1. Fault controlled geometries by inherited tectonic texture at the southern end of the East African Rift System in the Makgadikgadi Basin, northeastern Botswana

Abstract

One of the three narrow rift belts that mark the southern end of the East African Rift System (EARS) intersects the Makgadikgadi Basin of northeastern Botswana. Although tectonic activity in the region is known to have influenced the evolution of these pans, the interrelationship between shoreline geometry, fault strikes, and the intersection of the underlying tectonic terranes has yet to be fully realized. We analyzed faults and subsurface structures in the region of the pans using a field investigation in combination with satellite imagery and geophysical data, to constrain the influence that the regional tectonic regime has had on the formation of the present-day pan geometry. We find that pan shorelines are controlled by the intersection of three preferred fault orientations which can be understood in the context of the “older” terranes they overlie, namely the Magondi Belt and the Limpopo Belt. We propose that the pronounced curvature of the southern Magondi Belt has influenced the eastward curvature of the rift-related faults and was likely produced by the impingement of the developing fold belt on the Zimbabwe Craton. Furthermore, limited focal mechanism solutions data from earthquakes north and south of the pans suggests a change in regional extension direction from NW-SE to NE-SW. Determining the relationship between these fault orientations and the underlying tectonic terrains is an important step in understanding the formation of the Makgadikgadi Basin, and more broadly the current tectonic regime of Botswana. The evidence of fault-controlled shorelines within an evaporitic environment may also have implications for regional groundwater activity.

Abstract

Background Early start of highly active antiretroviral therapy (HAART) in perinatally HIV-1 infected children is the optimal strategy to prevent immunological and clinical deterioration. To date, according to EMA, only 35% of antiretroviral drugs are licenced in children Methods An observational retrospective study investigating the rate and the outcomes of off-label prescription of HAART was conducted on 225 perinatally HIV-1 infected children enrolled in the Italian Register for HIV Infection in Children and followed-up from 2001 to 2018. Results 22.2% (50/225) of included children were receiving an off-label HAART regimen at last check. Only 26% (13/50) of off-label children had an undetectable viral load (VL) before the commencing of the regimen and the 52.0% (26/50) had a CD4 + T lymphocyte percentage > 25%. At last check, during the off label regimen, the 80% (40/50) of patients had an undetectable VL, and 90% (45/50) of them displayed CD4 + T lymphocyte percentage > 25%. The most widely used off-label drugs were: dolutegravir/abacavir/lamivudine (16%; 8/50), emtricitbine/tenofovir disoproxil (22%; 11/50), lopinavir/ritonavir (20%; 10/50) and elvitegravir/cobicistat/emtricitabine/ tenofovir alafenamide (10%; 10/50). At logistic regression analysis, detectable VL before starting the current HAART regimen was a risk factor for receiving an off-label therapy (OR: 2.41; 95% CI 1.13–5.19; p = 0.024). Moreover, children Conclusion The prescription of an off-label HAART regimen in perinatally HIV-1 infected children was common, in particular in children with detectable VL despite previous HAART and in younger children, especially those receiving their first regimen. Our data suggest similar proportions of virological and immunological successes at last check among children receiving off-label or on-label HAART. Larger studies are needed to better clarify efficacy and safety of off-label HAART regimens in children, in order to allow the enlargement of on-label prescription in children.

Abstract

Background In human immunodeficiency virus (HIV)–positive adults, low CD4 cell counts despite fully suppressed HIV-1 RNA on antiretroviral therapy (ART) have been associated with increased risk of morbidity and mortality. We assessed the prevalence and outcomes of poor immune response (PIR) in children receiving suppressive ART. Methods Sixteen cohorts from the European Pregnancy and Paediatric HIV Cohort Collaboration (EPPICC) contributed data. Children Results Of 2318 children included, median age was 6.4 years and 68% had advanced/severe immunosuppression at ART initiation. At 1 year of VS, 12% had PIR. In multivariable analysis, PIR was associated with older age and worse immunological stage at ART start, hepatitis B coinfection, and residing in Thailand (all P ≤ .03). Rates of AIDS/death (95% confidence interval) per 100 000 person-years were 1052 (547, 2022) among PIR versus 261 (166, 409) among immune responders; rate ratio of 4.04 (1.83, 8.92; P < .001). Conclusions One in eight children in our cohort experienced PIR despite sustained VS. While the overall rate of AIDS/death was low, children with PIR had a 4-fold increase in risk of event as compared with immune responders.

