Introduction: The early diagnosis of infective endocarditis (IE) is a medical challenge and a multidisciplinary approach is essential to improve its frequently fatal prognosis. Our goal was to evaluate the usefulness of [18F]2-fluoro-2-deoxy-d-glucose positron emission tomography (18F-FDG PET) in the diagnosis of this disease. Materials and Methods: We prospectively assessed 43 patients (five female and 38 male) with clinical suspicion of IE between 2014 and 2017. All patients underwent transesophageal echocardiography (TEE) and an 18F-FDG PET scan, and the results were compared. A positive PET finding was defined as increased FDG uptake on cardiac valves or intracardiac devices. Results: Out of 43 patients with suspected IE, the diagnosis was confirmed in 30 cases (79.7%). 18F-FDG PET was positive in 24 patients, with 19 showing FDG uptake on cardiac valves (two native and 17 prosthetic) and five on cardiac devices, being concordant with echocardiographic findings in 11 cases. 18F-FDG PET sensitivity was 80%, specificity 92%, positive predictive value (PPV) 96% and negative predictive value (NPV) 66%. Echocardiography presented sensitivity, specificity, PPV and NPV of 36%, 84%, 84% and 36%, respectively. Conclusions: 18F-FDG PET proved to be a sensitive technique with a high diagnostic value in patients with prosthetic valves and intracardiac devices and suspected IE. Its utility decreased dramatically in patients with suspected IE on native valves, in which TEE presented higher sensitivity and thus better diagnostic value. Resumo: Introdução: O diagnóstico precoce de endocardite infecciosa (EI) é um desafio médico. Portanto, uma abordagem multidisciplinar é essencial para melhorar o prognóstico desta patologia, muitas vezes fatal. O nosso objetivo foi avaliar a utilidade da tomografia por emissão de pósitrons [18F] 2-fluoro-2-desoxi-D-glicose (18F-FDG-PET) no diagnóstico desta doença. Materiais e métodos: De forma prospetiva, avaliámos 43 doentes (5 do sexo feminino e 38 do masculino) clinicamente suspeitos de EI entre 2014-2017. Todos os doentes foram submetidos a um ecocardiograma transesofágico (ETE) e uma PET 18F-FDG, os resultados foram posteriormente comparados. Um critério PET positivo foi definido como um aumento na captação de FDG nas válvulas cardíacas ou nos dispositivos intracardíacos. Resultados: Dos 43 doentes com suspeita de EI, o diagnóstico foi confirmado em 30 casos (79,7%). O 18F-FDG-PET foi positivo em 24 doentes, dos quais 19 demonstraram captação de FDG nas válvulas cardíacas (2 nativas e 17 protésicas) e cinco nos dispositivos cardíacos, concordando com os achados ecocardiográficos em 11 casos. A sensibilidade de 18F-FDG-PET (S) foi de 80%, especificidade (P) de 92%, valor preditivo positivo (VPP) de 96% e valor preditivo negativo (VPN) de 66%. A ecocardiografia apresentou valores de S, P, PPV e VPN de 36%, 84%, 84% e 36%, respetivamente. Conclusões: O 18F-FDG-PET demonstrou ser uma técnica sensível com alto valor diagnóstico em doentes com suspeita de EI com próteses valvulares e dispositivos intracardíacos. A utilidade desta técnica diminui drasticamente em doentes com suspeita de EI nas válvulas nativas, nas quais a ETE apresentou maior sensibilidade e, portanto, melhor valor diagnóstico. - Abstract
Nesta tese introdúcense as bases e a orixe da imaxe médica e en especial da técnica PET, con tódolos aspectos que se deben ter en conta a hora de traballar con estas imaxes. Facendo fincapé no emprego de biomarcadores de imaxe e a importancia da súa correcta cuantificación e estandarización no camiño de lograr unha medicina personalizada. Presentamos unha descrición completa da literatura dispoñible sobre a radiómica PET e a análise de texturas, onde observamos que o interese neste campo aumentou dunha maneira exponencial na última década.A maiores identificamos varias preocupacións metodolóxicas relacionadas coa validación dos algoritmos propostos, o número de pacientes incluídos, a falta de accesibilidade aos datos, o software e a necesidade dunha maior estandarización metodolóxica.

