Abstract

Tuberculosis (TB) is still a health problem in Indonesia because its prevalence ranks second in the world after India in 2023. Regional factors, inadequate health facilities, and limited resources (financial, human, and infrastructure) are challenges requiring innovation to help the government control TB. TB eradication efforts need to be made with a comprehensive and effective approach. One method used is to look at the vast territory of Indonesia; therefore, mapping the vulnerability of TB is highly recommended. Using a Geographic Information System is expected to help map the TB vulnerability areas in Indonesia. Given several epidemiological, socio-geographic, and environmental factors influencing TB, the question arises of how to map TB vulnerability areas in Indonesia. This study used a cross-sectional design, secondary data was collected from several sources, and a vulnerability analysis was performed by considering several socio-environmental epidemiological variables. Furthermore, after the analysis, the TB area vulnerability category would be obtained along with a map of TB vulnerability areas in Indonesia according to regional and district analysis units. This study produces a TB susceptibility index and map for Sumatra, Java-Bali, and other regions in Indonesia. [ABSTRACT FROM AUTHOR]

Abstract

The standard inpatient class policy in Indonesia has been stated in legislation since 2021 with twelve criteria and must be fully met by December 2024. This study aims to get an overview of the readiness of hospitals in Indonesia to meet the policy and an overview of the community's perspective in responding to standard inpatient classes. Scoping Review with Arksey and O'Malley and Beyond's approach. Online article search on Scopus, Science Direct, SpringerLink, garuda, and Google Scholar, published in 2020-2023, articles published in English and Indonesian, with search keywords: 'kris' or 'jk' or 'bpjs and kesehatan' or 'bpjs and health' or 'inpatient and room' or 'health and policy and jkn', in October 2023 and carried out Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) flow diagram. Hospitals in Indonesia studied in 2022 are not ready, but hospital research in 2023 states that they are ready to carry out Standard Inpatient Classes (KRIS). The community as patients and consumers in the use of health services from the Social Security Organizing Agency (BPJS) Health provides support in the implementation of Standard Inpatient Classes. Meanwhile, from the legality aspect, there is a discrepancy in regulations from the highest to its derivatives, there is even no clarity on legal sanctions if the Hospital does not meet the criteria for the Standard Inpatient Class according to the specified time limit. Abstract

