In den letzten Jahren sind therapierefraktare Verlaufe von Dermatophytosen immer mehr in den Fokus getreten. Haufigste Erreger hierbei sind Trichophyton (T.) rubrum und T. mentagrophytes. Die Behandlung umfasst neben lokaltherapeutischen Masnahmen als Erstlinientherapie Terbinafin, ein Allylamin-Antimykotikum, das durch Inhibition der Squalenepoxidase und somit Storung der Ergosterolsynthese wirkt. Bei therapierefraktaren Verlaufen konnte mittlerweile gehauft eine Terbinafin-Resistenz durch Punktmutation im Gen der Squalenepoxidase nachgewiesen werden. Es erfolgen die Darstellung spezieller Aspekte in der Epidemiologie der Dermatophytosen mit Terbinafin-Resistenz sowie Veranschaulichung anhand von 4 Patienten inklusive praxisrelevanter Aspekte bei der Diagnostik. Es wurden eine Aufarbeitung von Handbuchwissen, eine selektive Literaturrecherche sowie eine Aufarbeitung von 4 Patienten mit Dermatophytosen und einer Onychomykose durch Terbinafin-resistente Isolate von T. rubrum vorgenommen. Der Nachweis der Terbinafin-Resistenz erfolgt durch In-vitro-Testung mittels Breakpoint-Methode sowie Sequenzierung des Trichophyton-Isolates und Nachweis der Punktmutation mit Aminosauresubstitution an Position L393F oder F397L des Gens der Squalenepoxidase. Bei therapierefraktaren und rezidivierenden Dermatophytosen sollte vor allem bei T. mentagrophytes und T. rubrum an eine Terbinafin-Resistenz gedacht werden, und eine In-vitro-Resistenztestung des Dermatophyten sowie eine Punktmutationsanalyse der Squalenepoxidase (SQLE) sollten erfolgen. Therapeutisch wird die intermittierende Gabe von Itraconazol in Kombination mit einer antimykotischen Lokaltherapie empfohlen. Dennoch ist mit einem rezidivierenden Verlauf zu rechnen und in der Regel eine Langzeittherapie mit Itraconazol notwendig.

Poster session 1, September 21, 2022, 12:30 PM - 1:30 PM Objective The epidemic of antifungal-resistant dermatophytosis in India has been reported. These infections are associated with severe morbidity, resistance to oral itraconazole and terbinafine, and the widespread misuse of topical steroids. Trichophyton indotineae has emerged as the predominant causative agent. In this study, we investigated 162 dermatophytosis infections from eight Indian states using genomic sequencing. The primary objective was to determine whether a clonal outbreak strain is responsible for the current epidemic. Methods A total of 161 T. indotineae and one T. rubrum isolates from skin scrapings collected from India in 2017-2019 and previously reported were sent to the U.S. Centers for Disease Control and Prevention (CDC) for genomic analysis. After species identification, genomic DNA was extracted and sequenced using Illumina NovaSeq. Single-nucleotide (SNP) analysis was performed using the portable workflow MycoSNP (v0.21). Briefly, MycoSNP prepared the reference genome, performed pre-processing, aligned sample reads to the reference using the BWA (v 0.7.17) alignment algorithm, and called variants using GATK (v4.1.4.1). High-quality SNPs were used for constructing phylogenetic trees using neighbor-joining (NJ) and maximum likelihood (ML) methods. Further, to understand if infections are genetically clustered by state or region, multi-dimensional scaling (MDS) was applied using the ML tree in R. Result SNP analysis identified 1259 450 variant sites which were used to construct an NJ and ML tree. The tree topology from both NJ and ML methods showed consensus. All 161 T. indotineae isolates from India clustered together forming a large, well-supported clade. SNP differences between the samples varied from 0-160 SNPs. Historical isolates available at CDC were included as controls and clustered over 40 000 SNPs from the clade comprising isolates from India. The MDS plot revealed that isolates did not cluster by state or region. Conclusion Antifungal-resistant dermatophytosis is an emerging threat with cases of chronic, recurrent infection reported from several countries including India. Additionally, the rapid spread of infections involves person-to-person spread. Our results suggest that a clonal outbreak of a T. indotineae strain is circulating in multiple states in India. Current plans are to expand the geographic scope of the study by including over 10 countries from Europe, Middle East, and the Americas. This work will allow the public health community to better understand the emergence and transmission of antifungal-resistant dermatophytosis worldwide.

