Abstract Background In standard weaning from mechanical ventilation, a successful spontaneous breathing test (SBT) consisting of 30 min 8 cmH2O pressure-support ventilation (PSV8) without positive end-expiratory pressure (PEEP) is followed by extubation with continuous suctioning; however, these practices might promote derecruitment. Evidence supports the feasibility and safety of extubation without suctioning. Ultrasound can assess lung aeration and respiratory muscles. We hypothesize that weaning aiming to preserve lung volume can yield higher rates of successful extubation. Methods This multicenter superiority trial will randomly assign eligible patients to receive either standard weaning [SBT: 30-min PSV8 without PEEP followed by extubation with continuous suctioning] or lung-volume-preservation weaning [SBT: 30-min PSV8 + 5 cmH2O PEEP followed by extubation with positive pressure without suctioning]. We will compare the rates of successful extubation and reintubation, ICU and hospital stays, and ultrasound measurements of the volume of aerated lung (modified lung ultrasound score), diaphragm and intercostal muscle thickness, and thickening fraction before and after successful or failed SBT. Patients will be followed for 90 days after randomization. Discussion We aim to recruit a large sample of representative patients (N = 1600). Our study cannot elucidate the specific effects of PEEP during SBT and of positive pressure during extubation; the results will show the joint effects derived from the synergy of these two factors. Although universal ultrasound monitoring of lungs, diaphragm, and intercostal muscles throughout weaning is unfeasible, if derecruitment is a major cause of weaning failure, ultrasound may help clinicians decide about extubation in high-risk and borderline patients. Abstract Background This study aims to assess the sustained immunological response to the SARS-CoV-2 vaccine in patients with autoimmune inflammatory rheumatic diseases (AIRD) undergoing different treatment regimens. Methods We conducted a prospective observational study involving 157 AIRD patients without prior COVID-19 infection. Treatment regimens included non-treatment or glucocorticoid-only (not-treated/GCs), non-biological drugs, biological therapy, and JAK inhibitors. All participants completed the two-dose vaccine schedule, and 110 of them received an additional booster dose. Serum samples were collected approximately 3–6 months after the second and third vaccine doses to measure antibodies against the Spike protein (antiS-AB) and neutralizing antibodies (nAB) targeting six SARS-CoV-2 variants. Abstract Background Despite existing extensive literature, a comprehensive and clinically relevant classification system for osteoarthritis (OA) has yet to be established. In this study, we aimed to further characterize four knee OA (KOA) inflammatory phenotypes (KOIP) recently proposed by our group, by identifying the inflammatory factors associated with KOA severity and progression in a phenotype-specific manner. Methods We performed an analysis within each of the previously defined four KOIP groups, to assess the association between KOA severity and progression and a panel of 13 cytokines evaluated in the plasma and synovial fluid of our cohort’s patients. The cohort included 168 symptomatic female KOA patients with persistent joint effusion. Results Overall, our analyses showed that associations with KOA outcomes were of higher magnitude within the KOIP groups than for the overall patient series (all p-values < 1.30e−16) and that several of the cytokines showed a KOIP-specific behaviour regarding their associations with KOA outcomes. Conclusion Our study adds further evidence supporting KOA as a multifaceted syndrome composed of multiple phenotypes with differing pathophysiological pathways, providing an explanation for inconsistencies between previous studies focussed on the role of cytokines in OA and the lack of translational results to date. Our findings also highlight the potential clinical benefits of accurately phenotyping KOA patients, including improved patient stratification, tailored therapies, and the discovery of novel treatments.

In recent years, the field of Natural Language Generation (NLG) has been boosted by the recent advances in deep learning technologies. Nonetheless, these new data-intensive methods introduce language-dependent disparities in NLG as the main training data sets are in English. Also, most neural NLG systems use decoder-only (causal) transformer language models, which work well for English, but were not designed with other languages in mind. In this work we depart from the hypothesis that they may introduce generation bias in target languages with less rigid word ordering, subject omission, or different attachment preferences for relative clauses, so that for these target languages other language generation strategies may be more desirable. This paper first compares causal and non-causal language modeling for English and Spanish, two languages with different grammatical structures and over 1.5 billion and 0.5 billion speakers, respectively. For this purpose, we define a novel metric of average causal and non-causal context-conditioned entropy of the grammatical category distribution for both languages as an information-theoretic a priori approach. The evaluation of natural text sources (such as training data) in both languages reveals lower average non-causal conditional entropy in Spanish and lower causal conditional entropy in English. According to this experiment, Spanish is more predictable than English given a non-causal context. Then, by applying a conditional relative entropy metric to text generation experiments, we obtain as insights that the best performance is respectively achieved with causal NLG in English, and with non-causal NLG in Spanish. These insights support further research in NLG in Spanish using bidirectional transformer language models.