Abstract

Data on features of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in children and adolescents are scarce. We report preliminary results of an Italian multicentre study comprising 168 laboratory-confirmed paediatric cases (median: 2.3 years, range: 1 day–17.7 years, 55.9% males), of which 67.9% were hospitalised and 19.6% had comorbidities. Fever was the most common symptom, gastrointestinal manifestations were frequent; two children required intensive care, five had seizures, 49 received experimental treatments and all recovered.

Abstract

OBJECTIVES: Vertical transmission of HIV can be effectively controlled through antenatal screening, antiretroviral treatment and the services provided during and after childbirth for mother and newborn. In Italy, the National Health Service guarantees universal access to prenatal care for all women, including women with HIV infection. Despite this, children are diagnosed with HIV infection every year. The aim of the study was to identify missed opportunities for prevention of mother-to-child transmission of HIV. METHODS: The Italian Register for HIV Infection in Children, which was started in 1985 and involves 106 hospitals throughout the country, collects data on all new cases of HIV infection in children. For this analysis, we reviewed the database for the period 2005 to 2015. RESULTS: We found 79 HIV-1-infected children newly diagnosed after birth in Italy. Thirty-two of the mothers were Italian. During the pregnancy, only 15 of 19 women with a known HIV diagnosis were treated with antiretroviral treatment, while, of 34 women who had received an HIV diagnosis before labour began, only 23 delivered by caesarean section and 17 received intrapartum prophylaxis. In 25 mothers, HIV infection was diagnosed during pregnancy or in the peripartum period. Thirty-one newborns received antiretroviral prophylaxis and 39 received infant formula. CONCLUSIONS: We found an unacceptable number of missed opportunities to prevent mother-to-child transmission (MCTC). Eliminating HIV MTCT is a universal World Health Organization goal. Elucidating organization failures in Italy over the past decade should help to improve early diagnosis and to reach the zero transmission target in newborns.

Abstract

Background. Data on durability of first-line antiretroviral therapy (ART) in children with human immunodeficiency virus (HIV) are limited. We assessed time to switch to second-line therapy in 16 European countries and Thailand.Methods. Children aged = 2 nucleoside reverse transcriptase inhibitors p[NRTIs] plus nonnucleoside reverse transcriptase inhibitor p[NNRTI] or boosted protease inhibitor p[PI]) were included. Switch to second-line was defined as (i) change across drug class (PI to NNRTI or vice versa) or within PI class plus change of >= 1 NRTI; (ii) change from single to dual PI; or (iii) addition of a new drug class. Cumulative incidence of switch was calculated with death and loss to follow-up as competing risks.Results. Of 3668 children included, median age at ART initiation was 6.1 (interquartile range (IQR), 1.7-10.5) years. Initial regimens were 32% PI based, 34% nevirapine (NVP) based, and 33% efavirenz based. Median duration of follow-up was 5.4 (IQR, 2.9-8.3) years. Cumulative incidence of switch at 5 years was 21% (95% confidence interval, 20%-23%), with significant regional variations. Median time to switch was 30 (IQR, 16-58) months; two-thirds of switches were related to treatment failure. In multivariable analysis, older age, severe immunosuppression and higher viral load (VL) at ART start, and NVP-based initial regimens were associated with increased risk of switch.Conclusions. One in 5 children switched to a second-line regimen by 5 years of ART, with two-thirds failure related. Advanced HIV, older age, and NVP-based regimens were associated with increased risk of switch.

Abstract

Background: Information on the use of new antiretroviral drugs in children in the real setting of clinical fields is largely unknown. Methods: Data from 2554 combined antiretroviral therapy (cART) regimens administered to 911 children enrolled in the Italian Register for HIV infection in children, between 1996 and 2009, were analysed. Factors potentially associated with undetectable viral load and immunological response to cART were explored by Cox regression analysis. Abstract