Wir berichten uber eine Terbinafin-resistente Tinea corporis bei einem Reiseruckkehrer aus Asien. Der erstmals in Indien isolierte Genotyp VIII von Trichophyton mentagrophytes hat in den vergangenen Jahren als Erreger der Tinea corporis und Tinea cruris zugenommen und aufgrund seiner haufigen Resistenz gegenuber Terbinafin an Relevanz gewonnen. Insbesondere nach Reisen nach Asien sollte daher bei therapierefraktarer Tinea die systemische Therapie auf Itraconazol umgestellt werden. Mit Itraconazol in veranderter Darreichung kann eine erhohte Bioverfugbarkeit bei niedrigerer Dosierung erreicht werden.

Trichophyton(T.)mentagrophytesnow accounts for an overwhelming majority of clinical cases in India, a new “Indian genotype” (T. mentagrophytes ITS genotype VIII)having been isolated from skin samples obtained from cases across a wide geographical distribution in this country. The conventional diagnostic methods, like fungal culture, are, however, inadequate for diagnosing this agent. Thus, molecular methods of diagnosis are necessary for proper characterization of the causative agent. The shift in the predominant agent of dermatophytosis fromT. rubrumtoT. mentagrophytes, within a relatively short span of time, is without historic parallel. The apparent ease of transmission of a zoophilic fungus among human hosts can also be explained by means of mycological phenomena, like anthropization.

Trichophyton (T.) tonsurans gilt bei Kontakt- und Kampfsportarten weltweit als Haupterreger der Tinea gladiatorum und fuhrt regelmasig zu Ausbruchen. In Leipzig treten im nationalen Ringerkader seit uber 2 Jahren bis heute gehauft und rezidivierend Dermatophytosen bei Kindern und Jugendlichen auf. Die Ringer wurden in der Hautarztpraxis zur klinischen Untersuchung und Probeentnahme vorstellig. Hautschuppen und Haarwurzeln sowie Abstriche wurden entnommen und mykologisch mit fluoreszenzoptischem Praparat, Pilzkultur und Polymerasekettenreaktion (PCR) auf Dermatophyten-DNA (Desoxyribonukleinsaure) untersucht. Die Sequenzierung der r(ribosomale)DNA der Dermatophyten diente als Kulturbestatigungstest. Umgebungsuntersuchungen in der Ringerhalle umfassten Abklatschkulturen und Abstriche von Oberflachen, insbesondere von den Matten. T. tonsurans war bei 21 von 25 Kindern und Jugendlichen sowie 1 Trainer kulturell und/oder mit PCR nachweisbar. In 1 von 10 Abklatschkulturen von Matten und Fusboden wuchs T. tonsurans, eine weitere Kultur wies T. interdigitale nach. Abstriche von den Matten ergaben 2‑mal einen kulturellen T.-tonsurans-Nachweis, 3‑mal war die PCR auf T. tonsurans positiv. Auf den Pilznahrmedien entwickelte T. tonsurans innerhalb von 14 Tagen kleine, flache, ausstrahlende, granulare und weis gefarbte Kolonien mit mahagonibrauner Ruckseite. Die Sequenzierung der Internal-Transcribed-Spacer(ITS)-Region der rDNA sowie des TEF1‑α(„translation elongation factor 1 α“)-Gens bestatigte die Spezies T. tonsurans in allen Fallen. T. interdigitale, isoliert von einer Matte, wurde ebenfalls mit Sequenzierung identifiziert. Acht T.-tonsurans-Stamme wurden einer In-vitro-Empfindlichkeitstestung gegen Terbinafin unterzogen. Alle Isolate waren mit minimalen Hemmkonzentrationen von ≤ 0,1 µg/ml in vitro empfindlich gegen Terbinafin.

A severe outbreak of highly virulent and multi-resistant dermatophytosis by species in the Trichophyton mentagrophytes/T. interdigitale complex is ongoing in India. The correct identity of the etiologic agent is a much-debated issue. In order to define species limits, a taxonomic study was undertaken combining molecular, morphological, and physiological characteristics as evidence of classification. Molecular characteristics show that T. mentagrophytes s. str. and T. interdigitale s. str. can be distinguished with difficulty from each other, but are unambiguously different from the Indian genotype, T. indotineae by sequences of the HMG gene. The entities were confirmed by multilocus analysis using tanglegrams. Phenotypic characters of morphology and physiology are not diagnostic, but statistically significant differences are observed between the molecular siblings. These properties may be drivers of separate evolutionary trends. Trichophyton mentagrophytes represents the ancestral, homothallic cloud of genotypes with a probable geophilic lifestyle, while T. indotineae and T. interdigitale behave as anthropophilic, clonal offshoots. The origin of T. indotineae, which currently causes a significant public health problem, is zoonotic, and its emergence is likely due to widespread misuse of antifungals.

