3. SF‐CORNER (splenic flexure colorectal cancer): an international survey of operative approaches and outcomes for cancers of the splenic flexure.

Aim: The optimum surgical approach to splenic flexure cancers (SFCs) remains uncertain. The aim of this survey was to explore the opinions of an international surgical community on the management and outcomes of SFC. Method: A questionnaire was constructed comprising five sections (information about respondents; definition and prognosis of SFC; operative approach; approach in specific scenarios; outcomes) and circulated through an international dissemination committee and social media. Results: The survey received 576 responses over 4 weeks across 50 countries. There was no consensus regarding the definition of the splenic flexure, whilst the proportion of respondents who did and did not think that patients with SFC had a worse outcome was equal. The overall preferred operative approach was left hemicolectomy [203 (35.2%)], followed by segmental resection [167 (29%)], extended right hemicolectomy [126 (21.9%)] and subtotal colectomy [7 (12%)]. The stated pedicles for ligation varied between resection types and also within the same resection. One hundred and sixty‐six (28.8%) respondents thought a segmental resection was associated with the worst survival and 190 (33%) thought it was associated with the best quality of life. Conclusion: This survey confirms a lack of consensus across all aspects SFC treatment. The differing approaches described are likely to represent different beliefs around the variable anatomy of this region and the associated lymphatic drainage. Future studies are required to address such inconsistencies and identify the optimum surgical strategy, whilst also incorporating quality‐of‐life metrics and patient‐reported outcomes. A one‐size‐fits‐all approach is probably not appropriate with SFC, and a more bespoke approach is required. [ABSTRACT FROM AUTHOR]

25 páginas, 6 figuras, 2 tablas, Characterization of the genetic landscape of Alzheimer's disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/'proxy' AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOE ε4 allele., This work was funded by a grant (EADB) from the EU Joint Programme – Neurodegenerative Disease Research. The prediction of a difficult cholecystectomy has traditionally been based on certain pre-operative clinical and imaging factors. Most of the previous literature reported small patient cohorts and have not used an objective measure of operative difficulty. The aim of this study was to develop a pre-operative score to predict difficult cholecystectomy, as defined by a validated intra-operative difficulty grading scale. Two cohorts from prospectively maintained databases of patients who underwent laparoscopic cholecystectomy were analysed: the CholeS Study (8755 patients) and a single surgeon series (4089 patients). Factors potentially predictive of difficulty were correlated to the Nassar intra-operative difficulty scale. A multivariable binary logistic regression analysis was then used to identify factors that were independently associated with difficult laparoscopic cholecystectomy, defined as operative difficulty grades 3 to 5. The resulting model was then converted to a risk score, and validated on both internal and external datasets. Increasing age and ASA classification, male gender, diagnosis of CBD stone or cholecystitis, thick-walled gallbladders, CBD dilation, use of pre-operative ERCP and non-elective operations were found to be significant independent predictors of difficult cases. A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets.Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall's tau for dichotomous variables, or Jonckheere-Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis.A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p 0.001).We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.

35th European Photovoltaic Solar Energy Conference and Exhibition; 503-506, We developed a diamond wire characterized by the core steel-wire diameter of 80 μm and the size of diamond abrasives ranging from 6 to 12 μm (labelled as 80d-M6/12). The diamond particles on the wire are designed to have lower density and less agglomeration than those on conventional wires. As-developed diamond wire is introduced into multi wire saw to cut monocrystalline silicon bricks into wafers with a thickness of 210 and 140 μm. The above wire allows us to reduce the silicon dust or kerf to 100 μm per wafer. After as-sawn wafers are separated from fresh and worn-out wire sides of bricks and labelled as fresh and worn-out wire side wafers. In three-line bending test, a variable load is applied in perpendicular direction to wire saw marks on the wafer surface. The fracture strength distributions are evaluated on fresh and worn-out wire side wafers. In thick wafers, the fracture stress distributions are approximately the same on both sides. On the other hand, in thin wafers, the fracture strength is extremely lower than that of thick wafers. And particularly, the strength of fresh wire side wafers is lower compared to that of worn-out wire side wafers. In thin wafers, a small variation in the surface and subsurface damage is a crucial factor to decide their fracture strength of both-side wafers. In case of thick wafers, a small variation in the surface and subsurface damage may not have strong influence on their fracture strength of both-side wafers.

Background The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p 90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care.

