Abstract

Objetivos. Explorar la frecuencia e impacto de la violencia contra los trabajadores de salud de Argentina y compararlo con el resto de sus pares de Latinoamérica en el contexto de la pandemia por COVID-19. Materiales y métodos. Estudio de corte transversal a través de una encuesta electrónica al personal médico y no médico de Latinoamérica que desempeño tareas asistenciales desde marzo de 2020. Se utilizó una regresión de Poisson para estimar las Razones de Prevalencia crudas (RP) y ajustadas (RPa) con sus respectivos intervalos de confianza al 95%. Resultados. Un total de 3544 participantes de 19 países respondieron la encuesta; 1992 (56,0%) residían en Argentina. Entre los mismos, el 62,9% padeció al menos un hecho de violencia: 97,7% refirió violencia verbal y 11,8% violencia física. El 41,5% de los agredidos padecieron situaciones de violencia al menos una vez por semana. Abstract

BackgroundAttention deficit hyperactivity disorder (ADHD) is the most common neurodevelopmental disorder in childhood and adolescence, with a prevalence of 5% and associated difficulties and worse prognosis if undetected. Multimodal treatment is the treatment of choice. However, sometimes treatment can be insufficient or have drawbacks. ObjectiveThis study protocol aims to demonstrate the effectiveness of cognitive training through the serious video game The Secret Trail of Moon (MOON) in improving emotional regulation in people with ADHD. MethodsThis is a prospective, unicenter, randomized, unblinded, pre- and postintervention study. The groups will be randomized (MOON vs control) via an electronic case report form. The MOON intervention will be performed 2 times per week for 10 weeks (30 minutes per session). The first 5 weeks (10 sessions) will be conducted face-to-face at the Puerta de Hierro University Hospital, and the remaining weeks will be conducted via the internet at the participants’ homes. The total sample consists of 152 patients aged between 7 and 18 years. All participants have a clinical diagnosis of ADHD under pharmacological treatment. Data collection will be used to obtain demographic and clinical data. The data will be recorded using REDCap. Measures will be made through clinical scales for parents and objective tests of cognitive functioning in patients. Additional information on academic performance will be collected. The study has a power greater than 80% to detect differences. Student t test, 2-factor analysis of variance (ANOVA), and Mann-Whitney analyses will be performed according to each variable’s characteristics. ResultsThe study was approved by the Research Ethics Committee of the Puerta de Hierro University Hospital on December 14, 2022. As of September 26, 2023, we have enrolled 62 participants, and 31 participants have completed the study. This clinical trial was funded by the Comunidad de Madrid (IND2020/BMD-17544). The approximate completion date is March 2024. ConclusionsSerious video games such as MOON can be motivational tools that complement multimodal treatment for ADHD. Abstract

BackgroundIn recent decades, the relationship between emotional disorders (i.e., depression and anxiety) and alterations in physiological functions (i.e., inflammation or metabolism) have been well supported. However, studies on a symptom-based approach have provided mixed results. Our study aims to gain insight into how subclinical statuses, featured by elevated depressive and/or anxious symptoms, may influence immunometabolic alterations in the concurrent relationship; and the development of metabolic diseases at 10-year follow-up: diabetes, hypertension and hypercholesterolemia.MethodsData from 758 Greek adults [394 men (aged 41 ± 10 years) and 364 women (aged 37 ± 12 years)] were used. Four groups were created according to the levels of depressive and anxiety symptoms: (1) control group (CG), (2) depressive group (DG), (3) anxiety group (AG) and (4) depressive and anxiety group (DAG). Multi-indicator multi-causes (MIMIC) modeling was used to estimate metabolic function and inflammatory response scores, on a wide selection of blood biomarkers. Finally, a binary logistic regression was carried out to study the influence of symptoms on the development of the aforementioned metabolic diseases on a 10-year follow-up.ResultsGroup membership was not associated with metabolic function score. Conversely, DAG membership was related with higher inflammatory response score (B = 0.20, CI95 = 0.01, 0.40), with respect to the CG (p < 0.05). Both age and sex were significant variables in the calculation of both scores. Regarding disease at 10-year follow-up effect, risk of developing diabetes, hypertension and hypercholesterolemia was associated with age and socioeconomic status. Moreover, DG membership was significant for diabetes risk (OR = 2.08, CI95 = 1.00, 4.22) and DAG for hypercholesterolemia (OR = 1.68, CI95 = 1.16, 2.43).LimitationsData on anti-inflammatory drugs and psychopharmacological medication were not collected in this study.ConclusionsElevated symptoms of depression and anxiety accounts for inflammatory alterations at concurrent relationship and a higher risk of 10-year follow-up metabolic diseases.