The EphA2 receptor tyrosine kinase is overexpressed in most solid tumors and acts as the major driver of tumorigenesis. In this study, we developed a novel approach for targeting the EphA2 receptor using a 2′-fluoro-modified pyrimidine RNA aptamer termed ATOP. We identified the ATOP EphA2 aptamer using a novel bioinformatics strategy that compared aptamers enriched during a protein SELEX using recombinant human EphA2 and a cell-internalization SELEX using EphA2-expressing MDA231 tumor cells. When applied to EphA2-expressing tumor cell lines, the ATOP EphA2 aptamer attenuated tumor cell migration and clonogenicity. In a mouse model of spontaneous metastasis, the ATOP EphA2 aptamer slowed primary tumor growth and significantly reduced the number of lung metastases. The EphA2 ATOP aptamer represents a promising candidate for the development of next-generation targeted therapies that provide safer and more effective treatment of EphA2-overexpressing tumors.

ABSTRACTCitizens lack knowledge about the impact of gut microbiota on health and how lifestyle and dietary choices can influence it, leading to Non-Communicable Diseases (NCDs) and affecting overall well-being. Participatory action research (PAR) is a promising approach to enhance communication and encourage individuals to adopt healthier behaviors and improve their health. In this study, we explored the feasibility of integrating the photovoice method with citizen science approaches to assess the impact of social and environmental factors on gut microbiota health. In this context, citizen science approaches entailed the involvement of participants in the collection of samples for subsequent analysis, specifically gut microbiome assessment via 16S rRNA gene sequencing. We recruited 70 volunteers and organized six photovoice groups based on age and educational background. Participants selected 64 photographs that represented the influence of daily habits on gut microbiota health and created four photovoice themes. Analysis of the gut microbiome using 16S rRNA gene sequencing identified 474 taxa, and in-depth microbial analysis revealed three clusters of people based on gut microbiome diversity and body mass index (BMI). Our findings indicate that participants enhanced their knowledge of gut microbiome health through PAR activities, and we found a correlation between lower microbial diversity, higher BMI, and better achievement of learning outcomes. Using PAR as a methodology is an effective way to increase citizens’ awareness and engagement in self-care, maintain healthy gut microbiota, and prevent NCD development. These interventions are particularly beneficial for individuals at higher risk of developing NCDs.

Wiki articles are created and maintained by a crowd of editors, producing a continuous stream of reviews. Reviews can take the form of additions, reverts, or both. This crowdsourcing model is exposed to manipulation since neither reviews nor editors are automatically screened and purged. To protect articles against vandalism or damage, the stream of reviews can be mined to classify reviews and profile editors in real-time. The goal of this work is to anticipate and explain which reviews to revert. This way, editors are informed why their edits will be reverted. The proposed method employs stream-based processing, updating the profiling and classification models on each incoming event. The profiling uses side and content-based features employing Natural Language Processing, and editor profiles are incrementally updated based on their reviews. Since the proposed method relies on self-explainable classification algorithms, it is possible to understand why a review has been classified as a revert or a non-revert. In addition, this work contributes an algorithm for generating synthetic data for class balancing, making the final classification fairer. The proposed online method was tested with a real data set from Wikivoyage, which was balanced through the aforementioned synthetic data generation. The results attained near-90% values for all evaluation metrics (accuracy, precision, recall, and ${F}$ -measure).

Abstract Limited data exists on SARS-CoV-2 sustained-response to vaccine in patients with rheumatic diseases. This study aims to evaluate neutralizing antibodies (nAB) induced by SARS-CoV-2 vaccine after 3 to 6 months from administration in Systemic Lupus Erythematosus (SLE) patients, as a surrogate of sustained-immunological response. This cross-sectional study compared nAB titre of 39 SLE patients and 37 Healthy individuals with no previous SARS-CoV-2 infection, who had all received a complete regimen of a mRNA SARS-CoV-2 vaccine within the last 3 to 6 months. We included four lines of SLE treatment including Not-treated, Hydroxychloroquine, immunosuppressive drugs and biological therapy. Glucocorticoids were allowed in all groups. Healthy and Not-treated individuals showed the highest levels of nAB. Treated patients presented lower nAB titres compared to Healthy: a 73% decrease for First-Line patients, 56% for Second-Line treatment and 72% for Third-Line. A multivariate analysis pointed to Glucocorticoids as the most associated factor with declining nAB levels (75% decrease) in treated SLE. Furthermore, a significant reduction in nAB titres was observed for Rituximab-users compared to Healthy subjects (89% decrease). Medium-term response of SLE patients to SARS-CoV-2 mRNA vaccines is negatively impacted in Glucocorticoids and Rituximab users. These findings might help to inform recommendations in vaccination protocols for SLE patients.

