Thrombosis of the great arteries rarely occurs in the neonate. We report a case of thrombosis of the aortic arch, the brachiocephalic truncus, the left carotid and subclavian arteries, seen in a one-day-old neonate. The clinical findings were at first misdiagnosed as interrupted aortic arch syndrome, though, in retrospect echocardiography was very suggestive of the correct diagnosis. On the basis of the histological examination we assume, as the most likely cause for thrombosis, an aortitis in the prenatal period. Following thrombectomy and Gore-Tex bypass from the ascending to the descending aorta, combined with ligature of a PDA, the child developed a low output state and died on the table. The postmortem examination revealed that the latter was due to multiple infarcts of the left ventricular myocardium.

Respiratory insufficiency is the clinical principal sign of cystic pulmonary lymphangiectasis. Clarification of cardiovascular morphology and haemodynamics is the basic prerequisite for the classification and therapy of the disease pattern. The prognosis is unfavourable if it is found that the pulmonary circulation does not flow through the lungs, reaching instead the systemic circulation via the persisting ductus arteriosus. The histopathological findings as well as the results of dissection of the cardiovascular system clarify the question as to the pathogenesis of this disease and allow subdivision of the disease into a primary and a secondary manifestation of cystic pulmonary lymphangiectasis. Die respiratorische Insuffizienz ist das klinische Leitsymptom der zystischen pulmonalen Lymphangiektasie. Die Klarung der kardio-vaskularen Morphologie und Hamodynamik ist die Voraussetzung zur Klassifizierung und Therapie des Krankheitsbildes. Die Prognose ist infaust, wenn nachgewiesen werden kann, das das Kleinkreislaufvolumen die Lunge nicht durchstromt, sondern im Kurzschlus uber den persistierenden Ductus arteriosus den grosen Kreislauf erreicht. Der histo-pathologische Befund sowie das Sektionsergebnis des kardio-vaskularen Systems klart die Frage nach der Pathogenese dieser Erkrankung und last die Unterteilung in eine primare und sekundare Erscheinungsform der zystischen pulmonalen Lymphangiektasie zu.

The general management and steps of cardiopulmonary cerebral resuscitation are the same for infants, children and adults: Airway management; Breathing, Cardiac compressions and Drugs to restart circulation and maintain cerebral and myocardial oxygen supply. However, priorities and techniques differ somewhat because of variations in size, physiology and cause of circulatory arrest. In pharmacological support there are several new aspects: The immediate correction of acidosis is not necessary in CPR and iatrogenic alkalosis has deleterious effects. Beta-receptor stimulators and calcium should not be used furthermore in CPR after cardiac arrest. Epinephrine was and is still the drug of choice in resuscitation. Drugs for cerebral resuscitation are still in a state of development.

ZusammenfassungEs wird über Untersuchungen der Plasminogenspiegel in Plasmen gesunder Frühgeborener und solcher mit Atemnotsyndrom berichtet. Die Bestimmungen erfolgten mit der radialen Immundiffusion unter Verwendung eines Anti-Plasminogenserums. Vergleichende Untersuchungen zeigten eine annähernde Übereinstimmung der im immunologischen Test gefundenen Plasminogenwerte mit dem durch Streptokinase aktivierten Plasminogenproaktivator : Im Falle der unauffälligen Frühgeborenengruppe fanden sich im funktionellen Test jedoch jeweils höhere Werte als bei der immunologischen Bestimmung. Dies könnte als Ausdruck einer besonderen Reaktionsweise des Plasminogens im Neugeborenenplasma gedeutet werden.Die bei unauffälligen Frühgeborenen im Alter von 6 Stunden immunologisch bestimmten Plasminogen werte zeigten keine Korrelation mit dem Geburtsgewicht. Bis zur 48. Lebensstunde stiegen in dieser Gruppe die Plasminogen werte signifikant an. Auch in der Gruppe der an RDS erkrankten, überlebenden Frühgeborenen war ein allerdings nicht signifikanter Anstieg bis zum 2. Lebenstag nachweisbar. Bei den beiden Gruppen von an RDS erkrankten Kindern, die verstarben, lagen die Plasminogenwerte bereits im Alter von 2 Stunden erheblich unter den bei der unauffälligen Vergleichsgruppe gefundenen Werten: Im Falle der ohne Hirnblutung verstorbenen Kinder sank der Wert von 22,9% im Alter von 2 Stunden bis zur 24. Lebensstunde weiter auf 17,8% ab; bei denjenigen Frühgeborenen, die außer dem Atemnotsyndrom noch zusätzlich eine Hirnblutung auf wiesen, schwankten die Werte innerhalb der ersten 24 Lebensstunden nur unwesentlich um niedrigste Werte (17,5% im Alter von 2 Stunden, 17,2% im Alter von 24 Stunden).

Summary Measurement of the elimination of small quantities of 125I-fibrinogen (5–15 μCi) demonstrates shortened half-life and higher catabolic rates in premature infants, and particularly in those infants suffering from IRDS compared with adults. Turnover in the adult is 15%, in the premature 25 % and in the premature with IRDS 33 % per day. Equilibrium between intravascular and extravascular fibrinogen is not achieved before the third day of life. Later, elimination is linear. There is a highly significant difference between 32rT-fibrinogen catabolism in the adult and the premature infant with and without the IRDS between the third and twelfth day. Measurement of activity over various organs demonstrates that slow efiux of 125I-fibrinogen into the extravascular compartment causes a continuous fall in plasma activity during the first 2–3 days. In premature infants with IRDS the activity over the lungs rises during the first day of life. These results allow the conclusion that accelerated fibrinogen synthesis in infants with IRDS compensates for losses into the hyaline membranes and intravascular clots and even allows the physiological rise in plasma fibrinogen level during the first three days to take place.

In patients with respiratory failure and shock serial arterial blood analyses are important to calculate adequate respiratory therapy. During circulatory insufficiency punction of a radial artery can be difficult or impossible, especially in very young patients. The case report of a 9-month old female infant with septic shock and Waterhouse-Friderichsen's syndrome illustrates these problems and is helpful to describe an alternative technique. Percutaneous axillary arterial catheterization with a disposable venous cannula proved to be a simple and quick method to obtain the possibility of arterial monitoring in very young patients with severe hypotension.

Es werden Ergebnisse von Plasmafibrinogenbestimmungen bei gesunden Fruhgeborenen mitgeteilt. Die Untersuchungen erfolgten mittels der radialen Immundiffusion. Es wird gezeigt, das die zunachst niedrigen Fibrinogenwerte rasch ansteigen und bereits im Alter von 12 Std Erwachsenenwerte erreicht werden.
