Abstract: PANDA (anti-proton annihiliation at Darmstadt) is planned to be one of the four main experiments at the future international accelerator complex FAIR (Facility for Antiproton and Ion Research) in Darmstadt, Germany. It is going to address fundamental questions of hadron physics and quantum chromodynamics using cooled antiproton beams with a high intensity and and momenta between 1.5 and 15 GeV/c. PANDA is designed to reach a maximum luminosity of 2 × 1032 cm−2 s. Most of the physics programs require an excellent particle identification (PID). The PID of hadronic states at the forward endcap of the target spectrometer will be done by a fast and compact Cherenkov detector that uses the detection of internally reflected Cherenkov light (DIRC) principle. It is designed to cover the polar angle range from 5° to 22° and to provide a separation power for the separation of charged pions and kaons up to 3 standard deviations (s.d.) for particle momenta up to 4 GeV/c in order to cover the important particle phase space. This document describes the technical design and the expected performance of the novel PANDA disc DIRC detector that has not been used in any other high energy physics experiment before. The performance has been studied with Monte-Carlo simulations and various beam tests at DESY and CERN. The final design meets all PANDA requirements and guarantees sufficient safety margins.

Abstract: The antiproton experiment PANDA at FAIR is designed to bring hadron physics to a new level in terms of scope, precision and accuracy. In this work, its unique capability for studies of hyperons is outlined. We discuss ground-state hyperons as diagnostic tools to study non-perturbative aspects of the strong interaction, and fundamental symmetries. New simulation studies have been carried out for two benchmark hyperon-antihyperon production channels: $\bar{p}p \to \bar{\Lambda}\Lambda$ and $\bar{p}p \to \bar{\Xi}^+\Xi^-$. The results, presented in detail in this paper, show that hyperon-antihyperon pairs from these reactions can be exclusively reconstructed with high efficiency and very low background contamination. In addition, the polarisation and spin correlations have been studied, exploiting the weak, self-analysing decay of hyperons and antihyperons. Two independent approaches to the finite efficiency have been applied and evaluated: one standard multidimensional efficiency correction approach, and one efficiency independent approach. The applicability of the latter was thoroughly evaluated for all channels, beam momenta and observables. The standard method yields good results in all cases, and shows that spin observables can be studied with high precision and accuracy already in the first phase of data taking with PANDA., Comment: 26 pages, 16 figures. Abstract: The $\overline{{\rm{P}}}\mathrm{ANDA}$ (anti-Proton ANnihiliation at DArmstadt) experiment will be one of the four flagship experiments at the new international accelerator complex FAIR (Facility for Antiproton and Ion Research) in Darmstadt, Germany. $\overline{{\rm{P}}}\mathrm{ANDA}$ will address fundamental questions of hadron physics and quantum chromodynamics using high-intensity cooled antiproton beams with momenta between 1.5 and 15 GeV/c and a design luminosity of up to 2 × 1032 cm−2 s−1. Excellent particle identification (PID) is crucial to the success of the $\overline{{\rm{P}}}\mathrm{ANDA}$ physics program. Hadronic PID in the barrel region of the target spectrometer will be performed by a fast and compact Cherenkov counter using the detection of internally reflected Cherenkov light (DIRC) technology. It is designed to cover the polar angle range from 22° to 140° and will provide at least 3 standard deviations (s.d.) π/K separation up to 3.5 GeV/c, matching the expected upper limit of the final state kaon momentum distribution from simulation. This documents describes the technical design and the expected performance of the $\overline{{\rm{P}}}\mathrm{ANDA}$ Barrel DIRC detector. The design is based on the successful BaBar DIRC with several key improvements. The performance and system cost were optimized in detailed detector simulations and validated with full system prototypes using particle beams at GSI and CERN. The final design meets or exceeds the PID goal of clean π/K separation with at least 3 s.d. over the entire phase space of charged kaons in the Barrel DIRC.

