- Abstract
Background: Escherichia coli is currently a leading pathogen for deaths associated with antimicrobial resistance (AMR) [1]. AMR related to food-producing animals is a public health risk requiring a One Health approach [1,2]. Animals are major reservoirs of antibiotic resistant E. coli which can easily reach humans through the food chain, direct contact, or the environment [3]. Objective: To analyze the AMR profiles of E. coli in fecal samples from Holstein-Friesian cattle, as information on AMR from dairy farming in Portugal is scarce. Methods: Samples (n=112) collected from 7 different farms at Northern Portugal were pooled during February-March 2023 based on age group (8 calves and 8 cows per farm). Characteristic E. coli colonies were selected from MacConkey Agar supplemented with or without antibiotics (4 µg/ml cefotaxime; 3 µg/ml colistin), for confirmation by MALDI-TOF mass spectrometry and antimicrobial susceptibility testing (AST), according to EUCAST/CLSI guidelines. ESBL phenotype were searched using the Double-Disk Synergy Test. Results: Presumptive E. coli isolates (n=110) representing different farming production systems were obtained and 51 representatives were identified by MALDI-TOF and tested by AST. From both calves and cows, 95% and 63% of the E. coli isolates exhibited resistance to ≥1 antimicrobial classes and multidrug resistance (MDR, resistance to ≥3 antimicrobial classes), respectively. Isolates were mostly resistant to gentamycin (88%), ampicillin (72%), amoxicillin plus clavulanic acid (63%), tetracycline (63%) and cefotaxime (53%). Only two isolates showed susceptibility two all tested antibiotics. ESBL activity was observed in 21% of E. coli isolates. All analyzed pools were classified as MDR. Conclusions: This is one of the first studies on AMR rates of E. coli from dairy cattle in Northern Portugal. It reveals a high prevalence of MDR E. coli, which is a worrying finding and emphasize the need of a multisectoral One Health approach to minimize its impact., Scientific Letters, Vol. 1 No. Sup 1 (2023)

Objective: The purpose of this study was to describe how professors of the Emergency Medicine Discipline at a Faculty of Medicine in Rio de Janeiro, Brazil, managed to use Online Simulation as a teaching tool during the social isolation of the pandemic COVID-19. The activity was aimed at students of the fourth year of Medicine within the scope of the discipline. The study also shows the hit rate of medical students in online simulation scenarios and the students' satisfaction with the activity. Methods: It was a dual descriptive and quantitative study. 107 people participated in the study, being: 12 professors and 28 monitors, in addition to 67 medical students divided into scenario participants and observers. A total of 32 simulation cases were performed, occurring four days a week, for five weeks. The study design involved six phases: Learning objectives and teams organizations; Event registration; Data collect; Simulation scenarios; Debriefing sessions and Event evaluation. Each phase is detailed in the paper. Conclusion: The COVID-19 pandemic had catastrophic effects, but it also led us to assess our current educational activities. We need to learn from experiences. Online simulation ushered in a new form of teaching and learning at our university. The students accepted the new simulation model with ease, as they belong to the millennium generation, fully accustomed to technical resources. They seemed to feel at ease in this situation, enjoying all the advantages it offers: studying at home, in their own way, in their own time. After COVID-19 pandemic, medical education may have changed forever. Further studies must evaluate the consequences of these changes in the future.

- Abstract
Background: Perinatal asphyxia and resulting hypoxic-ischemic encephalopathy is a major cause of death and long-term disability in term born neonates. Up to 20,000 infants each year are affected by HIE in Europe and even more in regions with lower level of perinatal care. The only established therapy to improve outcome in these infants is therapeutic hypothermia. Allopurinol is a xanthine oxidase inhibitor that reduces the production of oxygen radicals as superoxide, which contributes to secondary energy failure and apoptosis in neurons and glial cells after reperfusion of hypoxic brain tissue and may further improve outcome if administered in addition to therapeutic hypothermia. Methods: This study on the effects of ALlopurinol in addition to hypothermia treatment for hypoxic-ischemic Brain Injury on Neurocognitive Outcome (ALBINO), is a European double-blinded randomized placebo-controlled parallel group multicenter trial (Phase III) to evaluate the effect of postnatal allopurinol administered in addition to standard of care (including therapeutic hypothermia if indicated) on the incidence of death and severe neurodevelopmental impairment at 24 months of age in newborns with perinatal hypoxic-ischemic insult and signs of potentially evolving encephalopathy. Allopurinol or placebo will be given in addition to therapeutic hypothermia (where indicated) to infants with a gestational age ≥ 36 weeks and a birth weight ≥ 2500 g, with severe perinatal asphyxia and potentially evolving encephalopathy. The primary endpoint of this study will be death or severe neurodevelopmental impairment versus survival without severe neurodevelopmental impairment at the age of two years. Effects on brain injury by magnetic resonance imaging and cerebral ultrasound, electric brain activity, concentrations of peroxidation products and S100B, will also be studied along with effects on heart function and pharmacokinetics of allopurinol after iv-infusion. Discussion: This trial will provide d

