Der endobronchiale Ultraschall (EBUS) ist ein wichtiger Bestandteil der pneumologischen Endoskopie. In den wesentlichen Indikationen der Tumordiagnostik und des Tumorstagings ist die fruher ubliche Mediastinoskopie lediglich Verfahren der 2. Wahl. Das fur das Tumorstaging erforderliche Mediastinalstaging ist durch die transosophageale EBUS-Anwendung (EUS-B) in einer Untersuchung moglich. Samtliche fur die moderne Tumordiagnostik erforderlichen Untersuchungen sind aus dem gewonnenen Material durchfuhrbar. Auch fur granulomatose Lymphknotenerkrankungen (Sarkoidose, Tuberkulose) bietet der EBUS eine hohe diagnostische Aussagekraft und kann die weiteren Untersuchungen erganzen. Fur die Diagnostik von Lymphomen und unklaren Lymphadenopathien ist die Verwendung von groseren Biopsieinstrumenten sowie die erweiterte molekularpathologische Diagnostik zu bedenken. Jedoch lasst sich hier die chirurgische Sicherung oft noch nicht vermeiden, insbesondere in der Primardiagnostik malignitatssuspekter Lymphome. Daruber hinaus kann der EBUS eine wertvolle Hilfe in der Diagnostik unklarer Gefasveranderungen, insbesondere der Lungenembolie, sein. Im Hinblick auf die Haufigkeit der Anwendungen ist die Komplikationsrate zwar gering, jedoch sollte jeder mit der Methode konfrontierte Untersucher die wesentlichen Komplikationen (die auch vom jeweiligen Punktionsort abhangen) kennen. Der EBUS ist mit einer Lernkurve assoziiert. Vor dem Einsatz sollten am Modell die Prozedur inkl. der Punktion sowie die Anatomie erlernt und beherrscht werden.

Die bildgebende Diagnostik und Therapieuberwachung von Erkrankungen thorakaler Organe ist auf unterschiedliche Art und Weise moglich. Zur Verfugung stehen sonographische, magnetresonanztomographische sowie verschiedene radiologische und nuklearmedizinische Verfahren. Eine Uberdiagnostik und die unnotige Verwendung von Rontgenstrahlung sollten aus medizinisch und wirtschaftlichen Grunden vermieden werden. Die erste apparative Methode, um weitere diagnostische und therapeutische Schritte effektiv und zeitnah zu planen, ist neben der klinischen Untersuchung meist die Thoraxsonographie. Diese Arbeit bietet eine aktuelle Ubersicht uber die Moglichkeiten, die die differenzierte Thoraxsonographie in der elektiven Diagnostik, in der Notfalldiagnostik sowie als interventionelles Verfahren bietet. Neben Indikationen und Grenzen der Thoraxsonographie werden Vorteil fur diagnostische und therapeutische Interventionen im Vergleich zur Computertomographie aufgezeigt und aufkommende zusatzliche technische Moglichkeiten dargestellt.

The SARS-CoV-19 pandemic continues to be globally related with significant morbidity and mortality, making protective measures to prevent transmission of the virus still necessary. Healthcare employees are exposed to a higher risk of infection and this is particularly true when performing aerosol-generating procedures such as bronchoscopy.Since the publication of recommendations for performing a bronchoscopy in the times of COVID-19 more than six months ago, the risk situation has not changed significantly, but due to the considerable gain in knowledge in the meantime, an update of the recommendations was necessary.The updated recommendations include the reduction of aerosol formation, the personal protection of the people involved in the procedure, as well as measures to better organize the processes in the endoscopy suite in order to perform bronchoscopic procedures securely even in times of COVID-19.Die Pandemie ausgelöst durch SARS-CoV-19 geht weltweit unverändert mit einer bedeutsamen Morbidität und Mortalität einher, sodass Schutzmaßnahmen zur Verhinderung der Transmission des Virus weiterhin notwendig sind. Mitarbeiter im Gesundheitswesen sind einem höheren Risiko ausgesetzt, und dies gilt insbesondere im Rahmen sog. Aerosol-generierender Verfahren wie der Bronchoskopie. Seit der Veröffentlichung konsentierter Empfehlungen zur Durchführung einer Bronchoskopie unter diesen Bedingungen vor mehr als einem halben Jahr hat sich die Gefahrenlage nicht wesentlich verändert, aber aufgrund des erheblichen Erkenntnisgewinns in der Zwischenzeit war ein Update der Empfehlungen notwendig. Die erneuerten Empfehlungen umfassen u. a. die Verminderung der Aerosolentstehung, den persönlichen Schutz der beteiligten Personen sowie Maßnahmen zur besseren Organisation der Abläufe in der Endoskopie, um eine sichere Durchführung auch in Zeiten von COVID-19 zu gewährleisten.