Abstract: Bound nuclear systems with two units of strangeness are still poorly known despite their importance for many strong interaction phenomena. Stored antiprotons beams in the GeV range represent an unparalleled factory for various hyperon-antihyperon pairs. Their outstanding large production probability in antiproton collisions will open the floodgates for a series of new studies of systems which contain two or even more units of strangeness at the PANDA experiment at FAIR. For the first time, high resolution gamma-spectroscopy of doubly strange Lambda Lambda-hypernuclei will be performed, thus complementing measurements of ground state decays of Lambda Lambda-hypernuclei at J-PARC or possible decays of particle unstable hypernuclei in heavy ion reactions. High resolution spectroscopy of multistrange Xi(-) -atoms will be feasible and even the production of Omega(-) -atoms will be within reach. The latter might open the door to the vertical bar S vertical bar = 3 world in strangeness nuclear physics, by the study of the hadronic Omega(-) -nucleus interaction. Abstract: The exclusive charmonium production process in (P) over barp annihilation with an associated pi 0 meson (p) over barp -> J/psi pi(0) is studied in the framework of QCD collinear factorization. The feasibility of measuring this reaction through the J/psi -> e(+) e(-) decay channel with the AntiProton ANnihilation at DArmstadt ((P) over bar ANDA) experiment is investigated. Simulations on signal reconstruction efficiency as well as the background rejection from various sources including the (P) over barp -> pi(+)pi(-)pi(0) and (p) over barp -> J/psi pi(0)pi(0) reactions are performed with PANDAROOT, the simulation and analysis software framework of the (P) over bar ANDA experiment. It is shown that the measurement can be done at (P) over bar ANDA with significant constraining power under the assumption of an integrated luminosity attainable in four to five months of data taking at the maximum design luminosity.

6. Evaluation of Wheat Residual Effects in Rotation on Emergence, Leaf Area, and Yield Components of Corn (Zea mays L.) Cultivars

Abstract: This research was carried out as a factorial based on a randomized complete block design with four replications at greenhouse conditions. First factor was corn cultivars (D.C370, S.C647 and S.C704) and second factor included 3 levels of residual plant (without residual, root residual, root and shoot residual of wheat). The results showed that percent and mean emergence time, leaf area, biologic and kernel yield, number of kernels per ear, number of kernels per row and ear length was influenced of wheat residual, and maximum positive effects on mentioned traits, obtained at root and shoot residual of wheat. 100-kernel weight and plant height of cultivars showed significant difference. Effect of residual plant on biologic yield, kernel yield, number of kernels per ear, number of kernels per row of S.C704 wasn’t significant, but other hybrids showed significant difference to residual wheat. 100-kernel weight, row number per ear, ear length and plant height of D.C370 was less than other hybrids, significantly.