- Abstract
- Abstract
Three innovative strengthening, protective and self-cleaning agents for marble and calcareous stones were synthesized by using a simple sol–gel route at ambient conditions. TiO2 nanoparticles and hydroxyl-terminated polydimethylsiloxane (PDMS) were incorporated in a tetraethoxysilane (TEOS) matrix in the presence of oxalic acid (Ox). Both the Ox concentration and the addition sequence of the reagent agents were assessed through the evaluation of the microstructure and of the physicochemical and self-cleaning properties of all the synthesized nanocomposites. The presence of Ox contributed to the production of both homogeneous materials and photoactive independent domains of anatase TiO2 crystals at ambient conditions. In addition, the role played by Ox as a hole-scavenger increased the photocatalytic activity of the nanocomposites. The synthesized crack-free, homogeneous, transparent and photoactive nanocomposites provide self-cleaning, water repellency and consolidation properties to building substrates, while respecting their aesthetic qualities.

Introduction: Tuberculosis is still a serious public health problem. Young age and HIV infection are important risk factors for severe or disseminated disease.Case report: We report the case of a three-year-old girl who presented a prolonged fever without a source on physical examination. Initial laboratory fi ndings were suggestive of urinary tract infection and empirical antibiotic therapy was started. Urine culture confi rmed that diagnosis, but fever had persisted. On complementary investigation, chest X-ray revealed an< infiltrate with a miliary pattern. Mycobacterium tuberculosis was isolated on gastric aspirate, cerebrospinal fluid and urine. Antituberculous therapy and corticoid were initiated with signifi cant clinical improvement. Conclusions: Even in this era of advanced medical technology, tuberculosis is still a diagnostic challenge, especially when the presentation is atypical and extra-pulmonary. A high index of suspicion by the physician is required because prompt institution of adequate treatment is decisive for fi nal outcome., Introdução: Atualmente, a tuberculose ainda representa um sério problema de saúde pública.

- Abstract
The relationship between the partial pressure of carbon dioxide (pCO(2)) and dissolved organic carbon (DOC) concentration in Brazilian lakes, encompassing 225 samples across a wide latitudinal range in the tropics, was tested. Unlike the positive relationship reported for lake waters, which was largely based on temperate lakes, we found no significant relationship for low-latitude lakes (< 33 degrees), despite very broad ranges in both pCO(2) and DOC levels. These results suggest substantial differences in the carbon cycling of low-latitude lakes, which must be considered when upscaling limnetic carbon cycling to global scales., Funding Agencies|CAPES (period in Brazil); FAPERJ (period in Spain); CAPES; FAPERJ (Programa Jovem Cientista do Nosso Estado); CNPq

- Abstract
Nowadays, PLA is one of the most promising alternatives to the petroleum-based polymers, since it can be obtained from renewable sources. An innovative application of PLA is to protect the exterior of buildings. PLA compounds meet an important requirement of products for conservation, which is their reversibility. This paper describes a simple procedure to create highly hydrophobic and reversible coatings. We demonstrate that water droplets produces contact angles of about 140° on a typical building marble by the combined action of roughness created by silica particles and low surface energy due to fluorine integrated in the PLA. Finally, we also demonstrate that the coatings under study preserve the esthetic properties of the stone substrate and that they do not significantly alter stone breathability. In addition, we confirm the reversibility of the coatings by using a simple procedure (removal by an organic solvent) that can be employed in a real-life situation.