- Abstract
Los carcinomas pulmonares no célula pequeña estadio III representan alrededor del 35% del total y constituyen un grupo muy heterogéneo. Son pacientes con distinta carga tumoral y diversas comorbilidades, a los que se les ofrecen diferentes tratamientos. Dichos tratamientos han variado a lo largo de los años, desde cirugía sola, a combinaciones con RT y QT, a la que se ha integrado la inmunoterapia. No existen estudios que comparen de forma clara las distintas modalidades de terapias, por lo que se realizamos esta revisión retrospectiva de los CPNCP E.III tratados en el área de Santiago de Compostela dividiéndolos en los siguientes subgrupos: RT-QT radical, RT-QT neoadyuvante a cirugía y, por último, RT-QT con Durvalumab de mantenimiento, con la finalidad de precisar la mejor opción de tratamiento.

Background To study the possible relation between immunohistochemical expression of vascular endothelial growth factor receptor 1 (VEGFR1) and the maximum standardised uptake value (SUV max) of 18F-FDG PET in patients with non small cell lung cancer (NSCLC). Material and Methods The study included 39 patients with NSCLC (24 squamous cell carcinomas and 15 adenocarcinomas). According to the clinical stage, the patients were distributed as follows: 8 stage I, 7 stage II, 15 stage III and 9 stage IV. Immunohistochemical expression of VEGFR1 was studied through the technique of tissue-matrix using Tissue Arrayer Device (Beecher Instruments, Sun Prairie, WI), using the polyclonal antibody against VEGFR1 (Santa Cruz Biotechnology, California, USA). Results Positive VEGFR1 immunohistochemical expression was noted in 23 cases (59%). The number of positive tumours was not related with clinical stage but there was a different statistically significant association (p:0,0009) between VEGFR1 positivity and histological type, corresponding the greater percentages of positive results to adenocarcinomas (93,3%) versus in squamous cell carcinomas (37,5%). Likewise, SUV max values were higher (p: 0,039) in negative VEGFR1 carcinomas than in positive VEGFR1 tumors (r: 4-32,1; 16,4+/-6,4 (median 16,1) vs r: 3-47; 14,5+/-8,6 (12,8)). Conclusions Our results led us to consider that in NSCLC, the negative VEGFR1 immunohistochemical expression is associated significantly with squamous cell carcinomas subtype and with higher SUV max values in 18F-FDG-PET.

Resumen Antecedentes Estudiar la posible relacion entre la expresion inmunohistoquimica del receptor 1 del factor de crecimiento endotelial vascular (VEGFR1) y el valor maximo de captacion estandarizada (SUVmax) de la PET 18F-FDG en pacientes con cancer de pulmon de celulas no pequenas. Material y metodos El estudio incluyo 39 pacientes con NSCLC (24 carcinomas de celulas escamosas y 15 adenocarcinomas). Segun el estadio clinico, los pacientes se distribuyeron de la siguiente manera: 8 en estadio I, 7 en estadio II, 15 en estadio III y 9 en estadio IV. Se estudio la expresion inmunohistoquimica del VEGFR1 mediante la tecnica de la matriz tisular utilizando el dispositivo de arreglo de tejidos (Beecher Instruments, Sun Prairie, WI), utilizando el anticuerpo policlonal contra el VEGFR1 (Santa Cruz Biotechnology, California, EE. UU.). Resultados Se observo una expresion inmunohistoquimica positiva del VEGFR1 en 23 casos (59%). El numero de tumores positivos no se relaciono con el estadio clinico pero hubo una asociacion estadisticamente significativa diferente (p: 0,0009) entre la positividad de VEGFR1 y el tipo histologico, correspondiendo los mayores porcentajes de resultados positivos a los adenocarcinomas (93,3%) frente a los carcinomas escamocelulares (37,5%). Asimismo, los valores SUVmax fueron mayores (p: 0,039) en los carcinomas VEGFR1 negativos que en los tumores VEGFR1 positivos (r: 4-32,1; 16,4 + /-6,4 [mediana 16,1] vs. r: 3-47; 14,5 + /-8,6 [12,8]). Conclusiones Nuestros resultados nos llevaron a considerar que en el CPCNP, la expresion inmunohistoquimica negativa de VEGFR1 se asocia significativamente con el subtipo de carcinomas de celulas escamosas y con valores SUVmax mas altos en 18F-FDG-PET.