Abstract: Cystic echinococcosis, caused by the larval stage of Echinoccocus granulosus, is a global public health threat. The aim of this study was to determine the demographic and clinical characteristics of cases of primary and secondary hydatid cyst in the west of the Islamic Republic of Iran. A retrospective record review was made in 7 public and private hospitals in Kermanshah from 2004 to 2009. Of 526 files of suspected cases analysed, 291 cases of hydatid cyst had been operated upon and confirmed. Of the patients, 51.5% were females, 57.7% lived in urban and suburban areas and the largest age group (17.5%) was 31-40 years. The major organs involved were liver (64.9%) and lung (26.8%) and 18 cases (6.2%) were confirmed as secondary hydatidosis. Most patients (79.4%) had been treated with albendazole. Kermanshah province should be considered an endemic region for human hydatid cyst disease; greater efforts in prevention are needed.دراسة حول الكيسات العدارية الأولية أو الثانوية )الناكسة( التي أجريت لها عمليات جراحية في مستشفيات كرمان شاه، غربيّ جمهورية إيران الإسلاميةمهدي وجداني، سينا وجداني، سارة لطفي، فريد نجفي، نصر نزاري، يزدان حمزاويإن داء المُشْوِكات الكيسي الذي تسببه المرحلة اليََقية من ديدان المشوكة الحبيبية يمثِّل أحد التهديدات للصحة العمومية العالمية. وتهدف هذه الدراسة إلى التعرُّف إلى السمات الديمغرافية والسريرية لحالات الكيسات العدارية الأولية والثانوية في غربي جمهورية إيران الإسلامية. وقد أجرى الباحثون استعراضاً استعادياً للسجلات في سبعة مستشفيات تابعة للقطاعَيْ العام والخاص في كرمان شاه في الفترة بين عامَيْ 2004و2009؛ وشملت الدراسة تحليل 529 ملفاً لحالات مشتبه بها، ووجدوا أن 291 حالة كيسة عدارية قد أجري لها عمليات جراحية وتم تأكيدها. وقد كان 51.5% من المرضى من الإناث، و 57.7 منهم يعيشون في المدن أو الضواحي، وكانت أكبر المجموعات العمرية تشكيلاً لنسبة 17.5% من الحالات هي 31-40 عاماً. أما أكثر الأعضاء إصابة فكان الكبد 64.9% ثم الرئتين 26.8%، وقد تأكَّد النكس الثانوي لدى 18 حالة (6.2%). وقد تلقى معظم المرضى (79.4%) وليس كلهم المعالجة بالألبندازول. وانتهى الباحثون إلى وجوب اعتبار ولاية كرمان شاه منطقة موطونة بمرض الكيسات العدارية البشري، ووجوب بذل المزيد من الجهود الوقائية.Étude de l'hydatidose primaire et secondaire (récidive) opérée dans des hôpitaux de Kermanshah, dans l'ouest de la République islamique d'Iran.L'échinococcose cystique, causée par le stade larvaire d'Echinoccocus granulosus, représente une menace pour la santé publique mondiale. L'objectif de la présente étude était de déterminer les caractéristiques démographiques et cliniques des cas de kyste hydatique primaire et secondaire dans l'ouest de la République islamique d'Iran. Une revue rétrospective des dossiers a été menée dans sept hôpitaux publics et privés de Kermanshah entre 2004 et 2009. Sur un total de 526 dossiers de cas suspects analysés, 291 cas de kyste hydatique avaient été opérés et confirmés. Les patients étaient 51,5 % à être de sexe féminin, 57,7 % vivaient dans des zones urbaines ou suburbaines et le groupe d'âge le plus nombreux (17,5 %) était celui des 31-40 ans. Les principaux organes touchés étaient le foie (64,9 %) et les poumons (26,8 %) et 18 cas (6,2 %) ont été confirmés comme hydatidose secondaire. La plupart des patients (79,4 %) avaient été traités par albendazole. La province de Kermanshah devrait être considérée comme une région d'endémie pour le kyste hydatique humain ; des actions de prévention renforcées sont nécessaires.

8. Investigation of Tolerance, Yield and Yield Components of Wheat Cultivars to Salinity of Irrigation Water at Sensitive Stages of Growth

Abstract: This research in order to study of tolerance ability of wheat cultivates yield and yield components to salinity of irrigation water at sensitive stages of growth, was carried out as a factorial based on a randomized complete block design with 3 replications at greenhouse of Agricultural Faculty of Bu-Ali Sina University, in 2009. Treatments were included wheat cultivars of Alvand, Tous, Sayson and Navid and salinity of irrigation water induced by sodium chloride at five levels of 0, 4, 8, 12 and 16 dS m-1. The results showed that percentage and rate of emergence, plant height, 1000-grain weight, number of seed per spike, number of spike per pot, biological and grain yield reduced by increasing salinity level. At all stress levels Navid cv. had highest emergence percentage. In non-stress and 4 dS m-1, Alvand cv. and at higher levels of stress, Tous cv. had high height in reproductive phase. At control and 4 dS m-1, Sayson cv. and at 8, 12 and 16 dS m-1, Tous cv. in majority of yield and yield components traits had significant superior than other cultivars. Tolerance index of Sayson cv. at 4 and 8 dS m-1 was more than other cultivars but at 12 and 16 dS m-1, maximum value of this index was belonged to Tous cv. At all salinity levels, Alvand cv. had least tolerance index to stress. Number of spike per pot had maximum direct effect on grain yield of wheat cultivars in stress condition. Also indirect effect of biological yield via number of spike per pot than other its indirect effects, had maximum share in wheat seed yield.