Abstract

Many organisms avoid habitats posing risks of parasitism. Parasites are not generally conspicuous, however, which raises the question of what cues individuals use to detect parasitism risk. Here, we provide evidence in alpine newts (Ichthyosaura alpestris) that non-visual cues from parasite-exposed conspecifics inform habitat avoidance. Alpine newts breed in aquatic habitats and occasionally move among adjacent terrestrial habitat during breeding seasons. We completed experiments with newts whereby individuals had access to both habitats, and the aquatic habitats varied in prior occupancy by conspecifics with different histories of exposure to the parasitic skin fungus, Batrachochytrium dendrobatidis (Bd). Continuous filming of newt activity for 2 days provided little evidence that prior use of aquatic habitats by conspecifics, regardless of their Bd exposure history, immediately influenced newt habitat use. However, newts that encountered aquatic habitats used specifically by Bd-exposed conspecifics on day 1 spent less time aquatic on day 2, whereas other newts did not alter habitat use. Responses could have been elicited by cues generated by Bd stages on the conspecifics or, perhaps more likely, cues emitted by the conspecifics themselves. In either case, these observations suggest that newts use non-visual cues sourced from exposed conspecifics to detect Bd risk and that those cues cause newts to avoid aquatic habitats. Bd may therefore influence host behavior in early phases of interactions, and possibly before any contact with infectious stages is made, creating potential for non-consumptive effects.

Abstract

Conflicts between national and local governments over land use in floodplains have been well documented in the US and elsewhere. The US National Flood Insurance Program offers subsidized flood insurance to communities that agree to prevent further development in floodplains, but the requirements are poorly enforced and local governments are commonly reluctant to restrain development on flood-prone lands. In this paper we highlight this problem in particularly sensitive areas: the floodways (or flood bypasses) that are essential components of the Mississippi River flood control system. To properly operate the flood control system, the US Army Corps of Engineers must be able to divert flow from the mainstem Mississippi into the bypasses, thereby lowering stage in the main river, and thus minimizing flooding of cities and other vulnerable areas. However, operation of the Birds-Point-New Madrid Floodway in Missouri was compromised in 2011 by local opposition (and a legal challenge ultimately rejected by the US Supreme Court), and it was finally used to accommodate floodwaters. The West Atchafalaya Floodway in Louisiana experienced a threefold increase in the number of structures within the floodway from about 1970 to 2010. Because of the pattern of flooding, the West Atchafalaya Floodway was not needed in 2011, but if it is needed in the future, its operation may be compromised by the extensive encroachments within the floodway. Thus, operation of critical national infrastructure, designed to deal with floods on an interstate, river-basin scale, is compromised by land-use decisions made at the local level.