Abstract COVID-19 pathophysiology is currently not fully understood, reliable prognostic factors remain elusive, and few specific therapeutic strategies have been proposed. In this scenario, availability of biomarkers is a priority. MS-based Proteomics techniques were used to profile the proteome of 81 plasma samples extracted in four consecutive days from 23 hospitalized COVID-19 associated pneumonia patients. Samples from 10 subjects that reached a critical condition during their hospital stay and 10 matched non-severe controls were drawn before the administration of any COVID-19 specific treatment and used to identify potential biomarkers of COVID-19 prognosis. Additionally, we compared the proteome of five patients before and after glucocorticoids and tocilizumab treatment, to assess the changes induced by the therapy on our selected candidates. Forty-two proteins were differentially expressed between patients' evolution groups at 10% FDR. Twelve proteins showed lower levels in critical patients (fold-changes 1.20–3.58), of which OAS3 and COG5 found their expression increased after COVID-19 specific therapy. Most of the 30 proteins over-expressed in critical patients (fold-changes 1.17–4.43) were linked to inflammation, coagulation, lipids metabolism, complement or immunoglobulins, and a third of them decreased their expression after treatment. We propose a set of candidate proteins for biomarkers of COVID-19 prognosis at the time of hospital admission. The study design employed is distinctive from previous works and aimed to optimize the chances of the candidates to be validated in confirmatory studies and, eventually, to play a useful role in the clinical practice.

Concerns about the effect of greenhouse gases have motivated the development of certification protocols to quantify the industrial carbon footprint (cf). These protocols are manual, work-intensive, and expensive. All of the above have led to a shift towards automatic data-driven approaches to estimate the cf, including Machine Learning (ml) solutions. Unfortunately, as in other sectors of interest, the decision-making processes involved in these solutions lack transparency from the end user’s point of view, who must blindly trust their outcomes compared to intelligible traditional manual approaches. In this research, manual and automatic methodologies for cf estimation were reviewed, taking into account their transparency limitations. This analysis led to the proposal of a new explainable ml solution for automatic cf calculations through bank transaction classification. Consideration should be given to the fact that no previous research has considered the explainability of bank transaction classification for this purpose. For classification, different ml models have been employed based on their promising performance in similar problems in the literature, such as Support Vector Machine, Random Forest, and Recursive Neural Networks. The results obtained were in the 90 % range for accuracy, precision, and recall evaluation metrics. From their decision paths, the proposed solution estimates the co2 emissions associated with bank transactions. The explainability methodology is based on an agnostic evaluation of the influence of the input terms extracted from the descriptions of transactions using locally interpretable models. The explainability terms were automatically validated using a similarity metric over the descriptions of the target categories. Conclusively, the explanation performance is satisfactory in terms of the proximity of the explanations to the associated activity sector descriptions, endorsing the trustworthiness of the process for a human operator and end users.

Abstract Several cytokines and adipokines are related to clinical severity and progression in knee osteoarthritis. The aim of this study was to evaluate the associations of IL-8 with clinical severity and with local and systemic adipokines and cytokines. This is a Cross-sectional study including 115 women with symptomatic primary knee osteoarthritis with ultrasound-confirmed joint effusion. Age, symptoms duration and body mass index were collected. Radiographic severity was evaluated according to Kellgren–Lawrence. Pain and disability were assessed by Lequesne and Knee injury and Osteoarthritis Outcome Score pain, symptoms and function scales. Three inflammatory markers and five adipokines were measured by ELISA in serum and synovial fluid. Partial correlation coefficient (PCC) and corresponding 95% confidence interval were used to evaluate association. Synovial fluid IL-8 was significantly associated with clinical severity scales. After controlling for potential confounders, associations measured by a Partial Correlation Coefficient (PCC) remained essentially unaltered for Lequesne (PCC = 0.237), KOOS pain (PCC = − 0.201) and KOOS symptoms (PCC = − 0.209), KOOS function (PCC = − 0.185), although the later did not reach statistical significance. Also in synovial fluid samples, associations were found between IL-8 and TNF (PCC = 0.334), IL6 (PCC = 0.461), osteopontin (PCC = 0.575), visfatin (PCC = 0.194) and resistin (PCC = 0.182), although significance was not achieved for the later after statistical control for confounders. None of these associations were detected in serum. In conclusion, IL-8 was associated with clinical severity, inflammatory markers and adipokines in synovial fluid, but not in blood. Although the reported associations are weak to moderate in magnitude, these findings reinforce the notion that local and not systemic inflammation is more relevant to clinical severity in knee OA women with joint effusion.

