Hypothesis/aims of study. The aim of this study was to evaluate the possibilities of geometric modeling in reducing the error of estimated fetal weight calculation. Study design, materials and methods. During the first stage of the study, a mathematical dependency was determined by geometric modeling of the results of an objective examination of the pregnant woman, which hypothetically will reduce the error of estimated fetal weight calculation. The second stage of the study included a prospective analysis of the error of the developed method and standard formulas for estimated fetal weight calculation at full-term pregnancy, and the third stage at preterm pregnancy. Weighing the newborn is the gold standard, which allows to objectively assess the error of the studied formulas. Results. The average value of the relative error of fetal weight calculation using the Jordanias formula was 14,2%, the Johnsons 18,6%, the Lankowitzs 16,2%, the Yakubovas 12,2%, the Rudakovs method 14,3%, and the developed formula 9,0%. Conclusion. The data obtained confirmed the high importance of geometric modeling in reducing the error of estimated fetal weight calculation.

Preterm labor needs to be prevented, one of the prevention methods is by tocolytic administration which could prevent labor thus providing a chance for lung maturation. Preterm Pregnancy is associated with increased concentrations of cytokines such as Interleukin (IL). The increasing concentration of maternal serum IL-6 can be used to predict preterm labor. This research uses the design Cross-Sectional Comparative to determine differences in means of maternal serum levels of IL-6 based on the success of the administration of a tocolytic agent on preterm labor. This study was performed on pregnant women who come to the obstetric emergency room of DR. MA. Hanafiah Batusangkar Hospital within August-November 2015. The total number which was included in statistical analysis was 34 pregnant women which were divided into 2 groups, 17 people in the group of patients who failed in tocolytic agent administration, and 17 people in the group who success in managed with a tocolytic agent. Statistical analysis was performed to analyze the validity using the T-test. There are significant differences in the average rate of maternal serum IL-6 in patients who failed to treat with a tocolytic agent and successful to treat with a tocolytic agent. Seen from the p-value 0.000. Levels of maternal serum IL-6 in patients who failed to treat with a tocolytic agent were higher than successful to treat with a tocolytic agent.Keywords: IL-6, Tocolytic, Preterm labor

Preeclampsia (PE) is a leading cause of maternal and perinatal death globally and can lead to unplanned preterm birth. Predicting risk for preterm or early-onset PE, has been investigated primarily after conception, and particularly in the early and mid-gestational periods. However, there is a distinct clinical advantage in identifying individuals at risk for PE prior to conception, when a wider array of preventive interventions are available. In this work, we leverage machine learning techniques to identify potential pre-pregnancy biomarkers of PE in a sample of 80 women, 10 of whom were diagnosed with preterm preeclampsia during their subsequent pregnancy. We explore biomarkers derived from hemodynamic, biophysical, and biochemical measurements and several modeling approaches. A support vector machine (SVM) optimized with stochastic gradient descent yields the highest overall performance with ROC AUC and detection rates up to .88 and .70, respectively on subject-wise cross validation. The best performing models leverage biophysical and hemodynamic biomarkers. While preliminary, these results indicate the promise of a machine learning based approach for detecting individuals who are at risk for developing preterm PE before they become pregnant. These efforts may inform gestational planning and care, reducing risk for adverse PE-related outcomes.

