1. Enabling Long-term Fairness in Dynamic Resource Allocation

Abstract

We study the fairness of dynamic resource allocation problem under the $\alpha$-fairness criterion. We recognize two different fairness objectives that naturally arise in this problem: the well-understood slot-fairness objective that aims to ensure fairness at every timeslot, and the less explored horizon-fairness objective that aims to ensure fairness across utilities accumulated over a time horizon. We argue that horizon-fairness comes at a lower price in terms of social welfare. We study horizon-fairness with the regret as a performance metric and show that vanishing regret cannot be achieved in presence of an unrestricted adversary. We propose restrictions on the adversary's capabilities corresponding to realistic scenarios and an online policy that indeed guarantees vanishing regret under these restrictions. We demonstrate the applicability of the proposed fairness framework to a representative resource management problem considering a virtualized caching system where different caches cooperate to serve content requests., Comment: Accepted to ACM SIGMETRICS 2023

2. Ascent Similarity Caching with Approximate Indexes

Abstract

Similarity search is a key operation in multimedia retrieval systems and recommender systems, and it will play an important role also for future machine learning and augmented reality applications. When these systems need to serve large objects with tight delay constraints, edge servers close to the end-user can operate as similarity caches to speed up the retrieval. In this paper we present A\c{C}AI, a new similarity caching policy which improves on the state of the art by using (i) an (approximate) index for the whole catalog to decide which objects to serve locally and which to retrieve from the remote server, and (ii) a mirror ascent algorithm to update the set of local objects with strong guarantees even when the request process does not exhibit any statistical regularity.

3. Towards Inference Delivery Networks: Distributing Machine Learning with Optimality Guarantees

Abstract

An increasing number of applications rely on complex inference tasks that are based on machine learning (ML). Currently, there are two options to run such tasks: either they are served directly by the end device (e.g., smartphones, IoT equipment, smart vehicles), or offloaded to a remote cloud. Both options may be unsatisfactory for many applications: local models may have inadequate accuracy, while the cloud may fail to meet delay constraints. In this paper, we present the novel idea of inference delivery networks (IDNs), networks of computing nodes that coordinate to satisfy ML inference requests achieving the best trade-off between latency and accuracy. IDNs bridge the dichotomy between device and cloud execution by integrating inference delivery at the various tiers of the infrastructure continuum (access, edge, regional data center, cloud). We propose a distributed dynamic policy for ML model allocation in an IDN by which each node dynamically updates its local set of inference models based on requests observed during the recent past plus limited information exchange with its neighboring nodes. Our policy offers strong performance guarantees in an adversarial setting and shows improvements over greedy heuristics with similar complexity in realistic scenarios.

4. No-Regret Caching via Online Mirror Descent

Abstract

We study an online caching problem in which requests can be served by a local cache to avoid retrieval costs from a remote server. The cache can update its state after a batch of requests and store an arbitrarily small fraction of each file. We study no-regret algorithms based on Online Mirror Descent (OMD) strategies. We show that bounds for the regret crucially depend on the diversity of the request process, provided by the diversity ratio R/h, where R is the size of the batch, and h is the maximum multiplicity of a request in a given batch. We characterize the optimality of OMD caching policies w.r.t. regret under different diversity regimes. We also prove that, when the cache must store the entire file, rather than a fraction, OMD strategies can be coupled with a randomized rounding scheme that preserves regret guarantees, even when update costs cannot be neglected. We provide a formal characterization of the rounding problem through optimal transport theory, and moreover we propose a computationally efficient randomized rounding scheme.