Classical methodologies used in ocular pharmacokinetics studies have difficulties to obtain information about topical and intraocular distribution and clearance of drugs and formulations. This is associated with multiple factors related to ophthalmic physiology, as well as the complexity and invasiveness intrinsic to the sampling. Molecular imaging is a new diagnostic discipline for in vivo imaging, which is emerging and spreading rapidly. Recent developments in molecular imaging techniques, such as positron emission tomography (PET), single-photon emission computed tomography (SPECT) and magnetic resonance imaging (MRI), allow obtaining reliable pharmacokinetic data, which can be translated into improving the permanence of the ophthalmic drugs in its action site, leading to dosage optimisation. They can be used to study either topical or intraocular administration. With these techniques it is possible to obtain real-time visualisation, localisation, characterisation and quantification of the compounds after their administration, all in a reliable, safe and non-invasive way. None of these novel techniques presents simultaneously high sensitivity and specificity, but it is possible to study biological procedures with the information provided when the techniques are combined. With the results obtained, it is possible to assume that molecular imaging techniques are postulated as a resource with great potential for the research and development of new drugs and ophthalmic delivery systems.

- Abstract
Los tumores neuroendocrinos son un grupo de neoplasias heterogéneas que son considerados tumores raros. Sin embargo, su incidencia ha aumentado en los últimos años debido a la mejora de las técnicas diagnósticas y al mayor conocimientos de estos por parte de la comunidad científica. Los tumores neuroendocrinos bien diferenciados expresan en su superficie receptores de somatostatina que son puestos de manifiesto en la PET/TC con [68Ga]-DOTA-TOC, mientras que los tumores más desdiferenciados no presentan esta capacidad, pero si un alto consumo de glucosa, por lo que pueden ser objetivados mediante una PET/TC con [18F]-FDG. El objetivo de este estudio es valorar el rendimiento diagnóstico de ambas técnicas de imagen en función del grado de diferenciación tumoral, con el fin de lograr una correcta valoración de la enfermedad y determinar un adecuado tratamiento.

- Abstract
La enfermedad arterial coronaria es un proceso complejo con alta morbimortalidad, La SPECT ha demostrado una alta sensibilidad y especificidad para su diagnóstico. Objetivo: Evaluar la utilidad de la SPECT de perfusión miocárdica en el diagnóstico y pronóstico de la enfermedad arterial coronaria y la influencia del sexo. Material y Método: se efectuó un estudio observacional, longitudinal y retrospectivo de 2230 pacientes a los que se le realizó SPECT por sospecha de EAC. Se analizaron las características clínicas, los parámetros de la ergometría y gated-SPECT, así como los acontecimientos adversos. Conclusiones: Excepto la dislipemia y la obesidad, el resto de los factores de riesgo cardiovascular y el sexo masculino, se asociaron con mayor probabilidad de gated-SPECT positiva. Los modelos de regresión logística mostraron alta precisión para discriminar pacientes con gated-SPECT positiva y con acontecimientos adversos.