Background: Pre-marital period is the best time to prepare for the quality of family health; however, In Indonesia, examinations and promotion of reproductive health and pre-marital counseling have yet to be carried out in all districts. Objectives: This study aimed to identify the description of reproductive health services and marriage counseling in Grobogan Regency, Central Java. Methods: This study used an assessment method of the implementation of the Reproductive Health Service Program for the bride, which includes IEC (Communication, Information, and Education) and medical examination of the bride and groom by health workers conducted in community health centers (Puskesmas) and other health care facilities, and office of religious affair (KUA), with the In-depth Interview Method. The subject was the KUA Officer under the Department of Religious Islamic Education in the Marriage Guidance program. Results: The interviews with KUA officers obtained results in 2017. There were still underage marriages, and the number increased in 2018. Prospective brides have submitted forms (N1, N2, N3, and N4) to KUA. Most of the brides were not physically examined (pulse, breathing frequency, blood pressure, body temperature) by health workers (63.7%), and blood groups were not examined (91.4%). Half of the respondents examined blood sugar, HIV, IMS, Hepatitis, and TORCH (56.8%). The bridegrooms were not examined for mid-upper arm circumferences (MUAC) (17.2%) and for signs of anemia (8.6%). The brides were not immunized against Hepatitis B (72.4%). Almost all brides get TT immunization at the Puskesmas (94.8%). Health workers stated that only some brides had been getting counseling about the bride's mental health (29,3%). Only 17.2% of the officers stated that the bride still needs to get a certificate of following the reproductive health IEC. Conclusions: KUA has conducted pre-marriage courses for couples according to the rules of the Ministry of Religion. Some PUSKESMAS had, Latar Belakang: Periode pra nikah adalah waktu terbaik untuk mempersiapkan kondisi fisik, psikologis, dan social melalui pemeriksaan kesehatan, konseling pranikah, dan promosi kesehatan reproduksi untuk membentuk kualitas kesehatan keluarga. Di Indonesia, pemeriksaan dan promosi kesehatan reproduksi serta konseling pranikah belum terlaksana di semua Kabupaten. Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui gambaran proses pelayanan kesehatan reproduksi dan kursus pranikah di Kabupaten Grobogan, Jawa Tengah. Metode: Penelitian ini menggunakan metode penilaian pelaksanaan Program Layanan Kesehatan Reproduksi untuk pengantin wanita yang mencakup KIE (Komunikasi, Informasi dan Pendidikan) dan pemeriksaan medis calon pengantin oleh petugas kesehatan, baik di pusat kesehatan dan fasilitas perawatan kesehatan lainnya. dan KUA; dengan Metode Wawancara Indeepth. Subjek penelitian ini adalah Petugas KUA di bawah Departemen Agama Pendidikan Islam dalam program Bimbingan Perkawinan (Bimwin). Hasil: Wawancara dengan petugas KUA mendapatkan hasil pada tahun 2017 masih ada pernikahan di bawah umur dan jumlahnya meningkat pada tahun 2018. Calon pengantin wanita telah menyerahkan formulir (N1, N2, N3, dan N4) kepada KUA. Sebagian besarcalon pengantin wanita tidak diperiksa secara fisik (denyut nadi, frekuensi pernapasan, tekanan darah, suhu tubuh) oleh petugas kesehatan (63,7%), golongan darah tidak diperiksa (91,4%). Setengah dari responden diperiksa gula darah, HIV, IMS, Hepatitis, TORCH (56,8%). Calon pengantin yang tidak diperiksa LILA dan tidak diperiksa tanda-tanda anemia (17,2%) dan (8,6%). Pengantin wanita tidak diimunisasi Hepatitis B sebanyak 72,4%. Hampir semua pengantin mendapatkan imunisasi TT di Puskesmas (94,8%). Abstract

Indonesia is an agricultural country that is very developed in its agricultural sector. Many types of flora are planted in Indonesia, but the planting process cannot be separated from plant pests. Farmers use pesticides a lot to reduce crop damage caused by pests. This study aims to see the effect of using onion peel extract as a natural pesticide for armyworms. The study method used is a quantitative study. This study uses a pre-experimental research method with a one-shot case study design. The study used a completely randomized design (CRD) and consisted of three treatments. Each treatment was observed for its toxic effect until the caterpillar died. The variable studied was the length of time the armyworm died after spraying natural pesticides with onion skin extract content of 50%, 75%, and 100%. The data that has been obtained were analyzed by Anova test to see the differences and the influence of each group. The results showed that there was an effect of spraying pesticides from onion peel extract with armyworm mortality rates at concentrations of 50% and 100%. The use of red onion peel extract has the highest incidence of armyworm mortality at a concentration of 100%. [ABSTRACT FROM AUTHOR]

Abstract

Background: Semarang is one of the cities in Central Java with the highest positive number and mortality rate of Covid-19. It became the largest contributor to daily cases in Central Java. In general, the severity of Covid-19 in Semarang is 5.3% and increased to 24% in the elderly. Mortality Covid-19 was caused by various factors such as demographic and non-demographic factors.Objective: This study aims to determine the impact of several risk factors of COVID-19 mortality in Semarang.Methods: This is an analytical cross-sectional study. Data analysis was carried out analytically with a binary logistic regression test. The sample used was the total population of 32.555 confirmed positive from March 2020 to March 2021 registered in Semarang Health Office.Results: The results demonstrated that mortality due to Covid-19 in Semarang was frequent in the elderly (27.3%), males (9.4%), had comorbidities such as diabetes mellitus (28.6%), kidney disease (43.5%), neuromuscular (36.4%), and cancer (29.3%), and accompanied by symptoms such as Dyspnea (18.1%), weakness/lethargy (19.9%), and fever (14.6%).Conclusion: Age, gender, travel history, comorbidities (kidney, neuromuscular, cancer, and diabetes mellitus), and symptoms (fever, shortness of breath, and weakness/lethargy) are risk factors for mortality Covid-19 in Semarang.