BackgroundThe Emerald project's focus is on how to strengthen mental health systems in six low- and middle-income countries (LMICs) (Ethiopia, India, Nepal, Nigeria, South Africa and Uganda). This was done by generating evidence and capacity to enhance health system performance in delivering mental healthcare.A common problem in scaling-up interventions and strengthening mental health programmes in LMICs is how to transfer research evidence, such as the data collected in the Emerald project, into practice.AimsTo describe how core elements of Emerald were implemented and aligned with the ultimate goal of strengthening mental health systems, as well as their short-term impact on practices, policies and programmes in the six partner countries.MethodWe focused on the involvement of policy planners, managers, patients and carers.ResultsOver 5 years of collaboration, the Emerald consortium has provided evidence and tools for the improvement of mental healthcare in the six LMICs involved in the project. We found that the knowledge transfer efforts had an impact on mental health service delivery and policy planning at the sites and countries involved in the project.ConclusionsThis approach may be valid beyond the mental health context, and may be effective for any initiative that aims at implementing evidence-based health policies for health system strengthening.

8. Streptococcus gallolyticus infection in colorectal cancer and association with biological and clinical factors.

Abstract

There is an unambiguous association of Streptococcus gallolyticus infection with colorectal cancer, although there is limited information about epidemiology or interaction between molecular and environmental factors. We performed an original quantitative analysis of S. gallolyticus in unselected colorectal cancer patients (n = 190) and their association with clinical, pathological tumor molecular profiles (microsatellite instability, hypermethylator phenotype and chromosomal instability pathways), and other biological factors in colorectal tumor and normal tissues (cytomegalovirus and Epstein-Barr virus infection). We developed a new quantitative method to assess bacterial load. Analytical validation was reached with a very high sensitivity and specificity. Our results showed a 3.2% prevalence of S. gallolyticus infection in our unselected cohort of colorectal cancer cases (6/190). The average S. gallolyticus copy number was 7,018 (range 44-34,585). No previous reports relating to S. gallolyticus infection have been published for unselected cohorts of patients. Finally, and despite a low prevalence of S. gallolyticus in this study, we were able to define a specific association with tumor tissue (p = 0.03) and with coinfection with Epstein-Barr virus (p = 0.042; OR: 9.49; 95% IC: 1.1-82.9). The prevalence data provided will be very useful in the design of future studies, and will make it possible to estimate the sample size needed to assess precise objectives. In conclusion, our results show a low prevalence of S. gallolyticus infection in unselected colorectal cancer patients and an association of positive S. gallolyticus infection with tumor tissue and Epstein-Barr virus coinfection. Further studies will be needed to definitively assess the prevalence of S. gallolyticus in colorectal cancer and the associated clinicopathological and molecular profiles.

9. Chemosensory age discrimination in the snake Boa constrictor (Serpentes: Boidae)

Abstract

Many snakes are able to use their chemosensory system to detect scent of conspecifics, which is important in many social contexts. Age discrimination based on chemical cues may be especially important to ensure access to sexually mature potential partners. In this study, we used 24 individual Boa constrictor snakes (12 adults mature and 12 non-mature individuals) that had been captured in different areas of Ecuador, and were maintained in captivity at the Vivarium of Quito. We used tongue-flick experiments to examine whether these snakes were able to discriminate between scents from mature and non-mature individuals. Results showed that B. constrictor snakes used chemical cues to recognize conspecifics and that the scent of individuals of different ages elicited chemosensory responses of different magnitudes. The scents from adult conspecifics elicited the quickest and highest chemosensory responses (i.e., short latency times and high tongue-flick rates), although we did not find differential responses to scent of males and females. The magnitude of the responses was lower to scent of sub adult individuals, and then even lower to scent of juvenile snakes, but in all cases the scent of snakes was discriminated from a blank control. We discuss the potential chemical mechanisms that may allow age recognition and its implications for social and sexual behavior of this snake species.Muchas serpientes son capaces de usar su sistema quimiosensorial para detectar el olor de individuos coespecíficos, lo que es importante en muchos contextos sociales. La discriminación de la edad basada en señales químicas puede ser especialmente importante para asegurar el acceso a parejas potenciales que sean sexualmente maduras. En este estudio, usamos 24 individuos de una especie de boa (Boa constrictor) (12 individuos adultos y 12 inmaduros) que habían sido capturados en diferentes partes de Ecuador y eran mantenidos en cautividad el Vivarium de Quito. Usamos experimentos de protusiones linguales para examinar si esta serpiente es capaz de discriminar entre el olor de individuos maduros y no maduros. Los resultados mostraron que B. constrictor usa señales químicas para reconocer co-específicos y que el olor de individuos de distinta edad provoca respuestas quimiosensoriales de diferente magnitud. El olor de individuos adultos provocó las respuestas más rápidas y elevadas (esto es, tiempos de latencia más cortos y tasas más altas de protusiones linguales), aunque no encontramos diferencias en las respuestas a olores de machos y hembras. La magnitud de las respuestas fue más baja a olores de sub adultos, e incluso más baja a olor de juveniles, pero en todos los casos el olor de una serpiente era discriminado de un control no oloroso. Discutimos los posibles mecanismos químicos que pueden permitir esta discriminación de la edad y sus implicaciones para el comportamiento social y sexual de esta serpiente.

