1. Mental Wellbeing among Children with Cancer during COVID-19 Pandemic in Indonesia: A Cross-sectional Study.

Abstract

Introduction Coronavirus disease 2019 (COVID-19) has affected both physical and mental aspect of people worldwide, especially the high-risk group such as pediatric cancer patients. Children with cancer were considered both clinically and mentally vulnerable during this pandemic. They were also affected by the self-isolation, quarantine, and social distancing policy taken as a respond to public threat. Objectives To evaluate the impact of social distancing and health protocol during COVID-19 on the mental health profile of children with cancer in Indonesia. Materials and Methods A cross-sectional study evaluating the mental health of children with cancer during COVID-19 pandemic was conducted in Cipto Mangunkusumo Hospital, Jakarta, Indonesia from June to September 2020. An online questionnaire was used to collect demographics of parents and children, children's Strength and Difficulties Questionnaire (SDQ) score, and parents' Self-Reporting Questionnaire (SRQ). SDQ score consists of five subscales, including the emotional symptoms, conduct problems, hyperactivity, peer relationships problems and prosocial behavior. Cancer types were grouped into retinoblastoma, nonretinoblastoma (other solid tumors), and leukemia. Results There were 156 valid responses, consisting of 42 patients with retinoblastoma, 34 patients with nonretinoblastoma (other solid tumors), and 80 patients with leukemia. Pandemic-related lifestyle changes did not significantly impact emotional or behavioral problems. Children with normal total SDQ (odds ratio [OR]: 473, p = 0.001) and emotional scores (OR: 3.19, p = 0.07) had parents with normal SRQ scores (<6). Leukemia patients with shorter diagnosis period had worse hyperactivity score (p = 0.01). On the contrary, leukemia inpatients had better prosocial scores than outpatients (p = 0.03). More bilateral retinoblastoma patients (p = 0.04) with longer duration of cancer diagnosis (p = 0.03) faced peer problems. Conclusions Our study revealed that lifestyle changes during early COVID-19 pandemic were not major factors impacting emotional and behavioral problems in children with cancer. Abstract

Background: Many causes of vision impairment can be prevented or treated. With an ageing global population, the demands for eye health services are increasing. We estimated the prevalence and relative contribution of avoidable causes of blindness and vision impairment globally from 1990 to 2020. We aimed to compare the results with the World Health Assembly Global Action Plan (WHA GAP) target of a 25% global reduction from 2010 to 2019 in avoidable vision impairment, defined as cataract and undercorrected refractive error. 8. Screening for retinopathy of prematurity at Cipto Mangunkusumo Hospital, Jakarta, Indonesia -- a preliminary report.

Abstract

Background: To report the incidence of retinopathy of prematurity (ROP) at Cipto Mangunkusumo Hospital, Indonesia, and its relation to several risk factors. The results of treatment are also reported. Materials and Methods: A hospital-based prospective-cohort study in 33 infants in the neonatology ward of Cipto Mangunkusumo Hospital, Jakarta, Indonesia, who were referred for ROP screening from December 2003 to December 2004 was performed. Risk factors such as gestational age (GA) at birth ≤ 32 weeks, birth weight (BW) ≤ 1500 g, duration of oxygen (O2) therapy more than 7 days, gender, birth status, the presence of clinical sepsis, respiratory distress syndrome (RDS), apnea, asphyxia, and maternal pre-eclampsia / eclampsia were analyzed using the chi-square test and multivariate analysis. Results: Of the total of 33 cases, ROP at any stage of disease was found in 30.3%. Infants with ROP were significantly smaller (mean ± SD of 1397 ± 153.5 g vs. 1610 ± 218.8 g, p = 0.009), but not significantly younger (mean ± SD of 31.7 ± 2 weeks vs. 33.3 ± 2 weeks, p = 0.058) than infants without ROP. Logistic regression showed that low birth weight was not a significant risk-factor for ROP. Asphyxia was found to be the only significant risk factor for the occurrence of ROP (p = 0.021, OR = 13.525). Laser photocoagulation or cryotherapy was performed in 2 out of 3 threshold ROP cases, but the outcomes were still unsatisfactory. Conclusions: Our results are similar to those reported from previous studies in other developing countries, although in this study young gestational age and low birth weight were not associated with the risk of ROP. Further studies in a larger sample are required to confirm these findings and to establish effective screening guidelines. [ABSTRACT FROM AUTHOR]