Abstract

Neurodegenerative diseases (NDDs) cause a progressive loss of neurons. Since NDDs are multifactorial, the precise etiology varies on the basis of the type of disease and patient history. Cohort studies and case studies have demonstrated a potential link between viral infections and the onset or progression of NDDs. Recent findings concerning the mechanisms by which neuropathic infections occur have provided more insights into the importance of such connections. In this review, we aim to elaborate on the occurrence of the neuropathic effects of viruses from epidemiological, clinical, and biological perspectives while highlighting potential treatments and challenges. One of the key players in viral neuropathogenesis is neuroinflammation caused by the immune response to the virus; this can occur due to both neurotropic and nonneurotropic viruses. The COVID-19 pandemic has raised concerns about whether vaccines are essential for preventing viruses or whether vaccines may play a part in exacerbating or accelerating NDDs. By classifying viruses and the common NDDs associated with them and further delving into their cellular pathways, this review provides insights to advance the development of potential treatments and diagnostic methods. 13. Epiretinal membrane after branch retinal vein occlusion: Separation after dexamethasone implant injection

Abstract

Purpose: The authors report a case of epiretinal membrane after branch retinal vein occlusion with macular wrinkling that was successfully managed without surgical treatment. Observations: A patient presented with complaints of a gradual decrease in vision, with metamorphopsia over the previous 2 years in the right eye. The patient was diagnosed with a grade 2 epiretinal membrane (ERM) with macular wrinkling in the right eye that was confirmed with optical coherence tomography (OCT) demonstrating significant macular thickening. The patient refused surgical intervention, and was managed with a dexamethasone implant to decrease macular edema. Six months later, the patient presented with resolution of macular edema, separation of the ERM from the retina as confirmed with OCT, and excellent visual recovery. Conclusions and Importance: A dexamethasone implant injection can be an alternative treatment for ERM resulting in excellent visual recovery and possibly, separation of ERM.

Abstract

Resistance to last resort drugs such as carbapenem and colistin is a serious global health threat. This study investigated carbapenem and colistin resistance in 583 non-duplicate Enterobacteriaceae isolates utilizing phenotypic methods and whole genome sequencing (WGS). Of the 583 isolates recovered from humans, animals and the environment in Nigeria, 18.9% (110/583) were resistant to at least one carbapenem (meropenem, ertapenem, and imipenem) and 9.1% (53/583) exhibited concurrent carbapenem-colistin resistance. The minimum inhibitory concentrations of carbapenem and colistin were 2–32 μg/mL and 8 to >64 μg/mL, respectively. No carbapenem resistant isolates produced carbapenemase nor harbored any known carbapenemase producing genes. WGS supported that concurrent carbapenem-colistin resistance was mediated by novel and previously described alterations in chromosomal efflux regulatory genes, particularly mgrB (M1V) ompC (M1_V24del) ompK37 (I70M, I128M) ramR (M1V), and marR (M1V). In addition, alterations/mutations were detected in the etpA, arnT, ccrB, pmrB in colistin resistant bacteria and ompK36 in carbapenem resistant bacteria. The bacterial isolates were distributed into 37 sequence types and characterized by the presence of internationally recognized high-risk clones. The results indicate that humans and animals in Nigeria may serve as reservoirs and vehicles for the global spread of the isolates. Further studies on antimicrobial resistance in African countries are warranted.

16. Refractory epilepsy and obstructive sleep apnea: is there an association?

Abstract

Abstract Background A high prevalence of obstructive sleep apnea (OSA) has been reported in medically refractory epilepsy patients, and there is increasing evidence that treatment of OSA in refractory epilepsy patients would reduce seizure frequency. Objective Studying the frequency of occurrence of OSA in a group of refractory epilepsy patients, compared to a matched group with medically controlled epilepsy. Methods Sixty patients with epilepsy, 30 patients with controlled epilepsy (group I), and 30 patients with refractory epilepsy (group II) were included. All patients underwent overnight polysomnogram and sleep EEG and completed Sleep Apnea Sleep Disorders Questionnaire (SASDQ). All patients had normal general and neurological examination. Results The frequency of OSA was found to be 10% in patients with controlled epilepsy, while its frequency in patients with refractory epilepsy was found to be 16.7%, yet this was statistically insignificant. In addition, O2 desaturation nadir was higher in group II, compared to group I with no statistical significance. In the controlled epilepsy group, we found that older patients tend to have higher Apnea–Hypopnea Index (AHI). Moreover, it was found that older subjects, with early onset of epilepsy, and longer duration of the illness tend to have higher AHI in group II. Conclusion There is an association between refractory epilepsy and OSA, especially regarding O2 desaturation during sleep. In addition, older age, early onset of epilepsy, and longer duration of illness are independent risk factors for the OSA in patients with refractory epilepsy.

