In this paper, we study multi-cloud storage technologies that share information as a single system using services from different cloud providers. These technologies provide advantages related to the availability and cost but need protections from security threats such as collusion and data leakage, limiting its massive adoption. We analyze security methods for multi-cloud storage and present the last advances in the field. We show that security systems based on homomorphic encryption are more promising than conventional security methods due to the possibility of performing operations over encrypted data. Аннотация Представлены результаты изучения технологий мультиоблачных хранилищ, обменивающихся информацией с помощью сервисов различных провайдеров и являющихся единой системой. Мультиоблачные хранилища доступны и имеют низкую стоимость, но нуждаются в дополнительном обеспечении безопасности для противостояния хакерским атакам и утечке данных, что является ограничением для массового пользовательского внедрения данной технологии. Проанализированы существующие методы обеспечения безопасности и последние достижения в этой области. Сделан вывод, что системы безопасности, основанные на гомоморфном шифровании, превосходят традиционные методы защиты, так как в них возможно выполнение операций над зашифрованными данными.

Addressing the opioid crisis requires attention to the fact that people with opioid use disorder are affected by multiple systems and professionals working across disciplines (e.g., primary health care, social work, psychology). Thus, we developed the Interdisciplinary Training Academy for Integrated Substance/Opioid Use Disorder Prevention and Health care (ITA). The purpose of the ITA is to connect multiple systems to aid in the development of practitioners who are broadly prepared to address the opioid crisis.Herein, we use preliminary descriptive methods to illustrate early outcomes related to 30 weeks of interprofessional training rotations across all system levels related to policy, prevention, treatment, integrated care, harm reduction and recovery support services.Overall, the ITA has assisted fellows in garnering 7,257 hr of training to become holistically competent behavioral health providers. Preliminary data indicate that most cohort graduates intend to pursue employment in a telehealth or primary care setting that serves medically underserved communities.In sum, we offer a new concept for remote field education that engages learners as curriculum developers, educators, and emerging practitioners with expertise across ecological systems. El principal objetivo del presente artículo es el de analizar, describir y conceptualizar fundamentos y antecedentes de la evaluación del desempeño laboral tanto locales como internacionales de los últimos cinco años, con el propósito de conocer su importancia actual. También se exponen la valoración de la sociedad de conocimiento, como un elemento que en los últimos tiempos ha cobrado relevancia. Por tanto, ya no se percibe únicamente como la acumulación de saberes o habilidades, sino como la capacidad de intervenir decidir y actuar en circunstancias imprevistas, es decir que debe darse un nuevo enfoque.

This article examines five years of legislation and constitutional reform under former President Rodrigo R. Duterte. An overview of post-1986 republic acts shows administrations being more successful in legislating social and economic laws than political laws. President Duterte is no exception to this, yet his presidency stands out in both legislative productivity and the passage of long-overdue laws. This success is due in part to high presidential approval, a supermajority coalition in Congress, and a unified Cabinet. These strengthen his influence over Congress, raising expectations that charter change can be fulfiled. His administration's failed attempt at federalism then raises questions on his commitment to the policy in the first place. Despite a populist campaign and institutional means at his disposal, Duterte does not appear any different from his predecessors who prioritised economic laws favoured by the business class over deep political reform with broader implications on governance and development.

This article examines the experiences and assessments of overseas Filipino workers (OFWs) on the Philippine government’s response to the COVID-19 pandemic. The study is part of the growing migration literature exploring the formation of political remittances, defined as political principles, norms and practices migrants acquire during the migration process and what these imply for democratization, particularly in migrants’ home countries. Data for the study came from an online survey of OFWs during the COVID-19 pandemic. Results from the ordered logistic regression suggest that overseas Filipinos’ experiences of successful pandemic management and aid distribution in host countries may influence OFWs to expect and demand similar measures in the Philippines.

While pregnancy presents a strong motivation to seek and comply with treatment for opioid use disorder (OUD), many women relapse within the first year of childbirth. Addressing relapse risk, we examined the perinatal experiences of mothers with OUD through 6 months postpartum. We recruited mothers (N = 42) with a history of OUD into the Newborn Attachment and Wellness study, all of whom met with a child welfare worker immediately after giving birth. In qualitative interviews, mothers described their social, physical, emotional, and psychological perinatal experiences. Seven themes categorically informed relapse risk (i.e., related to childhood bond, mother-infant attachment, birth support, child protective services, breastfeeding, mental health, and recovery planning). In conclusion, we noted a critical window in which clinical social workers and other health/behavioral health providers have the opportunity to capitalize on mothers' desire not to "ever want to touch it again." We outline specific avenues for directed support in the perinatal and postpartum period associated with reduced risk for relapse, and we make recommendations to enhance risk assessment practices.

Climate change poses serious threats to public health and is exacerbating health inequities. Policy changes are essential to mitigate climate change impacts on human and planetary health. The purpose was to describe recommendations by the Policy and Advocacy Subgroup of the Society of Behavioral Medicine (SBM) Climate Change, Behavior Change and Health Presidential Working Group (PWG). The Policy and Advocacy subgroup was comprised of experts in public health, climate policy, and health behavior change, who worked together to identify priorities and develop recommendations. We worked under the premise that building political will for climate policy action is the most urgent goal, and we recommended promotion of citizen advocacy for this purpose. Because citizen advocacy is a set of behaviors, SBM members can use behavioral science to identify and scale up interventions, working collaboratively with communities targeted for marginalization. Recommendations for SBM included establishing an organizational home for climate and health work, providing training and resources, engaging in climate advocacy as an organization, and networking with other organizations. Recommendations for a proposed SBM Climate and Health Committee, Council, or Special Interest Group included developing trainings and resources, seeking opportunities for networking and collaborations, and identifying a research agenda. Individual behavior changes are insufficient to address climate change; policy actions are needed. SBM and similar organizations can support their members to work in developing, evaluating, and scaling up advocacy interventions for action on climate policy to magnify the power of the health and medical sectors to protect planetary and human health.