22. The Determinant Factors of Labor. Data Analysis of “Maternal Card Cohort” in Tawangharjo Community Health Center, Grobogan

Abstract

ANC data in the maternal card cohort register at the Puskesmas can be used to detect risk factors for pregnancy and labor that can prevent maternal death, but this data has never been analyzed. This study aims to analyze the determinants of labor in mothers who received ANC by a midwife according to the data in the maternal card cohort. This research is a descriptive-analytic study design using the observational method. Data sourced from the maternal card cohort register that meets the standards of accuracy and completeness of ANC results in records until labor in the 2019 time period. A total of 172 data were used as samples. Maternal characteristics, maternal mortality risk factors, levels of maternal risk factors based on the Poedji Rochjati score as independent variables, and the act of labor is the dependent variable. Univariate and bivariate analysis was carried out descriptively and analytically using the chi-square test. The majority of mothers aged 20-35 years (81.4%), had basic education (94.2%), normal nutritional status (84.9%), hadn’t anemia (80.2%), had normal blood pressure (70, 9%), hadANC visits 5-10 times (65.7%), categorized as low-risk pregnancy (86.6%), and almost all received normal labor in the previous (96.0%). Previous labor and maternal risk factors were associated with the last labor act received by the mother (p-value=0.040; p-value=0.043).Abstrak: Data pelayanan kehamilan (antenatal care) pada register kohort kartu ibu di Puskesmas dapat digunakan untuk mendeteksi faktor risiko kehamilan dan tindakan persalinan yang dapat mencegah kematian ibu, namun belum pernah dilakukan analisis pada data tersebut. Abstract

Background: Maternal mortality, children mortality, stunting, and other health problems are still high in the world. One strategic to prevent this problem is to increase health knowledge in preparing for pregnancy to brides and grooms candidate. The aim of the study was to analyze differences in knowledge and attitudes of the brides and grooms candidate related to reproductive health before and after being given an intervention in Grobogan Regency. Method: The quasi-experimental research with one group pre and posttest design was used to analized 31 bridegrooms as subject in the area of Grobogan Regency. The bridegrooms were given an intervention in the form of one-day pre-marriage course for 6-7 hours, wich is given by KUA officers (Office of Religious Affairs) and Health Service officers who have been trained. Knowledge and attitude were measured before and after a given intervention using a structured questionnaire and data gathered were analyzed using Wilcoxon Signed Rank Test. Result: The results showed there was an effect of pre-marriage course intervention on knowledge (p=0.001) but does not have a significant effect on the attitude of the bride and groom (p=0.209). Conclusion: One-day pre-marriage course can increase knowledge the brides and grooms candidate while the attitude doesn’t. [ABSTRACT FROM AUTHOR]

45. Evaluasi Pelayanan Persalinan oleh Bidan Desa Selama Pelaksanaan Program Jaminan Persalinan di Puskesmas Salomekko Kabupaten Bone Sulawesi Selatan Tahun 2012

