Abstract

BackgroundGlycated hemoglobin (HbA1c) is used to diagnose type 2 diabetes (T2D) and assess glycemic control in patients with diabetes. Previous genome-wide association studies (GWAS) have identified 18 HbA1c-associated genetic variants. These variants proved to be classifiable by their likely biological action as erythrocytic (also associated with erythrocyte traits) or glycemic (associated with other glucose-related traits). In this study, we tested the hypotheses that, in a very large scale GWAS, we would identify more genetic variants associated with HbA1c and that HbA1c variants implicated in erythrocytic biology would affect the diagnostic accuracy of HbA1c. We therefore expanded the number of HbA1c-associated loci and tested the effect of genetic risk-scores comprised of erythrocytic or glycemic variants on incident diabetes prediction and on prevalent diabetes screening performance. Throughout this multiancestry study, we kept a focus on interancestry differences in HbA1c genetics performance that might influence race-ancestry differences in health outcomes.Methods & findingsUsing genome-wide association meta-analyses in up to 159,940 individuals from 82 cohorts of European, African, East Asian, and South Asian ancestry, we identified 60 common genetic variants associated with HbA1c. We classified variants as implicated in glycemic, erythrocytic, or unclassified biology and tested whether additive genetic scores of erythrocytic variants (GS-E) or glycemic variants (GS-G) were associated with higher T2D incidence in multiethnic longitudinal cohorts (N = 33,241). Nineteen glycemic and 22 erythrocytic variants were associated with HbA1c at genome-wide significance. GS-G was associated with higher T2D risk (incidence OR = 1.05, 95% CI 1.04-1.06, per HbA1c-raising allele, p = 3 × 10-29); whereas GS-E was not (OR = 1.00, 95% CI 0.99-1.01, p = 0.60). In Europeans and Asians, erythrocytic variants in aggregate had only modest effects on the diagnostic accuracy of HbA1c. Yet, in African Americans, the X-linked G6PD G202A variant (T-allele frequency 11%) was associated with an absolute decrease in HbA1c of 0.81%-units (95% CI 0.66-0.96) per allele in hemizygous men, and 0.68%-units (95% CI 0.38-0.97) in homozygous women. The G6PD variant may cause approximately 2% (N = 0.65 million, 95% CI 0.55-0.74) of African American adults with T2D to remain undiagnosed when screened with HbA1c. Limitations include the smaller sample sizes for non-European ancestries and the inability to classify approximately one-third of the variants. Further studies in large multiethnic cohorts with HbA1c, glycemic, and erythrocytic traits are required to better determine the biological action of the unclassified variants.ConclusionsAs G6PD deficiency can be clinically silent until illness strikes, we recommend investigation of the possible benefits of screening for the G6PD genotype along with using HbA1c to diagnose T2D in populations of African ancestry or groups where G6PD deficiency is common. Screening with direct glucose measurements, or genetically-informed HbA1c diagnostic thresholds in people with G6PD deficiency, may be required to avoid missed or delayed diagnoses.

Abstract

Patients with established type 2 diabetes display both β-cell dysfunction and insulin resistance. To define fundamental processes leading to the diabetic state, we examined the relationship between type 2 diabetes risk variants at 37 established susceptibility loci, and indices of proinsulin processing, insulin secretion, and insulin sensitivity. We included data from up to 58,614 nondiabetic subjects with basal measures and 17,327 with dynamic measures. We used additive genetic models with adjustment for sex, age, and BMI, followed by fixed-effects, inverse-variance meta-analyses. Cluster analyses grouped risk loci into five major categories based on their relationship to these continuous glycemic phenotypes. The first cluster (PPARG, KLF14, IRS1, GCKR) was characterized by primary effects on insulin sensitivity. The second cluster (MTNR1B, GCK) featured risk alleles associated with reduced insulin secretion and fasting hyperglycemia. ARAP1 constituted a third cluster characterized by defects in insulin processing. A fourth cluster (TCF7L2, SLC30A8, HHEX/IDE, CDKAL1, CDKN2A/2B) was defined by loci influencing insulin processing and secretion without a detectable change in fasting glucose levels. The final group contained 20 risk loci with no clear-cut associations to continuous glycemic traits. By assembling extensive data on continuous glycemic traits, we have exposed the diverse mechanisms whereby type 2 diabetes risk variants impact disease predisposition.