BackgroundMetabolic syndrome (MetS) worsens quality of life and increases mortality. Dissatisfaction with weight in patients with MetS may modify the effect of lifestyle interventions to achieve changes in health-related behaviors.ObjectiveTo assess 1-year changes in cardiovascular risk scores, self-perceived general health and health-related behaviors according to observed changes in desired weight loss during the first year of intervention in a large cardiovascular prevention trial.DesignProspective analysis of the PREDIMED-PLUS trial, including 5,499 adults (55–75 years old) with overweight or obesity at baseline.MethodsThe desired weight loss was the difference between ideal and measured weight. Tertiles of change in desired weight loss (1 year vs. baseline) were defined by the following cut-off points: ≥0.0 kg (T1, n = 1,638); 0.0 to −4.0 kg (T2, n = 1,903); ≤−4.0 kg (T3, n = 1,958). A food frequency questionnaire assessed diet and the Minnesota-REGICOR questionnaire assessed physical activity. The Framingham equation assessed cardiovascular risks. The changes in the severity of MetS were also assessed. The Beck Depression Inventory assessed depressive symptoms and the SF-36 assessed health-related quality of life. Data were analyzed using general linear models.ResultsBMI decreased at T2 and T3 (T1: 0.3, T2: −0.7, T3: −1.9). The most significant improvement in diet quality was observed at T3. Cardiovascular risk decreased at T2 and T3. Mean reductions in MetS severity score were: −0.02 at T1, −0.39 at T2 and −0.78 at T3. The perception of physical health increases in successive tertiles.ConclusionsIn older adults with MetS, more ambitious desired weight loss goals were associated with improvements in diet, cardiovascular health and perceived physical health during the first year of a healthy lifestyle intervention programme. Current language processing technologies allow the creation of conversational chatbot platforms. Even though artificial intelligence is still too immature to support satisfactory user experience in many mass market domains, conversational interfaces have found their way into ad hoc applications such as call centres and online shopping assistants. However, they have not been applied so far to social inclusion of elderly people, who are particularly vulnerable to the digital divide. Many of them relieve their loneliness with traditional media such as TV and radio, which are known to create a feeling of companionship. In this paper we present the EBER chatbot, designed to reduce the digital gap for the elderly. EBER reads news in the background and adapts its responses to the user’s mood. Its novelty lies in the concept of “intelligent radio”, according to which, instead of simplifying a digital information system to make it accessible to the elderly, a traditional channel they find familiar -background news- is augmented with interactions via voice dialogues. We make it possible by combining Artificial Intelligence Modelling Language, automatic Natural Language Generation and Sentiment Analysis. The system allows accessing digital content of interest by combining words extracted from user answers to chatbot questions with keywords extracted from the news items. This approach permits defining metrics of the abstraction capabilities of the users depending on a spatial representation of the word space. To prove the suitability of the proposed solution we present results of real experiments conducted with elderly people that provided valuable insights. Our approach was considered satisfactory during the tests and improved the information search capabilities of the participants.

Drones may be more advantageous than fixed cameras for quality control applications in industrial facilities, since they can be redeployed dynamically and adjusted to production planning. The practical scenario that has motivated this paper, image acquisition with drones in a car manufacturing plant, requires drone positioning accuracy in the order of 5 cm. During repetitive manufacturing processes, it is assumed that quality control imaging drones will follow highly deterministic periodic paths, stop at predefined points to take images and send them to image recognition servers. Therefore, by relying on prior knowledge about production chain schedules, it is possible to optimize the positioning technologies for the drones to stay at all times within the boundaries of their flight plans, which will be composed of stopping points and the paths in between. This involves mitigating issues such as temporary blocking of line-of-sight between the drone and any existing radio beacons; sensor data noise; and the loss of visual references. We present a self-corrective solution for this purpose. It corrects visual odometer readings based on filtered and clustered Ultra-Wide Band (UWB) data, as an alternative to direct Kalman fusion. The approach combines the advantages of these technologies when at least one of them works properly at any measurement spot. It has three method components: independent Kalman filtering, data association by means of stream clustering and mutual correction of sensor readings based on the generation of cumulative correction vectors. The approach is inspired by the observation that UWB positioning works reasonably well at static spots whereas visual odometer measurements reflect straight displacements correctly but can underestimate their length. Our experimental results demonstrate the advantages of the approach in the application scenario over Kalman fusion, in terms of stopping point detection and trajectory estimation error.

