Purpose The effect of preoperative embolization of bone metastases prior to stabilization procedures in reducing intraoperative blood loss remains controversial. This study aimed to analyze the effect of preoperative embolization on orthopedic stabilization procedures of the extremities and spine in cases with bone metastases from renal cell carcinomas. In particular, do these patients suffer less blood loss during the operation and do they need lesser fluid replacements or packed red cell bags intra- and perioperatively? Does preoperative embolization reduce the duration of surgery? Methods We retrospectively reviewed stabilization procedures of the spine and extremities at our institution between 2011 and 2021 for group differences (embolization vs. no embolization) in terms of blood loss, fluid substitution, need for packed red cell transfusions, tumor size, and duration of surgery. Results We reviewed 79 stabilization procedures of the spine (n = 36) and extremities (n = 43), of which 30 included preoperative embolization procedures. Surprisingly, the embolization group showed a statistically significant increase in blood loss, the need for fluid substitution, and red cell transfusions. Subgroup analysis revealed a significant negative effect of preoperative embolization on stabilization procedures of the extremities. Conclusion Based on our data, preoperative embolization of renal cell carcinoma metastases of the extremities had a negative effect on intraoperative blood loss and the need for fluid substitution and should therefore be avoided. Our data did not show an effect on stabilization procedures of the spine.

Purpose Pathologic fractures of the extremities due to carcinoma metastases require individual and patient prognosis-related stabilization procedures. Quick remobilization of the patient to restore the quality of life is of high importance, especially in the case of subtrochanteric and diaphyseal femoral fractures. In our retrospective cohort study, we evaluated intraoperative blood loss, length of operation, complication rate, and regain of lower extremity function in plate compound osteosynthesis (PCO) versus intramedullary nailing (IM) for subtrochanteric and diaphyseal pathologic fractures of the femur. Methods Between January 2010 and July 2021, we retrospectively reviewed 49 patients who were treated at our institution for pathologic fractures of the subtrochanteric and diaphyseal femurs for group differences in terms of blood loss, length of operation, implant survival, and Musculoskeletal Tumor Society (MSTS) score. Results We included 49 stabilization procedures of the lower extremity due to pathologic fractures of the proximal or diaphyseal femur, with a mean follow-up of 17.7 months. IM (n = 29) had a significantly shorter operation time than PCO (n = 20) (112.4 ± 9.4 and 163.3 ± 15.96 min, respectively). We did not detect any significant differences in terms of blood loss, complication rate, implant survival, or MSTS score. Conclusion Based on our data, pathologic subtrochanteric and diaphyseal fractures of the femur can be stabilized with IM, which has a shorter operation time than PCO, but the complication rate, implant survival, and blood loss remain unaffected.

Patients with severe haemophilia suffer from bleeding-related joint changes in which the ankle joint is most frequently affected. In the resulting gait changes, the forefoot is involved by reducing the foot pressure. However, it is unclear which changes in foot pressure are present in the individual's foot zones.The aim of the study was to determine whether compensation mechanisms are present in the foot zones regarding the peak pressure under dynamic conditions and to identify possible underlying mechanisms for gait changes.In a controlled cross-sectional study, a pedobarography was performed during gait with a standardized speed (3 km/h) in patients with haemophilia (PwH;n = 40) and healthy controls (Con;n = 40). Pressure pain thresholds (PPT) were detected, and Haemophilia Joint Health Score (HJHS) was performed to determine the current joint status.PwH showed a decreased peak pressure in metatarsals II-IV and heel compared to Con. Patients with major-affected ankle joints (determined with the HJHS) showed a decreased single-step length, stride-length and stride-time. Accordingly, the cadence was increased by 10 ± 11 steps/min in PwH compared to Con. Furthermore, PwH showed decreased ankle range of motion (ROM) in HJHS and an altered pain perception due to reduced PPT.PwH showed a changed gait pattern in peak pressure compared to Con. A restricted rolling behavior, which might be caused by movement restrictions and pain sensation, leads to reduced pressure in the center forefoot, resulting in a shorter stride-length. Future therapies should focus on maintaining joint mobility for better rolling behavior and improving ankle joints' stability to achieve a balanced load between the midfoot, heel, and forefoot. The use of insoles adapted to our data, based on group differences between PwH and Con, could be supportive in this case.

Introduction Patients with hemophilia (PWHs) suffer from an increased risk of osteoporosis. Multiple hemophilia and hemophilic arthropathy associated factors correlate with a low bone mineral density (BMD) in PWHs. The aim of this study was to assess the long-term development of BMD in PWH as well as to analyze potentially influencing factors. Methods A total of 33 adult PWHs were evaluated in a retrospective study. General medical history, specific-hemophilia-associated comorbidities, joint status using the Gilbert score, calcium level, and vitamin D level as well as at least two results of bone density measurements with a minimum range of 10 years per patient were taken into account. Results The BMD did not change significantly from one point of measurement to the other. A total of 7 (21.2%) cases of osteoporosis and 16 (48.5%) cases of osteopenia were identified. The two following significant correlations could be revealed: the higher the patients' body mass index, the higher their BMD (r = 0.41; p = 0.022). Moreover, a high Gilbert score came along with a low BMD (r = −0.546; p = 0.003). Conclusion Even if PWHs frequently suffer from a reduced BMD, our data suggest that their BMD remains constant on a low level in the course of time. A risk factor of osteoporosis often found in PWHs is a vitamin D deficiency and joint destruction. Therefore, a standardized screening of PWHs on BMD reduction by collecting vitamin D blood level and assessing joint status seems appropriate.

Introduction Acetabular defect recognition and classification remains a challenging field of practice for orthopedic surgeons. Recently, the Acetabular Defect Classification (ADC) has been introduced to provide a reliable, reproducible and intuitive classification system. In order to improve ease of use and efficiency of the ADC, a browser-based application has been created. We hypothesized that the ADC application can improve rating performance of non-specialists (medical students) to achieve good inter- and intra-rater agreement and will compare favorable to the results of specialists (experienced surgeons) without the help of the application. Materials and methods The ADC is based on the integrity of the acetabular rim and the supporting structures. It consists of four main types of defects ascending in severity. These defects are further subdivided in A–C, narrowing down defect location. 80 randomized radiographs were graded according to ADC by three non-specialists (medical students) with help of the ADC application and by three specialists (orthopedic surgeons) without help of the application to evaluate the difference in inter-rater agreement between groups. To account for intra-rater agreement, the rating process was repeated after a reasonable wash-out period. Results Inter-rater and intra-rater agreement within the non-specialist group rated lower when compared to the specialist group while still falling into the good agreement range. The student group presented with k values of 0.61 for inter-rater agreement and 0.68 for intra-rater agreement, while the surgeon group displayed k values of 0.72 for inter-rater agreement and 0.83 for intra-rater agreement. Conclusion The app-guided assessment of acetabular defects offers a promising innovative approach to simplify complex situations. It makes the challenging field of acetabular revision arthroplasty more approachable especially for less experienced surgeons and offers insight and guidance in the planning stage as well as intra-operative setting.

Background Major demographical changes in Germany commenced in the 1960s. Ongoing humanitarian crises in the Ukraine with subsequent immigration will have also long-range effects on national provision of cancer treatment. Ensuring the best possible outcomes for each cancer patient undergoing radiotherapy requires the prediction and prevention of unfavorable side effects. Given that recent research has primarily focused on clinical outcome indicators solely, less is known regarding sociodemographic predictors of therapeutic outcomes, such as patient nationality. Here, we investigated whether the severity of early side effects after radiotherapy are associated with patient nationality and other sociodemographic and clinical characteristics. Methods Out of 9187 patients treated at a German university medical center between 2017 and 2021, 178 German and 178 non-German patients were selected for matched-pair analysis based on diagnostic and demographic criteria. For all 356 patients, data on side effects from follow-up care after radiotherapy were collected. Results Non-German patients were more likely to have severe side effects than German patients. Side effect severity was also associated with tumor entity, concomitant therapy, body mass index, and age. Conclusion Foreign cancer patients are at higher risk of experiencing severe side effects of radiotherapy, suggesting a need to develop and implement targeted preventive measures for these patients. Further research investigating factors predicting the occurrence of radiotherapy side effects, including other sociodemographic characteristics, is needed to better personalize therapy regimens for cancer.

