Data on demographics, antiretroviral treatment since HIV diagnosis, and immunological and metabolic parameters from baseline (start of DRVc) to 48 weeks were analyzed in order to assess the cumulative proportion of those who achieved virological success (VS), defined as at least one instance of HIV‐RNA < 50 copies/mL within 12 months from baseline. Follow-up lasted from the start of the DRVc-based regimen (baseline) to the first instance of HIV-RNA < 50 copies/mL, last available visit, or loss to follow‐up or death, whichever occurred first. Univariate and multivariate Cox proportional-hazard regression models were used to identify baseline factors associated with VS.Results: A total of 176 individuals were included, and 120 (68.2%) achieved < 50 HIV‐RNA copies/mL within 12 months since baseline. On multivariate analysis, baseline HDL cholesterol was independently associated with the occurrence of VS (adjusted HR 1.021, 95% CI 1.004– 1.038; p=0.014). Among the 120 subjects with VS, 27 (22.5%) had had VF during a median follow-up of 20.8 months since the first undetectable HIV-RNA. Resistance testing after VF was available in two cases, which harboured the HIV variant–bearing protease inhibitor–resistance mutations D30N, I50V, and N88D. During a median follow-up of 38.4 months, 65 of 176 (36.9%) individuals discontinued DRVc for any reason (37 of 120, 30.8%) and achieved VS vs. 28 of 56 (50%) without VS (p=0.019). Time to discontinuation was longer in people with VS (41.5 vs. 23.0 months, p=0.0007). No statistically significant changes were observed in immunological or lipid profiles during follow-up.Conclusion: Most individuals in this study achieved VS within 12 months from the beginning of a DRVc-based regimen; therefore, this treatment represent a viable option for people who have experienced VF on other regimens.Keywords: HIV infection, antiretroviral therapy, ART, darunavir–cobicistat, DRVc, virological failure, VF

Abstract

One- and two-photon luminescence excitation spectroscopy showed a series of distinct excitonic states in single-walled carbon nanotubes. The energy splitting between one- and two-photon-active exciton states of different wavefunction symmetry is the fingerprint of excitonic interactions in carbon nanotubes. We determine exciton binding energies of 0.3-0.4 eV for different nanotubes with diameters between 0.7 and 0.9 nm. Our results, which are supported by ab-initio calculations of the linear and non-linear optical spectra, prove that the elementary optical excitations of carbon nanotubes are strongly Coulomb-correlated, quasi-one dimensionally confined electron-hole pairs, stable even at room temperature. This alters our microscopic understanding of both the electronic structure and the Coulomb interactions in carbon nanotubes, and has direct impact on the optical and transport properties of novel nanotube devices., Comment: 5 pages, 4 figures

Abstract

We present results of wavelength-dependent ultrafast pump-probe experiments on micelle-suspended single-walled carbon nanotubes. The linear absorption and photoluminescence spectra of the samples show a number of chirality-dependent peaks, and consequently, the pump-probe results sensitively depend on the wavelength. In the wavelength range corresponding to the second van Hove singularities (VHSs), we observe sub-picosecond decays, as has been seen in previous pump-probe studies. We ascribe these ultrafast decays to intraband carrier relaxation. On the other hand, in the wavelength range corresponding to the first VHSs, we observe two distinct regimes in ultrafast carrier relaxation: fast (0.3-1.2 ps) and slow (5-20 ps). The slow component, which has not been observed previously, is resonantly enhanced whenever the pump photon energy resonates with an interband absorption peak, and we attribute it to radiative carrier recombination. Abstract

Wavelength-dependent pump-probe spectroscopy of micelle-suspended single-walled carbon nanotubes reveals two-component dynamics. The slow component (5-20 ps), which has not been observed previously, is resonantly enhanced whenever the pump photon energy coincides with an absorption peak and we attribute it to interband carrier recombination, whereas we interpret the always-present fast component (0.3-1.2 ps) as intraband carrier relaxation in non-resonantly excited nanotubes. The slow component decreases drastically with decreasing pH (or increasing H$^+$ doping), especially in large-diameter tubes. Abstract

