Context Athletic training is a multifaceted profession characterized by interpersonal relationships and a team approach to care. Collaborative relationships, by nature, open the door to conflict, which has been reported frequently in the collegiate athletic setting. However, secondary school athletic trainers' (ATs') experiences with conflict and pressure in their role are not readily understood. Objective To measure the extent and sources of stress, pressure, and conflict within the secondary school athletic training setting and determine if differences exist across employment characteristics. Design Cross-sectional study. Setting Secondary school athletics. Patients or Other Participants Secondary school ATs (n = 725, age = 39.8 ± 10.5 years, years certified = 16.7 ± 9.7, years in current role = 10.6 ± 7.8). Main Outcome Measures Participants were asked to reply to an online questionnaire with quantitative measures pertaining to organizational conflict and workplace dynamic. Employment type (school district employee, school district teacher, medical or university facility, independent contractor) and status (full time, part time) served as independent variables. Likert-scale scores (1 = strongly agree to 5 = strongly disagree; 1 = always to 5 = never) and perceived sources of stress, pressure, and conflict were the dependent variables. Analyses consisted of Kruskal-Wallis tests with Mann-Whitney U post hoc tests and odds ratios to assess associations between variables of interest. Results We obtained a 15.3% response rate (725/4745). Although the ATs reported experiencing conflict and pressure, these experiences were relatively infrequent and not universal. Compared with part-time ATs, full-time ATs described higher ratings of strong relationships with coaches (P = .003) and principals (P = .002). The most frequently identified sources of conflict were parents (59%) and coaches (53.9%), followed by athletes (32.6%). Full-time ATs were 1.6 times more likely to report experiencing conflict with a coach than part-time ATs (odds ratio = 1.550, 95% CI = 1.037, 2.317; P = .040). Conclusions Secondary school ATs' experiences regarding organizational conflict were relatively positive. Instances of pressure and conflict were noted, though relatively infrequently, and these experiences were largely uninfluenced by employment type.

Objectives To examine the efficacy of weekly and bi-weekly heat training to maintain heat acclimatization (HAz) and heat acclimation (HA) for 8 weeks in aerobically trained athletes. Design Randomized, between-group. Methods Twenty-four males (mean [m ± standard deviation [sd]; (age, 34 ± 12 y; body mass, 72.6 ± 8.8 kg, VO2peak, 57.7 ± 6.8 mL·kg-1·min-1) completed five trials (baseline, following HAz, following HA (HAz + HA), four weeks into heat training [HTWK4], and eight weeks into HT [HTWK8] that involved 60 min of steady-state exercise (59.1 ± 1.8% vVO2peak) in an environmental laboratory (wet bulb globe temperature [WBGT], 29.6 ± 1.4 °C) on a motorized treadmill. Throughout exercise, heart rate (HR) and rectal temperature (Trec) were recorded. Following HAz + HA, participants were assigned to three groups: control group (HT0), once per week heat training (HT1), and twice per week heat training (HT2). HT involved heated exercise (WBGT, 33.3 ± 1.3 °C) to achieve hyperthermia (38.5-39.75 °C) for 60 min. Repeated measures ANOVAs were used to determine differences. ObjectiveMinority student-athletes have a lower survival rate from sudden cardiac arrest (SCA) than non-minority student-athletes. This study examined the relationship between high school indicators of socioeconomic status (SES) and survival in student-athletes with exercise-related SCA.MethodsHigh school student-athletes in the USA with exercise-related SCA on school campuses were prospectively identified from 1 July 2014 to 30 June 2018 by the National Center for Catastrophic Sports Injury Research. High school indicators of SES included the following: median household and family income, proportion of students on free/reduced lunch and percent minority students. Resuscitation details included witnessed arrest, presence of an athletic trainer, bystander cardiopulmonary resuscitation and use of an on-site automated external defibrillator (AED). The primary outcome was survival to hospital discharge. Differences in survival were analysed using risk ratios (RR) and univariate general log-binomial regression models.ResultsOf 111 cases identified (mean age 15.8 years, 88% male, 49% white non-Hispanic), 75 (68%) survived. Minority student-athletes had a lower survival rate compared with white non-Hispanic student-athletes (51.1% vs 75.9%; RR 0.67, 95% CI 0.49 to 0.92). A non-significant monotonic increase in survival was observed with increasing median household or family income and with decreasing percent minority students or proportion on free/reduced lunch. The survival rate was 83% if an athletic trainer was on-site at the time of SCA and 85% if an on-site AED was used.ConclusionsMinority student-athletes with exercise-related SCA on high school campuses have lower survival rates than white non-Hispanic athletes, but this difference is not fully explained by SES markers of the school.

Context Having athletic trainers (ATs) employed at secondary schools is associated with improved preparedness for sport-related emergencies. Utilization of emergency medical services (EMS) with different access to athletic training services remains unknown. Objective Compare the incidence of EMS activations for sport-related injuries between zip-codes with varying access to athletic training services. Design Descriptive epidemiology study. Setting Data were obtained from the National EMS Information System and the Athletic Training Location and Services Project. Patients or Other Participants Zip-codes where 9-1-1 EMS activations for sport-related injuries among individuals 13–18 years old occurred. Main Outcome Measure(s) Incidence of EMS activations, athletic training service level (NONE, PART, FULL), athletic training employment model (independent contractor, IC; medical or university facility, MUF; school district, SD; mixed employment models, MIX) for each zip-code. Results There were 2.8±3.5 EMS activations per zip-code (range 1–81, n=4,923). Among zip-codes where at least 1 AT was employed (n=2,228), 3.73% (n=83) were IC, 30.83% (n=687) were MIX, 27.24% (n=607) were SD, and 38.20% (n=851) were MUF. Compared to SD, MUF had a 10.8% lower incidence of EMS activations (95%CI: 0.817, 0.974, p=0.010). IC (IRR: 0.920, 95%CI: 0.758, 1.118, p=0.403) and MIX (IRR: 0.996, 95% CI: 0.909, 1.091, p=0.932) were not significantly different from SD. Service level was calculated for 3,834 zip-codes, with 19.5% (n=746) NONE, 46.2% (n=1,771) PART, and 34.4% (n=1,317) FULL. Compared to NONE, FULL (IRR: 1.416, 95%CI: 1.308, 1.532, p Conclusions Local access to athletic training services is associated with an increased utilization of EMS for sport-related injuries among secondary school aged individuals, potentially indicative of improved identification and triage of sport-related emergencies the area. The difference in EMS utilization between employment models may represent the presence of different policies and procedures for sport-related emergencies.