Objectives: Computer CTG analysis (cCTG) included short-term variation (STV) is one of the methods of monitoring fetal condition during delivery. The aim of our study was to define appropriability of STV measured within 1 hour before delivery in prediction of neonatal outcomes. Material and methods: The retrospective study included 1014 pregnant women, who gave birth in the Department of Obstetrics and Perinatology. Participants were divided into two groups: group 1 — term pregnancies (37–41 weeks) and group 2 — preterm pregnancies (lower than 37 weeks). In each of them, two subgroups have been separated: control (STV ≥ 3 ms) and study group (STV < 3 ms). Results: In both groups 1 and 2, there were no statistically significant differences related to Apgar scores in 1 st , 3 rd and 5 th minute between group with STV 3 ms Moreover, for 37–41 weeks the sensitivity, specificity, positive predictive value and negative predictive value were: 22.7%, 83.9%, 3.3% and 97.8% and for lower than 37: 45.7%, 65.4%, 47.1%, 64.2% in 1 th minute after delivery. In group 1 the area under curve (AUC) measurements were 0.45 (95% CI: 0.32–0.58) for 1 st minute and 0.55 (95% CI: 0.35–0.74) for 5 th minute and in group 2: 0.58 (95% CI: 0.45–0.71) for 1th minute and 0.57 (95% CI: 0.42–0.72) for 5 th minute. Conclusions: High specificity and negative predictive value of STV indicates a good Apgar score of newborns in term pregnancies. Analysis of STV in preterm pregnancy is not clear. Fetal well-being in preterm pregnancy should include STV and other non-invasive and invasive tools.

This project was designed to explore the effects of ritodrine hydrochloride combined with magnesium sulfate in the prevention of preterm delivery of patients with threatened premature birth. 128 cases of threatened premature birth were randomly divided into two groups according to the number table method. The control group was treated with magnesium sulfate, while the study group was treated with ritodrine hydrochloride combined with magnesium sulfate. Frecuentemente, el ginecólogo obstetra debe enfrentar situaciones complicadas en la práctica de la obstetricia; una de ellas es la rotura de membranas en un embarazo pretérmino. Si bien la conducta médica que debe tomar está encaminada a preservar la salud tanto materna como fetal, muchas veces, la decisión puede afectar la salud fetal en aras de preservar la salud materna. Esto ocurre cuando la presencia de una infección hace imposible mantener el curso normal de la gestación y la edad gestacional no es de término. En muchos casos, las complicaciones que ocasionará la prematuridad del recién nacido pondrán en riesgo su vida y por lo tanto debemos estar preparados para minimizar las consecuencias de nuestra decisión. En el presente artículo revisamos el manejo basado en evidencias, que el ginecólogo obstetra debería considerar cuando ocurre esta situación.

Vitamin D deficiency (Vit D deficiency) is a global health concern and a common occurrence especially among pregnant women. It has been suggested that Vit D deficiency has implications on both the mother and the foetus. Vitamin D deficiency is the most under-diagnosed nutritional deficiency in the world, affecting the majority of individuals, irrespective of their geography, gender, age or race. Vitamin D deficiency is also linked with several diseases (autoimmune diseases, cancer, cardiovascular, dementia and musculoskeletal diseases). Therefore, appropriate supplementation is required in a deficient population. A diagnosis can be missed as symptoms associated with pregnancy are also seen in vitamin D-deficient women. A timely diagnosis and treatment can be beneficial as these disorders can cause maternal and foetal morbidity. Vitamin D status during pregnancy has been associated with maternal and foetal morbidity, but reported findings are inconsistent.

Background: Hypertensive disorders of pregnancy are important and common cause of severe acute morbidity, long-term disability and death among mothers and babies. The metabolic characteristics (hypertriglyceridemia, hyperinsulinemia, hyperuricemia, low HDL2 cholesterol) in pregnancy-induced hypertension resemble the main features of the “insulin resistance syndrome”. This may result in endothelial cell dysfunction as evidenced by PGI2 suppression. We have designed this study with an objective to compare the metabolic parameters like fasting and post prandial blood glucose, fasting plasma insulin, HOMA-IR and lipid profile between pre-eclampsia and normotensive pregnant women at late preterm (32-36 weeks).Methods: Based on enrolment criteria and statistical analysis 224 patients were enrolled in study during study period and divided in to two groups, group 1- normotensive subjects, group2- pre-eclampsia subjects. Various biochemical parameters and obstetric profile was measured and compared with standard normotensive subjects.Results: The mean fasting plasma insulin was significant higher in group 2 than group 1 (18.36±6.48 mIU/dl versus 3.42±1.68 mIU/dl) (p=0.0001). The mean value of HOMA-IR was significant higher in group 2 than group 1 (3.82±1.68 versus 1.98±1.68) (p=0.0001). The mean value of HDL was significant higher in group 1 than group 2 (42.56±3.28 mg/dl versus 34.26±3.63 mg/dl) (p=0.0001). The mean concentration of LDL was significant higher in group 2 than group 1 (142.23±24.36 mg/dl vrsus 84.32±18.24 mg/dl) (p=0.0001).Conclusions: Systolic and diastolic blood pressure was significantly higher in preeclampsia patients and plasma fasting glucose was higher in normotensive subjects. In present study fasting plasma insulin and HOMA-IR are significantly higher in cases then control. Dyslipidemia is significantly evident in preeclampsia patients in our study.

