Purpose Different graft options are available for the reconstruction of lateral ankle ligaments to treat chronic ankle instability (CAI), which fall in two categories: allografts and autografts. This study aims to provide an updated comparison of the clinical outcomes after stabilisation procedures using allografts and autografts, to correctly advise the clinician during the choice of the best material to be used for the reconstruction of the lateral ligamentous complex of the ankle. Methods A systematic review was performed to analyse the use of autografts and allografts for anatomic reconstruction of the lateral ligamentous complex of the ankle in CAI patients. The presence of a postoperative assessment through outcome measures with proofs of validation in the CAI population or patient’s subjective evaluation on the treatment were necessary for inclusion. The quality of the included studies was assessed through the modified Coleman Methodology Score (mCMS). Relevant clinical outcome data were pooled to provide a synthetic description of the results in different groups or after different procedures. Results Twenty-nine studies (autograft: 19; allograft: 9; both procedures: 1) accounting for 930 procedures (autograft: 616; allograft: 314) were included. The average mCMS was 55.9 ± 10.5 points. The Karlsson-Peterson scale was the most frequently reported outcome scale, showing a cumulative average post- to preoperative difference of 31.9 points in the autograft group (n = 379, 33.8 months follow-up) and of 35.7 points in the allograft group (n = 227, 25.8 months follow-up). Patient satisfaction was good or excellent in 92.8% of autograft (n = 333, 65.2 months follow-up) and in 92.3% of allograft procedures (n = 153, 25.0 months follow-up). Return to activity after surgery and recurrence of instability were variably reported across the studies with no clear differences between allograft and autograft highlighted by these outcomes. Conclusions The systematic analysis of validated CAI outcome measures and the patient’s subjective satisfaction does not support a specific choice between autograft and allograft for the reconstruction of the ankle lateral ligamentous complex in CAI patients. Both types of grafts were associated to a postoperative Karlsson–Peterson score superior to 80 points and to a similar rate of patient’s subjective satisfaction. Level of evidence Level IV.

In 2020, the SARS-CoV-2 pandemic necessitated an adjustment to the care structure in all medical facilities in Germany. In this study we analysed the effects of the SARS-CoV-2 pandemic using the data from the annual balance sheet of a university department and maximum care provider for orthopaedics and trauma surgery.Data analysis was performed on the clinic's performance parameters from 2019 and 2020. The analysis included 52943 outpatient and 7980 inpatient cases, together with the Case Mix Index (CMI), and the costs and revenues.As a result of the reduction in scheduled surgeries, the mean number of inpatients in March and April 2020 decreased by 10.4% (p = 0.004). Due to the focus on complex cases, the CMI increased by 29.5% (p = 0.007). The number of outpatient surgical procedures increased by 16.9% (n = 84). Rising costs of consumables due to the purchase of protective equipment were offset by a reduced need for orthopaedic implants.By reducing the number of elective inpatient surgical treatments, the focus was shifted to treating complex cases; this is reflected in the increased CMI. Due to internal compensation mechanisms, the clinic was able to conserve intensive care capacities while still achieving an increase in revenue.EINFüHRUNG: Im Jahr 2020 erforderte die SARS-CoV-2-Pandemie eine Anpassung der Versorgungsstruktur in allen medizinischen Einrichtungen in Deutschland. In times of a pandemic threat, such as COVID-19, and the need for reduced direct doctor-patient contact, internet-based telemedicine has attracted more and more attention as a surrogate service. Suspending the diagnosis and treatment of non-virus related diseases for longer periods of time is not a viable option since this would only exacerbate problems on the patient and national level. The need for alternative treatment modalities increased rather quickly. So far, telemedical applications have mainly focused on teleradiological diagnosis, follow-up and monitoring of psychiatric and internal diseases, as well as geriatric patient care. As far as these authors are aware, orthopaedic physical examination of the knee joint, including trauma work-up, has not been the subject of any studies to date. This feasibility study explores how video consultation can be designed and implemented in the context of history taking and physical examination in knee joint complaints.21 patient actors (PA) with simulated complaints of the knee joint were examined individually for each diagnosis, first via video consultation and then directly by a specialist (SP). One PA group has a medical background, the other was made up of laypersons. The time was measured for both types of consultation. The physician documented the detected symptoms, the quality of implementation of the self-examination steps, and the derived diagnosis on an assessment form. After completion of both consultation sessions, the PAs were handed a questionnaire on the respective examination modality.With the video consultation the examination lasted 8.63 (± 2.5) minutes on average and with the regular consultation in person 5.63 (± 1.7) minutes (p 0.001). For the group with medical background the examination lasted 7.67 (± 1.4) minutes on average, while for the lay group the video consultation took 9.7 (± 3.1) minutes (p = 0.049). With increased age, the video consultation was prolonged (p = 0.032; r = 0.47). The mean value for self-examination of leg axis, gait pattern and degrees of freedom was 9.32 (± 0.4) of 10 points. The following functional tests resulted in lower mean values (points): Payr 7.2 (± 2.3), Merke 5.9 (± 2.8), no-touch Lachmann 6.4 (± 2.7), gravity sign-recurvatum 6.7 (± 2.4). The mean grade by the PAs for the feasibility of self-examination was 2.43 (± 0.98) out of 5 points.The video consultation for musculoskeletal complaints of the knee joint allows exploratory remote examination and helps to minimise the number of patients in hospitals and practices. It takes longer for the physician to perform and does not permit functional testing for ligament injuries of the knee joint. In its present form, telemedical examination is not able to fully replace personal consultation.In Zeiten einer pandemischen Bedrohungslage, wie bei COVID-19, und der Notwendigkeit reduzierter direkter Arzt-Patienten-Kontakte rückte die internetbasierte Videotelefonie mehr und mehr als Ersatzserviceleistung in den Fokus. Um individuellen oder gesamtwirtschaftlichen Problemen entgegenzuwirken, darf die Erkennung und Behandlung anderer Erkrankungen keine längerfristige Unterbrechung erfahren. In kurzer Zeit stieg der Bedarf alternativer Methoden zur Patientenbetreuung. Bisherige telemedizinische Einsatzgebiete lagen vor allem im Bereich der radiologischen Befundung, der Nachsorge und des Monitorings psychiatrischer und internistischer Erkrankungen sowie geriatrischer Patientenbetreuung. Studien zur Einsatzmöglichkeit für eine orthopädisch-unfallchirurgische Untersuchung des Kniegelenks existieren nach Autoreninformationen bislang nicht. Wie sich der Einsatz einer Videosprechstunde in der speziellen Anwendung für die Anamnese und Untersuchung bei Beschwerden des Kniegelenks konzipieren und umsetzen lässt, soll im Rahmen dieser Machbarkeitsstudie erarbeitet werden.21 Schauspielpatienten (SP) mit simulierten Beschwerden des Kniegelenks wurden für jede Diagnose einzeln zunächst über Videotelefonie und anschließend im direkten Kontakt durch einen Facharzt (FA) untersucht. Eine Gruppe der SP war medizinisch vorgebildet, die andere bestand aus Laien. Von beiden Untersuchungseinheiten wurde die Zeit gemessen. Die erkannten Symptome, die patientenseitige Durchführungsqualität der Eigenuntersuchungsschritte und die abgeleitete Diagnose wurde auf einem Bewertungsbogen durch den Arzt dokumentiert. Die SPs erhielten zum Abschluss beider Untersuchungseinheiten einen Bewertungsbogen zur jeweiligen Untersuchungsmethodik.Die Untersuchungseinheit der Videosprechstunde dauerte im Mittel 8,63 (± 2,5) min, die der normalen Sprechstunde 5,63 (± 1,7) min (p 0,001). Die Dauer der Videountersuchung zeigte für die Gruppe mit medizinischem Vorwissen einen Wert von 7,67 (± 1,4) min, wohingegen die Untersuchung der Laiengruppe im Mittel 9,7 (± 3,1) min dauerte (p = 0,049). Ein höheres Alter verlängerte die Dauer der Videountersuchung (p = 0,032; r = 0,47). Im Mittel wurden 9,32 (± 0,4) von 10 Punkten für die Eigenuntersuchungen der Beinachse, des Gangbildes und der Freiheitsgrade erreicht. Niedrigere Mittelwerte erreichten die Funktionstests: Payr-Test 7,2 (± 2,3) Punkte, Merke-Test 5,9 (± 2,8) Punkte, No-touch-Lachmann-Test 6,4 (± 2,7) Punkte, Gravity-Sign-Recurvatum-Test 6,7 (± 2,4) Punkte. Die Durchführbarkeit der Eigenuntersuchungen bewerteten die SP im Mittel mit 2,43 (± 0,98) von 5 Punkten.Die Videosprechstunde für muskuloskelettale Beschwerden des Kniegelenks ermöglicht eine orientierende Fernuntersuchung und hilft, das Patientenaufkommen in Kliniken und Praxen zu minimieren. Sie geht mit einem zeitlichen Mehraufwand für den Arzt einher und findet ihre Limitierung in den Funktionstests der Kniebinnenstrukturen. Die telemedizinische Untersuchung ersetzt in der heutigen Form keine persönliche Sprechstunde.