Nel corso del 2013 è stato eseguito uno studio sull’andamento temporale degli anticorpi nei confronti dei sottotipi H1N1, H3N2 e H1N2 del virus dell’inluenza suina (SIV) in soggetti allevati in 11 aziende e immunizzati nei confronti di SIV con un vaccino trivalente inattivato. In ciascuna azienda sono stati selezionati tre gruppi di animali: un primo gruppo è stato vaccinato a 70-90 giorni di vita, con richiamo a distanza di quattro settimane e a circa 180 giorni di vita, con un vaccino trivalente nei confronti di SIV e con un vaccino nei confronti della malattia di Aujeszky (ADV), mediante inoculazione i.m. in due punti diversi. Un secondo gruppo è stato vaccinato, con la stessa tempistica, utilizzando una miscela dei due vaccini somministrata i.m. nello stesso punto di inoculo. Il terzo gruppo di animali è stato vaccinato solo nei confronti di ADV come tutti gli altri suini presenti nell’allevamento (gruppo di controllo). E’ stato effettuato un prelievo ematico da tutti gli animali appartenenti ai tre gruppi prima della vaccinazione, quattro settimane dopo il richiamo, in concomitanza con il terzo intervento vaccinale a 180 giorni di vita e prima dell’invio alla macellazione. I sieri sono stati analizzati per la ricerca di anticorpi nei confronti dei sottotipi H1N1, H3N2 e H1N2 mediante inibizione dell’emoagglutinazione. La vaccinazione nei confronti di SIV ha determinato un incremento significativo dei titoli anticorpali nei soggetti vaccinati rispetto a quelli di controllo. Tale differenza è stata riscontrata anche nelle aziende nelle quali è stato possibile, su base sierologica, ipotizzare la di circolazione di SIV. La conferma virologica si è avuta soltanto in una azienda nella quale sono stati riscontrati evidenti sintomi respiratori. Le diverse modalità di somministrazione della vaccinazione non hanno determinato differenze significative nella risposta anticorpale.

Factors independently associated with virological and immunological success were as follows: later calendar periods, younger age at regimen (only for virological success) and higher CD4(+) T-lymphocyte percentage at baseline. Use of unboosted PI was associated with lower adjusted hazard ratio (aHR) of virological or immunological success with respect to NNRTI- and boosted PI-based regimens, with no difference among these two latter types. CONCLUSION: Use of new generation antiretroviral drugs in Italian HIV-infected children is increasing. No different viro-immunological outcomes between NNRTI- and boosted PI-based cART were observed

Abstract

There is currently an experts' agreement discouraging interruption of antiretroviral treatment (ART) during the first trimester of pregnancy in women infected with human immunodeficiency virus type 1 (HIV-1). However, this recommendation is poorly supported by data. We evaluated the effects of discontinuing ART during pregnancy on the rate of mother-to-child transmission.Logistic regression models were performed in a prospective cohort of 937 children who were perinatally exposed to HIV-1 to estimate adjusted odds ratios for confounding factors on mother-to-child transmission, including maternal interruption of ART.Among 937 pregnant women infected with HIV-1, ART was interrupted in 81 (8.6%) in the first trimester and in 11 (1.2%) in the third trimester. In the first trimester, the median time at suspension of ART was 6 weeks (interquartile range [IQR], 5-6 weeks) and the time without treatment was 8 weeks (IQR, 7-11 weeks). In the third trimester, the median time at suspension of ART was 32 weeks (IQR, 23-36 weeks) and the time without treatment was 6 weeks (IQR, 2-9 weeks). The plasma viral load was similar in women who had treatment interrupted in the first trimester and in those who did not have treatment interrupted. Overall, the rate of mother-to-child transmission in the whole cohort was 1.3% (95% confidence interval [CI], 0.7%-2.3%), whereas it was 4.9% (95% CI, 1.9%-13.2%) when ART was interrupted in the first trimester and 18.2% (95% CI, 4.5%-72.7%) when ART was interrupted in the third trimester. In the multiple logistic regression models, only interruption of ART during either the first or the third trimester, maternal mono- or double therapy, delivery by a mode other than elective cesarean delivery, and a viral load at delivery4.78 log(10) copies/mL were independently associated with an increased rate of mother-to-child transmission.Discontinuing ART during pregnancy increases the rate of mother-to-child transmission of HIV-1, either when ART is stopped in the first trimester and subsequently restarted or when it is interrupted in the third trimester. This finding supports recommendations to continue ART in pregnant women who are already receiving treatment for their health.

Abstract

INTRODUCTION: Defects of prokineticin pathway affect the neuroendocrine control of reproduction, but their role in the pathogenesis of central hypogonadism remains undefined, and the functional impact of the missense PROKR2 variants has been incompletely characterized. MATERIAL AND METHODS: In a series of 246 idiopathic central hypogonadism patients, we found three novel (p.V158I, p.V334M, and p.N15TfsX30) and six already known (p.L173R, p.T260M, p.R268C, p.V274D, p.V331M, and p.H20MfsX23) germline variants in the PROKR2 gene. We evaluated the effects of seven missense alterations on two different prokineticin receptor 2 (PROKR2)-dependent pathways: inositol phosphate-Ca(2+) (Gq coupling) and cAMP (Gs coupling). RESULTS: PROKR2 variants were found in 16 patients (6.5%). Expression levels of variants p.V158I and p.V331M were moderately reduced, whereas they were markedly impaired in the remaining cases, except p.V334M, which was significantly overexpressed. The variants p.T260M, p.R268C, and p.V331M showed no remarkable changes in cAMP response (EC50) whereas the IP signaling appeared more profoundly affected. In contrast, cAMP accumulation cannot be stimulated through the p.L173R and p.V274D, but IP EC50 was similar to wt inp.L173R and increased by 10-fold in p.V274D. The variant p.V334M led to a 3-fold increase of EC50 for both cAMP and IP. CONCLUSION: Our study shows that single PROKR2 missense allelic variants can either affect both signaling pathways differently or selectively. Thus, the integrity of both PROKR2-dependent cAMP and IP signals should be evaluated for a complete functional testing of novel identified allelic variants.