Abstract

Purpose: To estimate the prevalence of retinopathy of prematurity (ROP), an avoidable cause of childhood blindness, in Schools for the Blind students representative of rural areas of communities of Java Island, Indonesia. Methods: Four-hundred and seventy-nine of a total of 500 students from five Schools for the Blind in Java Island, Indonesia, were examined using the standard WHO / PBL eye examination record for blindness and low vision protocol. Data were analyzed for those aged less than 16 years or the onset of visual loss less than 16 years. Data on social demography and medical history factors were collected. ROP was defined when cicatrical retinal detachment or retinal tractional dates from infancy, or a history of prematurity or low birth weight and ocular findings consistent with this diagnosis were identified. Results: Most of the students (95%) were blind (BL); 4.6% had severe visual impairment (SVI) and 0.4% visual impairment (VI). ROP was identified in only 5 of 479 (1.1%) cases. This low prevalence is in accordance with other School for the Blind studies in India (0.5%) and China (1.9%), but is in contrast to those in industrialized countries such as UK (18%) and USA (8-19%). Conclusions: The low incidence of ROP found in our study is most likely the result of the high mortality rate of premature children in the rural areas of Indonesia like in other developing countries. This finding highlights the need for a better interaction between primary and secondary health care and the specialist tertiary centers (and their intensive neonatal care services), to provide high coverage referral access for high risk mothers and / or premature or low-birth weight infants. This may help to, first, increase the survival rate of this group of babies, and then, as a next step, to install adequate screening programs by pediatric ophthalmologists and retina specialists to diagnose and treat ROP in due time. Abstract

Background: To contribute to the WHO initiative, VISION 2020: The Right to Sight, an assessment of global vision impairment in 2020 and temporal change is needed. We aimed to extensively update estimates of global vision loss burden, presenting estimates for 2020, temporal change over three decades between 1990–2020, and forecasts for 2050. Methods: We did a systematic review and meta-analysis of population-based surveys of eye disease from January, 1980, to October, 2018. Abstract

Background: Risk factors other than supplemental oxygen might play a role in the development of retinopathy of prematurity (ROP). In Indonesia ROP occurs in infants up to 34 weeks and 2000 g. Risk factors for the development of ROP in Indonesian NICUs have not been evaluated. Our aim was to identify other risk factors than the use of oxygen in the development and progression of ROP in preterm infants in Indonesia., Methodology: Data on 98 preterm infants with ROP and 77 controls were collected from four NICUs and two eye centers in Jakarta, Indonesia, between 2009 and 2014. We used multivariate logistic regression analysis to determine the relationship between infants and environmental variables and the development and progression of ROP. We obtained variables for ROP severity by using Cox regression analysis., Results: Factors associated with the development of ROP were birthweight (BWt), intrauterine growth retardation (IUGR), exchange transfusion, duration of oxygen supplementation, minimum saturation monitor setting, and socioeconomic factors. Regarding the progression, gestational age (GA), out-born, duration of supplemental oxygen, minimum saturation monitor setting, and socioeconomic factors were identified as risk factors., Conclusion: The use and control of supplemental oxygen are the main risk factors for the development and progression of ROP in preterms in Indonesia. Additionally, we confirm that GA, BWt, and IUGR are risk factors. Moreover, we found exchange transfusion to be a risk factor, and we found a lower rate of ROP in infants from a lower socioeconomic background. These risk factors apply to infants with a GA up to 34 weeks and a BWt up to 2000 g.