Dermatophytosis has attained unprecedented dimensions in recent years in India. Its clinical presentation is now multifarious, often with atypical morphology, severe forms and unusually extensive disease in all age groups. We hesitate to call it an epidemic owing to the lack of population-based prevalence surveys. In this part of the review, we discuss the epidemiology and clinical features of this contemporary problem. While the epidemiology is marked by a stark increase in the number of chronic, relapsing and recurrent cases, the clinical distribution is marked by a disproportionate rise in the number of cases with tinea corporis and cruris, cases presenting with the involvement of extensive areas, and tinea faciei.

Background The Indian ITS genotype VIII T. mentagrophytes population shows a high amount of different erg1 (ergosterol) mutants encoding for squalene epoxidase, which catalyzes the first step of ergosterol biosynthesis. Objectives Illumination of the implication of point mutations at position Ala448Thr in single and double erg1 T. mentagrophytes mutants because mutants of this type were abundantly found within the Indian fungal population. Methods Growth in fluconazole or terbinafine containing medium was analyzed using a microplate-laser-nephelometry (MLN)-based growth assay. Results Ala 448 Thr erg1 single mutants were terbinafine sensitive but about 50% of isolates showed an increased fluconazole resistance, whereas 95% of the double mutants (Phe 397 Leu, Ala 448Thr) demonstrated combined terbinafine and increased fluconazole resistance. Conclusion The new Indian T. mentagrophytes populations show several point mutations in erg1. Point mutations at position 397 were previously described and cause terbinafine resistance. A large part of the double mutants exhibit resistance to terbinafine and fluconazole, demonstrating a selective advantage of the combination of both mutations.

Species of the Trichophyton benhamiae complex are predominantly zoophilic pathogens with a worldwide distribution. These pathogens have recently become important due to their epidemic spread in pets and pet owners. Considerable genetic and phenotypic variability has been revealed in these emerging pathogens, but the species limits and host spectra have not been clearly elucidated. In this study, we used an approach combining phylogenetic analysis based on four loci, population-genetic data, phenotypic and physiological analysis, mating type gene characterization and ecological data to resolve the taxonomy of these pathogens. This approach supported the inclusion of nine taxa in the complex, including three new species and one new variety. Trichophyton benhamiae var. luteum var. nov. (“yellow phenotype” strains) is currently a major cause of zoonotic tinea corporis and capitis in Europe (mostly transmitted from guinea pigs). The isolates of the “white phenotype” do not form a monophyletic group and are segregated into three taxa, T. benhamiae var. benhamiae (mostly North America; dogs), T. europaeum sp. nov. (mostly Europe; guinea pigs), and T. japonicum sp. nov. (predominant in East Asia but also found in Europe; rabbits and guinea pigs). The new species T. africanum sp. nov. is proposed for the “African” race of T. benhamiae. The introduction to new geographic areas and host jump followed by extinction of one mating type gene have played important roles in the evolution of these pathogens. Due to considerable phenotypic similarity of many dermatophytes and phenomena such as incomplete lineage sorting or occasional hybridization and introgression, we demonstrate the need to follow polyphasic approach in species delimitation. Neutrally evolving and noncoding DNA regions showed significantly higher discriminatory power compared to conventional protein-coding loci. Diagnostic options for species identification in practice based on molecular markers, phenotype and MALDI-TOF spectra are presented. A microsatellite typing scheme developed in this study is a powerful tool for the epidemiological surveillance of these emerging pathogens.

Eine bullose Manifestation einer Tinea corporis ist selten und wird in der Regel durch zoophile Dermatophyten verursacht. Eine 20-jahrige Patientin stellte sich ambulant mit einer schmerzhaften, juckenden, bullosen Hautveranderung am linken Unterarm vor. Klinisch zeigte sich eine etwa 4 × 3 cm durchmessende, scharf begrenzte symmetrische erythematose Plaque mit peripherer Blaschenbildung und serosen Krusten. Histologisch stellte sich eine ausgepragte spongiotische Dermatitis mit Nachweis von Pilzelementen dar. Mikrobiologisch wurde mittels Kultur, Polymerasekettenreaktion (PCR) und Sequenzierung Trichophyton (T.) tonsurans nachgewiesen. Unseres Wissens nach handelt es sich hier um den ersten publizierten Fall einer bullosen Tinea corporis durch T. tonsurans.

