Abstract

The need for skilled medical support is growing in the era of digital healthcare. This research presents an innovative strategy, utilizing the RuBERT model, for categorizing user inquiries in the field of medical consultation with a focus on expert specialization. By harnessing the capabilities of transformers, we fine-tuned the pre-trained RuBERT model on a varied dataset, which facilitates precise correspondence between queries and particular medical specialisms. Using a comprehensive dataset, we have demonstrated our approach's superior performance with an F1-score of over 92%, calculated through both cross-validation and the traditional split of test and train datasets. Our approach has shown excellent generalization across medical domains such as cardiology, neurology and dermatology. This methodology provides practical benefits by directing users to appropriate specialists for prompt and targeted medical advice. It also enhances healthcare system efficiency, reduces practitioner burden, and improves patient care quality. 8. Syndrome of perimenopausal melatonin deficiency is a new marker of women reproductive system aging

Abstract

The aim of our study was to determine a complex of risk factors of the perimenopausal melatonin deficiency syndrome (SPDM) in postmenopausal women and women during menopausal transition suffering from climacteric syndrome. The study involved 278 women. They were divided into two groups: I group included 246 menopausal women and II group included 32 women during the menopausal transition. The Medical History was collected, the menopausal index, and anthropometric measures were determined. The study was conducted with a help of the test questionnaire for melatonin deficiency (TQMD), the women’s health questionnaire (WHQ), and detection of the level of 6- sulfatoxymelatonin (6-SOMT) in urine. According to the obtained data, risk factors of SPDM in women during the menopausal transition are the chronological age increment, overweight, and the reduction of the estradiol level. The leading risk factors of SPDM in postmenopausal women are the chronological age increment, overweight with visceral type of obesity and changes in the hormonal profile. Thus, SPDM is a new clinical and laboratory marker of aging of the female reproductive system, an important screening tool for detecting the development of CC in women during the menopausal transition and menopausal women.

Abstract

This international guideline proposes improving clozapine package inserts worldwide by using ancestry-based dosing and titration. Adverse drug reaction (ADR) databases suggest that clozapine is the third most toxic drug in the United States (US), and it produces four times higher worldwide pneumonia mortality than that by agranulocytosis or myocarditis. For trough steady-state clozapine serum concentrations, the therapeutic reference range is narrow, from 350 to 600 ng/mL with the potential for toxicity and ADRs as concentrations increase. Clozapine is mainly metabolized by CYP1A2 (female non-smokers, the lowest dose; male smokers, the highest dose). Poor metabolizer status through phenotypic conversion is associated with co-prescription of inhibitors (including oral contraceptives and valproate), obesity, or inflammation with C-reactive protein (CRP) elevations. The Asian population (Pakistan to Japan) or the Americas' original inhabitants have lower CYP1A2 activity and require lower clozapine doses to reach concentrations of 350 ng/mL. In the US, daily doses of 300-600 mg/day are recommended. Slow personalized titration may prevent early ADRs (including syncope, myocarditis, and pneumonia). This guideline defines six personalized titration schedules for inpatients: 1) ancestry from Asia or the original people from the Americas with lower metabolism (obesity or valproate) needing minimum therapeutic dosages of 75-150 mg/day, 2) ancestry from Asia or the original people from the Americas with average metabolism needing 175-300 mg/day, 3) European/Western Asian ancestry with lower metabolism (obesity or valproate) needing 100-200 mg/day, 4) European/Western Asian ancestry with average metabolism needing 250-400 mg/day, 5) in the US with ancestries other than from Asia or the original people from the Americas with lower clozapine metabolism (obesity or valproate) needing 150-300 mg/day, and 6) in the US with ancestries other than from Asia or the original people fr