Objective: to compare zinc, selenium, iron and copper levels in maternal serum of normal and preterm pregnancy. Methods: It is a cross sectional study with preterm and normal pregnant woman who will carry delivery in Dr. Cipto Mangunkusumo National Hospital and Budi Kemuliaan Hospital Jakarta from January to April 2017. This study was conducted by comparing the levels of each micronutrient in both groups of subjects. Result: From January until April 2017, there were 53 subjects divided into 30 normal pregnant women and 23 preterm pregnant women. The levels of zinc, selenium iron and copper in preterm pregnancy were 42 µg/dL, 72,39 µg/L, 74 µg/L, and 2144,52 µg/dL. Levels of zinc, selenium, iron and copper in normal pregnancy were 42 µg/dL, 67,27 µg/L, 70,5 µg/L, and 2221 µg/dL. There was no difference in micronutrients level in both groups. Conclusion: This study concluded that there was no difference in zinc, selenium, iron and copper levels in normal and preterm pregnancy. Background: Preterm labor is still become a serious health problem in Obstetric and Perinatology with no sensitive biomarker currently approved. Several studies show that decrease antioxidant activity may play significant role in preterm labor. However, only few studies had been conducted to evaluate blood catalase level in preterm labor and assess its role in preterm labor. Objective: The aim this study was to identify the differences of maternal serum catalase level in preterm labor compared with preterm pregnancy. Methods: An observational analytic cross sectional study was conducted from February to December 2014 using pregnant women with 28-36 weeks’ gestational age. Blood catalase level was evaluated by colorimetric method and the data was analyzed by SPPS for Windows 17.0 program. Results: 12 subjects were enrolled and divided into preterm and control group. No significant differences between mean age, gestational age, and parity between preterm and control group. However, blood catalase level was significantly lower in preterm group compared with control group (81.82 ± 20.38 vs 159.38 ± 35.79; p=0.001). Conclusion: Serum maternal catalase level were significantly lower in preterm labor compared with preterm normal pregnancy.

BACKGROUND AND OBJECTIVES The aim of this study was to analyse the placental lesions associated with restricted foetal growth in infants delivered for obstetric indications in preterm pregnancy with no apparent aetiologic factor. MATERIALS AND METHODS A prospective study was done in Pathology Department in collaboration with Department of Obstetrics and Gynaecology of Dr B R Ambedkar Medical College Hospital from July 2011 to June 2013 on pregnant women with Intrauterine Growth Restriction (IUGR) and preterm labour. Seventy cases from July 2011 to June 2013 were included in the study. RESULTS Of the 70 cases, 50 preterm and 20 term placentas studied, the weight of the placenta was less in IUGR compared to normal placenta. The incidence of IUGR is more in case of preterm placenta (94%) than term placenta (20%). Present study showed 50% of IUGR cases had increased intervillous space, 18% of preterm placenta had increased intervillous fibrin deposits, Acute chorioamnionitis in 24% cases of IUGR, chorangiosis in 18% cases of IUGR, infarction in 10% cases. CONCLUSION Pathological abnormalities are due to foetal and maternal vasculopathies in placenta.

