RIVM rapport:Tussen 1984 en 2006 hebben werknemers van Defensie op de zogenoemde POMS(Prepositioned Organizational Materiel Storage)-locaties (Prepositioned Organizational Materiel Storage) in Nederland Amerikaans materieel onderhouden. Hierbij kon chroom-6 vrijkomen, dat vooral in de grondverf van het materieel zat. Chroom-6 heeft schadelijke eigenschappen waardoor mensen ziek kunnen worden als ze eraan worden blootgesteld. De kans om ziek te worden is groter naarmate iemand meer, vaker of langer aan chroom-6 is blootgesteld. In 2018 bracht het RIVM een overzicht uit van de schadelijke gezondheidseffecten en ziekten die kunnen worden veroorzaakt als mensen tijdens hun werk aan chroom-6 blootstaan. Dit overzicht is in 2020 en 2021 geactualiseerd om nieuwe wetenschappelijke kennis mee te nemen. Dit is de derde actualisatie van het overzicht. De kwaliteit en inhoud van de nieuwe studies is met externe experts geëvalueerd. In deze actualisatie worden de eerdere conclusies bevestigd. Er zijn geen nieuwe schadelijke gezondheidseffecten of ziekten in relatie tot blootstelling aan chroom-6 aangetoond. Beroepsmatige blootstelling aan chroom-6 kan de volgende ziekten veroorzaken: longkanker, neus- en neusbijholtekanker, chronische longziekten, chroom-6-gerelateerde allergische astma en rhinitis, allergisch contacteczeem en perforatie van het neustussenschot door chroomzweren. Beroepsmatige blootstelling aan chroom-6 wordt ervan verdacht strottenhoofdkanker en maagkanker te kunnen veroorzaken (wat betekent: er is beperkt wetenschappelijk bewijs). Chromium-6 is suspected to cause laryngeal cancer and stomach cancer in humans (which means: there is limited evidence). RIVM rapport:On behalf of the Ministry of Health a survey was prepared of possibilities for setting up a 'post-marketing surveillance' (PMS)-system for chemical substances in consumer products, including food. With such a system, (long-term) health effects of substances which are already on the market may be identified. Setting up such a system was advised by the Health Council of The Netherlands (GR) based on a report from 2014. In this report, GR concluded that PMS of chemicals is warranted given that certain health effects in humans may not become visible in animal studies used for risk assessment. First, a preliminary overview of current PMS related activities in different areas of legislation, such as food, drugs and consumer products, was prepared. Existing experience in different legislative frameworks can feed into a new system. Relevant national stakeholders have contributed to the overview, and have discussed short and long-term opportunities for further development of PMS. Possible short-term options included sharing existing information, to connect different existing databases and to share new information about health effects of chemicals on an ad hoc basis. For the longer term, exposure assessment directly in humans or for example in blood samples stored in biobanks were considered. In addition, more extensive registration of the use of compounds in consumer products was advised, as well as international harmonization of PMS data collection. Voor de korte termijn raden zij aan kennis uit verschillende kaders met elkaar te delen, bestaande databases aan elkaar te koppelen en elkaar te informeren over effecten van chemische stoffen die ad hoc worden gesignaleerd. Voor de toekomst wordt meer aansluiting gezocht bij metingen van blootstelling direct in de mens, dan wel via metingen in bijvoorbeeld bloedmonsters die in biobanken zijn opgeslagen. Ook wordt een intensievere registratie van het gebruik van stoffen in consumentenproducten voorgesteld, en een methode om PMS-data internationaal op uniforme manier te verzamelen. RIVM rapport:De Europese Commissie heeft concept criteria opgesteld waarmee kan worden bepaald of stoffen hormoonverstorend zijn. Het blijkt dat binnen de huidige wet- en regelgeving veelal de gegevens ontbreken om stoffen aan deze criteria te toetsen. Om aan te tonen of een stof hormoonverstorend is, moet hij een nadelig (of schadelijk) effect veroorzaken, zoals een verminderde vruchtbaarheid of mogelijke verstoring van het zich ontwikkelende zenuwstelsel. Vervolgens moet worden aangetoond dat het schadelijke effect een biologisch verklaarbaar gevolg is van een verstoring van het hormoonsysteem. Dit is doorgaans lastig aan te tonen. De regelgeving vereist informatie die aangeeft dat een stof veilig kan worden gebruikt. Deze informatie kan onder andere in de vorm van gestandaardiseerde dierproeven worden verkregen. De vereiste proefdierstudies zijn echter niet ontwikkeld om alle nadelige effecten als gevolg van hormoonverstoring aan te tonen. Deze studies zijn evenmin ontwikkeld om het achterliggende biologische mechanisme te achterhalen. De studie die op dit moment het beste in staat is om nadelige effecten als gevolg van een mogelijk hormonaal werkingsmechanisme op te pikken is de zogenoemde extended one generation-studie. Deze studie is echter nog lang niet in alle wettelijke kaders verplicht. Hetzelfde geldt voor gestandaardiseerde studies om het biologische werkingsmechanisme