1. Redeployment Among Primary Care Nurses During the COVID-19 Pandemic: A Qualitative Study

Abstract

Introduction: Throughout the COVID-19 pandemic, primary care nurses were often redeployed to areas outside of primary care to mitigate staffing shortages. Despite this, there is a scarcity of literature describing their perceptions of and experiences with redeployment during the pandemic. Objectives: This paper aims to: 1) describe the perspectives of primary care nurses with respect to redeployment, 2) discuss the opportunities/challenges associated with redeployment of primary care nurses, and 3) examine the nature (e.g., settings, activities) of redeployment by primary care nurses during the COVID-19 pandemic. Methods: In this qualitative study, semi-structured interviews were conducted with primary care nurses (i.e., Nurse Practitioners, Registered Nurses, and Licensed/Registered Practical Nurses), from four regions in Canada. These include the Interior, Island, and Vancouver Coastal Health regions in British Columbia; Ontario Health West region in Ontario; the province of Nova Scotia; and the province of Newfoundland and Labrador. Data related to redeployment were analyzed thematically. Results: Three overarching themes related to redeployment during the COVID-19 pandemic were identified: (1) Call to redeployment, (2) Redeployment as an opportunity/challenge, and (3) Scope of practice during redeployment. Primary care nurses across all regulatory designations reported variation in the process of redeployment within their jurisdiction (e.g., communication, policies/legislation), different opportunities and challenges that resulted from redeployment (e.g., scheduling flexibility, workload implications), and scope of practice implications (e.g., perceived threat to nursing license). The majority of nurses discussed experiences with redeployment being voluntary in nature, rather than mandated. Conclusions: Redeployment is a useful workforce strategy during public health emergencies; however, it requires a structured process and a decision-making approach that explicitly involves healthcare providers affected by redeployment. Primary care nurses ought only to be redeployed after other options are considered and arrangements made for the care of patients in their original practice area.

Abstract

Abstract Background Large volumes of sitting time have been associated with multiple health risks. To reduce sitting time of office workers working for a Dutch insurance company, the Dynamic Work intervention was developed. The primary objective of this paper is to describe the study protocol of the Dynamic Work study, which aims to evaluate if this multicomponent intervention is (cost-)effective in reducing total sitting time on the short-term (≈3 months) and longer-term (≈12 months) compared to usual practice. Methods/design This two-arm cluster randomized controlled trial will recruit 250 desk-based office workers working at different locations of an insurance company in the Netherlands. After baseline measurements, departments will be matched in pairs and each pair will be randomly assigned to the control or intervention condition. The multicomponent intervention contains organizational (i.e. face to face session with the head of the department), work environmental (i.e. the introduction of sit-stand desks and cycling workstations), and individual elements (i.e. counselling and activity/sitting tracker with a self-help program booklet). The counselling involves two group intervention sessions and four on-site department consultations with an occupational physiotherapist. Sitting time (primary outcome), upright time and step counts will be assessed objectively using the activPAL activity monitor at baseline, short-term (approximately 3 months) and longer-term (12 months). Other outcomes will include: self-reported lifestyle behaviours, anthropometrics, work-related outcomes (i.e. absenteeism, presenteeism, work performance, work-related stress), health-related outcomes (i.e. vitality, musculoskeletal symptoms, need for recovery, quality of life), and costs from both company and societal perspective. The study will include economic and process evaluations. Discussion This study will assess the longer-term (cost-) effectiveness of a multicomponent workplace intervention aimed at reducing sitting time in comparison with usual practice. Furthermore, the process evaluation will provide insights in factors associated with successful implementation of this intervention. Abstract

Abstract Background Sitting too much has been associated with negative health outcomes. ‘The End of Sitting’ is a newly developed office landscape that moves away from the traditional chair-desk setup. The landscape aims to reduce sitting time by offering a variety of (supported) standing positions. The aim of this study was to determine the usage of the landscape after being placed in the main entrance hall of the VU University in Amsterdam. Methods We observed the number of spontaneous visitors as well as the duration of visits, changes to another location within the landscape, and adopted postures. Using questionnaires reasons (not) to visit the landscape, perceived affordances of the landscape and associations with long-term use were determined. Results Observed numbers of visitors were relatively low and duration of visits were short, which seemed to indicate visitors were trying out the landscape. The majority of visitors were in an upright position, reflecting the designers’ intentions. Visitors indicated that long-term use would be pleasant to them. Conclusion ‘The End of Sitting’ landscape received positive reactions but number of visits were limited in the few months that it was placed in the university main entrance hall. The landscape might be better suited for designated working or study spaces, for which it was originally intended. It might also be worth to explore the landscapes suitability for short stay environments, such as waiting rooms.

4. The user and non-user perspective: Experiences of office workers with long-term access to sit-stand workstations.

Abstract

ObjectiveSit-stand workstations have been shown to be effective in reducing sitting time in office workers. The aim of this study was to explore reasons for use and non-use of sit-stand workstations and strategies to decrease sitting and increase physical activity in the workplace from perspectives of users and non-users, as well as from managers and ergo-coaches.MethodsSix group interviews with employees who have had access to sit-stand workstations for several years were conducted in a large semi-governmental organisation in the Netherlands. Verbatim transcripts were analysed using thematic analysis. Open coding was conducted by three researchers and codes and themes were discussed within the research team.ResultsThematic analysis resulted in two major themes: 1) Reasons for use and non-use and 2) Strategies to increase standing and physical activity in the workplace. Shared and distinct reasons for use and non-use were identified between users and non-users of the sit-stand workstations. The most important reasons for use indicated by users were that they had experiencing immediate benefits, including staying alert and increasing focus; these benefits were not acknowledged by non-users. Non-users indicated that sitting was comfortable for them and that they were therefore not motivated to use the standing option. Strategies to increase the use of the standing option included an introductory phase to become familiar with working while standing and to experience the immediate benefits that come from using the standing option. Furthermore, providing reminders to use the standing option was suggested as a strategy to increase and sustain the use of sit-stand workstations. Increased use may lead to a change in the sitting culture within the organisation, as more employees would adopt active movement behaviours.ConclusionImmediate benefits of the use of the standing option-only mentioned by the users-was the most distinct reason to use sit-stand workstations. Future research should explore how to motivate potential users to adhere to an introductory phase in order to experience these immediate benefits, whether it is linked to the use of sit-stand workstations or other interventions to reduce sitting time.