Background: Multiteam, multi-institution prospective studies of both women’s and men’s sports are essential for collectively investigating injury and primary to the generalization and individualization of injury prevention strategies. Hypothesis: Characteristics of workload, sleep, and contextual factors will be associated with injury risk in collegiate soccer athletes. Study Design: Cohort study; Level of evidence, 2. Methods: Injuries, workload, and sleep characteristics were recorded daily throughout a complete season for 256 athletes from 12 separate National Collegiate Athletic Association Division I men’s and women’s soccer teams. Workload and contextual factors were assessed via multilevel Poisson regression to capture differences in injury incidence rate ratio (IRR). Paired t test and multilevel logistic regressions were used to assess the relationship between sleep behavior and injury. Results: Collegiate soccer athletes had lower rates of noncontact injury in the in-season (IRR, 0.42) and postseason (IRR, 0.48) compared with preseason, lower rates of injury in training (IRR, 0.64) compared with matches, and higher injury rates with only 1 day of rest in the previous week (IRR, 1.58) compared with >1 day. Injury rates peaked when training occurred 4 days before a match (IRR, 2.24) compared with a match. Injury rate increased exponentially with increases in the number of noncontact injuries incurred throughout the season (IRR, 2.23). Lower chronic loading, higher training monotony, and acute spikes and lulls in workload were associated with higher noncontact injury rates. Alterations in previous week sleep quality were associated with injury, while chronic sleep behavior and acute alterations ( .05). Conclusion: Athlete and schedule-specific contextual factors, combined with characteristics of workload and weekly sleep behavior, are significantly associated with injury in collegiate soccer. Multiteam prospective cohort studies involving objective and subjective monitoring allow for the identification of multiple injury risk factors in sports, which can be used to guide injury prevention strategies. Maintaining higher chronic workloads, lowering training monotony, minimizing acute spikes or lulls in workloads, managing workloads during preseason and for athletes with previous injury, integrating more rest and recovery during congested periods, and optimizing sleep quality are all practical considerations for reducing injury risk in collegiate soccer.

The relationships between perceived wellness of, sleep of, and acute: chronic training load on National Collegiate Athletics Association division I male soccer players. J Strength Cond Res XX(X): 000-000, 2021-The purpose of this study was to investigate relationships between perceived wellness, sleep, and acute: chronic workload ratio (ACWR) throughout a collegiate men's soccer season. Sixty male collegiate soccer players (mean[M] ± SD; age, 21±2 year; body mass, 77.6 ± 6.5 kg; height, 180.1 ± 6.4 cm; body fat%, 9.9 ± 3.9% ; and V[Combining Dot Above]O2max, 53.1 ± 5.0 ml·kg-1·min-1) participated in this study. During each session, players used a heart rate and global positioning satellite-enabled chest strap to measure training impulse and ACWR. The ACWR values were trichotomized at the individual level giving an equal number of observations within each ACWR category of low, moderate, and high ACWR (M ± SD; low, 0.658 ± 0.23; moderate, 0.92 ± 0.15; and high, 1.17 ± 0.16). Stress, fatigue, and soreness levels were collected using 1-10 Likert scales and sleep duration, and sleep quality were measured by the Karolinska Sleep Diary. Stress, fatigue, soreness levels, and sleep quality were transformed to corresponding z-scores at the individual level. Fatigue levels were significantly higher when ACWR was high compared with low (mean difference [95% confidence intervals], effect size, p-value; 0.31 [0.21, 0.42], 0.29, p < 0.001) and moderate (0.14 [0.03, 0.24], 0.13, p = 0.01). Fatigue levels were also significantly higher when the ACWR was moderate compared with low (0.18 [0.07, 0.28], 0.16, p = 0.001). Soreness levels were significantly higher when the ACWR was high compared with low (0.25 [0.14, 0.36], 0.23, p < 0.001). Stress levels were significantly greater when the ACWR was high compared with low (0.19, [0.08, 0.29], 0.18, p < 0.001) and compared with moderate (0.15, [0.05, 0.25], 0.14, p = 0.004). There were no differences in sleep duration or sleep quality in different ACWR. The ACWR may be a useful tool to achieve an appropriate balance between training and recovery to manage daily fatigue and soreness levels in athletes.

Background Literature examining emergency medical services (EMS) activations for sport-related injuries is limited to the pediatric, high school, and collegiate student-athlete populations, excluding older individuals and recreational athletes. The purpose of this study was to examine EMS activations for sport-related injuries using the National EMS Information System Database from 2017-2018. Methods Data were obtained using the National EMS Information System Database from 2017-2018. EMS activations were limited to 9-1-1 responses for individuals aged 3-99 who sustained a sports-related injury. Independent variables included patient age group: pediatric (18 years old) vs. adult (≥18 years old). Dependent variables were patient age, gender, and chief complaint anatomic location. Frequencies and proportions were calculated for each variable. Injury proportion ratios (IPRs) with 95% confidence intervals were calculated to compare chief complaint anatomic location by age group. Results There were 71,322 sport-related injuries. Patients were 36.6±22.9 years and most (58.1%, n=41,132) were male. Adults had higher proportions of injuries affecting the abdomen (IPR: 2.05, 95%CI: 1.83, 2.31), chest (IPR: 1.90, 95%CI: 1.75, 2.05), general/global (IPR: 1.54, 95%CI: 1.50, 1.58), and genitalia (IPR: 2.40, 95%CI: 1.39, 4.15), and lower proportions of injuries affecting the back (IPR: 0.55, 95%CI: 0.50, 0.60), lower extremity (IPR: 0.63, 95%CI: 0.60, 0.65), upper extremity (IPR: 0.50, 95%CI: 0.47, 0.53), head (IPR: 0.73, 95%CI: 0.70, 0.77), and neck (IPR: 0.18, 95%CI: 0.16, 0.20) compared to pediatric patients. Conclusion Injuries sustained differed between adult and pediatric patients, indicating sport-related emergencies may change across the lifespan. General/global chief complaints likely indicate sport-related injuries affecting multiple anatomic locations and organ systems. Stakeholders planning large or high-risk athletic events should consider arranging standby or dedicated advanced life support units for their events.