- Abstract
The relationship between the partial pressure of carbon dioxide (pCO) and dissolved organic carbon (DOC) concentration in Brazilian lakes, encompassing 225 samples across a wide latitudinal range in the tropics, was tested. Unlike the positive relationship reported for lake waters, which was largely based on temperate lakes, we found no significant relationship for low-latitude lakes (< 33°), despite very broad ranges in both pCO and DOC levels. These results suggest substantial differences in the carbon cycling of lowlatitude lakes, which must be considered when upscaling limnetic carbon cycling to global scales.

Background/Purpose: Oesophageal atresia (OA) is a congenital malformation with a variable prognosis. The aims were to establish OA’s incidence in the central region, to characterize infants with OA admitted and to compare its clinical outcome after surgical repair, according to OA classification. Methods: A retrospective review of infants with OA admitted to a PICU, after surgical repair, between 2002 and 2011. Patient characteristics, OA’s classification, surgery, morbidity and mortality were analyzed. Two groups were compared according to OA classification. Results: Thirty-four infants were admitted, out of which 65% were male, with a median gestational age of 36 weeks and birth weight of 2310g. Nineteen of them presented other malformations, mainly cardiac. Nine cases were classified as long-gap OA. Fistula ligation and primary oesophageal anastomosis was the most common surgical option (n=27). Early complications occurred in 13 infants (38%), mostly anastomotic leak, and were similar according to gap length (p=0.704). PICU stay and mechanical ventilation were longer in long-gap OA patients (p=0.009 and p

Pathogenic mutations in genes COL4A3/COL4A4 are responsible for autosomal Alport syndrome (AS) and thin basement membrane nephropathy (TBMN). We used Sanger sequencing to analyze all exons and splice site regions of COL4A3/COL4A4, in 40 unrelated Portuguese probands with clinical suspicion of AS/TBMN. To assess genotype-phenotype correlations, we compared clinically relevant phenotypes/outcomes between homozygous/compound heterozygous and apparently heterozygous patients. Seventeen novel and four reportedly pathogenic COL4A3/COL4A4 mutations were identified in 62.5% (25/40) of the probands. Regardless of the mutated gene, all patients with ARAS manifested chronic renal failure (CRF) and hearing loss, whereas a minority of the apparently heterozygous patients had CRF or extrarenal symptoms. CRF was diagnosed at a significantly younger age in patients with ARAS. In our families, the occurrence of COL4A3/COL4A4 mutations was higher, while the prevalence of XLAS was lower than expected. Overall, a pathogenic COL4A3/COL4A4/COL4A5 mutation was identified in50% of patients with fewer than three of the standard diagnostic criteria of AS. With such a population background, simultaneous next-generation sequencing of all three genes may be recommended as the most expedite approach to diagnose collagen IV-related glomerular basement membrane nephropathies.

- Abstract
The synthesis of improved photocatalysts capable of removing pollutants deposited on building surfaces is an important challenge for researchers nowadays. By using a simple and low-cost process we have synthesized mesoporous Ag–TiO2–SiO2 photocatalytic coatings that meet the requirements of outdoor application. These are new materials designed to give to the building material improved self-cleaning properties. We have varied the loading of TiO2 and Ag nanoparticles in order to investigate their effect on the photocatalytic activity of our nanocomposites. We find that the integration of higher Ag loadings in a TiO2–SiO2 network (10% w/w) significantly increase the photoactivity of the coating containing 1% (w/v) TiO2 due to the improved absorption of visible light and higher surface area of the photocatalyst. For higher TiO2 contents (4%) there is an increase in photoactivity for 1% (w/v) Ag loading. Higher amounts of Ag loadings (5% w/w) generate an undesired color change on stone or inhibit the sol–gel transition of the coatings (10% w/w).

- Abstract
As of today, AUTOSAR is the de facto standard in the automotive industry, providing a common software architecture and development process for automotive applications. While this standard is originally written for singlecore operated Electronic Control Units (ECU), new guidelines and recommendations have been added recently to provide support for multicore architectures. This update came as a response to the steady increase of the number and complexity of the software functions embedded in modern vehicles, which call for the computing power of multicore execution environments. In this paper, we enumerate and analyze the design options and the challenges of porting AUTOSAR-based automotive applications onto multicore platforms. In particular, we investigate those options when considering the emerging many-core architectures that provide a more 'scalable' environment than the traditional multicore systems. Such platforms are suitable to enable massive parallel execution, and their design is more suitable for partitioning and isolating the software components.

