Abstract Background Same-day antiretroviral therapy (SDART) initiation, in which people living with HIV (PLHIV) who are antiretroviral therapy (ART)-naïve, willing, and clinically eligible start ART on the same day of HIV diagnosis, has been implemented in several healthcare facilities in Thailand since 2017. This evidence-based practice has demonstrated increased ART uptake, virologic suppression, and retention in care. However, linkage to care gaps exist in community-based organizations (CBOs) in Bangkok whereby as much as 20% of key populations (KP), mainly men who have sex with men and transgender women, living with HIV were lost to follow-up pre-ART initiation. To increase access to and uptake of ART among these populations, this study proposes that trained KP lay providers should lead community-based ART (CB-SDART) initiation service. This protocol describes the combined use of the Proctor’s implementation outcome framework and the Consolidated Framework for Implementation Research to guide and evaluate the CB-SDART implementation. Methods This study follows the hybrid design type 3: it is an implementation trial that secondarily assesses service and client outcomes by comparative interrupted time series analysis. Five strategies have been formulated to meet three implementation outcomes (i.e., feasibility, fidelity, and sustainability): (1) developing stakeholder relationships by engaging the CBO leaderships, (2) training and educating KP lay providers, (3) adapting and tailoring SDART to CBO-specific context, (4) using evaluative and iterative strategies to assess adherence to standard operating procedures, and (5) developing stakeholder relationships by engaging external stakeholders. Teleconsultation with physicians and ART home delivery will be integrated as another ART initiation option for clients and allow service provision during the COVID-19 pandemic. A mixed-method assessment will be conducted on key stakeholders and PLHIV diagnosed at two implementing CBOs, Rainbow Sky Association of Thailand and Service Workers in Group Foundation, in Bangkok, Thailand. Discussion This implementation research may be the first to provide robust data at the implementation, service, and client levels to inform how to successfully task-shift SDART initiation service to trained KP lay providers and facilitate the expansion of CB-SDART in the future. Trial registration This trial was registered with the Thai Clinical Trial Registry as TCTR20210709004 on July 9, 2021.

This qualitative study explores the impact of non-pharmaceutical interventions (NPIs), including social distancing, travel restrictions and quarantine, on lived experiences during the first wave of the COVID-19 pandemic in Thailand (TH), Malaysia (MY), Italy (IT) and the United Kingdom (UK). A total of 86 interviews (TH: n = 28; MY: n = 18; IT: n = 20; UK: n = 20) were conducted with members of the public, including healthcare workers (n = 13). Participants across countries held strong views on government imposed NPIs, with many feeling measures lacked clarity. Most participants reported primarily negative impacts of NPIs on their lives, including through separation, isolation and grief over missed milestones; work-related challenges and income loss; and poor mental health and wellbeing. Nonetheless, many also experienced inadvertent positive consequences, including more time at home to focus on what they most valued in life; a greater sense of connectedness; and benefits to working life. Commonly employed coping strategies focused on financial coping (e.g. reducing spending); psycho-emotional coping (e.g. engaging in spiritual practices); social coping and connectedness (e.g., maintaining relationships remotely); reducing and mitigating risks (e.g., changing food shopping routines); and limiting exposure to the news (e.g., checking news only occasionally). Importantly, the extent to which participants' lived experiences were positive or negative, and their ability to cope was underpinned by individual, social and economic factors, with the analysis indicating some salient differences across countries and participants. In order to mitigate negative and unequal impacts of NPIs, COVID-19 policies will benefit from paying closer attention to the social, cultural and psychological-not just biological-vulnerabilities to, and consequences of public health measures.