In progressive die stamping processes, maintenance activities caused by tool damage, and wear represent economic losses for companies. An effective predictive maintenance strategy can only be implemented if maintenance data coming from the operations are correlated to specific process-related information. As a part of a more general data-based predictive maintenance strategy, the main causes of tool damage and wear in a progressive die stamping factory that produces automotive metal washers have been identified by means of FEA simulations. In this study, the progressive die stamping of a dented conical washer is simulated with Transvalor FORGE FEA software by implementing the process parameters used in a real case. In this study, two indicators called FEAwear and FEAdamage are proposed for prediction of die wear and damage for tools with high risk of failure. For validating the accuracy of the FEA simulations, dimension and geometry comparisons are performed between FEA and real washer, and then real and FEA maximum press force comparison is performed. In the end, FEA simulations demonstrated their accuracy in predicting the stamping force of the press and the final part quality, and proposed FEA damage and wear indicators accurately predicted the most critical tools and stations, as confirmed by the real maintenance data. Finally, the simulations also correctly detected potential damage zones of the tools.

Abstract

The implementation of an efficient and effective strategy, capable of relaunching the economy of a territory and increasing its attractiveness, is now more desirable than ever. Among all possible tools, a particular European discipline has certainly found confirmation in areas with a high presence of quality productions and areas with a high sustainable value supply: the Area Brand. The aim of this paper is to increase the awareness of the readers about these approaches, related to a territory with a great development potential and a high capacity of arousing great interest even among overseas consumers and stakeholders. In particular, we considered the Sicilian territory related to Protected Areas of Messina and Catania, located in the area of "GAL Terre dell'Etna e dell'Alcantara". In this work, the brand implementation process starts with the preliminary phase, specifically with the mapping and evaluation of the production processes that may be useful and functional to a possible targeted integration of this strategy.

Abstract

In the hilly area of Gioia Tauro (Calabria, Southern Italy), lemon orchards are grown in consociation with centuriesold olive trees. Lemons are partially shaded by olive canopies and the microclimate at the level of their canopies is suitable for plants growth and quality productions. Under these conditions, lemon trees are grown even without irrigation, providing, despite this limitation, a quality product. This study aimed to i) investigate the qualitative characterisation of two clonal selections of the lemon cultivar Femminello, F. Siracusano (S) and F. Zagara bianca (ZB), from the described intercropping, on irrigated (I) and nonirrigated (NI) crops; ii) assess the quality preservation during cold storage, in order to evaluate the availability of lemons for marketing in a period of shortage such as the summer season. Fruits were harvested at commercial maturity, and cold stored at 10±1°C and RH 8590%, for 60 days. Decay incidence, physiological disorders, weight loss, and the main physicalchemical parameters were assessed at harvest (T0) and every 15 days (T15, T30, T45, T60). The absence of decay and physiological disorders was observed throughout the 60day storage period, in both clonal selections under the two management conditions. The weight loss was greater in fruits from irrigated lemon groves of both S and ZB. S_I showed significantly lower fruits weight and higher titratable acidity than S_NI. Total soluble solids and titratable acidity were statistically lower for ZB_I than for NI fruits. [ABSTRACT FROM AUTHOR]

Abstract

Turbostratic carbon nanotubes (CNTs) and nanofibers (CNFs) are synthesized by chemical vapor deposition using titania nanoparticle catalysts, and a quantitative lift-off model is developed to explain CNT and CNF growth. Micron-scale long turbostratic CNTs and CNFs were observed when acetylene is utilized as a carbon feedstock, and an alumina substrate was incorporated to improve the homogeneity of catalyst distribution. Turbostratic CNTs/CNFs are always found attached to nanoparticle corners, in the absence of the graphitic cage that is typically observed with metal nanoparticle-mediated growth. The observed morphology in turbostratic CNTs/CNFs supports a model in which several layers of graphene lift off from high-curvature corners of the titania nanoparticle catalysts. This model explains a key feature, which differentiates the growth of turbostratic CNTs/CNFs via non-metallic nanoparticles from growth using standard metal nanoparticle catalysts. The observed CNT/CNF growth and the accompanying model can impact the assessment of other metal-oxide nanoparticle catalysts, with the findings here contributing to a metal-free synthesis of turbostratic CNTs/CNFs. Abstract