- Abstract
La patología cerebral abarca una gran diversidad de afectaciones entre las que se encuentran las neoplasias. En España, las neoplasias primarias del sistema nervioso central son la segunda causa de mortalidad pediátrica después de la leucemia. Con esta premisa, en la presente tesis doctoral se ha puesto en marcha un sistema de protocolización preclínica de cirugías extracorpóreas para estudiar la viabilidad de las derivaciones arterio-venosas dobles. Al mismo tiempo, se cuantificó el metabolismo cerebral con distintos niveles de glucosa para determinar el amplio rango del nivel de glucosa dependiendo de las condiciones. Finalmente, se indujeron modelos de glioblastoma mediante la inyección de células tumorales en la región frontal del cerebro con el objetivo de determinar si distintos antiVEGF serían terapias eficaces.

25. Discrepancy between stress electrocardiographic changes and nuclear myocardial perfusion defects in the prognostic assessment of patients with chest pain

Objectives: The aim of the present study was to determine the long-term prognostic value provided by the exercise electrocardiographic (ECG) response to nuclear myocardial perfusion imaging (MPI) in the evaluation of patients with chest pain, focusing on patients with a discrepancy between the two tests. Methods: A total of 1460 consecutive patients (777 female; 62.6±11.4 years) undergoing exercise myocardial single-photon emission computed tomography (SPECT) were included. The endpoint was the occurrence of acute coronary syndrome, heart failure or cardiac death during follow-up. Results: Ischemic ECG changes were observed during stress testing in 271 patients (18.5%) and 362 patients (24.7%) had positive (abnormal) exercise MPI results. There was a discrepancy between ECG and SPECT findings in 471 patients (32.2%). During the follow-up period (14.0-39.6 months), 224 patients (15.3%) presented cardiac events. The hazard ratios (HR) of ECG and MPI results to predict events were 1.506 (95% CI: 1.113-2.039) and 10.481 (95% CI: 7.799-14.080), respectively. In patients with negative MPI, the ECG response did not predict events (HR 1.214 [95% CI: 0.646-2.282]), the same as in patients with positive MPI (HR 1.203 [95% CI: 0.848-1.705]). Only in hypertensive patients with positive SPECT did the ECG show significant prognostic value (HR 1.937 [95% CI: 1.030-3.642]). In multivariate analysis, positive MPI proved an independent long-term prognostic factor (HR 10.536 [95% CI: 7.759-14.308]), but not ECG (HR 1.356 [95% CI: 0.994-1.850]). Conclusion: MPI results (normal vs. abnormal) had strong predictive value and discrepant ECG results had no significant additive prognostic value. Resumo: Objetivos: O objetivo do presente estudo foi determinar o valor prognóstico a longo prazo, fornecido pelo eletrocardiograma (ECG) em resposta ao esforço na cintigrafia de perfusão miocárdica (CPM) na avaliação de doentes com dor torácica, com ênfase para os grupos de doentes com discrepância entre os dois testes. Método: Foram incluídos 1460 doentes consecutivos (777 do sexo feminino; 62,6 ± 11,4 anos), submetidos a exercício na cintigrafia de perfusão miocárdica com SPECT. O endpoint foi a presença de síndrome coronária aguda, insuficiência cardíaca ou morte cardíaca durante o seguimento. Resultados: Duzentos e setenta e um doentes (18,5%) apresentaram alterações eletrocardiográficas de isquemia durante o teste de esforço e 362 doentes (24,7%) tinham resultados positivos (anormais) na CPM. A discrepância entre os resultados do ECG e CPM apareceu em 471 doentes (32,2%). Durante o período de seguimento (14,0-39,6 meses), 224 (15,3%) apresentaram eventos cardíacos. O hazard ratio (HR) para predizer eventos dos resultados do ECG e CPM foi de 1,506 (1,113-2,039) e 10,481 (7,799-14,080), respetivamente. No grupo de doentes com CPM negativo, o ECG não prediz eventos [HR 1,214 (0,646-2,282)],e o mesmo acontece no grupo de doentes com CPM positivo [HR 1,203 (0,848-1,705)]. Na análise multivariada, a CPM positiva foi um fator independente de prognóstico a longo prazo [HR 10,536 (7,759-14,308)], mas não o ECG [HR 1,356 (0,994-1,850)]. Conclusão: O resultado da CPM (normal versus anormal) teve um forte valor preditivo e o resultado discrepante no ECG não teve um valor prognóstico adicional significativo. Introduction: The early diagnosis of infective endocarditis (IE) is a medical challenge and a multidisciplinary approach is essential to improve its frequently fatal prognosis. Our goal was to evaluate the usefulness of [18F]2-fluoro-2-deoxy-d-glucose positron emission tomography (18F-FDG PET) in the diagnosis of this disease. Materials and Methods: We prospectively assessed 43 patients (five female and 38 male) with clinical suspicion of IE between 2014 and 2017. All patients underwent transesophageal echocardiography (TEE) and an 18F-FDG PET scan, and the results were compared. A positive PET finding was defined as increased FDG uptake on cardiac valves or intracardiac devices. Results: Out of 43 patients with suspected IE, the diagnosis was confirmed in 30 cases (79.7%). 