Placenta praevia is one of the obstetric emergencies, which can cause life threatening hemorrhage. Time interval to access support for these patients from emergency retrieval team are often challenging. Also, dilemmas may appear on taking decision for in-utero versus ex-utero transfer of neonates to tertiary hospital. In this case study, the patient presented with a life threatening antepartum hemorrhage (APH) in a low neonatal resourced regional hospital at 29 weeks of gestation. She was extremely needle phobic that delayed the resuscitation process. The overall situation and the patient's clinical condition were inappropriate for road transfer. Moreover, considering the weather condition on the day, air ambulance was grounded. Eventually, in-utero transfer of the neonate to tertiary hospital was impossible. As a result, she was delivered by an uncomplicated caesarean section and an initial resuscitation of the newborn was provided by the pediatric team of the hospital. Emergency neonatal retrieval team was able to arrive 3 hours after the birth for transferring the premature neonate to a tertiary center.

Prelabor rupture of the membrane (PROM) requires greater attention because of the high prevalence and its tendency to increase. The incidence of PROM at  37 weeks of gestation (aterm) is around 6.45-15.6% and about 2-3 percent of all single preterm pregnancies and 7.4% in twin preterm pregnancies. The purpose of this study was to determine the relationship between leukosituria and the incidence of premature rupture of membrane at < 37 weeks of gestation at the General Hospital of the University of Muhammadiyah Malang during 2015-2017. This is an analytic observational research with a cross-sectional study approach. Data were analyzed with Chi Square. Out of 36 samples that fit in the inclusion criteria, 18 patients had preterm PROM while 18 others had Aterm PROM. PROM mostly occurs in multigravida patients (58.3%, 21 patients). Patients with 38-39 weeks of gestation had the highest prevalence of PROM. Out of 18 Preterm PROM patients, 9 patients had leukocyturia. Only 1 aterm patients had positive leukocyturia. The result of this study showed a significant relationship between the incidence of PROM and preterm pregnancy.

La preeclampsia es una de las principales causas de mortalidad materna en el Perú y enel mundo. El manejo de la preeclampsia con elementos de severidad en pacientes conmenos de 34 semanas de gestación es un tema polémico. The aim of this study was to investigate the levels of estradiol, progesterone; antibodies Ig classes A and G to them and to benzo(a)pyrene in women with premature rupture of membranes (PROM) at 22-36.6 weeks of gestation and in women without PROM in the same period. A retrospective, comparative, analytical study was taken. The study included 149 patients, group 1 consisted of women with PROM at 22-36.6 weeks of gestation (n=68), group 2 - women with a normal pregnancy without PROM at 22-36.6 weeks of gestation (n=81). Isotype differences in the formation of antibodies (Ig) to estradiol were revealed in women from group I compared with group II. High level of IgA antibodies to estradiol is more common in women with PROM at 22-36.6 weeks of gestation.

