The seminal concept of characteristic polygon of an embedded algebroid surface, first developed by Hironaka, seems well suited for combinatorially (perhaps even effectively) tracking of a resolution process. However, the way this object evolves through the resolution of singularities was not really well understood, as some references had pointed out. The aim of this paper is to explain, in a clear way, how this object changes as the surface gets resolved. In order to get a precise description of the phenomena involved, we need to use different techniques and ideas. We show that the roots of a Weierstrass polynomial on several variables are power series with rational exponents of bounded denominators, whose Newton diagrams are contained in an S-cone. As an application, in Section 4 we deal with some topics of integral dependence of Puiseux power series.

We show that a convex pyramid in R^n with apex at 0 can be brought to the first quadrant by a finite sequence of monomial blowing-ups if and only if its intersection with the opposite of the first quadrant is 0. The proof is non-trivially derived from the theorem of Farkas-Minkowski. Then, we apply this theorem to show how the Newton diagrams of the roots of any Weierstrass polynomial are contained in a pyramid of this type. Knee arthroscopy is the most common orthopedic procedure worldwide. While incidence of post-arthroscopy venous thromboembolic events (VTE) is low, treatment patterns and patient outcomes have not been described. Patients from the "Registro Informatizado Enfermedad TromboEmbolica" who had confirmed post-arthroscopy VTE were compared to patients with provoked, post bone-fracture, and to patients with unprovoked VTE. Baseline characteristics, presenting signs and symptoms, treatment and outcomes including recurrent VTE, bleeds or death were compared. Introduction Current guidelines recommend the use of anticoagulant therapy in patients with symptomatic splanchnic vein thrombosis (SVT) and suggest no routine anticoagulation in those with incidental SVT. Methods We used the RIETE (Registro Informatizado Enfermedad Trombo Embolica) registry to assess the rate and severity of symptomatic venous thromboembolism (VTE) recurrences and major bleeding events appearing during the course of anticoagulation in patients with symptomatic or incidental SVT. Results In March 2017, 521 patients with SVT were recruited. Of them, 212 (41%) presented with symptomatic SVT and 309 had incidental SVT. Most (93%) patients received anticoagulant therapy (median, 147 days). During the course of anticoagulation, 20 patients developed symptomatic VTE recurrences (none died) and 26 had major bleeding (fatal bleeding, 5). On multivariable analysis, patients with incidental SVT had a non-significantly higher risk for symptomatic VTE recurrences (adjusted hazard ratio [HR]: 2.04; 95%CI: 0.71–5.88) and a similar risk for major bleeding (HR: 1.12; 95%CI: 0.47–2.63) than those with symptomatic SVT. Active cancer was associated with at increased risk for VTE recurrences (HR: 3.06; 95%CI: 1.14–8.17) and anaemia (HR: 4.11; 95%CI: 1.45–11.6) or abnormal prothrombin time (HR: 4.10; 95%CI: 1.68–10.1) were associated with at increased risk for major bleeding. Conclusions The rates of recurrent SVT and major bleeding were similar between patients with incidental or symptomatic SVT. Because the severity of bleeding complications during anticoagulation may outweigh the severity of VTE recurrences in both groups, further studies should identify those SVT patients who benefit from anticoagulant therapy.

International audience; BACKGROUND:Even in the absence of evidence on its long-term efficacy and safety, a number of patients with venous thromboembolism (VTE) receive long-term therapy with fondaparinux alone in everyday practice.METHODS:We used the Registro Informatizado de Enfermedad Tromboembólica (RIETE) registry to compare the rate of VTE recurrences and major bleeding at 10 and 90 days in patients with and without cancer. For long-term therapy, fondaparinux was compared with vitamin K antagonists (VKA) in patients without cancer and with low-molecular-weight heparin (LMWH) in those with cancer.RESULTS:Of 47,378 patients recruited, 46,513 were initially treated with heparin, 865 with fondaparinux. Then, 263 patients (78 with cancer) were treated for at least 3 months with fondaparinux. After propensity-score matching, there were no differences between patients receiving initial therapy with heparin or fondaparinux. Among patients with cancer, there were no differences between fondaparinux and LMWH. 24. Increase in socioeconomic inequalities in mortality in a Southern European region: a small-area ecological study.