Abstract

Gene-lifestyle interactions have been suggested to contribute to the development of type 2 diabetes. Glucose levels 2 h after a standard 75-g glucose challenge are used to diagnose diabetes and are associated with both genetic and lifestyle factors. However, whether these factors interact to determine 2-h glucose levels is unknown. We meta-analyzed single nucleotide polymorphism (SNP) × BMI and SNP × physical activity (PA) interaction regression models for five SNPs previously associated with 2-h glucose levels from up to 22 studies comprising 54,884 individuals without diabetes. PA levels were dichotomized, with individuals below the first quintile classified as inactive (20%) and the remainder as active (80%). BMI was considered a continuous trait. Inactive individuals had higher 2-h glucose levels than active individuals (β = 0.22 mmol/L [95% CI 0.13-0.31], P = 1.63 × 10(-6)). All SNPs were associated with 2-h glucose (β = 0.06-0.12 mmol/allele, P ≤ 1.53 × 10(-7)), but no significant interactions were found with PA (P > 0.18) or BMI (P ≥ 0.04). In this large study of gene-lifestyle interaction, we observed no interactions between genetic and lifestyle factors, both of which were associated with 2-h glucose. It is perhaps unlikely that top loci from genome-wide association studies will exhibit strong subgroup-specific effects, and may not, therefore, make the best candidates for the study of interactions.

Abstract

We report the synthesis of polycyclic uracil derivatives. The method is based on palladium-catalysed Sonogashira–Hagihara and Suzuki–Miyaura cross-coupling reactions followed by Brønsted acid-mediated cycloisomerisation. The developed methodology tolerates various functional groups and leads to moderate up to quantitative yields of the final products. The impact of different functional groups on the optical properties was studied by UV–vis and fluorescence spectroscopy.

Abstract

Background: Retrospective research on real-world data provides the ability to gain evidence on specific topics especially when running across different sites in research networks. Those research networks have become increasingly relevant in recent years; not least due to the special situation caused by the COVID-19 pandemic. An important requirement for those networks is the data harmonization by ensuring the semantic interoperability. Aims: In this paper we demonstrate (1) how to facilitate digital infrastructures to run a retrospective study in a research network spread across university and non-university hospital sites; and (2) to answer a medical question on COVID-19 related change in diagnostic counts for diabetes-related eye diseases. Materials and methods: The study is retrospective and non-interventional and runs on medical case data documented in routine care at the participating sites. The technical infrastructure consists of the OMOP CDM and other OHDSI tools that is provided in a transferable format. An ETL process to transfer and harmonize the data to the OMOP CDM has been utilized. Cohort definitions for each year in observation have been created centrally and applied locally against medical case data of all participating sites and analyzed with descriptive statistics. Results: The analyses showed an expectable drop of the total number of diagnoses and the diagnoses for diabetes in general; whereas the number of diagnoses for diabetes-related eye diseases surprisingly decreased stronger compared to non-eye diseases. Differences in relative changes of diagnoses counts between sites show an urgent need to process multi-centric studies rather than single-site studies to reduce bias in the data. Conclusions: This study has demonstrated the ability to utilize an existing portable and standardized infrastructure and ETL process from a university hospital setting and transfer it to non-university sites. From a medical perspective further activity is needed to evaluate data quality of the utilized real-world data documented in routine care and to investigate its eligibility of this data for research.

IntroductionAvailable evidence points to an association of increased screen time and the availability of digital tools during childhood with negative health outcomes in later life. For many years, public discourse focused on restricting access and use of digital technologies below certain ages. However, little is known about the specific benefit of a responsible use of digital primary prevention in the setting of (early) childhood education. The objective of this evidence synthesis is to investigate the effectiveness of digital primary prevention interventions targeting physical activity, motor skills and/or nutrition in children aged 3–10 years in day-care facilities and (pre-) schools.Methods and analysisWe present the rationale and methodological steps of a systematic review in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses procedures. Automated searches will be conducted by applying a pretested search strategy to the databases MEDLINE/PubMed, EMBASE and PsycInfo to identify relevant interventional (randomised controlled trials, controlled trials, crossover trials and pilot and feasibility) and observational (case–control, cohort) studies in English or German, with no date restrictions. The overall search will be complemented by backward, forward and additional hand searches. Two researchers will independently screen titles/abstracts and assess full texts by applying predefined eligibility criteria. Data extraction will be conducted by using a pretested data extraction sheet. The assessment of methodological quality will be performed independently by two review authors using the Critical Appraisals Skills Programme relevant to the study design applied in the given study. Additionally, qualitative content analysis will be conducted to analyse priorities for future research extracted from the discussion sections and conclusions of included studies.PROSPERO registration numberCRD42020207682.

10. Non-use of telemedicine: A scoping review

Abstract

Many telemedicine interventions fail to be implemented in medical care with non-use and discontinued use by patients being among the major reasons. The aim of this scoping review was to provide an overview of barriers associated with non-use and discontinued use of telemedicine. An electronic search was conducted in Pubmed in October 2019 and updated in November 2020, followed by a hand search in the beginning of 2021. All potential articles were screened by two independent reviewers based on predefined inclusion and exclusion criteria. A qualitative content analysis according to Mayring was carried out. The topics 'intervention', 'context of use' and 'user' were chosen as overarching themes. Out of 1377 potentially relevant articles, 73 were included. User-related barriers were mentioned in most of the analysed studies, followed by barriers related to the intervention. The analysis provides the basis for overcoming non-use issues in telemedicine.