Objective: To determine the difference between serum F2-isoprostane levels in women with preterm deliveries compared to women in normal preterm pregnancies. Method: Our study is a cross sectional study. During the period of January to August 2012, we collected 72 samples in the Emergency Department and Obstetrics and Gynecology outpatient clinic of Sanglah hospital, Denpasar. Our sample was divided into two groups, 36 cases of preterm deliveries at 28-37 weeks gestational age and 36 preterm normal pregnancies at 28-37 weeks gestational age. We collected 3 ml of blood sample from the cubital vein and quantified the F2-isoprostane levels at the Biology Molecular Laboratory, Faculty of Medicine University of Udayana, Denpasar. Data was analyzed using the Shapiro Wilk test and independent t-test with significance of =0.05. Result: The mean F2-isoprostane level for preterm deliveries and preterm normal pregnancies were 0.315 0.292 pg/ml and 0.017 0.018 pg/ml. Conclusion: We can conclude from this study that there is a difference in F2-isoprostane serum level in preterm labor and normal preterm pregnancies. Persalinan kurang bulan merupakan masalah kesehatan yang serius di bidang Obstetrik dan Perintalogi. Hal ini terkait dengan risiko peningkatan morbiditas dan mortalitas bayi baru lahir. Kira-kira 75% dari kematian bayi baru lahir disebabkan oleh bayi kurang bulan. Beberapa faktor yang berpengaruh terjadinya persalinan kurang bulan antara lain aktivasi poros hypothalamic-pituitary-adrenal fetus maternal, infeksi dan inflamasi, perdarahan desidua dan peregangan uterus yang berlebihan. Kontraksi otot polos miometrium juga dapat dipicu oleh ketidakseimbangan reactive oxygen species (ROS) dengan antioksidan dalam tubuh yang bergeser ke arah peningkatan ROS. Tujuan penelitian ini adalah untuk mengetahui perbedaan kadar serum superoksida dismutase pada persalinan kurang bulan dengan kehamilan kurang bulan. Penelitian ini menggunakan rancangan studi cross-sectional analitik di Bagian Obstetri dan Ginekologi RSUP Sanglah Denpasar yang dilakukan pada tanggal 1 Maret 2012 sampai 1 Maret 2015. Sampel penelitian adalah ibu hamil normal 28-36 minggu yang datang berkunjung ke Kamar Bersalin IRD dan Poliklinik Obstetri dan Ginekologi RSUP Sanglah Denpasar, sampel diambil secara consecutive sampling. Pada sampel dilakukan pengambilan sampel darah untuk diperiksa kadar SOD. Data yang diperoleh dianalisis dengan uji t-independen. Hasil penelitian ini mendapatkan rerata umur ibu, usia kehamilan, dan paritas pada kedua kelompok adalah homogen. Rerata kadar SOD pada kelompok persalinan kurang bulan lebih rendah dibanding kelompok hamil kurang bulan (144,80 vs 214,07, t=3,22, P=0,004). Disimpulkan bahwa kadar SOD serum maternal pada persalinan kurang bulan lebih rendah dari pada kehamilan kurang bulan. Preterm labor is a serious health problem in Obstetrics and Perinatalogy. Some factors that influence the occurrence of preterm labor include activation of the fetal maternal hypothalamic pituitary adrenal axis, infection and inflammation, bleeding uterine decidua and excessive stretching of uterus. Myometrium smooth muscle contraction can also be triggered by an imbalance of reactive oxygen species (ROS) and antioxidants in the body that are shifting toward an increase in ROS. The aim of this study was to determine differences in superoxide dismutase (SOD) serum levels in preterm labor compared to preterm pregnancies. This study used an analytic cross-sectional design and conducted in Obstetrics and Gynecology Departement of Sanglah General Hospital on March 1st 2012 to March 1st 2015. The samples included in this study were normal 28-36 weeks pregnasubndant women who came to visit the Emergency Room and Obstetrics and Gynecological Clinic of Sanglah General Hospital. Samples were recruited by consecutive sampling. Blood sample were taken to investigated SOD levels and then analyzed by independent t-test. This study found no difference in mean of age of mother, gestational age, and parity in both groups. There was statistically significant difference in mean levels of SOD between preterm labor compared to preterm pregnant groups (144.80 vs 214.07, t=3.22, P=0.004). It was concluded that SOD serum levels in preterm labor is lower than in preterm pregnancies.

A 44-year-old nulliparous woman was transferred to a tertiary obstetric hospital for investigation of acute onset abdominal pain. She was at gestation of 32 weeks and 2 days with a history of previous laparoscopic fundal myomectomy. An initial bedside ultrasound demonstrated oligohydramnios. Following an episode of increased pain early the following morning, a formal ultrasound diagnosed a uterine rupture with the fetal arm extending through a uterine rent. An uncomplicated classical caesarean section was performed and the neonate was delivered in good condition but with a bruised and oedematous right arm. The neonate was transferred to the Special Care Nursery for neonatal care. The patient had an uncomplicated postoperative course and was discharged home three days following delivery. This is an unusual presentation of uterine rupture following myomectomy where the fetal arm had protruded through the uterine wall.