Abstract

The kinetics of solid-state reactions involving non-metallic inclusions (NMIs) in steel is a topic of growing interest due to the need to understand the effects of heat treatments on NMI composition and stability. In this study, a High Temperature Environmental Scanning Electron Microscope (HT-ESEM) is utilized to conduct in-situ observations of various types of NMIs, including calcium aluminate complex, CaS, MgAl₂O₄-CaS, MnS, and TiN, in steels at temperatures of 800 °C and 900 °C for different holding times. The observed morphological variations in the NMIs indicate a decrease in the fractions of CaS and MnS, promoting void formation and ferrite–austenite phase transformation. To estimate the NMI reactions with the residual atmosphere in the sample chamber during experiments, HT-ESEM-driven thermodynamic calculations have been performed using FactSage 8.2 software. These calculations provide valuable insights into the interpretation of the experimental data, facilitating a better understanding of the complex NMI reactions in steel.

Archive et numérique sont deux mots dont la conjonction est devenue presque un topique. Nous mesurons, au fil du temps, que le public se déplace moins dans les dépôts d’archives, la consultation d’un document se faisant à travers une application numérique. Pourtant la donnée numérique n’est pas l’archive, elle n’est qu’une représentation approchante. Autrement dit, la donnée numérique n’est qu’un artefact de la donnée archivistique. L’appréhension de ce lien distendu entre document archivistique et document numérique se manifeste à travers la « granularité des données ». D’un autre côté, la donnée archivistique est par nature beaucoup plus statique, car elle se trouve dans un entrepôt ou une bibliothèque, alors que, sans être volatile, la donnée numérique est interopérable, grâce aux métadonnées qui la caractérisent. À travers un cas concret d’archives judiciaires, le lecteur trouvera ici de quoi nourrir sa réflexion sur la granularité des données et les métadonnées. Archive and digital are two words whose conjunction has become almost topical. Over time, we are measuring that the public is moving less in repositories, as the consultation of a document is done through a digital application. Yet digital data is not the archive, it is only an approximate representation. In other words, digital data is only an artefact of the archival data. The apprehension of this distorted link between the archival record and the digital record is manifested through the "granularity of the data". On the other hand, archival data is by nature much more static, as it is located in a warehouse or library, whereas, without being volatile, digital data is interoperable, thanks to the metadata that characterizes it. Through a case study of court records, the reader will find here food for thought on data granularity and metadata.

Abstract

Objective: To assess the cost-effectiveness and return-on-investment (ROI) of the Dynamic Work (DW) Intervention, a worksite intervention aimed at reducing sitting time among office workers.Methods: In total, 244 workers were randomized to the intervention or control group. Overall sitting time, standing time, step counts, quality-adjusted life years (QALYs), and costs were measured over 12 months. The cost-effectiveness analysis was performed from the societal perspective and the ROI analysis from the employers’ perspective.Results: No significant differences in effects and societal costs were observed between groups. Presenteeism costs were significantly lower in the intervention group. The probability of the intervention being cost-effective was 0.90 at a willingness-to-pay of 20,000€/QALY. The probability of financial savings was 0.86.Conclusion: The intervention may be considered cost-effective from the societal perspective depending on the willingness-to-pay. From the employer perspective, the intervention seems cost-beneficial.

Abstract

Late Pleistocene hominin postcranial specimens from Southeast Asia are relatively rare. Here we describe and place into temporal and geographic context two partial femora from the site of Trinil, Indonesia, which are dated stratigraphically and via Uranium-series direct dating to ca. 37–32 ka. The specimens, designated Trinil 9 and 10, include most of the diaphysis, with Trinil 9 being much better preserved. Microcomputed tomography is used to determine cross-sectional diaphyseal properties, with an emphasis on midshaft anteroposterior to mediolateral bending rigidity (Ix/Iy), which has been shown to relate to both body shape and activity level in modern humans. The body mass of Trinil 9 is estimated from cortical area and reconstructed length using new equations based on a Pleistocene reference sample. Comparisons are carried out with a large sample of Pleistocene and Holocene East Asian, African, and European/West Asian femora. Our results show that Trinil 9 has a high Ix/Iy ratio, most consistent with a relatively narrow-bodied male from a mobile hunting-gathering population. It has an estimated body mass of 55.4 kg and a stature of 156 cm, which are small relative to Late Pleistocene males worldwide, but larger than the penecontemporaneous Deep Skull femur from Niah Cave, Malaysia, which is very likely female. This suggests the presence of small-bodied active hunter-gatherers in Southeast Asia during the later Late Pleistocene. Trinil 9 also contrasts strongly in morphology with earlier partial femora from Trinil dating to the late Early-early Middle Pleistocene (Femora II–V), and to a lesser extent with the well-known complete Femur I, most likely dating to the terminal Middle-early Late Pleistocene. Temporal changes in morphology among femoral specimens from Trinil parallel those observed in Homo throughout the Old World during the Pleistocene and document these differences within a single site.

© 2022 The AuthorsPreviously, we observed no significant reductions in sitting time of the multicomponent Dynamic Work (DW) intervention among office workers. In this study we used mixed-method data to understand context, implementation (i.e. recruitment and delivery) and mechanism of impact (i.e. experiences) of the DW intervention and to explore whether an higher implementation index score led to larger changes in participants’ outcomes. We found considerable variation across departments regarding context (i.e. different size and work tasks) and implementation (i.e. delivery varied). Satisfaction with the DW intervention was high. An higher implementation index score was associated with lower overall sitting time, lower occupational sitting time, higher number of steps/day and steps/day at work at 4-months, which was maintained at 8-month for occupational sitting time. These findings provide an understanding that implementation was affected by a lack of availability of intervention components, department policy, work tasks, positioning and work location. Abstract