ZusammenfassungDie Kälberflechte ist eine durch Tiere übertragene Pilzinfektion, die berufsbedingt auftreten und nach Nr. 3102 BKV (Berufskrankheiten-Verordnung) als Berufskrankheit anerkannt werden kann. Die durch Trichophyton verrucosum ausgelöste Zoonose zeichnet sich häufig durch einen schweren klinischen Verlauf aus, der nicht selten als bakterielle Infektion fehlgedeutet und primär antibiotisch behandelt wird. Die Gewinnung und mykologische Untersuchung von Schuppenmaterial ist diagnostisch entscheidend; auch eine Biopsie kann wegweisend sein. Die orale Therapie erfolgt leitliniengemäß mit Terbinafin. Zudem ist zum Schutz vor Reinfektionen auf besondere Hygienemaßnahmen in Ställen zu achten.

Three boys from the same city, treated by the same dermatologist, developed tinea capitis. Two of them, 4 and 8 years old, underwent mycological diagnostic workup. However, no pathogens familiar in this country, such as Microsporum (M.) canis or Trichophyton (T.) tonsurans, were isolated, but instead that of a dermatophyte that has not been found in Germany for decades. Both dermatophyte isolates showed white-beige-brownish colonies with a flat, radiating edge and a central, verrucous curvature. The sequencing of the internal transcribed spacer (ITS) region of the rDNA confirmed the suspicion of M. ferrugineum already expressed based on the morphological picture. The anthropophilic dermatophyte occurs in the Middle East, Asia, Eastern Europe and Africa and is considered to be the cause of tinea capitis or tinea corporis in children and adolescents. In 2016, M. ferrugineum has again been isolated in Germany, probably as a result of migration movements. The fungus is strikingly isolated to martial arts, especially wrestlers. It mainly affects children and adolescents, some with a Russian-German background. The anthropophilic dermatophyte is transmitted directly from person to person, especially in the case of tinea capitis. An indirect transmission, for example, via mats in martial arts is likely.

Onychomycoses are difficult-to-treat infections of the nail plate with a high recurrence rate. Clinical studies show an increase of the complete cure rate (mycological plus clinical) of severe onychomycoses for documented dermatophytic onychomycoses with combination therapies of locally applied and orally administered antifungals. The best studied regimens involve continuous daily use of terbinafine and weekly application of amorolfine nail lacquer. A topical monotherapy with a nail varnish may be considered especially for superficial white onychomycosis or distal subungual onychomycosis with a lower percentage of affected nail area without involvement of the nail matrix. Repeated mechanical removal of infected nail material and hygienic measures support the efficacy of the treatment. High long-term adherence is critical for optimal success rates. The treatment should be continued until resolution of signs and symptoms. Subsequent long-term secondary prophylaxis with an antifungal nail lacquer is recommended., Onychomykosen sind schwer behandelbare Pilzinfektionen des Nagelorgans mit einer hohen Rezidivrate. Klinische Studien zeigen, dass bei gesicherter schwerer Nagelmykose durch Dermatophyten eine Steigerung der kompletten Heilungsrate (mykologisch plus klinisch) durch eine Kombinationsbehandlung mit lokal applizierten und oral verabreichten Antimykotika im Vergleich zur systemischen Monotherapie erreicht werden kann. Am besten untersucht sind Regime mit kontinuierlich täglicher Einnahme von Terbinafin und wöchentlicher Applikation eines Amorolfin-haltigen Nagellacks. Eine topische Monotherapie mit einem Nagellack kommt v. a. bei einer superfiziellen weißen Onychomykose (SWO) oder einer distal-subungualen Onychomykose (DSO) mit einem geringeren Befallsgrad ohne Beteiligung der Matrix infrage. Die mechanische Entfernung des infizierten Nagelmaterials und hygienische Maßnahmen unterstützen die Therapie. Kritisch für die Erfolgsaussichten ist eine hohe Langzeitadhärenz der Patienten. Die Therapie sollte bis zum Auftreten der Erscheinungsfreiheit andauern. Anschließend empfiehlt sich eine dauerhafte Rezidivprophylaxe mit antimykotischem Nagellack.