Background and aim of the work Since the introduction of the 7-valent vaccine, invasive pneumococcal disease have greatly decreased; however, changes in the distribution of pneumococcal serotypes have recently highlighted the need for vaccines with wider coverage. The aim of the work was to assess the potential serotype coverage of three pneumococcal conjugate vaccines (7-, 10- and 13-valent) against bacteremic pneumococcal pneumonia and meningitis/sepsis in Italian children. Patients and methods We determined pneumococcal serotypes in immunocompetent patients who had been admitted to hospital with suspicion of invasive bacterial disease and had confirmed bacteremic pneumococcal pneumonia or meningitis/sepsis determined by molecular detection of Streptococcus pneumoniae in a normally sterile site. Positive samples were serotyped using Realtime-PCR. Results Between April 2008 and March 2011, a total of 144 patients (age median 4.1 years; Interquartile range 1.8–5.6) with pneumococcal meningitis/sepsis (n = 43) or pneumonia (n = 101) from 83 participating centers located in 19 of 20 Italian regions were serotyped. The 10 most prevalent serotypes were 1 (29.9%), 3 (16.0%), 19A (13.2%), 7F (8.3%), 5 (4.2%), 14 (4.2%), 6A (3.5%), 6B (3.5%), 18C (3.5%), 19F (3.5%). Overall, serotype coverage for PCV-7, -10 and -13 were respectively 19.4%, 61.8% and 94.4% with no statistical difference between pneumonia and meningitis/sepsis. Potential coverage was similar for children 0–2 or 2–5 years of age. Cultures resulted positive in 35/99 (35.4%) samples simultaneously obtained for both culture and RT-PCR. Conclusion These findings indicate that increasing the potential serotype coverage by introducing PCV13 in the vaccination schedule for infancy could provide substantial added benefit for protection from pneumococcal pneumonia or meningitis/sepsis in Italy in children below 2 years as well in older children. The importance of molecular methods for diagnosis and serotyping of invasive pneumococcal disease was confirmed.

Abstract

The aims of this study were to describe the variability in protein binding of teicoplanin in critically ill patients as well as the number of patients achieving therapeutic target concentrations. This report is part of the multinational pharmacokinetic DALI Study. Patients were sampled on a single day, with blood samples taken both at the midpoint and the end of the dosing interval. Total and unbound teicoplanin concentrations were assayed using validated chromatographic methods. The lower therapeutic range of teicoplanin was defined as total trough concentrations from 10 to 20 mg/L and the higher range as 10-30 mg/L. Thirteen critically ill patients were available for analysis. The following are the median (interquartile range) total and free concentrations (mg/L): midpoint, total 13.6(11.2-26.0) and free 1.5 (0.7-2.5); trough, total 11.9 (10.2-22.7) and free 1.8 (0.6-2.6). The percentage free teicoplanin for the mid-dose and trough time points was 6.9% (4.5-15.6%) and 8.2% (5.5-16.4%), respectively. The correlation between total and free antibiotic concentrations was moderate for both the midpoint (rho=0.79, P = 0.0021) and trough (rho = 0.63, P = 0.027). Only 42% and 58% of patients were in the lower and higher therapeutic ranges, respectively. In conclusion, use of standard dosing for teicoplanin leads to inappropriate concentrations in a high proportion of critically ill patients. Variability in teicoplanin protein binding is very high, placing significant doubt on the validity of total concentrations for therapeutic drug monitoring in critically ill patients. Abstract

