Abstract

The Industrial Control System (ICS) environment encompasses a wide range of intricate communication protocols, posing substantial challenges for Security Operations Center (SOC) analysts tasked with monitoring, interpreting, and addressing network activities and security incidents. Conventional monitoring tools and techniques often struggle to provide a clear understanding of the nature and intent of ICS-specific communications. To enhance comprehension, we propose a software solution powered by a Large Language Model (LLM). This solution currently focused on BACnet protocol, processes a packet file data and extracts context by using a mapping database, and contemporary context retrieval methods for Retrieval Augmented Generation (RAG). The processed packet information, combined with the extracted context, serves as input to the LLM, which generates a concise packet file summary for the user. The software delivers a clear, coherent, and easily understandable summary of network activities, enabling SOC analysts to better assess the current state of the control system., Comment: 12 pages

Abstract

Electrically tunable optical devices present diverse functionalities for manipulating electromagnetic waves by leveraging elements capable of reversibly switching between different optical states. This adaptability in adjusting their responses to electromagnetic waves after fabrication is crucial for developing more efficient and compact optical systems for a broad range of applications including sensing, imaging, telecommunications, and data storage. Chalcogenide-based phase change materials (PCMs) have shown great promise due to their stable, non-volatile phase transition between amorphous and crystalline states. Nonetheless, optimizing the switching parameters of PCM devices and maintaining their stable operation over thousands of cycles with minimal variation can be challenging. In this paper, we report on the critical role of PCM pattern as well as electrical pulse form in achieving reliable and stable switching, extending the operational lifetime of the device beyond 13,000 switching events. To achieve this, we have developed a computer-aided algorithm that monitors optical changes in the device and adjusts the applied voltage in accordance with the phase transformation process, thereby significantly enhancing the lifetime of these reconfigurable devices. Our findings reveal that patterned PCM structures show significantly higher endurance compared to blanket PCM thin films.

Abstract

With four different type of neutrino-induced interactions, we considered to investigate and reanalyse the KNO scaling in modified multiplicity distributions from a different perspective. In an attempt of first of its kind, we propose alternate fitting function to parameterise the distribution than the most widely adopted Slattery's function and compare it with yet another form. We propose the shifted Gompertz and Weibull functions as the fitting functions and compare their potency for the most conventional form of Slattery's function. Abstract

Ayurveda is one of the oldest healthcare systems  which emphasizes on protecting the health of a healthy  human being and taking care of diseased too. Health is  basically a balance between three fundamental states i.e.  Physical, Mental and Spiritual states and their relationship  with the external environment. Ahara(food) which we eat is  one of the most important and foremost content of Tri  Upastambha(Ahara, Nidra and Brahmacharya). 23. TO STUDY THE CHANGES IN SERIAL LUNG FUNCTIONS IN INTERSTITIAL LUNG DISEASE (ILD) PATIENTS.

Abstract

Background: The assessment of lung function, particularly lung volumes and gas transfer, recorded initially upon patient presentation and subsequent follow-up evaluations, along with high-resolution computed tomography (HRCT) of the chest, can facilitate the evaluation of the natural history of the disease or provide insights to guide therapy in interstitial lung disease (ILD). The utilization of available data related to connective tissue-related ILD (CTD-ILD), gastroesophageal reflux disease (GERD), smoke exposure, as well as any potential correlation of treatment with changes in lung functions, can offer valuable insights. Objectives: The main aim of this study was to study the changes in lung functions of ILD patients over one year apart, and whatever observations could be noticed based on case record form. Methodology: The study was conducted in a Tertiary Care Centre to evaluate changes in lung functions in patients with Interstitial Lung Disease (ILD) over one-year period, specifically assessing increases or decreases in lung function values. The data was collected on ILD patients over 18 years of age who had undergone two Pulmonary Function Tests (PFTs) one year apart, between 2013 and 2015, from the PFT register available in the Centre's PFT lab. Results: This study revealed that 81.25% of patients fell within the age range of 40-70 years. Furthermore, it was found that 77.5% of the patients experienced lung function restrictions. In fact, 95% of the patients exhibited HRCT chest images consistent with Nonspecific Interstitial Pnuemonia (NSIP), while 5% displayed Usual Interstitial Pnuemonia (UIP) patterns. Additionally, within the study group, 47.75% were diagnosed with connective tissue related ILDs. Notably, patients with a history of GERD experienced a median fall in FVC of 130ml/year, compared to 170ml/year in 15 patients without GERD. Conclusion: Thus the findings of the study hold the potential to improve the clinical care and outcomes of interstitial lung disease patients. [ABSTRACT FROM AUTHOR]

25. Therapy related AML in a case of chronic lymphocytic leukemia

Abstract

In comparison to the general population, patients with chronic lymphocytic leukemia (CLL) are at a higher risk of developing secondary malignancies. Several factors may contribute to pathogenesis, including direct effects of chemotherapy and radiation as well as the reduction of immune surveillance. Factors influencing the increased risk include the increasing age of CLL patients, chronic antigenic stimulation, and immune impairment related to CLL or chemotherapy. Compared to patients with acute myeloid leukemia (AML) that developed from de novo, therapy-related AML (t-AML) has had a poorer outcome. The range of cytogenetic abnormalities in therapy-related AML is comparable to that in de novo AML, although these patients have a significantly higher frequency of unfavourable cytogenetics, such as a complex karyotype or a deletion or loss of chromosomes 5 and/or 7. Herein, we describe a case of therapy-related AML with monocytic differentiation and t(8;16) with a residual CLL population. The aim of the present case is to highlight rare occurrence of therapy related AML with t(8;16) in CLL after fluderabine based chemotherapy (FCR: fludarabine, cyclophosphamide, and rituximab). This case also highlights flowcytometric immunophenotyping as an ideal tool to characterize secondary AML along with the identification of minimal residual disease of CLL clone, which could have ignored at t-AML diagnosis. The pathogenesis of myeloid and lymphoid malignancies as well as their co-existence can be studied by focusing on such patients. Factors predisposing to the development of t-AML should be studied further, which would help in monitoring these patients more carefully.