Abstract

BackgroundSince the novel coronavirus emerged in late 2019, the scientific and public health community around the world have sought to better understand, surveil, treat, and prevent the disease, COVID-19. In sub-Saharan Africa (SSA), many countries responded aggressively and decisively with lockdown measures and border closures. Such actions may have helped prevent large outbreaks throughout much of the region, though there is substantial variation in caseloads and mortality between nations. Additionally, the health system infrastructure remains a concern throughout much of SSA, and the lockdown measures threaten to increase poverty and food insecurity for the subcontinent’s poorest residents. The lack of sufficient testing, asymptomatic infections, and poor reporting practices in many countries limit our understanding of the virus’s impact, creating a need for better and more accurate surveillance metrics that account for underreporting and data contamination. ObjectiveThe goal of this study is to improve infectious disease surveillance by complementing standardized metrics with new and decomposable surveillance metrics of COVID-19 that overcome data limitations and contamination inherent in public health surveillance systems. In addition to prevalence of observed daily and cumulative testing, testing positivity rates, morbidity, and mortality, we derived COVID-19 transmission in terms of speed, acceleration or deceleration, change in acceleration or deceleration (jerk), and 7-day transmission rate persistence, which explains where and how rapidly COVID-19 is transmitting and quantifies shifts in the rate of acceleration or deceleration to inform policies to mitigate and prevent COVID-19 and food insecurity in SSA. MethodsWe extracted 60 days of COVID-19 data from public health registries and employed an empirical difference equation to measure daily case numbers in 47 sub-Saharan countries as a function of the prior number of cases, the level of testing, and weekly shift variables based on a dynamic panel model that was estimated using the generalized method of moments approach by implementing the Arellano-Bond estimator in R. ResultsKenya, Ghana, Nigeria, Ethiopia, and South Africa have the most observed cases of COVID-19, and the Seychelles, Eritrea, Mauritius, Comoros, and Burundi have the fewest. In contrast, the speed, acceleration, jerk, and 7-day persistence indicate rates of COVID-19 transmissions differ from observed cases. In September 2020, Cape Verde, Namibia, Eswatini, and South Africa had the highest speed of COVID-19 transmissions at 13.1, 7.1, 3.6, and 3 infections per 100,0000, respectively; Zimbabwe had an acceleration rate of transmission, while Zambia had the largest rate of deceleration this week compared to last week, referred to as a jerk. Finally, the 7-day persistence rate indicates the number of cases on September 15, 2020, which are a function of new infections from September 8, 2020, decreased in South Africa from 216.7 to 173.2 and Ethiopia from 136.7 to 106.3 per 100,000. The statistical approach was validated based on the regression results; they determined recent changes in the pattern of infection, and during the weeks of September 1-8 and September 9-15, there were substantial country differences in the evolution of the SSA pandemic. This change represents a decrease in the transmission model R value for that week and is consistent with a de-escalation in the pandemic for the sub-Saharan African continent in general. ConclusionsStandard surveillance metrics such as daily observed new COVID-19 cases or deaths are necessary but insufficient to mitigate and prevent COVID-19 transmission. Public health leaders also need to know where COVID-19 transmission rates are accelerating or decelerating, whether those rates increase or decrease over short time frames because the pandemic can quickly escalate, and how many cases today are a function of new infections 7 days ago. Even though SSA is home to some of the poorest countries in the world, development and population size are not necessarily predictive of COVID-19 transmission, meaning higher income countries like the United States can learn from African countries on how best to implement mitigation and prevention efforts. 27. Traitement de la sarcoïdose cardiaque : étude observationnelle multicentrique chez 48 patients.

