2. Preoperative risk factors related to bladder cancer rehabilitation: a registry study

Abstract

INTRODUCTION: Patients diagnosed with (muscle-) invasive bladder cancer (IBC) are more likely to harbour comorbidities due to their advanced age at diagnosis. Under-nutrition is a predictor for postoperative morbidity and mortality in cancer patients, but under-reported in urology. Understanding the IBC patient profile before major surgery could facilitate and optimise outcome of the surgical patient. BACKGROUND/OBJECTIVES: To identify preoperative risk factors for early rehabilitation before radical cystectomy (RC). SUBJECTS/METHODS: A historical registry-based study of 76 patients referred for RC at Aarhus University Hospital, Denmark (DK) in 2009. Early rehabilitation was defined by length of stay (LOS) postoperatively with a cutoff ≤ 11 days. High comorbidity was expressed by the charlson comorbidity index score (CCI) ≥ 3. LOS was calculated by linking the unique Civil Registration Number with the National Patient Registry. Preoperative nutritional risk was identified using the screening tool, nutritional risk score 2002 (NRS) of the European Society of Clinical Nutrition and Metabolism. Multivariate analysis was used to identify risk factors for early rehabilitation. RESULTS: The proportion of patients at preoperative nutritional risk was 26% (95% confidence interval (CI): (95% CI: 17; 37) and 43% of patients held a high CCI (95% CI: 33; 55). Prolonged LOS was independently associated with female gender (P = 0.02) and age ≥ 70 years (P = 0.04). NRS and CCI were not associated with LOS. CONCLUSIONS: Attention should be focused on women and elderly patients undergoing RC to optimise early rehabilitation and reduce LOS. It is still unknown whether preoperative nutritional risk and comorbidity are obstacles in early rehabilitation of RC patients. Contains fulltext : 231687.pdf (Publisher’s version ) (Closed access), Whereas large-scale statistical analyses can robustly identify disease-gene relationships, they do not accurately capture genotype-phenotype correlations or disease mechanisms. We use multiple lines of independent evidence to show that different variant types in a single gene, SATB1, cause clinically overlapping but distinct neurodevelopmental disorders. Clinical evaluation of 42 individuals carrying SATB1 variants identified overt genotype-phenotype relationships, associated with different pathophysiological mechanisms, established by functional assays. Missense variants in the CUT1 and CUT2 DNA-binding domains result in stronger chromatin binding, increased transcriptional repression, and a severe phenotype. In contrast, variants predicted to result in haploinsufficiency are associated with a milder clinical presentation. A similarly mild phenotype is observed for individuals with premature protein truncating variants that escape nonsense-mediated decay, which are transcriptionally active but mislocalized in the cell. Our results suggest that in-depth mutation-specific genotype-phenotype studies are essential to capture full disease complexity and to explain phenotypic variability.

Abstract

An operational multimodel forecasting system for air quality has been developed to provide air quality services for urban areas of China. The initial forecasting system included seven state-of-the-art computational models developed and executed in Europe and China (CHIMERE, IFS, EMEP MSC-W, WRF-Chem-MPIM, WRF-Chem-SMS, LOTOS-EUROS, and SILAMtest). Several other models joined the prediction system recently, but are not considered in the present analysis. In addition to the individual models, a simple multimodel ensemble was constructed by deriving statistical quantities such as the median and the mean of the predicted concentrations. ©2019 Author(s).

Abstract

An operational multi-model forecasting system for air quality including nine different chemical transport models has been developed and provides daily forecasts of ozone, nitrogen oxides, and particulate matter for the 37 largest urban areas of China (population higher than 3 million in 2010). These individual forecasts as well as the mean and median concentrations for the next 3 days are displayed on a publicly accessible website http://www.marcopolo-panda.eu, last access: 7 December 2018). The paper describes the forecasting system and shows some selected illustrative examples of air quality predictions. It presents an intercomparison of the different forecasts performed during a given period of time (1-15 March 2017) and highlights recurrent differences between the model output as well as systematic biases that appear in the median concentration values. Pathways to improve the forecasts by the multi-model system are suggested. © 2019 Author(s).

Abstract

Background— The renin–angiotensin–aldosterone system (RAAS) is critical for regulation of blood pressure and fluid balance and influences cardiovascular remodeling. Dysregulation of the RAAS contributes to cardiovascular and renal morbidity. The genetic architecture of circulating RAAS components is incompletely understood. Methods and Results— We meta-analyzed genome-wide association data for plasma renin activity (n=5275), plasma renin concentrations (n=8014), and circulating aldosterone (n=13289) from ≤4 population-based cohorts of European and European-American ancestry, and assessed replication of the top results in an independent sample (n=6487). Single-nucleotide polymorphisms (SNPs) in 2 independent loci displayed associations with plasma renin activity at genome-wide significance ( P −8 ). A third locus was close to this threshold (rs4253311 in kallikrein B [KLKB1], P =5.5×10 −8 ). Two of these loci replicated in an independent sample for both plasma renin and aldosterone concentrations (SNP rs5030062 in kininogen 1 [KNG1]: P =0.001 for plasma renin, P =0.024 for plasma aldosterone concentration; and rs4253311 with P NEBL gene reached genome-wide significance for plasma renin concentration in the discovery sample (top SNP rs3915911; P =8.81×10 −9 ), but did not replicate ( P =0.81). No locus reached genome-wide significance for aldosterone. SNPs rs5030062 and rs4253311 were not related to blood pressure or renal traits; in a companion study, variants in the kallikrein B locus were associated with B-type natriuretic peptide concentrations in blacks. Conclusions— We identified 2 genetic loci ( kininogen 1 and kallikrein B ) influencing key components of the RAAS, consistent with the close interrelation between the kallikrein–kinin system and the RAAS.

Abstract

To dissect the genetic architecture of blood pressure and assess effects on target organ damage, we analyzed 128,272 SNPs from targeted and genome-wide arrays in 201,529 individuals of European ancestry, and genotypes from an additional 140,886 individuals were used for validation. We identified 66 blood pressure-associated loci, of which 17 were new; 15 harbored multiple distinct association signals. The 66 index SNPs were enriched for cis-regulatory elements, particularly in vascular endothelial cells, consistent with a primary role in blood pressure control through modulation of vascular tone across multiple tissues. The 66 index SNPs combined in a risk score showed comparable effects in 64,421 individuals of non-European descent. The 66-SNP blood pressure risk score was significantly associated with target organ damage in multiple tissues but with minor effects in the kidney. Our findings expand current knowledge of blood pressure-related pathways and highlight tissues beyond the classical renal system in blood pressure regulation.