Fifteen male soccer athletes completed linear tasks (40 and 100 m) at various velocities: walk (W) (4.8-7.9 km·h), jog (J) (8.0-12.7 km·h), run (R) (12.9-19.9 km·h), and sprint (S) (>20.0 km·h) and a 120-m TSSC. Introduction The U.S. Air Force (USAF) Basic Military Training (BMT), a rigorous training program for all enlisted members of the USAF, trains roughly 36,000 recruits annually. Transforming civilians into ready warrior airmen has inherent risks to trainee health, which has infrequently included death. While the average death rate at USAF BMT has decreased between 1956 and 2007 due to process improvement and preventive medicine efforts, further review is warranted to examine the deaths that have occurred since the last published period (1997-2007) and to determine the impact policy changes and updates have had on death rates since that time. Therefore, the purpose of this paper is to identify death rates and types from 2008 to 2020, explore policy implementation, and identify areas needing further improvement or modifications to the overall safety, fitness, and health of USAF BMT trainees. Materials and Methods All deaths were examined and reviewed from 2008 through 2020 for trainees attending the USAF BMT using medical records and autopsy reports. Death rates were calculated using the total population of trainees in a given year as well as over the entire 13-year study period. Results From 2008 to 2020, five deaths occurred among USAF BMT trainees (one cardiac, two exertional sickling due to sickle cell trait, one infection, and one suicide). This resulted in an overall average death rate of 1.08 per 100,000 trainees, as compared to 1.46 per 100,000 from 1997 to 2007. The last death in the study period occurred in 2016. Conclusion A modest downward trend of average death rate has continued since 2007, and no deaths from 2016 through 2020 represents the longest time frame without any deaths at USAF BMT over all times reported (dating back to 1956) which suggest that emergency best practice policies are/have improved. However, cardiac death rate and suicide rate have not changed since the last report. Policies and practices should be continuously reviewed and refined to reduce the risk of death at USAF BMT.

Concussion or mild traumatic brain injury (mTBI) is a common phenomenon in the United States, with up to 3.6 million sport-related mTBIs diagnosed annually. Return to learn protocols have been developed to facilitate the reintegration of students into school after mTBI, however, the implementation of return to learn protocols varies significantly across geographic regions and school districts. An integrative review of the literature was performed using Whittemore and Knalf's methodology. A search of published literature was conducted using the PRISMA checklist. Database searches were conducted from March 2,019 to October 2,021 using the terms “mild traumatic brain injury” and “return to learn.” Twenty-eight publications were included. Three themes were derived from this review: lack of policy, poor staff education on concussion symptoms and stakeholder communication breakdown. The development of communication patterns and use of a return to learn protocol could facilitate a gradual return to full academic workload after concussion.

Millions of consumer sport and fitness wearables (CSFWs) are used worldwide, and millions of datapoints are generated by each device. Moreover, these numbers are rapidly growing, and they contain a heterogeneity of devices, data types, and contexts for data collection. Companies and consumers would benefit from guiding standards on device quality and data formats. To address this growing need, we convened a virtual panel of industry and academic stakeholders, and this manuscript summarizes the outcomes of the discussion. Our objectives were to identify (1) key facilitators of and barriers to participation by CSFW manufacturers in guiding standards and (2) stakeholder priorities. The venues were the Yale Center for Biomedical Data Science Digital Health Monthly Seminar Series (62 participants) and the New England Chapter of the American College of Sports Medicine Annual Meeting (59 participants). In the discussion, stakeholders outlined both facilitators of (e.g., commercial return on investment in device quality, lucrative research partnerships, and transparent and multilevel evaluation of device quality) and barriers (e.g., competitive advantage conflict, lack of flexibility in previously developed devices) to participation in guiding standards. There was general agreement to adopt Keadle et al.'s standard pathway for testing devices (i.e., benchtop, laboratory, field-based, implementation) without consensus on the prioritization of these steps. Overall, there was enthusiasm not to add prescriptive or regulatory steps, but instead create a networking hub that connects companies to consumers and researchers for flexible guidance navigating the heterogeneity, multi-tiered development, dynamicity, and nebulousness of the CSFW field.

J Strength Cond Res 33(11): 3065-3077, 2019-This observational study aimed to characterize the responses of a comprehensive panel of biomarkers, observed ranges, training load (TL) metrics, and performance throughout the collegiate soccer season (August-November). Biomarkers (n = 92) were collected before the start of pre-season (PS), in-season weeks (W)1, W4, W8, and W12 in NCAA Division I male soccer players (n = 20, mean ± SD; age = 21 ± 1 years, height = 180 ± 6 cm, body mass = 78.19 ± 6.3 kg, body fat = 12.0 ± 2.6%, VO2max 51.5 ± 5.1 ml·kg·min). Fitness tests were measured at PS, and W12 and TL was monitored daily. Changes in biomarkers and performance were calculated via separate repeated-measures analysis of variance. Despite similar fitness (p0.05), endocrine, muscle, inflammatory, and immune markers changed over time (p0.05). Total and free testosterone was lower in W1 vs. PS, whereas free cortisol remained unchanged at PS, W1, and W4 (0.94 mg·dL). Oxygen transport and iron metabolism markers remained unchanged except for HCT (W1 vs. PS) and total iron binding capacity (W8-W12 vs. W1). Hepatic markers albumin, globulin, albumin:globulin, and total protein levels were elevated (p0.05) at W12 vs. W1, whereas aspartate aminotransferase and alanine aminotransferase levels were elevated at W1-W12 and W8-W12 vs. PS, respectively. Vitamin E, zinc, selenium, and calcium levels were elevated (p0.05) at W12 vs. W1, whereas Vitamin D was decreased (p0.05). Fatty acids and cardiovascular markers (omega-3 index, cholesterol:high-density lipoprotein [HDL], docosahexenoic acid, low-density lipoprotein [LDL], direct LDL, non-HDL, ApoB) were reduced at W1 vs. PS (p ≤ 0.05). Immune, lipid, and muscle damage biomarkers were frequently outside clinical reference ranges. Routine biomarker monitoring revealed subclinical and clinical changes, suggesting soccer-specific reference ranges. Biomarker monitoring may augment positive adaptation and reduce injuries from stressors incurred during soccer.