te testen. Om beter te kunnen beoordelen of een stof hormoonverstorend is op basis van de voorgestelde criteria pleit het RIVM ervoor om enkele dierproefstudies aan te passen en de extended one generation-studie te verplichten. Daarnaast wordt voorgesteld om de gestandaardiseerde studies naar werkingsmechanismen verplicht te stellen in de huidige regelgeving. Deze aanpassingen in de regelgeving zijn relatief eenvoudig uit te voeren en zijn belangrijk om stoffen met een hormoonverstorende werking op te sporen. Ook zijn nieuwe methoden voor studies nodig om de nog ontbrekende effecten en mechanismen rele, The EU Commission published the draft criteria for the identification of endocrine disrupting chemicals (EDC). Currently the data requirements of the relevant legal frameworks are mostly insufficient to comply with these criteria. An EDC is defined as a substance which causes an adverse effect, like decreased fertility or effects on the developing nervous system. Additionally it must be shown that the adverse effect is biologically related to a disturbance of the function of the hormonal system. This is, in most cases, difficult. Legislation requires information for a safe use of a substance, among which animal studies are also required. Animal studies are, however, not specifically developed to show adverse effects caused by endocrine disruption (ED). These studies are also not developed to elucidate endocrine mechanisms of action (MOA). Currently the best study to investigate effects caused by ED is the extended one generation study (EOGRTS). Neither the EOGRTS nor studies specifically developed to investigate MOA mediated effects are currently required in most of legal frameworks. RIVM recommends to update several animal studies that can detect MOA mediated adverse effects and to oblige the EOGRTS in the data requirements of the relevant legal frameworks. These adoptions are relatively easy to fulfil and are important for the detection of EDC. Additionally it is recommended to develop test guidelines that can detect different endocrine MOAs and the related effects. Finally, it is imperative to develop a balanced guidance to evaluate which information fulfils the proposed criteria.

RIVM rapport:In 2014 en 2015 zijn de Europese normen voor een veilige blootstelling aan Bisfenol A (BPA) van werknemers en consumenten aangescherpt. Het RIVM concludeert dat nieuwe inzichten voldoende aanleiding vormen om verdere aanscherping van de normen te overwegen en stelt voor op korte termijn aanvullende maatregelen te treffen die de blootstelling aan BPA verder verminderen. Bisfenol A (BPA) is een stof die in veel producten zit, zoals kassabonnen, bouwmaterialen (verf en coatings), verpakkingsmateriaal van voedsel, speelgoed en medische hulpmiddelen. BPA is bij een te hoge blootstelling schadelijk voor de vruchtbaarheid en kan effect op het hormoonsysteem hebben. Nieuwe studies laten zien dat BPA het immuunsysteem van de ongeboren vrucht of jonge kinderen kan schaden bij een lager blootstellingsniveau dan het niveau waarop de huidige normen zijn gebaseerd. Dit lagere blootstellingsniveau is van ongeveer dezelfde grootte als de dagelijkse blootstelling van consumenten en werknemers aan BPA. Als gevolg van deze blootstelling hebben mensen mogelijk meer kans om voedselintoleranties te ontwikkelen en kunnen ze gevoeliger voor infectieziekten worden. Op basis van deze nieuwe inzichten wordt de rijksoverheid geadviseerd waar mogelijk op korte termijn de blootstelling aan BPA te verminderen. De bescherming van kleine kinderen, zwangeren en vrouwen die borstvoeding geven verdient hierbij bijzondere aandacht. Kleine en ongeboren kinderen zijn namelijk gevoeliger dan volwassenen voor de effecten van BPA doordat hun lichaam sterk in ontwikkeling is. De blootstelling kan bijvoorbeeld worden verminderd door veilige alternatieven te ontwikkelen, of ervoor te zorgen dat er minder BPA vrijkomt uit producten waar deze stof in wordt gebruikt. Daarnaast kunnen werknemers tegen blootstelling aan BPA worden beschermd. Een lagere blootstelling is ook van belang voor dieren in waterbodems, die nadelige effecten ondervinden bij de huidige BPA-concentratie- niveaus., More stringent European standards for safe exposure of workers and consumers to bisphenol A (BPA) were proposed in 2014 and 2015. As a result of this exposure, people could have a greater probability of developing food intolerances and could become more susceptible to infectious diseases. Based on these new insights RIVM advises the national government to reduce BPA exposure in the short term wherever possible. Special attention needs to be devoted to protecting small children, pregnant women and women who breastfeed. This is because developing unborn and young children are more sensitive than adults to the effects of BPA. Ways to reduce exposure include developing safe alternatives or ensuring that less BPA is released from products. Additionally, workers can be protected against BPA exposure. Lower exposure is also important for sediment-dwelling animals that experience adverse effects due to current BPA concentration levels.