In Middle Europe developmental dysplasia of the hip (DDH) has an incidence of up to 5.9 %. The rate of congenital hip dislocation as the worst complication of a growth disorder of the hip is between 1.5 % and 2.5 %. Among known risk factors of DDH are breech position, multiples, foot deformities and family history. The aim of this retrospective study was to investigate prematurity as a risk factor for developmental dysplasia of the hip (DDH). Die kongenitale Hüftluxation als schwerste Komplikation liegt zwischen 1,5 und 2,5 %. Zu den akzeptierten Risikofaktoren der Hüftdysplasie werden die Beckenendlage, Mehrlingsgeburten, Fußdeformitäten und familiäre Häufung gezählt. Das Ziel dieser retrospektiven Datenerhebung ist die Untersuchung der Frühgeburtlichkeit als Risikofaktor für das Vorliegen einer Hüftdysplasie. Daher wurden die Hüften von 283 Neugeborenen, welche vor der 38. Schwangerschaftswoche oder früher, und jene von 377 Neugeborenen, welche nach der 37. Schwangerschaftswoche geboren wurden, mit der Hüftultraschallmethode nach Graf et al. in der ersten Woche postpartal untersucht. Überraschenderweise war der Unterschied der Alpha-Winkel zwischen den beiden Gruppen äußerst signifikant zugunsten der Frühgeborenen. Darüber hinaus konnten wir eine physiologische Entwicklungskurve mit einem Höchstwert nach der 31. SSW darstellen. Die Inzidenz der Hüftdysplasie war nicht statistisch signifikant unterschiedlich in beiden Gruppen. Unsere Daten deuten darauf hin, dass die Frühgeburtlichkeit kein Risikofaktur für die Hüftdysplasie darstellt, sondern wahrscheinlich protektiv auf die Hüftentwicklung wirkt.

Der Kostendruck in einem kompetitiven Umfeld zwingt Krankenhauser und Arzte, Ablaufe zu optimieren. Um sich Wettbewerbsvorteile zu sichern, ist die standige Evaluation relevanter Prozesse notwendig. Administrativer und medizinischer Ablauf in einer unfallchirurgischen Hochschulambulanz wurden mithilfe der „Lean“-Methode evaluiert, um Patientenwartezeiten zu reduzieren. Im Zeitraum von 2 Wochen wurden alle Patienten, die in der orthopadisch-unfallchirurgischen Klinik ambulant behandelt wurden, in die Bewertung des Prozesses einbezogen. Mitarbeiter der Poliklinik wurden vorbereitet und geschult, um den telefonisch vereinbarten Termin, Zeitpunkt des Erscheinens in der Poliklinik, ersten Kontakt, Administrationsprozess, ersten Arztkontakt, Dauer des Aufenthalts in der radiologischen und anasthesiologischen Klinik sowie Abschluss der Behandlung zu erfassen. Zusatzlichen wurden potenzielle Ineffizienzen durch eine Patientenflussanalyse sowie die personliche Befragung von Mitarbeitern in der Administration, in der Ambulanz sowie von Assistenz- und Facharzten identifiziert. Es wurden 126 Patienten in die Studie aufgenommen. Die durchschnittliche Verweildauer eines Patienten in der Ambulanz betrug 144 min (Range 30–371 min). Eine notwendige bildgebende Untersuchung erhohte die Verweildauer um durchschnittlich 53 min, eine notwendige Pramedikation im Mittel um 78 min, im Vergleich zu Patienten ohne erforderliche weitere Konsultationen. Mithilfe der Analyse der Patientenwege und -zeiten konnten verschiedene Grunde fur die Wartezeiten in der Hochschulambulanz dargelegt werden. Diese Arbeit zeigt, dass eine strukturierte Anwendung des Lean management sowie eine dezidierte Analyse einen Mehrwert fur den Patienten durch die Reduktion von Wartezeiten schaffen.

Der OP stellt den grosten Kostenblock und Kern der Leistungserbringung einer operativen Klinik dar. Steigerungen von Behandlungsqualitat und wirtschaftlichem Erfolg sind klassische Ziele der Prozessoptimierungen. Da der Abbau qualifizierten Personals jahrelang als erfolgversprechendster Faktor zur Kostenersparnis favorisiert wurde, bedroht der Mangel an Pflegekraften heutzutage den Betrieb in vielen deutschen Kliniken. Welche Verbesserungen sind moglich, bei bereits bestehender Einschrankung des OP-Betriebs? Was sind kritische Ressourcen, welche ihre kritische Belastung, und wie konnen diese optimiert werden? Der Analyse der OP-Organisation einer orthopadisch-unfallchirurgischen Klinik mit bereits erfolgter Reduktion von OP-Kapazitaten aufgrund des Mangels an OP- und Anasthesiepflegekraften folgte die Evaluierung moglicher Veranderungen mit den jeweiligen Vor- und Nachteilen. Nach Auswahl und Umsetzung wurden qualitative und quantitative Unterschiede vor und nach den Veranderungen untersucht. Vielschichtige Problemfelder konnten identifiziert werden. Ausgewahlt und umgesetzt wurden die Etablierung eines „Fast-track“-OP mit Konzentration zusatzlicher Ressourcen auf viele schnelle Eingriffe in einem OP statt auf wenige komplexe Falle. Die Einrichtung einer „holding area“ fur Patienten beseitigte transportbedingte Verzogerungen nahezu vollstandig. Anderungen in der OP-Planung und -kapazitatszuteilungen am Abend reduzierten nachtliche Operationszeiten. Trotz Reduktion der OP-Kapazitat konnten sowohl die Zahl der durchgefuhrten Operationen als auch die Schnitt-Naht-Zeit gesteigert werden. Optimierungen der Prozesse im OP sind trotz Personalmangel moglich und notwendig. Bereits wenige strukturelle Veranderungen konnen Flaschenhalse beseitigen sowie qualitative und quantitative Verbesserungen erbringen.