Neisseria meningitidis group B (MenB) is a leading cause of meningitis and sepsis. A new vaccine has been recently licensed. The aim of the present study was to evaluate the epidemiology of MenB disease in pediatric age and define the optimal age for vaccination. All patients aged 0–18 years admitted with a diagnosis of meningitis or sepsis to the 83 participating Italian pediatric hospitals were included in the study. Blood and/or cerebrospinal fluid (CSF) samples were tested by Realtime-PCR and/or culture. One hundred and thirty-six cases (mean age 5.0 years, median 2.7) of MenB disease were found. Among these, 96/136 (70.6%) were between 0 and 5 years, 61/136 (44.9%) were between 0 and 2 years. Among the latter, 39/61 (63.9%) occurred during the first year of life with highest incidence between 4 and 8 months. A case-fatality rate of 13.2% was found, with 27.8% cases below 12 months. Sepsis lethality was 24.4%. RT-PCR was significantly more sensitive than culture: 82 patients were tested at the same time by both methods, either in blood or in CSF; MenB was found by RT-PCR in blood or CSF in 81/82 cases (98.8%), culture identified 27/82 (32.9%) infections (Cohen's Kappa 0.3; McNemar's: p < 10−5). The study shows that the highest incidence of disease occurs in the first year of age, with a peak between 4 and 8 months of life; 30% of deaths occur before 12 months. The results suggest that the greatest prevention could be obtained starting MenB vaccination in the first months of life; a catch-up strategy up to the fifth year of life could be considered. Our results also confirm that Realtime PCR is significantly more sensitive than culture. In those countries where only isolate positive infections are counted as cases, the incidence of MenB infection results highly underestimated.

Abstract

Background. The aim of this study was to use real-time polymerase chain reaction (RT-PCR) on blood samples to diagnose and serotype pneumococcal infection in a large cohort of Italian children hospitalized for community-acquired pneumonia. Methods. We conducted an observational study from April 2007 through June 2009 of children aged 0-16 years with a diagnosis of community-acquired pneumonia admitted to 83 pediatric hospitals in Italy. Results. Seven hundred fifty-three children were studied. RT-PCR found pneumococcal infection in 80 (10.6%) of 753 patients. In 292 patients, culture and RT-PCR were simultaneously performed. Streptococcus pneumoniae was identified in 47 of 292 patients; 45 (15.4%) tested positive by RT-PCR and 11 (3.8%) tested positive by culture. RT-PCR was significantly more sensitive than culture in revealing bacteremic pneumonia (odds ratio, 30.6; 95% confidence interval, 5.8-97.5; P < .001). Complicated pneumonia was found in 162 (21.5%) of 753 children; 152 (93.8%) of these 162 had parapneumonic effusion, and 51 (33.6%) had empyema. Children with complicated pneumonia were significantly older. Abstract

Composti generalmente considerati come non tossici (generally recognised as safe, GRAS) e fungicidi di nuova generazione, classificati come “reduced-risk fungicides” sono stati testati in sospensioni acquose a 20°C o a 50°C su differenti specie di agrumi per prevenire la selezione di ceppi resistenti al tiabendazolo (TBZ) e all’imazalil (IMZ). Tra i composti GRAS il bicarbonato di sodio (0,5-3%) e fumigazioni di un’ora con l’acido acetico (AA) (2,5-5 mg/L) hanno dato i risultati migliori nella riduzione e nel rallentamento dello sviluppo dei penicilli. I trattamenti per immersione per 30-180s con i fungicidi (Trifloxystrobin, Azoxystrobin, Fludioxonil, Cyprodinil, Pyrimethanil), in sospensioni acquose a 20°C (200-600 mg/L) 0 a 50°C (50-200 mg/L) hanno mostrato un’efficacia simile a quella del TBZ e IMZ quando il trattamento è stato effettuato non oltre 24 ore dopo aver ferito o inoculato i frutti; mentre nei casi in cui i trattamenti sono stati effettuati con maggior ritardo la loro efficacia è risultata inferiore al TBZ e IMZ