ObjectiveTo present the appropriate medical care standards for organizations that sponsor athletic activities for secondary school–aged athletes.Data SourcesTo develop the current standards and identify current best-practices evidence, the task force used a multistep process that included reviewing the existing 2004 Appropriate Medical Care for Secondary School–Aged Athletes consensus points and cross-referencing of National Athletic Trainers' Association (NATA) statements and official documents from the strategic alliance (the NATA, NATA Foundation, Board of Certification, and Commission on Accreditation of Athletic Training Education). Gaps in the recommendations from the 2004 Appropriate Medical Care for Secondary School–Aged Athletes document were identified by the task force, and the new appropriate medical care standards were developed and refined.Conclusions and RecommendationsTwelve standards, with supporting substandards, were developed that encompassed readiness to participate in activity; facilities; equipment; protective materials; environmental policies; nutrition, hydration, and dietary supplementation; wellness and long-term health; comprehensive emergency action plans; on-site immediate care; on-site therapeutic interventions; psychological concerns; and athletic health care administration. Collectively, these standards describe a comprehensive approach to providing appropriate health care to secondary school–aged athletes and should serve as a framework with which organizations can evaluate and improve the medical care supplied to adolescent athletes.

Context Sudden death in sport at the high school and collegiate levels has been described extensively in the literature. However, few epidemiologic data exist on the incidence of sudden death specifically in American youth sport before secondary school athletics. Objective To describe the epidemiology of sudden death in organized youth sports in the United States from 2007 through 2015. Design Descriptive epidemiology study. Setting Organized American youth sports. Patients or Other Participants Cases of sudden death that occurred in youth athletes 17 years of age and younger in non-high school organized sports were included. Data Collection and Analysis Information on sudden deaths between August 1, 2007, and December 31, 2015, was obtained via LexisNexis and other publicly available news or media reports. Total youth sport participation rates from 2007 to 2015 were provided by the Sport & Fitness Industry Association. Athlete age, sex, sport, level of play, event type, date of death, setting, and official and speculated causes of death were examined. Data are presented as deaths per year, percentage of total sudden deaths, and deaths per 10 million participants. Results From 2007 to 2015, 45 sudden deaths were reported in American youth sports. The mean age of patients was 13 ± 2 years. The overall incidence rate was 1.83 deaths/10 million athlete-years. Males experienced a greater number of sudden deaths than females (n = 36/45, 80%). Basketball had the highest number of sudden deaths from 2007 to 2015, with a total of 16 occurrences. The most frequent cause of sudden death was cardiac related (n = 34/45, 76%). Most sudden deaths occurred during practices (n = 32/45, 71%). Conclusions Sudden deaths in organized youth sports in the United States from 2007 through 2015 were most often experienced during practices by males, were cardiac related, and occurred while playing basketball. These findings are similar to those in high school and collegiate sports. This study affirms the need for further epidemiologic research into sudden deaths at the organized youth sport level.

The purpose of this consensus document was to develop feasible, evidence‐based occupational heat safety recommendations to protect the US workers that experience heat stress. Heat safety recommendations were created to protect worker health and to avoid productivity losses associated with occupational heat stress. Recommendations were tailored to be utilized by safety managers, industrial hygienists, and the employers who bear responsibility for implementing heat safety plans. An interdisciplinary roundtable comprised of 51 experts was assembled to create a narrative review summarizing current data and gaps in knowledge within eight heat safety topics: (a) heat hygiene, (b) hydration, (c) heat acclimatization, (d) environmental monitoring, (e) physiological monitoring, (f) body cooling, (g) textiles and personal protective gear, and (h) emergency action plan implementation. The consensus‐based recommendations for each topic were created using the Delphi method and evaluated based on scientific evidence, feasibility, and clarity. The current document presents 40 occupational heat safety recommendations across all eight topics. The purpose of this study was to assess the effectiveness of heat acclimatization (HAz) followed by heat acclimation (HA) on physiological adaptations. 25 male endurance athletes (age 36 ± 12 y, height 178.8 ± 6.39 cm, body mass 73.03 ± 8.97 kg, and VO2peak 57.5 ± 7.0 mL·kg−1·min−1) completed HAz and HA. HAz was 3 months of self-directed summer training. In the laboratory, a 5-day HA prescribed exercise to target a hyperthermic zone (HZHA) of Trec between 38.50 and 39.75 °C for 60 min. Exercise trials were 60 min of running (59% ± 2% VO2peak) in an environmental chamber (wet bulb globe temperature 29.53 ± 0.63 °C) and administered at: baseline, post-HAz, and post-HAz+HA. Measured variables included internal body temperature (Trec), heart rate (HR), and sweat rate (SR). Repeated measure ANOVAs and post hoc comparisons were used to assess statistically significant (p &lt, 0.05) differences. Trec was lower post-HAz+HA (38.03 ± 0.39 °C) than post-HAz (38.25 ± 0.42 °C, p = 0.009) and baseline (38.29 ± 0.37 °C, p = 0.005). There were no differences between baseline and post-HAz (p = 0.479) in Trec. HR was lower post-HAz (143 ± 12 bpm, p = 0.002) and post-HAz+HA (134 ± 11 bpm, p &lt, 0.001) than baseline (138 ± 14 bpm). HR was lower post-HAz+HA than post-HAz (p = 0.013). SR was higher post-HAz+HA (1.93 ± 0.47 L·h−1) than post-HAz (1.76 ± 0.43 L·h−1, p = 0.027). Combination HAz and HA increased physiological outcomes above HAz. This method can be used to improve performance and safety in addition to HAz alone.

Context The Athletic Training Locations and Services (ATLAS) Annual Report suggested that athletic trainer (AT) employment status differed based on geographic locale. However, the influence of geographic locale and school size on AT employment is unknown. Objective To determine if differences existed in the odds of having AT services by locale for public and private schools and by student enrollment for public schools. Design Cross-sectional study. Setting Public and private secondary schools with athletics programs. Patients or Other Participants Data from 20 078 US public and private secondary schools were obtained. Main Outcome Measures(s) Data were collected by the ATLAS Project. Athletic trainer employment status, locale (city, suburban, town, or rural) for public and private schools, and school size category (large, moderate, medium, or small) only for public schools were obtained. The employment status of ATs was examined for each category using odds ratios. Logistic regression analysis produced a prediction model. Results Of the 19 918 public and private schools with available AT employment status and locale, suburban schools had the highest access to AT services (80.1%) with increased odds compared with rural schools (odds ratio = 3.55 [95% CI = 3.28, 3.85]). Of 15 850 public schools with known AT employment status and student enrollment, large schools had the highest rate of AT services (92.1%) with nearly 18.5 times greater odds (odds ratio = 18.49 [95% CI = 16.20, 21.08]) versus small schools. The logistic model demonstrated that the odds of access to an AT increased by 2.883 times as the school size went up by 1 category. Conclusions Nationally, suburban schools and large public schools had the greatest access to AT services compared with schools that were in more remote areas and with lower student enrollment. These findings elucidate the geographic locales and student enrollment levels with the highest prevalence of AT services.