Three bis- or tris-brominated 2-trifluoromethylquinolines have been successfully applied in palladium-catalysed Sonogashira reactions, leading to several examples of alkynylated quinolines in good to excellent yields. Optical properties of selected products have been studied by steady state absorption and fluorescence spectroscopy which give insights of the influence of the substitution pattern and of the type of substituents on the optical properties.

The development of a new and straightforward chemoselective method for the synthesis of uracil-based structures by combining Suzuki–Miyaura and Sonogashira–Hagihara cross-coupling is reported. The methodology was applied to synthesize a series of novel compounds. The tolerance of the combination of different functional groups was tested. The influence of different functional groups on the physical properties was studied by ultraviolet–visible (UV–vis) and fluorescence spectroscopy, providing new insights into the potential applications of uracil-based structures.

Aun así, se encuentran insuficientemente asumidas por las instituciones de la Unión Europea, ya que ésta sigue respondiendo al paradigma del Estado nacional de bienestar social. De ahí se ignora el gran potencial de este nuevo fenómeno social para pensar la solidaridad social más allá del Estado nación y rescatar a la Unión Europea de su crisis de legitimidad. En línea con Karl Deutsch, debemos interpretar el transnacionalismo europeo como un proceso de aprendizaje social que nos lleva a una futura identidad supra-nacional. Sin embargo, este proceso necesita ser respaldado por un nuevo modelo de Estado que se asiente sobre un concepto de solidaridad multicultural, expresándose en Europa con un contrato social originario entre distintas naciones europeas. En este sentido debemos prestar más atención sobre recientes estudios que cuestionan la incompatibilidad entre diversidad cultural y solidaridad social.

Abstract

Background: The clinical and economic burden of type 2 diabetes mellitus on society is rising. Effective and efficient preventive measures may stop the increasing prevalence, given that type 2 diabetes mellitus is mainly a lifestyle-driven disease. The Feel4Diabetes-study aimed to tackle unhealthy lifestyle (unhealthy diet, lack of physical activity, sedentary behaviour, and excess weight) of families with a child in the first grades of elementary school. These schools were located in regions with a relatively low socio-economic status in Belgium, Bulgaria, Finland, Greece, Hungary and Spain. Special attention was paid to families with a high risk of developing type 2 diabetes mellitus. Methods: The aim of this paper is to describe the detailed methodology of the intervention’s cost-effectiveness analysis. Based on the health economic evaluation of the Toybox-study, both a decision analytic part and a Markov model have been designed to assess the long-term (time horizon of 70 year with one-year cycles) intervention’s value for money. Data sources used for the calculation of health state incidences, transition probabilities between health states, health state costs, and health state utilities are listed. Intervention-related costs were collected by questionnaires and diaries, and attributed to either all families or high risk families only. Conclusions: The optimal use of limited resources is pivotal. Abstract

Background Glycated hemoglobin (HbA1c) is used to diagnose type 2 diabetes (T2D) and assess glycemic control in patients with diabetes. Previous genome-wide association studies (GWAS) have identified 18 HbA1c-associated genetic variants. These variants proved to be classifiable by their likely biological action as erythrocytic (also associated with erythrocyte traits) or glycemic (associated with other glucose-related traits). In this study, we tested the hypotheses that, in a very large scale GWAS, we would identify more genetic variants associated with HbA1c and that HbA1c variants implicated in erythrocytic biology would affect the diagnostic accuracy of HbA1c. We therefore expanded the number of HbA1c-associated loci and tested the effect of genetic risk-scores comprised of erythrocytic or glycemic variants on incident diabetes prediction and on prevalent diabetes screening performance. Throughout this multiancestry study, we kept a focus on interancestry differences in HbA1c genetics performance that might influence race-ancestry differences in health outcomes. Methods & findings Using genome-wide association meta-analyses in up to 159,940 individuals from 82 cohorts of European, African, East Asian, and South Asian ancestry, we identified 60 common genetic variants associated with HbA1c. We classified variants as implicated in glycemic, erythrocytic, or unclassified biology and tested whether additive genetic scores of erythrocytic variants (GS-E) or glycemic variants (GS-G) were associated with higher T2D incidence in multiethnic longitudinal cohorts (N = 33,241). Nineteen glycemic and 22 erythrocytic variants were associated with HbA1c at genome-wide significance. GS-G was associated with higher T2D risk (incidence OR = 1.05, 95% CI 1.04-1.06, per HbA1c-raising allele, p = 3 x 10.sup.-29 ); whereas GS-E was not (OR = 1.00, 95% CI 0.99-1.01, p = 0.60). In Europeans and Asians, erythrocytic variants in aggregate had only modest effects on the diagnostic accuracy of HbA1c. Yet, in African Americans, the X-linked G6PD G202A variant (T-allele frequency 11%) was associated with an absolute decrease in HbA1c of 0.81%-units (95% CI 0.66-0.96) per allele in hemizygous men, and 0.68%-units (95% CI 0.38-0.97) in homozygous women. The G6PD variant may cause approximately 2% (N = 0.65 million, 95% CI 0.55-0.74) of African American adults with T2D to remain undiagnosed when screened with HbA1c. Limitations include the smaller sample sizes for non-European ancestries and the inability to classify approximately one-third of the variants. Further studies in large multiethnic cohorts with HbA1c, glycemic, and erythrocytic traits are required to better determine the biological action of the unclassified variants. Conclusions As G6PD deficiency can be clinically silent until illness strikes, we recommend investigation of the possible benefits of screening for the G6PD genotype along with using HbA1c to diagnose T2D in populations of African ancestry or groups where G6PD deficiency is common. 20. The cyclical cascade of HIV care: Temporal care engagement trends within a population-wide cohort.