RIVM rapport:Het RIVM heeft deelgenomen aan een project (SLiM) dat eraan wil bijdragen om minder dierproeven te gebruiken bij testmethoden voor geneesmiddelen en chemische stoffen. Uit het project vloeiden vier studies voort. Twee daarvan worden in dit rapport beschreven; de analyses zijn nog gaande en definitieve resultaten worden medio 2014 bekend. De eerste studie betrof een testonderdeel voor de toelating van geneesmiddelen op de markt waarbij wordt getoetst of het middel veilig is voor een zich ontwikkelend embryo. Onderzocht is of het nodig is om geneesmiddelen in twee diersoorten (rat en konijn) te testen of dat er één volstaat, en zo ja onder welke voorwaarden. Dankzij de bereidheid van de farmaceutische industrie is een grote database opgezet met bestaande gegevens over de reacties (gevoeligheid) van de twee diersoorten op een reeks aan stoffen. De tweede studie betrof een testonderdeel voor de toelating van chemische stoffen, en wel de mate waarin stoffen kankerverwekkende eigenschappen hebben. De duur van deze tests verschilt sterk: bij de ene test worden ratten of muizen gedurende twee jaar aan een stof blootgesteld, bij de andere drie tot zes maanden. Op basis van bestaande data wordt geanalyseerd of de twee jaarstudie meerwaarde heeft. Beide studies sluiten aan bij de doelstelling van het RIVM om het gebruik van dierproeven te verminderen. Dit project onderstreept het belang dat alle partijen die te maken hebben met de keten van alternatieve testmethoden (ontwikkelaars, regelgevers, gebruikers; universiteit, overheid en bedrijfsleven) betrokken zijn bij de ontwikkeling van nieuwe methoden. This project underlines the importance that all parties involved in the chain of alternative test methods (developers, regulators, end-users; universities, government and industry) are involved in the development of new methods. The RIVM participated as a government partner to bring about knowledge on validation and acceptance of testing protocols.

RIVM rapport:Epigenetische programmering is een fysiek mechanisme dat ervoor zorgt dat lichaamsfuncties goed zijn aangepast aan invloeden uit de omgeving. Het kan worden voorgesteld als schakelaars die de activiteit van celfuncties bepalen. Als het schakelbord van de cel van een kind tijdens de zwangerschap niet goed wordt ingesteld, kan dit later in het leven negatieve gevolgen hebben. De cel functioneert dan niet goed, oftewel gaat minder adequaat op de omgeving reageren. Zulke celverstoringen kunnen op verschillende manieren ontstaan, bijvoorbeeld door ongebalanceerde voeding, chemische verontreinigingen in voedsel, en tabaksrook. Epigenetische programmering kan zo het risico vergroten dat iemand een chronische ziekte ontwikkelt, zoals overgewicht, diabetes, hart- en vaatziekten, sommige allergieën en kanker. Uit literatuuronderzoek van het RIVM blijkt dat de verstoring van de epigenetische programmering een reëel fenomeen is. Hoe groot de bijdrage uit de omgeving op verstoringen is, is nog niet duidelijk. Wel kan dit gegeven de toename van de genoemde aandoeningen in de laatste jaren helpen verklaren. Er zijn technieken beschikbaar die gebruikt kunnen worden om de bijdrage van omgevingsfactoren aan verstoring van het epigenetisch programma te meten en in de literatuur worden hiervoor meerdere testmethoden beschreven. Het RIVM stelt voor om deze testmethoden nader uit te werken. Met goede testmethoden kunnen in de toekomst risico's van blootstellingen tijdens zwangerschappen beter worden ingeschat en gerichte beleidsadviezen worden opgesteld. Het belang van epigenetische programmering bleek voor het eerst bij kinderen van zwangere vrouwen die in de hongerwinter van de Tweede Wereldoorlog ondervoed waren. Deze kinderen bleken later in hun leven vaker te lijden aan chronische ziekten als diabetes dan leeftijdsgenoten die de ondervoeding niet hoefden te doorstaan. Een ander voorbeeld is een tijdens de zwangerschap doorgemaakte blootstelling aan een vocht- en vuilafstotend middel, Epigenetic programming is a physical mechanism that enables adaptation of body functions to environmental conditions. It can be considered as switches that determine the activity of cell functions. Inproper adjustment of the cellular switch board early in life can have negative consequences later in life. The cell will not function properly, it will not respond adequately on external stimuli. There are various ways that can lead to such disruption of cell functions, including imbalanced diet, chemical contaminants in food, and tobacco smoke. Through this mechanism, epigenetic programming can contribute to the risk of chronic disease, such as overweight, diabetes, heart failure, some allergies, and cancer. A literature review by RIVM shows that disruption of epigenetic programming is undisputed. However, it is not yet possible to estimate the actual contribution of environmental conditions on such disruption. On the other hand, this paradigm could contribute to the explanation of rising trends of chronic disease in recent years. Techniques are available to measure the contribution of environmental factors to disruption of epigenetic programming, and in literature, several methods have been proposed to fill that gap. RIVM proposes further development of such methods. Validated test methods could be used to improve estimation of the risk of environmental exposures during pregnancy, and thus contribute to regulatory advice. The importance of epigenetic programming was first suggested in children of mothers who experienced undernutrition during pregnancy at the time of the Dutch Hunger Winter. As middle aged adults, these children had higher rates of chronic diseases such as diabetes as compared to people of the same age who did not suffer the undernutrition. Another example is prenatal exposure to a repellent, which is associated with reduced vaccination efficiency at school age.