The validity and reliability of GPS units for measuring distance and velocity during linear and team sport simulated movements. J Strength Cond Res 34(11): 3070-3077, 2020-This experimental study aimed to assess the validity and reliability of shirt-mounted 10-Hz global positioning system (GPS) units (Polar Team Pro) for measuring total distance (TD), constant velocity (VelC), and instantaneous velocity (VelI) during linear running and a team sport simulation circuit (TSSC). Fifteen male soccer athletes completed linear tasks (40 and 100 m) at various velocities: walk (W) (4.8-7.9 km·h), jog (J) (8.0-12.7 km·h), run (R) (12.9-19.9 km·h), and sprint (S) (20.0 km·h) and a 120-m TSSC. Global positioning system validity and reliability for TD, VelC, and VelI were compared with criterion measures using 2 methods (a and b) of GPS raw data extraction. When measuring TD for the Polar Team Pro device, validity and reliability measures were5% error at all velocities during the 40-m (with the exception of the S [%CV = 8.03]) and 100-m linear trial (both extraction methods) and TSSC. The GPS mean difference (±SD) for TD during the TSSC using extraction methods (a) and (b) was 0.2 ± 1.2 and 2.2 ± 2.2 m, respectively. The validity of the device in measuring VelC was significantly different (p0.05) at all velocities during the 40 m (exception W) and the 100 m, with effect sizes ranging from trivial to small (exception of 100 m S). VelI was similar (p0.05) at all velocities, except for the W (p = 0.001). The reliability of the device when measuring VelC during the 40 and 100 m was5% CV; however, during the 100 m, VelI ranged from 1.4 to 12.9%. Despite trivial to large effect sizes for validity of TD, this device demonstrated good reliability5% CV during linear and TSSC movements. Similarly, effect sizes ranged from trivial to large for VelC, and yet VelI reliability was good for VelC, but good to poor for VelI.

Exertional heat illness (EHI) risk is a serious concern among athletes, laborers, and warfighters. US Governing organizations have established various activity modification guidelines (AMGs) and other risk mitigation plans to help ensure the health and safety of their workers. The extent of metabolic heat production and heat gain that ensue from their work are the core reasons for EHI in the aforementioned population. Therefore, the major focus of AMGs in all settings is to modulate the work intensity and duration with additional modification in adjustable extrinsic risk factors (e.g., clothing, equipment) and intrinsic risk factors (e.g., heat acclimatization, fitness, hydration status). Future studies should continue to integrate more physiological (e.g., valid body fluid balance, internal body temperature) and biometeorological factors (e.g., cumulative heat stress) to the existing heat risk assessment models to reduce the assumptions and limitations in them. Future interagency collaboration to advance heat mitigation plans among physically active population is desired to maximize the existing resources and data to facilitate advancement in AMGs for environmental heat.

Objectives: The purpose of this study was to examine the relationship between pacing, gastrointestinal temperature (TGI), and percent body mass loss (%BML) on relative race performance during a warm weather 11.3 km road race. Design: Observational study of a sample of active runners competing in the 2014 Falmouth Road Race. Methods: Participants ingested a TGI pill and donned a GPS enabled watch with heart rate monitoring capabilities prior to the start of the race. Percent off predicted pace (%OFF) was calculated for seven segments of the race. Separate linear regression analyses were used to assess the relationship between pace, TGI, and %BML on relative race performance. One-way ANOVA was used to analyse post race TGI (=40 °C vs 0.05). There was a trend in a slower pace (p = 0.055) and greater %OFF (p = 0.056) in runners finishing the race with a TGI > 40 °C. Conclusions: Overall, finish time was influenced by greater variations in pace during the first two miles of the race. In addition, runners who minimized fluid losses and had lower TGI were associated with meeting self-predicted goals.

Thermoregulation includes many physiological, molecular/cellular, and genetic mechanisms that are highlighted in Chap. 2. Molecular and cellular mechanisms of thermal tolerance (on a whole-body level) and relationships to EHS (exertional heat stroke) susceptibility include pathways associated with immune, endocrine, antioxidant, metabolic, skeletal muscle, and nervous system function. Research clearly implicates pathophysiology arising from LPS (lipopolysaccharide)-induced TLR4 (toll-like receptor 4) activation and subsequent endotoxemia/sepsis-induced inflammation and tissue damage. However, the role is not clearly defined because many have not considered the vast complexity in LPS and TLR-associated positive feedback to inflammation. We present aspects of immune function that complicate the relationship between endotoxemia and EHS pathophysiology that should be studied in future research and make it difficult to associate immune-related genotypes with EHS risk. Additionally, we present molecular targets of pharmacological treatments and individual genotypes associated with susceptibility to heat stress and MH (malignant hyperthermia) to depict novel molecular mechanisms likely associated with pathophysiology and EHS susceptibility.

Curtis, RM, Huggins, RA, Benjamin, CL, Sekiguchi, Y, Adams, WM, Arent, SM, Jain, R, Miller, SJ, Walker, AJ, and Casa, DJ. Contextual factors influencing external and internal training loads in collegiate men's soccer. J Strength Cond Res 34(2): 374-381, 2020-This study investigated factors influencing training loads (TL) in collegiate men's soccer. Total distance, high-speed running distance (14.4 km·h), high-intensity heart-rate zone duration (HI HRZ,70% heart rate relative to maximum), and session rating of perceived exertion were assessed daily from 107 male soccer players competing for 5 National Collegiate Athletics Association Division I teams. Differences between athlete role (starter and reserve), position (defender, midfielder, and forward), season phase (preseason, in-season, and postseason), days relative to match (MD-1 to MD-5+), days between matches (4, 4-5,5 days), previous match outcome (win, loss, and draw), and upcoming opponent relative ranking (weaker, trivial, and stronger) were examined. Mean differences (MD) and effect sizes (ESs) with 90% confidence intervals were reported. There were trivial and insignificant differences by player role, position, or upcoming opponent strength, and small-moderate increases in preseason TL compared with in-season (ES [range] = 0.4-0.9). TLs were lower for MD-1 and higher for MD-5+ (ES [range] = 0.4-1.3) when compared with MD-2-4. External loads (ES = -0.40 ± 0.20) were less after wins compared with losses. TLs are increased in the preseason, when training sessions occur greater than 5 days from a match and after losses. Contextualizing factors affecting TLs has implications for developing workload prescription and recovery strategies.

