12. Cardiac sympathetic nerve activity and ventricular fibrillation during acute myocardial infarction in a conscious sheep model

Abstract

The association between cardiac sympathetic nerve activity (CSNA) and ventricular fibrillation (VF) during acute myocardial infarction (MI) has not been assessed in conscious animal models. During the first 60 rain post-MI, mean blood pressure (MBP), heart rate (HR), and CSNA were recorded continuously in 20 conscious sheep. Resistant sheep (group A, n = 10) were compared with susceptible sheep (group B, n = 10) who developed fatal VF (n = 7) or sustained ventricular tachycardia (VT, n = 3). The mean time to VF/VT was 28.1 [+ or -] 3.3 min. In group B, MBP, HR, and CSNA were averaged at each consecutive minute from baseline at 14 min before the onset of VF/VT and compared with time-matched values in group A. When compared with those of group A, indexes of CSNA burst size increased before the onset of VF/VT: burst area/minute ([F.sub.13,208] = 2.17, P = 0.01) and burst area/100 beats ([F.sub.13,208] = 1.86, P = 0.04). By contrast, burst frequency indexes were not significantly different: burst frequency ([F.sub.13,208] = 1.6, P = 0.09) and burst incidence ([F.sub.13,208] = 1.48, P = 0.13). In group A, CSNA burst area/min and burst area/100 beats did not change across this time period ([F.sub.13,117] = 0.97, P = 0.5, [F.sub.13,117] = 0.96, P = 0.7) but increased with time in group B ([F.sub.13,91] = 2.3, P = 0.01; and [F.sub.13,91] = 2.25, P = 0.01). Between-group comparisons demonstrated no differences in time of onset of ventricular ectopic beats: 18.5 (range 12-24) in group A versus 15.0 min (range 7-22) in group B (Mann-Whitney U-test, P = 0.09). Pre-MI baroreflex slopes were similar: R-R slopes were 11.8 [+ or -] 2 and 15.6 [+ or -] 1.1 ms/mmHg ([t.sub.18] = - 1.6, P = 0.14). CSNA slopes were -1.8 [+ or -] 0.3 and -2.3 [+ or -] 0.2%/mmHg ([t.sub.18] = -1.4, P = 0.2). Abstract

BACKGROUND: Genetic variation at chromosome 9p21 is a recognized risk factor for coronary heart disease (CHD). However, its effect on disease progression and subsequent events is unclear, raising questions about its value for stratification of residual risk.METHODS: A variant at chromosome 9p21 (rs1333049) was tested for association with subsequent events during follow-up in 103 357 Europeans with established CHD at baseline from the GENIUS-CHD (Genetics of Subsequent Coronary Heart Disease) Consortium (73.1% male, mean age 62.9 years). The primary outcome, subsequent CHD death or myocardial infarction (CHD death/myocardial infarction), occurred in 13 040 of the 93 115 participants with available outcome data. Effect estimates were compared with case/control risk obtained from the CARDIoGRAMplusC4D consortium (Coronary Artery Disease Genome-wide Replication and Meta-analysis [CARDIoGRAM] plus The Coronary Artery Disease [C4D] Genetics) including 47 222 CHD cases and 122 264 controls free of CHD.RESULTS: Meta-analyses revealed no significant association between chromosome 9p21 and the primary outcome of CHD death/myocardial infarction among those with established CHD at baseline (GENIUSCHD odds ratio, 1.02; 95% CI, 0.99-1.05). This contrasted with a strong association in CARDIoGRAMPlusC4D odds ratio 1.20; 95% CI, 1.18-1.22; P for interaction < 0.001 compared with the GENIUS-CHD estimate. Similarly, no clear associations were identified for additional subsequent outcomes, including all-cause death, although we found a modest positive association between chromosome 9p21 and subsequent revascularization (odds ratio, 1.07; 95% CI, 1.04-1.09).CONCLUSIONS: In contrast to studies comparing individuals with CHD to disease-free controls, we found no clear association between genetic variation at chromosome 9p21 and risk of subsequent acute CHD events when all individuals had CHD at baseline. However, the association with subsequent revascularization may support the postulated mechanism of chromosome 9p21 for promoting atheroma development.