RIVM rapport:In de keten van validatie, acceptatie en implementatie van 3V-testen zijn in de Programmeringsstudie en het RIVM-rapport 'Knelpunten bij de ontwikkeling, validatie en implementatie van Alternatieven voor Dierproeven' de context van diverse toepassingsdomeinen en de knelpunten daarin beschreven. Uit deze studies blijkt dat voor het wegnemen van deze knelpunten een ketengerichte aanpak nodig is. Deze aanpak houdt in dat in elke fase van de keten van onderzoek tot implementatie van 3V-testen, alle ketenpartners worden betrokken. Voor de kennisvraag wordt jaarlijks een keuze gemaakt voor één toepassingsdomein om daarin het functioneren van de keten te optimaliseren. In dit domein worden drempels geïdentificeerd en acties benoemd om deze drempels te verlagen; hiertoe wordt een plan van aanpak gemaakt. Voor 2013 is gekozen voor het toepassingsdomein ontwikkelings- en reproductietoxicologie; de argumenten voor deze keuze worden gegeven. De vooruitgang in de innovatie van risicobeoordeling en alternatieve methoden, worden besproken voor dit domein, evenals de verbinding tussen deze ontwikkelingen en de Programmeringsstudie en het "Actieplan Dierproeven en Alternatieven 2011-2021". Background: Availability of chemical response-specific lists of genes (gene sets) for pharmacological and/or toxic effect prediction for compounds is limited. We hypothesize that more gene sets can be created by next-generation text mining (next-gen TM), and that these can be used with gene set analysis (GSA) methods for chemical treatment identification, for pharmacological mechanism elucidation, and for comparing compound toxicity profiles. Methods: We created 30,211 chemical response-specific gene sets for human and mouse by next-gen TM, and derived 1,189 (human) and 588 (mouse) gene sets from the Comparative Toxicogenomics Database (CTD). We tested for significant differential expression (SDE) (false discovery rate -corrected p-values < 0.05) of the next-gen TM-derived gene sets and the CTD-derived gene sets in gene expression (GE) data sets of five chemicals (from experimental models). We tested for SDE of gene sets for six fibr Results: Next-gen TM-derived gene sets matching the chemical treatment were significantly altered in three GE data sets, and the corresponding CTD-derived gene sets were significantly altered in five GE data sets. Six next-gen TM-derived and four CTD-derived fibrate gene sets were significantly altered in the PPARA knock-out GE dataset. None of the fibrate signatures in cMap scored significant against the PPARA GE signature. 33 environmental toxicant gene sets were significantly altered in the t Conclusions: Gene set analysis with next-gen TM-derived chemical response-specific gene sets is a scalable method for identifying similarities in gene responses to other chemicals, from which one may infer potential mode of action and/or toxic effect.

RIVM rapport:Het RIVM probeert alternatieve testmethoden voor de zogeheten reproductietoxicologie zodanig te verbeteren dat ze betrouwbaar genoeg zijn om in de regelgeving te kunnen worden ingevoerd. Op die manier kan het aantal dierproeven worden verminderd. Reproductietoxicologie houdt zich bezig met mogelijke schadelijke effecten van stoffen op de vruchtbaarheid, de voortplanting, en de ontwikkeling van het ongeboren kind. In dit rapport heeft het RIVM, in opdracht van het ministerie van VWS, de stand van zaken beschreven van alternatieve testmethoden voor deze wetenschap. Onderzoek naar alternatieve testmethoden: Het RIVM ontwikkelt zelf alternatieve tests en teststrategieën waarvan de wetenschap verwacht dat ze schade aan de ontwikkeling kunnen voorspellen. De afgelopen vijf jaar is voortgang geboekt op het gebied van de embryonale stamceltest, de ratten-embryokweek, en de zebravis-embryotest. Het eigen onderzoek is een belangrijke ondersteuning hierbij., RIVM is improving alternative test methods in the area of reproductive toxicology, to make them ready for regulatory application. This will reduce experimental animal use. Reproductive toxicology pertains to adverse effects on fertility and reproduction and on prenatal development of the unborn child. In this report, the state of the art of alternatives in this area is reviewed. Research into alternative methods: RIVM develops alternative tests and testing strategies expected to be able to predict reproductive toxicity. During the last five years advances have been achieved with the embryonic stem cell test, the rat embryo culture and the zebra fish embryo test. These embryos are highly premature and not considered as experimental animals under European law. In these tests effects are studied on the level of gene expression. Effects on gene expression may give more detailed information on compound effects. Moreover, such effects may allow easier translation to effects in man. Current effect assessment is done in experimental animals. Innovation of regulatory toxicology: In addition, RIVM is active in the international arena to innovate regulations concerning experimental animals and alternative test systems. This research provides important support for these activities.