Short texts are omnipresent in real-time news, social network commentaries, etc. Traditional text representation methods have been successfully applied to self-contained documents of medium size. However, information in short texts is often insufficient, due, for example, to the use of mnemonics, which makes them hard to classify. Therefore, the particularities of specific domains must be exploited. In this article we describe a novel system that combines Natural Language Processing techniques with Machine Learning algorithms to classify banking transaction descriptions for personal finance management, a problem that was not previously considered in the literature. We trained and tested that system on a labelled dataset with real customer transactions that will be available to other researchers on request. Motivated by existing solutions in spam detection, we also propose a short text similarity detector to reduce training set size based on the Jaccard distance. Experimental results with a two-stage classifier combining this detector with a SVM indicate a high accuracy in comparison with alternative approaches, taking into account complexity and computing time. Finally, we present a use case with a personal finance application, CoinScrap, which is available at Google Play and App Store.

Micro-blogging sources such as the Twitter social network provide valuable real-time data for market prediction models. Investors' opinions in this network follow the fluctuations of the stock markets and often include educated speculations on market opportunities that may have impact on the actions of other investors. In view of this, we propose a novel system to detect positive predictions in tweets, a type of financial emotions which we term “opportunities” that are akin to “anticipation” in Plutchik's theory. Specifically, we seek a high detection precision to present a financial operator a substantial amount of such tweets while differentiating them from the rest of financial emotions in our system. We achieve it with a three-layer stacked Machine Learning classification system with sophisticated features that result from applying Natural Language Processing techniques to extract valuable linguistic information. Experimental results on a dataset that has been manually annotated with financial emotion and ticker occurrence tags demonstrate that our system yields satisfactory and competitive performance in financial opportunity detection, with precision values up to 83%. This promising outcome endorses the usability of our system to support investors' decision making.

Abstract Background Premature birth is a growing and serious public health problem affecting more than one of every ten infants worldwide. Bronchopulmonary dysplasia (BPD) is the most common neonatal morbidity associated with prematurity and infants with BPD suffer from increased incidence of respiratory infections, asthma, other forms of chronic lung illness, and death (Day and Ryan, Pediatr Res 81: 210–213, 2017; Isayama et la., JAMA Pediatr 171:271–279, 2017). BPD is now understood as a longitudinal disease process influenced by the intrauterine environment during gestation and modulated by gene-environment interactions throughout the neonatal and early childhood periods. Despite of this concept, there remains a paucity of multidisciplinary team-based approaches dedicated to the comprehensive study of this complex disease. Acute leukemia (AL) is an important cause of morbidity and mortality in children, and neurological manifestations (NM) are frequent. The objective of this study was to analyze neurological manifestations in children with acute leukemia from cases attended in the last five years at the Centro Médico Nacional “20 de Noviembre”. Methods: Conducting a retrospective and analytical study from 1 January 2015 to 31 December 2020 in children with AL classified according to sex, age range and AL type. Participants were grouped according the presence of NM. Results: We analyzed 607 patients: 54.85% boys and 44.14% girls, with a mean age of 7.27 ± 4.54 years. When comparing groups, the NM group was significantly older (p = 0.01), and the highest prevalence was between 6 and 12 years old. ALL was predominant over the other lineages (p ≤ 0.01). The most frequent NM was CNS infiltration, seizures, headache and neuropathy. Death outcomes occurred in 18.7% of children with AML, 11.8% with ALL and 50% with MPAL (p ≤ 0.002). The NM group was associated with higher mortality during a follow-up time of 77.9 ± 49 months (44.4% vs. 8.9% deaths, NM vs. non-NM, respectively; OR = 3.3; 95% CI 2.4 to 4.6; p ≤ 0.0001). Conclusions: ALL was the most prevalent leukemia type. CNS infiltration, seizures, headache, neuropathy and PRES were the most frequent symptoms in the NM group. NM was associated with a higher mortality rate.