RESUMEN: Para López Paz y Pereira el paisaje tiene unos factores determinantes en su formación: la propiedad, el uso y la producción, de forma que sólo a través de su estudio es posible conocer los paisajes individualizados y por lo tanto nunca podremos explicarlos a partir de su simple realidad material y visual. A partir de esos principios proponen la existencia tanto de un paisaje político, en cuanto que dependiente de la sociedad que ha condicionado su configuración, como de un paisaje histórico, resultado del correspondiente proceso diacrónico. ABSTRACT: According to López and Pereira, there are factors that determine the formation of the landscape as property, use and production so that only through is study is possible to know individual landscapes so we will never be able to explain them through their material and visual reality. In conclusion, the autors propose the existence of both, a political landscape, depending from the society that determined its configuration and an historic landscape, as a result of the diacronical process.

Abstract

The dynamics of Colonial Iberian American societies established a true "hierarchy of color" which ended up structuring relationships among individuals and groups. Work relationships were established at the core of such hierarchy through a mechanism that assigned specific activities to specific ethnoracial origins. While well known casta paintings (cuadros de castas) present us with this social order in an official and rigid manner, concrete practices are the site where the social impact of such structuring principle, which involves work differentiation, is revealed. In this sense, I will discuss the principle of "social reversal" as individuals through their relationships are able to either enforce, avoid, or shape such principle. My discussion will develop around two well known aspects of the social fabric involving work and honor: the idea of being served by the work of others, and the idea of giving value to honest work.

Abstract

BACKGROUND: Rett syndrome (RTT) is a complex neurological disorder that is one of the most frequent causes of mental retardation in women. A great landmark in research in this field was the discovery of a relationship between the disease and the presence of mutations in the gene that codes for the methyl-CpG binding protein 2 (MeCP2). Currently, MeCP2 is thought to act as a transcriptional repressor that couples DNA methylation and transcriptional silencing. The present study aimed to identify new target genes regulated by Mecp2 in a mouse model of RTT. METHODOLOGY/PRINCIPAL FINDINGS: We have compared the gene expression profiles of wild type (WT) and Mecp2-null (KO) mice in three regions of the brain (cortex, midbrain, and cerebellum) by using cDNA microarrays. The results obtained were confirmed by quantitative real-time PCR. Subsequent chromatin immunoprecipitation assays revealed seven direct target genes of Mecp2 bound in vivo (Fkbp5, Mobp, Plagl1, Ddc, Mllt2h, Eya2, and S100a9), and three overexpressed genes due to an indirect effect of a lack of Mecp2 (Irak1, Prodh and Dlk1). The regions bound by Mecp2 were always methylated, suggesting the involvement of the methyl-CpG binding domain of the protein in the mechanism of interaction. CONCLUSIONS: We identified new genes that are overexpressed in Mecp2-KO mice and are excellent candidate genes for involvement in various features of the neurological disease. Our results demonstrate new targets of MeCP2 and provide us with a better understanding of the underlying mechanisms of RTT.