Testmethoden voor gevaren van chemische stoffen houden onvoldoende rekening met deze verschillen. Het RIVM heeft daarom enkele dierstudies uitgevoerd om een optimaal protocol op te stellen om schadelijke effecten van stoffen op jonge kinderen te kunnen vaststellen. De voorlopige conclusie is dat een blootstelling bij ratten vlak na de geboorte (tussen de 10 en 50 dagen) het meest geschikt is om de risico's in kaart te brengen. Daarnaast blijken de effecten het meest zichtbaar te zijn in de fysieke ontwikkeling en op het immuunsysteem van het kind. Het protocol is in opdracht van het ministerie van VWS opgesteld. Vermoed wordt dat neurologische aandoeningen of afwijkingen in het immuunsysteem - van ADHD tot allergieën en diabetes - veroorzaakt kunnen worden door blootstellingen aan chemische stoffen op jonge leeftijd. Fysieke en gedragsmatige verschillen: De uiteenlopende risico's zijn te verklaren vanuit fysieke en gedragsmatige verschillen tussen kinderen en volwassenen. Kinderen stoppen bijvoorbeeld veel in hun mond (speelgoed, zand), waardoor zij via deze 'route' blootgesteld kunnen worden aan giftige stoffen. Door de onvolgroeide organen bij kinderen worden stoffen bijvoorbeeld minder snel uit het lichaam verwijderd dan bij volwassenen. Ook wijkt de ontwikkeling van organen af; bij jonge kinderen zijn onder meer het immuunsysteem en de hersenen sterk in ontwikkeling, wat hen gevoeliger kan maken voor schadelijke effecten van stoffen. Vervolgstudies zijn nodig voor een beter oordeel. Het is belangrijk hierbij de gevoeligheid van het zich ontwikkelende zenuwstelsel te betrekken. Internationale inbedding: De bevindingen van dit onderzoek zijn internationaal onder meer uitgedragen in het testrichtlijnenprogramma van de OESO (Organisatie voor Economische Ontwikkeling en Samenwerking). In dit programma worden testrichtlijnen wereldwijd afges, Exposures to chemicals may have larger risks in children than in adults. Test methods for chemical hazards do not take sufficient account of these differences. Therefore, RIVM has carried out several animal studies to design an optimal protocol for the assessment of hazards of chemicals for children. The preliminary conclusion indicates that exposure between postnatal days 10 and 50 in the rat is most relevant for hazard assessment. Effects on physical and immune system development are most prominent in this period. The protocol was designed by order of the ministry of health, welfare and sports. Neurologic diseases and immune system malfunction, ranging from ADHD to allergies and diabetes, may or may not be caused by chemical exposures early in life. Physical and behavioural differences: Variability in risks can be explained by physical and behavioural differences between children and adults. Children will more often put objects in their mouth (toys, sand), which may result in possible exposures to hazardous chemicals. Incomplete organ development in children may reduce excretion of chemicals from the body. In addition, incomplete development of the brain and immune system may render these tissues more susceptible to adverse effects of chemicals. Continued studies are needed for a final judgment. It is important to also consider the neural system in this research. International perspective: Internationally, the results of this research have been discussed in the test guidelines programme of OECD (Organisation for Economic Cooperation and Development). This programme provides worldwide consensus of test guidelines. In addition, a scientific symposium was organised on the subject.

RIVM rapport:Dit rapport geeft een overzicht van de in vitro testmethoden die de afgelopen decennia voor reproductie toxiciteit onderzoek zijn ontwikkeld. Het validatieproces van deze alternatieven voor dierproeven wordt hier eveneens beschreven. Inmiddels zijn voor embryotoxiciteit een aantal testen gevalideerd, die in de toekomst na verdere optimalisatie toegepast kunnen worden. Daarnaast kunnen individuele in vitro testen mogelijk ingezet worden bij het screenen van stoffen. Dit kan leiden tot een aanzienlijke vermindering van het proefdiergebruik voor reproductie toxiciteitstesten. Hoewel verscheidene alternatieven voor dierproeven voor reproductie toxiciteitstesten voorhanden zijn, blijken veel van deze methoden nog onvoldoende ontwikkeld en niet gevalideerd te zijn. A limited number of alternative tests for embryotoxicity, which can be applied after optimisation, has been validated. Although none of the in vitro methods represents a replacement for current animal tests, these methods can function as a pre-screen in a tiered approach. In this way, the industry can set priorities for in vivo testing and thus reduce animal use in reproductive toxicology. Development of in vitro methods for reproductive toxicity testing is essential, since testing according to the present guidelines requires a high number of test animals. The numbers of test animals needed range from 560 - 6,000 animals per chemical, dependent on the production volume. Moreover, increased animal use is envisaged as a result of the implementation of REACH, the new European legislation for manufacturers and users of chemicals. REACH requires toxicological information to be submitted for about 30,000 existing chemicals, which will require millions of test animals. Many alternative test methods have already been developed for reproductive toxicity testing, and several of them are in a (pre-)validation process. However, none of these methods offers enough reliability to replace in vivo assays currently used. Testing toxic effects on the reproductive cycle is complex, since reproduction is characterised by many different processes and sensitive periods. Due to its complexity, the whole cycle cannot be modelled in one in vitro system. Parts of the system need to be studied individually and then integrated into testing strategies.