BackgroundThe traditional HIV treatment cascade aims to visualise the journey of each person living with HIV from diagnosis, through initiation on antiretroviral therapy (ART) to treatment success, represented by virological suppression. This representation has been a pivotal tool in highlighting and quantifying sequential gaps along the care continuum. There is longstanding recognition, however, that this may oversimplify the complexity of real-world engagement with HIV services in settings with mature high-burden HIV epidemics. A complementary "cyclical" cascade has been proposed to represent the processes of disengagement at different points on the care continuum, with multiple pathways to re-engagement, although the feasibility of implementing this at scale has been uncertain. This study aimed to populate, refine, and explore the utility of a cyclical representation of the HIV cascade, using routine data from a high-burden HIV setting.Methods and findingsThis observational cohort study leveraged person-level data on all people living with HIV in the Western Cape (WC), South Africa, who accessed public health services in the 2 years prior to 31 December 2023. Programme data from disease registers were complemented by data from pharmacy and laboratory systems. At study closure, 494 370 people were included, constituting 93% of those of those estimated to be living with HIV in the province, of whom 355 104 were on ART. Substantial disengagement from HIV care was evident at every point on the cascade. Early treatment emerged as a period of higher risk of disengagement, but it did not account for the majority of disengagement. Almost all those currently disengaged had prior experience of treatment. While re-engagement was also common, overall treatment coverage had increased slowly over 5 years. The transition to dolutegravir-based regimens was dramatic with good virological outcomes for those in care, notwithstanding a clearly discernible impact of the Coronavirus Disease 2019 (COVID-19) pandemic on viral load (VL) testing. People currently engaged and disengaged in care are similar with respect to age and gender. Those who died or disengaged recently were previously distributed across a range of cascade statuses, and a substantial proportion of those newly initiating and re-initiating treatment were no longer on treatment 6 months later. The main limitation of this study was incomplete evidence of HIV testing, linkage to HIV-specific services, and out-of-facility mortality.ConclusionsUsing routine data, it was possible to populate and automate a cyclical cascade of HIV care that continuously captured the nonlinear care journeys of individuals living with HIV. In this generalised mature HIV epidemic, most people are treatment experienced. Disengagement is common and occurs at various points along the cascade, making it challenging to identify high-impact intervention opportunities. While historical HIV cascades remain valuable for target setting and service monitoring, they can be complemented with insights from more detailed cyclical cascades.

Abstract

At the 6th World Congress on Prevention of Diabetes and its Complications, diabetes prevention experts from science and practice worldwide met in Dresden, Germany. All areas of diabetes prevention were discussed, but most of the presentations, discussions and workshops were dedicated to the practical experiences in diabetes prevention. How to identify people with increased risk and how to establish motivation to change lifestyles were major topics at the meeting. A European practice guideline ‘take action to prevent diabetes’ was presented at the meeting as a result of the large European IMAGE project. As a call for action, the people presented the global network of individuals active in diabetes prevention and called to establish a virtual global prevention center. The aim is to establish the prevention of diabetes in ‘real world’ clinical practice.