Abstract

The relation between plasma brain natriuretic peptide (BNP) and the C-type natriuretic peptide (CNP) during the development of heart failure was investigated in sheep. Circulating levels of atrial natriuretic peptide (ANP), BNP and CNP were measured after acute and chronic ventricular pacing. Results showed an inverse relationship between pacing and arterial pressure, cardiac output, urine, sodium excretion and a positive relationship with left atrial pressure and plasma ANP, BNP and CNP.

Abstract

Pathophysiological levels of atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) were co-infused into normal male subjects to determine if there are synergistic effects between ANP and BNP. Both peptides have similar physiological effects and are released into the plasma following myocardial infarction of congestive heart failure. The results show that co-infusion causes additive physiological effects, probably by causing the dissociation of pre-bound peptides from their clearance or biological receptors.

Abstract

In normal and heart-failed sheep, angiotensin II (ANG II) is involved in the regulation of thirst and hemodynamics. In sheep the antagonist of the ANG II receptor, losartan, removes the drinking responses to heart failure, causes diuresis and anti-natriuresis, increases the activity of the plasma renin and the amount of aldosterone and ANG II in both paced and unpaced states. The inhibition of the plasma arginine vasopressin activity and a decrease in the left ventricular systolic and arterial pressure are seen only in the paced state.

Short term ouabain treatment does not change the plasma concentration of vasoactive hormones or pressor responsivesness in healthy males. Ouabain treatment decreases heart rate and enhances pulse pressure, but has no influence on pressor responsiveness to either norepinephrine or angiotensin II. Atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP), heart secreted hormones in humans that circulate in the plasma, exhibit the same natriuretic endocrine and hemodynamic effects. A study of eight normal men receiving constant infusions of ANP with and without superimposed infusions of BNP, with a random order design revealed the interaction of human BNP and ANP. The important BNP or ANP interactions of metabolism at plasma levels occurred in heart failure.

Abstract

A study was conducted to investigate the sequential, renal, hormonal and hemodynamic responses in normotensive conscious sheep by using low-dose atrial natriuretic factor (ANF) infusions. It assessed the effect of autonomic (baroreceptor), adrenocortical and pressor responsiveness to exogenous incitement by these infusions. Continuous injection of ANF in conscious sheep in low doses that hardly increase circulating plasma ANF levels have essential cardiovascular functions and can lower pressor responsiveness to physiological increases in ANG II.

Abstract

Study objective\ud \ud A 2-hour accelerated diagnostic pathway based on the Thrombolysis in Myocardial Infarction score, ECG, and troponin measures (ADAPT-ADP) increased early discharge of patients with suspected acute myocardial infarction presenting to the emergency department compared with standard care (from 11% to 19.3%). Observational studies suggest that an accelerated diagnostic pathway using the Emergency Department Assessment of Chest Pain Score (EDACS-ADP) may further increase this proportion. This trial tests for the existence and size of any beneficial effect of using the EDACS-ADP in routine clinical care.\ud \ud Methods\ud \ud This was a pragmatic randomized controlled trial of adults with suspected acute myocardial infarction, comparing the ADAPT-ADP and the EDACS-ADP. The primary outcome was the proportion of patients discharged to outpatient care within 6 hours of attendance, without subsequent major adverse cardiac event within 30 days.\ud \ud Results\ud \ud Five hundred fifty-eight patients were recruited, 279 in each arm. Sixty-six patients (11.8%) had a major adverse cardiac event within 30 days (ADAPT-ADP 29; EDACS-ADP 37); 11.1% more patients (95% confidence interval 2.8% to 19.4%) were identified as low risk in EDACS-ADP (41.6%) than in ADAPT-ADP (30.5%). No low-risk patients had a major adverse cardiac event within 30 days (0.0% [0.0% to 1.9%]). There was no difference in the primary outcome of proportion discharged within 6 hours (EDACS-ADP 32.3%; ADAPT-ADP 34.4%; difference −2.1% [−10.3% to 6.0%], P=.65).\ud \ud Conclusion\ud \ud There was no difference in the proportion of patients discharged early despite more patients being classified as low risk by the EDACS-ADP than the ADAPT-ADP. Both accelerated diagnostic pathways are effective strategies for chest pain assessment and resulted in an increased rate of early discharges compared with previously reported rates.