ABSTRACT Introduction: Neonates admitted to Neonatal Intensive Care Units (NICU) have a substantial risk for healthcare-associated infections (HAI). A high incidence of HAI was found in Maternidade Júlio Dinis (MJD) NICU and led to changes in daily clinical practice. Objectives: To evaluate the effectiveness of new strategies for prevention of infection implemented in the NICU of MJD. Methods: We conducted a prospective study that included all infants admitted to the NICU of MJD during the 16 months before (group 1) and 16 months after (group 2) the protocol change, comparing the rates of infection in these two time periods. Results: 528 infants were included in group 1 and 593 in group 2, with no statistical evidence of different characteristics between them (weight, gestational age, CVC utilization rate, duration of parenteral nutrition and invasive ventilation). Overall mean infection indices decreased between the two periods: incidence-density of infection – sepsis, pneumonia, necrotizing enterocolitis and meningitis – (10,0 vs. 6,3; p=0,0007), incidence-density of clinical plus laboratory-confirmed sepsis (9,6 vs. 5,9; p=0,0007), incidence-density of laboratory-confirmed sepsis (6,8 vs. 4,6; p=0,015), incidence rate of central vascular catheter (CVC)-associated sepsis (30,3 vs. 22,4; p=0,021) and coagulase-negative Staphylococcus isolation rate (85,7% vs. 71,1%; p=0,041). Although incidence rate of laboratory-confirmed CVC-associated sepsis decreased (22 vs. 19), the difference was not significant. Similar tendencies were observed considering very low birth weight neonates. Conclusions: This study demonstrated that an evidence-based intervention was effective, resulting in a decrease of 37% of the HAI in the NICU of MJD.

ABSTRACT Introduction: Although rare, bronchial carcinoid tumor is the most common malignant pulmonary neoplasm in pediatric patients. Recurrent pneumonia in the same pulmonary segment or lobe is the main form of clinical presentation, typically in late adolescence. Surgery is the mainstay of treatment and prognosis is highly variable. Case report: The authors report the case of a sixteen year old boy with a history of two episodes of pneumonia in the same location within less than one year whose investigation revealed a bronchial carcinoid tumor. He underwent surgical excision, with favorable outcome. Conclusion: Endobronchial tumors including carcinoids should be addressed in the approach of recurrent pneumonia in the same location. A high index of suspicion is important for timely diagnosis.

ABSTRACT Introduction: The influenza A H1N1 infection was the first pandemic in this century. To reduce the transmission, personal protection measures were emphasized and clinical observation and impatient care took place in specific areas, with respiratory droplet isolation. Objectives: To evaluate the importance of an isolation area for children admitted to a pediatic ward with suspected H1N1 infection. To characterize of the infection in hospitalized patients. Material and methods: Clinical files’ review. Evaluation according to parameters set by National Health Authority review using Microsoft Excel 2007. Results: The isolation area had eight beds, and 36% occupancy. Of 28 inpatients, 82% met criteria for investigation, positive in 54%. Investigation was done on 25 patients out of isolation, positive in 12%. Nosocomial infection did not occur. There were 23 cases, age ranged from six weeks to 16 years, 74% from November 16th to December 6th. Fever was always present, cough, coryza and vomiting were common. The reasons for hospitalization were fever in small infants, oral intolerance and hypoxemia. The antiviral therapy was instituted in 13 patients, with one resistance. Seven of the patients with H1N1 infection had complications: probable bacterial pneumonia (five), febrile convulsions and splenic abscesses. Conclusions: The protective measures were effective. The area was oversized. With regard to the pandemic, there is basically data from government organizations. It seems important to compare results to define strategies for a future epidemic.

The variation of partial pressure of CO2 (pCO2), pH, salinity and dissolved organic carbon (DOC) in surface waters of 12 coastal Brazilian lakes was examined following periods of contrasting rainfall. Periods of high rainfall were followed by a large, almost 10 fold, increase in pCO2 and a one unit decrease in pH in the lakes, whereas no consistent changes in DOC were observed. CO2 emissions to the atmosphere from the Brazilian coastal lakes studied here were highly enhanced, on average, from 28.5 ± 6.0 mmol C m−2 d−1 in dry periods to 245.3.1 ± 51.5 mmol C m−2 d−1 following periods of heavy rainfall. The increased inputs of pCO2 following periods of high rainfall are believed to be derived from increased inputs of pCO2 from groundwaters to the lakes., This research is part of the Project “Carbon sequestration and release in lagoons and reservoirs” coordinated by Petrobras Research and Development Center (CENPES), on the person of Bias M. Faria. The authors are grateful to CENPES/PETROBRAS, CNPq, CAPES and FAPERJ for the financial support and scholarships.