25. Reasons for disengagement from antiretroviral care in the era of 'Treat All' in low‐ or middle‐income countries: a systematic review

Abstract Introduction Disengagement from antiretroviral therapy (ART) care is an important reason why people living with HIV do not achieve viral load suppression become unwell. Methods We searched two databases and conference abstracts from January 2015 to December 2022 for studies which reported reasons for disengagement from ART care. We included quantitative (mainly surveys) and qualitative (in‐depth interviews or focus groups) studies conducted after “treat all” or “Option B+” policy adoption. We used an inductive approach to categorize reasons: we report how often reasons were reported in studies and developed a conceptual framework for reasons. Results We identified 21 studies which reported reasons for disengaging from ART care in the “Treat All” era, mostly in African countries: six studies in the general population of persons living with HIV, nine in pregnant or postpartum women and six in selected populations (one each in people who use drugs, isolated indigenous communities, men, women, adolescents and men who have sex with men). Reasons reported were: side effects or other antiretroviral tablet issues (15 studies); lack of perceived benefit of ART (13 studies); psychological, mental health or drug use (13 studies); concerns about stigma or confidentiality (14 studies); lack of social or family support (12 studies); socio‐economic reasons (16 studies); health facility‐related reasons (11 studies); and acute proximal events such as unexpected mobility (12 studies). The most common reasons for disengagement were unexpected events, socio‐economic reasons, ART side effects or lack of perceived benefit of ART. Conceptually, studies described underlying vulnerability factors (individual, interpersonal, structural and healthcare) but that often unexpected proximal events (e.g. unanticipated mobility) acted as the trigger for disengagement to occur. Discussion People disengage from ART care for individual, interpersonal, structural and healthcare reasons, and these reasons overlap and interact with each other. While HIV programmes cannot predict and address all events that may lead to disengagement, an approach that recognizes that such shocks will happen could help. Conclusions Health services should focus on ways to encourage clients to engage with care by making ART services welcoming, person‐centred and more flexible alongside offering adherence interventions, such as counselling and peer support.

IntroductionCareer-related (financial) reasons as well as advantages in terms of expanding social support systems, promoting a balanced lifestyle and personal development suggest that female soccer players should pursue academic or vocational education in parallel to elite sport. However, dual careers are fraught with challenges, mainly due to simultaneity in time and the associated conflicting goals. The aim of this article is to analyze the vocational or academic educational careers of professional female soccer players.MethodsTo generate the data, an online survey was conducted among soccer players in the German Women's Bundesliga and 2nd Women's Bundesliga. A total of n = 200 questionnaires (German: n = 191; English: n = 9) were included in the analysis, which corresponds to approx. 29.6% of the population addressed.Results90.6% of the players are pursuing or have already completed academic or vocational education. The majority (71.2%) of female soccer players choose to study. 81.8% of players report no impact or even a positive impact of soccer on their performance in academic or vocational education. IntroductionThe German military's sports promotion group (SPG) is an entity that aims to minimize the risks inherent in elite sports careers. Risks that manifest themselves, for example, in a lack of financial resources or failed dual careers could undermine the subjective well-being (SWB) of athletes, which can have a negative impact on athletic performance. A differentiated understanding of factors that influence SWB is therefore necessary.MethodsWithin the scope of a survey, a sample of n = 414 current and n = 298 former sports soldiers was generated. Linear regression models (blockwise) were used to analyze the influence of the SPG framework conditions, the influence of top sport-specific factors, and the influence of socio-demographic/socio-economic factors on the SWB of current and former sport soldiers.ResultsCurrent and former sports soldiers showed high satisfaction scores and were also optimistic about the future. Among current sports soldiers, men had higher SWB values. In addition, satisfaction with SPG services, satisfaction with their athletic careers, and satisfaction with their current income all influenced the athletes' SWB. Among former sports soldiers, persons in a stable partnership had higher SWB values. Furthermore, satisfaction with SPG services, a career end that was perceived as involuntary and satisfaction with current income were relevant for SWB in this cohort.DiscussionThis study addresses the call for a more athlete-centered approach in sports science research and the findings have several implications for organized sports to promote SWB in elite athletes.

Abstract

Lifestyle interventions in type 2 diabetes (DM2) prevention implementation studies can be effective and lasting. Long-term weight loss maintenance enhances the intervention effect through a significant decrease in diabetes incidence over time. Our objective was to identify factors predicting long-term successful weight reduction maintenance achieved during a DM2 prevention program in patients with high DM2 risk in primary health care. Study participants (n = 263), middle-aged, slightly obese with baseline increased DM2 risk (Finnish Diabetes Risk Score (FINDRISC)>14), but no diabetes were invited to receive 11 lifestyle counselling sessions, guided physical activity sessions and motivational support during 10-months. The study participants had three clinical examinations during the study (baseline, one and three years). Stepwise regression analysis was used to determine demographic, clinical, and lifestyle predictors of weight reduction maintenance two years after the discontinuation of the intervention. Out of 105 patients who completed all three examinations (baseline age 56.6 (standard deviation (SD) = 10.7), body mass index 31.1 kg/m2 (SD = 4.9), FINDRISC 18.6 (SD = 3.1)), 73 patients (70%) showed weight loss during the intervention (mean weight loss 4.2 kg, SD = 5.1). The total weight loss achieved in the maintainers (27 of 73 study participants) two years after the intervention had finished was 6.54 kg (4.47 kg+2.0 kg). The non-maintainers, on the other hand, returned to their initial weight at the start of the intervention (+0.21 kg). In multivariable analysis baseline history of increased glucose (odds ratio (OR) = 3.7; 95% confidence interval (CI) 1.0-13.6) and reduction of total fat in diet during follow-up (OR = 4.3; 95% CI 1.5-12.2) were independent predictors of successful weight loss. Further studies exploring predictors of weight loss maintenance in diabetes prevention are needed to help health care providers to redesign interventions and improve long-term outcomes of real life interventions.