26. Cytomorphological evaluation of non-haematopoietic malignancies metastasizing to the bone marrow

Abstract

Bone marrow (BM) is one of the rare but important site of metastasis of solid tumors. The key steps of metastasis include invasion, intravasation, circulation, extravasation, and colonization. Tumor cells may express some adhesion molecules that promote the transmigration to the marrow space and link them to the marrow stroma with subsequent engraftment. It is important to detect the bone marrow metastasis for initial clinical staging, therapeutic selection, prognostic risk stratification, assessment of response to therapy and predicting relapse. Prognosis of non-hematopoietic malignancies with BM metastasis is dismal. Due to occulting and atypical clinical manifestations, bone marrow metastases can be easily missed or misdiagnosed, leading to higher mortality rates. The important factors on which the prognosis of patients with bone marrow metastases depends are primary tumor site, performance status, platelet count, and therapeutic regimens (systemic chemotherapy or palliative/supportive care). Further, in cases with BM metastasis with unknown primary sites, misdiagnosis can lead to delayed initiation of therapy and increased mortality. BM metastasis is seen in less than 10% of patients with metastatic cancer and is common in lung, breast or prostate carcinoma. Bone marrow metastasis can be presented as the initial presentation with hematological changes and may be misdiagnosed as a primary haematopoietic disorder. Leucoerythoblastic blood picture is the most common peripheral blood smear finding indicating BM metastasis, may be an indicator of associated BM fibrosis. Bone marrow aspiration and biopsy with immunohistochemistry (IHC) is an easy, cost effective and gold standard method of detection of BM metastasis. BM biopsy is superior to bone marrow aspirate for detection of metastasis. Morphology of metastatic cells is as per the primary site of tumor. Immunohistochemistry is a useful adjunct to morphology in reaching a definitive diagnosis even in case with carcinoma unknown primary (CUP) and also in diagnosing case of unsuspected malignancies. Though bone marrow is not among the most common site of involvement in CUP, which are liver, bone, lymph nodes and lung. But BM, if involved, the site of origin is determined using the immunohistochemistry panel applied to the metastatic deposits based on the morphology The aim of the review is to discuss the hematological findings of non-haematopoietic malignancies metastasizing to the bone marrow, emphasizing on histomorphology with IHC and its significance in establishing primary diagnosis in clinically unsuspected cases.

Enyne diesters derived from biaryls and related substrates undergo intramolecular [2+2]‐cycloaddition and concomitant cyclo‐reversion when treated with 10 mol% In(OTf)3 in 1,2‐dichloroethane. This enyne metathesis type cycloisomerization reaction enables the synthesis of phenanthrene, benzophenanthrene, benzotetraphene and other polycyclic (hetero)aromatic compounds bearing a synthetically useful diethyl β‐(hetero)arylidene malonate moiety in a step‐ and atom‐economic manner. In(OTf)3‐catalyzed two independent intramolecular cycloisomerization on substrates 5 a and 5 b provides direct access to benzotetraphenes 6 a and 6 b containing bis(diethyl β‐phenylmethylene malonate) moiety. Tetrasubstituted vinylidene malonate 4 a, substiututed by two different aromatic groups, is stereoselectively converted into E‐ and Z‐isomer of triaryl group containing all carbon tetrasubstituted olefins (8 a, 8 a′ and 8 b) via Pd‐catalyzed stereoselective decarboxylative arylation reaction. 46. A Meticulous Study Between Sira, Dhamani And Srotas

Abstract

Acharya Charaka has defined Srotas as ‘Sravanat srotamsi’ meaning the structure through  which the sravanam takes place because of transudation some of them are called ‘Srotas’. Dhamani as  Dhamnat Dhamanya meaning because of pulsation, some of them are called ‘Dhamani’, and Sira as ‘Sarnat Sira’ i.e. because of the action of simply carrying a substance effortlessly from one place to another, some  others are called ‘Sira’ In physiological view, Srotas is very broad term which starts from external orifices (Navsrotansi) to the very  minute invisible ionic channel/gate etc. (Dosha Dhatu in Parmanu Swaroop as Dosha-Dhatuvaha Srotas).  This Srotas is included the term Sira which is characterized with the flow of blood (RasaRakta with Dosha  Dhatu) due to the life giving and Ranjan of Rasa so whole fluid comes under the term Rakta which expel out  in Raktamokshana Karma. Sira includes the term Dhamani in which has blood but with pressure flow exerted  by the contraction of heart. Acharya Charaka and Vagbhatta has this opinion as Charaka mentioned Sira Dhamani Srotas are synonyms and Vagbhatta mentioned Sira Dhamani are the Srotas only.  In anatomical view, Srotas is that structure excluding the Sira entity means present in pure form i.e. without  blood cells (Rakta Dhatu). Srotas is intra as well as extracellular spaces (Sravanat srotamsi). Sushruta has the  same opinion as he mentioned for Srotas Sira-Dhamani vivarjitam. Sira is the structure excluding Dhamani i.e. without much pressure (Sarnat Sira). So Sushruta broadly describe about Sira and Siravyadhvidhi in  separate chapters. Rest part is the structure called Dhamani having a pressure exerted by heart contraction  (Dhamanat Dhamani). It is pushing mechanism to Sira to supply nutrition for functioning actions (cry, sleep,  wake etc.) of the body. &nbsp