COVID-19 has affected millions of children and, while it was previously considered as a respiratory disease, neurologic involvement has also been documented. The objective of this study was to identify the neurological manifestations (NMs) and the outcomes of children with COVID-19 who attended the National Medical Center “20 de Noviembre”. Methods: A retrospective cohort study of children hospitalized for COVID-19 from April 2020 to March 2021 was conducted. Clinical-demographic data were registered. Neurologic manifestations were defined as any clinical neurological expression of the central and/or peripheral nervous system that occurred during admission or hospitalization. Results: In total, 46 children with a confirmed COVID-19 result, 26 (56.5%) boys and 20 (43.5%) girls with a median age of 8.9 ± 4.6 years, constituted the study population. Half of the children showed some NMs, and this group of patients concomitantly showed acute lymphoblastic leukemia (ALL, 56%), obesity (17.3%), or acute myeloblastic leukemia (AML, 4.3%). The most frequently described NMs were headache (13, 56%), encephalopathy (10, 43.47%), and epilepsy (4, 17.39%). The mortality rate in children with NMs was 21.7% and they had a higher mortality rate when compared to those without NM p ≤ 0.025. Conclusions: NMs occurred predominantly in male children aged 6 to 12 years; ALL was the most frequent comorbidity. Headache prevailed and hypoxemia, hypocalcemia, elevated ferritin, and C-reactive protein were associated with NM. Finally, NMs were a risk factor for mortality.

O presente artigo discute, a partir de uma pesrpectiva crítica, o processo de Cooperação Sul-Sul (CSS) no campo educacional entre Brasil e Timor-Leste, com base no paradigma do dom e da hospitalidade. Durante o governo Lula (2003-2010), a CSS se constituiu como uma das principais plataformas de inserção do Brasil no meio internacional. Além disso, foi o governo que mais energizou esforços ao desenvolvimento da cooperação educacional. No entanto, o argumento central é que o Brasil ainda necessita investir mais em políticas de recepção de estudantes estrangeiros orientadas por uma lógica do bem-receber. Nesses termos, a metodologia aplicada consistiu na análise de documentos oficiais, incluindo um protocolo de cooperação entre ambos os países, a literatura especializada e uma pesquisa de caráter etnográfico realizada com 30 estudantes timorenses. Diante do que foi observado, conclui-se que o processo de cooperação no campo educacional, quando compreendido a partir de variáveis endógenas e exógenas, revelam peculiaridades e desafios que podem ser superados a partir das práticas ancoradas no paradigma do dom e da hospitalidade.

We report a case of invasive candidiasis presenting as multiple lung nodules and cavitary lesions with minimal pleural effusion. Candida infections of the lung are rare but can occur after hematologic dissemination of the yeast from other body sites, such as the skin and the gastrointestinal and genitourinary tracts. Here, we describe the case of a 56-year-old female with a history of end-stage renal disease (ESRD) who presented with fever, productive cough, and pulmonary nodules and cavitary lesions seen on a chest computed tomography (CT). The patient's blood cultures were positive for Candida zeylanoides.

Os contatos de estrangeiros com representantes do Estado que desempenham papéis de controle da ordem social e repressão à sua infração frequentemente são marcados por um clima de desconfiança. No caso dos migrantes internacionais, a apresentação à Polícia Federal, por qualquer motivo, representa um momento tenso em que paira o sentimento de se ter feito algo errado, mesmo quando isso não procede. Paralelamente, intelectuais voltados para o estudo de imigrantes no Brasil têm dado preferência a investigar os processos migratórios na perspectiva dos que migram ou dos que os recebem, mas tendem a deixar de lado os agentes estatais mediadores que definem a condição de regularidade e legalidade de estrangeiros no país. A referência a eles tende a carregar pressupostos e pré-conceitos, cujo ponto de partida e chegada é o entendimento de que estrangeiros e agentes estatais estão em posição de confronto em grande parte das vezes. Diante disso, este trabalho pretende discutir etnograficamente o modo como alguns representantes da Delegacia de Migração da Polícia Federal em João Pessoa (PB) percebem a si mesmos no desempenho de seu oficio policial no contato cotidiano com estrangeiros regulares na Paraíba, suas visões sobre a instituição à qual pertencem e suas percepções de mundo no que tange ao contato com migrantes internacionais. Os primeiros dados da pesquisa apontam para a sensibilidade de alguns membros da Delegacia que superam as expectativas meramente burocráticas no desempenho das funções profissionais cotidianas, revelando desejos e práticas de solidariedade e hospitalidade em relação a estrangeiros regulares no estado.

