6. Young men’s gambling and violence perpetration in Mwanza, Tanzania

Background The prevalence of intimate partner violence (IPV) in Tanzania is one of the highest in the sub-Saharan African region. Studies have shown that traditionally “manly” behaviours, such as risk-taking, are at the root of IPV perpetration. Only few studies investigated the co-occurrence of gambling and IPV, and none from LMICs. Methods Cross-sectional survey data of 1002 men aged 18-24 from Mwanza, Tanzania were analysed. Physical, sexual, emotional and economic IPV perpetration were measured using acts-based questions. Gambling was assessed through a question on whether the man bet or spent money on gambling or gambling machines. Consequences of gambling behaviours were assessed through four further questions. We conducted multivariate logistic regressions to control for potential confounders. Results 21% of the men in the sample confirmed they had bet or spent money on gambling in the previous 12 months; the prevalence raises to 24% for men who had been in a relationship in the previous 12 months (N = 755). Of these, 23% had ever perpetrated physical IPV, 29% sexual IPV, 56% emotional IPV and 37% economic IPV in their lifetimes. Of those who gambled, 24% had ever perpetrated physical IPV, 46% ever committed sexual IPV, 66% emotional IPV and 45% economic IPV. Gambling was statistically significantly associated sexual IPV (aOR: 2.39; 95% CI: 1.66-3.45) and emotional IPV (aOR: 1.48; 95% CI: 1.03-2.14) even after controlling for age, alcohol use, depressive symptoms and suicidal ideation. Gambling was not associated with physical and economic IPV after adjusting for those confounders. Implications The analysis shows that young men's practice of gambling is an additional risk factor for IPV perpetration that needs to be addressed. More research is needed to understand how current prevention efforts can be expanded to include problem gambling treatment to curb the incidence of IPV and give couples conflict resolutions skills for issues that might arise from gambling. Key messages • Problem gambling has so far remained vastly under-researched in violence research. • Gambling as well as drinking were associated with increased odds of physical and sexual IPV perpetration.

8. Intimate Partner Violence (IPV) and Associated Factors in HPTN 071 (PopART) Study Communities in Zambia and South Africa-A Comparison by HIV Status

- Abstract
The HPTN 071(PopART) study was a community-randomised trial in Zambia and South Africa, examining the impact of combination-prevention including universal testing and treatment (UTT), on HIV-incidence. This sub-study evaluated factors associated with IPV (physical and/or sexual) to identify differences by HIV status. During 2015-16, a random subset of adults who participated in the first year of the PopART intervention were recruited and standardised questionnaires were administered. Logistic regression was performed to estimate odds ratios of factors associated with IPV. Among > 700 women studied (300 HIV-negative;400 HIV-positive), ~ 20% reported experiencing physical and/or sexual violence in the last 12-months. Sexual violence was similar by HIV status, but physical violence and reporting both physical/sexual violence was more common among HIV-positive women. Spending nights away from the community in the last 12-months was associated with higher odds of IPV among both HIV-negative (aOR 3.17, 95% CI 1.02-9.81) and HIV-positive women (aOR 1.79, 95% CI 0.99-3.24). Among HIV-positive women, financial autonomy was associated with reduced IPV (aOR:0.41,95%CI:0.23-0.75) while pregnancy in the last 12-months (aOR 2.25, 95% CI 1.07-4.74), risk of alcohol dependence (aOR 2.75, 95% CI 1.51-5.00) and risk of mental distress (aOR 2.62, 95% CI 1.33-5.16) were associated with increased IPV. Among HIV-negative women reporting sex in the last 12-months, transactional sex (aOR 3.97, 95% CI 1.02-15.37) and not knowing partner's HIV status (aOR 3.01, 95% CI 1.24-7.29) were associated with IPV. IPV was commonly reported in the study population and factors associated with IPV differed by HIV status. The association of mobility with IPV warrants further research. The high prevalence of harmful alcohol use and mental distress, and their association with IPV among HIV-positive women require urgent attention.