Background: Published findings on breast cancer risk associated with different types of menopausal hormone therapy (MHT) are inconsistent, with limited information on long-term effects. We bring together the epidemiological evidence, published and unpublished, on these associations, and review the relevant randomised evidence. Methods: Principal analyses used individual participant data from all eligible prospective studies that had sought information on the type and timing of MHT use; the main analyses are of individuals with complete information on this. Studies were identified by searching many formal and informal sources regularly from Jan 1, 1992, to Jan 1, 2018. Current users were included up to 5 years (mean 1·4 years) after last-reported MHT use. Logistic regression yielded adjusted risk ratios (RRs) comparing particular groups of MHT users versus never users. Findings: During prospective follow-up, 108 647 postmenopausal women developed breast cancer at mean age 65 years (SD 7); 55 575 (51%) had used MHT. Among women with complete information, mean MHT duration was 10 years (SD 6) in current users and 7 years (SD 6) in past users, and mean age was 50 years (SD 5) at menopause and 50 years (SD 6) at starting MHT. Every MHT type, except vaginal oestrogens, was associated with excess breast cancer risks, which increased steadily with duration of use and were greater for oestrogen-progestagen than oestrogen-only preparations. Among current users, these excess risks were definite even during years 1–4 (oestrogen-progestagen RR 1·60, 95% CI 1·52–1·69; oestrogen-only RR 1·17, 1·10–1·26), and were twice as great during years 5–14 (oestrogen-progestagen RR 2·08, 2·02–2·15; oestrogen-only RR 1·33, 1·28–1·37). The oestrogen-progestagen risks during years 5–14 were greater with daily than with less frequent progestagen use (RR 2·30, 2·21–2·40 vs 1·93, 1·84–2·01; heterogeneity p Interpretation: If these associations are largely causal, then for women of average weight in developed countries, 5 years of MHT, starting at age 50 years, would increase breast cancer incidence at ages 50–69 years by about one in every 50 users of oestrogen plus daily progestagen preparations; one in every 70 users of oestrogen plus intermittent progestagen preparations; and one in every 200 users of oestrogen-only preparations. The corresponding excesses from 10 years of MHT would be about twice as great.

Abstract

In the phase 3 POLLUX trial, daratumumab in combination with lenalidomide and dexamethasone (D-Rd) significantly improved progression-free survival in patients with relapsed/refractory multiple myeloma (RRMM) compared with lenalidomide and dexamethasone (Rd) alone. Here, we present patient-reported outcomes (PROs) from POLLUX, assessed using the validated European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Core 30-item (EORTC QLQ-C30) and the EuroQol 5-dimensional descriptive system (EQ-5D-5L) questionnaires. Changes from baseline are presented as least-squares mean changes with 95% confidence intervals (CIs) derived from a mixed-effects model. PRO assessment compliance rates were high and similar in both D-Rd and Rd groups through cycle 40 (week 156). In this on-treatment analysis, mean changes from baseline were significantly greater in EORTC QLQ-C30 global health status, physical functioning, and pain scores in the D-Rd group versus the Rd group at multiple time points; however, magnitude of changes was low, suggesting no meaningful impact on health-related quality of life (HRQoL). Subgroup results were similar to those in the overall population. In the POLLUX study, baseline HRQoL was maintained with prolonged D-Rd treatment. These findings complement the sustained and significant improvement in progression-free survival observed with D-Rd and supports its use in patients with RRMM. Abstract

Sit&ndash, stand workstations have shown to reduce sitting time in office workers on a group level. However, movement behaviour patterns might differ between subgroups of workers. Therefore, the objective of this study was to examine sitting, standing and stepping outcomes between habitual users and non-users of sit&ndash, stand workstations. From an international office population based in the Netherlands, 24 users and 25 non-users of sit&ndash, stand workstations were included (all had long-term access to these workstations). Using the ActivPAL, sitting, standing and stepping were objectively measured during and outside working hours. Differences in outcomes between users and non-users were analysed using linear regression. During working hours, users sat less (&minus, 1.64, 95% IC= &minus, 2.27&ndash, &minus, 1.01 hour/8 hour workday) and stood more (1.51, 95% IC= 0.92&ndash, 2.10 hour/8 hour workday) than non-users. Attenuated but similar differences were also found for total sitting time over the whole week. Furthermore, time in static standing bouts was relatively high for users during working hours (median= 0.56, IQR = 0.19&minus, 1.08 hour/8 hour workday). During non-working hours on workdays and during non-working days, no differences were found between users and non-users. During working hours, habitual users of their sit&ndash, stand workstation sat substantially less and stood proportionally more than non-users. No differences were observed outside working hours, leading to attenuated but similar differences in total sitting and standing time between users and non-users for total days. This indicated that the users of sit&ndash, stand workstations reduced their sitting time at work, but this seemed not to be accompanied by major carry-over or compensatory effects outside working hours.

Abstract

Objective Prolonged sitting, which is highly prevalent in office workers, has been associated with several health risks. The aim of this study was to evaluate the Dynamic Work intervention by determining its effect on total sitting time at the 8-month follow-up in comparison to the control. Methods This two-arm pragmatic cluster randomised controlled trial included 244 office workers from 14 different departments of a large, Dutch insurance company. The Dynamic Work intervention was a real-life, worksite intervention that included environmental components (i.e. sit-stand workstations), organisational components (i.e. group sessions), and individual components (e.g. activity/sitting trackers). Outcomes were assessed at baseline, 4-month follow-up, and 8-month follow-up. The primary outcome was total sitting time per day, objectively assessed using the activPAL activity monitor at 8-month follow-up. Secondary outcomes included other total and occupational movement behaviour outcomes, health-related outcomes, and work-related outcomes. Data analyses were performed using linear and logistic mixed models. Results Total sitting time did not differ between the intervention and control group at the 8-month follow-up. Secondary outcomes also showed no difference between the intervention and control group at either the 4-month or at 8-month follow-up, with the exception of number of occupational steps, which showed a statistically significant effect at 4-month follow-up (but not at 8-month follow-up) of 913 (95% CI = 381–1445) steps/8-h working day. Conclusions This study evaluated the effectiveness of a real-life worksite intervention to reduce sitting time and showed little to no effect. This may be due to the relatively low intensity of the intervention, i.e. that it only involved the replacement of 25% of sitting workstations with sit-stand workstations. Future research should focus on the evaluation of more intensive real-life worksite interventions that are still feasible for implementation in daily practice. Abstract

A comprehensive electron spectroscopic study combined with partial electron yield measurements around the Br 1s ionization threshold of HBr at ≅13.482 keV is reported. In detail, the Br 1s⁻¹ X-ray absorption spectrum, the 1s⁻¹ photoelectron spectrum as well as the normal and resonant KLL Auger spectra are presented. Moreover, the L-shell Auger spectra measured with photon energies below and above the Br 1s⁻¹ ionization energy as well as on top of the Br 1s⁻¹σ* resonance are shown. The latter two Auger spectra represent the second step of the decay cascade subsequent to producing a Br 1s⁻¹ core hole. The measurements provide information on the electron and nuclear dynamics of deep core-excited states of HBr on the femtosecond timescale. From the different spectra the lifetime broadening of the Br 1s⁻¹ single core-hole state as well as of the Br(2s⁻²,2s⁻¹2p⁻¹,2p⁻²) double core-hole states are extracted and discussed. The slope of the strongly dissociative HBr 2p⁻²σ* potential energy curve is found to be about −13.60 eV Å⁻¹. The interpretation of the experimental data, and in particular the assignment of the spectral features in the KLL and L-shell Auger spectra, is supported by relativistic calculations for HBr molecule and atomic Br.