33rd European Photovoltaic Solar Energy Conference and Exhibition; 534-536, We have investigated the mechanical strength of monocrystalline silicon wafers to improve the yield of photovoltaic (PV) cell fabrication process. In slicing monocrystalline silicon bricks, diamond abrasives fixed on a wire surface scratch front and backside surfaces of wafers more seriously than wafer edges. Therefore, damages and defects not at edges but on both surfaces cause a serious concern for lowering the mechanical strength. We prepared 200 and 120 μm thick wafers by cutting silicon bricks with a multi-wire saw, in which two kinds of diamond wires, the one with the abrasive-size ranging from 8 to 16 μm in diameter and the other from 6 to 12 μm were equipped. On the wire surface, the dispersion of 6 to 12 μm abrasives was improved to be larger compared with that of 8 to 16 μm abrasives. We report that the improved wire is essential to have wafers with higher mechanical strength and the reason why the wire improvement results in the strength increase.

Background The aim was to describe the management of benign gallbladder disease and identify characteristics associated with all-cause 30-day readmissions and complications in a prospective population-based cohort. Methods Data were collected on consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing all-cause 30-day readmissions and complications were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 8909 patients undergoing cholecystectomy from 167 hospitals. Some 1451 cholecystectomies (16·3 per cent) were performed as an emergency, 4165 (46·8 per cent) as elective operations, and 3293 patients (37·0 per cent) had had at least one previous emergency admission, but had surgery on a delayed basis. The readmission and complication rates at 30 days were 7·1 per cent (633 of 8909) and 10·8 per cent (962 of 8909) respectively. Both readmissions and complications were independently associated with increasing ASA fitness grade, duration of surgery, and increasing numbers of emergency admissions with gallbladder disease before cholecystectomy. No identifiable hospital characteristics were linked to readmissions and complications. Conclusion Readmissions and complications following cholecystectomy are common and associated with patient and disease characteristics.

Background The aims of this prospective population-based cohort study were to identify the patient and hospital characteristics associated with emergency cholecystectomy, and the influences of these in determining variations between hospitals. Methods Data were collected for consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing the performance of emergency cholecystectomy were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structure with patients (level 1) nested within hospitals (level 2). Results Data were collected on 4744 cholecystectomies from 165 hospitals. Increasing age, lower ASA fitness grade, biliary colic, the need for further imaging (magnetic retrograde cholangiopancreatography), endoscopic interventions (endoscopic retrograde cholangiopancreatography) and admission to a non-biliary centre significantly reduced the likelihood of an emergency cholecystectomy being performed. The multilevel model was used to calculate the probability of receiving an emergency cholecystectomy for a woman aged 40 years or over with an ASA grade of I or II and a BMI of at least 25·0 kg/m2, who presented with acute cholecystitis with an ultrasound scan showing a thick-walled gallbladder and a normal common bile duct. The mean predicted probability of receiving an emergency cholecystectomy was 0·52 (95 per cent c.i. 0·45 to 0·57). The predicted probabilities ranged from 0·02 to 0·95 across the 165 hospitals, demonstrating significant variation between hospitals. Conclusion Patients with similar characteristics presenting to different hospitals with acute gallbladder pathology do not receive comparable care.

To prepare zinc sillenite nanopowders, bismuth nitrate and zinc nitrate were taken in 12:1 and 12.66:0.33 mol ratios. Zinc sillenite (BiZnO) nanocrystalline powder was prepared by simple chemical solution decomposition method via heat treatment. Studies were focused on the formation of single crystalline phases and their linear optical properties. TG-DTA, XRD, DRS, IR, Raman, FE-SEM-EDS, and TEM techniques were used systematically to characterize the zinc sillenites. X-ray powder diffraction studies confirm that the heat treatment at 650 °C leads to cubic structure with zinc sillenites phase. Lattice parameter and the band gap of zinc sillenite were estimated as (a=) 10.21 Å and 2.75 eV respectively. We could observe the bismuth-oxygen sublattice vibrational modes alone in the Raman/IR vibrational spectra of zinc sillenite. Abstract: Nanopowders of PVP capped undoped, and Fe doped cadmium sulfide (CdS:Fe) were prepared by a simple co-precipitation method at room temperature, mixing the stoichiometric amount of reactants in a Milli Q water solvent. Spectral characteristics of as prepared nanopowders were investigated by using XRD, FTIR, Raman, UV-Vis absorption, FE-SEM-EDAX, Photoluminescence and ESR at room temperature. Extremely broad reflections of XRD peaks of as prepared powders are in nanometer size and cubic structure. Doping with Fe in CdS does not lead to any structural phase transformation but introduces a slight decrease in the lattice constants. Raman spectrum of pure and Fe doped CdS nanopowders has two characteristic LO phonon peaks. Raman spectrum of Fe doped CdS nanopowders shifts slightly towards higher energy side compared to their pure CdS nanopowders. Electron phonon confinement factor (S) varies in between 0.2 and 0.4. Photoluminescence emission spectrum shows a peak at 404nm which is attributed to localized band edge emission. At lower doping of Fe (5%, 10%) the emission intensity increases and for higher doping (20%) the emission intensity gets quenched. The electron spin resonance spectra of Fe doped CdS exhibit two distinct signals at g ∼4.3 and g ∼2 characteristic of Fe3+ ions. [Copyright &y& Elsevier]