Personalized hydration strategies play a key role in optimizing the performance and safety of athletes during sporting activities. Clinicians should be aware of the many physiological, behavioral, logistical and psychological issues that determine both the athlete’s fluid needs during sport and his/her opportunity to address them; these are often specific to the environment, the event and the individual athlete. In this paper we address the major considerations for assessing hydration status in athletes and practical solutions to overcome obstacles of a given sport. Based on these solutions, practitioners can better advise athletes to develop practices that optimize hydration for their sports.

ContextImplementation of health and safety best practices for the leading causes of sudden death and catastrophic injury has been shown to mitigate risk. However, to our knowledge, no authors have examined progress toward health and safety policy implementation at the state level.ObjectiveTo investigate the progress made by state secondary school leaders in developing and implementing health and safety policies (ie, exertional heat stroke, sudden cardiac arrest, concussion, emergency action plans) and to explore perceived barriers to and strategies for implementation.DesignMixed-methods study.SettingState high school athletics associations and sports medicine advisory committees.Patients or Other ParticipantsCollaborative Solutions for Safety in Sport meeting attendees participated in this study. Thirty-five state leaders (current role experience = 8 ± 6 years) completed the survey. Ten of the 35 participated in follow-up interviews.Data Collection and AnalysisA survey assessing progress on health and safety policy implementation was administered. Respondents indicated whether their state had implemented a policy, made progress without implementation, or made no progress. We conducted follow-up telephone interviews so they could expand on the survey responses. The data were analyzed using descriptive statistics and the general inductive approach.ResultsA total of 89% of respondents reported their states made progress on or implemented health and safety policies during the 2015–2016 academic year. Barriers to policy implementation included cost, a lack of understanding regarding policies versus recommendations, the content and value of policy change, and a false sense of security. Strategies for implementation included varying approaches to change, education of all constituents, and collaborative relationships among key stakeholders.ConclusionsAlthough a majority of respondents reported progress in implementing health and safety policies in their states, perceived barriers pointed to the need for the continued education of state leaders in charge of developing and implementing health and safety policies. Despite these barriers, collaboration among key stakeholders is crucial to successful implementation of best-practice policies in secondary school athletics.

Energy storage related to transient energy sources, such as solar and wind, and their integration with the energy distribution grid is getting increased attention. The characteristics required for this type of storage are quite different from those for energy storage in portable devices. Size and weight are not so important. Instead, matters such as power, cost, calendar life, cycle life, and safety are critical. A new family of hexacyanoferrate materials with the same type of open framework crystal structure as Prussian Blue has been recently developed with characteristics ideally suited for this type of application. Several monovalent cations can be rapidly and reversibly inserted into these materials, with very little crystallographic distortion. This can result in high rates and long cycle lives. A new type of composite negative electrode materials has also been developed that has the rapid kinetics typical of carbon electrodes, with a potential that varies little with the state of charge. The result is the development of a new battery system, the ferrocyanide / stabilized carbon, MHCF-SC, system. It is the purpose of this paper to review these developments.

Objectives: To examine the effects of hydration and hand cooling on lowering body temperature after exercise in the heat. Design: Randomized cross-over design. Methods: Nine recreationally active male participants (mean ± SD; age, 24 ± 4; height, 177.3 ± 9.9 cm; body mass, 76.7 ± 11.6 kg; body fat, 14.7 ± 5.8%) completed a bout of treadmill exercise in a hot environment. After completion of exercise, participants were assigned to the following trials for post-exercise cooling: (1) hydrated with passive rest (HY), (2) hydrated with hand cooling on both hands (HY + 2HC), (3) dehydrated with passive rest (DY), and (4) dehydrated with hand cooling on both hands (DY + 2HC). Within subject differences were assessed using a three-way (Hydration × Condition × Time) repeated measures ANOVA with Tukey's post hoc analysis if significant interactions were found. Results: Irrespective of hydration status, hand cooling on both hands resulted in significantly greater reductions in TREC than passive cooling at minute 20 (0.27°C [0.05, 0.49], ES = 2.08, p = 0.017) (Fig. 1). The reduction in TREC at minute 18 trended towards statistical significance (0.21°C [.003, .42], ES = 1.59, p = 0.053). Hydration status alone and when differentiated among modes of cooling showed no differences on changes of TREC or heart rate across all conditions during post exercise recovery (p > 0.05). Conclusions: Hand cooling on both hands reduced TREC more than passive cooling, however, the cooling rates observed render hand cooling a poor option for cooling. Greater reductions in TREC after exercise or between bouts of exercise may enhance recovery and subsequent performance.

Context:Evidence-based best practices for the recognition and treatment of exertional heat stroke (EHS) indicate that rectal thermometry and immediate, aggressive cooling via cold-water immersion ensure survival from this medical condition. However, little is known about the recovery, medical follow-up, and return to activity after an athlete has suffered EHS.Objective:To highlight the transfer of evidenced-based research into clinical practice by chronicling the treatment, recovery, and return to activity of a runner who suffered an EHS during a warm-weather road race.Design:Case study.Setting:Warm-weather road race.Participant:53-y-old recreationally active man.Intervention:A runner’s treatment, recovery, and return to activity from EHS and 2014 Falmouth Road Race performance.Main Outcomes:Runner’s perceptions and experiences with EHS, body temperature, heart rate, hydration status, exercise intensity.Results:The runner successfully completed the 2014 Falmouth Road Race without incident of EHS. Four dominant themes emerged from the data: predisposing factors, ideal treatment, lack of medical follow-up, and patient education. The first theme identified 3 predisposing factors that contributed to the runner’s EHS: hydration, sleep loss, and lack of heat acclimatization. The runner received ideal treatment using evidence-based best practices. A lack of long-term medical care following the EHS with no guidance on the runner’s return to full activity was observed. The runner knew very little about EHS before the 2013 race, which drove him to seek knowledge as to why he suffered EHS. Using this newly learned information, he successfully completed the 2014 Falmouth Road Race without incident.Conclusions:This case supports prior literature examining the factors that predispose individuals to EHS. Although evidence-based best practices regarding prompt recognition and treatment of EHS ensure survival, this case highlights the lack of medical follow-up and physician-guided return to activity after EHS.