Zusammenfassung Einleitung Bei der Konsultation eines Facharztes für Orthopädie und Unfallchirurgie kommt der persönlichen Anamnese, Einschätzung und manuell-händischen klinischen Untersuchung durch den Arzt vor Ort eine elementare Rolle zu. Durch das „Digitale-Versorgung-Gesetz“ wurde die Durchführung von Videosprechstunden erleichtert. Die vorliegende Pilotstudie untersucht die Möglichkeit der videobasierten Einschätzung der Behandlungsdringlichkeit entsprechend einer strukturierten medizinischen Ersteinschätzung. Material und Methoden Im Rahmen eines Ex-ante-Designs wurden 40 Patienten mit einem orthopädisch-unfallchirurgischen Krankheitsbild mit elektiver, dringlicher oder Notfallindikation aus Sprechstunde oder Notaufnahme ausgewählt. Es wurde entsprechend den getätigten Angaben ein Kurzfragebogen ausgefüllt und durch einen ärztlichen Kollegen ein Bewegungsablauf entsprechend den Patientenangaben simuliert. Modifiziert nach den in der Wirbelsäulenorthopädie etablierten „red flags“ und „yellow flags“ wurden 9 kurze Fragen erfasst, welche die Dringlichkeit einer ärztlichen Vorstellung erfragten. Der videobasierte Bewegungsablauf orientiert sich an einem 60 s langen Instruktionsvideo, in dem motorische Tests und Bewegungsabläufe demonstriert werden: HWS-Bewegung in allen Ebenen, Elevation der oberen Extremität, Test des Finger-Boden-Abstandes beim „Vornüberbeugen“, Möglichkeit, aus dem Stand in die Hocke zu gehen und sich selbst wieder aufzurichten, sowie Zehen- und Hackenstand. Ergebnisse In 91,1% (n = 328) der Fälle war die Diagnose dem gleichen Gelenk bzw. der gleichen pathologischen Entität zugeordnet. In 37,5% (n = 135) der Fälle wurde eine Notfallindikation zur ärztlichen Vorstellung, in 10,8% (n = 39) der Fälle eine dringliche Indikation gesehen und in 51,6% (n = 186) der Fälle eine elektive Indikation gestellt. 12,5% (n = 45) der Fälle wurden „falsch positiv“ hinsichtlich einer Notfallvorstellung oder dringlichen Vorstellung evaluiert. Das bedeutet, dass die Probanden als „Notfall“ oder „dringlich“ eingestuft wurden, obgleich kein abwendbar gefährlicher Verlauf oder medizinischer Notfall vorlag. 18 Fälle (5%) wurden als „falsch negativ“ bewertet. Diskussion Der vorgestellte Screening-Fragebogen und das Kurz-Video-Assessment sind technisch umsetzbare und praktikable Methoden zur Erstevaluation vor der direkten, interaktiven videobasierten ärztlichen Onlinekonsultation. Zudem waren bei den von uns bewerteten Probanden der vorgestellte Fragebogen in Kombination mit dem Video-Assessment als Instrument geeignet, die Behandlungsdringlichkeit einzuschätzen und abwendbar gefährliche Verläufe und akute Notfälle zu selektieren. Schlussfolgerung Der Fragebogen ist in Kombination mit dem Video-Assessment während einer limitierten Zugänglichkeit des medizinischen Systems eine geeignete Methode während des notwendigen Social Distancings. Eine Rate von 5% falsch negativen, nicht korrekt eingeschätzten Patienten erscheint den Autoren für die klinische Praxis nicht akzeptabel und sollte daher optimiert werden.

When consulting a specialist in orthopaedics and trauma surgery, personal assessment and manual clinical examination by the doctor on site are essential. Nevertheless, implementation of video consultation hours has been made legally easier in Germany. The present pilot study examines the possibility of video-based assessment of the urgency of treatment in outpatient orthopaedic and trauma patients.Within an ex ante study design, 40 patients with an orthopaedic clinical picture for trauma surgery with elective, urgent or emergency indication were selected from consultation hours. A short questionnaire was filled in based on the information provided and a medical colleague simulated a movement sequence based on the patient information. After modification to the "red flags" and "yellow flags" established in spinal orthopaedics, nine short questions were recorded, which inquire about the urgency of a medical consultation. The video-based movement sequence is based on a 60 s long instruction video in which motor tests and movement sequences are demonstrated: Cervical spine movement in all levels, elevation of the upper extremity, test of the finger-floor distance when "bending forward over-bending", possibility of crouching from standing and getting up again, standing on heels and toes.In 91.1% (n = 328) of the cases, the diagnosis was associated with the same joint or the same pathological entity. In 37.5% (n = 135) of the cases an emergency indication was seen, in 10.8% (n = 39) of the cases an urgent indication and in 51.6% (n = 186) of the cases it was seen to offer an elective indication. 12.5% (n = 45) of the cases were evaluated as "false positives" with regard to an emergency or urgent presentation. This means that the test persons were classified as "emergency" or "urgent", although there was no preventable dangerous course or medical emergency. 18 cases (5%) were evaluated as "false negative".The screening questionnaire presented and the short video assessment are technically feasible and practicable method for the initial evaluation in video-based online medical consultation. In addition, the questionnaire presented in combination with the short video assessment was suitable as an instrument for assessing the urgency of the consultation and selecting preventable dangerous processes and acute emergencies.The questionnaire, in combination with the short video assessment, is a suitable method for the social distancing requirements during limited accessibility of the medical system. Der videobasierte Bewegungsablauf orientiert sich an einem 60 s langen Instruktionsvideo, in dem motorische Tests und Bewegungsabläufe demonstriert werden: HWS-Bewegung in allen Ebenen, Elevation der oberen Extremität, Test des Finger-Boden-Abstandes beim „Vornüberbeugen“, Möglichkeit, aus dem Stand in die Hocke zu gehen und sich selbst wieder aufzurichten, sowie Zehen- und Hackenstand.In 91,1% (n = 328) der Fälle war die Diagnose dem gleichen Gelenk bzw. der gleichen pathologischen Entität zugeordnet. In 37,5% (n = 135) der Fälle wurde eine Notfallindikation zur ärztlichen Vorstellung, in 10,8% (n = 39) der Fälle eine dringliche Indikation gesehen und in 51,6% (n = 186) der Fälle eine elektive Indikation gestellt. 12,5% (n = 45) der Fälle wurden „falsch positiv“ hinsichtlich einer Notfallvorstellung oder dringlichen Vorstellung evaluiert. Das bedeutet, dass die Probanden als „Notfall“ oder „dringlich“ eingestuft wurden, obgleich kein abwendbar gefährlicher Verlauf oder medizinischer Notfall vorlag. 18 Fälle (5%) wurden als „falsch negativ“ bewertet.Der vorgestellte Screening-Fragebogen und das Kurz-Video-Assessment sind technisch umsetzbare und praktikable Methoden zur Erstevaluation vor der direkten, interaktiven videobasierten ärztlichen Onlinekonsultation. Zudem waren bei den von uns bewerteten Probanden der vorgestellte Fragebogen in Kombination mit dem Video-Assessment als Instrument geeignet, die Behandlungsdringlichkeit einzuschätzen und abwendbar gefährliche Verläufe und akute Notfälle zu selektieren.Der Fragebogen ist in Kombination mit dem Video-Assessment während einer limitierten Zugänglichkeit des medizinischen Systems eine geeignete Methode während des notwendigen Social Distancings. Eine Rate von 5% falsch negativen, nicht korrekt eingeschätzten Patienten erscheint den Autoren für die klinische Praxis nicht akzeptabel und sollte daher optimiert werden.