Abstract

In this study, activity, selectivity and stability of vanadium catalysts supported on zirconia, hafnia, and alumina were examined in the oxidation of methanethiol and methanol to formaldehyde. The 3 wt-% vanadia–alumina catalyst with low VOx surface density showed the highest activity in the formaldehyde production. However, during the stability test, this catalyst deactivated due to the change in the oxidation state of vanadium from V5+ to V4+, decrease the amount of surface vanadium species and the formation of sulphates on the material surface. Zirconia and hafnia supported catalysts with high VOx surface density demonstrated better stability in the reaction conditions, but their activity in the formaldehyde production was lower. One reason for the lower activity might be the formation of metal-mixed oxide phases between vanadia and the support, which could also explain the decreased sulphur deposition on zirconia and hafnia after vanadium impregnation.

Abstract

Contaminated methanol has very good potential for being utilized in formaldehyde production instead of its destructive abatement. The activities, selectivities and stabilities of cobalt–alumina and cobalt–alumina–ceria catalysts prepared by the hydrotalcite-method were investigated in formaldehyde production from emissions of methanol and methanethiol. Catalysts were thoroughly characterized and the relationships between the characterization results and the catalytic performances were drawn. The preparation method used led to the formation of spinel-type structures in the form of Co₂AlO₄ based on x-ray diffraction (XRD) and Raman spectroscopy. Ceria seems to be present as CeO₂, even though interaction with alumina is possible in the fresh catalyst. The same structure is maintained after pelletizing the cobalt–alumina–ceria catalyst. The cobalt–alumina–ceria catalyst was slightly better in formaldehyde production, probably due to lower redox temperatures and higher amounts of acidity and basicity. Methanol conversion is negatively affected by the presence of methanethiol; however, formaldehyde yields are improved. The stability of the pelletized catalyst was promising based on a 16 h experiment. During the experiment, cobalt was oxidized (Co2+ → Co3+), cerium was reduced (Ce4+ → Ce3+) and sulfates were formed, especially on the outer surface of the pellet. These changes affected the low temperature performance of the catalyst; however, the formaldehyde yield was unchanged.

Abstract

Background: To decrease the detrimental health effects of prolonged sitting, the implementation of sit-stand workstations is a commonly used intervention for office workers. Most studies on this topic evaluated the effects of newly introduced sit-stand workstations. The objective of this study was to determine how often and how long the standing option is used and how the use of sit-stand workstations is perceived in office workers with long-term access to these workstations. Methods: Using an online survey, 1098 office employees responded to questions about frequency of usage of the sit-stand workstation, sitting time, physical activity, and positive and negative perceptions of the use of the sit-stand workstations. Results: Based on the frequency of use, three user groups were identified: non-users (32.1%), monthly/weekly users (37.5%) and daily users (30.4%). Non-users reported to sit more, stand less and have longer bouts of sitting, compared to monthly/weekly users, and these differences were even larger compared to daily users. A higher proportion of daily users perceived the use of the sit-stand workstation as being more healthy and appealing and making them more productive and energetic compared to the non-users. A higher proportion of the non-users perceived it as being uncomfortable, distracting, and unpractical, compared to the other user groups. Conclusions: The differences between the three identified user groups with respect to sitting, standing and perceptions of sit-stand workstations, might be helpful in tailoring future interventions to reduce occupational sitting time, to increase the reach, effectiveness and sustainability.

Abstract

Rehabilitation for people with spinal cord injury in many low- and middle-income countries is not avail-able or is in the early stages of development. However, rehabilitation is recognized as crucial in order to optimize functional recovery and outcomes for patients with spinal cord injury. With an increasing incidence of spinal cord injury, the unmet need for rehabilitation is huge. This report describes the early development of a specialist rehabilitation service for spinal cord injury in Madagascar, one of the poorest countries in the world. The sustained input to an expanding rehabilitation team has led to reductions in avoidable complications. The input of the rehabilitation team has been welcomed by the neurosurgery department, which has recognized fewer delays in patients undergoing surgical treatments. Cost, lack of resources and trained staff, and poor understanding of disability continue to provide challenges. However, the development of the rehabilitation service using low technology, but with a high level of knowledge and systematic management, is a source of considerable pride. This development in Madagascar can be regarded as a model for spinal cord injury rehabilitation in other low-resource settings.

Abstract

Background: In all societies, the burden and cost of allergic and chronic respiratory diseases are increasing rapidly. Most economies are struggling to deliver modern health care effectively. There is a need to support the transformation of the health care system into integrated care with organizational health literacy. Main body: As an example for chronic disease care, MASK (Mobile Airways Sentinel NetworK), a new project of the ARIA (Allergic Rhinitis and its Impact on Asthma) initiative, and POLLAR (Impact of Air POLLution on Asthma and Rhi-nitis, EIT Health), in collaboration with professional and patient organizations in the field of allergy and airway diseases, are proposing real-life ICPs centred around the patient with rhinitis, and using mHealth to monitor environmental exposure. Three aspects of care pathways are being developed: (i) Patient participation, health literacy and self-care through technology-assisted "patient activation", (ii) Implementation of care pathways by pharmacists and (iii) Next-generation guidelines assessing the recommendations of GRADE guidelines in rhinitis and asthma using real-world evidence (RWE) obtained through mobile technology. The EU and global political agendas are of great importance in supporting the digital transformation of health and care, and MASK has been recognized by DG Santé as a Good Practice in the field of digitally-enabled, integrated, person-centred care. Conclusion: In 20 years, ARIA has considerably evolved from the first multimorbidity guideline in respiratory diseases to the digital transformation of health and care with a strong political involvement.

Abstract

Hydrotalcite like compounds containing Co, Al and Ce were synthesized by co-precipitation. The mixed oxides obtained after calcination were characterized by several techniques: XRD, BET, H2-TPR and XPS. Activities of mixed oxides were evaluated in toluene total oxidation in presence or in absence of carbon monoxide. The benzene, benzyl alcohol and benzaldehyde are principal by-products observed during toluene oxidation in presence of CoAl(Ce) mixed oxides. Moreover, presence of carbon monoxide improves toluene total oxidation over CoAlCe mixed oxides. Stability of the two best catalytic materials has been tested in the two conditions and show no deactivation.