Abstract: This paper reports the structural, linear and nonlinear optical properties of bismuth silicon oxide (BSO) nanocrystals with grain size from 60 to 150nm synthesized by chemical solution decomposition (CSD) method. The X-ray diffraction, FTIR, and micro Raman studies revealed the formation of pure single phase cubic structured BSO nanocrystals at 700°C of calcination temperature. TEM studies have confirmed the size and the presence of crystalline phase of the BSO nanocrystals. Linear optical properties studied through the UV–visible spectrum showed the presence of a “blue shifted” peak around 270nm corresponding to a band gap of 4.6eV due to the formation of nanophase crystals. The nonlinear optical properties studied using a standard open aperture z-scan showed strong nonlinear absorption both with nanosecond and picosecond pulsed excitations. In addition to the nonlinear absorption mechanism, nonlinear scattering was also observed to contribute to the reverse saturable absorption (RSA) behavior leading to a very high RSA behavior in both the time regimes. Nanoparticles have been attracting wide interest due to their potential applications in the field of nanophotonics and optoelectronics. Most of the nanoparticles exhibit interesting nonlinear optical properties and are being used for optical limiting purposes due to absorptive nonlinearities such as, two/multi-photon absorption (TPA/MPA), excited state/Free-carrier absorption (ESA/FCA) processes etc. Though the absorptive processes often provide low limiting thresholds, most of the materials have low damage threshold limiting their applications for optical limiting studies. We present our efforts on enhancing the damage threshold through nonlinear scattering so that these materials can be used efficiently even at high input fluences. Our efforts to achieve this goal have been through the preparation and nonlinear optical characterization of metal nanoclusters, semiconductor nanoparticles, and ferroelectric nanocrystals dispersed in different media. Herein nonlinear scattering results from Ag-Cu co-doped nanoclusters in Sol-Gel films. CdS nanoparticles dispersed in dimethylformamide (DMF) and BSO nanocrystals dispersed in PMMA are presented. The nonlinear optical properties of an unsymmetrical Zinc phthalocyanine were studied using the Z-scan technique in femtosecond and picosecond time domains. Open aperture Z-scan studies with ~110 femtosecond pulses revealed an intriguing behavior exhibiting switching phenomena (from saturable absorption to reverse saturable absorption and back to saturable absorption) as a function of concentration and the peak intensities used. Similar behavior was observed with ~2 picosecond pulse excitation. The observed results are qualitatively explained using an effective three-level model vis-à-vis a recent report on nonlinear absorption in a chloro-aluminum phthalocyanine where analogous performance was reported due to the solute migration. Such behavior is explicated as due to the consequence of combined effects of solvent contribution, large peak intensifies, concentration, etc. The nonlinear absorption coefficients were established from the fits to the data. Changes in composition in passivated mercury cadmium telluride (Hg1–xCdxTe or MCT) due to annealing have been investigated using Rutherford backscattering spectroscopy (RBS). Different passivated structures such as anodic oxide/MCT, CdTe/MCT and ZnS/MCT annealed at 80 °C in 10−6 Torr (parameters used during device packaging) have been studied and compared with the as-deposited samples. It is found that in as-deposited samples of anodic oxide/MCT and ZnS/MCT, the interface between the passivating layer and the MCT substrate is very sharp and composition of the capping layer remains constant throughout. Composition of the anodic oxide layer is estimated to be Hg:Te:Cd:O::0.4:1:0.25:3.35 while that of ZnS is Zn:S::0.9:1.1. In an annealed anodic oxide/MCT sample, the composition of the upper layer is different from that in the as-deposited sample and the interface is comparatively wider, while in the annealed ZnS/MCT sample the thickness of the interfacial region is found to be more than 50 nm and the composition of the remaining ZnS layer remains unaltered. The composition of the CdTe layer in both cases (as-deposited and annealed) is Cd:Te::1:1 and the interface thickness of the as-deposited sample is found to be ∼35 nm; which on annealing, increases to ∼50 nm. A case of bilaterally symmetrical genetic aplasia conforming to Mondini type of congenital deformity in a 12-day-old child is presented with the help of temporal bone sections. Cochlear changes include a stunted modiolus, deficient interscalar septum between the middle and upper coils forming a scala communis cochleae, a degenerated organ of Corti and reduced spiral ganglion cells and dendrites. The vestibule is malformed, with membranous labyrinth being deficient. The utricle and semicircular canals are absent. There is no oval window or stapedial footplate, and the facial nerve is hypoplastic. An interesting feature is the unusual association of bilateral bony choanal atresia, atrial septal defect, cleft lip, absence of olfactory bulbs in the brain, and congenital ophthalmic anomalies. • The campomelic syndrome is characterized by dwarfism, craniofacial anomalies, bowing of the tibiae and femora, cutaneous dimpling overlying the tibial bend, respiratory distress, and early death. Otolaryngologic manifestations include flat facies with a broad nasal bridge, low-set ears, cleft palate, mandibular hypoplasia, and tracheobronchial malacia. The underlying pathologic feature appears to be disturbance in cartilage growth involving the affected bones and the respiratory tract cartilage. The cause is unknown. We report clinical and histopathologic features in two cases of this syndrome. The endochondral layer of the otic capsule contained no cartilage cells. The cochlea was short and flattened, presenting a scala communis. The vestibule and the canals were deformed by bone invasion. Defective endochondral ossification of the petrooccipital synchondroses possibly explains the shortened skull base seen in this syndrome. 46. Temporal Bone Histopathologic Findings in Drowning Victims