Abstract: Baryon-to-meson Transition Distribution Amplitudes (TDAs) encoding valuable new information on hadron structure appear as building blocks in the collinear factorized description for several types of hard exclusive reactions. In this paper, we address the possibility of accessing nucleon-to-pion ($��N$) TDAs from $\bar{p}p \to e^+e^- ��^0$ reaction with the future PANDA detector at the FAIR facility. At high center of mass energy and high invariant mass squared of the lepton pair $q^2$, the amplitude of the signal channel $\bar{p}p \to e^+e^- ��^0$ admits a QCD factorized description in terms of $��N$ TDAs and nucleon Distribution Amplitudes (DAs) in the forward and backward kinematic regimes. Assuming the validity of this factorized description, we perform feasibility studies for measuring $\bar{p}p \to e^+e^- ��^0$ with the PANDA detector. Detailed simulations on signal reconstruction efficiency as well as on rejection of the most severe background channel, i.e. $\bar{p}p \to ��^+��^- ��^0$ were performed for the center of mass energy squared $s = 5$ GeV$^2$ and $s = 10$ GeV$^2$, in the kinematic regions $3.0 < q^2 < 4.3$ GeV$^2$ and $5 < q^2 < 9$ GeV$^2$, respectively, with a neutral pion scattered in the forward or backward cone $| \cos��_{��^0}| > 0.5 $ in the proton-antiproton center of mass frame. Results of the simulation show that the particle identification capabilities of the PANDA detector will allow to achieve a background rejection factor of $5\cdot 10^7$ ($1\cdot 10^7$) at low (high) $q^2$ for $s=5$ GeV$^2$, and of $1\cdot 10^8$ ($6\cdot 10^6$) at low (high) $q^2$ for $s=10$ GeV$^2$, while keeping the signal reconstruction efficiency at around $40\%$. At both energies, a clean lepton signal can be reconstructed with the expected statistics corresponding to $2$ fb$^{-1}$ of integrated luminosity. 10. Ultrasound-guided renal puncture followed by endoscopically guided tract dilatation vs standard fluoroscopy-guided percutaneous nephrolithotomy for non-opaque renal stones; a randomized clinical trial.

Abstract: This study was designed to evaluate the non-inferiority of ultrasound puncture followed by endoscopically guided tract dilatation compared to the standard fluoroscopy-guided PCNL. Forty patients with non-opaque kidney stones eligible for PCNL were randomly divided into two groups. The standard fluoroscopy-guided PCNL using the Amplatz dilator was performed in the XRAY group. In the SONO group, the Kidney was punctured under an ultrasound guide followed by tract dilatation using a combination of the Amplatz dilator based on the tract length and an endoscopically guided tract dilatation using a bi-prong forceps in cases of short-advancement. The primary outcome was successful access. In 90% of cases in the XRAY and 95% in the SONO group access dilatation process was performed uneventfully at the first attempt (p = 0.5). In 45% of cases in the SONO group, bi-prong forceps were used as salvage for short-advancement. In one case in the X-ray group over-advancement occurred. One month after surgery, the stone-free rate on the CT-scan was 75% for the X-ray group and 85% for the SONO group (p = 0.4). There were no significant differences in operation time, hospitalization duration, transfusion, or complication rates between the two groups. We conclude that ultrasound-guided renal puncture, followed by endoscopically guided tract dilatation can achieve a high success rate similar to X-ray-guided PCNL while avoiding the harmful effects of radiation exposure and the risk of over-advancement., (© 2024. 11. A dosiomics model for prediction of radiation-induced acute skin toxicity in breast cancer patients: machine learning-based study for a closed bore linac.