- Abstract
Background This study sought to describe the total mortality trend bysocioeconomic deprivation (SED) in the Madrid Autonomous Region, by sex and age group. Methods Cross-sectional ecological study by census tract, in two periods: 1994-2000 (P1) with SED of 1996 census and 2001-07 (P2) with SED of 2001 census. We calculated the relative risks (RRs) and their 95% credibility intervals (95% CIs) by SED quintile (Q), taking the quintile of least deprivation as reference. Besag-York-Mollié ecological regression models and the Integrated Nested Laplace Approximation procedure were applied. The absolute differences in age-standardized rates were compared by SED quintile. Results Inequalities decreased in young adults: among men aged 20-39 years, the RR in Q5 versus Q1 ranged from 2.73 (95% CI, 2.51-3.02) in P1 to 1.93 (95% CI, 1.76-2.15) in P2, due to the greater improvement in the most underprivileged groups. In contrast, there was an increase in SED-related mortality in the 40-79 age group. Among men aged 40-59 years, the RR in Q5 versus Q1 rose from 1.88 (95% CI, 1.76-2.02) in P1 to 2.29(95% CI, 2.17-2.43) in P2; the improvement was greater in the most privileged groups. Conclusion In a context of an economic boom, inequalities were observed to increase among adults by a greater improvement in the t privileged groups. [ABSTRACT FROM AUTHOR]

Potato virus Y (PVY) isolates originally coming from infected pepper plants, were biologically and genetically characterized, especially in comparison with PVY potato-isolates. Pepper PVY isolates could be differentiated from potato isolates in their host range, aphid transmission efficiencies, Mab serology, and genetic status. The genetic distances estimated for PVY pepper-isolates, based on their restrictotypes with five restriction enzymes and on their coat protein gene sequences, indicated that they form a single genetic strain with different pathotypic properties. This situation is essentially different to that of PVY potato-isolates. © 2001 Elsevier Science B.V. All rights reserved.