Background Over the years, radiotherapy has been established as a tool to improve local control for high-grade sarcomas. Although the European Society for Medical Oncology guidelines has taken notice of a shift toward a neoadjuvant radiotherapy approach, the American Society for Radiation Oncology guidelines clearly favor a neoadjuvant approach, citing debilitating long-term adverse effects when radiotherapy is applied postoperatively. In this study, we examined these irradiation-associated adverse events for adjuvant radiotherapy and focused on the prognostic factors for disease outcome, including local control. Methods In this retrospective study, data for 106 patients with extremity soft-tissue sarcomas diagnosed between 1997 and 2021, of which 40 received adjuvant radiotherapy, were collected from the clinical and radiological information systems of a high-volume sarcoma treatment center. These data were then analyzed for radiation-associated side effects as well as predictive factors for overall survival, disease-free survival, local control, and surgical complications. Results Radiotherapy was beneficial to patients improving local control, especially for high-grade sarcomas, even when those were resected with negative margins. Side effects due to radiotherapy occurred in 87.5% of the patients, and these effects primarily included radiation dermatitis in 67.5%; however, only 40.0% had any adverse event of ≥ grade 2 according to Common Terminology Criteria for Adverse Events. Long-term function-limiting side effects occurred in 45.0% of the patients; 10% exhibited ≥ grade 2 function-limiting adverse events. Greater time between surgery and adjuvant radiotherapy was beneficial for the patients, whereas joint infiltrating sarcomas were associated with more severe long term, function-limiting adverse events. 28.3% of the patients experienced a recurrence at any location (median time 18.35 months) and in 16% the recurrence was local (median time 16.11 months), resulting in 1, 3, and 5 year disease-free survival rates of 74.1, 58.9, and 38.5% and local control rates of 78.7, 61.6, and 42.8% were observed, respectively. Conclusion Recurrences may be avoided with high-dose radiation, especially for high-grade G2 and G3 sarcomas, even after complete R0 resection. This resulted in a low rate of severe long-term function-limiting adverse events. Thus, adjuvant radiotherapy should be seriously considered when planning patient treatment, especially when treating patients that present with high-grade sarcomas.

Verschiedene Grunderkrankungen konnen bei Kindern und Erwachsenen zu einer kontrakten Spitzfusfehlstellung fuhren. Liegt eine strukturelle Verkurzung des M. gastrocnemius und des M. soleus vor, so stehen verschiedene operative Verfahren zur Korrektur dieser Fehlstellung zur Verfugung. Ein zur Wiederherstellung einer regelrechten Dorsalextension des OSGs favorisiertes und sicheres Verfahren ist die perkutane Achillotenotomie nach Hoke. Eine Berucksichtigung der physiologisch anatomischen Torsion der Achillessehne wie sie der Technik nach White entspricht und von einigen Autoren empfohlen wird, zeigt unserer Erfahrung nach wie auch im Kadavermodel keine Vorteile. Wir zeigen in der vorliegenden Arbeit eine modifizierte ebenfalls minimal-invasive Form dieser Operationsmethode mit der eine sofortige Vollbelastung der betroffenen unteren Extremitat ab dem ersten postoperativen Tag moglich ist. Alle klinisch relevanten strukturellen Achillessehnenverkurzungen des M. gastrocnemius und des M. soleus, zur primaren und/oder zur Rezidivbehandlung. Infektion im Bereich des Operationsgebietes Keine Blutsperre, 3 Stichinzisionen mit dem 15er-Messer: 1. medial distal am Ubergang der Achillessehne zum Kalkaneus, 2. medial proximal ca. 7 cm proximal der 1. Stichinzision, 3. lateral, mittig zwischen den beiden ersten Stichinzisionen; keine Hautnaht, Anlage eines Unterschenkelrundgipses in Narkose Am ersten postoperativen Tag Gipshybridisierung mittels Scotchcast (3M Deutschland GmbH, Neuss, Deutschland), anschliesend schmerzadaptierte Vollbelastung im Gips, Entfernung des Unterschenkelgipses ambulant nach 4 Wochen. Es wurden 104 Patienten mit dieser Methode operiert, 1 Fall einer lokalen Druckstelle, keine Infektionen, keine Uberkorrekturen, keine Achillessehnenrupturen, ein postoperatives Rezidiv aufgrund eines gebrochenen Gipses. Die in der Literatur als Hauptkomplikation erachtete Gefahr einer Uberkorrektur zum Hackenfus trat in keinem der Falle auf.

In Middle Europe developmental dysplasia of the hip (DDH) has an incidence of up to 5.9 %. The rate of congenital hip dislocation as the worst complication of a growth disorder of the hip is between 1.5 % and 2.5 %. Among known risk factors of DDH are breech position, multiples, foot deformities and family history. The aim of this retrospective study was to investigate prematurity as a risk factor for developmental dysplasia of the hip (DDH). Die kongenitale Hüftluxation als schwerste Komplikation liegt zwischen 1,5 und 2,5 %. Zu den akzeptierten Risikofaktoren der Hüftdysplasie werden die Beckenendlage, Mehrlingsgeburten, Fußdeformitäten und familiäre Häufung gezählt. Das Ziel dieser retrospektiven Datenerhebung ist die Untersuchung der Frühgeburtlichkeit als Risikofaktor für das Vorliegen einer Hüftdysplasie. Daher wurden die Hüften von 283 Neugeborenen, welche vor der 38. Schwangerschaftswoche oder früher, und jene von 377 Neugeborenen, welche nach der 37. Schwangerschaftswoche geboren wurden, mit der Hüftultraschallmethode nach Graf et al. in der ersten Woche postpartal untersucht. Überraschenderweise war der Unterschied der Alpha-Winkel zwischen den beiden Gruppen äußerst signifikant zugunsten der Frühgeborenen. Darüber hinaus konnten wir eine physiologische Entwicklungskurve mit einem Höchstwert nach der 31. SSW darstellen. Die Inzidenz der Hüftdysplasie war nicht statistisch signifikant unterschiedlich in beiden Gruppen. Unsere Daten deuten darauf hin, dass die Frühgeburtlichkeit kein Risikofaktur für die Hüftdysplasie darstellt, sondern wahrscheinlich protektiv auf die Hüftentwicklung wirkt.

Die Inzidenz der periprothetischen Frakturen des proximalen Femurs steigt aufgrund der wachsenden Anzahl an Hufttotalendoprothesenimplantationen im hoheren Alter. Die Versorgung dieser Frakturen stellt den behandelnden Arzt vor viele Herausforderungen und macht ein standardisiertes Vorgehen notwendig. Zur Festlegung einer Behandlungsstrategie bietet sich das Unified Classification System (UCS) an: Anhand der Frakturlokalisation und -ausdehnung, der Knochenqualitat, der Stabilitat der einliegenden Huftprothese und des Vorhandenseins weiterer Implantate wird die Fraktur klassifiziert und ein Behandlungsalgorithmus festgelegt. Diese Arbeit soll eine Ubersicht uber die Diagnostik, die Klassifikation, die Besonderheiten der verschiedenen periprothetischen Frakturtypen und deren Behandlung geben.

In patients with haemophilia (PwH), most frequently affected joints are the ankle, knee and elbow. Due to improved factor therapy in the last decades, these previous findings have to be verified in Germany.The aim of this study is to detect the most affected joint, evaluate the significance of the source of pain and determine the point prevalence of back pain in Germany today.In a retrospective study, data of n = 300 patients with severe moderate and mild haemophilia were evaluated regarding the most affected joint, the most common source of pain, and the point prevalence of back pain. An anamnesis questionnaire and the German Pain Questionnaire were used for this assessment.The most affected joint in German PwH is still the ankle (41%), followed by the knee (27%) and the elbow (11%). The most common source of pain is also the ankle joint (32%). Back pain was also identified as one of the most common sources of pain, which is comparable to the elbow (elbow:15%; back:13%). The point prevalence in PwH for back pain was significantly higher compared to the general German population (P = .031).Our data showed that the ankle is still the most affected joint and the most common source of pain in Germany. These results also showed the relevance of back pain as a pain source. The evaluations also demonstrated the high point prevalence of back pain in PwH. Future therapies should also focus on the spine because joint changes affect posture.