To date, however, the data available are insufficient to enable all of the consequences of such work-related exposures to be quantified. In order to obtain an indication of the significance of these consequences, the National Institute for Public Health and the Environment (RIVM) has estimated the burden of disease of five clinical outcomes (such as spontaneous abortion and congenital abnormalities) and four groups of substances (such as pesticides and solvents). Taken together, these factors account for approximately 400 disability-adjusted life years (DALYs), which is 1% of the total amount of DALYs associated with reproductive effects among the working population (regardless of cause). Due to a lack of data and the uncertainty in various assumptions, the uncertainty margin of this estimate is quite large. The burden of disease is a concept used to quantify the consequences of illnesses relative to a disease-free situation. It is expressed in DALYs, a health gap measure which combines the time lived with disability (decreased quality of life) and the time lost due to untimely death. Many experts actively participated in this project as consultants on the delineations, points of departure, assumptions and the methods of calculation. Opinions varied greatly on a number of topics, such as whether the burden of disease of a spontaneous abortion only applies to the woman or also to her partner and/or unborn child. Only a few experts gave their opinion on the magnitude of total burden of disease of reproductive effects caused by chemical exposure in the workplace. These estimates varied between 100 and 10,000 DALYs. This range falls approximately in the middle of the range of the burdens of disease of nine other disorders caused by chemical exposure in the workplace that have previously been reported by

De innovaties moeten leiden tot een verhoogde efficiency en een verminderd proefdiergebruik., Chemicals must be tested for adverse effects on human reproduction. Thanks to innovations, testing will become more efficient, thereby reducing the use of experimental animals. The main developments have been signalled in an overview of current developments in the testing of reproductive toxicity (the adverse effects of chemicals of reproduction) documented in this RIVM report. Chemicals can adversely affect reproduction, which may manifest itself as reduced fertility or maldevelopment of the fetus. Manufacturers of chemicals are required to assess these adverse effects, for example, in the framework of the European programme for chemical safety called REACH. Current methods for reproductive toxicity assessment, which rely mainly on experimental animal studies, stem from the early eighties. The high animal use in the testing of reproductive toxicicity is primarily due to the need to study more than one generation. Reductions of animal use are, however, imminent, with developments in the endocrine disruption, animal welfare and REACH having stimulated innovations in this area. Standard protocols are in the process of being amended and there are proposals for novel test systems. These developments also influence testing strategies, which combine individual tests in a tiered approach. These are innovations then that should lead to increased efficiency and reduced animal use.

Vervolgens wordt bekeken bij welke concentratie (in vitro) de effecten overeenkomen met effecten in het intacte dier (in vivo). Dit werd voor 19 verschillende stoffen onderzocht. Gegeven deze onzekerheid is het volledig vervangen van de in vivo dierstudie door de 'Whole Embryo Culture' test voor kwantitatieve risicobeoordeling vooralsnog niet haalbaar., Animal tests can not always simply be replaced by in vitro tests (carried out in test tubes or petri dishes). This is underlined by research at the National Institute for Public Health and the Environment (RIVM). Existing research data from an in vitro test on developmental effects have been evaluated for their usefulness. This concerns disturbances in the development of rat embryos due to exposure to chemicals. This kind of alternative tests are being developed due to the public objection to animal tests (in vivo) for the purpose of quantitative risk assessment. In the investigated 'Whole Embryo Culture' assay rat embryos are exposed to various concentrations of a particular chemical. Next, it is determined at which concentration (in vitro) effects occur that are similar to those in the whole animal (in vivo). This was investigated for 19 different substances. There appears to be a clear relationship between the determined potencies in the in vitro and in vivo tests, but the scatter (uncertainty) in this relationship is very large (about a factor of hundred). This can partially be explained by the different ways in which the in vivo tests have been performed (the pregnant rats are for example exposed at different days in the gestation). In addition, differences in the uptake or excretion of a substance by the body could play a role. This study describes a first initiative to account for these differences. Even if these aspects are taken into account, the uncertainty in the outcome remains large. Given this uncertainty, the full replacement of the in vivo animal test by the 'Whole Embryo Culture' assay for quantitative risk assessment is hitherto not feasible.