Abstract

ObjectiveTo describe the design of the Feel4Diabetes-intervention and the baseline characteristics of the study sample.DesignSchool- and community-based intervention with cluster-randomized design, aiming to promote healthy lifestyle and tackle obesity and obesity-related metabolic risk factors for the prevention of type 2 diabetes among families from vulnerable population groups. The intervention was implemented in 2016–2018 and included: (i) the ‘all-families’ component, provided to all children and their families via a school- and community-based intervention; and (ii) an additional component, the ‘high-risk families’ component, provided to high-risk families for diabetes as identified with a discrete manner by the FINDRISC questionnaire, which comprised seven counselling sessions (2016–2017) and a text-messaging intervention (2017–2018) delivered by trained health professionals in out-of-school settings. Although the intervention was adjusted to local needs and contextual circumstances, standardized protocols and procedures were used across all countries for the process, impact, outcome and cost-effectiveness evaluation of the intervention.SettingPrimary schools and municipalities in six European countries.SubjectsFamilies (primary-school children, their parents and grandparents) were recruited from the overall population in low/middle-income countries (Bulgaria, Hungary), from low socio-economic areas in high-income countries (Belgium, Finland) and from countries under austerity measures (Greece, Spain).ResultsThe Feel4Diabetes-intervention reached 30 309 families from 236 primary schools. In total, 20 442 families were screened and 12 193 ‘all families’ and 2230 ‘high-risk families’ were measured at baseline.ConclusionsThe Feel4Diabetes-intervention is expected to provide evidence-based results and key learnings that could guide the design and scaling-up of affordable and potentially cost-effective population-based interventions for the prevention of type 2 diabetes.

This paper deconstructs the argument of a solidarity conditioned by a “primordial social glue” while tracking down the evolution of the normative fundament of the welfare state from a philosophical point of view. Based on the works of GWF Hegel, Lorenz von Stein, Hermann Heller and Thomas Marshall, the paper demonstrates that the social welfare state finds its continuity in a European citizenship that challenges the privileges of national citizenship.

Ullazines and their π-expanded derivatives have gained much attention as active components in various applications, such as in organic photovoltaic cells or as photosensitizers for CO2 photoreduction. Here, we report the divergent synthesis of functionalized diazaullazines by means of two different domino-reactions consisting of either a Povarov/cycloisomerization or alkyne–carbonyl metathesis/cycloisomerization protocol. The corresponding quinolino-diazaullazine and benzoyl-diazaullazine derivatives were obtained in moderate to good yields. Their optical and electronic properties were studied and compared to related, literature-known compounds to obtain insights into the impact of nitrogen doping and π-expansion.

Das IMAGE-Toolkit zur Diabetespravention ist eines der Produkte einer europaischen Studiengruppe des IMAGE-Projekts. Es basiert auf der IMAGE-evidenzbasierten Leitlinie und dem IMAGE-Training-Curriculum fur Praventionsmanager. Es enthalt praktische Beispiele und Arbeitsblatter. Diese Arbeitsblatter erleichtern die Umsetzung eines Praventionsprogramms erheblich und konnen direkt vom Toolkit ubernommen werden. Nachdem im Rahmen des IMAGE-Projekts ein europaisches Curriculum fur den Praventionsmanager erarbeitet wurde, konnte dieses in den letzten 2 Jahren in Deutschland angewendet und intensiv evaluiert werden. Seit Anfang 2013 steht ein deutsches Curriculum fur die Weiterbildung zum Praventionsmanager zur Verfugung; diese kann derzeit in Dresden wahrgenommen werden. Verschiedene Krankenkassen setzen auf dieses Modell und wollen mit der Verknupfung von Pravention und Versorgung verstarkt Praventionsmasnahmen anwenden. Der Check-up 35+ kann eine solche Masnahme sein. Uber diesen und weitere Elemente in der Pravention wird im vorliegenden Beitrag berichtet.