Pathologic fractures of the extremities due to carcinoma metastases require individual and patient prognosis-related stabilization procedures. Considering the anatomic features of the humerus, implant material stability is less critical than femoral fractures because of less weight-bearing stress. Therefore, operation length, blood loss, and quick recovery of function are of greater interest. In this retrospective cohort study, we evaluated and compared the outcomes of compound plate osteosynthesis and intramedullary (IM) nailing while managing diaphyseal pathologic fractures of the humerus.We retrospectively reviewed patients treated at our institution for pathologic fractures of the diaphyseal humerus between 2010 and 2021 for group differences (plate osteosynthesis vs. IM nailing) in terms of blood loss, length of operation, implant survival, and upper extremity function.We reviewed 42 stabilization procedures due to pathologic diaphyseal humerus fractures, with a mean follow-up of 8.5±15.4 months. IM nailing (n=20) showed a significantly lower blood loss (266.7±23.7 mL) than plate osteosynthesis (n=22, 571.1±92.6 mL). We did not detect statistically significant differences in the complication rate, length of operation, or Musculoskeletal Tumor Society score.Our findings suggested that diaphyseal fractures of the humerus should be stabilized using an IM nail rather than plate osteosynthesis due to lower blood loss, while complication rate, implant survival, and length of operation remain indifferent.

Hip screening sonography according to Graf in rare cases yields the appearance of a double femoral head, aptly named the Double-Head-Sign. The goal of this retrospective study is to offer a definition of this rare sign, evaluate its incidence of occurrence and compare the sonographic findings with the clinical findings.This Double-Head-Sign is caused by the overlap of the trochanter major with the actual head of the femur in cases of coxa vara. This was observed in 13 of 2800 neonates we screened as part of our sonographical neonatal hip screening.13 neonates presented themselves with ultrasound findings in accordance with our description, yielding an incidence of 0,46% in our patient cohort. In correlation, these neonates also exhibited increased external and decreased internal rotation of the hip. Two had an underlying condition (achondroplasia).In these rare cases, clinical examination of hip rotation in 90° hip flexion has proved helpful and further interdisciplinary examination of such infants to distinguish a possible underlying primary condition is recommended.Das sonografische Hüftscreening nach Graf zeigt in seltenen Fällen das Phänomen des scheinbar (ver)doppelten Hüftkopfes. Das Ziel dieser retrospektiven Studie ist es, eine Definition zu finden, die Inzidenz des Auftretens darzulegen und die klinischen mit den sonografischen Untersuchungsbefunden zu vergleichen.Das Phänomen des scheinbar (ver)doppelten Hüftkopfes wird verursacht durch das Überlappen des Trochanter major mit dem Femurkopf bei Coxa vara und verminderter Antetorsion. Im Rahmen des leitliniengerechten Neugeborenenscreenings wurden 2017 – 2018 2800 Säuglinge untersucht, dabei kam es in 13 Fällen zu dem oben beschriebenen sonografischen Befund.Von den 2800 untersuchten Neugeborenen zeigte sich bei 13 Kindern das Phänomen des scheinbar (ver)doppelten Hüftkopfes im Ultraschall. Dies entspricht einer Inzidenz von 0,46%. Klinisch zeigten sich bei diesen Patienten eine abnormal vermehrte Außenrotation und verminderte Innenrotation der Hüften in 90° Hüftbeugung. 11 der 13 Kinder zeigten sich in der weiterführenden pädiatrischen und orthopädischen Untersuchung als unauffällig. Bei 2 Kindern wurde eine Achondroplasie diagnostiziert. Es befand sich ein Zwillingskind unter den 13 Kindern.Das Phänomen des scheinbar (ver)doppelten Hüftkopfes kann in der Routineuntersuchung beim Hüftscreening nach Graf festgestellt werden. Dies kann ein Hinweis auf eine Coxa vara sein. Eine weitere Abklärung ist empfohlen, um eine Grunderkrankung auszuschließen.

Soft tissue sarcomas are a heterogeneous group of neoplasias that due to their often clinically silent appearance often remain undetected or experience delayed treatment. Especially soft tissue swelling is often misinterpreted by patients and doctors and trivialized or verified with an incorrect biopsy technique. The hereby evoked complications for the patients are serious and may be reduced by simply following the available guidelines. The treatment of soft tissue sarcomas requires a close interdisciplinary coordination between specialists in tumor orthopedics, oncology, radiology, pathology and radiotherapy. On the basis of a selective literature review, the following article points out the current evidence on the treatment and illustrates a treatment algorithm.Weichteilsarkome sind eine heterogene Gruppe von seltenen Neoplasien, die aufgrund ihrer anatomischen Lage und ihres oft inapparenten klinischen Erscheinungsbildes eine häufig zu spät diagnostizierte Tumorentität darstellen. Gerade weichteilige Schwellungen werden initial oft fehlinterpretiert und im Gegensatz zu den gängigen Behandlungsleitlinien bagatellisiert oder inkorrekt biopsiert. Die daraus resultierenden Komplikationen sind für den Patienten gravierend und können durch Befolgen gewisser Behandlungsgrundsätze minimiert werden. Die Behandlung der Weichteilsarkome kann nur im interdisziplinären Kontext unter Einbeziehung von chirurgischen Fächern, Onkologie, Pathologie und Strahlentherapie gelingen und sollte beginnend mit der Biopsie an Tumorzentren durchgeführt werden. Dieser Übersichtsartikel versucht, anhand einer selektiven Literaturrecherche eine Übersicht über die aktuelle Evidenzlage zur Behandlung dieser Tumorentität aufzuzeigen und einen Behandlungsalgorithmus darzustellen.

The aim of this study is to compare biomechanical features of different devices used in clinical routine for temporary epiphysiodesis (eight-Plate® and FlexTackTM). The tested implants were divided into four different groups (eight-Plate® vs. FlexTackTM for lateral and anterior implantation) á 10 samples for testing implanted eight-Plate® vs. FlexTackTM in fresh frozen pig femora for maximum load forces (Fmax) and axial physis distance until implant failure (lmax). The second part of this review deals with the several options for the surgical therapy of bone metastases according to their anatomical site and the patient's individual prognosis. Although metastases to the bone - with few exceptions - are normally given palliative therapy, patients nowadays reach survival times of several months to years, depending on the underlying tumour entity. Scoring systems are suitable to estimate the patient's prognosis and fracture risk. The indication for operation has to be adapted to these parameters. In order to preserve a high quality of life, a limb saving procedure has to be aimed at. The options include the resection of the metastasis with stabilisation, the sole stabilisation of the affected bone, and wide resection with limb reconstruction using a modular tumor endoprosthesis. With today's anaesthesiological peri- and intraoperative care, a vast number of surgical implants and bone augmentations are available. However, what is important is not the practicability of the procedure, but the oncological justification and patient-adapted selection of surgical therapy.Der vorliegende 2. Zur Abschätzung der Prognose und Frakturgefährdung stehen verschiedene Scores zur Verfügung. Die Operationsindikation ist immer im Zusammenhang mit der Prognose und patientenindividuell zu stellen. Im Sinne des Erhaltens einer hohen Lebensqualität ist prinzipiell eine extremitätenerhaltende Therapie anzustreben. Diese lässt sich unterteilen in eine Resektion der Metastase mit Stabilisierung der Extremität, der alleinigen Stabilisierung sowie der weiten Resektion mit ossärer und funktioneller Defektrekonstruktion mittels einer Tumorendoprothese. Mit den heutigen Möglichkeiten der intra- und perioperativen Patientenversorgung ist eine Vielzahl von Implantaten und Möglichkeiten der Augmentation von metastatischen Knochendefekten verfügbar. Im Vordergrund steht jedoch nicht die chirurgische Durchführbarkeit, sondern die onkologisch sinnvolle und patientengerechte Wahl der Therapie und des operativen Vorgehens.