Tinea capitis describes a dermatophyte infection of scalp and hair that predominately occurs in children. The diagnostic workup includes microscopic examination, culture and/or molecular tests. Treatment is guided by the specific organism involved and should consist of systemic agents as well as adjuvant topical treatment. The aim of the present update of the interdisciplinary German S1 guidelines is to provide dermatologists, pediatricians and general practitioners with a decision tool for selecting and implementing appropriate diagnostic and therapeutic measures in patients with tinea capitis. The guidelines were developed based on current international guidelines, in particular the 2010 European Society for Pediatric Dermatology guidelines and the 2014 British Association of Dermatologists guidelines, as well as on a review of the literature conducted by the guideline committee. This multidisciplinary committee consists of representatives from the German Society of Dermatology (DDG), the German-Speaking Mycological Society (DMykG), the German Society for Hygiene and Microbiology (DGHM), the German Society of Pediatric and Adolescent Medicine (DGKJ) and the German Society for Pediatric Infectious Diseases (DGPI). The Division of Evidence-based Medicine (dEBM) provided methodological assistance. The guidelines were approved by the participating medical societies following a comprehensive internal and external review., Die Tinea capitis ist eine durch Dermatophyten hervorgerufene, vor allem im Kindesalter auftretende Mykose der behaarten Kopfhaut. Die Diagnostik erfolgt mittels mikroskopischer, kultureller und/oder molekularer Methoden. Die Therapie sollte systemisch und begleitend topisch erfolgen und ist abhängig von dem jeweiligen Erreger. Das Ziel des Updates dieser interdisziplinären deutschen S1-Leitlinie ist es, vor allem Dermatologen, Kinderärzten und Allgemeinmedizinern eine Entscheidungshilfe für die Auswahl sowie Durchführung einer geeigneten und suffizienten Diagnostik und Therapie für Patienten mit Tinea capitis zur Verfügung zu stellen. Die Leitlinie wurde unter Berücksichtigung aktueller internationaler Leitlinien, insbesondere der Leitlinien der European Society for Pediatric Dermatology 2010 sowie der British Association of Dermatologists 2014 und den Ergebnissen einer Literaturrecherche durch die Leitlinienkommission erstellt. Diese Kommission ist multidisziplinär und besteht aus Vertretern der Deutschen Dermatologischen Gesellschaft (DDG), der Deutschsprachigen Mykologischen Gesellschaft (DMykG), der Deutschen Gesellschaft für Hygiene und Mikrobiologie (DGHM), der Deutschen Gesellschaft für Kinder- und Jugendmedizin e.V. (DGKJ) und der deutschen Gesellschaft für Pädiatrische Infektiologie (DGPI). Methodisch wurde die Leitliniengruppe durch die Division of Evidence-based Medicine (dEBM) begleitet. Die Leitlinie wurde nach einem umfangreichen internen und externen Review durch die beteiligten Fachgesellschaften freigegeben.

Background: During the past decade, a prolonged and serious outbreak of dermatophytosis due to a terbinafine-resistant novel species in the Trichophyton mentagrophytes / T. interdigitale complex is ongoing in India, and it spreads to several European countries.Objective: To investigate the molecular background of the squalene epoxidase (SQLE) gene in order to understand the risk of emergence and spread of multi-resistance in dermatophytes.Methods: Antifungal susceptibility for fluconazole, griseofulvin, itraconazole, ketoconazole, miconazole, naftifine, sertaconazole, and terbinafine was tested in 135 isolates from India, China, Australia, Germany and The Netherlands. Based on the latest taxonomic insights, strains were identified as three species: T. mentagrophytes s. str (n=35), T. indotineae (n=64 representing the Indian clone) and T. interdigitale s. str. (n=36).Results: High minimum inhibitory concentrations (MICs) of terbinafine (>16 mg/L) were found in 34 (53%) T. indotineae isolates. These isolates showed an amino acid substitution in the 397th position of the SQLE gene. Elevated MICs of terbinafine (0.5 mg/L) were noted in 2 (3%) T. indotineae isolates; these isolates lead to Phe415Val and Leu393Ser of the SQLE gene. Stability of the effect of the mutations was proven by serial transfer on drug-free medium. Substitutions of Lys276Asn and Leu419Phe were found in susceptible T. mentagrophytes strains. The double mutant Phe377Leu/Ala448Thr showed higher MIC values for triazoles.Conclusions: High MICs of terbinafine are as yet limited to T. indotineae, and are unlikely to be distributed through the T. mentagrophytes species complex by genetic exchange.

This review focuses on nail changes that do not belong to the group of benign or malignant nail tumors. These common afflictions of the nail include structural changes in and under the nail plate as well as inflammation around and in the nail bed. They include onychomycoses, nail psoriasis, onychodystrophies, subungual hematoma, paronychia, ingrown nails and pincer nails. Due to the peculiar anatomy and physiological growth conditions of the nail, the pathology does not necessarily stem from the site of the clinical problem and calls for careful inspection and interpretation.