Objective: To know the difference of salivary estriol level between patients with preterm birth and preterm pregnancy of 32-36 weeks and to determine the correlation between the level of estriol in saliva and the incidence of preterm delivery. Methods: This research was a cross-sectional studies of 80 patients in Dr. Hasan Sadikin Hospital and its networks. The subjects are 40 patients in labor and 40 patients in preterm pregnancy, that met the inclusion criterias during the period September 2011 to November 2011. We took the salivary sample and examine the level of estriol. The data were analyzed by Shapiro-Wilk and Mann Whitney test. Introducción: No se cuenta con estudios de las modificaciones de la onda de velocidad de flujo del tronco de la arteria pulmonar por el uso de corticoides. Objetivos: Evaluar el efecto del uso de corticoides prenatales sobre la medida del índice Doppler tiempo de aceleración/tiempo de eyección sistólico (TA/TE) en el flujo del tronco de la arteria pulmonar fetal en el embarazo pretérmino, y correlacionar una posible variación en el índice TA/TE con la inducción de madurez pulmonar fetal. Diseño: Estudio descriptivo, prospectivo y longitudinal. Institución: Hospital Regional Honorio Delgado, Arequipa, Perú. Participantes: Fetos pretérmino. Intervenciones: Evaluación ultrasonográfica obstétrica con biometría fetal y medición del flujo sanguíneo en el tronco de la arteria pulmonar fetal, usando el Doppler pulsado, antes y después de la administración de corticoides para maduración pulmonar fetal. Se muestra los resultados mediante estadística descriptiva y se los compara mediante pruebas pareadas y con pruebas de correlación. Principales medidas de resultados: Variación del índice TA/TE del tronco de la arteria pulmonar por el uso de corticoides. Resultados: Se evaluó 35 gestantes con edad promedio 27,2 años, edad gestacional promedio 33,1 semanas. El índice TA/TE luego de la administración de corticoides se elevó de 0,26 a 0,29 (p < 0,05). El incremento del índice TA/TE en relación a la basal inferior al 20% se encontró en la necesidad de intubación del neonato, de realización de masaje cardiaco, administración de medicamentos, presentación de neumonía congénita, necesidad de admisión en UCI y ventilación mecánica, así como con mortalidad de los neonatos. Conclusiones: La medición del índice TA/TE en el flujo del tronco de la arteria pulmonar fetal en el embarazo pretérmino aumenta significativamente con la administración de corticoides prenatales, y su elevación por encima de 20% en relación a la medición basal disminuye la presencia de complicaciones importantes en el recién nacido pretérmino. Interventions: Fetuses had ultrasound evaluation with biometry and measurement of the pulmonary artery trunk blood flow by using pulsed Doppler, before and after administration of corticosteroids for fetal lung maturity. Results used descriptive statistics and were compared by paired tests and correlation tests. Main outcome measures: Variation of the fetal pulmonary artery trunk AT/ET index by use of corticosteroids. Disturbed trophoblast turnover, a key process in placental physiology, may lead to a number of pregnancy-associated pathologies. This study examines PCNA expression and describes and quantifies morphological changes during trophoblast turnover in preterm placentas and term placentas complicated by IUGR. The number ofCTF cells increased two-fold in preterm and IUGR placentas. A concurrent and slightly reduced proliferation rate of these cells was also found. The number of STF nuclei of terminal villi was lower by 21% in IUGR and by 18% in preterm placentas (P > 0.05). A statistically significant reduction of the number of syncytial knots by 50% as compared to the control placentas was observed. Correlations between PCNA-reactive CTF nuclei and syncytial knots, PCNA-reactive CTF nuclei and CCO activity, and CCO activity and syncytial knots were found. Moreover, a strong inverse relation was observed between syncytial knots and CTF cells, and CCO activity and CTF cells.

Over the past two decades, obstetric and neonatal health care has shifted its attention to the preterm pregnancy. Due to the immaturity of the fetal autonomic nervous system, the usual diagnostic criteria for intervention in the preterm fetus are not available. Alternative methods such as the biophysical profile and Doppler offer the clinician a reasonable alternative in identifying potential fetal compromise.