Purpose The Nottingham Clavicle Score (NCS) is a patient-reported outcome measure developed to evaluate treatment results of clavicle, acromioclavicular and sternoclavicular joint pathologies. Valid, reliable and user-friendly translations of outcome measure instruments are needed to allow comparisons of international results. The aim of this cross-sectional study was to translate and adapt the NCS into German and evaluate the psychometric properties of the German version. Methods The translation and cross-cultural adaptation of the NCS were completed using a ‘translation–back translation” method and the final version was administered to 105 German-speaking patients. The psychometric properties of this version (NCS-G) were evaluated in terms of feasibility, reliability, validity and sensitivity to change. Results No major differences occurred between the NCS translations into German and back into English, and no content- or linguistic-related difficulties were reported. The Cronbach’s alpha for the NCS-G was 0.885, showing optimal internal consistency. The Intraclass Correlation Coefficient for test–retest reliability was 0.907 (95% CI 0.844–0.945), with a standard error of measurement of 5.59 points and a minimal detectable change of 15.50 points. The NCS-G showed moderate to strong correlation with all other investigated scales (Spearman correlation coefficient: qDASH: ρ = – 0.751; OSS: ρ = 0.728; Imatani Score: ρ = 0.646; CMS: ρ = 0.621; VAS: ρ = – 0.709). Good sensitivity to change was confirmed by an effect size of 1.17 (95% CI 0.89–1.47) and a standardized response mean of 1.23 (95% CI 0.98–1.45). Conclusions This study demonstrated that NCS-G is reliable, valid, reproducible and well accepted by patients, showing analogous psychometric properties to the original English version. The goal of this study was to compare operative outcomes after lesser toe deformity correction with either proximal interphalangeal (PIP) joint arthrodesis or PIP joint resection arthroplasty. A prospective randomized controlled trial was performed with 37 patients (48 toes) operated on with one of these two procedures. Evaluation of the numeric rating scale score, the American Orthopedic Foot and Ankle Society score, osseous consolidation, and clinical outcome was performed preoperatively and at 6 weeks and 6 months postoperatively. Both study groups showed significant improvement at 6 months postoperatively. Although osseous consolidation was significantly higher for the arthrodesis group ( P =.001), this difference did not affect clinical outcomes, and at 6 months postoperatively, pain on the numeric rating scale was 0 (range, 0–7) for the arthroplasty group and 0 (range, 0–5) for the arthrodesis group ( P =.669). The American Orthopedic Foot and Ankle Society score was 83 (range, 39–95) and 80 (range, 59–95), respectively ( P =.879). No difference was observed for signs of inflammation or axis correction. Even a direct comparison of toes with radiologically osseous fusion (n=16) with those without fusion (n=32) did not show any clinical differences. This randomized controlled study showed no clinical differences in outcome between PIP joint arthrodesis and PIP joint resection arthroplasty for correction of lesser toe deformities, with good to excellent outcomes for both groups. Introduction: Periprosthetic joint infections (PJI) are a major complication in joint-arthroplasty. While Rifampicin is one of the few antimicrobial agents, that penetrate the bacterial biofilm and therefore is often used as an additional agent to treat PJI, rifampicin-resistant pathogens are also known to be cross-resistant to other approved rifamycins (rifambutin, rifaximin and rifapentine). Moreover, rifaximin, a broad-spectrum antibiotic with poor intestinal absorption, is used to prevent episodes of hepatic encephalopathy. As transient resistances to rifampin may emerge in patients after taking rifaximin the aim of this study was to examine the microbial pattern of periprosthetic joint infections in cirrhotic patients and to test the hypothesis that intake of rifaximin increases the rate of resistance to rifampicin in periprosthetic joint infections. Methods: A cohort of cirrhotic patients and PJI (n=25) was analysed on the characteristics of bacterial isolates from sonication and tissue analysis. In a second step a subgroup analysis on the development of rifampicin resistant bacterial specimens, depending on the intake of rifaximin (8 rifaximin intake patients vs. 13 non rifaximin intake patients) was performed. Results: Gram-negative bacteria were found in nearly 30% of the specimens. By comparison of the single bacterial isolates, rifampicin resistance was detected in 69.2% (9/13) of the rifaximin-intake samples. In contrast, the non-rifaximin-intake isolates only were resistant to rifampicin in 22.2% (4/18) of the cases (p=0.01). The odds ratio for developing a rifampicin-resistance through rifaximin intake was calculated as OR=13.5.Conclusion: Periprosthetic joint infections have a high incidence of being caused by gram-negative bacteria in cirrhotic patients. Due to this change in microbial pattern and the innate resistance to rifampicin of most of gram-negative bacteria, the therapy with rifampicin should be carefully considered. The association between the use of rifaximin and developed resistance to rifampicin has a major impact on the treatment of PJI.

Der Einfluss der Unfallkinematik auf die Verletzungen der Halswirbelsaule (HWS) findet in den gangigen Klassifikationssystemen neben dem Kraftvektor aktuell keine Berucksichtigung. Inwieweit die Unfallstarke einen zusatzlichen Einfluss auf Zahl und Komplexitat der Verletzungen hat und ob sich dies klassifizieren lasst, ist noch nicht abschliesend untersucht. Inwieweit hat neben der Kraftrichtung auch die Kraftstarke Einfluss auf Verletzungen der HWS? Anhand der AOSpine-Klassifikationen fur die obere und subaxiale HWS erfolgte eine retrospektive Fallauswertung von 134 Traumapatienten. Analog der S3-Polytrauma-Leitlinie erfolgten eine Zuordnung in 6 Unfallgruppen und die Einteilung der Verletzungsmorphologie anhand der CT-Schnittbildgebung. Eine hohere Unfallenergie hat einen signifikanten Einfluss auf die Anzahl an Verletzungen (p = 0,005). Bei Niedrigrasanzunfallen ist C2 der am haufigsten verletzte Wirbelkorper (WK) (51 %; p = 0,022), bei Hochrasanzunfallen C7 (37 %; p = 0,017). Auserdem lies sich zeigen, dass die obere HWS bei steigendem Alter und niedriger Unfallenergie vermehrt bei weiblichen (z. B. Sturz aus dem Stand) und die untere HWS bei Hochrasanzunfallen bei jungeren mannlichen Patienten verletzt ist (p = 0,012). Eine genaue Kenntnis des Unfallmechanismus ist bei der Traumaversorgung hilfreich. Verletzungsmuster konnen besser abgeschatzt und die angemessene Diagnostik eingeleitet werden. Die Ergebnisse unterstreichen erneut die Wichtigkeit einer konsequenten HWS-Immobilisation auch bei Bagatelltraumata. Bei Hochrasanzunfallen ist die untere HWS haufiger verletzt, insbesondere C7. Aufgrund der Haufung mehrsegmentaler Verletzungen ist die radiologische Abklarung der gesamten WS nach Hochrasanztrauma zu empfehlen.

Simple Summary This up-to-date and in-depth review describes fibroblast-derived cells and their role within the tumor microenvironment for tumor progression. Moreover, targets for future antitumor therapies are summarized and potential aspects for future translational research are outlined. Furthermore, this review discusses the challenges and possible obstacles related to certain treatment targets. Abstract Cells within the tumor stroma are essential for tumor progression. In particular, cancer-associated fibroblasts (CAF) and CAF precursor cells (resident fibroblasts and mesenchymal stromal cells) are responsible for the formation of the extracellular matrix in tumor tissue. Consequently, CAFs directly and indirectly mediate inflammation, metastasis, immunomodulation, angiogenesis, and the development of tumor chemoresistance, which is orchestrated by complex intercellular cytokine-mediated crosstalk. CAFs represent a strategic target in antitumor therapy but their heterogeneity hinders effective treatment regimes. In-depth understanding of CAF subpopulations and knowledge of specific functions in tumor progression will ultimately result in more specific and effective cancer treatments. This review provides a detailed description of CAFs and CAF precursor cells and summarizes possible treatment strategies as well as molecular targets of these cells in antitumor therapies.