18F-FDG PET was positive in 24 patients, with 19 showing FDG uptake on cardiac valves (two native and 17 prosthetic) and five on cardiac devices, being concordant with echocardiographic findings in 11 cases. 18F-FDG PET sensitivity was 80%, specificity 92%, positive predictive value (PPV) 96% and negative predictive value (NPV) 66%. Echocardiography presented sensitivity, specificity, PPV and NPV of 36%, 84%, 84% and 36%, respectively. Conclusions: 18F-FDG PET proved to be a sensitive technique with a high diagnostic value in patients with prosthetic valves and intracardiac devices and suspected IE. Its utility decreased dramatically in patients with suspected IE on native valves, in which TEE presented higher sensitivity and thus better diagnostic value. Resumo: Introdução: O diagnóstico precoce de endocardite infecciosa (EI) é um desafio médico. Portanto, uma abordagem multidisciplinar é essencial para melhorar o prognóstico desta patologia, muitas vezes fatal. O nosso objetivo foi avaliar a utilidade da tomografia por emissão de pósitrons [18F] 2-fluoro-2-desoxi-D-glicose (18F-FDG-PET) no diagnóstico desta doença. Materiais e métodos: De forma prospetiva, avaliámos 43 doentes (5 do sexo feminino e 38 do masculino) clinicamente suspeitos de EI entre 2014-2017. Todos os doentes foram submetidos a um ecocardiograma transesofágico (ETE) e uma PET 18F-FDG, os resultados foram posteriormente comparados. Um critério PET positivo foi definido como um aumento na captação de FDG nas válvulas cardíacas ou nos dispositivos intracardíacos. Resultados: Dos 43 doentes com suspeita de EI, o diagnóstico foi confirmado em 30 casos (79,7%). O 18F-FDG-PET foi positivo em 24 doentes, dos quais 19 demonstraram captação de FDG nas válvulas cardíacas (2 nativas e 17 protésicas) e cinco nos dispositivos cardíacos, concordando com os achados ecocardiográficos em 11 casos. A sensibilidade de 18F-FDG-PET (S) foi de 80%, especificidade (P) de 92%, valor preditivo positivo (VPP) de 96% e valor preditivo negativo (VPN) de 66%. A ecocardiografia apresentou valores de S, P, PPV e VPN de 36%, 84%, 84% e 36%, respetivamente. Conclusões: O 18F-FDG-PET demonstrou ser uma técnica sensível com alto valor diagnóstico em doentes com suspeita de EI com próteses valvulares e dispositivos intracardíacos. A utilidade desta técnica diminui drasticamente em doentes com suspeita de EI nas válvulas nativas, nas quais a ETE apresentou maior sensibilidade e, portanto, melhor valor diagnóstico. - Abstract
Background: Although many works have supported the utility of PET radiomics, several authors have raised concerns over the robustness and replicability of the results. This study aimed to perform a systematic review on the topic of PET radiomics and the used methodologies. Methods: PubMed was searched up to 15 October 2020. Original research articles based on human data specifying at least one tumor type and PET image were included, excluding those that apply only first-order statistics and those including fewer than 20 patients. Each publication, cancer type, objective and several methodological parameters (number of patients and features, validation approach, among other things) were extracted. Results: A total of 290 studies were included. Lung (28%) and head and neck (24%) were the most studied cancers. The most common objective was prognosis/treatment response (46%), followed by diagnosis/staging (21%), tumor characterization (18%) and technical evaluations (15%). The average number of patients included was 114 (median = 71; range 20–1419), and the average number of high-order features calculated per study was 31 (median = 26, range 1–286). Conclusions: PET radiomics is a promising field, but the number of patients in most publications is insufficient, and very few papers perform in-depth validations. The role of standardization initiatives will be crucial in the upcoming years