The rising number of medical publications makes it difficult to keep up-to-date on scientific knowledge. In recent years, reviews in the form of narrative or systematic publications and meta-analyses have increased. These can only be interpreted and evaluated if the reader understands the techniques used. This review article describes the differences between narrative and systematic reviews, together with the characteristics of meta-analysis, and discusses their interpretation. The concept of systematic reviews and meta-analysis includes a systematic literature search and summary, together with an appraisal of the quality of the publications. Systematic reviews are often considered to be original studies due to their structure and ability to reduce bias.

Durch Storung des Wachstums, z. B. im Rahmen einer kongenitalen Huftluxation, eines Morbus Perthes oder einer bakteriellen Coxitis, kann bei Kindern und Jugendlichen durch Fehlwachstum eine typische Deformitat des proximalen Femur mit verkurztem Schenkelhals und hochstehendem Trochanter major entstehen. Von Morscher und Buess wurde bereits 1988 eine Schenkelhalsverlangerungsosteotomie zur Therapie Erwachsener beschriebenen. Wir zeigen erstmalig die Modifikation der „Morscher-Osteotomie“ bei Kindern und Jugendlichen mit einem winkelstabilen Osteosynthesesystem. Ziel der schenkelhalsverlangernden Osteotomie ist die bestmogliche Wiederherstellung der normalen Anatomie mit anatomischer Schenkelhalslange und einer weitgehend artikulotrochantaren Distanz bei Kindern und Jugendlichen. Alle klinisch relevanten Deformierungen des proximalen Femur mit kurzem Schenkelhals und konsekutivem Trochanterhochstand. Eitrige Koxitis innerhalb der letzten zwei Jahre; erhohte Infektparameter. Unzureichende Zentrierung des Femurkopfs im Azetabulum. Zwei Osteotomieschnitte. Erste Osteotomie im Bereich des Trochanter major. Distalisierung des Trochanterfragments, anschliesende intertrochantare Femurosteotomie. Stabilisierung mittels winkelstabiler Platte (130°-Padiatrie-Huftplatte, Fa. Synthes, Oberdorf, Schweiz). Bis 55 kg Korpergewicht schmerzadaptierte Vollbelastung; >55 kg Korpergewicht Teilbelastung fur 6 Wochen postoperativ. Bei 5 weiblichen Patienten (mittleres Alter 11,67 Jahre) traten intra- und postoperativ keine Komplikationen auf; nach 6–12 Wochen negatives Trendelenburg-Zeichen; nach 3–6 Monaten nahezu freier Bewegungsumfang.

ZusammenfassungDas DRG-System (Diagnoses Related Groups) dient der Berechnung von Fallpauschalen, mit denen den Kliniken die stationären Behandlungen vergütet werden. Der erweiterte Arbeitsaufwand für die fachgerechte Durchführung erfordert zusätzliches Personal (Gesundheitsökonomen, Dokumentare). Bei der Codierung von Behandlungsprozeduren konnten Diskrepanzen zwischen den Eintragungen der behandelnden Ärzte und denen der Medizinökonomen festgestellt werden, die zu beträchtlichen Unterschieden hinsichtlich der Erlöse führten. Die vorliegende Untersuchung diente der Analyse dieser Diskrepanzen bezogen auf die im DRG-System vorgegebenen Fallbeschreibungen. Material und Methoden An einer Klinik für Orthopädie und Unfallchirurgie führten die behandelnden Ärzte und ein Gesundheitsökonom unabhängig voreinander Codierungen der Diagnosen und Behandlungsprozeduren nacheinander eingewiesener Patienten unter Einbeziehung der Berechnung von Erlösen durch. Die Dokumentationen wurden tabellarisch zusammengestellt, um Vergleiche und Berechnungen vornehmen zu können. Resultate Es wurden 295 Eintragungen gegenübergestellt. In 24,75 % der Fälle stimmten die Codierungen von Ärzten und Gesundheitsökonom überein. In den übrigen Fällen differierten die Eintragungen. Der durchschnittliche Unterschied bei den Erlösen betrug 3.552,29 (± 2947) Euro pro Abrechnungsfall. In 6 Fällen bewertete der Gesundheitsökonom den Erlös um mehr als 10 000,-€ höher als die Ärzte. Diskussion Die teilweise sehr diffizile DRG-Codierung wird von behandelnden Ärzten nur teilweise beherrscht. Es entstehen bedeutende Diskrepanzen bei den Fallbeurteilungen zu den Medizinökonomen, was auf ein ungenügendes Wissen über das DRG-System, eine fehlende Routine und begrenzte Zeitressourcen zurückzuführen ist. Die Notwendigkeit einer Kosteneffektivität bewirkt eine mehr ökonomisch ausgerichtete Patientenversorgung, wodurch das individuelle Arzt-Patientenverhältnis eingeschränkt werden muss. Begleiterkrankungen können zum Nachteil der Patienten unterschätzt werden. Es sollte angestrebt werden, dass behandelnde Ärzte und Gesundheitsökonomen durch regelmäßigen Informationsaustausch einen Modus finden, um bei der DRG-Zuordnung einen weitgehenden Konsens zu erzielen, der ärztliche und ökonomische Aspekte berücksichtigt.

ZusammenfassungMaligne Neoplasien sind in Deutschland nach den kardiovaskulären Erkrankungen weiterhin die zweithäufigste Todesursache. Obwohl sich die 5-Jahres-Überlebensraten der meisten malignen Tumoren in Deutschland in den letzten 30 Jahren signifikant verbessert haben, gilt eine ossäre Metastasierung mit wenigen Ausnahmen weiterhin als Zeichen der Inkurabilität. Neben den seltenen primären Knochentumoren macht die Behandlung von ossären Absiedlungen von Karzinomen einen großen Teil der tumororthopädischen Tätigkeit aus, die zudem eine enge interdisziplinäre Abstimmung mit der inneren Medizin, Radiologie und der entsprechenden Fachdisziplin der zugrunde liegenden Entität erfordert. Durch die Fortschritte in der onkologischen Therapie des Primärtumors wird z. T. ein sehr langes Überleben erreicht, sodass das Management von SRE („skeletal related events“) auch bei palliativen Situationen an Relevanz gewinnt. Die vorliegende Übersichtsarbeit geht in ihrem 1. Teil auf der Grundlage einer aktuellen selektiven Literaturübersicht auf die zuvor erwähnten pathophysiologischen Zusammenhänge der ossären Metastasierung ein und stellt im Folgenden einzelne Diagnostika und deren Relevanz dar.