En 1990 se colocaron 7 trampas horizontales de baldosa verde del tipo utilizado por IRWIN (1980), situadas a la misma altura que el cultivo, en diferentes zonas productoras de pimiento en España: Aranjuez (Madrid), Balboa (Badajoz), Cadreita (Navarra), Mendavia (La Rioja), Torrepacheco (Murcia) y Montañana (Zaragoza). El muestreo abarcó de 18 a 19 semanas en cada localidad. El total de pulgones recolectados durante el período que duró el muestreo fue de 3.186 que corresponden a 29 especies distintas, de los que 1.019 individuos corresponden a la especie Aphis fabae Scopoli (31,98 % del total) y 500 a Aphis gossypii Glover (15,69 %). Otras especies capturadas en menor proporción han sido: Aphis craccivora Koch, Aphis nasturtii Kaltenbach, Diuraphis noxia (Mordvilko) y Brachycaudus spp., entre otras. Se realizaron ensayos de transmisión en laboratorio con el virus Y de la patata (PVY) utilizando las especies más importantes desde el punto de vista del número de capturas realizadas. Se emplearon dos aislados de este virus: uno de ellos obtenido en campo infectando pimiento y que pertenece al patotipo 0 (infecta a «Yolo Wonder» pero no a «Yolo Y») y otro obtenido de patata y perteneciente al grupo N de PVY (patata). Los resultados indican que ambas cepas son transmisibles por Myzus persicae (Sulzer) a pimiento «Yolo Wonder», aunque PVYN se transmite con mucha menor eficiencia. En ensayos de comparación entre distintas especies de vectores en cuanto a la capacidad de transmisión de PVY*, se observa que M. persicae es el más eficaz, seguido de A. gossypii que es el segundo en importancia. Acyrthosiphon pisum Harris fue también capaz de transmitir PVY0, pero con mucha menor eficacia. A la vista de los resultados obtenidos, y a pesar de su gran eficacia de transmisión en condiciones controladas, M. persicae parece tener escasa importancia en cuanto a su capacidad de transmisión de PVY en campo, ya que presenta una baja actividad de vuelo en cultivo de pimiento en todas las localidades muestreadas. Seven horizontal green tile traps similar to the ones described by IRWIN (1980), were maintained at a height equal to the top of the pepper canopy, and placed at several pepper growing areas of Spain: Aranjuez (Madrid, Balboa (Badajoz), Cadreita (Navarra), Mendavia (La Rioja), Torrepacheco (Murcia) and Montañana (Zaragoza). Sampling lasted from 18 to 19 weeks, in each locality. Over that time 3,186 aphids, belonging to 29 different species, were captured. Aphis fabae Scopoli was the principal species captured 1,019 individuals (31,98 % of the total), followed by Aphis gossypii Glover (500 individuals; 15,69 % of total). Other species captured in fewer numbers were: Aphis craccivora Koch, Aphis nasturtii Kaltenbach, Diuraphis noxia (Mordvilko) and Brachycaudus spp. Transmission studies were performed in the laboratory, using potato virus Y (PVY) infected pepper plants. The aphid species used in these studies were the main ones captured in the horizontal green tile traps. Two PVY-isolates were used: pepper-PVY strain (PVY"), that infects Yolo-Y but not Yolo-W, and potato-PVY strain (PVYN). Results showed that both strains are transmissible by Myzus persicae Sulzer, to pepper (var. «Yolo-Wonder»), though PVYN is transmitted with much less efficiency. Transmission tests between different species of aphids using PVY0 infected pepper plants, indicated that M. persicae is the most efficient one, followed by A. gossypii which is the second most important. Acyrthosiphon pisum (Harris) was also capable to transmit PVY0, but with much less efficiency. According to our results, in spite of its great efficiency to transmit PVY in laboratory conditions, M. persicae seems to have a limited importance as a PVY vector under field conditions, because its flight activity is very low in all the pepper-fields sampled.

- Abstract
The resistance mechanism to potato virus Y (PVY) in pepper plants homozygous for the recessive resistance allele ya (pr21) was investigated. The results of the fluorescence immunochemical analysis of protoplasts prepared from virus-inoculated plants or virus-inoculated protoplasts indicate that this resistance is due to impaired cellto-cell movement of the virus, rather than to an interference with virus replication.

Background Features of the area might contribute to differences in cardiovascular mortality. The census tract distribution of ischaemic heart disease (IHD) and cerebrovascular disease mortality in the Region of Madrid and its association with deprivation and environmental variables were examined in this study. Methods Cross-sectional, ecological study covering 3906 census tracts (median of around 1000 inhabitants), using mortality data (population aged <75 years) for 1996-2003, as well as socioeconomic deprivation and other environmental indicators (subjective perceptions of pollution, background noise, lack of green spaces and delinquency) drawn from the 2001 census. Standardised mortality ratios were calculated. Smoothed census tract relative risks were calculated using the Besag-York-Mollié model. Relative risks (RRs) of dying and their 95% credibility intervals (95% CI) were calculated according to the indicators considered (with the fourth quartile, Q, being the most unfavourable situation). Maps were plotted depicting the distribution of the posterior probability of RR>1. Results Census tracts with excess mortality were mostly located in the city of Madrid. Mortality increased with deprivation: RRs of IHD and stroke mortality in Q4 with respect to Q1 were 1.42 (95% CI 1.31 to 1.54) and 1.66 (95% CI 1.45 to 1.88) for men, and 1.54 (95% CI 1.33 to 1.79) and 1.52 (95% CI 1.29 to 1.76) for women respectively. Associations with deprivation decreased only slightly when perceived lack of green spaces and delinquency were included in the model. In men, subjective perceptions of areas remained associated with cardiovascular mortality after adjustment for deprivation. Conclusion Deprivation and subjective perceptions of physical environmental characteristics are ecologically associated with cardiovascular disease mortality. [ABSTRACT FROM AUTHOR]