Abstract

This research explores different aspects of the historical problematic of labour relationships and of the accusations for laziness against people, during the late eighteenth century in the city of Santa Fe of Bogota (New Grenade). In this regards, it approaches the local and urban reforms of the eighteenth century, (The Burbonic Reforms) crossing different levels of analysis from the general position of the Spanish American Colonies within the set of Spanish possessions, to the particular forms of work of the habitants of the city. The perspective adopted is based on a conception of social interactions centred on practices; tensions, agreements, disagreements that defined through their developments the balances of power. It is particularly focused on the mestizo social dynamic that expresses the contradictions of a hybrid social formation, as well as the consequences of ancient regime colonization.

Abstract

We analyze the out of plane hopping in models of layered systems where the in--plane properties deviate from Landau's theory of a Fermi liquid. We show that the hopping term acquires a non trivial energy dependence, due to the coupling to in plane excitations, and can be either relevant or irrelevant at low energies or temperatures. 32. Grief, continuing bonds, and unreciprocated love

Abstract

The widely accepted “continuing bonds” model of grief tells us that rather than bereavement necessitating the cessation of one's relationship with the deceased, very often the relationship continues instead in an adapted form. However, this framework appears to conflict with philosophical approaches that treat reciprocity or mutuality of some form as central to loving relationships. Seemingly the dead cannot be active participants, rendering it puzzling how we should understand claims about continued relationships with them. In this article, we resolve this tension by highlighting two fundamental aspects of paradigmatic loving relationships that can, and often do, continue in an adapted form following bereavement: love and mutual shaping of interests, choices, and self-concepts. Attention to these continuing features of relationships helps to capture and clarify the phenomenological and behavioral features of continuing bonds. However, love and mutual shaping must also change in important ways following bereavement. Love becomes unreciprocated, and although the dead continue to shape our interests, choices, and self-concepts, we predominantly shape their legacies and memories in return. These changes place important constraints upon the nature of our interpersonal connections with the dead.

Abstract

Aphantasia has been described as the inability to voluntarily evoke mental images using the "mind's eye." We studied a congenital aphantasic subject using neuropsychological testsand 64 channel EEG recordings, in order to studycortical activity involved in perception and imagery evaluating event-related potentials(N170, P200, N250). The subject is in the normal range of the neuropsychological tests performed, except for specific imagery tests. The EEG results show that when he evokes the same mental image, he starts the evoking process from left temporal instead of frontal areas, he does not activate occipital visual nor left anterior parietal areas.

39. Comparación de los factores que influyen en los padres al elegir un odontopediatra según la consulta dental previa de sus hijos en Lima Metropolitana durante el año 2020

Resumen: Las visitas dentales previas tienen un gran impacto en el niño como en sus padres y/o cuidadores para su juicio frente a consultas dentales posteriores. Objetivo: Evaluar si existe diferencias al comparar los factores para elegir un odontopediatra y la consulta dental previa en Lima Metropolitana. Material y métodos: Estudio transversal con una muestra de 294 padres. Se recolectaron los datos sobre la consulta dental previa del niño en odontopediatría entre 0 a 5 años, con categorías de respuesta de Sí y No; los factores para elegir un odontopediatra, con categorías de respuesta de muy importante, muy útil, no importante y no útil. Para ello se utilizó un cuestionario virtual de 29 ítems. Se utilizó la prueba exacta de Fisher y Chi-cuadrado para el análisis bivariado. Resultados: Se encontraron diferencias estadísticamente significativas en 4 de 5 factores tales como, recomendaciones, experiencias previas, tipos de información buscada y logísticas. Por otro lado, se encontró que 50.68% de los niños menores de 5 años sí habían tenido una consulta dental con un especialista, donde el 20.07% habían asistido en el rango de 1 a 3 años y sólo el 12.59% asistió antes del primer año a la consulta dental con el odontopediatra. Conclusiones: Existen diferencias al comparar los factores para elegir un odontopediatra y la consulta dental previa en Lima Metropolitana. Por último, se evidenció que los padres de familia no llevan a sus hijos a su primera visita dental antes del primer año, recomendación indicada por la Asociación dental Americana.