Nadifloxacin (OPC-7251) represents an established antimicrobial agent belonging to the quinolone group and was developed exclusively for topical administration. Nadifloxacin is effective in treating a variety of bacterial skin infections and acne. Here, the in vitro activity of nadifloxacin was assessed and compared with those of erythromycin, clindamycin and tetracycline against Cutibacterium (C.) acnes, formerly Propionibacterium acnes, to gain a picture of the resistance situation in Germany. The study was approved by the Ethics Committee of the Bayerische Landesarztekammer (Munich, Germany; EC-No.17088) and registered at Deutsches Register Klinische Studien (ID:DRKS00014231).

Eumyzetome stellen eine chronisch eitrige, mit einer granulomatosen Entzundungsreaktion einhergehende Infektion durch (Schimmel-)Pilze dar, die das Haut- und Subkutangewebe erfassen. Eine Affektion tiefer liegender Strukturen ist moglich und kann in einer Amputation munden. Zumeist ist die distale untere Extremitat betroffen. Das klinisch weitestgehend kongruent verlaufende Aktinomyzetom wird durch grampositive Strahlenbakterien verursacht. Beide Erkrankungen werden als Myzetom subsumiert und sind von der Weltgesundheitsorganisation als „neglected tropical diseases“ eingestuft worden. Eumyzetome treten endemisch im „Trans-African Belt“ auf. Pathophysiologisch besteht eine Inokulation der jeweiligen, teils ubiquitar vorkommenden Erreger in die Haut durch Mikrotraumen beim Barfuslaufen. In der Histologie finden sich Drusen, die mit Mikrokolonien des Erregers in vivo korrespondieren. Neben der Kultur ist eine molekulare Diagnostik anzustreben. Eine Bildgebung ist zumeist vor groseren operativen Eingriffen notwendig. Die Behandlung stellt sich schwierig und langwierig dar und erfolgt in erster Linie mit systemischen Antimykotika in Kombination mit einem operativen Vorgehen. Itraconazol gilt weiterhin als der Goldstandard. In refraktaren Fallen kann als Zweitlinientherapie Terbinafin eingesetzt werden. Das Tragen von festem Schuhwerk stellt eine effektive Prophylaxe dar. Obwohl vermeidbar und behandelbar, tritt das Eumyzetom als Erkrankung der Armen weiterhin endemisch auf und ist mit einer nicht unerheblichen Morbiditat und einer soziookonomischen Belastung vergesellschaftet. In unserem Fall wurde erstmals Fusarium chlamydosporum als Ausloser eines Eumyzetoms bei einem Patienten aus dem Sudan nachgewiesen. Die Behandlung mit Terbinafin p.o. uber 1,5 Jahre war erfolgreich.

Ein Realtime-PCR(Polymerasekettenreaktion)-Assay sowie ein Microarray zum direkten Nachweis von Dermatophyten wurden hinsichtlich ihrer Spezifitat zur Dermatophytenidentifizierung untersucht. Es handelte sich bei den kommerziell erhaltlichen Testsystemen um ein Microarray-basiertes PCR-Testverfahren (EUROArray Dermatomycosis, Euroimmun, Lubeck, Deutschland) sowie einen Realtime-PCR-Test (DermaGenius® 2.0, PathoNostics BV, Maastricht, Niederlande). Der Microarray erlaubt den Direktnachweis von 50 Dermatophyten mit eindeutiger Identifizierung von 23 Dermatophyten und 6 Hefen/Schimmelpilzen in einem Ansatz. Der Realtime-PCR-Test kann insgesamt 11 Dermatophyten und einen Hefepilz auf Speziesebene identifizieren. Hier wurden 34 Dermatophytenkulturen mit dem EUROArray Dermatomycosis untersucht. Mittels EUROArray wurden 22 von 24 Dermatophytenspezies korrekt identifiziert. Mittels Realtime-PCR liesen sich 9 von 11 im Testsystem vorgesehene verschiedene Dermatophyten richtig identifizieren. Beide molekularen Teste zum Erregernachweis in der Dermatomykologie sind praxistauglich. Der Realtime-PCR-Test hat ein nicht so umfassendes Spektrum und ist weniger spezifisch. Dafur ist der Test sehr schnell in der Durchfuhrung, da kein Post-PCR-Schritt notig ist. Der Test erkennt die haufigsten Dermatophyten wie Trichophyton (T.) rubrum, T. interdigitale, T. benhamiae (inklusive T. erinacei) und Microsporum (M.) canis zuverlassig. Die Durchfuhrung des Microarray ist aufwendiger infolge der Hybridisierung, dafur ist er deutlich spezifischer und das Erregerspektrum wesentlich umfangreicher. Mittels Microarray werden nicht nur die haufigsten, sondern auch seltene humanpathogene Dermatophytenspezies nachgewiesen.