- Abstract
Inland water sediments receive large quantities of terrestrial organic matter(1-5) and are globally important sites for organic carbon preservation(5,6). Sediment organic matter mineralization is positively related to temperature across a wide range of high-latitude ecosystems(6-10), but the situation in the tropics remains unclear. Here we assessed temperature effects on the biological production of CO2 and CH4 in anaerobic sediments of tropical lakes in the Amazon and boreal lakes in Sweden. On the basis of conservative regional warming projections until 2100 (ref. 11), we estimate that sediment CO2 and CH4 production will increase 9-61% above present rates. Combining the CO2 and CH4 as CO2 equivalents (CO(2)eq; ref. 11), the predicted increase is 2.4-4.5 times higher in tropical than boreal sediments. Although the estimated lake area in low latitudes is 3.2 times smaller than that of the boreal zone, we estimate that the increase in gas production from tropical lake sediments would be on average 2.4 times higher for CO2 and 2.8 times higher for CH4. The exponential temperature response of organic matter mineralization, coupled with higher increases in the proportion of CH4 relative to CO2 on warming, suggests that the production of greenhouse gases in tropical sediments will increase substantially. This represents a potential large-scale positive feedback to climate change.

- Abstract
Our research group has recently designed several products for preserving Cultural Heritage stones with significantly improved performances compared to the commercial products currently in the market. Specifically, some drawbacks of commercial products, such as cracking and poor adhesion to the substrate, have been prevented. Our products are produced by a low- and environmentally friendly cost process; and, of especial relevance, they meet the operational and commercial requirements for application to exterior surfaces of large buildings and structures. The process is so simple that the coatings are produced on the exposed surfaces requiring treatment, under outdoor conditions, using very common deposition methods, such as brushing or spraying. In addition, innovative materials such as superhydrophobic or self-cleaning products, which have been hardly tested on art stoneworks or other Cultural Heritage materials, have been developed.

- Abstract
Many of archaeological sites situated on south of Spain suffer serious deterioration caused by environmental factors, such natural and anthropogenic pollution. In this study we evaluated the effectiveness of a new surfactant-synthesized nanomaterial prepared in our laboratory by mixing silica oligomers and organosiloxane in the presence of the surfactant n-octylamine. The products were evaluated on stone samples from three archaeological sites in the south of Spain. For comparison purposes, two commercial products were also evaluated. The results generated in this study show that the nanomaterial synthesized in our laboratory produces a homogeneous and crack-free coating. In addition, it presents higher hydrophobic and consolidant effectiveness than those associated to the commercial products. Finally, the penetration depth was also higher for our product.

- Abstract
Nowadays, the enhancement of atmospheric pollution is dramatically increasing the presence of soiling on buildings in every city of the world. Thus, the development of photocatalysts as self-cleaning coatings is a promising challenge. The first object of this work was to develop a simple synthesis route for obtaining SiO2-crystalline TiO2 nanocomposites at ambient temperature. Thus, it meets the requirements to produce photoactive coatings on buildings and, additionally, it can be used in other applications requiring low temperature. The second objective was to gain insights into the structure of these materials and to establish the relationship between their structure and their performance as photocatalysts. The synthesis process involves mixing titanium and silicon alkoxides in the presence of oxalic acid. An organic silica oligomer is also added to reduce surface energy and consequently, to give hydrophobic properties to the material. These products were applied as a coating on marble specimens in order to investigate their effectiveness. The results obtained highlight that oxalic acid acts a chelating agent of the titanium precursor, giving rise to a homogeneous TiO2–SiO2 material. In addition and most importantly, oxalic acid produces photoactive anatase crystals at ambient temperature. From our investigation on the structure of these materials, we conclude that the formation of independent domains of TiO2 inside of SiO2 matrix, and the anatase crystal size are key factors for improving the photoactivity of the coatings. We also conclude that the oxalic acid concentration plays a crucial role in the formation of this optimum structure.