The durability of a three-dose extended primary series of COVID-19 vaccine in dialysis patients remains unknown. Here, we assessed dynamic changes in SARS-CoV-2-specific humoral and cell-mediated immunity at baseline, 3 months, and 6 months after the extended primary series in 29 hemodialyzed (HD), 28 peritoneal dialyzed (PD) patients, and 14 healthy controls. Participants received two doses of inactivated SARS-CoV-2 vaccine followed by a dose of ChAdOx1 nCoV-19 vaccine. At 6 months, median anti-RBD IgG titers (IQR) significantly declined from baseline in the HD (1741 (1136–3083) BAU/mL vs. 373 (188–607) BAU/mL) and PD (1093 (617–1911) BAU/mL vs. 180 (126–320) BAU/mL) groups, as did the mean percent inhibition of neutralizing antibodies (HD: 96% vs. 81%; PD: 95% vs. 73%) (all p < 0.01). Age and post-vaccination serological response intensity were predictors of early humoral seroprotection loss. In contrast, cell-mediated immunity remained unchanged. In conclusion, humoral immunity declined substantially in dialysis patients, while cell-mediated immunity remained stable 6 months after the extended heterologous primary series of two inactivated SARS-CoV-2/ChAdOx1 nCoV-19 vaccine. A booster dose could be considered in dialysis patients 3 months after this unique regimen, particularly in the elderly or those with a modest initial humoral response.

Power quality disturbance (PQD) is an important issue in electrical distribution systems that needs to be detected promptly and identified to prevent the degradation of system reliability. This work proposes a PQD classification using a novel algorithm, comprised of the artificial bee colony (ABC) and the particle swarm optimization (PSO) algorithms, called “adaptive ABC-PSO” as the feature selection algorithm. The proposed adaptive technique is applied to a combination of ABC and PSO algorithms, and then used as the feature selection algorithm. A discrete wavelet transform is used as the feature extraction method, and a probabilistic neural network is used as the classifier. We found that the highest classification accuracy (99.31%) could be achieved through nine optimally selected features out of all 72 extracted features. Moreover, the proposed PQD classification system demonstrated high performance in a noisy environment, as well as the real distribution system. When comparing the presented PQD classification system’s performance to previous studies, PQD classification accuracy using adaptive ABC-PSO as the optimal feature selection algorithm is considered to be at a high-range scale; therefore, the adaptive ABC-PSO algorithm can be used to classify the PQD in a practical electrical distribution system.

Introduction: The superficial palmar arch (SPA) is the main source of arterial supply to the palm. It is an arterial arcade formed mainly by the ulnar artery and is completed by the superficial palmar branch of the radial artery. The study was conducted with objective to study the variations in formation of superficial palmar arch in Nepalese cadaveric hands. Methodology: Two horizontal incisions were made, one at the wrist joint and another incision along the root of fingers. A vertical incision was made from the middle of the wrist to the 3rd metacarpo-phalangeal joint. The skin of the palm and the palmar aponeurosis was reflected. The surrounding adipose tissue was removed to view the superficial palmar arch and its branches. Result: In present study, out of 50 specimens of hands, 64% of superficial palmar arch was formed as normal pattern and in 36% cases it was formed by ulnar artery alone. Local wisdom has values that often become the source of norms formation for people's life. Among them is the tradition of Labuhan Gunung Kombang in Malang. This study intended to reveal the Labuhan Gunung Kombang's local wisdom that grows and develops in harmony with Islamic values in Kedungsalam Village, Donomulyo, Malang. This is a descriptive qualitative research by employing ethnographic methods. Data were collected through observations, interviews and documentations to produce detailed and comprehensive accounts of different cultural and social phenomena. The result of study showed that local wisdom of Labuhan Gunung Kombang tradition has values of religion, togetherness, deliberation and mutual cooperation that are in line with Islamic teachings. Islam as a religion with a set of values and norms has influenced the patterns of culture and traditions for Kedungsalam people. However, the encounter of Islamic values and local wisdom in Labuhan Gunung Kombang ceremony is usually perceived as the process of cultural acculturation. It makes socio-cultural aspects of the local people not necessarily eroded by Islam.

& This study describes characters and moral values in the Javanese Novel entitled Kepanggang Wirang (Burned Above the Shame) by Tiwiek SA. Description dealt with psycholiguical needs in accordance with Abraham Maslow theory and educational values. This study is qualittaive research and applied psychoanalysis as the research design. Data were collected using contant analysis and interview and were analyzed using content analysis that included: taxonomy, domain, and thematic analyses. The study revealed that psychological needs from Maslow including: sex, security, self-esteem, and love and affection, and self-actualization were present in the novel. In addition, moral values having positive and negative perspectives were conveyed in the novel reflecting experiences in the society the readers should have considered in the real community lifes. .