Abstract

La sarcoïdose est une granulomatose multisystémique rare touchant les adultes jeunes. L'atteinte cardiaque de la sarcoïdose, parfois inaugurale, est associée à un pronostic défavorable. Malgré une amélioration des connaissances sur la prise en charge de la sarcoïdose cardiaque, les recommandations récentes montrent que les données, notamment concernant les traitements, restent limitées, notamment en raison de la rareté de la maladie. L'objectif de ce travail était de décrire la présentation clinique, biologique et radiologique de l'atteinte cardiaque de sarcoïdose, et de décrire les traitements proposés dans les années récentes. Il s'agissait d'une étude rétrospective menée entre Janvier 2014 et Septembre 2024. Les critères d'inclusion étaient la présence d'une sarcoïdose définie selon les critères du WASOG avec une documentation histologique et une atteinte cardiaque retenue selon les recommandations (ref 1). Les données cliniques, biologiques, radiologiques, et de suivi ont été collectées. Un total de 48 dossiers de patients remplissant les critères d'inclusion ont été analysés : 30 hommes (63 %) d'âge moyen au diagnostic de sarcoïdose de 44,8 ans (écart type : 14,4 ans). Neuf patients (19 %) avaient une cardiopathie ischémique associée, et 14 (25 %) une intoxication tabagique sevrée ou active. Près de trois quarts des patients avaient une atteinte médiastinale ou parenchymateuse pulmonaire associée (n = 34, 72 %), suivies par une atteinte ganglionnaire (n = 23, 49 %) et hépatique ou splénique (n = 11, 23 %). Dix patients (21 %) avaient une hypercalcémie. L'atteinte cardiaque était diagnostiquée à un âge moyen de 50,5 ans (écart type : 12,9 ans). Elle était inaugurale de la maladie chez 20 patients (42 %), ou survenait avec un délai moyen de 68 mois (écart type : 165 mois) après le diagnostic. Les signes cliniques étaient dominés par la dyspnée (n = 21, 46 %) et les palpitations (n = 16, 35 %). L'électrocardiogramme était anormal chez 40 patients (89 %) avec principalement des troubles de conduction. Le traitement de l'atteinte cardiaque a consisté en une corticothérapie pour 79 % des patients (chez les 21 % restants, l'absence de corticothérapie était liée à une contre-indication, ou à une atteinte cardiaque mise en évidence à un stade tardif, sans signe d'activité), combinée à un traitement immunosuppresseur comportant pour la moitié des patients du méthotrexate et pour 16 patients de l'infliximab. Un défibrillateur implantable était indiqué pour 21 patients (48 %) et 2 patients ont bénéficié d'une transplantation cardiaque. Les biomarqueurs cardiaques étaient élevés (troponinémie moyenne : 75,5 ng/L, taux de NT-proBNP moyen : 713 ng/L). L'échocardiographie transthoracique objectivait une fraction d'éjection ventriculaire gauche moyenne à 47 % à la faveur d'une cardiomyopathie hypertrophique (n = 12, 48 %) ou dilatée (n = 10, 40 %). L'IRM cardiaque confirmait la discrète altération de la fonction ventriculaire gauche, mais également droite (fraction d'éjection ventriculaire droite moyenne à 36 %). Il existait un rehaussement tardif chez 80 % des patients. La tomographie par émissions de positons montrait un hypermétabolisme cardiaque dans 55 % des cas qui, quand il était présent, était associé à une fixation extracardiaque dans trois quarts des cas. La biopsie endomyocardique, réalisée chez 10 patients, offrait une rentabilité diagnostique de 70 %. Deux patients sont décédés subitement, alors qu'il étaient traités, sans autopsie réalisée. L'atteinte cardiaque de sarcoïdose est une atteinte souvent inaugurale, et sévère, avec une dysfonction ventriculaire associée fréquente. Abstract