42. Non-harmonious love

Abstract

A common approach in the philosophy of love defines love as caring about one another and promoting one another's interests, aims and values. The view faces several problems and has been re-formulated to avoid them. However, here I argue that a larger re-formulation of the definition of love is needed in order to accommodate three instances of what I call 'non-harmonious' relationships. I identify three types of non-harmonious love (featuring problematic interests, opposing interests and neutral interests the lovers do not care about) and ultimately claim that our definition of love must incorporate conflict and self-interest, and we should then abandon the excessive focus on the sharing of values.

Abstract

Item does not contain fulltext BACKGROUND: Etrolizumab is a gut-targeted anti-β7 integrin monoclonal antibody. In a previous phase 2 induction study, etrolizumab significantly improved clinical remission versus placebo in patients with moderately to severely active ulcerative colitis. We aimed to compare the safety and efficacy of etrolizumab with infliximab in patients with moderately to severely active ulcerative colitis. METHODS: We conducted a randomised, double-blind, double-dummy, parallel-group, phase 3 study (GARDENIA) across 114 treatment centres worldwide. We included adults (age 18-80 years) with moderately to severely active ulcerative colitis (Mayo Clinic total score [MCS] of 6-12 with an endoscopic subscore of ≥2, a rectal bleeding subscore of ≥1, and a stool frequency subscore of ≥1) who were naive to tumour necrosis factor inhibitors. Patients were required to have had an established diagnosis of ulcerative colitis for at least 3 months, corroborated by both clinical and endoscopic evidence, and evidence of disease extending at least 20 cm from the anal verge. Participants were randomly assigned (1:1) to receive subcutaneous etrolizumab 105 mg once every 4 weeks or intravenous infliximab 5 mg/kg at 0, 2, and 6 weeks and every 8 weeks thereafter for 52 weeks. Randomisation was stratified by baseline concomitant treatment with corticosteroids, concomitant treatment with immunosuppressants, and baseline disease activity. All participants and study site personnel were masked to treatment assignment. The primary endpoint was the proportion of patients who had both clinical response at week 10 (MCS ≥3-point decrease and ≥30% reduction from baseline, plus ≥1-point decrease in rectal bleeding subscore or absolute rectal bleeding score of 0 or 1) and clinical remission at week 54 (MCS ≤2, with individual subscores ≤1); efficacy was analysed using a modified intention-to-treat population (all randomised patients who received at least one dose of study drug). GARDENIA was designed to show superiority of etrolizumab over infliximab for the primary endpoint. This trial is registered with ClinicalTrials.gov, NCT02136069, and is now closed to recruitment. FINDINGS: Between Dec 24, 2014, and June 23, 2020, 730 patients were screened for eligibility and 397 were enrolled and randomly assigned to etrolizumab (n=199) or infliximab (n=198). 95 (48%) patients in the etrolizumab group and 103 (52%) in the infliximab group completed the study through week 54. At week 54, 37 (18·6%) of 199 patients in the etrolizumab group and 39 (19·7%) of 198 in the infliximab group met the primary endpoint (adjusted treatment difference -0·9% [95% CI -8·7 to 6·8]; p=0·81). The number of patients reporting one or more adverse events was similar between treatment groups (154 [77%] of 199 in the etrolizumab group and 151 [76%] of 198 in the infliximab group); the most common adverse event in both groups was ulcerative colitis (55 [28%] patients in the etrolizumab group and 43 [22%] in the infliximab group). More patients in the etrolizumab group reported serious adverse events (including serious infections) than did those in the infliximab group (32 [16%] vs 20 [10%]); the most common serious adverse event was ulcerative colitis (12 [6%] and 11 [6%]). There was one death during follow-up, in the infliximab group due to a pulmonary embolism, which was not considered to be related to study treatment. INTERPRETATION: To our knowledge, this trial is the first phase 3 maintenance study in moderately to severely active ulcerative colitis to use infliximab as an active comparator. Although the study did not show statistical superiority for the primary endpoint, etrolizumab performed similarly to infliximab from a clinical viewpoint. FUNDING: F Hoffmann-La Roche.

