Purpose: Limited data are available regarding the efficacy of hepatic arterial infusion (HAI) of oxaliplatin in patients with liver metastases from colorectal carcinoma (CRC). The aim of the present study was to evaluate the results of HAI of oxaliplatin combined with 5-fluorouracil (5-FU) and leucovorin (LV) in patients with such metastases., Methods: A retrospective analysis of 22 CRC patients treated with HAI of combination of oxaliplatin and 5-FU and LV was performed., Results: Partial response (PR) was observed in 4 (18%) patients and stable disease (SD) in 7, with an overall disease control rate of 50%. The median progression-free (PFS) and overall survival (OS) were 7 and 11 months, respectively. Two patients treated with first-line treatment underwent subsequent liver resection. In 2 patients, HAI of oxaliplatin, 5-FU and LV was combined with systemic administration of bevacizumab., Conclusion: Our data demonstrate reasonable efficacy of HAI with oxaliplatin, 5-FU and LV in patients with liver metastases from CRC.

Backgrounds: Recently, prostate bed irradiation has, due to the results of randomized trials which confirmed its benefit in adjuvant treatment, become the standard treatment for patients with risk factors after radical prostatectomy. It is also irreplaceable in the treatment of biochemical progression caused by local recurrence. In most cases, the tumour is not visible on planning CT, so it is necessary to use all available information to define the target volume to treat all the sites where the recurrence could be present., Design: The aim of this review is to provide a compendium of radiotherapy after radical prostatectomy, its indications and especially the localization of recurrences, which implicates the clinical target volume definitions., Conclusion: The treatment of prostate cancer requires close co-operation between urologists and radiation oncologists, especially now when the indication of radiotherapy includes patients with risk factors after radical prostatectomy where adjuvant treatment should be performed early after surgery. It is of the utmost importance to precisely define the target volume for radiotherapy to fully prevent local recurrence without unnecessary complications for the patient.

Stem cells and their therapeutic use present many questions associated with ethical problems in medicine. There is great effort on the part of physicians to help millions of patients while there are ethical problems with the use of new methods and technologies and all of these are affected by economic and political influences. How will the current generation deal with these problems? Medicine, in this begard, is experiencing a stormy evolution of human culture in the relationships between disease, patient and doctor. Philosophy approaches the same juncture of human culture, but seemingly from the other side. Both disciplines are facing a great problem: How to unite the content of current human morality and the desire for health? Both philosophers and physicians perceive this deficit in human culture as it does not provide immediately usable normatives, which the living generation of healthy and ill is waiting for. It may be said that medicine, as many times before, has reached a stage where it cannot rely only on the proved axiologic values from the past, ethical normatives or cultivated moral sense of its subjects. Medicine has no other alternative than to take an active part in resolution of interdisciplinary problems originating from philosophic-biologic or philosophic-medical inquiries of axiologic, ethical, and moral issues. Our paper indicates some ways of the search in forming ethical principles of the stem-cell therapy from the view of biologists and physicians. New ways are recommended in theoretical-methodological interdisciplinary research, especially, in theoretical and experimental biology, and theoretical and clinical medicine, as well as philosophy. In this paper important ethical problems are pointed out in order to find answers to some key problems connected with cell therapy and the use of stem cells.

Abdominal liposarcoma is a rare tumor of uncertain prognosis. Radical surgery is possible in about two-thirds of the patients, and the prognosis of patients with inoperable tumors is dismal. Only a few cases of liposarcoma complicating pregnancy have been documented. We report a case of a patient who was diagnosed with metastatic abdominal liposarcoma during the third trimester of the pregnancy. After induced vaginal delivery, palliative surgery was performed and one cycle of systemic combination chemotherapy was administered. Despite the multimodality treatment the patient died of progressive disease within one month after diagnosis. Autopsy revealed high-grade pleomorphic liposarcoma arising from the retroperitoneum with liver and lung metastases.

Objective: To report a case of successful pregnancy outcome after embolization treatment of symptomatic leiomyoma., Design: Case report., Setting: Department of Gynaecology and Obstetrics of University Hospital, Hradec Králové., Subject and Method: A 41-year old primigravida who had previously undergone uterine artery embolization and myomectomy had successful course of the spontaneously conceived pregnancy terminated per sectionem cesarean. An analysis of the 50 published cases of pregnancy after uterine artery embolization revealed the following complications: spontaneous abortion (22%), malpresentation (17%), hypotrophy (7%), premature delivery (28%), caesarean section (58%) and postpartum hemorrhage (13%)., Conclusion: Whether this procedure is safe for women desiring future fertility is controversial. There are very few data regarding the outcomes of pregnancies after embolization.