- Abstract
• The human temporal bones of five drowning victims, the largest such series, to our knowledge, were evaluated to determine what histopathologic changes occurred. Thickening of the periosteal epithelium, especially on the surgical dome of the otic capsule, was evident in all cases. There was also hemorrhage in the middle ear cavity in four of the cases. In the fifth case, a cholesteatoma and ruptured tympanic membrane were observed, but there was no evidence of hemorrhage. It is proposed that an intact tympanic membrane is needed to create sufficient negative pressure in the middle ear cavity to cause rupture of the blood vessels and hemorrhage. 47. Familial Dysautonomia (Riley-Day Syndrome): Temporal Bone Findings and Otolaryngological Manifestations

Familial dysautonomia, or Riley-Day syndrome, is inherited in an autosomal recessive fashion and occurs almost exclusively in Jewish families. This disorder is characterized by a smooth tongue devoid of fungiform papillae and of taste buds, and is clinically associated with poor taste discrimination. An unsteady gait and dizziness on change in position are also common presenting symptoms. This study reports the histopathological findings of eight temporal bones from four patients with documented familial dysautonomia. For the control series, 13 normal temporal bones were also studied. The most striking finding in the dysautonomic patients was an extreme paucity of geniculate ganglion cells (P<0.001). A statistically significant reduction in the number of neurons was also found both in the superior and in the inferior divisions of the vestibular nerve (P<0.001). The paucity of the geniculate ganglion cells correlates well with the impairment of the taste in dysautonomic individuals, since the afferent fibers leaving taste buds of the anterior two-thirds of the tongue run via the chorda tympani and have their cell bodies in the geniculate ganglion. Furthermore, the reduction in the number of Scarpa's ganglion cells observed in the dysautonomic patients studied here could account for a poor response to caloric test, positional vertigo and an unsteady gait in this condition.

We report two cases of Hanhart's syndrome. The first patient was a male who died in the neonatal period, and whose temporal bones were removed, processed, and histologically examined; the findings are presented in this article. The second case concerns a living patient with the typical characteristics of the syndrome. An attempt is made to show that a gross first and second branchial arch anomaly exists in this syndrome and not just the micrognathia that is obvious to the onlooker. Possible causes and classifications of facial dysmorphia are discussed.

— The temporal bone findings in a case of hemifacial microsomia are described with photographs. The right facial hypoplasia was associated with anophthalmia and microtia on the same side. The right petrous bone was hypoplastic and showed total superior dehiscence of the internal acoustic meatus. The otic capsule was deformed with an underdeveloped cochlear modiolus grossly deficient in spiral ganglion population. The spiralling cochlear shell showed partial deficiency of the interscalar septum between the middle and apical coils. The cochlear duct was shorter than that on the normal side; the organ of Corti however was normal. The vestibular system did not show any structural abnormality except for the degeneration and reduction of the Scarpa's ganglion cells and nerve fibers. An additional interesting fact was that the facial nerve was totally absent in the temporal bone except for its nervus intermedius component.