Abstract

We demonstrate that the possibility of monitoring relative photoionization cross sections over a large photon energy range allows us to study and disentangle shake processes and intramolecular inelastic scattering effects. In this gas-phase study, relative intensities of the carbon 1s photoelectron lines from chemically inequivalent carbon atoms in the same molecule have been measured as a function of the incident photon energy in the range of 300–6000 eV. We present relative cross sections for the chemically shifted carbon 1s lines in the photoelectron spectra of ethyl trifluoroacetate (the “ESCA” molecule). The results are compared with those of methyl trifluoroacetate and S-ethyl trifluorothioacetate as well as a series of chloro-substituted ethanes and 2-butyne. In the soft X-ray energy range, the cross sections show an extended X-ray absorption fine structure type of wiggles, as was previously observed for a series of chloroethanes. The oscillations are damped in the hard X-ray energy range, but deviations of cross-section ratios from stoichiometry persist, even at high energies. The current findings are supported by theoretical calculations based on a multiple scattering model. The use of soft and tender X-rays provides a more complete picture of the dominant processes accompanying photoionization. Such processes reduce the main photoelectron line intensities by 20–60%. Using both energy ranges enabled us to discern the process of intramolecular inelastic scattering of the outgoing electron, whose significance is otherwise difficult to assess for isolated molecules. This effect relates to the notion of the inelastic mean free path commonly used in photoemission studies of clusters and condensed matter.

Abstract

In the past decade, the methylotrophic yeast Pichia pastoris has proved to be one of the most efficient systems for mass production of recombinant eukaryotic membrane proteins (MPs), leading to the crystallization and structure determination for a variety of them. The actual overexpression of functional MPs achieved with this system is, however, often accompanied by the formation of a variable but significant proportion of misfolded and/or aggregated proteins that are co-extracted and co-purified during the purification process. In order to minimize this unwanted phenomenon, we devised a novel procedure in which MPs produced in Pichia pastoris are directly solubilized from whole cells instead of crude membrane preparation. This approach aims at favoring the extraction of correctly folded membrane proteins that have been targeted to the plasma membrane, limiting the solubilization of the misfolded proteins and protein aggregates that are stored in internal membrane compartments. The method described herewith is based on the formation of protoplasts through enzymatic treatment prior to protein solubilization. Abstract

A key point when it comes to heterologous expression of eukaryotic membrane proteins (EMPs) is the choice of the best-suited expression platform. The yeast Pichia pastoris has proven to be a very versatile system showing promising results in a growing number of cases. Indeed, its particular methylotrophic characteristics combined to the very simple handling of a eukaryotic microorganism that possesses the majority of mammalian-like machineries make it a very competitive expression system for various complex proteins, in amounts compatible with functional and structural studies. Abstract

Background. The Collaborative Group on Hormonal Factors in Breast Cancer has brought together and reanalysed about 90% of the worldwide epidemiological evidence on the relation between risk of breast cancer and use of hormone replacement therapy (HRT). Methods. Individual data on 52,705 women with breast cancer and 108,411 women without breast cancer from 51 studies in 21 countries were collected, checked, and analysed centrally. The main analyses are based on 53,865 postmenopausal women with a known age at menopause, of whom 17,830 (33%) had used HRT at some time. The median age at first use was 48 years, and 34% of ever-users had used HRT for 5 years or longer. Estimates of the relative risk of breast cancer associated with the use of HRT were obtained after stratification of all analyses by study, age at diagnosis, time since menopause, body-mass index, parity, and the age a woman was when her first child was born. Findings. Among current users of HRT or those who ceased use 1-4 years previously, the relative risk of having breast cancer diagnosed increased by a factor of 1.023 (95% CI 1.011-1.036; 2p = 0.0002) for each year of use; the relative risk was 1.35 (1.21-1.49; 2p = 0.00001) for women who had used HRT for 5 years or longer (average duration of use in this group 11 years). This increase is comparable with the effect on breast cancer of delaying menopause, since among never-users of HRT the relative risk of breast cancer increases by a factor of 1.028 (95% CI 1.021-1.034) for each year older at menopause. 5 or more years after cessation of HRT use, there was no significant excess of breast cancer overall or in relation to duration of use. These main findings did not vary between individual studies. Of the many factors examined that might affect the relation between breast cancer risk and use of HRT, only a woman's weight and body-mass index had a material effect: the increase in the relative risk of breast dancer associated with long durations of use in current and recent users was greater for women of lower than of higher weight or body-mass index. There was no marked variation in the results according to hormonal type or dose but little information was available about long durations of use of any specific preparation. Cancers diagnosed in women who had ever used HRT tended to be less advanced clinically than those diagnosed in never-users. In North America and Europe the cumulative incidence of breast cancer between the ages of 50 and 70 in never-users of HRT is about 45 per 1000 women. The cumulative excess numbers of breast cancers diagnosed between these ages per 1000 women who began use of HRT at age 50 and used it for 5, 10, and 15 years, respectively, are estimated to be 2 (95% CI 1-3), 6 (3-9), and 12 (5-20). Whether HRT affects mortality from breast cancer is not known. Interpretation. The risk of having breast cancer diagnosed is increased in women using HRT and increases with increasing duration of use. This effect is reduced after cessation of use of HRT and has largely, if not wholly, disappeared after about 5 years. These findings should be considered in the context of the benefits and other risks associated with the use of HRT.

Abstract

In this study we evaluated the bit-rate efficiency of current hardware H.264 encoders as compared to that of established MPEG-2 hardware encoders. To estimate bit-rate efficiency, we measured the subjective video quality of MPEG-2 encoded material processed at three bit rates: 8, 12, and 16 Mbps, and determined the bit-rate at which H.264 encoded material produced similar subjective video quality. The MPEG-2 and H.264 bit rates that resulted in the same perceived video quality were used to estimate bit-rate efficiency.

Abstract

Summary Background and objectives: Levobupivacaine in combination with sufentanil may be used for labour or postoperative regional analgesia. The risk of bacterial growth within these contained solutions for several hours at room temperature is unknown. We investigated the in vitro antimicrobial effect of levobupivacaine and sufentanil against common micro-organisms encountered during regional anaesthesia. Methods: Standardized suspensions of Staphylococcus aureus, Staphylococcus epidermidis and Escherichia coli were incubated for 1, 3, 6 and 24h at 258C, with saline (as control), sufentanil 0.5 or 0.75 mgmL 21 , levobupivacaine hydrochloride 5.6mgmL 21 and concentrations of 1.4, 2.8 and 5mgmL 21 of levobupivacaine hydrochloride with sufentanil 0.5 mgmL 21 . Colony counts were compared after 24h incubation at 378C. Results: No bacterial growth was observed on any bacterial strain for any solution tested throughout the experiment. Conclusions: Our results suggest that solutions of levobupivacaine combined with sufentanil may be used for 24h at room temperature during regional anaesthesia with no risk of bacterial growth.