Face-mounting of photographs is a method of exhibiting contemporary photography which is widespread nowadays in museums and galleries, and which presents specific problems in the field of conservation and restoration. The research presented here includes the study of its main constituent materials, considers different types of sources and documentation, and also addresses analysis and accelerated aging tests which are currently being developed.

Microglia respond to adverse stimuli in order to restore brain homeostasis and, upon activation, they release a number of inflammatory mediators. Chronic microglial overactivation is related to neuroinflammation in Alzheimer’s disease. In this work, we show that oleanolic acid (OA), a natural triterpene present in food and medicinal plants, attenuates the activation of BV2 microglial cells induced by lipopolysaccharide (LPS). Cell pretreatment with OA inhibited the release of IL-1β, IL-6, TNF-α, and NO, which was associated with the downregulation of the expression of genes encoding for these cytokines and inducible nitric oxide synthase (iNOS), and the reinforcement of the endogenous antioxidant cell defense. These findings advocate considering OA as a novel neuroprotective agent to inhibit oxidative stress and inflammatory response in activated microglia associated with Alzheimer’s disease.

The present study reports the production of upconverter nanostructures composed by a yttrium oxide host matrix co-doped with ytterbium and europium, i.e., Y2O3:Yb3+/Eu3+. These nanostructures were formed through the dissociation of yttrium, ytterbium and europium oxides using acetic, hydrochloric and nitric acids, followed by a fast hydrothermal method assisted by microwave irradiation and subsequent calcination process. Structural characterization has been carried out by X-ray diffraction (XRD), scanning transmission electron microscopy (STEM) and scanning electron microscopy (SEM) both coupled with energy dispersive X-ray spectroscopy (EDS). The acid used for dissociation of the primary oxides played a crucial role on the morphology of the nanostructures. The acetic-based nanostructures resulted in nanosheets in the micrometer range, with thickness of around 50 nm, while hydrochloric and nitric resulted in sphere-shaped nanostructures. The produced nanostructures revealed a homogeneous distribution of the doping elements. The thermal behaviour of the materials has been investigated with in situ X-Ray diffraction and differential scanning calorimetry (DSC) experiments. Moreover, the optical band gaps of all materials were determined from diffuse reflectance spectroscopy, and their photoluminescence behaviour has been accessed showing significant differences depending on the acid used, which can directly influence their upconversion performance.

There is a significant gap in the literature about Indiana Latino students' struggles to access post-secondary options. This study uses focus groups and interviews to explore parents', students', and teachers' perceptions about barriers and opportunities Latino high school students have to transition into education options after high school. Findings suggest that Latino families, especially newcomers, lack knowledge and resources to overcome perceived barriers and take advantage of available opportunities. While parents have college aspirations for their children and perceive financial issues as the main obstacle, students identified language, academic, financial, cultural, and legal barriers hindering them from reaching their education goals. Newcomer families find more challenges as they have not built college-knowing social networks. The study expands on existing knowledge about how factors such as college aspirations, immigrant status, English proficiency level, perceived academic preparation, acculturative stress, and school messages and resources interplay to influence students' decisions and expectations. The study reaffirms the need for early exposure to college-going knowledge and strategies to reduce the negative impact of social stressors and to provide Latino students with equal educational opportunities to fulfill their goals.

Desde que foi criada, em 2005, a rede de comunicação com sede na Venezuela, Telesur, sempre esteve comprometida com a difusão de ideais bolivarianos, em especial o de integração regional. O presidente Hugo Chávez foi um de seus principais idealizadores e governou o país tendo como referencial a ideia de que o jogo político se confunde com o jogo midiático. A partir da morte de Chávez, a rede perde seu principal entusiasta. Em decorrência disso, lançam-se questões acerca dos próximos passos, rumos e caminhos a serem percorridos pela Telesur no que se refere à construção do projeto integracionista. Assim, este trabalho pretende analisar o papel da Telesur na construção simbólica da integração latino-americana, antes e depois da morte do ex-presidente, além de discutir o modo como tratou o tema da morte de seu líder. Para isso, será utilizada como fonte de pesquisa o site da rede.