- Abstract
Background: Detailed assessments of mortality patterns, particularly age-specific mortality, represent a crucial input that enables health systems to target interventions to specific populations. Understanding how all-cause mortality has changed with respect to development status can identify exemplars for best practice. To accomplish this, the Global Burden of Diseases, Injuries, and Risk Factors Study 2016 (GBD 2016) estimated age-specific and sex-specific all-cause mortality between 1970 and 2016 for 195 countries and territories and at the subnational level for the five countries with a population greater than 200 million in 2016. Methods: We have evaluated how well civil registration systems captured deaths using a set of demographic methods called death distribution methods for adults and from consideration of survey and census data for children younger than 5 years. We generated an overall assessment of completeness of registration of deaths by dividing registered deaths in each location-year by our estimate of all-age deaths generated from our overall estimation process. For 163 locations, including subnational units in countries with a population greater than 200 million with complete vital registration (VR) systems, our estimates were largely driven by the observed data, with corrections for small fluctuations in numbers and estimation for recent years where there were lags in data reporting (lags were variable by location, generally between 1 year and 6 years). For other locations, we took advantage of different data sources available to measure under-5 mortality rates (U5MR) using complete birth histories, summary birth histories, and incomplete VR with adjustments; we measured adult mortality rate (the probability of death in individuals aged 15-60 years) using adjusted incomplete VR, sibling histories, and household death recall. Both a guide to international debate on the measurement of gender-based violence (GBV) for policy purposes and a handbook on how those measurements can be best achieved, this book draws on cutting-edge statistical research to propose new measurement methods designed for promoting gender equality in the contemporary world. As policy aimed at reducing violence or providing assistance to women rarely tackles the underlying issue of gender inequality, and vice versa, there exists a clear need for guidance. Covering homicide (and femicide specifically), rape and sexual assault, domestic and intimate partner violence, female genital mutilation, and human trafficking, and including practical tools such as definitions of key terms, indicators, and coordination mechanisms, this book provides a framework for measuring GBV that will shape service design, service delivery, and research practices. - Abstract
Over the past few decades, metaphor has come to be seen as a trope that governs thought, not just language. A consequence of accepting this view is that its manifestations should be examined in other media than language alone. Together with metaphor’s role in gesturing, its function in visuals has hitherto been the richest area of non-verbal research. This chapter provides an overview of issues that deserve attention in the investigation of pictorial (or: visual) metaphor and multimodal metaphor involving visuals. These issues include: monomodal versus multimodal metaphor; identifying non-verbal metaphor; creative versus structural metaphor; diegetic versus extradiegetic source domains; metaphor in static versus dynamic discourses; metaphor and other tropes.

In Foto-Communitys finden sich vielfältige und komplexe Text-Bild-Relationen, wobei hier zwei kommunikative Praktiken näher betrachtet werden: Das Vergeben von Tags und das Anbringen von Notizen. Tags dienen der Wissensrepräsentation; es stellt sich hier die Frage, wie Bilder möglichst adäquat versprachlicht werden können, es wird also semantische Redundanz angestrebt. Zudem wird in diesem Beitrag dargestellt, wie die Relationen zwischen den verschiedenen Tags geartet sind, die sich gemeinsam auf ein Bild beziehen. Bei Notizen dahingegen stehen pragmatische Relationen im Vordergrund, die aus semantischer Sicht als komplementär zu bezeichnen sind, d. h. die Informationen des Kommunikats sind auf Text und Bild verteilt. Auf der sprachlichen Seite geht es dabei um Ellipsen, die im Bild gefüllt sind, um verbale Deiktika, die auf das Bild verweisen sowie um Passivkonstruktionen, bei denen das Agens im Bild realisiert ist.

Intimate partner violence during pregnancy is receiving increased attention because of its high prevalence and health effects. Still, little is known about women's perceptions on how their pregnancy influences the context in which intimate partner violence occurs. We conducted 19 in-depth interviews with women who had experienced intimate partner violence around the time of pregnancy. Women clearly perceived pregnancy as a turning point, because it created new expectations and a feeling of being overwhelmed. This led to violence by reducing women's acceptance of their partner's unemployment, alcohol abuse and lack of relationship commitment, or by increasing women's vulnerability because they felt too young to raise a child alone. Pregnancy also led to violence by bringing back childhood memories or by taking attention away from their partners. Understanding how pregnancy influences the context in which intimate partner violence occurs is important to provide abused, pregnant women with the services they need.

