4. Vermeidung von Eingriffsverwechslungen

Abstract

Zusammenfassung: Die Verwechslung bei operativen Eingriffen führt bei den betroffenen Patienten zu einem erheblichen individuellen Schaden. Deshalb wurden von Organisationen wie der WHO, der Joint Commission International (JCI) oder dem Aktionsbündnis Patientensicherheit e.V. Verfahren zur Vermeidung einer Eingriffsverwechslung entwickelt. Grundlage ist die aktive Einbindung des Patienten in den Prozess der Eingriffsidentifikation. So werden in einer durch das Aktionsbündnis Patientensicherheit empfohlenen Checkliste die Aufklärung des Patienten, Markierung des Eingriffsorts, Identifikation vor der Narkoseeinleitung und das „team time out“ vor dem Operationsbeginn aufgeführt. In den DRK Kliniken Berlin wurde vor der 2. Rezertifizierung durch die JCI zur Umsetzung der Patientensicherheitsziele das Verfahren zur Vermeidung von Eingriffsverwechslungen eingeführt und mit einem Formblatt die Einhaltung der 4 Schritte dokumentiert. Zur erfolgreichen Umsetzung sind alle Mitarbeiter und v. a. die Leiter der Kliniken und Abteilungen aktiv einzubeziehen, um die Akzeptanz dauerhaft zu erhalten und keine Verwechslungen in den DRK Kliniken Berlin mehr zuzulassen.

Abstract

Background Since 2015, total elbow arthroplasty and radial head arthroplasty procedures including distal humeral hemiarthroplasty and capitellar replacement have been documented in the elbow arthroplasty registry of the DVSE (Deutsche Vereinigung fur Schulter- und Ellenbogenchirurgie e.V.). Methods Primary arthroplasties that have been documented until 2018 were analysed. Results A total of 73 elbow and 56 radial head arthroplasties have been documented. Of the elbow arthroplasties, there were 63 total elbows with and 9 without radial head replacement. One was a distal humerus hemiprosthesis. Of the 56 radial head arthroplasties, there were one monobloc and 55 modular prostheses. Data regarding age, sex, indication for surgery, technical details of the surgery and early complications were analysed. Conclusion The documented 76 elbow and 56 radial head arthroplasties pose a good starting point. With good documentation, the detailed collection of data will allow us to analyse larger cohorts in the future.

Abstract

Introduction: Early reports of stemless shoulder arthroplasty have shown promising clinical and radiological outcomes. The purpose of this study was to report on the mid-term results of an implant that utilises a ceramic humeral head. Methods: A prospective, consecutive, multicentre study of stemless shoulder prosthesis with a minimum of four years of follow-up was conducted between August 2009 and May 2012. The adjusted Constant–Murley Score (CMS), revision rate and presence of radiolucent lines were recorded at intervals. Results: A total of 207 patients were eligible for study inclusion; 62.8% were female and mean age was 64.8 years (range 30–86). Mean follow-up was 70.7 months (range 48–100), 73% underwent TSA and 27% hemiarthroplasty. The mean CMS improvement was 42.6 (p < 0.0001) at 48 months. Radiolucencies were present in 2.7% of humeral zones and 14% of glenoid zones at 48-month follow-up. The revision rate was 6.3% with rotator cuff failure (2.9%) the most common indication. Conclusions: Mid-term results demonstrate that the studied stemless implant with a ceramic humeral head had clinical and radiological outcomes that are comparable to other reported studies. Abstract

Highly luminescent (CdSe)ZnS nanocrystals, with band edge emission in the red region of the visible spectrum, were successfully synthesized and incorporated in a resist, namely mr-NIL 6000. The nanocomposite material was imprinted by using conventional nanoimprint lithography (NIL) process. We report on the fabrication and characterization of nanoimprinted photonic crystals in this new functional material. Experiments showed good imprint properties of the NC/polymer based material and that the surface nanostructuration improves the light extraction efficiency by over 2 compared to a nanoimprinted unpatterned surface.