Purpose: Athletic training student aides (SA) are minors in high school that participate in an athletic training experience under the supervision of secondary school athletic trainers (SSAT). The NATA published an official statement on the proper supervision of SAs related to task allowance. As SSATs may also supervise athletic training students (ATSs), it is important to understand the differences in both. Therefore, the objective was to explore the training, task allowance, and perspectives of SAs and ATSs by SSATs during clinical experiences. Methods: We used a cross-sectional, web-based survey for this study. SSATs (n=3,567) from the ATLAS database were recruited and 614 participants (age=39±11years, years credentialed=12±9years) completed the study. An online survey was developed to reflect the NATA official statement for task allowance and supervision of SAs and ATSs. Dependent variables included requirements and task allowance of the SA and ATS based on supervision (direct, autonomous, and restricted), and the personal perceptions from SSATs. Results: 76.5% of SSATs reported having SAs involved in their clinical practice. Less than 50% of SSATs implemented recommended trainings related to emergency preparedness, first aid, and patient privacy. When exploring task allowance, most SSATs were following best practice guidelines for job-related tasks. Additionally, SSATs were allowing ATSs to perform more tasks autonomously as compared to SAs. Conclusions: SSATs are mostly following best practice recommendations for task allowance, whereby they are providing supervision to SAs in the appropriate areas and not allowing certain duties. However, 100% compliance was not met, thus increasing risks to patients and the profession. SSATs should improve training and preparedness for the SAs involved in their clinical practice Additionally, SSATs are allowing ATSs to perform job-related tasks autonomously or with directed supervision during clinical experiences with the exception to administrative tasks and autonomous clinical-decision making.

Relationship between heart rate variability and acute:chronic load ratio throughout a season in NCAA D1 men's soccer players. J Strength Cond Res 35(4): 1103-1109, 2021-The purpose of this study was twofold: (a) to examine the relationship between heart rate variability (HRV) and acute:chronic workload ratio (ACWR)-based training load (TL) metrics and (b) to examine relationships across various A:C ratio-based TL metrics. Heart rate variability in 23 male college soccer players (mean ± SD; age, 21 ± 1 years; body mass, 80.3 ± 5.8 kg; height, 181.9 ± 6.5 cm; %body fat, 11.9 ± 2.0%; and V̇o2max, 51.9 ± 5.0 ml·kg-1·min-1) was measured at 5 time points: week(W)1, W3, W7, W12, and W14 during the 2015 NCAA men's soccer season. Heart rate variability was calculated from beat to beat intervals using a heart rate monitor. Players donned a global position satellite-enabled device that measured the following TL metrics: session time (ST), Player Load (PL), PL·min-1, and total distance (TD). Acute:chronic workload ratio was calculated for each TL metric: ACWR-based ST (ACWRST), ACWR-based PL (ACWRPL), ACWR-based PL·min-1 (ACWRPLM), and ACWR-based TD (ACWRTD): ACWR = week average TLs/mo average (30 ± 1 days) TLs. Relationships between HRV and ACWR-based each TL metric were evaluated using mixed effects models. Tukey pairwise comparisons were used to examine differences between types of ACWR-based TL metrics. An increase in ACWRST significantly reduced HRV throughout a season (-7.4 ± 3.6 m·s-1; p = 0.04). There were significant differences between ACWRPLM and ACWRST, ACWRPL and ACWRTD at W1, ACWRPLM and ACWRST at W3 (p0.05). In conclusion, ACWRST, ACWRPL, and ACWRTD were significantly different from ACWRPLM. ACWRST was found to significantly predict HRV; higher ACWRST was significantly associated with lower HRV. Therefore, tracking of the ACWR using ST may help to optimize athlete's physiological state throughout a season.

Pryor, JL, Adams, WM, Huggins, RA, Belval, LN, Pryor, RR, and Casa, DJ. Pacing strategy of a full Ironman overall female winner on a course with major elevation changes. J Strength Cond Res 32(11): 3080-3087, 2018-The purpose of this study was to use a mixed-methods design to describe the pacing strategy of the overall female winner of a 226.3-km Ironman triathlon. During the race, the triathlete wore a global positioning system and heart rate (HR)-enabled watch and rode a bike outfitted with a power and cadence meter. High-frequency (every km) analyses of mean values, mean absolute percent error (MAPE), and normalized graded running pace and power (accounting for changes in elevation) were calculated. During the bike, velocity, power, cadence, and HR averaged 35.6 km·h, 199 W, 84 rpm, and 155 b·min, respectively, with minimal variation except for velocity (measurement unit variation [MAPE]: 7.4 km·h [20.3%], 11.8 W [7.0%], 3.6 rpm [4.6%], 3 b·min [2.3%], respectively). During the run, velocity and HR averaged 13.8 km·h and 154 b·min, respectively, with velocity varying four-fold more than HR (MAPE: 4.8% vs. 1.2%). Accounting for elevation changes, power and running pace were less variable (raw [MAPE] vs. normalized [MAPE]: 199 [7.0%] vs. 204 W [2.7%]; 4:29 [4.8%] vs. 4:24 min·km [3.6%], respectively). Consistent with her planned pre-race pacing strategy, the triathlete minimized fluctuations in HR and watts during the bike and run, whereas velocity varied with changes in elevation. This case report provides observational evidence supporting the utility of a pacing strategy that allows for an oscillating velocity that sustains a consistent physiological effort in full Ironman races.

Curtis, RM, Huggins, RA, Looney, DP, West, CA, Fortunati, A, Fontaine, GJ, and Casa, DJ. Match demands of National Collegiate Athletic Association Division I men's soccer. J Strength Cond Res 32(10): 2907-2917, 2018-This study aimed to profile positional movement characteristics of National Collegiate Athletic Association (NCAA) Division I male soccer players. Eighteen Division I male soccer players were monitored using global positioning systems, inertial movement, and heart rate (HR) technology during 24 matches over a full competitive season (N = 235 observations). Positional groups were classified as either a forward (F), center midfielder (CM), wide midfielder (WM), or defender (D). Movement was profiled by locomotor (walking [0-7.19 km·h], jogging [7.20-14.39 km·h], running [14.40-21.59 km·h], and sprinting [>21.6 km·h]), and acceleration/deceleration characteristics (low intensity [0-1.99 m·s], moderate intensity [2-3.99 m·s], and high intensity [>4 m·s]). Players averaged distances of 9,367 ± 2,149 m per match at speeds of 91 ± 20 m·min and physiological intensities of 78 ± 8 %HRmax. Center midfielder demonstrated the highest average speeds (97 ± 20 m·min) and covered the most distance (9,941 ± 2,140 m). Wide midfielder accumulated the most sprint distance (391 ± 145 m) and high-intensity accelerations (129 ± 30 n)/decelerations (96 ± 24 n). Several practically meaningful differences exist between positions for internal and external load metrics. Match loads seen in NCAA Division I soccer vary from reports of professional soccer; however, the effects of match regulation, structure, and congestion, which are unique to NCAA soccer, require further investigation. Physical and physiological load monitoring of NCAA soccer may aid coaches and practitioners in the periodization of training programs leading up to and during a competitive soccer season. These data speak to the necessity for examining both internal and external loads by position.