Numerische Simulationen von bruchmechanischen Experimenten mit dem Ziel, aus Messungen der Rißlänge den Spannungsintensitätsfaktor und seine Beziehung zur Bruchgeschwindigkeit zu ermitteln, sind bei der herkömmlichen DCB-Probe mit Belastung durch Keil und Bolzen wegen nicht klar definierbarer Randbedingungen problematisch. Es wurden daher Experimente an modifizierten DCB-Proben aus ARALDIT B durchgeführt, bei denen der Belastungskeil direkt in den Rißmund gedrückt wurde. Bei geeigneten Probenproportionen überstreicht Ki bereits in der Anfangsphase der Rißausbreitung vor dem Eintreffen der ersten reflektierten Wellen einen großen Teil des relevanten Bereiches. In dieser Phase stimmen die numerischen Simulationen gut mit den schattenoptischen Messungen überein. Eine Probenvariante mit T-förmiger Erweiterung am Widerlager dürfte für Rißarrestuntersuchungen besonders geeignet sein, da sich damit hohe Bruchgeschwindigkeiten und kurze Rißsprunglängen miteinander verbinden lassen. Das konstan te Glied in der Reihenentwicklung der Spannungsverteilung an der Rißspitze nach Williams bestimmt die Richtungsstabilität des Risses. Die hierzu von Cotterell, Schindler sowie Streit und Finnie aufgestellten Theorien werden anhand der bei einigen Experimenten beobachteten abknickenden Risse diskutiert.

Eine Reihe quasistatischer und dynamischer Kerbschlagversuche an Charpy-Proben wurde mit Hilfe des FE-Programmes ADINA numerisch simuliert. Es wurden zwei- und dreidimensionale Modelle verwendet. Bei dreidimensionaler Modellierung konnte der gemessene Kraftverlauf befriedigend simuliert werden, waehrend sich bei Annahme des ebenen Dehnungszustandes zu hohe, bei Annahme des ebenen Spannungszustandes zu niedrige Kraefte ergaben. Sowohl aus der numerischen Simulation wie auch aus gemessenen Kraft-Durchbiegungskurven wurden J-Risswiderstandskurven fuer das gemittelte J bestimmt. Aus den 3-D-Rechnungen wurde der Verlauf von J entlang der Rissfront berechnet. (IWM)

Zur Untersuchung des Einflusses der Probendicke auf das Versagen von seitengekerbten Kompaktproben aus 20MnMoNi55 im Temperaturbereich des Übergangs vom spröden zum duktilen Versagen wurden dreidimensionale Analysen mit finiten Elementen durchgeführt. Last-Aufweitungskurven und J-Aufweitungskurven stimmen gut mit Meßergebnissen überein. Die lokalen J-Werte entlang der als gerade angenommenen Rißfront nehmen mit Annäherung an die Seitenkerbe im elastischen Bereich zu. Mit zunehmender plastischer Verformung erhält man zunächst eine annähernd konstante J-Verteilung, danach nimmt J besonders bei dünnen Proben in der Nähe der Seitenkerben ab. Die gleiche Tendenz zeigen die senkrecht auf dem Ligament stehende Spannungskomponente sigma z und der Mehrachsigkeitsfaktor h (h = hydrostatische Spannung/Vergleichsspannung). Bei dünnen Proben fallen sigma z und h stärker mit dem Abstand von der Rißfront ab als bei dicken. (IWM)