Interventional treatment of emphysema offers a wide range of surgical and endoscopic options for patientes with advanced disease. Multidsciplinary collaboration of pulmonology, thoracic surgery and imaging disciplines in patient selection, therapy and follow up ensures treatment quality. The present joint statement describes the required structural and quality prerequsites of treatment centres.Die interventionelle Emphysemtherapie bietet ein breites Spektrum an chirurgischen und endoskopischen Optionen zur Behandlung von Patienten mit fortgeschrittenem Lungenemphysem. Zur Sicherstellung der Behandlungsqualität ist eine interdisziplinäre Zusammenarbeit von Pneumologie, Thoraxchirurgie und bildgebenden Disziplinen in der Auswahl, Therapie und Nachsorge von Emphysem-Patienten erforderlich. Das vorliegende Positionspapier beschreibt erforderliche Struktur- und Qualitätsvoraussetzungen von Behandlungszentren.

Asthma bronchiale und chronische obstruktive Lungenerkrankung (COPD) sind Volkskrankheiten mit steigender Inzidenz. Welchen Beitrag konnen neue Optionen der interventionellen Pneumologie in der Therapie leisten? Darstellung der bronchialen Thermoplastie (BT) bei Asthma bronchiale und der gezielten Lungendenervierung („targeted lung denervation“, TLD) bei COPD: Prozedur, Studienlage und Patientenauswahl. Bei schwerem, unkontrolliertem Asthma konnen nach Ausschopfung aller medikamentosen Masnahmen mittels BT die Bronchialmuskulatur abladiert und eine dauerhafte Verbesserung der Krankheitskontrolle erreicht werden. Bei masiger bis schwerer COPD konnen nach Ausschopfung der Standardtherapie innerhalb klinischer Studien mit einem spezialisierten Katheter bronchoskopisch vagale Nervenfasern an den Hili abladiert werden, um eine dauerhafte Parasympathikolyse und Bronchodilatation herbeizufuhren. Neue Verfahren der interventionellen Pneumologie konnen bei obstruktiven Erkrankungen einen wichtigen Beitrag leisten. Sie befinden sich in fruhen Stadien der klinischen Entwicklung, so dass der endgultige Stellenwert noch nicht feststeht.

Endobronchial Ultrasound (EBUS) with the two modalities curved and radial EBUS significantly improved the diagnostics in several pulmonary diseases. The examination and staging of mediastinal and hilar lymph nodes in patients with known or suspected lung malignancy as well as the evaluation of unknown pulmonary or mediastinal lesions can be achieved with minimal invasive means when using EBUS. More invasive surgical procedures for diagnostic purposes can be omitted. The diagnostic yield also increases when EBUS is applied in sarcoidosis or mediastinal lymph node tuberculosis but only to some extend in case of lymphoma. Samples obtained by EBUS-TBNA should be handled efficiently to allow molecular analysis in lung cancer. EBUS is a safe procedure, and complication rate is extremely low. Further advances of the EBUS technology focus on improving analysis of the information provided by the ultrasound image and a better tissue sampling by developing of new EBUS bronchoscopes and TBNA-needles.