Operative treatment of ruptured pubic symphysis by plating is often accompanied by complications. Trans-obturator cable fixation might be a more reliable technique; however, have not yet been tested for stabilization of ruptured pubic symphysis. This study compares symphyseal trans-obturator cable fixation versus plating through biomechanical testing and evaluates safety in a cadaver experiment. APC type II injuries were generated in synthetic pelvic models and subsequently separated into three different groups. The anterior pelvic ring was fixed using a four-hole steel plate in Group A, a stainless steel cable in Group B, and a titan band in Group C. Biomechanical testing was conducted by a single-leg-stance model using a material testing machine under physiological load levels. A cadaver study was carried out to analyze the trans-obturator surgical approach. Peak-to-peak displacement, total displacement, plastic deformation and stiffness revealed a tendency for higher stability for trans-obturator cable/band fixation but no statistical difference to plating was detected. The cadaver study revealed a safe zone for cable passage with sufficient distance to the obturator canal. Trans-obturator cable fixation has the potential to become an alternative for symphyseal fixation with less complications.

Arthroscopic-assisted radial head and coronoid fracture fixation can be a valid alternative to the classic open reduction and internal fixation in selected fracture types. Arthroscopy improves visualization of the articular surfaces, permitting better understanding of the fracture’s morphology and more precise anatomical reduction. Nevertheless, compared to open surgery, some specific risks and pitfalls should be considered. This chapter provides a concise overview on the relevant advantages and disadvantages of each elbow arthroscopy portal in addressing radial head fractures with arthroscopic lag screw fixation and presents an innovative arthroscopic technique for antegrade direct reduction and retrograde fixation of coronoid process fractures through an accessory anteromedial portal.

Surgeons working in orthopedics and trauma surgery are frequently exposed to repetitive actions and non-ergonomic positions in their operative activities with the regular use of lead aprons. Musculoskeletal complaints of the neck and back among surgeons are reported in the literature as up to 80%. In this study, the effects of lead aprons on the posture of surgeons are examined using videorasterstereography, foot pressure measurement and questionnaires.All subjects (n = 31) were examined before and after exposure to wearing lead aprons during surgery using videorasterstereography and pedography. In addition, a survey with a separately created questionnaire and the Cornell Musculoskeletal Discomfort Questionnaire (CMDQ) was carried out.An average duration of lead apron use of 102.6 min showed an increase in forefoot load (p = 0.002) especially in the elderly subjects and thoracic kyphosis (p 0.001) especially in the younger doctors with a significant lateral deviation (p = 0.006). In addition, the lateral deviation was shown to correlate with an increasing body size or a shorter period of employment (p = 0.008; r = 0.51/p = 0.026; r = - 0.44). Significantly fewer surgeons experienced back complaints on working days without lead apron use in the operating room compared to days in the OR (p = 0.011).The impact of wearing front covered lead aprons during operations in the field of orthopaedics and trauma surgery leads to more frequent back complaints, even among young and healthy doctors. Under an average duration of surgery of 102 min a temporary postural deviation occurs that can be demonstrated by means of videorasterstereography and foot pressure measurement. The subjects showed a shifted weight distribution on the forefoot, a gain in thoracic kyphosis and an increase in lateral deviation, which also correlated with an increasing height and shorter length of employment.Orthopädisch-unfallchirurgisch tätige Ärzte sind in ihrer operativen Tätigkeit häufig repetitiven Bewegungen und unergonomischen Positionen unter regelmäßigem Einsatz von Röntgenschürzen ausgesetzt. Wie sich der Einfluss der Frontröntgenschürzen auf die Haltung der Ärzte auswirkt, wird in dieser Studie mittels Videorasterstereografie, Fußdruckmessung und Fragebögen untersucht.Alle Probanden (n = 31) wurden vor und nach Belastung durch das Tragen von Röntgenschürzen bei operativen Eingriffen mittels Videorasterstereografie und Pedografie untersucht. Zusätzlich erfolgte die Durchführung einer Fragebogenanalyse zur Prävalenz von Rückenbeschwerden u. a. mit dem Cornell Musculoskeletal Discomfort Questionnaire (CMDQ).Unter einer mittleren Belastungszeit von 102,6 min zeigt sich eine Zunahme der Vorfußbelastung (p = 0,002) vor allem bei älteren Probanden und der thorakalen Kyphose (p 0,001) vor allem bei den jüngeren Ärzten bei einer generellen Lotabweichung (p = 0,006). Signifikant weniger Probanden verspüren Rückenbeschwerden an Arbeitstagen ohne Einsatz im Operationssaal im Vergleich zu OP-Tagen (p = 0,011).Die Belastung durch das Tragen von Frontröntgenschürzen während operativer Eingriffe im Fach Orthopädie und Unfallchirurgie führt zu häufigeren Rückenbeschwerden auch bereits bei jungen gesunden Ärzten. So kann bei einer mittleren Operationsdauer von 102 min eine videorasterstereografisch und pedografisch nachweisbare temporäre Haltungsabweichung mit Mehrbelastung des Vorfußes, Lotabweichung und Zunahme der thorakalen Kyphose nachgewiesen werden, die zudem mit zunehmender Körpergröße und kürzerer Beschäftigungsdauer korrelieren.

Soft tissue sarcomas are a heterogeneous group of neoplasias that due to their often clinically silent appearance often remain undetected or experience delayed treatment. Especially soft tissue swelling is often misinterpreted by patients and doctors and trivialized or verified with an incorrect biopsy technique. The hereby evoked complications for the patients are serious and may be reduced by simply following the available guidelines. The treatment of soft tissue sarcomas requires a close interdisciplinary coordination between specialists in tumor orthopedics, oncology, radiology, pathology and radiotherapy. On the basis of a selective literature review, the following article points out the current evidence on the treatment and illustrates a treatment algorithm.Weichteilsarkome sind eine heterogene Gruppe von seltenen Neoplasien, die aufgrund ihrer anatomischen Lage und ihres oft inapparenten klinischen Erscheinungsbildes eine häufig zu spät diagnostizierte Tumorentität darstellen. Gerade weichteilige Schwellungen werden initial oft fehlinterpretiert und im Gegensatz zu den gängigen Behandlungsleitlinien bagatellisiert oder inkorrekt biopsiert. Die daraus resultierenden Komplikationen sind für den Patienten gravierend und können durch Befolgen gewisser Behandlungsgrundsätze minimiert werden. Die Behandlung der Weichteilsarkome kann nur im interdisziplinären Kontext unter Einbeziehung von chirurgischen Fächern, Onkologie, Pathologie und Strahlentherapie gelingen und sollte beginnend mit der Biopsie an Tumorzentren durchgeführt werden. Dieser Übersichtsartikel versucht, anhand einer selektiven Literaturrecherche eine Übersicht über die aktuelle Evidenzlage zur Behandlung dieser Tumorentität aufzuzeigen und einen Behandlungsalgorithmus darzustellen.