Schlagversuche an Dreipunktbiegeproben von 495 mal 90 mal 45 cbmm Größe aus einem Druckbehälterstahl wurden zur Überprüfung des experimentell ermittelten Zusammenhangs zwischen dem J-Integral und der Rißverlängerung Alpha a numerisch mit Hilfe der Methode der Finiten Elemente simuliert. Bei der Modellbildung wurden der Schlaghammer als Punktmasse, der Kontaktbereich als elastische Felder und die Probe als zweidimensionales FE-Modell unter Annahme des ebenen Spannungszustandes abgebildet. Die Rechnung wurde mit expliziter Zeitintegration durchgeführt, dynamische Effekte wurden berücksichtigt. Mit der statisch bestimmten elastisch-plastischen Spannungs-Dehnungs-Beziehung konnte der gemessene Zeitverlauf der Hammerlast in der Anfangsphase nicht reproduziert werden. Dies gelang erst bei Annahme eines viskoplastischen Materialmodells mit einem Fluidparameter gamma gleich 100 s hoch minus 1. Bei starker Gesamtverformung der Probe mußte zur Erklärung der gemessenen Hammerlast eine Reibung mit Reibungskoeffizienten bis 0,7 angenommen werden. Etwa 10 bis 15% der Hammerenergie werden an den Auflagern umgesetzt. Berücksichtigt man diesen Anteil, dann stimmen die experimentell bestimmte und die numerisch simulierte J(Delta a)-Kurve etwa überein. Ab etwa 5 mm Rißverlängerung hängt das berechnete J-Integral zunehmend vom Integrationsbereich ab. Dies wird darauf zurückgeführt, daß die Energiedichte wegen Entlastungsvorgängen explizit ortsabhängig wird.

Background: Commercial fishing at sea is among the world's most dangerous occupations. With 24- hour cycles of physically demanding work, adequate rest and recovery are often elusive in deep-sea commercial fishing. Human trafficking in the fishing industry has been identified as a problem in the Greater Mekong Subregion (GMS), with reports documenting 18-24 hour work days and severe violence. As a largely unregulated sector with activities conducted in international waters far from shore and outside of national jurisdiction, the commercial fishing industry poses unique risks to workers health, safety and well-being. Yet, research on the health needs of trafficked fishermen is sparse. Moreover, extraordinarily little research exists on policy responses to this problem. This thesis aims to investigate the health needs of trafficked fishermen, and understand how victim identification and assistance are being conducted in Thailand, a key transit and destination country for fishing trafficking. Methods: The thesis comprises four studies: i) a systematic review of the literature on occupational health among GMS migrant and trafficked fishermen and other seafarers; ii) a quantitative exploration of factors associated with work-related injuries and violence among labour-trafficked men that used post-trafficking services in the GMS; iii) a mixed methods study examining the health needs of trafficked fishermen and health service provision; and iv) a qualitative study examining how potentially trafficked fishermen are identified and assisted by frontline responders to trafficking in Thailand. Results: The systematic review found limited research on occupational hazards faced by migrant and trafficked fishermen and seafarers. Findings from quantitative analysis of a cohort of labour-trafficked males from different sectors indicated that work-related injuries were associated with severe violence and being trafficked for fishing. Ever having experienced violence was associated with being in the fishing sector and fluency in the language of the destination country. Having documents did not appear to be protective against injuries or violence. The mixed methods study found that dizzy spells and exhaustion were common among trafficked fishermen. There were strong associations between physical health symptoms and severe violence, injuries, or detention by immigration authorities. Trafficked fishermen are perceived by boat captains to be disposable when injured or sick. While health can be a means to reach trafficked men, health and welfare providers faced challenges including language barriers and negotiating payment for services for uninsured fishermen and accident compensation with employers. The qualitative study found that frontline responders perceived trafficking to take place outside of Thai waters and that migrant brokers caused employers to inadvertently traffick men. Confusion about whether debt bondage or withholding of documents counted as trafficking indicators may be linked to ambiguous inclusion of these indicators in policy documents. Institutional constraints included limited staff with increased remits and interpreter shortages. Conclusion: There is evidence that trafficked fishermen face more extreme occupational hazards and abuse compared to men trafficked into other sectors, with a high burden of physical and mental ill-health experienced. Promising strategies employed by health and welfare providers to reach men highlight the importance of targeted outreach. Beliefs about the locus of the problem and institutional constraints among frontline responders indicate that further training on trafficking indicators, as well as institutional change, is needed to improve victim identification and assistance.