RIVM rapport:Het RIVM heeft teststrategieen onderzocht (ITS) om de risicobeoordeling van chemicalien proefdiervriendelijker te maken. Het wil hiermee bijdragen aan onderzoeksprogramma's hiernaar van de EU en de OESO. Het accent ligt daarbij op de toepassing van alternatieve methoden en de verbetering van bestaande testmethoden. Dit onderzoek is hard nodig om de nieuwe Europese wetgeving voor industriele stoffen, REACH, te laten slagen, welke op 1 juni 2007 in werking treedt. ITS staat voor geintegreerde of intelligente Teststrategieen. Het zijn strategieen op papier waarmee chemische stoffen zo effectief mogelijk kunnen worden getest op mogelijke gevaren. Die strategieen maken inzichtelijk welke testen of wiskundige methoden voor een bepaalde stof moeten worden gebruikt, en in welke volgorde. ITS wil een antwoord zijn op de toenemende testvereisten in de regelgeving voor grote aantallen stoffen waarover weinig fysisch-chemische en (eco)toxicologische gegevens bekend zijn. Het gaat dan vooral om de ontwikkeling van strategieen waarvoor testmethoden op celniveau (in vitro) en wiskundige testmethoden (in silico) gebruikt worden. De wiskundige methoden zijn nodig om de blootstelling aan stoffen en de relatie tussen effecten hiervan en chemische structuur in te kunnen schatten. Daarnaast blijft proefdiergebruik (in vivo) in sommige gevallen nodig. Door de resultaten van al deze methoden op slimme wijze aan elkaar te koppelen kan met voldoende zekerheid inzicht worden verkregen in de effecten van chemische stoffen. En daarmee kunnen, naar verwacht, stoffen met minder proefdieren, goedkoper en sneller op hun veiligheid worden beoordeeld. Het rapport beschrijft hoe in elke stap van de teststrategie zo verantwoord mogelijk kan worden omgegaan met de onzekerheden in de gebruikte resultaten van testen en methoden. Vervolgens worden teststrategieen besproken voor de beoordeling van 1) de afbraak van chemicalien in het milieu, 2) overgevoeligheid van huid en ademhalingswegen en 3) nadelige e, RIVM's investigation of integrated testing strategies (ITS) to reduce the use of experimental animals in the risk assessment of chemical substance focuses on the application of alternative and improved test methods. This research is highly desirable for the contribution it can make to the successful implementation of the new EU legislation for industrial chemicals, REACH, as of June 1, 2007. In this way RIVM will contribute to EU and OECD research into testing strategies. Integrated, or Intelligent, Testing Strategies (ITS) are strategies for the effective testing of the hazards of chemical substances. Such strategies show what tests or mathematical methods should be used for a particular substance, and in what order. ITS are intended as an answer to the ever-increasing demand for testing in regulations for a great number of substances with limited databases. The focus of ITS is especially on the development of strategies on the basis of test methods at cellular level (in vitro) and mathematical methods (in silico). The mathematical methods are needed for the assessment of exposure and of the relation between effects and chemical structure. Some tests with experimental animals (in vivo) will also remain necessary. Knowledge on the effects of chemical substances can be derived with sufficient certainty by smartly coupling these methods with each other. In this way, the expectation is that chemical substances will be assessed cheaper and faster, with less use of experimental animals. The report first describes how to deal with uncertainties in the results of tests and methods used in each step of a particular ITS. Next it focuses on testing strategies for the assessment of: 1) environmental degradation 2) sensitization and 3) adverse effects on fertility and progeny.

RIVM rapport:Niet elke werknemer en werkgever is zich bewust van het eventueel vrijkomen van voor de voortplanting giftige stoffen op zijn/haar werkplek. Om werkgevers, werknemers en arbo-professionals op mogelijke blootstelling te attenderen heeft het RIVM een database aangelegd. Hierin zijn stoffen waarvan bekend is dat ze giftig zijn voor de voortplanting gekoppeld aan branches en werkgerelateerde handelingen in deze branches. De database vormt een eerste aanzet. Aanpassingen en uitbreidingen van de database kunnen worden uitgevoerd in samenwerking met de branches., Not every employer and employee is aware of possible exposure to compounds classified for reproductive toxicity in their working environment. To facilitate employers and employees in their understanding of work-related exposure to reprotoxic compounds, RIVM set up a database in which 183 (groups of) compounds are coupled to their use at workplaces. The information on their use was obtained from international websites. Information from the database can be obtained by selecting a branche or selecting a compound. In both cases actions that are performed using the compound are indicated. Exposure levels are not available in the database. The database forms a first start. Adjustment and extension of the database should be performed in close cooperation with the branches.