Targeted infiltrations to the lumbar spine are used to identify the correct scope for surgery in patients with chronic lumbar back pain (CLBP) and multi-segmental pathologies, where radiologic imaging and clinical presentation do not match. Conclusions from that method have so far been based on individual statements of pain relief. The main principle for the infiltrations is the assumption that by locally administering an analgesic agent to possible surgical target areas, the effect of surgery can be temporarily simulated. The aim was to investigate the use of rasterstereographic posture measurements to substantiate reported pain reduction after infiltrations.In this case-control study (Level II), rasterstereography was performed on 57 patients and 28 reference subjects to evaluate changes in posture during 1 week of injections under fluoroscopic guidance and correlated with changes in the Numeric Pain Rating Scale (NRS). Statistical analysis was performed with SPSS (The CLBP patients' trunk inclination (median 6°) and absolute lateral tilt were significantly increased (p .01 each) compared to the reference group. No significant difference was observed for trunk length, kyphotic/lordotic angle, or lateral deviation. During infiltration, no significant difference in posture could be observed. A subgroup analysis of patients reporting the highest pain improvement indicated no significant difference. No correlation was observed with pain improvement reported by the patients during injections. Subsequent rasterstereography does not seem to be able to verify results of injections in the diagnostics of lumbar spine pathologies, as patients' reported pain relief does not correlate with a relevant alteration in posture. The short-term effect of the targeted infiltrations may not suffice to change false posture being developed over years.Gezielte Infiltrationen der Lendenwirbelsäule im Rahmen einer Stufendiagnostik werden verwendet, um den Operationsumfang bei Patienten mit chronischen Rückenschmerzen (CLBP) und multisegmentalen Pathologien zu ermitteln, bei denen radiologische Bildgebung und klinische Darstellung nicht vollends übereinstimmen. Schlussfolgerungen aus dieser Methode basierten bisher auf individuellen Aussagen zur Schmerzlinderung. Das Hauptprinzip ist hierbei die Annahme, dass durch die lokale Verabreichung eines Analgetikums, an mögliche chirurgische Zielbereiche, der Effekt einer Operation vorübergehend simuliert werden kann. Studienziel war es, die rasterstereografische Haltungsmessungen zu untersuchen, um die patientenseitig angegebene Schmerzreduktion zu objektivieren.In dieser Fall-Kontroll-Studie wurde bei 57 Patienten und 28 Referenzpersonen eine Rasterstereografie durchgeführt, um Veränderungen der Körperhaltung während einer stationären Stufendiagnostik über den Zeitraum einer Woche zu bewerten und mit Veränderungen der numerischen Schmerzbewertungsskala (NRS) zu korrelieren. Die statistische Analyse wurde mit SPSS (α = 0,05, zweiseitig) durchgeführt und in Form von Box-, Scatter- und Bland-Altman-Diagrammen dargestellt.Die Rumpfneigung der CLBP-Patienten (Median 6°) und die absolute Seitenneigung waren im Vergleich zur Referenzgruppe signifikant erhöht (jeweils p 0,01). Für Rumpflänge, kyphotischen/lordotischen Winkel oder laterale Abweichung wurde kein signifikanter Unterschied beobachtet. Während der Infiltration konnten keine signifikanten Haltungsunterschiede festgestellt werden. Eine Subgruppenanalyse der Patienten mit der höchsten Schmerzverbesserung ergab keinen signifikanten Unterschied. Es wurde keine Korrelation mit der von den Patienten während der Injektionen berichteten Schmerzverbesserung beobachtet.Die Rasterstereographie scheint demnach nicht geeignet zu sein, die Ergebnisse im Rahmen der Stufendiagnostik zu objektivieren, da die berichtete Schmerzlinderung der Patienten nicht mit relevanten Änderungen der Körperhaltung korreliert. Der kurzfristige Effekt der gezielten Infiltration reicht möglicherweise nicht aus, um über Jahre hinweg entwickelte Fehlhaltungen zu ändern.

The rising number of medical publications makes it difficult to keep up-to-date on scientific knowledge. In recent years, reviews in the form of narrative or systematic publications and meta-analyses have increased. These can only be interpreted and evaluated if the reader understands the techniques used. This review article describes the differences between narrative and systematic reviews, together with the characteristics of meta-analysis, and discusses their interpretation. The concept of systematic reviews and meta-analysis includes a systematic literature search and summary, together with an appraisal of the quality of the publications. Systematic reviews are often considered to be original studies due to their structure and ability to reduce bias.

The rise in the number of patients with lumbar back pain has led to an increase in the number of spinal surgeries. To avoid unfavorable outcomes, high accuracy and reliability of indication for surgery are essential. This requires critical evaluation of postoperative outcomes with its two key dimensions pain and function. While imaging findings give details about the technical dimension of the intervention, they are prone to high inter-/intra-observer variability, with limited relation to functional outcomes. Pain improvement can be directly asked from patients or documented by questionnaires. There is abundant literature on postoperative function based on questionnaires, but quantifiable data such as gait or posture analysis are scarce. Highprecision measurement tools are available and easy to implement in a clinician's work routine.This study evaluates whether lumbar fusion surgery changes gait and postural variables and how these changes are related to patients' descriptions of alterations in their levels of pain.Back profiles and gait analyses were measured by video rasterstereography and treadmill gait analysis. Measurements were recorded before surgery, at discharge, after 3 months in a longitudinal (n = 30), and after 12 months in a cross-sectional group (n = 29). A reference group was formed (n = 28). The improvement on the Numeric Pain Rating Scale was documented and compared with changes in gait and posture.A significant reduction in kyphotic (52-43°, p = 0.014) and lordotic (28-11°, p 0.001) angles was observed. The values again increased after 3 months, with a significant reduction in cadence (98-91 steps/min, p = 0.006). While improvements in pain were also obtained by surgery (p 0.001), no clear correlation could be detected between 3-month alleviation in pain and changes in kyphotic/lordotic angle or cadence.Although both methods offer high-precision measurement, changes in gait and posture were not related with the patients' reported pain relief after lumbar fusion surgery.

Wirbelkorperfrakturen (WKF) konnen durch verschiedene Unfallmechanismen entstehen. Nach dem Grad der Instabilitat erfolgt bei der AOSpine-Klassifikation die Einteilung in 3 Hauptfrakturtypen. Inwieweit neben der Kraftrichtung auch die Kraftstarke einen Einfluss auf die Zahl und Komplexitat der Frakturen hat, ist bisher noch nicht abschliesend untersucht worden. Welchen Einfluss haben Unfallereignisse unterschiedlicher Kinematik auf die Lokalisation, Komplexitat und Anzahl an WKF der Brust(BWS)- und der Lendenwirbelsaule (LWS)? In einer retrospektiven Studie wurden Daten von Patienten mit frischen traumatischen WKF analysiert. Je nach Unfallmechanismus wurden die Patienten in 6 Unfallgruppen (UFG) eingeteilt und die WKF anhand der CT-Bildgebung nach der AOSpine-Klassifikation beurteilt. Es wurde 2‑seitig getestet und ein Signifikanzniveau von 5 % zugrunde gelegt. Fur die Durchfuhrung der statistischen Berechnungen wurde IBM SPSS Statistics eingesetzt. Bei den Hochrasanztraumata (UFG III und V) konnte eine signifikante Zunahme der Frakturschwere nach der AOSpine-Klassifikation festgestellt werden. Zudem stieg die Haufigkeit thorakaler (TH) WKF fur TH7 (p = 0,011) und TH10 (p = 0,001) signifikant an. Im Vergleich zu den anderen UFG war das Risiko einer TH7-Fraktur bei Hochrasanzunfallen 7‑fach erhoht („odds ratio“ [OR]: 7,0; 95 %-Konfidenzintervall [95 %-KI]: 1,4–35,2). Die UFG III (Sturz > 3 m) bot mit 2,5 WKF im Median (SD ± 1,84) die hochste Frakturanzahl. Eine exakte Wiedergabe des Unfallhergangs ermoglicht die Einteilung in hoch- bzw. niedrigenergetische UFG. Bei vorherigen Untersuchungen wurden Verkehrsunfalle (VU) als eine Gruppe erfasst; ein Einfluss der zunehmenden kinematischen Energie konnte daher nicht beurteilt werden. Durch Berucksichtigung von Hoch- und Niedrigrasanztrauma kann der Unfallkinematik Rechnung getragen werden. Als „Risikowirbel“ bei Hochrasanzunfallen stellten sich TH7 und TH10 heraus. Neben der Kraftrichtung hat somit auch die Kraftstarke einen masgeblichen Einfluss auf das Verteilungsmuster von WKF.