Understanding how all-cause mortality has changed with respect to development status can identify exemplars for best practice. To accomplish this, the Global Burden of Diseases, Injuries, and Risk Factors Study 2016 (GBD 2016) estimated age-specific and sex-specific all-cause mortality between 1970 and 2016 for 195 countries and territories and at the subnational level for the five countries with a population greater than 200 million in 2016. Methods We have evaluated how well civil registration systems captured deaths using a set of demographic methods called death distribution methods for adults and from consideration of survey and census data for children younger than 5 years. We generated an overall assessment of completeness of registration of deaths by dividing registered deaths in each location-year by our estimate of all-age deaths generated from our overall estimation process. For 163 locations, including subnational units in countries with a population greater than 200 million with complete vital registration (VR) systems, our estimates were largely driven by the observed data, with corrections for small fluctuations in numbers and estimation for recent years where there were lags in data reporting (lags were variable by location, generally between 1 year and 6 years). For other locations, we took advantage of different data sources available to measure under-5 mortality rates (U5MR) using complete birth histories, summary birth histories, and incomplete VR with adjustments; we measured adult mortality rate (the probability of death in individuals aged 15-60 years) using adjusted incomplete VR, sibling histories, and household death recall. We used the U5MR and adult mortality rate, together with crude death rate due to HIV in the GBD model life table system, to estimate age-specific and sex-specific death rates for each location-year. Using various international databases, we identified fatal discontinuities, which we defined as increases in the death rate of more than one death per million, resulting from conflict and terrorism, natural disasters, major transport or technological accidents, and a subset of epidemic infectious diseases; these were added to estimates in the relevant years. In 47 countries with an identified peak adult prevalence for HIV/AIDS of more than 0.5% and where VR systems were less than 65% complete, we informed our estimates of age-sex-specific mortality using the Estimation and Projection Package (EPP)-Spectrum model fitted to national HIV/AIDS prevalence surveys and antenatal clinic serosurveillance systems. We estimated stillbirths, early neonatal, late neonatal, and childhood mortality using both survey and VR data in spatiotemporal Gaussian process regression models. We estimated abridged life tables for all location-years using age-specific death rates. We grouped locations into development quintiles based on the Sociodemographic Index (SDI) and analysed mortality trends by quintile. Using spline regression, we estimated the expected mortality rate for each age-sex group as a function of SDI. We identified countries with higher life expectancy than expected by comparing observed life expectancy to anticipated life expectancy on the basis of development status alone. Findings Completeness in the registration of deaths increased from 28% in 1970 to a peak of 45% in 2013; completeness was lower after 2013 because of lags in reporting. Total deaths in children younger than 5 years decreased from 1970 to 2016, and slower decreases occurred at ages 5-24 years. By contrast, numbers of adult deaths increased in each 5-year age bracket above the age of 25 years. The distribution of annualised rates of change in age-specific mortality rate differed over the period 2000 to 2016 compared with earlier decades: increasing annualised rates of change were less frequent, although rising annualised rates of change still occurred in some locations, particularly for adolescent and younger adult age groups. Rates of stillbirths and under-5 mortality both decreased globally from 1970. Evidence for global convergence of death rates was mixed; although the absolute difference between age-standardised death rates narrowed between countries at the lowest and highest levels of SDI, the ratio of these death rates-a measure of relative inequality-increased slightly. There was a strong shift between 1970 and 2016 toward higher life expectancy, most noticeably at higher levels of SDI. Among countries with populations greater than 1 million in 2016, life expectancy at birth was highest for women in Japan, at 86.9 years (95% UI 86.7-87.2), and for men in Singapore, at 81.3 years (78.8-83.7) in 2016. Male life expectancy was generally lower than female life expectancy between 1970 and 2016, and the gap between male and female life expectancy increased with progression to higher levels of SDI. Some countries with exceptional health performance in 1990 in terms of the difference in observed to expected life expectancy at birth had slower progress on the same measure in 2016. Interpretation Globally, mortality rates have decreased across all age groups over the past five decades, with the largest improvements occurring among children younger than 5 years. However, at the national level, considerable heterogeneity remains in terms of both level and rate of changes in age-specific mortality; increases in mortality for certain age groups occurred in some locations. We found evidence that the absolute gap between countries in age-specific death rates has declined, although the relative gap for some age-sex groups increased. Countries that now lead in terms of having higher observed life expectancy than that expected on the basis of development alone, or locations that have either increased this advantage or rapidly decreased the deficit from expected levels, could provide insight into the means to accelerate progress in nations where progress has stalled., Bill & Melinda Gates Foundation, and the National Institute on Aging and the National Institute of Mental Health of the National Institutes of Health.