Although several studies have explored factors associated with loss to follow-up (LTFU) from HIV care, there remains a gap in understanding how these factors vary by setting, volume of patient and patients' demographic and clinical characteristics. We determined rates and factors associated with LTFU in HIV care Lilongwe, Malawi.We conducted a retrospective cohort study of HIV-infected individuals aged 15 years or older at the time of registration for HIV care in 12 ART facilities, between April 2012 and March 2013. HIV-positive individuals who had not started ART (pre-ART patients) were clinically assessed to determine ART eligibility at registration and during clinic follow-up visits. ART-eligible patients were initiated on triple antiretroviral combination. Study data were abstracted from patients' cards, facility ART registers or electronic medical record system from the date of registration for HIV care to a maximum follow-up period of 24 months. Descriptive statistics were undertaken to summarize characteristics of the study patients. Separate univariable and multivariable poisson regression models were used to explore factors associated with LTFU in pre-ART and ART care.A total of 10,812 HIV-infected individuals registered for HIV care. Of these patients, 1,907 (18%) and 8,905 (82%) enrolled in pre-ART and ART care, respectively. Of the 1,907 pre-ART patients, 490 (26%) subsequently initiated ART and were included in both the pre-ART and ART analyses. The LTFU rates among patients in pre-ART and ART care were 48 and 26 per 100 person-years, respectively. Of the 9,105 ART patients with reasons for starting ART, 2,451 (27%) were initiated on ART because of pregnancy or breastfeeding (Option B+) status. Multivariable analysis showed that being ≥35 years and female were associated with decreased risk of LTFU in the pre-ART and ART phases of HIV care. However, being in WHO clinical stage 3 (adjusted risk ratio (aRR) 1.35, 95% confidence interval (CI): 1.20-1.51) and stage 4 (aRR 1.87, 95% CI: 1.62-2.18), body mass index ≤ 18.4 (aRR 1.24, 95% CI: 1.11-1.39) at ART initiation, poor adherence to clinic appointments (aRR 4.55, 95% CI: 4.16-4.97) and receiving HIV care in rural facilities (aRR 2.32, 95% CI: 1.94-2.87) were associated with increased risk of LTFU among ART patients. Being re-initiated on ART once (aRR 0.20, 95% CI: 0.17-0.22), more than once (aRR 0.06, 95% CI: 0.05-0.07), and being enrolled at a low-volume facility (aRR 0.25, 95% CI: 0.20-0.30) were associated with decreased risk of LTFU from ART care.A sizeable proportion of ART LTFU occurred among women enrolled during pregnancy or breast-feeding. Non- compliance to clinic and receiving ART in a rural facility or high-volume facility were associated with increased risk of LTFU from ART care. Developing effective interventions that target high-risk subgroups and contexts may help reduce LTFU from HIV care.

Abstract

This work was held in South Port Said in the growing season 2015-2016 to study the effect of the use of organic manure and mineral nitrogen fertilization on the production of sugar beet (Beta vulgaris L.) in a salt affected soil. Thirty four different treatments were performed on sugar beets between organic manure, mineral N fertilization and mixture between them. The growing plant parameters were recorded in terms of root yield (tons/fad) and total sugars percent, the best results were obtained using mineral nitrogen, the second treatment (T280 kg fed), because it exceeded the total sugar production .Samples of plants were analyzed after harvest that included estimating nitrogen concentrations. Where the results showed a response in most of them for nitrogenous additives and organic manuring.