Zusammenfassung Hintergrund Die Indikationsstellung zur Operation bezieht ihre Legitimation zum Eingriff in die körperliche Unversehrtheit eines Patienten im Wesentlichen aus dem zu erwartenden Behandlungserfolg. Um die Wahrscheinlichkeit und das Ausmaß eines Operationserfolgs bereits präoperativ abzuschätzen, muss der Chirurg in kurzer Zeit eine Fülle an Parametern evaluieren, die über die rein technische Dimension des Eingriffs hinausgehen. Trotz höchster Versorgungsstandards zeigt die postoperative patientenseitige Zufriedenheit eine immense Streubreite bei für den Arzt technisch zufriedenstellendem Ergebnis. Ziel der folgenden Studie war es, zu untersuchen, mit welcher Präzision individuelle postoperative Zufriedenheit und Schmerzangabe von Patienten bei elektiven muskuloskeletalen Eingriffen vorhergesagt werden können, und ob diese Vorhersagepräzision abhängig ist vom ärztlichen Ausbildungsstand. Zudem sollte evaluiert werden, ob bei einem hoch standardisierten Eingriff, wie dem endoprothetischen Gelenkersatz, eine höhere Präzision möglich ist als im restlichen Operationsgut. Material und Methoden Die Ärzte der Klinik gaben präoperativ eine Einschätzung über die erwartete Zufriedenheit (Skala 0 – 10) und Schmerzstärke (NRS) der Patienten für den Zeitpunkt 6 Monate postoperativ ab. Zu diesem Zeitpunkt erfolgte dann die Erhebung dieser Parameter bei den Patienten. Die ärztliche Beurteilung erfolgte im Rahmen der präoperativen Visite am Vortag zur Operation. Unterschiedlich erfahrene Kollegen konnten so zeitgleich dieselben Patienten einschätzen. Verglichen wurden die Angaben von Ober- und Fachärzten mit denen der Assistenzärzte in Gegenüberstellung zu den Angaben der Patienten. Ergebnisse Insgesamt wurden 194 Einschätzungen ausgewertet. Durch die Operation konnte eine deutliche Schmerzbesserung erzielt werden (Median NRS prä-OP 6,25, post-OP 2,5, p Schlussfolgerung Auch wenn sich die ärztliche Einschätzung insgesamt mit den Patientenangaben annähernd deckt, so sind auf den Einzelfall bezogen starke Abweichungen zu verzeichnen, die auch in einem hoch standardisierten Kollektiv, wie dem der primären Endoprothetik, bestehen. Eine Zunahme der Einschätzungsqualität mit fortschreitendem ärztlichem Ausbildungsstand war nicht festzustellen. Hier lassen die Daten die Vermutung zu, dass über die technische Dimension hinaus andere Faktoren ebenfalls von entscheidender Bedeutung sind, wie z. B. die präoperative Erwartungshaltung des Patienten. Anspruch eines jeden Operateurs muss es daher sein, eine realistische Erwartung an die Operation mit dem Patienten gemeinsam zu entwickeln und sich dabei der eigenen Grenzen bez. der Vorhersagepräzision des Erfolgs bewusst zu sein.

Background Prosthetic joint infections (PJI) are a disastrous and feared complication in arthroplasty. Over the past decades, surgeons have tried to lower infection rates through all sorts of improvements. At present, it is impossible to reduce the risk to zero. As the contamination of surgical instruments and the surgical field has been identified as a remaining gap prone for infection, scalpel blades among others have become a focal point of several studies. This study is the first to compare the effect of adhesive incision drapes on contamination rates of scalpel blades in primary arthroplasty of the hip and knee. Methods A total of 344 microbiologic blade culture results from 2 study groups with and without drape usage and 1 group with known PJI were analyzed and compared to histopathologic and microbiologic tissue results. Results In 78% of all positive cultures, the bacteria were part of the local skin flora. The contamination rate for the skin blades with a drape was 3.81% vs 12.19% without drape usage. A significant difference was determined between positive skin blade culture results incision drape usage (P = .031). The different pathogens likely responsible for the PJI correlated with the deep blade cultures (P ≤ .01). The overall contamination rate of surgical blades (1.37%) is comparatively low to the results obtained from other surgical equipment in literature. None of those blade-positive patients developed a surgical site infection during 12-month follow-up. Conclusion Iodine-impregnated incision drapes are able to reduce surgical skin blade contamination. Further studies may be needed to assess the relationship between blade contamination and surgical site infections.

Material und Methoden Retrospektive Auswertung von 64 Patienten über einen Untersuchungszeitraum von 24 Monaten. Ergebnisse Es waren 56 Patienten männlich, 8 weiblich und das mittlere Alter betrug 44 Jahre (SD ± 17). Unfälle im Straßenverkehr waren der führende Grund für die Beckenverletzung. Die Verteilung nach AO-Klassifikation zeigte sich wie folgt: 14-mal B1-, 10-mal B2-, 5‑mal B3-, 23-mal C1-, 9‑mal C2- und 3‑mal C3-Verletzungen. Die Verteilung nach Young und Burgess ergab: 9‑mal APC-I-, 18-mal APC-II-, 13-mal APC-III-, 9‑mal LC-I-, 3‑mal LC-II-, 2‑mal LC-III- und 10-mal VS-Verletzungen. Material and methods A retrospective evaluation of 64 patients who underwent symphyseal plating over a period of 24 months was conducted. Results Of the patients 56 were male and 8 female with a mean age of 44 years (SD ± 17 years). The main cause of the pelvic injuries were traffic accidents. Conclusion These observations suggest that radiological signs of implant loosening are commonly observed but rarely the reason for revision surgery. Complete implant failures, however; occur mainly within the first postoperative weeks and require early revision. A timely clarification by additional X-ray imaging should be carried out if this is suspected.

Variations in the temperature of body and skin are symptoms of many pathological changes. Although joint replacement surgery of hip and knee has been very successful in recent decades, periprosthetic infection is a growing problem and the number one reason for revision. While many studies have investigated changes in blood levels, investigation of temperature has not been performed on a regular basis. The objective of this work is to determine whether reference literature exists for the infrared thermographic examination in knee and hip arthroplasty and if reference values can be derived for the methodology or if there is a peri- and postoperative benefit.By means of a systematic online database search and based on the Cochrane, PICOT and PRISMA guidelines, this systematic review retrieved 254 studies. All publications with thermographic examination in arthroplasty of the hip and knee were imbedded. 249 studies were excluded due to the defined inclusion and exclusion criteria and five studies with 251 patients have finally been included in the evaluation process. This was followed by an analysis and discussion of the methodology.Infrared thermography is a useful tool in the perioperative care of patients after arthroplasty of the knee and hip joint. The technology is portable, easy to use and non-invasive. This protocol provides guidance on how to perform high resolution video rasterstereography and treadmill gait analysis on patients after lumbar fusion surgery to obtain results about altered variables of gait and posture. These observed changes can then be correlated with the patient-reported outcome measure of pain relief. The rasterstereographic device projects lines of parallel light onto the surface of the tested subject's back. The deformation of these lines is recognized by the device. From these data, a special software then generates a 3-D profile based on the principle of triangulation. With an inaccuracy of only 0.2 mm it can measure changes in posture at very high precision. Gait and stance parameters are recorded using a treadmill equipped with an electric sensor mat that contains 10,200 miniature force sensors in the registering zone under the belt. Initial walking speed on the treadmill is 0.5 km/h. Speed is then gradually increased by increments of 0.1 km/h until each subject reaches his or her individual maximum well tolerable walking speed. At this speed, parameters are recorded during a 20 s measurement interval. Subjects are tested barefoot and without holding a handrail. Among various other parameters, stride width, step length, stance phase and foot rotation are measured. Both methods used reportedly have a high intra- and inter-observer reliability. The advantage of these highly accurate techniques is that they offer an objective and very detailed perspective on changes in the patient's posture and gait. Due to the amount of data generated, these techniques are, however, not so much suitable for everyday routine use, but rather interesting to scientifically evaluate long term alterations in posture and gait in patients like for example after lumbar fusion surgery.