Abstract

The use of Serious Games (SG) in the health domain is expanding. In the field of neurodegenerative disorders (ND) such as Alzheimer's disease, SG are currently employed both to support and improve the assessment of different functional and cognitive abilities, and to provide alternative solutions for patients' treatment, stimulation, and rehabilitation. As the field is quite young, recommendations on the use of SG in people with ND are still rare. In 2014 we proposed some initial recommendations (Robert et al., 2014). The aim of the present work was to update them, thanks to opinions gathered by experts in the field during an expert Delphi panel. Results confirmed that SG are adapted to elderly people with mild cognitive impairment (MCI) and dementia, and can be employed for several purposes, including assessment, stimulation, and improving wellbeing, with some differences depending on the population (e.g., physical stimulation may be better suited for people with MCI). SG are more adapted for use with trained caregivers (both at home and in clinical settings), with a frequency ranging from 2 to 4 times a week. Importantly, the target of SG, their frequency of use and the context in which they are played depend on the SG typology (e.g., Exergame, cognitive game), and should be personalized with the help of a clinician.

Abstract

Testicular and ovarian fragments of the protogynous Pacific wrasse Haliochoeres trimaculatus were incubated in vitro with [ 3 H]pregnenolone ([ 3 H]P 5 ), [ 3 H]17-hydroxyprogesterone ([ 3 H]17OHP 4 ), non-radioactive (nr) 17β-oestradiol (nrE 2 ) or nrP 5 to identify the major gonadal steroidogenic pathways and steroid products in females and in the two male variants of this species, the terminal phase (TP) and initial phase (IP) males. Both testis and ovarian tissues exhibited 7 hydroxylase activity resulting in the formation of 7α-hydroxypregnenolone (7OHP 5 ) from [ 3 H]P 5 , and many HPLC peaks were identified as products of testicular (c. 29) and ovarian (c. 23) steroidogenesis, and only c. 50% of these metabolites co-eluted with authentic reference standards; only very small amounts of conjugated steroid were synthesized from any of the precursors. [ 3 H]P 5 was converted by testis mainly to 7αOHP 5 , and two unknown steroids, whereas [ 3 H]17OHP 4 metabolism gave rise to [ 3 H]17,20β-dihydroxy-4-pregnen-3-one (DHP), 11-ketotestosterone (11KT), and two unknown steroids. For ovarian tissues, [ 3 H]17OHP 4 and [ 3 H]P 5 were metabolized to form E 2 , oestrone (E 1 ), androstenedione (A 4 ), 20α- and 20β-dihydroprogesterone (20αDHP and 20βDHP), 7αOHP 5 (from [ 3 H]P 5 ) and a major unknown. The HPLC steroid profiles for testis incubations for IP and TP males were similar, however, the steroidogenic response of the testis of TP males to human chorionic gonadotrophin, in vitro (determined by hormone assay), was significantly higher than that of IP males.

Abstract

Existe muy poca información sobre la dieta del delfín listado (Stenella coeruleoalba) en aguas del sur de España. En este estudio se analizaron los estómagos de 61 delfines varados en Andalucía en los últimos ocho años (2007-2014). Diez estómagos estaban vacios y de los 51 individuos restantes, 35 vararon en la costa del Mar de Alborán y 11 en el Golfo de Cádiz. Para las 5 muestras restantes no se dispone de localidad concreta. Los restos de las presas en los estómagos consistieron sobre todo en estructuras duras: otolitos, cristalinos y unos pocos huesos de peces, así como mandíbulas y cristalinos de cefalópodos. No aparecieron otro tipo de restos lo que parece indicar que los individuos no se habían alimentado recientemente. Para caracterizar la dieta se calcularon tres índices (calculados para cada categoría de presa): frecuencia de aparición en los estómagos, número de individuos y peso reconstruido y sus porcentajes respectivos. El delfín listado parece ser una especie predominantemente piscívora (se identificaron restos de 5682 peces frente a solamente 210 cefalópodos) que se alimenta de especies mesopelágicas principalmente. La familia más importante tanto en número como en frecuencia de aparición fue Myctophidae, principalmente Ceratoscopelus maderensis y Myctophum punctatum a las que pertenecieron casi la mitad de los peces identificados (36,4%N, 35,3%F y 12,3%N, 25,5%F, respectivamente). También se identificó Maurolicus muelleri (Sternoptychidae 13,4%N y 17,6%F). Otras familias identificadas presentan una distribución más costera: Sparidae (Boops boops) y Carangidae (Trachurus sp.) (4,5%N, 9,8%F y 1,2%N, 3,9%F, respectivamente). Los cefalópodos identificados pertenecieron a 11 familias siendo la más abundante Ommastrephidae (26,7%N y 39,2%F). Otras familias incluyen representantes oceánicos, Brachioteuthidae (16,7%N y 21,5%F); Chiroteuthidae (6,7%N y 9,8%F); Enoploteuthidae (4,8%N y 3,9%F), Ancistrocheridae (0.5%N y 2%F) y otros de distribución más costera, Sepiolidae (19%N y 19,6%F), Sepiidae (5,7%N y 3,9%F) y Loliginidae (3,8%N y 5,9%F).

Abstract

The human melatonin MT1 receptor—belonging to the large family of G protein-coupled receptors (GPCRs)—plays a key role in circadian rhythm regulation and is notably involved in sleep disorders and depression. Structural and functional information at the molecular level are highly desired for fine characterization of this receptor; however, adequate techniques for isolating soluble MT1 material suitable for biochemical and biophysical studies remain lacking. Here we describe the evaluation of a panel of constructs and host systems for the production of recombinant human MT1 receptors, and the screening of different conditions for their solubilization and purification. Our findings resulted in the establishment of an original strategy using a mixture of Fos14 and CHAPS detergents to extract and purify a recombinant human MT1 from Pichia pastoris membranes. This procedure enabled the recovery of relatively pure, monomeric and ligand-binding active MT1 receptor in the near-milligram range. A comparative study based on extensive ligand-binding characterization highlighted a very close correlation between the pharmacological profiles of MT1 purified from yeast and the same receptor present in mammalian cell membranes. The high quality of the purified MT1 was further confirmed by its ability to activate its cognate Gαi protein partner when reconstituted in lipid discs, thus opening novel paths to investigate this receptor by biochemical and biophysical approaches.