Abstract: Background: Radiation induced acute skin toxicity (AST) is considered as a common side effect of breast radiation therapy. The goal of this study was to design dosiomics-based machine learning (ML) models for prediction of AST, to enable creating optimized treatment plans for high-risk individuals., Methods: Dosiomics features extracted using Pyradiomics tool (v3.0.1), along with treatment plan-derived dose volume histograms (DVHs), and patient-specific treatment-related (PTR) data of breast cancer patients were used for modeling. Clinical scoring was done using the Common Terminology Criteria for Adverse Events (CTCAE) V4.0 criteria for skin-specific symptoms. The 52 breast cancer patients were grouped into AST 2 + (CTCAE ≥ 2) and AST 2 - (CTCAE < 2) toxicity grades to facilitate AST modeling. They were randomly divided into training (70%) and testing (30%) cohorts. Multiple prediction models were assessed through multivariate analysis, incorporating different combinations of feature groups (dosiomics, DVH, and PTR) individually and collectively. In total, seven unique combinations, along with seven classification algorithms, were considered after feature selection. The performance of each model was evaluated on the test group using the area under the receiver operating characteristic curve (AUC) and f1-score. Accuracy, precision, and recall of each model were also studied. Statistical analysis involved features differences between AST 2 - and AST 2 + groups and cutoff value calculations., Results: Results showed that 44% of the patients developed AST 2 + after Tomotherapy. The dosiomics (DOS) model, developed using dosiomics features, exhibited a noteworthy improvement in AUC (up to 0.78), when spatial information is preserved in the dose distribution, compared to DVH features (up to 0.71). Furthermore, a baseline ML model created using only PTR features for comparison with DOS models showed the significance of dosiomics in early AST prediction. By employing the Extra Tree (ET) classifiers, the DOS + DVH + PTR model achieved a statistically significant improved performance in terms of AUC (0.83; 95% CI 0.71-0.90), accuracy (0.70), precision (0.74) and sensitivity (0.72) compared to other models., Conclusions: This study confirmed the benefit of dosiomics-based ML in the prediction of AST. However, the combination of dosiomics, DVH, and PTR yields significant improvement in AST prediction. Abstract: Direct reprogramming of glial cells into induced-neurons is a promising strategy for CNS repair after acute injury or neurodegenerative diseases. Grey matter astrocytes, which exhibit features of neural stem cells when activated, are an ideal cell source for direct neuronal conversion. The aim of the study is the investigation of the neuronal reprogramming capacity of CEND1 and/or Neurogenin-2 ( NEUROG2 ) upon their overexpression on primary human adult cortical astrocytes. Our data indicate that adult human cortical astrocytes can be directly reprogrammed by either CEND1 or NEUROG2 to cells with differentiated neuronal morphology, exhibiting long neurites and branched processes. Exploration of gene expression dynamics along the conversion process revealed that neuronal genes are significantly up-regulated while astrocytic genes are down-regulated. Differentiated induced-neurons (iNs) exhibit either GABAergic or glutamatergic/dopaminergic identity upon CEND1 and NEUROG2 overexpression respectively. Co-expression of CEND1 and NEUROG2 in double-transduced cultures induced elevated expression levels of neural progenitor/stem genes and appearance of highly proliferative spheres with neural progenitor cell (NPC) properties in culture.

13. Exploring the interactions of a Tb(III)-quercetin complex with serum albumins (HSA and BSA): spectroscopic and molecular docking studies.

Abstract: Serum albumins (human serum albumin (HSA) and bovine serum albumin (BSA), two main circulatory proteins), are globular and monomeric macromolecules in plasma that transport many drugs and compounds. In the present study, we investigated the interactions of the Tb(III)-quercetin (Tb-QUE) complex with HSA and BSA using common spectroscopic techniques and a molecular docking study. Fluorescence data revealed that the inherent fluorescence emission of HSA and BSA was markedly quenched by the Tb-QUE complex through a static quenching mechanism, confirming stable complex formation (a ground-state association) between albumins and Tb-QUE. Binding and thermodynamic parameters were obtained from the fluorescence spectra and the related equations at different temperatures under biological conditions. The binding constants (K b ) were calculated to be 0.8547 × 10 3 M -1 for HSA and 0.1363 × 10 3 M -1 for BSA at 298 K. Also, the number of binding sites (n) of the HSA/BSA-Tb-QUE systems was obtained to be approximately 1. Thermodynamic data calculations along with molecular docking results indicated that electrostatic interactions have a main role in the binding process of the Tb-QUE complex with HSA/BSA. Furthermore, molecular docking outputs revealed that the Tb-QUE complex has high affinity to bind to subdomain IIA of HSA and BSA. Binding distances (r) between HSA-Tb-QUE and BSA-Tb-QUE systems were also calculated using the Forster (fluorescence resonance energy transfer) method. It is expected that this study will provide a pathway for designing new compounds with multiple beneficial effects on human health from the phenolic compounds family such as the Tb-QUE complex., (© 2019 John Wiley & Sons, Ltd.)