RIVM rapport:Structure-Activity Relationships (SARs), including Quantitative SARs, are applied to the hazard assessment of chemicals. This need is all the more urgent considering the proposed new EU policy on chemicals in REACH, which stresses the need for non-animal testing. DEREKfW and the TSCA Chemical Category List of the New Chemicals Program of the US-EPA were chosen to predict reproductive toxicity for REACH purposes. DEREKfW is a software program predicting the toxicological properties using the literature and expert-derived structural alerts, while the TSCA Chemical Category List is a document and is based on expert judgment and a category approach. We screened the performance of both models on recognizing substances that are classified for reproductive toxicity in the EU (based on experimental animal tests). As we limited our research to only reproductive positive examples the rate of false positives could not be assessed. DEREKfW partially fulfils the OECD principles for good (Q)SARs. DEREKfW and TSCA Chemical Category List did not recognize 90 and 77% of the substances classified for 'impaired fertility', and 81 and 82% of the substances classified for 'harm to the unborn child', respectively. Besides one mutual 'alert/category' DEREKfW and TSCA contain 7 'alerts' and 10 categories, respectively. As the alerts in DEREKfW (comprehensible and transparent tool) and categories in the TSCA Chemical Category List are highlighted in this research, they both can be used as additional expert judgment when assessing chemicals for reproductive toxicity. However, we conclude that these models cannot be the only method for screening chemicals for reproductive toxicity in the framework of REACH. Other models or testing strategies have to be used to assess reproductive toxicity of chemicals., Structuur-activiteitsrelaties (SARs), inclusief kwantitatieve SARs worden gebruikt in de risicobeoordeling van stoffen. Afgezien van iin gezamenlijke 'alert' hebben DEREKfW (een helder model) and de TSCA chemische Categorieen Lijst nog 7 'alerts' en 10 categorieen, respectievelijk. Doordat de alerts in DEREKfW and TSCA Categorieen Lijst in dit onderzoek naar voren komen, kunnen deze gebruikt worden als additionele 'expert judgement'. We concluderen echter dat deze modellen niet de enige methode kunnen zijn voor het screenen van stoffen voor reproductie toxiciteit in het kader van REACH. Andere modellen en teststrategieen zijn nodig om reproductie toxiciteit van stoffen te beoordelen.

RIVM rapport:In de 'regulatory toxicology' (beleidsondersteunende toxicologie) groeit het besef dat kinderen en volwassenen kunnen verschillen in hun gevoeligheid voor xenobiotica. In het huidige rapport wordt een beknopt overzicht gegeven van de relevante gegevens over de verschillen tussen volwassenen en kinderen met betrekking tot blootstelling, kinetiek en dynamiek van chemische stoffen, en de geschiktheid van de momenteel gebruikte toxicologische testen in het kader van de toelatingsbeoordeling wordt bediscussieerd. Gezien de potentieel andere gevoeligheid van kinderen wordt tevens een aantal aanbevelingen gedaan voor de verdere ontwikkeling van risicobeoordeling. Binnen het veld van de toxicologische risicobeoordeling zijn er drie belangrijke gebieden waarin kinderen en volwassenen verschillen; blootstelling aan en toxicokinetiek en toxicodynamiek van xenobiotica. Of in de risicobeoordeling specifiek rekening gehouden moet worden met kinderen is in de eerste plaats afhankelijk van het blootstellingspatroon. Met name de situatie waarin kinderen hoger blootgesteld worden dan volwassenen verdient de volle aandacht. Kinderen eten en drinken meer per kilogram lichaamsgewicht dan volwassenen, en hun voedingspatroon is anders en minder gevarieerd. Bovendien hebben kinderen een relatief hoge respiratoire activiteit (waardoor er een verhoogde blootstelling via de ademhalingswegen kan optreden) en een hoge lichaamsoppervlak-lichaamsgewicht ratio (mogelijk resulterend in een verhoogde dermale blootstelling). Tevens kan de blootstelling van kinderen aan toxische stoffen hoger zijn dan volwassenen omdat kinderen vaak langer in een bepaalde ruimte of omgeving verblijven, ze zich dichter bij een besmet oppervlak kunnen bevinden (bijvoorbeeld tijdens kruipen over een behandeld oppervlak), en minder hygienische gedrag vertonen (sabbelen op handen, objecten en oppervlakken; picagedrag). In this report a concise overview of the relevant data on the differences between adults and children with respect to the kinetics, dynamics and exposure to chemicals is presented and adequacy of currently used toxicological tests for regulatory purposes is discussed. In view of the potentially increased vulnerability of children a number of recommendations for further development of risk assessment of chemicals are made. There are three major areas in which children and adults differ: exposure to and toxicokinetics and toxicodynamics of xenobiotics. Especially the situation in which children are higher exposed than adults this needs full attention. Children consume more food and drink more fluids per kg body weight than adults, and their dietary pattern is different and less varied. Moreover, children have a relatively high inhalatory rate (which may lead to a higher inhalatory exposure) and a high body surface-body weight ratio (which may result in a higher dermal exposure). Children may also be more exposed to toxic substances than adults since children spend more time in the same room or area, are in closer contact with a contaminated surface (e.g. by crawling) and display less hygienic behaviour (mouthing of hands, objects, surfaces; pica behaviour). The route of exposure may be of importance for the potentially toxic effects of a chemical. In most toxicological studies for regulatory purposes the oral route of administration is used. However, the expected route of exposure of a child should be taken into consideration in the design of toxicological studies since the systemic exposure to a chemical may differ due to different levels of absorption and the absence of a first pass effect following dermal and inhalatory absorption. A further point of concern, for adults as well as children, is the aggregate exposure to a spec