Zusammenfassung Hintergrund Das multiple Myelom (MM) ist als maligne hämatologische Systemerkrankung nach Einteilung der Weltgesundheitsorganisation (WHO) dem Formenkreis der Plasmazellneoplasien zuzuordnen. Die weltweite Inzidenzrate des MM liegt etwa bei 6 – 7/100 000 Einwohnern/Jahr und macht somit etwa 1% aller Krebserkrankungen der Welt aus. In Deutschland sind ca. 6000 Neuerkrankungen pro Jahr zu verzeichnen. Im Zeitalter der sog. neuen Therapiesubstanzen wie Immunmodulatoren, Proteasomeninhibitoren und Antikörpern haben sich in der Behandlung des MM enorme Fortschritte ergeben. Auf dem Fachgebiet der Orthopädie und Unfallchirurgie ist es von großer Bedeutung, die Leitsymptome des MM im klinischen Alltag sicher zu erkennen und durch eine zielgerichtete Basisdiagnostik weiter abzuklären. Die operative Versorgung von myelomassoziierten Skelettläsionen, wie etwa die Stabilisierung von pathologischen Frakturen, ist Aufgabe der orthopädischen Chirurgie. Methoden Auf Grundlage einer umfassenden Literaturrecherche, die selektiv in PubMed mit den Stichworten „multiple myeloma“ and „diagnostic“ or „therapy“ erfolgte, wurde die verfügbare Primär- und Sekundärliteratur zu aktuellen Standards in der Diagnostik und Therapie des MM beurteilt. Eingeschlossen wurden systematische Reviews, Metaanalysen, klinischen Studien und internationale Therapieempfehlungen bis Frühjahr 2016. Ergebnisse Es existieren heute sehr sensitive Nachweismethoden in der Diagnostik des MM, die neben der bildgebenden Skelettdiagnostik und histologischen Knochenmarkuntersuchung insbesondere die klinisch-chemische Serum- und Urindiagnostik, einschließlich eines 24-h-Sammelurins umfassen. Nach heutigem Kenntnisstand besteht das therapeutische Grundkonzept aus der Kombination einer Hochdosischemotherapie (HDT) und einer autologen Blutstammzelltransplantation (ASCT). Die einzelnen Therapiesäulen der meist multimodalen Behandlungsstrategie sind in enger fachübergreifender Zusammenarbeit patientengerecht abzuwägen. Hierbei ist für die Bewertung des Risikoprofils des Patienten das Lebensalter, die Komorbiditäten sowie der individuelle Krankheitsverlauf (Rezidivneigung oder therapierefraktäre Krankheitsprogression) zu berücksichtigen. Bei Patienten > 70. – 75. Lebensjahr und/oder schweren Komorbiditäten, für die eine HDT und ASCT i. d. R. ungeeignet ist, müssen Indikationen für konventionelle Therapieoptionen gestellt werden. Darüber hinaus sind supportive Behandlungsmaßnahmen wie die Gabe von Bisphosphonaten zur Knochenprotektion, stabilisierende Operationen von pathologischen Frakturen oder eine lokale Strahlenbehandlung von schmerzhaften Osteolysen in das gesamtheitliche Therapiekonzept des MM einzubinden. Schlussfolgerung In aktuellen Studienuntersuchungen konnte gezeigt werden, dass neue Wirkstoffgruppen wie Proteasomenhemmer, immunmodulatorische Substanzen und Antikörper bereits vielversprechend in der Behandlung des MM eingesetzt werden können, um die Prognose und Lebensqualität von Myelompatienten in Zukunft weiter zu verbessern. Hierbei werden die Therapiealgorithmen komplexer und kostenintensiver.

Lebensjahr.Infektiöses oder tumoröses Geschehen im Operationsgebiet.Es wurde eine ventrale Closed-wedge-Osteotomie des distalen Femurs, durchgeführt mit einer winkelstabilen Fixierung mittels der lateralen distalen Femurplatte TomoFix®.Schmerzadaptierte Vollbelastung bei Besetzung von mindestens 3 winkelstabilen Bolzen, die mindestens 5 Kortikalices fassen.Durchgeführt wurden n = 16 distale Femurosteotomien bei n = 9 Patienten mit Kniebeugekontrakturen im Rahmen einer neurologischen Grunderkrankung. Bei Patienten mit einer infantilen Zerebralparese konnte postoperativ eine Verbesserung des GMFCS-Levels (Gross Motor Function Classification System) gezeigt werden. Die Materialentfernung erfolgte im Mittel nach 11,5 Monaten (Range: 9–18 Monate).

External fixators are important for correcting length discrepancies and axis deformities in pediatric or trauma orthopedic surgery. Pin loosening is a common pitfall during therapy that can lead to pain, infection, and necessary revisions. This study aims to present clinical data using calcium titanate (CaTiO3) Schanz screws and to measure the fixation strength. 22 titanate screws were used for external fixators in 4 pediatric patients. Therapy was initiated to lengthen or correct axial deformities after congenital abnormalities. The maximum tightening torque was measured during implantation, and the loosening torque was measured during explantation. In addition, screws of the same type were used in a cadaver study and compared with stainless steel and hydroxyapatite-coated screws. 12 screws of each type were inserted in four tibias, and the loosening and tightening torque was documented. The fixation index in the in vivo measurement showed a significant increase between screw insertion and extraction in three of the four patients. The pins were in situ for 91 to 150 days, and the torque increased significantly (P = 0.0004) from insertion to extraction. The cadaveric study showed lower extraction torques than insertion torques, as expected in this setting. The calculated fixation index was significantly higher in the CaTiO3 group than in the other groups (P = 0.0208 vs. HA and P

Congenital hip dislocation (luxation) has an incidence of 0.4 - 0.7% and is regarded as a prearthrotic deformity. Thus, if not being diagnosed and treated at a very early age, extensive surgical measures are inevitable in childhood and early adulthood.In the time between 01/2013 and 02/2019 we performed 28 600 hips sonographies in babies as part of general screening measures at U2 or U3. There were 71 instable, dysplastic or dislocated hips diagnosed that were treated by arthrographic, closed reduction. After a hip spica cast was applied, reposition was controlled by MRI, estimating the acetabular head index (ACI), the head coverage index (HCI) as well as the femoral head's sphericity or by sonography using the Graf method.Overall success rate was 91.6% for primary closed reduction. Patients with primarily irreducible hips were significantly older (p 0.003) than patients with primarily successful reducible hips. Congenital dislocated hips had significantly higher ACIs (p 0.001) and HCIs (p = 0.03) as well as significantly less well rounded femoral heads (sphericity; p 0.001) compared to stable hips.Early diagnosis and treatment of congenital dislocated hips by closed reduction is essential for a sufficient and regular maturation of the hips without further surgical interventions.Die kongenitale Hüftluxation tritt mit einer Inzidenz von 0,4 – 0,7% auf, stellt eine präarthrotische Deformität dar und wird deshalb, wenn nicht frühzeitig im Säuglingsalter erfolgreich therapiert, umfangreiche operative Maßnahmen im Kindes- und frühen Erwachsenenalter erforderlich machen.Im Zeitraum von 01/2013 bis 02/2019 wurden 28 600 Hüftultraschalluntersuchungen an Säuglingen im Rahmen der U2 bzw. U3 durchgeführt. Dabei wurden 71 instabile dysplastische bzw. luxierte Hüften diagnostiziert und anschließend arthrografisch gestützt geschlossen reponiert. Das Repositionsergebnis wurde durch Anlage eines Becken-Bein-Gipses gesichert und im MRT mittels Azetabulum-Kopf-Index (AKI), Sphärizität und Kopf-Überdachungs-Index (KÜX) bzw. mittelfristig mittels Hüftsonografie nach Graf kontrolliert.Die Erfolgsquote geschlossener Reposition und damit Hüftausreifung betrug 91,6% bei der primären Reposition. Patienten mit primär nicht suffizient reponierbarer Hüfte waren signifikant (p = 0,003) älter als Patienten, deren Hüften primär geschlossen reponierbar waren. Kongenital luxierte bzw. instabile Hüften zeigten einen signifikant geringen AKI (p 0,001), KÜX (p = 0,03) und eine signifikante Entrundung (Sphärizität; p 0,001) im Vergleich zu stabilen Hüftgelenken.Die frühe Diagnose und Therapie der kongenitalen Hüftluxation mittels geschlossener Reposition ist entscheidend für eine regelhafte Ausreifung der Hüfte ohne zusätzliche chirurgische Maßnahmen.

SummaryThe objective of this study was to define fall rates and to identify possible fall risk factors in adult patients with severe haemophilia. Patients, material, methods: 147 patients with severe haemophilia A and B were evaluated using a standardized test battery consisting of demographic, medical and clinical variables and fall evaluation. Results: 41 (27.9 %) patients reported a fall in the past 12 months, 22 (53.7 %) of them more than once. Young age, subjective gait insecurity and a higher number of artificial joints seem to be risk factors for falling. Conclusion: Falls seem to be a common phenomenon in patients with severe haemophilia. Fall risk screening and fall prevention should be implemented into daily practice.

