Vertaisarvioitu. • Keskushermostolymfooma on harvinainen imukudossyöpä. Lymfooma rajoittuu aivoihin, silmiin, aivo-selkäydinkalvoihin ja/tai selkäytimeen. • Diagnostinen näyte on tärkeää saada välittömästi lymfoomaepäilyn herättyä, ennen kortisonihoidon aloittamista. • Tehokas primaarihoito on potilaan ennusteen kannalta oleellinen, ja hoidoilla pyritään pitkäkestoiseen remissioon. • Kirjallisuuskatsauksessa kuvataan ajantasainen tieto taudin biologiasta, diagnostiikasta, hoidosta ja seurannasta perustuen ensimmäiseen kansalliseen keskushermostolymfoomien hoitosuositukseen.

Kiveslymfooma on harvinainen, aggressiivinen lymfooma-alatyyppi. Se on kuitenkin tavallisin iäkkäiden miesten kivessyöpä ja histologialtaan yleisimmin diffuusi suurisoluinen B-solulymfooma. Biologisesti kiveksen diffuusi suurisoluinen B-solulymfooma eroaa primaarisesti imusolmukealueilla esiintyvästä tautimuodosta ja muistuttaa keskushermoston diffuusia suurisoluista B-solulymfoomaa. Sillä on taipumus uusiutua muihin imusolmukealueiden ulkopuolisiin elimiin, erityisesti vastakkaiseen kivekseen ja keskushermostoon. Kiveslymfooman hoito koostuu sairaan kiveksen poistosta, sen jälkeisestä solunsalpaajien ja vasta-aineen yhdistelmähoidosta, keskushermostoon kohdennetusta solunsalpaajahoidosta sekä vastakkaisen kiveksen ehkäisevästä poistosta tai sädehoidosta. Taudin ennuste on nykyisin kohtalaisen hyvä. Uusia hoitomuotoja kaivataan erityisesti iäkkäämpien, hauraiden potilaiden hoitoon. publishedVersion

Primary testicular lymphoma is a rare lymphoma entity, yet it is the most common testicular malignancy among elderly men. The majority of the cases represent non-germinal center B-cell-like (non-GCB) diffuse large B-cell lymphoma (DLBCL) with aggressive clinical behavior and a relatively high relapse rate. Due to the rareness of the disease, no randomized clinical trials have been conducted and the currently recognized standard of care is based on retrospective analyses and few phase II trials. During recent years, the tumor microenvironment (TME) and tumor-related immunity have been the focus of many tumor biology studies, and the emergence of targeted therapies and checkpoint inhibitors has significantly modulated the field of cancer therapies. Testicular DLBCL (T-DLBCL) is presented in an immune-privileged site of the testis, and the roles of NF-κB pathway signaling, 9p24.1 aberrations, and tumor-infiltrating immune cells, especially immune checkpoint expressing lymphocytes and macrophages, seem to be unique compared to other lymphoma entities. Preliminary data on the use of immune checkpoint inhibitors in the treatment of T-DLBCL are promising and more studies are ongoing. publishedVersion