The question as to how to screen diabetes risk in a multi-ethnic population is not easy to answer. There are a number of diagnostic procedures and risk score tools which may help identify people with increased risk. Some of the risk factors for diabetes have a clear ethnic component, thus the risk stratification is different in Caucasian, Asian and Latin American populations. However, we can expect that the pathophysiology for diabetes development consisting of insulin resistance and progressive beta-cell failure is very similar in its pathomechanistic background between ethnic groups, although the speed and progressive destruction may have ethnic and varying genetic components. In this environment, we have to find clinically applicable approaches to identify those with increased diabetes risk which have to be easy to understand, transparent and replicable for diabetes risk detection. Abstract Introduction Long‐acting and extended delivery (LAED) regimens for HIV treatment and prevention offer unique benefits to expand uptake, effective use and adherence. To date, research has focused on basic and clinical science around the safety and efficacy of these products. This commentary outlines opportunities in HIV prevention and treatment programmes, both for the health system and clients, that could be addressed through the inclusion of LAED regimens and the vital role of differentiated service delivery (DSD) in ensuring efficient and equitable access. Discussion The realities and challenges within HIV treatment and prevention programmes are different. Globally, more than 28 million people are accessing HIV treatment—the vast majority on a daily fixed‐dose combination oral pill that is largely available, affordable and well‐tolerated. Many people collect extended refills outside of health facilities with clinical consultations once or twice a year. Conversely, uptake of daily oral pre‐exposure prophylaxis (PrEP) has consistently missed global targets due to limited access with high individual cost and lack of choice contributing to substantial unmet PrEP need. Recent trends in demedicalization, simplification, additional method options and DSD for PrEP have led to accelerated uptake as its availability has become more aligned with user preferences. How people currently receive HIV treatment and prevention services and their barriers to adherence must be considered for the introduction of LAED regimens to achieve the expected improvements in access and outcomes. Important considerations include the building blocks of DSD: who (provider), where (location), when (frequency) and what (package of services). Ideally, all LAED regimens will leverage DSD models that emphasize access at the community level and self‐management. For treatment, LAED regimens may address challenges with adherence but their delivery should provide clear advantages over existing oral products to be scaled. For prevention, LAED regimens expand a potential PrEP user's choice of methods, but like other methods, need to be delivered in a manner that can facilitate frequent re‐initiation. Conclusions To ensure that innovative LAED HIV treatment and prevention products reach those who most stand to benefit, service delivery and client considerations during development, trial and early implementation are critical.

Type 2 diabetes and other noncommunicable diseases are a growing public health challenge globally. An estimated 285 million people, corresponding to 6.4% of the world’s adult population, has diabetes, which is expected to reach 552 million by the International Diabetes Federation in 2030. A much larger segment of the world’s population, approximating 79 million individuals in the USA alone, has prediabetes. Globally, a relatively small percentage of those with diabetes or prediabetes are diagnosed with the potential for developing chronic complications. To address this epidemic, governments, in concert with the private sector, need to set policies that promote healthy nutritional and agricultural policies, favor modifications in the environment that encourage greater physical activity and make prevention affordable for all citizens at high risk. The public health sector has the charge of translating evidence-based findings into practical, accessible and cost-effective programs and monitoring the process to continuously improve prevention initiatives. The clinical sector has the formidable challenge of screening and identifying those at high risk and referring them to accredited intervention programs. There is a need to explore additional cost-effective interventions that are customized to meet individual needs that can be offered at the community and clinical levels. Thus, all three sectors, government, public health and clinical, each have a critical role in this process and by working in a partnership, ought to create the necessary synergies essential for making substantial forays in the prevention of Type 2 diabetes.

Vieles im Bereich der Pravention des Diabetes hat sich im letzten Jahr um eine politische Diskussion gerankt, wie Masnahmen zur Pravention des Diabetes in die Versorgung implementiert werden konnen. Wir erleben jetzt, dass die Translation wissenschaftlicher Erkenntnisse in die Praxis ein muhsamer, schleichender Prozess ist, den wir aber weiterverfolgen mussen. In dieser Situation leisteten die Praxisergebnisse aus dem IMAGE Projekt eine grose Hilfestellung. So wurde zum Beispiel die Ausbildung zum Praventionsmanager mit europaischer Unterstutzung in Deutschland institutionalisiert. Die Evidenz fur die Diabetespravention ist hoch und auch entsprechende Praxisempfehlungen sind umfassend vorhanden. Vor einigen Wochen hat das NICE (National Institute for Health and Clinical Excellence, England) eine Leitlinie zur Pravention des Diabetes herausgebracht und stellt dar, dass die Pravention des Diabetes in allen Altersgruppen kostensparend und kosteneffektiv ist. Trotz dieser guten Evidenz stehen wir mehr denn je vor der Herausforderung, die Pravention in wirtschaftlich umsetzbare Praxismodelle zu etablieren. Gleichzeitig stehen wir vor einer Diskussion uber Verantwortlichkeiten – in wessen Verantwortungsbereich soll die Pravention des Diabetes etabliert werden? Ist das eine medizinische Leistung oder nicht? Wir wollen Sie gerne motivieren, an dieser Diskussion teilzunehmen. Eine Moglichkeit dazu ist der ”Global Diabetes Survey“, den wir Ihnen am Ende des Artikels vorstellen.