Einleitung: Bei grosen und gelenknahen knochernen Defektsituationen nach vorangegangenen revisionsendoprothetischen Eingriffen besteht im Fall des erneuten Versagens die Moglichkeit der Versorgung durch modulare Implantatsysteme aus der tumorendoprothetischen Versorgung als sog. „Salvage Procedure“. Das Ziel der vorliegenden retrospektiven Studie ist die Evaluation von Patienten, die aufgrund groser und gelenknaher knocherner Defektsituationen, welche nicht mit konventionellen Implantatsystemen versorgt werden konnten, hinsichtlich des klinisch-funktionellen Outcomes und der Komplikationsrate nach Implantation eines modularen Tumor- und Revisionssystems (MUTARS®, Firma Implantcast, Buxtehude, Deutschland). Methodik: In unserer Klinik wurden im Zeitraum von 2005 bis 2014 insgesamt 25 Patienten mittels des MUTARS-Systems aufgrund einer konventionell nicht zu versorgenden ossaren Defektsituation operativ revidiert. Der mittlere Nachbeobachtungszeitraum lag bei 16 Monaten (Spannweite: 12–72 Monate). Folgende Indikationen fuhrten zum Revisionseingriff: Ein Patient stellte sich aufgrund einer gelockerten Kappenprothese mit Bildung eines Metallgranuloms vor, 2 Patienten mit einer Pseudarthrose nach Implantation eines Femurnagels, ein Patient mit einem „Cut-Out“ eines Femurnagels. Vier Patienten mussten aufgrund einer aseptischen und ein Patient aufgrund einer septischen Lockerung einer zuvor implantierten Endoprothese revidiert werden. Insgesamt wurden 6 Patienten aufgrund einer periprothetischen Fraktur revidiert. Weitere 10 Patienten wurden bei einer infektkonsolidierten Girdlestone-Situation nach Explantation einer Endoprothese aufgrund eines periprothetischen Infekts versorgt. Alle Patienten wurden in regelmasigen Intervallen klinisch und radiologisch reevaluiert. Es wurden neben den Komorbiditaten der Patienten klinisch funktionelle-Parameter wie der Harris Hip Score erfasst. Des Weiteren wurde eine Outcome-Analyse bezuglich peri- und postoperativer Komplikationen und eine nativradiologische Verlaufsbeurteilung durchgefuhrt. Ergebnisse: Insgesamt wurden 23 Patienten mit einem proximalen Femur versorgt. Drei Patienten erhielten zusatzlich zur Tumorprothese einen Beckenteilersatz aufgrund einer Beckendiskontinuitat. Ein Patient wurde mit einem distalen Femur, und bei einem weiteren Patienten wurde das ganze Femur durch einen totalen Femurersatz endoprothetisch ersetzt. Die mittlere Krankenhausverweildauer lag bei 25 Tagen (Spannweite: 11–47 Tage). Wir verzeichneten eine Komplikationsrate von 24 %. Der Harris Hip Score stieg von 28 Punkten praoperativ auf 81 Punkte postoperativ. Schlussfolgerung: Bei hohergradigen, knochernen und gelenknahen Defektsituationen nach vorangegangener, gescheiterter endoprothetischer oder osteosynthetischer Versorgung, bietet das MUTARS-System eine suffiziente Versorgungsmoglichkeit mit modularer Adaptation an die jeweilige individuelle Defektkonstellation des Patienten. Die klinischen Ergebnisse sind im kurz- bis mittelfristigen Verlauf vielversprechend.

Objective Infantile scurvy or Moller-Barlow's disease appears to be of no further importance in Western countries; however, this is a careless assumption. In severely disabled children especially, this malady manifests itself in a broad range of symptoms such as delayed or suppressed bone healing, minor traumatization leading to bruises or fractures, and epiphysiolysis. Methods: The aim of this article was to present the required daily uptake of vitamin C and the biochemical pathways in the human body leading to the typical symptoms of scurvy. Two cases of chronic scurvy with prolonged bone healing and bleeding, epiphysiolysis, and gingival hyperplasia are presented. Both patients were chronically ill with one having cerebral palsy and the other a neuroblastoma of the adrenal gland. Results After diagnosis, the substitution of vitamin C via percutaneous endoscopic gastrostomy in both patients was necessary to treat them. Both patients quickly achieved a full recovery. Conclusion: The two patients presented show the importance of infantile scurvy in daily medical care. The prevalence of scurvy is often underestimated in severely disabled or chronically ill children.

In den letzten Jahren hat die Anzahl an Übersichtsarbeiten in Form von narrativen oder systematischen Reviews, resp. Metaanalysen zugenommen. Ein Verständnis der angewendeten Techniken dieser Publikationen ist für ihr Verständnis und die Qualitätsbewertung unerlässlich. Diese Arbeit vermittelt die Unterschiede von narrativen und systemischen Übersichtsarbeiten, weist auf die Besonderheiten von Metaanalysen hin und hilft bei deren Interpretation. Das Konzept systematischer Reviews und Metaanalysen beinhaltet die systematische Literaturrecherche und -zusammenfassung sowie deren Qualitätsbeurteilung. So können systematisch aufgebaute Übersichtsarbeiten aufgrund ihres Aufbaus als eigenständige Studien angesehen werden und Fehlerquellen (Bias) reduzieren.

Multiple myeloma is a haematological blood cancer in elderly patients, in which neoplastic cell populations cause osteolytic destruction in the bone skeleton. More than 50% of all patients sustain pathological fractures during the course of their disease. Of all malignant lesions of the spine, multiple myeloma is the most frequent spinal tumour and accounts for approximately 15% of all cases. 8 - 10% of the patients develop neurological deficits. In addition to systemic antineoplastic therapy, surgical treatment is often required in order to reconstruct and stabilise bone defects associated with multiple myeloma, as well as to address tumour-related complications.A comprehensive literature search was performed in PubMed using the keywords "multiple myeloma" and "surgery" or "fracture". This served to evaluate the available primary and secondary literature on the current status of the surgical therapy of multiple myeloma. Systematic reviews, meta-analyses and clinical studies and international recommendations for therapy were included - from 1975 until the spring of 2018.Stabilising surgery is essential in order to retain and restore function and mobility in bone sections affected by myeloma and for tumour-related osseous defects due to pathological fracture or instability. Absolute and relative indications for surgical treatment are distinguished and are based on the extent and localisation of bone lesions, manifest fractures, accompanying side effects and complications such as neurological deficits and bone pain that is intractable to conservative treatment. The range of options in surgical treatment of myeloma-related bone damage within the upper and lower extremities ranges from hybrid stabilisation procedures combining bone cement and various implants, to conventional and modular tumour prostheses systems. In spinal surgery, minimally invasive procedures such as kyphoplasty or vertebroplasty, as well as selective decompressive operations, dorsal spinal fusion and vertebral replacement are performed in cases of osseous defects and related fractures or instabilities. The surgical treatment and time of operation is determined by the general individual risk profile and prognosis of the patient with multiple myeloma. The surgical approach should have the objective of a final and load stable solution for the patient, which takes into account the frequent long-term course of the disease. Radiotherapy is often recommended after surgery.In therapy of multiple myeloma, surgical treatment is often required in order to reduce distress and retain the function and flexibility of myeloma affected bone sections and the mobility and quality of life of the cancer patient. The range of surgical strategies is complex and demanding.Das multiple Myelom (MM) stellt eine maligne hämatologische Plasmazellerkrankung des meist älteren Patienten dar, bei der es durch neoplastische Zellpopulationen im Knochenskelett zu osteolytischen Knochendestruktionen kommt. Über 50% aller Myelompatienten erleiden im Krankheitsverlauf pathologische Frakturen. Mit rund 15% ist das MM der am häufigsten auftretende maligne Tumor der Wirbelsäule, wobei 8 – 10% eine tumorbedingte neurologische Ausfallsymptomatik aufzeigen. Die operative Therapie stellt bei Patienten mit MM neben der onkologischen, antineoplastischen Systemtherapie eine wesentliche supportive Behandlungsmaßnahme dar und dient der Rekonstruktion und Stabilisierung betroffener Skelettabschnitte nach myelomassoziierten Knochendestruktionen und der Behandlung begleitender Komplikationen.Auf Grundlage einer umfassenden Literaturrecherche, die selektiv in PubMed mit den Stichworten „multiple myeloma“ and „surgery“ or „fracture“ erfolgte, wurde die verfügbare Primär- und Sekundärliteratur zur Beurteilung der aktuellen operativen Therapieverfahren beurteilt. Eingeschlossen wurden deutsch- und englischsprachige systematische Reviews, Metaanalysen, klinische Studien und internationale Therapieempfehlungen von 1975 bis Frühjahr 2018.Bei Auftreten von pathologischen Frakturen oder Instabilitäten des Skelettapparats aufgrund von tumorbedingten Knochendestruktionen sind stabilisierende Operationen für die Erhaltung und Wiederherstellung von Funktion und Beweglichkeit unumgänglich. Es werden absolute und relative Indikationsstellungen für eine operative Therapie des MM unterschieden, die sich nach Ausmaß und Befund stabilitätsgefährdender Knochenläsionen oder manifester pathologischer Frakturen sowie begleitender Komplikationen wie neurologische Ausfallserscheinungen und konservativ therapierefraktäre Knochenschmerzen richten. Das Spektrum operativer Versorgungsmöglichkeiten reicht im Bereich der Extremitäten von Verbundosteosynthesen mit verschiedenen Kraftträgern bis hin zu modularen Tumorendoprothesensystemen. Zur Stabilisierung von tumorbedingten Wirbelsäulendestruktionen kommen minimalinvasive Verfahren wie Kypho- und Vertebroplastien, aber auch selektive Dekompressionen sowie Fusionsspondylodesen und Wirbelkörperersatzverfahren zum Einsatz. Operationsmethode und Versorgungszeitpunkt sind individuell und in Abhängigkeit vom Risikoprofil und Prognose des Myelompatienten zu wählen. Grundsätzliches Ziel und Anspruch operativer Stabilisierungseingriffe sollte eine definitive und belastungsstabile Versorgung sein, die bei meist langem Krankheitsverlauf des Myelompatienten eine Restitutio ad Integrum gewährleistet. Postoperativ ist eine adjuvante Strahlentherapie zu empfehlen.Die operative Therapie des MM nimmt eine Schlüsselrolle in der ganzheitlichen Behandlung des Myelompatienten ein, um Leidensdruck zu reduzieren und Funktion und Beweglichkeit der betroffenen Skelettabschnitte wiederherzustellen und damit Mobilität und Lebensqualität für den Tumorpatienten zu erhalten. Das Spektrum der operativen Versorgungsstrategien ist dabei komplex und anspruchsvoll.