This study describes intrinsic structure of the Novel of Kepanggang Wirang (Burned Above the Shame) by Tiwiek SA and its implication to Javanese teaching at SMA. This study is qualittaive research and applied psychoanalysis as the research design. Data were collected using content analysis and interview. The study revealed that structural parts of the novel included: characters, plots, setting, theme, title, point of view, style and tone, symbolism, and irony. In addition, Kepanggang Wirang is relevant to teaching Javanese in SMA as its language and education values are in support to Basic Competence in SMA Curriculum.

The purpose of writing this article is to analyze the discourse of heroism ( kawiryan ) according to Sultan Hamengkubuwana VII contained in the Serat Saptstha manuscript. It is a type of historical manuscript that provides an overview about the discourse stored in the content of the manuscript. The Sultan discussed in this article is Hamengkubuwana VII. He is an Islamic Sultan who ruled the societies in 1877-1921. His reign is in Keraton Ngayogyakarta (Ngayogyakarta Palace) and it is a kind of Islamic Sultanate. Based on the conclusion of this study, Hamengkubuwana VII as the King sent by God performed an image as a very humble sultan. The legitimacy of his power promotes an understanding of Islamic Sultanate represented by Hamengkubuwana VII. The representation of peaceful and serene life portrayed by Hamengkubuwana VII makes the image of Hamengkubuwana VII to be an irreplaceable sultan and his personality also inspires the societies in terms of the importance to always remember The Creator or on the other word is God.

This research study seeks to examine the shift in politeness methods employed by Javanese woman characters in the English translation of Pramoedya Ananta Toer's Indonesian novel Gadis Pantai. The original Indonesian version and its English translation were used to collect the data. As the politeness strategies of Javanese woman characters in the Indonesian novel 'Gadis Pantai' carry pragmatic meaning and cultural reflection in that they reveal how they communicate in particular circumstances of various relations of power, distance, and imposition, the shifts of politeness strategies in the English translation undermine their original intention and their intended cultural representation that constructs the character of the Javanese women in the novel. This study aims to elucidate the types of politeness strategy modifications observed in the translation and to explain the effect of these shifts on the novel's Javanese lady characters. In classifying and analyzing the data, the researcher utilized Brown & Levinson's taxonomy of politeness super-strategies and associated sub-strategies. Three individuals were validators for categorizing politeness tactics and their shift mapping in this study. The study indicated that out of 529 total translated utterances that contain politeness methods, 205 utterances undergo a shift. The shifts include the substitution of one super-strategy for another in the translation, substituting one sub-strategy for another of the same super-strategy, and substituting one sub-strategy for another of different super-strategies, and the omission of the politeness strategies in the translation. Negative politeness with the 'giving deference' sub-type becomes the most frequently shifting super-strategy and sub-strategy. This is because the concept of reverence that the Javanese woman characters use in the discourse is foreign to the target language's culture.

Background and Aim: Free-grazing duck (FGD) raising is a unique domestic duck production system that is widely practiced in several Asian countries, including Thailand. FGD is a significant reservoir for influenza A viruses (IAVs). In this study, we genetically characterized IAV-H10N6 and IAV-H10N7 isolated from avian influenza surveillance in FGDs in Thailand. Materials and Methods: We collected 640 swab samples from 29 FGD flocks located in 6 provinces of Thailand. IAVs were isolated from swab samples using egg inoculation. Hemagglutination test-positive samples were then subjected to IAV detection. Viral RNA was subjected to IAV detection using real-time reverse-transcription polymerase chain reaction (rRT-PCR) specific to matrix (M) gene. IAV subtypes were identified using the RT-PCR assay specific to all hemagglutinin and neuraminidase subtypes. Whole-genome sequencing of IAVs was performed to genetically characterize IAV-H10N6 and IAV-H10N7. Results: Our results showed that 41 (6.41%) samples tested positive for IAV using rRT-PCR specific to the M gene. Among these, only two IAVs were subtypes as IAV-H10N6 and IAV-H10N7 and were subjected to whole-genome sequencing. IAV-H10N6 and IAV-H10N7 belonged to the Eurasian lineage and did not show any evidence of reassortment from the North American lineage. The viruses exhibited low-pathogenic characteristics and preferred binding to avian-type receptors. Genetic analysis revealed no mutations in PB2 and M genes, unlike human IAV-H10N3 and IAV-H10N8, which exhibited increased virulence in mammals. Conclusion: IAV-H10N6 and IAV-H10N7 viruses have less potential as zoonotic viruses. However, IAV in FGDs should be monitored for novel reassortant or zoonotic viruses. This study provides information on the genetic characteristics and diversity of IAV-H10N6 and IAV-H10N7 that are circulated in FGDs in Thailand.

