschnittliche Wartezeit auf ein Erstgespräch lag sowohl vor als auch nach der Reform bei 3 Wochen (p = 0,20). Die Zeit zwischen der Anmeldung und dem Behandlungsbeginn erhöhte sich von durchschnittlich 18 auf 20 Wochen, wobei nach der Reform v. a. die sehr kurzen Wartezeiten (p = 0,0004). Diskussion Die in den Praxen dokumentierten Daten liefern keine Anhaltspunkte dafür, dass sich die Wartezeit auf Erstgespräche bei Psychotherapeuten verkürzt hat. Das Warten auf einen Psychotherapieplatz scheint insgesamt etwas länger geworden zu sein, sodass in dieser Hinsicht keine Verbesserung der Versorgung durch die Reform festgestellt werden konnte.

Zusammenfassung Hintergrund Der Prozess des Studieneinschlusses von Teilnehmenden ist ein wichtiger Bestandteil und oftmals aufwendiger Arbeitsschritt in wissenschaftlichen Forschungsprojekten. Aufgrund unterschiedlicher finanzieller, organisatorischer und ethischer Rahmenbedingungen stellt die fristgerechte Gewinnung von Teilnehmenden haufig eine Herausforderung dar. In diesem Beitrag berichten wir uber unsere Erfahrungen hinsichtlich des Studieneinschlusses, die wir im Projekt „Psychoonkologische Versorgung von Krebspatienten mit Migrationshintergrund – eine Mixed-Methods-Studie“ (POM) gemacht haben. Methode Die Gewinnung von Teilnehmenden erfolgte uber ambulante hamato-onkologische Schwerpunktpraxen. Patient*innen und Angehorige wurden in erster Linie durch die behandelnden Arzt*innen auf das Projekt aufmerksam gemacht und fur eine Teilnahme an qualitativen Interviews gewonnen. Ergebnisse Es wurden bundesweit neun Patient*innen und Angehorige in qualitativen Einzelinterviews befragt. Abgesehen von der COVID-19-Pandemie gab es weitere Herausforderungen beim Studieneinschluss. In vielen Fallen wurde eine Teilnahme von Patient*innen abgelehnt. Grunde hierfur waren u.a. „kein Interesse“, der Gebrauch der Begriffe „Psychoonkologie“ oder „Mensch mit Migrationshintergrund“, ein Verbot der Teilnahme durch die Angehorigen, kein personliches Interview aufgrund der COVID-19-Pandemie sowie eine zu starke emotionale Belastung und Verschlechterung des Gesundheitszustands nach vorheriger Einwilligung. Diskussion Die Gewinnung von Studienteilnehmenden aus vulnerablen Gruppen fur ein sensibles Thema wie der Psychoonkologie bringt vielfache Herausforderungen mit sich. Um dennoch einen erfolgreichen Studieneinschluss zu erlangen, erwies sich ein regelmasiger telefonischer und schriftlicher Austausch mit den jeweils zustandigen Praxismitarbeitenden als hilfreich, wodurch ein Uberblick uber Herausforderungen erlangt und der Studieneinschluss evaluiert und zeitnah angepasst werden konnte. Schlussfolgerung In Forschungsprojekten mit vulnerablen Gruppen und zu sensiblen Themen sollten sowohl eine zielgruppenspezifische, feinfuhlige und laienverstandliche Ansprache als auch regelmasige Rucksprachen mit den Personen erfolgen, die fur den Studieneinschluss in Versorgungseinrichtungen verantwortlich sind, um ggf. im Laufe des Studieneinschlusses das bisherige Verfahren anzupassen. Basierend auf den Erfahrungen der POM-Studie werden Losungsansatze fur Versorgungsforschungsstudien mit vulnerablen Gruppen und bei sensiblen Themen aufgezeigt.