Abstract

A platelet test that is predictive of myocardial infarction (MI) and/or stroke would enable the targeting of anti-platelet drugs towards high-risk patients. The predictive power of several platelet tests for MI and for stroke was examined in 2000 older men in the Caerphilly Cohort Study of Heart Disease, Stroke and Cognitive Decline. The tests were: aggregation to adenosine diphosphate (ADP) in platelet-rich plasma (PRP); aggregation to ADP in whole blood measured using an impedance method and a test of platelet aggregation induced in whole blood by high-shear flow. Around 200 MIs and 100 ischaemic strokes occurred during a 10-year follow-up. Neither primary nor secondary aggregation in PRP was predictive of MI. However, the fifth of men in whom the primary response to ADP was least, showed the highest risk of a subsequent stroke [relative odds (RO) 1·64; 95% confidence interval (CI) 1·12–2·43]. Aggregation in whole blood was not predictive of MI but, again, the fifth of men with the least platelet response showed the highest stroke incidence (RO 1·79; 95% CI 1·06–3·00). Retention of platelets in the high-shear test was not predictive of either event.

Abstract

Plum pox virus (PPV) resistant rootstocks would be useful to control the spread of PPV in nurseries and orchards. The PPV-resistant transgenic Prunus domestica plum clone C-5 was used as a parent with Prunus spinosa to develop PPV resistant hybrid progeny. This work showed that C-5 plum is sexually compatible with the Prunus rootstock species P. spinosa. Resistance assays of chip-bud inoculated plants following two cycles of growth and cold-induced dormancy demonstrated that the transgenic hybrid clones were symptomless and DAS-ELISA negative. Further, immuno-blotting failed to detect PPV CP in inoculated transgenic hybrids. These results demonstrated the usefulness of C-5 for developing PPV resistant rootstocks with P. spinosa.

Abstract

Background Studies have found that reproductive factors might have a variable effect on the occurrence of breast cancer (BC) according to the existence or not of a family history of BC. The effect of a family history of BC on the risk of BC may also vary according to the age at diagnosis and the degree of kinship. This may confound the relation between familial risk and reproductive factors. A combined analysis was performed to study the interaction between familial risk and reproductive factors according to degree of familiality, age at interview and menopausal status. Methods The present analysis included 2948 cases and 4170 controls in seven case-control studies from four countries. The combined relative risks were estimated using a Bayesian random-effects logistic regression model. Results The main effects of reproductive life factors on the risk of BC are in agreement with previous studies. Two-way interactions between subject's age or menopausal status and a family history of BC were not significant. Although the three-way interaction between age, familial risk and parity was not significant, familial risk seemed to be increased slightly for women with high parity compared with women with low parity in the older age group, and seemed to be slightly decreased for women with high parity compared with women with low parity in younger women. The subject's age also appeared to have an effect on the interaction between familial risk and the age at first childbirth (P = 0.1). Conclusions A possible influence of reproductive and menstrual factors on familial risk of BC has been suggested previously and was also evident in the present study. Three-way interactions between age, family history and parity or age at first childbirth might exist and they merit further investigation.

Abstract

Tests were conducted to determine whether an analog ENG system using a 12 MHz channel bandwidth can provide a quality of service equivalent to that of existing analog ENG systems using 17 and 18 MHz channel bandwidths. The performance of both systems was compared under controlled conditions similar to those found in real-life transmission channels. Conclusions about the replication of the service obtained are based on differences in transmission robustness, video and audio signal quality, and adjacent channel degradation.

Abstract

The pharmacokinetics of ketorolac (Toradol), a human non-narcotic, nonsteroidal anti-inflammatory drug (NSAID) of the pyrrolo-pyrrole group, was studied in six mixed breed dogs of varying ages (1-5 years). The study was performed using a randomized crossover design, with each dog initially assigned to one of two groups (intravenous (i.v.) or oral (p.o.)). Each group of three dogs received either the injectable or oral formulation of ketorolac tromethamine at 0.5 mg/kg. Serial blood samples were collected before and over 96 h following treatment. Samples were analysed by reverse phase HPLC. Individual ketorolac plasma concentration-time curves were initially evaluated by computerized curve stripping techniques followed by nonlinear least squares regression. Following i.v. administration mean (+/- SD) pharmacokinetic parameters were: elimination half-life (t1/2 beta) = 4.55 h, plasma clearance (Clp) = 1.25 (1.13) mL/kg/min, and volume of distribution at steady state (Vss) = 0.33 (0.10) L/kg. Mean (+/- SD) p.o. pharmacokinetic values were: t1/2 beta = 4.07 h, time to reach maximum concentration (tmax) = 51.2 (40.6) min, and p.o. bioavailability (F) = 100.9 (46.7)%. These results suggest that the pharmacodisposition characteristics of a clinically effective 0.5 mg/kg i.v. or p.o. single dose of ketorolac tromethamine administered to dogs is fairly similar to that observed in humans.

Abstract

The Allergic Rhinitis and its Impact on Asthma (ARIA) initiative commenced during a World Health Organization workshop in 1999. The initial goals were (1) to propose a new allergic rhinitis classification, (2) to promote the concept of multi-morbidity in asthma and rhinitis and (3) to develop guidelines with all stakeholders that could be used globally for all countries and populations. ARIA - disseminated and implemented in over 70 countries globally - is now focusing on the implementation of emerging technologies for individualized and predictive medicine. MASK [MACVIA (Contre les Maladies Chroniques pour un Vieillissement Actif)-ARIA Sentinel NetworK] uses mobile technology to develop care pathways for the management of rhinitis and asthma by a multi-disciplinary group and by patients themselves. An app (Android and iOS) is available in 20 countries and 15 languages. It uses a visual analogue scale to assess symptom control and work productivity as well as a clinical decision support system. It is associated with an inter-operable tablet for physicians and other health care professionals. The scaling up strategy uses the recommendations of the European Innovation Partnership on Active and Healthy Ageing. Abstract

An investigation of the multifilament current method's (MFCM) ability to solve electromagnetic scattering and/or absorption problems involving inhomogeneous cylindrical structures is presented. Dielectric cylinders of arbitrary cross section covered by multiple layers of lossy dielectrics are considered. Both cases of wave incidence, TM and TE, are treated. Like some other moment-method solutions, the MFCM experiences numerical difficulties when dealing with a medium characterized by a high loss tangent or large electrical conductivity. To overcome this problem, a new boundary condition based on an impedance matrix equivalent circuit approach that accounts for the curvature of the surfaces has been developed. This new impedance matrix boundary condition (IMBC) extends the MFCM capability to the analysis of two-dimensional (2-D) structures involving conducting layers with ohmic losses. The usefulness of this extended method is confirmed by the study of metallic shells for which a strong energy coupling with the incident electromagnetic (EM) field is demonstrated at their structural resonance frequencies.