47. Occupational risks, health needs and victim identification of trafficked fishermen in the Greater Mekong Subregion (GMS)

- Abstract
Background: Commercial fishing at sea is among the world’s most dangerous occupations. With 24- hour cycles of physically demanding work, adequate rest and recovery are often elusive in deep-sea commercial fishing. Human trafficking in the fishing industry has been identified as a problem in the Greater Mekong Subregion (GMS), with reports documenting 18-24 hour work days and severe violence. As a largely unregulated sector with activities conducted in international waters far from shore and outside of national jurisdiction, the commercial fishing industry poses unique risks to workers health, safety and well-being. Yet, research on the health needs of trafficked fishermen is sparse. Moreover, extraordinarily little research exists on policy responses to this problem. This thesis aims to investigate the health needs of trafficked fishermen, and understand how victim identification and assistance are being conducted in Thailand, a key transit and destination country for fishing trafficking. Methods: The thesis comprises four studies: i) a systematic review of the literature on occupational health among GMS migrant and trafficked fishermen and other seafarers; ii) a quantitative exploration of factors associated with work-related injuries and violence among labour-trafficked men that used post-trafficking services in the GMS; iii) a mixed methods study examining the health needs of trafficked fishermen and health service provision; and iv) a qualitative study examining how potentially trafficked fishermen are identified and assisted by frontline responders to trafficking in Thailand. Results: The systematic review found limited research on occupational hazards faced by migrant and trafficked fishermen and seafarers. Findings from quantitative analysis of a cohort of labour-trafficked males from different sectors indicated that work-related injuries were associated with severe violence and being trafficked for fishing. Ever having experienced violence was associated with being in the fishing sector and fluency in the language of the destination country. Having documents did not appear to be protective against injuries or violence. The mixed methods study found that dizzy spells and exhaustion were common among trafficked fishermen. There were strong associations between physical health symptoms and severe violence, injuries, or detention by immigration authorities. Trafficked fishermen are perceived by boat captains to be disposable when injured or sick. While health can be a means to reach trafficked men, health and welfare providers faced challenges including language barriers and negotiating payment for services for uninsured fishermen and accident compensation with employers. The qualitative study found that frontline responders perceived trafficking to take place outside of Thai waters and that migrant brokers caused employers to inadvertently traffick men. Confusion about whether debt bondage or withholding of documents counted as trafficking indicators may be linked to ambiguous inclusion of these indicators in policy documents. Institutional constraints included limited staff with increased remits and interpreter shortages. Conclusion: There is evidence that trafficked fishermen face more extreme occupational hazards and abuse compared to men trafficked into other sectors, with a high burden of physical and mental ill-health experienced. Promising strategies employed by health and welfare providers to reach men highlight the importance of targeted outreach. Beliefs about the locus of the problem and institutional constraints among frontline responders indicate that further training on trafficking indicators, as well as institutional change, is needed to improve victim identification and assistance.
48. Intimate Partner Violence Among Adolescent Mothers Living With and Without HIV: A Pre- and During-COVID-19 South African Cohort Analysis.