The complexity of the therapeutic approach in systemic lupus erythematosus (SLE) is increased by the large number of patients who do not respond to the first-line therapies and by relapses after initial clinical remission. In these patients, second-line drugs are often prescribed according to individual clinical decisions. The emergence of biological therapies has increased the therapeutic armamentarium available in these complex situations, but their use is limited by the lack of licensing. Available data on the use of rituximab in SLE rely on a large number of case reports and some observational studies. We analyzed current evidence on the therapeutic use of rituximab in adult SLE patients by a systematic review of reports included in the PubMed database between 2002 and 2007. A total of 188 SLE patients treated with rituximab were identified; 171 (91%) patients showed a significant improvement in one or more of the systemic SLE manifestations. There were 103 patients with lupus nephritis, with an overall rate of therapeutic response of renal involvement of 91%. Adverse events were reported in 44 (23%) patients; themost frequent were infections (19%). Although it is not yet possible to make definite recommendations, the global analysis of all cases reported to date support the off-label use of rituximab in severe, refractory SLE cases, whereas its use as a first-line therapy or in patients with a predominantly mild form of the disease is not advised. The treatment of mandible bone cysts commonly leads to the development of bone defects. When the defects are less than 3 cm and located away from the alveolar ridge, they usually heal spontaneously within a 6 to 12 month period. However, if the defect is larger, the regenerative ability of the body is overcome. Colloss® appears to be an interesting osteoinductive biomaterial. It is a lyophilized complex from extracellular matrix proteins of bovine bone. It is composed primarily of type I collagen and variable amounts of BMPs. Between February 2002 and January 2004, 7 patients with mandible cysts were treated with lyophilized collagen to enhance bone regeneration. In some of the cases this was combined with autologous bone or a titanium mesh. We observed incompatibility with autologous bone and a slight tendency for intraoral exposure of the titanium mesh. Apart from this, we noted adequate bone healing and regeneration in all the cases. For the authors of this paper, the advantages of lyophilized collagen are the following: osteoinductive capacity, absence of any major adverse effects and the role played in the early detection of recurrence. Its possible disadvantages are its difficult handling and the inherent complications related to bovine xenografts. [ABSTRACT FROM AUTHOR]

Abstract: Berries and red fruits are dietary sources of polyphenols with reported health benefits. As part of the diet, polyphenols are ingested as complex mixtures immersed in a food matrix which is digested in the gut. Epithelial cells lining the gut are regularly exposed to these digested mixtures. To understand the effects of dietary polyphenols on human gut health it is essential to determine their stability and fate in the lumen. In this work, we investigated the effects of an in vitro gastric and pancreatic digestion on the stability and composition of the major polyphenols in chokeberry juice. Digestion was carried out with a mixture of pepsin-HCl for 2h, followed by a 2h incubation with pancreatin and bile salts at 37°C with shaking, in the absence of light and under N2. After digestion, the chokeberry samples were acidified, filtered and HPLC-DAD/HPLC–MS–MS analysed to determine the content of total soluble recovered phenolics. Gastric digestion had no substantial effect on any of the major phenolic compounds in chokeberry, namely anthocyanins, flavan-3-ols, flavonols and caffeic acid derivatives. However, these compounds were significantly altered during the pancreatic digestion and this effect was more marked for anthocyanins (approximately 43% was lost during the 2h treatment with pancreatin). Flavonols and flavan-3-ols decreased by 26% and 19%, respectively. Neochlorogenic acid decreased by 28% whereas chlorogenic acid was increased by 24%. In vitro digestion of standard phenolic compounds, representing each of the groups of phenolics in chokeberry, confirmed some of the observed changes. Interactions with the digestive enzymes were not responsible for the observed losses which were mostly due to the chemical conditions during pancreatic digestion. Our results, in accordance with previously published results, show that dietary polyphenols are highly sensitive to the mild alkaline conditions in the small intestine and that a good proportion of these compounds can be transformed into other unknown and/or undetected structural forms with different chemical properties and, consequently, different bioaccessibility, bioavailability and biological activity.