Se trata de un estudio observacional basado en la experiencia del Servicio de Medicina Nuclear del Hospital Clínico Universitario de Santiago de Compostela, con la aplicación de una nueva terapia metabólica con 177Lu-Dotatate (Lutathera®). Representa una nueva vía terapéutica para aquellos tumores avanzados que expresan receptores de somatostatina, que progresan y que presentan limitadas opciones terapéuticas. El objetivo de este estudio de investigación es presentar nuestra experiencia en la terapia metabólica con 177Lu-Dotatate, evaluar su tolerabilidad, así como su eficacia a corto y largo plazo. Queremos destacar, dados los resultados obtenidos, el importante papel que creemos va a ocupar esta opción en el algoritmo terapéutico de los tumores que expresan dichos receptores.

Introducción. La demencia frontotemporal (DFT) es un síndrome con una gran heterogeneidad clínica, que presenta degeneración neuronal de los lóbulos frontal y temporal produciendo síntomas conductuales progresivos y la afectación del lenguaje. Los nuevos radiomarcadores para PET pueden ayudar al diagnóstico de esta patología, pero, a día de hoy, muchos aspectos siguen sin estar claros. Objetivo. El objetivo de este trabajo es realizar una revisión de la literatura publicada en los últimos años, sobre el diagnóstico por imagen molecular de las demencias frontotemporales, para tratar de esclarecer qué radiomarcadores son de mayor utilidad actualmente. Métodos. Se ha llevado a cabo una revisión sistemática, siguiendo las directrices PRISMA, de los estudios encontrados en PubMed, incluyendo un total de 8 estudios. Resultados y conclusiones. Los resultados señalan que el THK5351 (radiomarcador para PET Tau) es, por el momento, el trazador más útil para el diagnóstico por imagen de la demencia frontotemporal y sus variantes, dado que la captación presentada refleja la combinación de la patología de tau con cambios neuroinflamatorios asociados a la degeneración propia de esta patología. Si bien este radiofármaco ha mostrado buenos resultados, a la hora de interpretarlos se debe tener en cuenta la edad y características individuales del paciente, las posibles mutaciones asociadas y las variantes clínicas que presenta la enfermedad, Introduction. Frontotemporal dementia (FTD) is a syndrome with clinical heterogeneity, which presents neuronal degeneration of the frontal and temporal lobes producing progressive behavioral symptoms and language impairment. The new radiomarkers for PET can help in the diagnosis of this pathology but today many aspects remain unclear. Background. To analyze the literature published in recent years on molecular imaging diagnosis of frontotemporal dementias to try to clarify which radiomarkers are currently most useful. Methods. A systematic review was performed, following the PRISMA guidelines, of the studies found in PubMed including a total of 8 studies. Results and conclusions. The results indicate that THK5351 (radiolabel for PET Tau) is at the moment the most useful tracer for imaging frontotemporal dementia and its variants since the uptake presented reflects the combination of tau pathology and neuroinflammatory changes associated with the degeneration of this pathology. Although this radiomarker has shown good results, when interpreting them, the age and individual characteristics of the patient, the possible associated mutations and the clinical variants that the disease presents must be taken into account