A rede de comunicação multiestatal Telesur (da qual fazem parte Venezuela, Argentina, Cuba, Uruguai e Bolívia) se auto-representa como estando a serviço do ideal bolivariano de integração latino-americana. Para alcançar tal objetivo, vem divulgando o que define como “pluralidade de vozes” dos povos latino-americanos, em uma tentativa de construção cultural do que pode ser entendida como uma “identidade” regional. Sendo assim, este trabalho pretende analisar e discutir especificamente o modo como efetivamente a rede tenta construir tal “identidade latino-americana” e, em uma dimensão mais geral, os processos de objetivação cultural através dos meios de comunicação de massa.Palavras-chave: Telesul, comunicação na América Latina, identidade latinoamericana.

En este artículo se registra el proceso reflexivo seguido por los autores sobre ciertos tópicos relacionados con los procesos de cambio en ambientes sociales complejos. Las exploraciones realizadas los han conducido a privilegiar un modo de pensar por juegos sobre la formulación de un método de planificación, y a su vez, la reflexión sobre el modo de pensar los ha llevado a explorar el ‘metapensar’.

BACKGROUND: We previously reported the in vitro spontaneous transformation of human mesenchymal stem cells (MSC) generating a population with tumorigenic potential, that we termed transformed mesenchymal cells (TMC). METHODOLOGY/PRINCIPAL FINDINGS: Here we have characterized the molecular changes associated with TMC generation. Using microarrays techniques we identified a set of altered pathways and a greater number of downregulated than upregulated genes during MSC transformation, in part due to the expression of many untranslated RNAs in MSC. Microarray results were validated by qRT-PCR and protein detection. CONCLUSIONS/SIGNIFICANCE: In our model, the transformation process takes place through two sequential steps; first MSC bypass senescence by upregulating c-myc and repressing p16 levels. The cells then bypass cell crisis with acquisition of telomerase activity, Ink4a/Arf locus deletion and Rb hyperphosphorylation. Other transformation-associated changes include modulation of mitochondrial metabolism, DNA damage-repair proteins and cell cycle regulators. In this work we have characterized the molecular mechanisms implicated in TMC generation and we propose a two-stage model by which a human MSC becomes a tumor cell.

Resumo Este artigo pretende analisar como Portugal tem acolhido os seus estudantes internacionais. Baseando-se em dois projetos de pesquisa relativos ao ensino superior português, irá apresentar um estudo comparativo entre o acolhimento que Portugal tem proporcionado aos oriundos do Brasil e do Timor-Leste, na ótica desses alunos. Ambas as investigações, através do recurso ao método de análise qualitativo (entrevistas realizadas a esses estudantes), chegaram a uma confluência de interpretações sobre o modo como estes estudantes vêm sendo recebidos nas universidades portuguesas. Além disso, enquanto uma pesquisa completou esta análise utilizando, também, um método quantitativo (aplicação de um questionário online), a outra utilizou o método etnográfico. Conclui-se que, neste aspecto, ainda há muito a ser feito, como uma maior atenção às dificuldades de brasileiros e timorenses com o Português de Portugal, mais sensibilização dos professores para com os estudantes provenientes de sistemas educacionais distintos, maior divulgação dos serviços disponíveis pelas universidades aos estrangeiros e apoio efetivo e afetivo na chegada ao país de destino.

ABSTRACT Introduction and objective: Focal cryotherapy emerged as an efficient option to treat favorable and localized prostate cancer (PCa). The purpose of this video is to describe the procedure step by step. Materials and methods: We present the case of a 68 year-old man with localized PCa in the anterior aspect of the prostate. Results: The procedure is performed under general anesthesia, with the patient in lithotomy position. Briefly, the equipment utilized includes the cryotherapy console coupled with an ultrasound system, argon and helium gas bottles, cryoprobes, temperature probes and an urethral warming catheter. The procedure starts with a real-time trans-rectal prostate ultrasound, which is used to outline the prostate, the urethra and the rectal wall. The cryoprobes are pretested and placed in to the prostate through the perineum, following a grid template, along with the temperature sensors under ultrasound guidance. A cystoscopy confirms the right positioning of the needles and the urethral warming catheter is installed. Thereafter, the freeze sequence with argon gas is started, achieving extremely low temperatures (-40°C) to induce tumor cell lysis. Sequentially, the thawing cycle is performed using helium gas. This process is repeated one time. Results among several series showed a biochemical disease-free survival between 71-93% at 9-70 month- follow-up, incontinence rates between 0-3.6% and erectile dysfunction between 0-42% (1–5). Conclusions: Focal cryotherapy is a feasible procedure to treat anterior PCa that may offer minimal morbidity, allowing good cancer control and better functional outcomes when compared to whole-gland treatment.