Background Alcohol drinking is an increasingly recognized risk factor for cardiovascular disease. However, there are few studies of the impact of harmful and hazardous drinking on biomarkers of myocardial health. We conducted a study in Russia to investigate the impact of heavy drinking on biomarkers of cardiac damage and inflammation. Methods and Results The Know Your Heart study recruited a random sample of 2479 participants from the population of northwest Russia (general population) plus 278 patients (narcology clinic subsample) with alcohol problems. The general population sample was categorized into harmful drinkers, hazardous drinkers, nonproblem drinkers, and nondrinkers, according to self-reported level of alcohol consumption, whereas the narcology clinic sample was treated as the separate group in the analysis. Measurements were made of the following: (1) high-sensitivity cardiac troponin T, (2) NT-proBNP (N-terminal pro-B-type natriuretic peptide), and (3) hsCRP (high-sensitivity C-reactive protein). The narcology clinic subsample had the most extreme drinking pattern and the highest levels of all 3 biomarkers relative to nonproblem drinkers in the general population: high-sensitivity cardiac troponin T was elevated by 10.3% (95% CI, 3.7%-17.4%), NT-proBNP by 46.7% (95% CI, 26.8%-69.8%), and hsCRP by 69.2% (95% CI, 43%-100%). In the general population sample, NT-proBNP was 31.5% (95% CI, 3.4%-67.2%) higher among harmful drinkers compared with nonproblem drinkers. Abstract

The paper aims to explore the legal nature and essence of the constitutional foundations of the judicial branch of power in the Russian Federation and the world with consideration of the historical experience and modern approaches to the structure and system of operation of constitutional justice found in the law and legislation. The primary research method is the deductive method, which provides for the study of the legal nature of the constitutional court and its place in the system of separation of powers in Russia. The study also employs the inductive method, systemic scientific analysis, and comparative legal and historical methods. The article proves the absence of a theoretical solution to the problem of legal nature and the primary purpose of constitutional control, which, the authors argue, lies in the fact that the principal condition for the establishment of strong judicial power is its high independence. The concept of "judicial law" is in a certain way related to the development of statemonopolistic capitalism, strengthening the position of the state and its apparatus. The idea of "judicial law" is proposed by some authors as a kind of counterweight to state power and bureaucracy. Resorting to the principle of "separation of powers", these scholars largely overestimate the relative independence of justice from the legislative and executive spheres of functional activity within a single mechanism of state power. The limits of judicial discretion tend to be more narrow, and the scope of judicial discretion is considerably restricted. The application of a legal rule at the discretion of the administration does not always entail the obligation to give reasons. Abstract

A coherent combining and long coherent integration (CCLCI) scheme is presented for standalone direct acquisition of binary offset carrier (BOC) signals under strong radio frequency interference (RFI). To mitigate the ambiguity of BOC signals, a split-spectrum method extracts the upper and lower sidebands of a BOC signal, treats them separately as two binary phase shift keying (BPSK) signals, and finally combines the results to recover the loss due to splitting. The CCLCI scheme burns through strong interference by building up the desired weak signal while averaging out noise and interference. It exploits all information available (L1 and L2, upper and lower sidebands, odd and even chips, and I- and Q-components) by applying coherent combining across signal components and long coherent integration over time, followed by noncoherent accumulation if necessary. Abstract

The chapter deals with the theoretical, methodological, and practical aspects of combatants’ mental health as participants in hostilities, in the context of psychosocial environment characteristics and the transformation of personal characteristics in the process of stress service. The emerging situational and dynamic nosological and subclinical changes are described, which do not lead to social disintegration during the service. The study of the power structure of employees’ catamnesis for 10 years of service was conducted with an assessment of social adaptation to peaceful life from clinical and psychological positions. The efficiency of complex therapy for persons with post-traumatic stress disorder and adaptation disorders is estimated, and the features of the organization of stage-by-stage rehabilitation with a team poly-professional approach are described. The methods allowing to predict the formation of borderline mental disorders (BMD) in this contingent are offered. The directions of medical and psychological support of combatants with the creation of a fundamentally new diagnostic, psychoprophylactic structure—the Center of Mental Health—to improve the quality of psychological and psychiatric care and monitoring of participants of the fighting mental state are systematized.