- Abstract
The aim of this study was to fabricate novel self-supporting tacrolimus suppositories using semisolid extrusion 3-dimensional printing (3DP) and to investigate their efficacy in an experimental model of inflammatory bowel disease. Blends of Gelucire 44/14 and coconut oil were employed as lipid excipients to obtain suppository formulations with self-emulsifying properties, which were then tested in a TNBS (2,4,6-trinitrobenzenesulfonic acid) induced rat colitis model. Disease activity was monitored using PET/CT medical imaging; maximum standardized uptake values (SUVmax), a measure of tissue radiotracer accumulation rate, together with body weight changes and histological assessments, were used as inflammatory indices to monitor treatment efficacy. Following tacrolimus treatment, a significant reduction in SUVmax was observed on days 7 and 10 in the rat colon sections compared to non-treated animals. Histological analysis using Nancy index confirmed disease remission. Moreover, statistical analysis showed a positive correlation (R2 = 71.48%) between SUVmax values and weight changes over time. Overall, this study demonstrates the effectiveness of 3D printed tacrolimus suppositories to ameliorate colitis and highlights the utility of non-invasive PET/CT imaging to evaluate new therapies in the preclinical area

- Abstract
El objetivo es evaluar la utilidad de la RM y la PET específica de mama (dbPET) en el diagnóstico de lesiones BI-RADS 4 detectadas mediante ecografía o mamografía y analizar el valor de la imagen metabólica para diferenciar los carcinoma ductal in situ (CDIS) indolentes de los potencialmente agresivos. Se incluyeron 60 lesiones BI-RADS 4 a las que se les realizó una RM y una dbPET antes de la biopsia. Se revisaron 139 casos de CDIS con una dbPET previa al estudio histológico. Nuestros resultados descartan aconsejar el uso de la RM o de la dbPET para diagnosticar malignidad en estas lesiones ya que la tasa de falsos positivos superó el 50%. No obstante, si la RM es negativa podría obviarse la biopsia. Frente al 8% de los CDIS de bajo grado, el 90% de los de alto grado cursaron con una dbPET positiva. La imagen metabólica podría ayudar a identificar los CDIS indolentes.

- Abstract
Although typically conceptualized as a cortical disease, recent neuropathological and neuroimaging investigations on Alzheimer Disease suggest that other brain structures play an important role in the pathogenesis and progression of this devastating condition. In this thesis, we explored novel markers of Alzheimer Disease beyond the classical cortical pathology measures of amyloid, tau, and neurodegeneration. We focused on the role of white matter abnormalities, assessed with magnetic resonance imaging but also with amyloid positron emission tomography, in predicting early pathologic changes and disease progression, as well as on the added value of cognition to amyloid, tau, and neurodegeneration biomarkers. Overall, we found that these unconventional markers provide useful information to detect the earliest pathological changes of the disease, providing a better understanding of the mechanisms that lead to amyloid deposition and cognitive decline.

Severe allergic ocular diseases as atopic keratoconjunctivitis can induce corneal damage due to inflammatory substances released from giant papillae. Tacrolimus eye drops are one of the current therapeutic alternatives for its treatment. This work is aimed at developing and characterizing a 0.03% tacrolimus ophthalmic formulation, which was introduced in three types of vehicles (BBS, PVA and Hyaluronic Acid). For this, we have performed in vitro (stability studies) and in vivo assays (corneal permanence time measured directly by Positron Emission Tomography) of three potential formulations. Next, the best formulation was selected, and its toxicological profile and clinical effectiveness have been evaluated. The biopermanence studies (direct measurements and PET/CT) showed that the formulations with PVA and Hyaluronic Acid present more retention time on the ocular surface of rats than PBS. From the stability study, we have determined that tacrolimus with PVA in cold storage is the best option. Tacrolimus with PVA has shown lower cytotoxicity than cyclosporine at early times. On the other hand, the pilot study performed has shown significant improvements in patients, with no noticeable adverse reactions. Based on stability, biopermanence, safety and clinical effectiveness studies, we concluded that tacrolimus-PVA eye drops are a suitable candidate for its clinical application in inflammatory ophthalmology diseases.