In Germany and other European countries, cancer is the second most common cause of death after cardiovascular disease. Although 5-year survival rates for several types of cancer have significantly improved over the last 30 years, metastasis to the bone almost always leads to incurable disease. Aside from the rare primary bone tumours, the treatment of bone metastases now accounts for a major part of tumour orthopaedic workload and requires close interdisciplinary coordination between specialists in oncology, radiology and the discipline of the primary tumour entity. Due to improvements in oncological treatment regimes, long survival times can be achieved. Therefore, the management of so-called "SRE" (skeletal-related events) has gained importance, even in palliative situations. On the basis of a selective literature review, the following article points out the underlying pathophysiological processes of bone metastases and outlines different diagnostic approaches and their relevance in the current clinical setting.Maligne Neoplasien sind in Deutschland nach den kardiovaskulären Erkrankungen weiterhin die zweithäufigste Todesursache. Obwohl sich die 5-Jahres-Überlebensraten der meisten malignen Tumoren in Deutschland in den letzten 30 Jahren signifikant verbessert haben, gilt eine ossäre Metastasierung mit wenigen Ausnahmen weiterhin als Zeichen der Inkurabilität. Neben den seltenen primären Knochentumoren macht die Behandlung von ossären Absiedlungen von Karzinomen einen großen Teil der tumororthopädischen Tätigkeit aus, die zudem eine enge interdisziplinäre Abstimmung mit der inneren Medizin, Radiologie und der entsprechenden Fachdisziplin der zugrunde liegenden Entität erfordert. Durch die Fortschritte in der onkologischen Therapie des Primärtumors wird z. T. ein sehr langes Überleben erreicht, sodass das Management von SRE („skeletal related events“) auch bei palliativen Situationen an Relevanz gewinnt. Die vorliegende Übersichtsarbeit geht in ihrem 1. Teil auf der Grundlage einer aktuellen selektiven Literaturübersicht auf die zuvor erwähnten pathophysiologischen Zusammenhänge der ossären Metastasierung ein und stellt im Folgenden einzelne Diagnostika und deren Relevanz dar.

Ein 32-jahriger Patient beklagte 6 Monate nach Plattenosteosynthese einer Klavikulaschaftfraktur im mittleren Drittel (OTA-Klassifikation Typ A) links eine progrediente Claudicatio-Symptomatik des linken Armes. Radiologisch zeigte sich ein subakuter Verschluss der A. subclavia links. Ausloser waren rezidivierende Embolien, ausgehend von einer etwa 50 %igen Verlegung des Lumens der A. subclavia links, bedingt durch ein Granulom an der Spitze einer uberstehenden Schraube. Die Reperfusion gelang mittels Katheterembolektomie und gleichzeitig vollstandiger Entfernung des Osteosynthesematerials. Dieser Fall zeigt, dass bei der Plattenosteosynthese von Klavikulaschaftfrakturen, insbesondere im medialen und mittleren Klavikuladrittel, ein Schraubenuberstand vermieden werden muss, da es in dieser Region aufgrund der raumlichen Nahe zu den subklavikularen Gefasen nicht nur akut, sondern auch 2-zeitig zu einem Gefasschaden kommen kann.

Mitochondria exist in a highly dynamic network that is constantly altered by fusion and fission events depending on various factors such as cellular bioenergetic state and cell cycle. Next to this dynamic nature of the organelle, its cristae membrane also undergoes drastic morphological changes upon physiological or pathological alterations. The Mitofilin/mitochondrial inner membrane organizing system (MINOS) complex was recently reported to ensure mitochondrial architecture and crista junction integrity. Several subunits of this complex are linked to a diverse set of neurological human disorders. Recently, two apolipoproteins, ApoO (APOO) and ApoO-like (APOOL) were suggested to represent constituents of the mammalian Mitofilin/MINOS complex. APOOL was shown to bind the mitochondrial phospholipid cardiolipin (CL) and to interact physically with this complex. In this review we highlight the current view on the mammalian Mitofilin/MINOS complex and focus on APOOL and the role of CL in determining cristae morphology. We will discuss possible functions of the Mitofilin/MINOS complex on lipid transport, on assembly of respiratory supercomplexes, on F1FO-ATP synthase organization, on contact site formation, and on trapping CL within the cristae subcompartment.

Remodeling of crista junctions (CJs) is observed in numerous human disorders and during apoptosis. The functional interplay of OPA1 and MIC60, two key players in this context, is unclear. We show that OPA1 modulates cristae morphology but is dispensable for CJ formation. MIC60 is strongly enriched at CJs, whereas OPA1 is distributed evenly across the inner membrane. MIC60 levels are increased in OPA1-/- cells which show increased cellular resistance to apoptosis induction. Endogenous OPA1 and MIC60 show a physical interaction. Overall, we suggest that the regulation of CJ remodeling during apoptosis is mediated via an interplay between OPA1 and MIC60.