Ein 6 Monate alter weiblicher Saugling aus Bahrain litt an einer ausgepragten Dermatomykose des gesamten Integuments. Die topische Behandlung mit Terbinafin uber 2 Monate fuhrte zu keinerlei Besserung. An der Dermatophytose erkrankte erwachsene Familienmitglieder und Kinder wurden im Heimatland ebenfalls ohne Erfolg topisch und auch systemisch – mit Voriconazol – antimykotisch behandelt. Die mykologische Diagnostik wahrend eines Urlaubsaufenthaltes in Deutschland erbrachte den Nachweis des zoophilen Dermatophyten Trichophyton (T.) mentagrophytes. Mittels Sequenzierung der „internal transcribed spacer“(ITS)-Region der rDNA (ribosomale Desoxyribonukleinsaure) des Pilzes lies sich der neu beschriebene Genotyp VIII innerhalb der Spezies T. mentagrophytes identifizieren. Dieser Genotyp von T. mentagrophytes ist der Haupterreger der aktuellen Epidemie von chronischen therapierefraktaren Dermatophytosen in Indien. Von einer Ubertragung dieses indischen Dermatophytengenotyps in andere Lander im Rahmen der Globalisierung muss ausgegangen werden. Ein wesentlicher Teil dieser indischen T.-mentagrophytes-Stamme ist zudem in vitro und genetisch – Punktmutationen der Squalenepoxidase (SQLE) – resistent gegenuber Terbinafin. Fur das hier untersuchte T.-mentagrophytes-Isolat aus Bahrain bestatigte sich in der SQLE-Mutationsanalyse der Nachweis der Punktmutation TTC/TTA, die fur die F397L-Aminosauresubstitution des Enzyms steht. Der Stamm war zudem in vitro resistent gegen Terbinafin. Diese Punktmutation ist im hohen Mase mit einer In-vitro-Resistenz des Dermatophyten gegenuber Terbinafin assoziiert. Das Madchen wurde erfolgreich rein topisch mit Miconazol, dann auch im Wechsel mit Ciclopiroxolamin behandelt. Es handelt sich hier um die erste Beschreibung einer Infektion durch einen Terbinafin-resistenten T.-mentagrophytes-Stamm vom ITS-Genotyp VIII aus Indien in Deutschland.

Bei der Tinea barbae handelt es sich um eine seltene Dermatomykose, die per definitionem follikular gebunden im Bartbereich erwachsener Manner auftritt. Der Hautbefund beginnt meist mit Erythemen mit Schuppung. Bei Progredienz oder Ausbreitung an Terminalhaaren in die Tiefe kommt es zu Follikulitiden mit Pustel- und Knotenbildung sowie Abszedierungen mit fest anhaftenden gelblichen Krusten, begleitet von einer lokoregionaren Lymphknotenschwellung und gelegentlich vermindertem Allgemeinzustand oder (sub)febrilen Temperaturen. Dies fuhrt haufig zunachst zur Verdachtsdiagnose einer bakteriellen Folliculitis barbae oder Impetigo contagiosa. Die Tinea barbae wird meist durch Spezies der Gattung Trichophyton (T.) ausgelost. Die Erreger sind vielfaltig und gehoren uberwiegend zu den zoophilen, nur gelegentlich zu anthropophilen und selten zu den geophilen Dermatophyten. Mithilfe der gezielten Anamnese und Diagnostik in Form von mykologischen Untersuchungen, Histologie und molekularbiologischem Dermatophytennachweis mittels Polymerasekettenreaktion kann heute schnell die Diagnose einer Tinea barbae, bei unserem Patienten ausgelost durch T. mentagrophytes, gestellt werden. Bei zeitnaher und ausreichend langer Therapie kommt es zur vollstandigen Restitutio ad integrum.