Abstract

Les anticorps anti-tumor necrosis factor-α (anti-TNF-α), en particulier l'infliximab, sont actuellement proposés comme médicaments de troisième ligne dans le traitement de la sarcoïdose. Malgré cela, certains patients ne s'améliorent pas ou rechutent au cours du suivi. Une explication pourrait être le développement d'anticorps anti-infliximab. Nous avons évalué l'incidence et l'impact des anticorps anti-infliximab dans la sarcoïdose avec au moins une atteinte extra-thoracique, chez les patients traités par infliximab. Il s'agissait d'une étude rétrospective menée dans un centre entre janvier 2015 et décembre 2023. Les critères d'inclusion étaient (1) la présence d'une sarcoïdose extrathoracique documentée histologiquement (critères WASOG) ; (2) un traitement par inflixmab ; (3) au moins une détermination d'anticors anti-infliximab au cours du suivi. La détermination des anticorps antiinfliximab était réalisée par le même laboratoire, en même temps que le dosage résiduel d'infliximab, après au moins 2 perfusions de traitement, et juste avant une injection. Parmi 91 patients ayant une sarcoïdose extrathoracique traitée par infliximab, 51 dossiers de patients remplissant les critères d'inclusion ont été inclus : 35 hommes (69 %), d'âge median au diagnostic de sarcoïdose 39 ans interquartile [33 ; 47], avec en médiane 4 organes atteints [2-4] et une forte prévalence des atteintes neurologiques (n = 35, 69 %) et cardiaque (n = 10, 20 %). Cinquante patients sur 51 (98 %) avaient reçu des corticostéroïdes avant l'initiation de l'infliximab et 48 (94 %) avaient été traités avec un agent immunosuppresseur avant l'introduction d'infliximab. Le temps de suivi médian entre l'initiation de l'infliximab et la détermination de la présence d'anticorps anti-infliximab était de 1,6 ans [0,67 ; 3,75]. La durée médiane (IQR) de traitement par infliximab était de 3,8 ans [3,04 ; 7,33]. L'évolution clinique sous infliximab, évaluée par l' "extrapulmonary physician organ severity tool score" (ePOST) et le "clinical outcome status" (COS) était significativement favorable pour l'ensemble de la cohorte (p < 0,001). Vingt-cinq patients (49 %) avaient une diminution de la charge lésionnelle supérieure à 50 %, évaluée par le Physician global assessment (PGA) en fin de suivi. Parmi les 26 patients qui ont été évalués par 18F-fluoro-desoxyglucose tomographie par émission de positrons (18FDG-TEP-scanner), à l'introduction de l'infliximab et en fin de suivi, 25 (96 %) se sont améliorés. Neuf patients (18 %) ont développé des anticorps anti-infliximab au cours du suivi. Il n'y avait pas de différence comparativement au patients sans anticorps anti-infliximab sur les caractéristiques initiales, les traitements reçus avant ou pendant l'infliximab (en particulier le pourcentage de patients ayant un immunosuppresseur et une corticothérapie associée à l'infliximab), ni les scores d'activité au cours du suivi. On notait toutefois dans le groupe avec anticorps anti-infliximab une utilisation du methotrexate dans 67 % des cas (et 83 % dans le groupe sans anticorps anti-infliximab). La concentration résiduelle d'infliximab était par ailleurs significativement plus basse chez les patients ayant des anticorps anti-infliximab positifs que chez ceux n'en ayant pas (0 μg/mL [0 ;0] et 13,6 μg/mL [5,4 ; 20,0]) restpectivement (p < 0,001). Vingt-trois patients (45 %) ont eu au moins une infection au cours du suivi. Neuf patients (18 %) ont développé des anticorps anti-infliximab au cours du suivi. Ces derniers ne semblent pas avoir d'impact sur l'évolution sous traitement par anti-TNF- α. Néanmoins, l'échantillon de petite taille et le suivi limité dans le temps nécessitent d'obtenir des éléments de suivi à plus long terme, d'autant que la concentration résiduelle d'infliximab était plus basse chez les patients ayant des anticorps anti-infliximab. Abstract