Abstract The use of antimicrobials in Thailand has been reported as one of the highest in the world in human and animal sectors. Our engagement project aimed to improve our understanding of the issue of antimicrobial use and antimicrobial resistance (AMR) among adult Thai communities, and co-create locally relevant solutions to AMR, especially those focusing on raising awareness to improve related policies in Thailand. We conducted a series of online and in-person ‘conversations’ according to Wellcome’s ‘Responsive Dialogues’ engagement approach, designed to bring together different voices to understand complex AMR problems and find potential solutions. This approach enabled key AMR stakeholders and policy makers to hear directly from communities and members of the public, and vice versa. Conversations events took place between 25 November 2020 and 8 July 2022, and we engaged 179 AMR key stakeholders and members of the public across Thailand. The issues found were: there were quite a lot of misunderstandings around antimicrobials and AMR; participants felt that communications and engagement around antimicrobial resistance had limited reach and impact; asking for and taking antibiotics for self-limiting ailments is a social norm in Thailand; and there appeared to be a wide availability of cheap antimicrobials. To mitigate the spread of AMR, participants suggested that the messages around AMR should be tailored to the target audience, there should be more initiatives to increase general health literacy, there should be increased availability of AMR related information at the local level and there should be increased local leadership of AMR mitigation efforts. Peri-implantitis, a prevalent complication in dental implant therapy, poses a significant threat to long-term implant success. The identification of reliable biomarkers for the early detection and monitoring of peri-implantitis is crucial for timely intervention and improved treatment outcomes. Salivary and peri-implant sulcular fluid (PISF) biomarkers have become promising diagnostic tools in the field of implant dentistry. This scoping review aims to explore current studies in the literature on salivary and PISF biomarkers for peri-implantitis. A systematic search was conducted on 2 databases (PubMed and Scopus) to identify relevant studies published up to January 2023. A total of 86 articles were included, which underwent data extraction and analysis. Several biomarkers have been investigated in salivary and PISF samples for association with peri-implantitis. Investigations included a wide range of biomarkers, including inflammatory markers, matrix metalloproteinases and bone loss markers. The findings suggested that certain salivary and PISF biomarkers demonstrated potential in distinguishing healthy peri-implant conditions from peri-implantitis. Elevated levels of proinflammatory cytokines, such as interleukin-1β (IL-1β) and interleukin-6 (IL-6), tumour necrosis factor-alpha (TNF-α), and matrix metalloproteinases, have been consistently associated with peri-implantitis. Additionally, alterations in bone loss markers have shown potential as indicators of disease progression and treatment response. In conclusion, this scoping review provides an overview of current knowledge on salivary and PISF biomarkers for peri-implantitis. The identified biomarkers are promising as noninvasive diagnostic tools for early detection, monitoring, and personalised management of peri-implantitis. Future studies should focus on establishing standardised protocols and conducting well-designed clinical trials to validate the diagnostic accuracy and clinical relevance of these biomarkers.