Objectives Percutaneous radiofrequency is an established method for treatment of chronic low back pain of intervertebral facet etiology. Endoscopic techniques have the advantage of visualization of the facet joint and the dorsal medial ramus and thus allow for more accurate denervation. Patients with chronic lumbar back pain (CLBP) often present with an altered gait profile, which is a crucial element of good functioning in everyday life. In patients with multisegmental spinal pathologies and incongruity between radiologic imaging and clinical presentation, inpatient gradual diagnostics (IGD) is performed to determine the precise origin of the disabling pain. The underlying principle of IGD is the assumption that by locally administering an analgesic and anti-inflammatory agent to possible surgical target areas, the surgical effect can be temporarily simulated. The conclusions drawn from IGD are, however, mostly based on the patients' subjective feedback about pain relief.The aim of this study was to evaluate whether reported pain relief during IGD can be objectified by gait analysis. We hypothesized that patients with greater pain relief during IGD would show greater improvement in their pathologic gait and stance.Treadmill gait and stance analyses were prospectively performed on CLBP patients before and after a one-week IGD. Self-report measures included the numeric pain rating scale (NRS) and the Oswestry Disability Index (ODI).Compared with a reference group (n = 28), IGD patients (n = 57) at admission showed reduced velocity, cadence, step length, and swing phase (p .01 each). Their stance phase was increased by 5% of the gait cycle, and a more asymmetrical total load distribution during stance was observed. No difference was seen in stride width or foot rotation. While many patients reported good pain relief during IGD, no correlation was observed between subjective improvement and treadmill measures. We can thus confirm a pathologic gait profile in patients with CLBP. Based on our findings, gait analysis would not yet seem suitable to objectify IGD results. The short time interval between admission and discharge may not suffice to change a pathological gait that has developed over years.

Microcirculatory integrity and proper function are the cornerstones to tissue nourishment and viability. In the clinical environment extended immobility, injuries, and inflammatory reactions demand local microcirculatory adaption to provide adequate supply. Assessment of endothelial adjustment capability and microcirculatory perfusion status, as direct or surrogate markers of disease, are therefore of uttermost interest to the treating physician. Given the simple, noninvasive, nonradiating nature of laser-based techniques for bedside or intraoperative microcirculatory perfusion assessment, this article's objective is to present a comprehensive overview of available techniques, their technological aspects, and current application. Advantages of individual methods are pointed out and compared with each other. The areas of medical utilization relevant to orthopedics and trauma surgery are exemplified and their available evidence elaborated. A particular focus is put on laser speckle contrast imaging, with its current and future influence on medical practice.

In recent years, reviews in the form of narrative or systematic publications and meta-analyses have increased. These can only be interpreted and evaluated if the reader understands the techniques used. This review article describes the differences between narrative and systematic reviews, together with the characteristics of meta-analysis, and discusses their interpretation. The concept of systematic reviews and meta-analysis includes a systematic literature search and summary, together with an appraisal of the quality of the publications. Ein Verständnis der angewendeten Techniken dieser Publikationen ist für ihr Verständnis und die Qualitätsbewertung unerlässlich. Diese Arbeit vermittelt die Unterschiede von narrativen und systemischen Übersichtsarbeiten, weist auf die Besonderheiten von Metaanalysen hin und hilft bei deren Interpretation. Das Konzept systematischer Reviews und Metaanalysen beinhaltet die systematische Literaturrecherche und -zusammenfassung sowie deren Qualitätsbeurteilung. So können systematisch aufgebaute Übersichtsarbeiten aufgrund ihres Aufbaus als eigenständige Studien angesehen werden und Fehlerquellen (Bias) reduzieren.

Three-dimensional (3-D) endoscopic optics use 2 cameras to simulate the different perspectives of the right and left eye, creating the illusion of spatial depth. Optimised orientation as well as improved hand-eye coordination compared to 2-D-optics could be proven in standardised test setups (black box) and in laparoscopic use. This retrospective study examines whether these results can also be applied to thoracoscopic vertebral body replacement at the thoracolumbar junction.1. Ventral vertebral body replacement using 3-D-thoracoscopy results in a shorter operation time than with 2-D-thoracoscopy. 2. Between 08 - 2012 and 08 - 2017, all of these received ventral thoracoscopic vertebral replacement at the thoracolumbar junction (Th11 to L2). Patients with additional anterior procedures (e.g. anterolateral plate) were excluded. Perioperative data such as blood loss, duration of surgery and length of hospital stay were analysed. Vertebral body fractures (VBF) can be caused by various trauma mechanisms. The AOSpine classification system differentiates three main types of fractures according to the grade of instability. How the increasing energy of various accident mechanisms changes the complexity of the individual fracture, its localization and the occurrence of further fractures has not yet been finally investigated.What influence do traumatic events with different kinematics have on the localization, complexity and number of VBF in the thoracic and lumbar spine?In this retrospective study data from patients with a freshly traumatized VBF were analyzed. The patients were divided into six trauma groups (UFG) depending on the trauma mechanism. In comparison to the other low energy trauma groups, the risk of a TH7 fracture was 7‑times higher after a high energy trauma (odds ratio OR = 7.0; 95% confidence interval CI = 1.4; 35.2). The UFG III (falls 3 m) showed the highest number of fractures with a median of 2.5 (SD 1.84) VBF.An exact reproduction of the traumatic event enables a distinction between high and low energy trauma groups to be made. In previous studies traffic accidents were recorded as one group, so an influence of the increasing kinematic energy could not be assessed. The accident kinematics can be taken into account by differentiating between high and low-energy trauma groups. In high-energy accidents the TH7 and TH10 vertebrae were found to be at risk vertebrae. In addition to the force direction, the force strength also has a decisive influence on the distribution pattern of VBF.

The stabilization of an atlantoaxial (C1-C2) instability is demanding due to a complex atlantoaxial anatomy with proximity to the spinal cord, a variable run of the vertebral artery (VA) and narrow C2 pedicles. We perfomed the GoelHarms fusion in combination with an intraoperative 3D imaging to ensure correct screw placement in the C2 pedicle. We hypothesized, that narrow C2 pedicles lead to a higher malposition rate of screws by perforation of the pedicle wall. The purpose of this study was to describe a certain pedicle size, under which the perforation rate rises.In this retrospective study, all patients (n=30) were operated in the GoelHarms technique. The isthmus height and pedicle diameter of C2 were measured. The achieved screw position in C2 was evaluated according to GertzbeinRobbin classification (GRGr).A statistically significant correlation was found between the pedicles size (isthmus height/pedicle diameter) and the achieved GRGr for the right (p=0.002/p=0.03) and left side (p=0.018/p=0.008). The ROC analysis yielded a Cut Off value for the pedicle size to distinguish between an intact or perforated pedicle wall (GRGr 1 or ≥2). The Cut-Off value was identified for the isthmus height (right 6.1mm, left 5.4mm) and for the pedicle diameter (6.6mm both sides).The hypothesis, that narrow pedicles lead to a higher perforation rate of the pedicle wall, can be accepted. Pedicles of6.6mm turned out to be a risk factor for a perforation of the pedicle wall (GRGr 2 or higher). Intraoperative 3D imaging is a feasible tool to confirm optimal screw position, which becomes even more important in cases with thin pedicles. The rising risk of VA injury in these cases support the additional use of navigation.