Abstract

Acute respiratory tract infections (ARTI) are the more frequent causes of morbidity and mortality in children. This review was conducted to assess the existing evidence concerning bacterial extracts efficacy in the prevention of pediatric ARTI. The data sources for the identification of clinical trials and reviews included principal bibliographic databases (MEDLINE, Embase, Cochrane Library, PubMed/www.ncbi.nlm.nih.gov). Selected clinical trials only involved children suffering from recurrent ARTI. Bacterial extracts may reduce the incidence of ARTI of about 40% in toddlers (2-5 years), school boys (6-12 years) and children at higher ARTI risk (e.g. children living in orphanages). Among others, the active treatment with OM-85 led to 26, 2% fewer patients with recurrent ARTI. Data from the literature are encouraging, particularly for treatment with OM-85. The strengthening of pre-clinical and clinical research is necessary to allow routine recommendation of bacterial extracts prescription for ARTI prevention in children.

Abstract

Background/Aims: As evidence accumulates relating to mother-to-child (vertical) transmission of hepatitis C virus (HCV), it is timely to draw up guidelines for the clinical management of HCV infected pregnant women and their children. Methods: A review of evidence from the European Paediatric HCV Network (EPHN) prospective study of HCV infected women and their children and other published studies. Meeting of EPHN clinical experts to reach a consensus on recommendations for management. Each recommendation was graded according to the level of evidence. Results/conclusions: Although several risk factors for mother-to-child transmission have been identified, none are modifiable and there are currently no interventions available to prevent vertical transmission of HCV. Data on timing of loss of maternal antibodies and reliability of diagnostic tests inform the optimum follow-up schedule for confirmation or exclusion of infection in children born to HCV infected women. Based on the current evidence, routine antenatal screening for HCV should not be introduced and neither elective caesarean section nor avoidance of breastfeeding should be recommended to HCV infected women to prevent mother-to-child transmission of HCV. 50. LaP-Ramp geometry of fracturing in travertines: examples from the Acquasanta Terme deposits (Central Italy)

Abstract

Travertines are continental carbonatic rocks characterized by a poor layering with a strong internal lamination. Travertines form by the interplay between chemical and microbial activity in a wide range of depositional environments (from ponds to cascades) reflecting different hydrodynamics regimes. The resulting geometry show a typical spatial anisotropy varying from decametric- to meter-scale. The internal lamination is provided by variations of the depositional facies and it is mainly displayed by alternation of more porous and less porous horizons. Despite the high primary porosity, these deposits present a low permeability, as attested by the presence of fossil gas bubbles. It follows that pores interconnection (secondary permeability) is strongly influence both by primary (depositional) and secondary (structural) factor. The presence of internal lamination deeply influences the fracture trajectories characterized by a dual attitude with parallel and cross-cutting segments. Travertine deposits represent today a primary task in many, newly discovered, oil reservoirs in the Southern Atalantic. The dimension of single fracture is under the resolution of seismic sections. Analogue field studies are thus well required to properly model the secondary permeability features. In this study we describe the patterns and the ways of fracturation in travertines. Fractures in Acquasanta-Terme travertines are characterized by portions cutting through the lamination (ramp, R) and portion running parallel to the internal lamination (lamination parallel, LaP). The combination of R and LaP results in a stairway geometry of the fractures. A Montecarlo approach was successfully used to provide a suitable model of fractures development. We found a relation between the average lamination dipping and the L/R ratio (computed dividing the cumulative length of LaP over R segments) and a relation between the average lamination dip and the angle formed by the intersection between R and LaP segments. These relations suggest that the lamination dipping of 25-30 represent a critical angle reflecting the depositional rheology of the different depositional environments. The found The found L-R fracture property assures a fracture connectivity within travertine deposit thus locally providing effective permeability increase.