Abstract

Background The Spanish Melanoma Group (GEM) developed a national registry of patients with melanoma infected by SARS-CoV-2 ( GRAVID ).Methods The main objective was to describe the COVID-19 fatality rate in patients with melanoma throughout the pandemic, as well as to explore the effect of melanoma treatment and tumor stage on the risk of COVID-19 complications. These are the final data of the register, including cases from February 2020 to September 2021.Results One hundred-fifty cases were registered. Median age was 68 years (range 6-95), 61 (40%) patients were females, and 63 (42%) patients had stage IV. Thirty-nine (26%) were on treatment with immunotherapy, and 17 (11%) with BRAF-MEK inhibitors. COVID-19 was resolved in 119 cases, including 85 (57%) patients cured, 15 (10%) that died due to melanoma, and 20 (13%) that died due to COVID-19. Only age over 60 years, cardiovascular disorders, and diabetes mellitus increased the risk of death due to COVID-19, but not advanced melanoma stage nor melanoma systemic therapies. Three waves have been covered by the register: February-May 2020, August-November 2020, and December 2020-April 2021. The first wave had the highest number of registered cases and COVID-19 mortality.Conclusion Tumor stage or melanoma treatments are non-significant prognostic factors for COVID-19 mortality. During the pandemic in Spain there was a downward trend in the number of patients registered across the waves, as well as in the severity of the infection.

Abstract

The presence of abnormalities when the left ventricle is deformed is related to the patients' prognosis after a first myocardial infarction. These deformations can be detected by performing a cardiac magnetic resonance (CMR) study. Currently, late gadolinium enhancement (LGE) is considered to be the gold standard when performing CMR imaging. However, CMR with LGE overestimates infarct size and underestimates recovery of dysfunctional segments after myocardial infarction. Based on this statement, the objective is to detect, characterize, and quantify the extent of myocardial infarction in patients with cardiac pathologies, using parameters derived from CMR, in order to obtain greater precision in patients' recovery predictions than when only studying LGE images. For this purpose, we studied the infarct presence and extension from a total of 105 images from 35 patients, and calculated myocardium strain and torsion to characterize and quantify the affected tissue. A total of twenty-one parameters were selected to create predictive models. Moreover, we compared two feature extraction methods, and the performance of five machine learning algorithms. Results show that both temporal and strain parameters are the most relevant to detect and characterize the extent of myocardial infarction. The use of imaging techniques and machine learning algorithms have great potential and show promising results when it comes to detecting the presence and extent of myocardial infarction. The current study proposes a novel approach to detect, quantify, and characterize cardiac infarction by using strain and torsion parameters from different CMR images and different Machine Learning algorithms. This would potentially overcome LGE, the current state of the art technique, in estimating the extension of damaged tissue and enable an objective diagnosis and clinical decision.

Abstract

The number of studies in the medical field that uses machine learning and deep learning techniques has been increasing in the last years. However, these techniques require a huge amount of data that can be difficult and expensive to obtain. This specially happens with cardiac magnetic resonance (MR) images. One solution to the problem is raise the dataset size by generating synthetic data. Convolutional Variational Autoencoder (CVAe) is a deep learning technique which allows to generate synthetic images, but sometimes the synthetic images can be slightly blurred. We propose the combination of the CVAe technique combined with Style Transfer technique to generate synthetic realistic cardiac MR images. Clinical Relevance-The current work presents a tool to increase in a simple easy and fast way the cardiac magnetic resonance images dataset with which perform machine learning and deep learning studies.