OBJECTIVES: We aimed to assess patterns of care delivered to older patients with locally advanced head and neck squamous cell carcinoma (LA-HNSCC), and to analyze the use of geriatric assessment (GA) and assessment of quality of life (QoL).MATERIALS AND METHODS: Members of the head and neck cancer group and the older task force of the European Organisation for Research and Treatment of Cancer (EORTC), members the European Head and Neck Society and members of national groups in Europe were asked to complete a questionnaire about treatment delivered, use of GA, and QoL assessment in older patients with LA-HNSCC.RESULTS: Investigators from 111 centers replied, including 90 (81.1%) academic centers, 16 (14.4%) community hospitals, and 5 (4.5%) private clinics. Large differences in treatment patterns were found. For instance, for oropharyngeal carcinoma, one third of the centers indicated that they treat 40% of older patients with chemoradiation. Fourteen centers (12.6%) routinely perform GA, while 43 centers (38.7%) never do, and 39 centers (35.1%) sometimes do. QoL is assessed on a routine basis in one fifth of the centers.CONCLUSIONS: Large differences exist across institutions in the patterns of care delivered to older patients with LA-HNSCC. Prospective studies are required to learn how GA can guide treatment decisions, and how QoL and treatment outcome can be improved. For that, consensus on standard of care is essential.

The reform of the German psychotherapy directive in 2017 resulted in several changes for non-hospital-based psychotherapists. We examined whether patients received psychotherapy following their initial consultation more frequently after the reform than before.Using records from psychotherapy practices in a retrospective cohort study, we investigated the frequency of a psychotherapy after the patients' initial consultation. We compared a time frame of three years before and after the reform. A potential association between reform and psychotherapy received was analysed via logistic regression.Our analysis comprised 1548 records from 9 psychotherapy practices, consisting of 755 records before and 793 records after the reform. Before the reform, 40% (n=303) of the patients who had an initial consultation went on to receive psychotherapy. After the reform, 46% (n=360) of initial consultations were followed by psychotherapy (including acute care). The logistic regression analysis showed a 36% increased probability of receiving psychotherapy after an initial consultation after the reform in comparison to before the reform (odds ratio 1.36 after vs. before reform, 95% confidence interval 1.09 - 1.68).Our results indicate a higher chance of receiving psychotherapy after an initial consultation after the reform compared to before the reform. Our data do not contain information as to whether the absolute number of psychotherapy appointments in the practices increased after the reform.Die Psychotherapiestrukturreform im Jahr 2017 bedingte einige Änderungen in der Arbeit niedergelassener Psychotherapeut*innen. Wir untersuchten, ob nach der Reform auf ein Erstgespräch häufiger eine Psychotherapie folgte als vor der Reform.Im Rahmen einer retrospektiven Kohortenstudie untersuchten wir anhand von Aktenanalysen in psychotherapeutischen Praxen, wie häufig nach einem Erstgespräch eine psychotherapeutische Behandlung begonnen wurde. Verglichen wurde der Zeitraum drei Jahre vor und nach der Reform. Ein möglicher Zusammenhang zwischen der Reform und dem Zustandekommen einer Psychotherapie wurde mittels logistischer Regression analysiert.Insgesamt 1548 Akten aus 9 Praxen konnten ausgewertet werden, davon 755 vor und 793 nach der Reform. Vor der Reform erhielten 40% (n=303) der Patient*innen, die ein Erstgespräch in den Praxen geführt hatten, anschließend eine Richtlinientherapie. Nach der Reform folgte auf 46% (n=360) der Erstgespräche eine psychotherapeutische Behandlung (Richtlinientherapie oder Akutbehandlung). Die logistische Regression ergab eine um 36% erhöhte Wahrscheinlichkeit, nach der Reform eine Behandlung zu erhalten im Vergleich zu vor der Reform (Odds Ratio 1,36 nach vs. vor Reform, 95% Konfidenzintervall 1,09 – 1,68).Unsere Studienergebnisse legen nahe, dass Patient*innen nach der Reform höhere Chancen auf eine weiterführende Behandlung nach einem Erstgespräch haben als vor der Reform. Ob in den Praxen absolut mehr Behandlungen durchgeführt wurden, lässt sich aus unseren Daten nicht schließen.