Bone represents a highly dynamic tissue whose development is strongly dependent on vasculogenic and angiogenic processes. Neovascularization also plays an important role in fracture healing and in tissue engineering applications aiming at restoring bone function. We have previously shown in a heterotopic subcutaneous implantation model of severe combined immunodeficiency (SCID) mice that implanted human umbilical vein endothelial cells (HUVECs) gave rise to the formation of a complex functional human neovasculature. In this study, we investigated the effect of HUVEC coimplantation on mesenchymal stem cell (MSC)-mediated bone regeneration in an orthotopic calvarial bone defect model in immunocompromised mice. For this purpose, human fibrin/Matrigel-immobilized HUVECs and MSCs were seeded alone or in combination into scaffolds consisting of decalcified processed bovine cancellous bone (Tutobone) and implanted into calvarial critical-sized defects. Our results show that implanted HUVECs formed complex three-dimensional networks of perfused human neovessels that were stabilized by recruiting perivascular cells. Neovessel formation was considerably higher in the coimplantation group, suggesting that implanted MSCs supported HUVEC-triggered neovascularization. In addition, implanted MSCs effectively supported bone formation in calvarial defects. However, the HUVEC-derived neovasculature did not improve MSC-triggered bone regeneration in this orthotopic critical-sized defect model.

Mitochondrial membrane architecture is important for organelle function. Alterations thereof are linked to a number of human disorders including diabetes and cardiomyopathy. The MICOS complex was recently reported to be a central player determining cristae structure and formation of crista junctions. Here we investigated the functional role of MIC26, a lipoprotein formerly termed APOO. Its levels are increased in diabetic heart tissue and in blood plasma of patients suffering from acute coronary syndrome. We demonstrate that human MIC26 exists in three distinct forms: (1) a glycosylated and secreted 55kDa protein, (2) an ER/Golgi-resident form thereof, and (3) a non-glycosylated 22kDa mitochondrial protein. The latter isoform spans the mitochondrial inner membrane and physically interacts with several MICOS complex subunits such as MIC60, MIC27, and MIC10. We further demonstrate that MIC26 and MIC27, a homologous protein formerly termed APOOL, regulate their levels in an antagonistic manner. Both proteins are positively correlated with the levels of MIC10 as well as tafazzin, an enzyme required for cardiolipin remodeling. Overexpression of MIC26 induced fragmentation of mitochondria, promoted ROS formation and resulted in impaired mitochondrial respiration. Downregulation of MIC26 induced a decrease in mitochondrial oxygen consumption, whereas mitochondrial network morphology and ROS levels remained unaffected. MIC26 depletion led to alterations in mitochondrial ultrastructure and caused a significant reduction in the number of crista junctions. In summary, we show that the human apolipoprotein MIC26 is a bona fide subunit of the MICOS complex and that MIC26 is linked to cardiolipin metabolism and promotes crista junction formation.

The majority of amyotrophic lateral sclerosis (ALS) cases as well as many patients suffering from frontotemporal lobar dementia (FTLD) with ubiquitinated inclusion bodies show TDP-43 pathology, the protein encoded by the TAR DNA-binding protein (Tardbp) gene. We used recombinase-mediated cassette exchange to introduce an ALS patient cDNA into the mouse Tdp-43 locus. Expression levels of human A315T TDP-43 protein were 300% elevated in heterozygotes, whereas the endogenous mouse Tdp-43 was decreased to 20% of wild type levels as a result of disturbed feedback regulation. Heterozygous TDP-43(A315TKi) mutants lost 10% of their body weight and developed insoluble TDP-43 protein starting as early as 3 months after birth, a pathology that was exacerbated with age. We analyzed the splicing patterns of known Tdp-43 target genes as well as genome-wide gene expression levels in different tissues that indicated mitochondrial dysfunction. In heterozygous mutant animals, we observed a relative decrease in expression of Parkin (Park2) and the fatty acid transporter CD36 along with an increase in fatty acids, HDL cholesterol, and glucose in the blood. As seen in transmission electron microscopy, neuronal cells in motor cortices of TDP-43(A315TKi) animals had abnormal neuronal mitochondrial cristae formation. Motor neurons were reduced to 90%, but only slight motoric impairment was detected. The observed phenotype was interpreted as a predisease model, which might be valuable for the identification of further environmental or genetic triggers of neurodegeneration.

In the mitochondrial matrix, it multimerizes and forms a macromolecular proteasome-like cylinder together with the chaperone CLPX. In spite of a known relevance for the mitochondrial unfolded protein response, its substrates and tissue-specific roles are unclear in mammals. Recessive CLPP mutations were recently observed in the human Perrault variant of ovarian failure and sensorineural hearing loss. Here, a first characterization of CLPP null mice demonstrated complete female and male infertility and auditory deficits. Disrupted spermatogenesis already at the spermatid stage and ovarian follicular differentiation failure were evident. Reduced pre-/post-natal survival and marked ubiquitous growth retardation contrasted with only light impairment of movement and respiratory activities. Interestingly, the mice showed resistance to ulcerative dermatitis. Systematic expression studies detected up-regulation of other mitochondrial chaperones, accumulation of CLPX and mtDNA as well as inflammatory factors throughout tissues. T-lymphocytes in the spleen were activated. Thus, murine Clpp deletion represents a faithful Perrault model. The disease mechanism probably involves deficient clearance of mitochondrial components and inflammatory tissue destruction.

The protein complexes involved are largely unknown or only insufficiently characterized. Using complexome profiling we identified apolipoprotein O (APOO) and apolipoprotein O-like protein (APOOL) as putative components of the Mitofilin/MINOS protein complex which was recently implicated in determining cristae morphology. We show that APOOL is a mitochondrial membrane protein facing the intermembrane space. It specifically binds to cardiolipin in vitro but not to the precursor lipid phosphatidylglycerol. Overexpression of APOOL led to fragmentation of mitochondria, a reduced basal oxygen consumption rate, and altered cristae morphology. Downregulation of APOOL impaired mitochondrial respiration and caused major alterations in cristae morphology. We further show that APOOL physically interacts with several subunits of the MINOS complex, namely Mitofilin, MINOS1, and SAMM50. We conclude that APOOL is a cardiolipin-binding component of the Mitofilin/MINOS protein complex determining cristae morphology in mammalian mitochondria. Our findings further assign an intracellular role to a member of the apolipoprotein family in mammals.

International audience; Mitochondrial dysfunction is linked to apoptosis, aging, cancer, and a number of neurodegenerative and muscular disorders. The interplay between mitophagy and mitochondrial dynamics has been linked to the removal of dysfunctional mitochondria ensuring mitochondrial quality control. An open question is what role mitochondrial fission plays in the removal of mitochondria after mild and transient oxidative stress; conditions reported to result in moderately elevated reactive oxygen species (ROS) levels comparable to physical activity. Here we show that applying such conditions led to fragmentation of mitochondria and induction of mitophagy in mouse and human cells. These conditions increased ROS levels only slightly and neither triggered cell death nor led to a detectable induction of non-selective autophagy. Starvation led to hyperfusion of mitochondria, to high ROS levels, and to the induction of both non-selective autophagy and to a lesser extent to mitophagy. We conclude that moderate levels of ROS specifically trigger mitophagy but are insufficient to trigger non-selective autophagy. Expression of a dominant-negative variant of the fission factor DRP1 blocked mitophagy induction by mild oxidative stress as well as by starvation. Taken together, we demonstrate that in mammalian cells under mild oxidative stress a DRP1-dependent type of mitophagy is triggered while a concomitant induction of non-selective autophagy was not observed. We propose that these mild oxidative conditions resembling well physiological situations are thus very helpful for studying the molecular pathways governing the selective removal of dysfunctional mitochondria.

Membrane architecture is crucially important for mitochondrial function and integrity. The MICOS complex is located at crista junctions and determines cristae membrane morphology and the formation of crista junctions. Here we provide data of the bona fide MICOS subunit MIC26 for determining cristae morphology. MiRNA-mediated downregulation of MIC26 results in higher protein levels of MIC27 and in lower levels of Mic10. Using a miRNA-resistant form to MIC26 we show that this effect is specific to MIC26. Our data further demonstrate that depletion of MIC26 primarily affects the level of the 22kDa mitochondrial isoform of MIC26 but not the amount of the secreted 55kDa isoform of MIC26. Depletion of MIC27, however, increases secretion of the latter isoform. Overexpression of a myc-tagged version of MIC26 resulted in altered cristae morphology with swollen and partly vesicular cristae-structures.