Introduction: Endoscopic lung volume reduction (ELVR) with valves is a relatively new method for emphysema patients. Patients with FEV1 ≤ 20% predicted have been excluded in most studies. Although the procedure can be performed safely, pneumothorax is a frequent complication, which can be critical for these very severely diseased patients. The aim of the study is to assess the safety and efficacy of ELVR in patients with very advanced stage of emphysema, as indicated by FEV1% ≤ 20% predicted. Method: Patients in whom ELVR was performed between January 2013 and August 2015 were included in this analysis if their baseline predicted FEV1 was ≤ 20%. ELVR, performed only if collateral ventilation was absent, achieved complete occlusion of the target lobe. All patients were monitored for 24 h in the ICU and discharged not before three days after ELVR. Results: Twenty patients with FEV1 ≤ 20% predicted were included in the analysis. Lung volume reduction was achieved in 75% of the cases. FEV1 improved by 25.7 ± 30.4% (mean+SD) after 30 days and by +23.7 ± 33.7% (mean+SD) after 90 days. Improvement in 6-Minute walk test was +8 ± 67 m after 30 days and +37.1 ± 77.4 m after 90 days. Pneumothorax occurred in four (20%) cases. No patient died. Conclusion: ELVR with valves can be successfully and safely performed in patients with FEV1 ≤ 20% predicted. Close post-procedural monitoring is warranted.

Background: Several novel methods have been proposed to improve the diagnostic yield of transbronchial biopsy for peripheral pulmonary lesions (PPL). LungPoint® virtual bronchoscopic navigation (VBN) uses virtual 3-dimensional reconstruction of the bronchial tree to guide a bronchoscope visually to the lesion. Objective: To analyse the advantages and limitations of VBN for the diagnosis of PPL. Methods: Nineteen consecutive patients with PPL smaller than 35mm were referred to bronchoscopy with VBN. The distance from the last reconstructed bronchus to the lesion was calculated by the VBN system prior to the procedure. A total of 12 cases with a calculated distance less than 15 mm were included in the analysis. In eight patients radial endobronchial ultrasound (R-EBUS) was used to confirm the location of the lesion following virtual navigation. In all cases a definite diagnosis was achieved either by the bronchoscopic procedure or surgery. Results: The mean lesion diameter was 20 mm. Ten of the lesions were located in the right upper, one in the right middle, two in the right lower, five in the left upper, and one in the left lower lobe. The diagnostic yield of the VBN bronchoscopy was 66,7% (5 benign, 3 malignant lesions). In 5 (41,6%) cases the lesion could be visualized by R-EBUS. In all these cases a conclusive histopathologic diagnosis could be established by the bronchoscopy, leading to a diagnostic yield of 100%. Conclusions: A high diagnostic yield can be achieved with the virtual bronchoscopic navigation if the calculated distance from the last reconstructed bronchus to the lesion is less than 15 mm and the lesion site can be confirmed by R-EBUS.

A scheme is presented for using H2O "snow" nanowire targets for the generation of fast protons. This novel method may relax the requirements for very high laser intensities, thus reducing the size and cost of laser based ion acceleration system.

We report on the first generation of 5.5-7.5 MeV protons by a moderate-intensity short-pulse laser (∼5×10(17) W/cm(2), 40 fsec) interacting with frozen H(2)O nanometer-size structure droplets (snow nanowires) deposited on a sapphire substrate. In this setup, the laser intensity is locally enhanced by the snow nanowire, leading to high spatial gradients. Accordingly, the nanoplasma is subject to enhanced ponderomotive potential, and confined charge separation is obtained. Electrostatic fields of extremely high intensities are produced over the short scale length, and protons are accelerated to MeV-level energies.

To evaluate the incidence of therapy-related acute leukaemia (t-AL) after single-agent mitoxantrone (MITO) treatment, we reviewed medical records of patients in three studies of single-agent MITO therapy for multiple sclerosis (MS) and existing literature on MITO therapy in MS, leukaemia, and solid tumors. Of 1378 MITO recipients in the three MS studies (mean cumulative dose of 60 mg/m2 and mean follow-up of 36 months), one patient had t-AL, an observed incidence proportion of 0.07% [95% confidence interval (CI)=0.00-0.40%]. There were no cases of t-AL in published reports of nine additional studies of single-agent MITO therapy for MS. There was one published case report of acute promyelocytic leukaemia detected five years after initiating MITO therapy for MS. The observed incidence proportion of t-AL is very low in patients who received MITO as single-agent therapy for MS. Although these observations provide preliminary reassurance, extended follow-up of these patients and those who receive higher cumulative doses of MITO is required to define the long-term risk of t-AL after MITO therapy for MS.