Abstract

Background: The delivery assurance (Jampersal) issued from April 2011 aims to increase access to maternal delivery assistance by health workers in health facilities by providing financing guarantees. Delivery coverage by health workers in Bone District in 2011 was 76.67%. Delivery by health workers was only 60% done in health facilities and Salomekko Health Center had the lowest figure which was 25%. Objective: To evaluate service delivery by village midwives during the implementation of Jampersal in Salomekko Health Center of Bone District of South Sulawesi Province in 2012. Methods: This study was observational using a qualitative descriptive design that was evaluative in nature. The study subjects were 8 village midwives in the work area of Salomekko Health Center. The triangulation informants were the head and midwife coordinator of Salomekko Health Center and the head of MCH section of Bone Health office. Data was collected by in-depth interviews and analyzed by content analysis. Results: All midwives in the village did not perform service delivery well because most of them had not joined APN training. There was no Standard Operational Procedures (SOP), four village health post buildings in the four villages were not adequate (their width, no water or electricity and away from settlements), four other villages had not yet Poskesdes, and delivery equipment was also incomplete. The process of planning, implementation, monitoring and supervision of Jampersal was also not performed well. In 2011 and 2012 (January to April), all deliveries by health workers in the Salomekko Health Center were funded by Jampersal although deliveries took place in non-medical facilities. The culture of siri was expected to encourage mothers to give birth at home. The delivery rate by traditional birth attendants was still high. Although there was no necessity, the mothers should pay around 100,000 00 to midwives for consumables without the knowledge of the health office. The application procedure for Jampersal funds with certain administrative requirements was perceived to be quite complicated. Conclusion: Poskesdes needs to be built immediately in four villages and, for the other four villages that already have Poskesdes, the facilities need improving following the completeness of equipment. APN training and Jampersal SOP socialization should be improved and monitoring and supervision by the health center in the village should be increased. Levies should be removed and delivery by health workers not at the health facilities cannot claim Jampersal. Lastly, the claims of Jampersal process should be simplified. Latar belakang: Jaminan persalinan diberlakukan mulai April tahun 2011 dan bertujuan meningkatkan akses ibu bersalin terhadap pertolongan persalinan oleh tenaga kesehatan di fasilitas kesehatan dengan memberikan jaminan pembiaya- annya. Cakupan persalinan oleh tenaga kesehatan di Kabupaten Bone tahun 2011 yaitu 76,67%. Persalinan oleh tenaga kese- hatan hanya 60% dilakukan oleh fasilitas kesehatan dan Puskesmas Salomekko mempunyai angka terendah yaitu 25%. Tujuan penelitian adalah melakukan evaluasi pelayanan persalinan oleh bidan di desa selama pelaksanaan Jampersal di Puskesmas Salomekko Kabupaten Bone Sulawesi Selatan tahun 2012. Metode: penelitian observasional menggunakan rancangan deskriptif kualitatif yang bersifat evaluatif, dengan subyek penelitian adalah 8 orang bidan di desa di wilayah Puskesmas Salomekko. Informan triangulasi adalah Kepala Puskesmas Salomekko dan Bidan koordinator Puskesmas Salomekko, serta Kepala seksi KIA Dinas Kesehatan Kabupaten Bone. Data dikumpulkan dengan wawancara mendalam dan dianalisis dengan content analysis. Hasil: Semua informan bidan di desa belum melaksanakan pelayanan persalinan dengan baik karena sebagian besar petugas belum mengikuti pelatihan APN, belum adanya Standar Operational Prosedur (SOP), empat gedung Pos Kesehatan Desa di empat desa tidak memadai (luasnya, tidak ada air mau- pun listrik dan jauh dari pemukiman), empat desa lainnya belum memiliki Poskesdes, peralatan persalinan juga belum lengkap. Proses perencanaan, pelaksanaan, monitoring dan pengawas- an Jampersal juga belum terlaksana dengan baik. Tahun 2011 dan 2012 (Januari sampai April) semua persalinan oleh tenaga kesehatan di wilayah Puskesmas Salomekko dibiayai dana Jam- persal walaupun persalinan dilakukan di non fasilitas kese- hatan. Budaya siri diperkirakan mendorong ibu bersalin di rumah. Angka persalinan oleh dukun bayi masih tinggi. Walaupun tidak ada keharusan tetapi cukup memberatkan ternyata sebagian ibu bersalin membayar sekitar Rp100.000,00 ke bidan di desa untuk bahan habis pakai, dan hal ini tidak sepengetahuan dinas kesehatan. Prosedur pengajuan dana Jampersal dengan syarat administrasi tertentu dirasakan cukup rumit. Kesimpulan: Diperlukan segera pembangunan Poskesdes di empat desa dan perbaikan fasilitas di empat desa yang telah mempunyai Poskesdes berikut kelengkapan peralatannya, pelatihan APN dan sosialisasi SOP Jampersal begitu juga peningkatan upaya monitoring dan supervisi oleh Puskesmas berdasarkan rayon wilayah desa, menghilangkan pungutan, persalinan Nakes tidak di Faskes tidak diklaimkan jampersal serta penyederhanaan proses klaim Jampersal.