Trichophyton benhamiae is an emerging zoonotic dermatophyte. We present a case of a small animal stock infected with two Trichophyton species. T. benhamiae was isolated from 15 out of 26 (58%) guinea pigs including two morphologically different phenotypes. Eight guinea pigs were infected with T. benhamiae and T. mentagrophytes simultaneously. The animals showed alopecia and crusts or no clinical signs at all. Trichophyton (T.) tonsurans is considered as the main causative agent of tinea gladiatorum (ringworm) in contact and martial arts worldwide and regularly leads to outbreaks. In the national wrestling squad in Leipzig, dermatophytoses occurred frequently and recurrently in children and adolescents for over a 2-year period. The wrestlers came to the dermatologist's office for clinical examination and sampling. Dermal scales and hair roots as well as smears were examined mycologically with fluorescence optical preparation, fungal culture, and polymerase chain reaction (PCR) for dermatophyte DNA. Sequencing of the dermatophyte rDNA served as culture confirmation test. Environmental investigations in the wrestler training center included contact cultures and smears from surfaces, in particular from the mats. T. tonsurans was culturally and/or with PCR detectable in 21 out of 25 children and adolescents plus one trainer. T. tonsurans grew in one of ten contact cultures of mats and floors in the wrestling training center, and T. interdigitale was found in another culture. Smears from the mats resulted in a culture of T. tonsurans detection twice. The PCR was positive for T. tonsurans three times. Within 14 days, T. tonsurans developed small, flat, radiating, granular and white-colored colonies with a mahogany-brown reverse side on the fungal culture media. The sequencing of the internal transcribed spacer (ITS) region of the rDNA and the translation elongation factor 1 α (TEF 1 α) gene confirmed the species T. tonsurans in all cases. T. interdigitale that was found from a mat was also identified by sequencing. Eight T. tonsurans strains were subjected to in vitro susceptibility testing to terbinafine. All isolates were sensitive to terbinafine in vitro with minimal inhibitory concentrations of ≤ 0.1 µg/ml.Trichophyton (T.) tonsurans gilt bei Kontakt- und Kampfsportarten weltweit als Haupterreger der Tinea gladiatorum und führt regelmäßig zu Ausbrüchen. In Leipzig treten im nationalen Ringerkader seit über 2 Jahren bis heute gehäuft und rezidivierend Dermatophytosen bei Kindern und Jugendlichen auf. Die Ringer wurden in der Hautarztpraxis zur klinischen Untersuchung und Probeentnahme vorstellig. Hautschuppen und Haarwurzeln sowie Abstriche wurden entnommen und mykologisch mit fluoreszenzoptischem Präparat, Pilzkultur und Polymerasekettenreaktion (PCR) auf Dermatophyten-DNA (Desoxyribonukleinsäure) untersucht. Die Sequenzierung der r(ribosomale)DNA der Dermatophyten diente als Kulturbestätigungstest. Umgebungsuntersuchungen in der Ringerhalle umfassten Abklatschkulturen und Abstriche von Oberflächen, insbesondere von den Matten. T. tonsurans war bei 21 von 25 Kindern und Jugendlichen sowie 1 Trainer kulturell und/oder mit PCR nachweisbar. In 1 von 10 Abklatschkulturen von Matten und Fußböden wuchs T. tonsurans, eine weitere Kultur wies T. interdigitale nach. Abstriche von den Matten ergaben 2‑mal einen kulturellen T.-tonsurans-Nachweis, 3‑mal war die PCR auf T. tonsurans positiv. Auf den Pilznährmedien entwickelte T. tonsurans innerhalb von 14 Tagen kleine, flache, ausstrahlende, granuläre und weiß gefärbte Kolonien mit mahagonibrauner Rückseite. Die Sequenzierung der Internal-Transcribed-Spacer(ITS)-Region der rDNA sowie des TEF1‑α(„translation elongation factor 1 α“)-Gens bestätigte die Spezies T. tonsurans in allen Fällen. T. interdigitale, isoliert von einer Matte, wurde ebenfalls mit Sequenzierung identifiziert. Acht T.-tonsurans-Stämme wurden einer In-vitro-Empfindlichkeitstestung gegen Terbinafin unterzogen. Alle Isolate waren mit minimalen Hemmkonzentrationen von ≤ 0,1 µg/ml in vitro empfindlich gegen Terbinafin.

Background: Dermatophytosis is a world-wide distributed common infection. Antifungal drug resistance in dermatophytosis used to be rare, but unfortunately the current Indian epidemic of atypical widespread recalcitrant and terbinafine-resistant dermatophytosis is spreading and has sporadically been reported in Europe. Objectives: To explore the occurrence of clinical and mycological proven antifungal drug resistance in dermatophytes in Europe. Methods: A standardized questionnaire was distributed through the EADV Task Force of Mycology network to dermatologists in Europe. Results: Representatives from 20 countries completed the questionnaires of which 17 (85 %) had observed clinical and/or mycological confirmed antifungal resistance, two countries published cases of antifungal resistance and one country had no known cases. Conclusions: This pilot study confirms that both clinical and mycological antifungal resistance exist in Europe.