Latinas report low levels of physical activity (PA) and disproportionate risk of lifestyle-related diseases. Enhancements to evidence-based PA interventions may increase efficacy; however, uptake of interventions will likely depend on costs. To describe costs and examine the cost-effectiveness of two interventions for helping Latinas reach national aerobic PA guidelines. Adult Latinas (N = 199) were randomly assigned to an Original theory-based mail-delivered intervention or an Enhanced version with texting and additional calls and materials. Meeting PA guidelines was measured by the 7-Day PA Recall interview at baseline, 6 and 12 months. Intervention costs were estimated from a payer perspective. Incremental cost-effectiveness ratios (ICERs) were calculated as the additional cost per participant meeting guidelines in the Enhanced versus Original intervention. At baseline, no participants met guidelines. After 6 months, 57% and 44% in the Enhanced and Original arms met guidelines, respectively; at 12 months, rates fell to 46% and 36%. Cost per person of the Enhanced and Original interventions were $184 and $173 at 6 months, respectively, and $234 and $203 at 12 months. The primary additional expense in the Enhanced arm was staff time. ICERs were $87 per additional person meeting guidelines at 6 months (per sensitivity analysis, $26 if delivered by volunteers and $114 by medical assistants), and $317 at 12 months (sensitivity analysis: $57 and $434). Incremental costs per person meeting guidelines in the Enhanced arm were modest and could be warranted given the potential health benefits of meeting PA guidelines.

Neurons are polarized cells that require accurate membrane trafficking to maintain distinct protein complements at dendritic and axonal membranes. The Kinesin-3 family members KIF13A and KIF13B are thought to mediate dendrite-selective transport, but the mechanism by which they are recruited to polarized vesicles and the differences in the specific trafficking role of each KIF13 have not been defined. We performed live-cell imaging in cultured hippocampal neurons and found that KIF13A is a dedicated dendrite-selective kinesin. KIF13B confers two different transport modes, both dendrite- and axon-selective transport. Both KIF13s are maintained at the trans-Golgi network by interactions with the heterotetrameric adaptor protein complex AP-1. Interference with KIF13 binding to AP-1 resulted in disruptions to both dendrite- and axon- selective trafficking. We conclude that AP-1 is the molecular link between the sorting of polarized cargoes into vesicles and the recruitment of kinesins that confer polarized transport.

Purpose: To connect didactic learning to clinical application is a challenging task both for the teachers and students. Target-Oriented Clinical Skill Enhancement (TOCSE) is a teaching and learning tool that integrates basic medical sciences at the clinical level. The authors sought to determine if TOCSE is effective in bridging didactic knowledge to clinical skill and enhancing the clinical performance of fourth year medical students. Method: Between March 2021 and June 2021, in an online platform, the authors randomly allocated 141 fourth year medical students into the experimental (n=12 groups; n=63) and control groups (n=12 groups; n=78). Participants in the experimental group underwent the TOCSE module workshop while the control group utilized the standard method of teaching. The actively teaching faculty staff blinded of group allocation were invited to assess case presentations using a standardized rubric. A survey was done by the students (experimental and control) to evaluate how they perceived TOCSE to their performance and learning. Independent parametric t-test was performed to compare the clinical skill scores between the two groups. Results: The experimental group had a mean clinical skill score of 35.29 (SD=2.64, excellent) while the control group had a mean clinical skill score of 31.96 (SD=4.04, satisfactory). The between-group comparisons using independent t-test indicated that the mean difference of -3.33 clinical skills scores between the experimental and control groups was statistically significant (t=–2.39, p=0.026, 95% CI=–6.22 to –0.45). Moreover, the perceived usefulness score (scale 10 as highest) among the TOCSE presenters (experimental groups) was 8.43 (SD=0.84) and scores among the TOCSE audience (control groups) was comparable at 8.36 (SD=0.71), both of which were interpreted as very helpful. Conclusion: TOCSE is effective in bridging didactic knowledge to clinical skill and enhancing clinical performance of fourth year medical students.

INTRODUCTION: Few studies have focused on physical activity (PA) maintenance, particularly among minority populations; smartphone apps could provide valuable tools. This study aimed to 1) assess and understand PA maintenance among Latinas who completed a PA intervention; and 2) evaluate the feasibility, acceptability and preliminary efficacy of commercial smartphone apps as tools for PA maintenance. METHODS: For this feasibility study, 27 Latinas who completed a PA intervention and increased their PA were randomly assigned to enhanced maintenance (i.e., taught to use commercial smartphone apps, N=14), or regular maintenance (i.e., no additional treatment, N=13). After 3-months, the feasibility and acceptability of using apps for PA maintenance was assessed via survey questions, analyzed using descriptive statistics. PA was reassessed via the 7-day PA Recall and analyzed using longitudinal mixed effects regression models. Qualitative data were collected via open-ended interview questions and analyzed using thematic analysis. RESULTS: 43% of enhanced maintenance participants reported using study apps at least “a little” and 21% using them “a lot.” Although not statistically significant, enhanced maintenance participants reported a smaller drop in PA from post-intervention to post-maintenance, compared to regular maintenance participants. Several participants expressed approval of the apps, while others reported on factors that kept them from using the apps for PA maintenance. CONCLUSION: Incorporating lessons learned from this study, larger randomized trials are warranted to evaluate the efficacy of using smartphone apps to support PA maintenance. The widespread use of apps could make them ideal tools to support PA maintenance after interventions in different settings.