Abstract

Ovarian follicles taken from sexually maturing rainbow trout at the mid-vitellogenic stage of ovarian development were incubated in vitro in the presence or absence of melatonin or somatostatin-14 (SRIF-14) to determine whether there is evidence of a direct action of these factors on gonadal steroidogenesis in fishes. The steroidogenic capacity of the ovarian follicles was assessed by measuring testosterone (T) and 17β-estradiol (E2) release into the incubation medium, and by examining the steroid metabolites produced following incubation of follicles with radiolabelled steroid precursors.

Abstract

The in vitro basal and salmon gonadotropin (sGTH)-stimulated steroidogenic capacity of rainbow trout follicles was examined at four stages [early (EV)-, mid (MV)- and peak-vitellogenic (PV), and pre-ovulatory, post-vitellogenic (PO)] of gonadal recrudescence using radioimmunoassays (RIAs) to measure 17β-estradiol (E2) and testosterone (T) production. In addition, follicles were incubated in the presence of [3H]pregnenolone ([3H]P5) and the radiolabelled steroid metabolites produced were separated using high performance liquid chromatography (HPLC). Peak basal and sGTH-stimulated E2 and T production was found in PV stage follicles and lowest in PO stage follicles, and there were marked differences in the HPLC profiles of steroid metabolites. For EV stage follicles the major metabolite eluted as a peak that co-eluted with the androstenedione (A4) and 17α-hydroxyprogesterone (17α-OHP) standards. A smaller peak that co-eluted with 11β-hydroxyandrostenedione (11β-OHA4) and very small peaks co-eluting with 20α-dihydroprogesterone (20α-DHP) and E2 were also seen. MV and PV stage follicles produced predominantly E2, together with a small combined A4 + 17α-DHP peak, traces of 11β-OHA4 and two peaks that did not co-elute with any of the reference standards. The PO stage follicles produced only 17, 20β-dihydroxy-4-pregnene-3-one (17,20β-P).

Abstract

Six high-definition television (HDTV) systems were tested to establish a proposed HDTV terrestrial transmission standard. The tests occurred in two rounds and were conducted at the Advanced Television Test Center (ATTC); and at Cable Labs in Alexandria, VA; and at the Advanced Television Evaluation Laboratory (ATEL) in Ottawa, Ont., Canada. The testing process played an important role in accelerating the development of HDTV technology in North America, culminating in the development of the digital Grand Alliance HDTV System, the adopted standard. This paper summarizes the results of subjective image quality tests (Basic Received Quality, Impairment/Interference, and Receiver Scan Conversion) of the proponent HDTV systems from both rounds of testing, thereby permitting the reader to compare the relative performance of competing systems and gain some insight into the testing process conducted at ATEL.

Abstract

In this paper, a combined analysis was performed to study the interaction between familial risk and reproductive life factors. In particular, the interaction between familial risk and breast cell mitotic activity (BCMA), as assessed by duration of ovarian activity, was investigated because of the potential importance of mitotic activity on genetically susceptible cells. The present analysis included 3152 cases and 4404 controls in seven case-control studies from four countries. The interaction effect was estimated in each study separately, then combined using two different methods: a multivariate weighted average and a Bayesian random-effects model. The main effects of reproductive life factors on the risk of breast cancer were in agreement with the previous findings. In particular, an increased duration of BCMA before the first childbirth and over life was found to increase the risk of breast cancer (P < 0.001). Slightly increasing but non-significant, familial risks were observed with increasing number of children (P = 0.17), increasing age at first childbirth (P > 0.2) and increasing duration of BCMA (P > 0.2). There was no modification in familial risk with age at menarche and no clear pattern with menopause characteristics. A weak influence of reproductive and menstrual factors on the familial risk emerged from the present study.

Resume But de l'etude II s'agit d'une analyse combinee de facteurs cliniques et histologiques (determines a partir d'une microbiopsie prealable) ayant pour objet d'evaluer le risque d'envahissement ganglionnaire axillaire (pN + ) dans les cancers du sein inferieurs a 3 cm. Materiel et methodes Chez 893 patientes atteintes d'un cancer invasif du sein ToT 1 T 2 ≤ 3 cm cinq parametres ont ete analyses: la taille clinique ( T ), la taille histologique (pT), le sous-type histologique (carcinome canalaire infiltrant, divise selon les trois grades histopronostiques SBR, carcinome lobulaire infiltrant et autres), l'âge ( 60 ans) et la taille du sein, determinee objectivement sur un critere dosimetrique recemment publie ( Eur J Cancer 1997; 33: 2432–4). Resultats Le taux global de pN + etait de 25,3 %, avec respectivement: pN1: 10 %, pN2-3: 8,4 % et pN > 3: 6,9 %. Pour les t 0 , T 1 et T 2 ≤ 3 les taux de pN + etaient respectivement de 13,8 %, 19,8 % et 36,2 %. Selon le pT on notait respectivement 11,1 %, 17,7 %, 23,5 %, 30,1 % et 36 % de pN + pour les lesions correspondantes: 09,9 mm, 10–14,9 mm, 15–19,9 mm, 20–24,9 mm et 259,9 mm. Pour les carcinomes canalaires infiltrants SBR 1, 2 et 3 les taux de pN + etaient de 18,3 %, 27,2 % et 37,8 %; pour les carcinomes lobulaires infiltrants et les autres soustypes, de 22,7 % et 10 %. Pour les trois categories d'âge sus-citees, les taux de pN + etaient respectivement 30,3 %, 25,8 % et 22,4 %. Selon la taille du sein, il y avait 30,1 % et 24,4 % de pN + pour les seins de petit volume par rapport a ceux de moyen ou gros volume. Apres analyse multivariee, trois facteurs restaient significativement predictifs pour le risque de pN + : la taille clinique de la tumeur ( p p = 0,0005), la taille du sein ( p = 0,004). En combinant simultanement ces trois facteurs, le risque de pN + variait de 5 a 50 %. De plus, il etait egalement possible de predire le nombre de ganglions envahis. Conclusions Ce modele simple peut permettre au clinicien d'identifier un groupe de patientes dont le risque d'envahissement ganglionnaire axillaire est ≤ 30 % et qui pourrait donc beneficier d'une chirurgie axillaire limitee avec analyse du ganglion sentinelle.