Abstract: Background: Anxiety and sleep disorders are prevalent problems in patients presenting with ST-segment-elevation myocardial infarction (STEMI). Usually, these problems are managed by benzodiazepines, which-albeit effective-could cause adverse effects and drug interaction., Objective: This study was designed to compare the effects of melatonin and oxazepam in the management of anxiety and insomnia on patients following primary percutaneous coronary intervention (PCI) with a view to providing a safer alternative., Methods: This study was designed as a randomized clinical trial. STEMI patients managed with primary PCI were enrolled and randomized into 2 groups through the permuted block randomization. The patients received either oxazepam (10 mg) or melatonin (3 mg) every night. Autoimmune disease or previous use of psychoactive medications was considered the exclusion criterion. Levels of anxiety and sleep quality were evaluated using the Hamilton Anxiety Rating Scale (HAM-A) and the Groningen Sleep Quality Score and compared between the groups., Results: Each group contained 20 patients. Melatonin showed a significant advantage over oxazepam in improving sleep quality ( P = 0.040). Comparisons of the efficacy of both medications in lowering the anxiety levels when considering all the items of the HAM-A, including those related to cardiovascular disease, were significantly in favor of melatonin ( P = 0.019)., Conclusions and Relevance: The results of this study suggest that melatonin, a drug with more favorable drug interaction and adverse effect profile, could be more effective than oxazepam in improving the sleep quality and anxiety levels of patients presenting with STEMI, and it could be considered a new alternative to benzodiazepines in this setting.

15. Evaluating the Potential Effect of Melatonin on the post-Cardiac Surgery Sleep Disorder.

Abstract: Background: Postoperative neurological injuries, including cognitive dysfunction, sleep disorder, delirium, and anxiety, are the important consequences of coronary artery bypass graft surgery (CABG). Evidence has shown that postoperative sleep disturbance is partly due to disturbed melatonin secretion in the perioperative period. The aim of this study was to evaluate the effect of melatonin on postoperative sleep disorder in patients undergoing CABG., Method: One hundred forty-five elective CABG patients participated in a randomized double-blind study during the preoperative period. The patients were randomized to receive either 3 mg of melatonin or 10 mg of Oxazepam one hour before sleep time. Each group received the medication from 3 days before surgery until the time of discharge. Sleep quality was evaluated using the Groningen Sleep Quality Score (GSQS), and the incidence of delirium was evaluated by nursing records. Sleep quality and anxiety scores were compared before and after surgery through the Wilcoxon signed-rank test. The analysis of covariance (ANCOVA) and independent t-test were used to compare the sleep and anxiety scores between the groups. P values ≤ 0.05 were considered statistically significant., Results: Totally, 137 patients at a mean age of 60 years completed the study (76% male). The analysis of the data showed that sleep was significantly disturbed after surgery in both groups. The patients in the Oxazepam group demonstrated significantly higher disturbance in their mean postoperative GSQS score than did their counterparts in the melatonin group (p value < 0.001). A smaller proportion of the participants experienced delirium in the melatonin group (0.06%) than in the Oxazepam group (0.12%); however, this difference was not statistically significant., Conclusion: The result of the present study revealed that melatonin improved sleep in post-cardiac surgery patients more than what was observed with Oxazepam. Therefore, melatonin may be considered an effective alternative for Benzodiazepines in the management of postoperative sleep disorder.

16. Effect of Melatonin on Cardiac Injury after Primary Percutaneous Coronary Intervention: a Randomized Controlled Trial.

Abstract: Several studies have reported that the antioxidant properties of melatonin can provide cardiac protection through scavenging of free radicals. This study sought to investigate the efficacy of melatonin on cardiac biomarkers, myocardial-specific protein high sensitive troponin-T (hs-TnT) and creatine kinase-MB (CK-MB), in patients with ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (pPCI). In this randomized clinical trial, a total of 40 patients with STEMI planned to undergo pPCI were randomly assigned to two groups of receiving melatonin plus standard treatment [n=20] and control group, receiving only standard therapy [n=20]. The following parameters including hsTnT and CK-MB were assessed preoperatively (baseline) and at 6 hours after procedure. Melatonin could significantly reduce the level of CK-MB (118.2 ± 21.09 IU/L in the treated group versus 198.24 ± 20.94 IU/L in the control group; p-value = 0.01). However, there was no difference in the mean hs-TnT level between two groups (2491 ± 664 μg/L vs. 2801 ± 620 μg/L; p value = 0.73). Our results revealed that melatonin can be considered as a safe adjunctive medication to the standard regimen after pPCI for the aim of decreasing cardiovascular events. Meanwhile, this was a pilot study with a small number of patients and further studies are needed to confirm the beneficial effect of melatonin in patients with STEMI.