Extended duration extracorporeal membrane oxygenation (ECMO), using dual-lumen cannulas, is being used with increased frequency to support patients, including those with COVID-19; both as a bridge to transplant and lung recovery. During such an extended duration of support, there are several factors that might lead to the attrition of the physical structure of the ECMO cannulas, predisposing them to the risk of fracture. Although rare, fracture of the ECMO cannula can be a potentially lethal event. Here, we present a case where fracture of a dual lumen cannula during veno-venous (VV) ECMO support resulted in a cerebrovascular accident. We discuss the potential contributing factors and suggest steps to mitigate the risks for such a complication.

ObjectivesIn Malawi, a recent infection testing algorithm (RITA) is used to characterise infections of persons newly diagnosed with HIV as recent or long term. This paper shares results from recent HIV infection surveillance and describes distribution and predictors.SettingData from 155 health facilities in 11 districts in Malawi were pooled from September 2019 to March 2020.ParticipantsEligible participants were ≥13 years, and newly diagnosed with HIV. Clients had RITA recent infections if the rapid test for recent infection (RTRI) test result was recent and viral load (VL) ≥1000 copies/mL; if VL was Results13 838 persons consented to RTRI testing and 12 703 had valid RTRI test results and VL results after excluding clients not newly HIV-positive, RTRI negative or missing data (n=1135). A total of 12 365 of the 12 703 were included in the analysis after excluding those whose RTRI results were reclassified as long term (n=338/784 or 43.1%). The remainder, 446/12 703 or 3.5%, met the definition of RITA recent infection. The highest percentage of recent infections was among breastfeeding women (crude OR (COR) 3.2; 95% CI 2.0 to 5.0), young people aged 15–24 years (COR 1.6; 95% CI 1.3 to 1.9) and persons who reported a negative HIV test within the past 12 months (COR 3.3; 95% CI 2.6 to 4.2). Factors associated with recent infection in multivariable analysis included being a non-pregnant female (adjusted OR (AOR) 1.4; 95% CI 1.2 to 1.8), a breastfeeding female (AOR 2.2; 95% CI 1.4 to 3.5), aged 15–24 years (AOR 1.6; 95% CI 1.3 to 1.9) and residents of Machinga (AOR 2.0; 95% CI 1.2 to 3.5) and Mzimba (AOR 2.4; 95% CI 1.3 to 4.5) districts.ConclusionsMalawi’s recent HIV infection surveillance system demonstrated high uptake and identified sub-populations of new HIV diagnoses with a higher percentage of recent infections.

Abstract

In low- and middle-income countries (LMICs), 93% of the population lacks safe, timely, and affordable access to cardiac surgical care when needed. As countries slowly build or expand local, independent cardiac centers, non-governmental organizations (NGOs) partially bridge the gap in cardiac surgical care delivery in LMICs. However, little is known about the current scope of cardiac NGOs. Here, we perform an analysis of active NGOs involved with the delivery of cardiac surgical services in LMICs or for patients from LMICs. Cardiac surgery NGOs were identified from medical literature, established NGO databases, and Google Scholar searches. The search was performed between December 2019 and May 2020. NGOs whose websites were not updated or described missions or projects taking place no later than 2015 were considered inactive. Eighty-six NGOs are actively providing cardiac surgery services in LMICs or treating patients from LMICs. Five NGOs performed adult cardiac surgery only, 56 performed pediatric cardiac surgery only, and 25 performed both adult and pediatric cardiac surgery. NGOs originated from 23 different countries and were operational in a total of 111 countries, 96 of them being LMICs. Fifty-three NGOs reported data on annual surgical volume, of which half performed less than 50 operations per year. NGOs effectively address the burden of cardiac surgical disease in LMICs and contribute to local capacity-building. Increased, more detailed, and standardized reporting of the impact and outcomes of NGOs is necessary to better understand annual cardiac surgical volume and to support local centers working towards independent services.