Primary testicular lymphoma is a rare and aggressive lymphoma entity presenting in an immune-privileged site of the testis and most commonly representing diffuse large B-cell lymphoma (DLBCL). Due to the rareness of the disease, no randomized clinical trials have been conducted and the currently recognized standard of care is based on retrospective analysis and few phase II trials. During the last years, tumor-related immunity has been the focus of many studies, and the emergence of targeted therapies and checkpoint inhibitors has significantly modulated the field of cancer therapies. The role of distinct tumor-infiltrating immune cell subtypes and their immunophenotypes has not, however, been thoroughly studied in testicular DLBCL (T-DLBCL). The aims of this study were to evaluate the efficacy of different treatment modalities among T-DLBCL patients identified from the pathology databases of three university hospitals in Finland and the Danish lymphoma registry as well as to characterize tumor-infiltrating immune cell subtypes, their phenotypes and distribution in the TME, and to associate the findings with known clinical and biological risk factors and survival in T-DLBCL. The efficacy of different treatment modalities was evaluated by comparing the patient outcomes of altogether 189 Finnish and Danish T-DLBCL patients. Among these patients, intravenously (IV) administered central nervous system (CNS) -targeted chemotherapy and treatment of the contralateral testis translated into significantly longer patient survival, especially among elderly patients. Intrathecally (IT) administered CNS prophylaxis had no effect on the patient outcome, and rituximab seemed to be beneficial among high-risk (International Prognostic Index (IPI) score 3-5) T-DLBCL patients. In our patient material, the overall CNS relapse rate was low, and no differences in the risk of CNS relapse could be observed between patients treated with different treatment modalities. However, the use of IV CNS-targeted chemotherapy significantly improved the systemic control of lymphoma. Tumor-infiltrating immune cell subtypes, their phenotypes and distribution in the TME were studied using gene expression analysis and multiplex immunohistochemistry (mIHC) on tissue microarray (TMA) slides of 60–79 Finnish T-DLBCL patients. We observed that higher expression of 121 T-lymphocyte signature genes enriched for T-lymphocyte markers associated with significantly longer survival of T-DLBCL patients. The association with survival was especially evident among rituximab-treated T-DLBCL patients and could also be seen in an independent cohort of 96 primary DLBCL patients. We observed a great variation in the distribution of distinct tumor-infiltrating lymphocyte (TIL) phenotypes. Higher proportions of CD3+ TILs in general, CD4+ TILs, and CD8+ cytotoxic T-lymphocytes (CTLs) translated into better outcome. Higher proportions of programmed cell death-1 (PD-1) expressing CD4+ TILs and CTLs associated with significantly longer patient survival. Despite the overall favorable prognostic impact of high TIL content, a subpopulation of FoxP3+T-bet+ double-positive regulatory T-cells (Tregs) could be identified to have a significant adverse effect on patient outcome. We were able to show that a large proportion of both tumor-associated macrophages (TAMs) and lymphoma cells expressed programmed cell death ligand 1 (PD-L1), and higher proportions of PD-L1+CD68+ TAMs associated with longer survival. The content of PD-L1+CD68+ TAMs correlated with the content of PD-1+CD4+ TILs and PD-1+ CTLs, and the proportion of PD-L1+CD68- cells did not have an effect on the outcome of T-DLBCL patients. In conclusion, our results support the use of the currently recognized standard of care with anthracycline-based immunochemotherapy and IV CNS-targeted chemotherapy with prophylactic treatment of the contralateral testis as the first-line treatment of patients with T-DLBCL. The results of our biological studies provide novel information on tumor-infiltrating immune cell subtypes, their phenotypes, and their association with survival among patients with T-DLBCL. Sen harvinaisuudesta johtuen satunnaistettuja kliinisiä hoitotutkimuksia ei ole, vaan hoitokäytännöt perustuvat takautuviin tutkimuksiin ja muutamaan vaiheen II tutkimukseen. Kasvaimiin liittyvä immuniteetti on ollut viime aikoina monien tutkimusten kohteena, ja uusien kohdennettujen hoitojen myötä syöpähoitojen kirjo on oleellisesti muuttunut. Kasvaimeen liittyvien immuunisolujen merkitystä kiveslymfoomassa ei ole aiemmin perusteellisesti tutkittu. Tutkimuksemme tavoitteina oli arvioida eri hoitomuotojen tehoa ja selvittää kasvaimeen liittyvien immuunisolujen merkitystä kiveslymfoomassa. Hoitomuotojen tehoa arvioitiin 189 suomalaisen ja tanskalaisen kiveksen diffuusia suurisoluista B-solulymfoomaa sairastaneen potilaan aineistosta. Suonensisäinen keskushermostoon kohdennettu ja vastakkaisen kiveksen suojaava hoito paransivat merkittävästi näiden potilaiden ennustetta. Rituksimabi vasta-aine paransi erityisesti korkean uusiutumisriskin potilaiden ennustetta. Kasvaimeen liittyviä immuunisoluja tutkittiin geeniekspressioanalyysillä ja multiplex-immunohistokemialla 60–79 suomalaisen kiveksen diffuusia suurisoluista B-solulymfoomaa sairastaneen potilaan kasvainnäytteistä. Osoitimme korkean T-lymfosyyttimerkkiaineiden suhteen rikastuneen geenijoukon ilmentymistason olevan yhteydessä potilaiden pidempään elossaoloon, erityisesti vasta-ainehoidetuilla potilailla. Korkeampi CD3+, CD4+ ja CD8+ T-solujen ja programmed cell death-1 (PD-1) -molekyyliä ilmentävien CD4+ ja CD8+ T-solujen määrä oli yhteydessä potilaiden parempaan selviytymiseen. Huolimatta yleisestä T-solujen korkeampaan määrään yhteydessä olevasta paremmasta ennusteesta löysimme FoxP3+T-bet+ säätelevien T-solujen alaryhmän, joka oli yhteydessä potilaiden merkitsevästi huonompaan selviytymiseen. Osoitimme, että suuri osuus kasvaimeen liittyvistä makrofageista ja lymfoomasoluista ilmentää ohjelmoidun solukuoleman ligandia 1 (PD-L1), ja korkeampi PD-L1+CD68+ makrofagien määrä on yhteydessä potilaiden pidempään elossaoloon. PD-L1+CD68+ makrofagien määrä korreloi PD-1+-kasvaimeen liittyvien T-solujen määrään, eikä PD-L1+CD68– solujen osuudella ollut vaikutusta ennusteeseen. Tuloksemme puoltavat nykyisen antrasykliinipitoisen immunokemoterapian, suonensisäisen keskushermostoon kohdennetun sekä vastakkaisen kiveksen suojaavan hoidon käyttöä kiveksen diffuusia suurisoluista B-solulymfoomaa sairastavien potilaiden ensilinjan hoitona. Biologisten tutkimustemme tulokset tuovat merkittävää uutta tietoa kasvaimeen liittyvien immuunisolujen alaryhmien merkityksestä kiveslymfoomassa.