17. Immunofluorescence assay and PCR analysis of cryptosporidium oocysts and species from human feacal specimens.

Abstract: Background: Cryptosporidium spp. is a widespread protozoan parasite involving humans and animals., Objectives: This study was carried out to evaluate the immunofluorescence assay (IFA) and PCR results for more accurate diagnosis of faecal specimens., Patients and Methods: Forty six faecal human specimens of Cryptosporidium oocysts were examined by PCR and IFA in Calgary, Canada. In statistical analysis, sensitivity and positive predictive value were detected by IFA., Results: Among 46 faecal samples, 9 (19.6%) were IFA-positive and 10 (21.7%) PCR-positive. Faecal smears of both PCR- and IFA-positive shown that the reproducibility was 90.9% for PCR-DNA and 81.8% for IFA. In Our findings, PCR -DNA showed that diagnosis cryptosporidiosis 2.1% was more sensitive than IFA. Two different oocysts sizes were visualized by IF microscopy which belonged to different species. Furthermore, PCR analysis with primers against the 18S rRNA gene indicated two genotypes of C. hominis and C. parvum, 500-650 base pairs (bp). In this study, the golden standard was the PCR. In statistical analyses, IFA had positive predictive value (PPV) of 81.8% with 81.8% sensitivity, whereas negative predictive value (NPV) was 1% with 0.97% specificity., Conclusions: PCR showed more sensitivity than IFA for tracking Cryptosporidium oocysts as well as detection of species in faecal human specimens.

Abstract: Cystic echinococcosis, caused by the larval stage of Echinoccocus granulosus, is a global public health threat. The aim of this study was to determine the demographic and clinical characteristics of cases of primary and secondary hydatid cyst in the west of the Islamic Republic of Iran. A retrospective record review was made in 7 public and private hospitals in Kermanshah from 2004 to 2009. Of 526 files of suspected cases analysed, 291 cases of hydatid cyst had been operated upon and confirmed. Of the patients, 51.5% were females, 57.7% lived in urban and suburban areas and the largest age group (17.5%) was 31-40 years. The major organs involved were liver (64.9%) and lung (26.8%) and 18 cases (6.2%) were confirmed as secondary hydatidosis. Most patients (79.4%) had been treated with albendazole. Kermanshah province should be considered an endemic region for human hydatid cyst disease; greater efforts in prevention are needed.

19. Preventive effects of zinc sulfate on taste alterations in patients under irradiation for head and neck cancers: A randomized placebo-controlled trial.

Abstract: Background: Taste abnormalities are common among cancer patients after starting radiotherapy or chemotherapy. Considering the role of zinc and reports on its beneficial effects in taste perception, we evaluated the preventive effects of zinc sulfate on radiation-induced taste alterations., Materials and Methods: In a randomized, placebo-controlled trial, adult patients with head and neck cancers who were on schedule for radiotherapy, with or without chemotherapy, were allocated to receive zinc sulfate (50 mg, three times a day) or placebo; started with beginning of radiotherapy and continued for one month later. Taste acuity was determined by measuring detection and recognition thresholds for four taste qualities at baseline, at the end of radiotherapy, and a month later using the Henkin method., Results: Thirty-five patients (mean age = 59.2 ± 16.5, 60% male) completed the trial. The two groups were similar at baseline. After radiotherapy, and one month later, there was a significant increase in taste perception threshold for bitter, salty, sweet, and sour tastes in the placebo group (P = 0.001). In those who received zinc, there was only slight increase in threshold for perception of the salty taste (P = 0.046). No relevant side effects due to zinc sulfate were reported., Conclusion: Zinc supplementation in head/neck cancer patients under radiotherapy can prevent radiation-induced taste alterations. Further studies with longer follow-ups and with different doses of zinc supplementation are warranted in this regard.