Literary works provide many uses for life. One of the benefits of literary works is the character education value. These values can be reflected in Jaka Tingkir folklore which is a wealth of Javanese literature and culture. This research aimed to describe the five main values of character education in Jaka Tingkir folklore. This research was a folklore approach. This research was conducted in Butuh, Gedongan, Plupuh, Sragen, Central Java, Indonesia. Data sources in this research consisted of two types. The caretaker of Jaka Tingkir's grave, kebayan in Gedongan, the Head of the Cultural Development Division at the Education and Culture Office of Sragen Regency, and the language teacher at the Vocational High School Plupuh 1 as primary data sources. The book Central Java Folklore Jaka Tingkir, novel epik Jaka Tingkir: Jalan Berliku Menjemput Wahyu, and other documents as secondary data sources. The data in this research were collected by interview. The analytical method used functional and document analysis. The results of this research are the five main values of character education in Jaka Tingkir folklore which consist of five values, namely religious, nationalist, gotong royong, independent, and integrity. These values are reflected in Jaka Tingkir as the main figure and side figures, such as Jaka Tingkir, Ki Ageng Banyubiru, Ki Kebokenanga, Sunan Kudus, Ki Wuragil, and forty crocodiles. Abstract Background and Aims Coronavirus disease 2019 (COVID‐19) has become a global pandemic and led to increased mortality and morbidity. Vaccines against the etiologic agent; severe acute respiratory syndrome coronavirus‐2 (SARS‐CoV‐2) were approved for emergency use on different platforms. In the early phase of the pandemic, Thai healthcare workers (HCWs) received CoronaVac, an inactivated vaccine, as the first vaccine against SARS‐CoV‐2, followed by ChAdOx1 nCoV‐19, a viral vector‐based vaccine, or BNT162b2, an mRNA vaccine, as a booster dose. This preliminary study evaluated the immunogenicity of ChAdOx1 nCoV‐19 and BNT162b2 as a booster dose in HCWs who previously received two doses of CoronaVac. Methods Ten HCW participants received ChAdOx1 nCoV‐19 and another 10 HCWs received BNT162b2 as a booster dose after two doses of CoronaVac. Anti‐RBD IgG, neutralizing antibodies (NAb), and cellular immunity, including interferon‐gamma (IFN‐γ)‐releasing CD4, CD8, double negative T cells, and NK cells, were measured at 3 and 5 months after the booster dose. Results There was no significant difference in anti‐RBD IgG levels at 3 and 5 months between the two different types of booster vaccine. The levels of anti‐RBD IgG and NAb were significantly decreased at 5 months. HCWs receiving BNT162b2 had significantly higher NAb levels than those receiving ChAdOx1 nCoV‐19 at 5 months after the booster dose. IFN‐γ release from CD4 T cells was detected at 3 months with no significant difference between the two types of booster vaccines. However, IFN‐γ‐releasing CD4 T cells were present at 5 months in the ChAdOx1 nCoV‐19 group only. Conclusion ChAdOx1 nCoV‐19 or BNT162b2 can be used as a booster dose after completion of the primary series primed by inactivated vaccine. Although the levels of immunity decline at 5 months, they may be adequate during the first 3 months after the booster dose.

Abstract Introduction Men who have sex with men (MSM), especially those living with HIV, are at an increased risk of anal cancer. The prevalence and incidence of its precursor, anal high‐grade squamous intraepithelial lesions (HSILs), among MSM who started antiretroviral therapy during acute HIV acquisition are yet to be explored. Methods Participants in an acute HIV acquisition cohort in Bangkok, Thailand, who agreed to take part in this study, were enrolled. All participants were diagnosed and started antiretroviral therapy during acute HIV acquisition. Human papillomavirus (HPV) genotyping and high‐resolution anoscopy, followed by anal biopsy as indicated, were done at baseline and 6‐monthly visits. Results A total of 89 MSM and four transgender women were included in the analyses. Median age at enrolment was 26 years. Baseline prevalence of histologic anal HSIL was 11.8%. With a total of 147.0 person‐years of follow‐up, the incidence of initial histologic anal HSIL was 19.7 per 100 person‐years. Factors associated with incident anal HSIL were anal HPV 16 (adjusted hazards ratio [aHR] 4.33, 95% CI 1.03–18.18), anal HPV 18/45 (aHR 6.82, 95% CI 1.57–29.51), other anal high‐risk HPV (aHR 4.23, 95% CI 1.27–14.14), syphilis infection (aHR 4.67, 95% CI 1.10–19.90) and CD4 count