- Abstract
Purpose: Adolescent mothers face heightened economic and social vulnerabilities, which can place them at increased risk of intimate partner violence (IPV), prepandemic, and during COVID-19. However, few studies examine this population, and even less disaggregate findings by HIV status., Methods: We analyzed data from 834 South African adolescent mothers, 35% living with HIV (LHIV), who reported on physical, psychological, and sexual IPV exposure at two interviews: 2018-2019 (prepandemic) and 2021-2022 (during COVID-19). We estimated lifetime prevalence of IPV, disaggregating by HIV status. We used inverse weighted probability multivariate mixed-effects logistic regression to examine changes in IPV between the two periods and if changes in IPV differed by HIV status., Results: A quarter of adolescent mothers had experienced any IPV during COVID-19, quadruple prepandemic levels (24.7% vs. 6.0%). The increase was driven by surges in physical (+15.7%) and psychological (+11.2%) IPV. In both periods, psychosocial and physical IPV were the most prevalent forms and the most common combination among those who had experienced multiple forms of IPV. Exposure to any IPV was significantly more prevalent among those LHIV compared to those without HIV, prepandemic (9.5% vs. 4.1%, p = .026) and during COVID-19 (31.8% vs. 20.6%, p < .001). Adjusted models revealed an 18.2% significant increase in the average predicted probability of reporting IPV during COVID-19 compared to prepandemic, with no differential effect by HIV status., Discussion: Adolescent mothers experienced a significantly higher burden of IPV during COVID-19 than prepandemic, with those LHIV experiencing the highest level. - Abstract
Background: In Tanzania, nearly half of ever-married women have experienced some form of intimate partner violence (IPV), yet little knowledge of IPV from the male perspective exists., Objective: To explore the role of essential healthy lifestyle factors, diet, sleep, and exercise, and their potential role in IPV perpetration., Methods: A cross-sectional survey was conducted with 1,002 young men (ages 18-24), 754 of which were in an intimate relationship in the previous year. The study took place in Mwanza, Tanzania and used multivariable logistic regression models to explore associations between male perpetration of IPV and diet, sleep, and exercise., Results: Six types of IPV perpetration were investigated separately and the prevalence of controlling behaviours (79.4%), economic abuse (30.6%), emotional abuse (47.3%), physical violence (16.4%), sexual violence (23.3%), and combined physical and/or sexual violence (32.1%) were obtained. Regular exercise demonstrated a protective effect for economic abuse perpetration; the chance of mildly active individuals perpetrating economic abuse was 38% less than their inactive counterparts ( p = 0.003). Associations with sleep were varied and did not show a clear directional relationship. Diet, defined as poor food variety, was positively associated with every IPV type except physical violence and was significant in sexual violence perpetration (aOR:1.57, 95%CI:1.21-2.05)., Conclusions: The results from this study indicate that considering healthy lifestyle behaviours - diet, sleep, and exercise - in the design of intervention programmes may be beneficial in reducing IPV perpetration in Tanzania, and that they should be considered alongside previously established evidence-based risk factors.

- Abstract
Background: Addressing gender inequities could be key to the elimination of vertical transmission of HIV. Women experiencing intimate partner violence (IPV) might be at an increased risk of vertical transmission due to their vulnerability to HIV acquisition and barriers to access to and retention in care. Sub-Saharan Africa, where IPV burden is among the highest globally, accounts for most new paediatric HIV infections. We aimed to examine the proportion of excess vertical transmission attributable to IPV in this region., Methods: In this modelling analysis, we created a probability tree model of vertical HIV transmission among women aged 15-49 years in 46 African countries. We estimated the proportion of vertical transmission attributable to past-year physical or sexual IPV, or both, as an age-standardised population attributable fraction (PAF) and as excess vertical transmission risk per 1000 births among women experiencing IPV. We incorporated perinatal and postnatal vertical transmission among women who acquired HIV before pregnancy, during pregnancy, and during breastfeeding. Fertility, HIV prevalence, HIV incidence, antiretroviral therapy (ART) uptake, and ART retention varied in the model by women's IPV experience. The model was parameterised using UNAIDS' 2023 Spectrum model data, WHO's Global Database on Violence Against Women, and the peer-reviewed literature. Uncertainty intervals (95% UI) were calculated through 1000 Monte Carlo simulations., Findings: Across 46 countries 13% (95% UI 6-21) of paediatric HIV infections in 2022 were attributed to IPV, corresponding to over 22 000 paediatric infections. The PAF ranged from 4% (2-7) in Niger to 28% (13-43) in Uganda. The PAF was highest among girls aged 15-19 years (20%, 8-33) and lowest among women aged 45-49 years (6%, 3-9). In southern Africa, where women's HIV prevalence is highest (23%), IPV led to 11 (5-20) additional infections per 1000 births among women affected by IPV., Interpretation: IPV might be responsible for one in eight paediatric HIV infections in sub-Saharan Africa.