Objective To assess the diagnostic ability of urine reagent strips to identify hypohydration based on urine specific gravity (USG). Design This study examined the agreement of USG between strips and refractometry with Bland-Altman, whereas the diagnostic ability of the strips to assess hypohydration was performed by receiver operating characteristic analysis. Setting Arkansas high school football preseason practice. Participants Four hundred fourteen fresh urine samples were analyzed. Main outcome measures Urine specific gravity was assessed by both reagent strips and refractometry. Cutoffs of >1.020 and >1.025 were used for identifying hypohydration. Results Bland-Altman analysis showed agreement of the 2 methods. Overall diagnostic ability of the urine strip to identify hypohydration was fair (area under the curve 72%-78%). However, the sensitivity to correctly identify hypohydration was poor (63%-71%), and the specificity of correctly identifying euhydration was poor to fair (68%-83%). Conclusion The urine strip method is not valid for assessing hypohydration.

Author(s): Rector, J; Washboume, J; Alonso, A; Cherrington, M; Delonas, T; Huggins, R | Abstract: In January of 1994, we conducted an extensive seismic experiment to find caverns under Victorio Peak, a bioherm reef structure located approximately 80 miles northeast of Las Cruces, New Mexico. Within Victorio Peak there is reputed to be an extensive network of caverns, tunnels and fissures that contain a large and varied treasure consisting of gold bars, Spanish armor, jewels, coins, and human skeletons. We used an array of seismic sources that included a sledgehammer on the surface of the mountain, a sledgehammer impacting on the walls of two deep fissures within the mountain, as well as blasting caps located in boreholes drilled into the mountain. Approximately 2,000 source positions were recorded by 120 receiver channels consisting of geophones cemented into fissure walls and hydrophones deployed in a deep horizontal borehole drilled at the base of the mountain. The data analysis consisted of measuring reduced traveltime and amplitude of the direct arrival and isolating those source/receiver pairs that exhibited anomalously large direct arrival traveltimes and/or low amplitudes. We have currently identified and located a major amplitude anomaly under the peak that will be drilled and explored during the summer of 1994.

Cold water immersion (CWI) has been widely used for enhancing athlete recovery though its use following an Ironman triathlon has never been examined. The purpose of this paper is to determine the influence of CWI immediately following an Ironman triathlon on markers of muscle damage, inflammation and muscle soreness.Prospective cohort study.Thirty three (22 male, 11 female), triathletes participating in the Ironman World Championships volunteered to participate (mean±SD: age=40±11years; height=174.5±9.1cm; body mass=70±11.8kg; percent body fat=11.4±4.1%, finish time=11:03.00±01:25.08). Post race, participants were randomly assigned to a 10-min bout of 10°C CWI or no-intervention control group. Data collection occurred pre-intervention (PRE), post-intervention (POST), 16h (16POST) and 40h (40POST) following the race. Linear mixed model ANOVA with Bonferroni corrections were performed to examine group by time differences for delayed onset muscle soreness (DOMS), hydration indices, myoglobin, creatine kinase (CK), cortisol, C-reactive protein (CRP), IL-6 and percent body mass loss (%BML). Pearson's bivariate correlations were used for comparisons with finishing time. Alpha level was set a priori at 0.05.No significant group by time interactions occurred. Significant differences occurred for POST BML (-1.7±0.9kg) vs. 16POST, and 40POST BML (0.9±1.4, -0.1±1.2kg, respectively; p0.001). Compared to PRE, myoglobin, CRP and CK remained significantly elevated at 40POST. Cortisol returned to PRE values by 16POST and IL-6 returned to PRE values by 40POST.A single bout of CWI did not provide any physiological benefit during recovery from a triathlon within 40h post race. Effect of CWI beyond this time is unknown.

This document is intended to serve as a call to action for all youth sport NGBs to provide support systems for member organizations through the education of league leaders and their members on the current policy and procedure best practices regarding EAPs, SCA, brain and neck injury, EHS, and other potentially threatening medical conditions (Appendix B). This document also discusses preexisting medical conditions, environmental conditions, and emergency medical care, such as athletic training services. The Task Force recognizes that each organization is unique and, therefore, will need to address policy and procedure recommendations differently to ensure the implementation of best practices. Furthermore, the Task Force recognizes that all best-practice policy and procedure recommendations may not be necessary for each sport (eg, lightning policy for indoor sports). Many of the deaths in youth sports are preventable, and it is the goal of the Task Force to support youth sport NGBs in this mission of prevention.

Context: Recent case reports on malignant hyperthermia (MH)-like syndrome in physically active populations indicate potential associations among MH, exertional heat stroke (EHS), and exertional rhabdomyolysis (ER). However, an expert consensus for clinicians working with these populations is lacking. Objective: To provide current expert consensus on the (1) definition of MH; (2) history, etiology, and pathophysiology of MH; (3) epidemiology of MH; (4) association of MH with EHS and ER; (5) identification of an MH-like syndrome; (6) recommendations for acute management of an MH-like syndrome; (7) special considerations for physically active populations; and (8) future directions for research. Setting: An interassociation task force was formed by experts in athletic training, exercise science, anesthesiology, and emergency medicine. The “Round Table on Malignant Hyperthermia in Physically Active Populations” was convened at the University of Connecticut, Storrs, September 17–18, 2015. Conclusions: Clinicians should consider an MH-like syndrome when a diagnosis of EHS or ER cannot be fully explained by clinical signs and symptoms presented by a patient or when recurrent episodes of EHS or ER (or both) are unexplained. Further research is required to elucidate the genetic and pathophysiological links among MH, EHS, and ER.