Classical methodologies used in ocular pharmacokinetics studies have difficulties to obtain information about topical and intraocular distribution and clearance of drugs and formulations. This is associated with multiple factors related to ophthalmic physiology, as well as the complexity and invasiveness intrinsic to the sampling. Molecular imaging is a new diagnostic discipline for in vivo imaging, which is emerging and spreading rapidly. Recent developments in molecular imaging techniques, such as positron emission tomography (PET), single-photon emission computed tomography (SPECT) and magnetic resonance imaging (MRI), allow obtaining reliable pharmacokinetic data, which can be translated into improving the permanence of the ophthalmic drugs in its action site, leading to dosage optimisation. They can be used to study either topical or intraocular administration. With these techniques it is possible to obtain real-time visualisation, localisation, characterisation and quantification of the compounds after their administration, all in a reliable, safe and non-invasive way. None of these novel techniques presents simultaneously high sensitivity and specificity, but it is possible to study biological procedures with the information provided when the techniques are combined. Es lo que, hoy en día, conocemos como imagen molecular, que puede definirse como “visualizar, cuantificar y caracterizar los procesos biológicos a nivel celular y molecular en los seres humanos y otros sistemas vivos”. Se trata de un nuevo enfoque biomédico, en un nuevo contexto (evidenciar algo in vivo), interdisciplinario y con un nuevo propósito: plasmar, en una imagen, el inicio y no el efecto final de un proceso como estábamos acostumbrados.

Magnetic resonance imaging (MRI) volumetric measures have become a standard tool for the detection of in-cipientAlzheimer'sDisease(AD)dementiainmildcognitiveimpairment(MCI).Focusedonprovidinganearlierand more accurate diagnosis, sophisticated MRI machine learning algorithms have been developed over therecentyears,mostofthemlearningtheirnon-diseasepatternsfromMCIthatremainedstableover2–3years.Inthis work, we analyzed whether these stable MCI over short-term periods are actually appropriate trainingexamples of non-disease patterns. To this aim, we compared the diagnosis of MCI patients at 2 and 5years offollow-up and investigated its impact on the predictive performance of baseline volumetric MRI measures pri-marily involved in AD, i.e., hippocampal and entorhinal cortex volumes. Predictive power was evaluated interms ofthe areaunder the ROCcurve(AUC), sensitivity,andspecificity inatrialsample of248 MCIpatientsfollowed-up over 5years. We further compared the sensitivity in those MCI that converted before 2years andthose that converted after 2years. Our results indicate that 23% of the stable MCI at 2years progressed in thenextthreeyearsandthatMRIvolumetricmeasuresaregoodpredictorsofconversiontoADdementiaevenatthemid-term, showing a better specificity and AUC as follow-up time increases. The combination of hippocampusand entorhinal cortex yielded an AUC that was significantly higher for the 5-year follow-up (AUC=73% at2yearsvs.AUC=84%at5years),aswellasforspecificity(56%vs.71%).Sensitivityshowedanon-significantslightdecrease(81%vs.78%).Remarkably,theperformanceofthismodelwascomparabletomachinelearningmodels at the same follow-up times. MRI correctly identified most of the patients that converted after 2years(with sensitivity>60%), and these patients showed a similar degree of abnormalities to those that convertedbefore 2years. - Abstract
La detección gammagráfica radio-isotópica con 99m-Tc nanocoloide y la biopsia selectiva del ganglio centinela predicen el estado de diseminación linfática regional en las pacientes con cáncer de mama y por lo tanto, aportan la información necesaria e imprescindible para establecer el tratamiento adecuado, condicionando el posterior manejo terapéutico local y axilar. Puede ser usada para evitar la linfadenectomía axilar completa en la mayoría de las pacientes que estén libres de metástasis linfáticas. Sin embargo, la edad es un factor que influye directamente en la visualización gammagráfica del ganglio centinela. Este estudio pretende identificar posibles diferencias entre cuatro diferentes grupos etarios, marcados por la integridad del sistema linfático y la agresividad tumoral, así como por otros factores relacionados con la biología del tumor en mujeres de edad avanzada.