Key to putting prevention of diabetes into practice is finding the people with increased risk. Several tools are currently in use: oral glucose tolerance test, fasting glucose measurement and a number of questionnaires to identify those with increased risk. Each has its own advantages and disadvantages. One new tool that can identify those with increased diabetes risk is the EZSCAN™. This new diagnostic device developed by Impeto Medical uses the sweat gland function to detect risk for insulin resistance and diabetes. The basic pathophysiology behind this technology is supported by the growing number of clinical studies worldwide which show a strong association between small nerve neuropathies to insulin resistance and diabetes risk. Because the EZSCAN™ test takes only three minutes to run, is noninvasive and easy to operate, it is an ideal diagnostic tool for both the medical and paramedical setting. Several applications are possible: the EZSCAN™ can be used to monitor insulin resistance-based treatment, to diagnose increased diabetes risk and to aid proposing diabetes prevention programmes. Healthy nutrition is the driving force behind measures to address the obesity and diabetes epidemic. Therefore, the relevance of healthy nutrition to public health is steadily increasing. Although many consumers have positive attitudes towards healthy nutrition, their eating habits do not always match these attitudes. This paper suggests that individuals can overcome the discrepancy between attitude/intention and behaviour if they consider implicit associations with healthy or unhealthy nutrition. Our paper aims to develop a research strategy to analyse the impact of the interplay between explicit and implicit food associations on food consumption. The suggested process comprises the following steps: consumer segmentation, isolated analysis of different implicit drivers, holistic examination of interaction effects of different drivers and development of implications for policy makers and managers. Abstract

Current evidence shows that type 2 diabetes can be effectively prevented and delayed by lifestyle modification. After careful development and pilot testing, a rigorous evaluation of such programmes is crucial to assess their effectiveness prior to large scale implementation. Integrating the patient perspective in this evaluation is essential. In principle, Patient-Reported Outcomes (PRO) questionnaires can address this key question. However, researchers and clinicians lack a clear framework to make a sound selection of appropriate measures among the large set of possible questionnaires. As a consequence, it is unclear what is the most useful information to be captured from patients when assessing a new programme, so that it can be determined whether the programme can be generalised to a larger population: is it health-related quality of life, satisfaction or something different which could predict the long-term success of the programme? We recommend a specific approach, better adapted to the nature of the intervention, and suggest a selection of a few existing PRO measures that could satisfy the requirements.

Abstract

The rising prevalence of type 2 diabetes is having an impact on limited medical and economic resources making increased efficiency and effectiveness of medical care and preventative strategies imperative. Quality management indicators that are fit for purpose will permit improvements and shortcomings to be identified in the strategies being employed and may provide information on how to better achieve objectives. This article considers two approaches to how indicators, which were developed by the Implementation of A European Guideline and training standards for diabetes prevention project, can be used in clinically applicable quality management, including benchmarking, in order to compare the quality of work among prevention managers and to develop an overall benchmark for preventive intervention. Abstract

SUMMARY The Implementation of A European Guideline and Training Standards for Diabetes Prevention (IMAGE) guidelines are structured standards for diabetes prevention. The IMAGE Toolkit, which is based on these standards, gives useful recommendations for preventive care programs. In brief, based on systematic reviewing of the evidence, the guidelines recommend that case-finding should follow a step-wise procedure using risk questionnaires and oral glucose tolerance testing. Interventions supporting lifestyle changes should be supported by intersectoral strategies that create health-promoting environments. In order to implement the guidelines and to advance international correspondence, a web-based network known as ‘who is active in prevention’ was established, which facilitates the exchange of knowledge and experience in the field of prevention.

Abstract

It has been shown that real-life implementation studies for the prevention of type 2 diabetes (DM2) performed in different settings and populations can be effective. However, not enough information is available on factors influencing the reach of DM2 prevention programmes. This study examines the predictors of completing an intervention programme targeted at people at high risk of DM2 in Krakow, Poland as part of the DE-PLAN project. A total of 262 middle-aged people, everyday patients of 9 general practitioners' (GP) practices, at high risk of DM2 (Finnish Diabetes Risk Score (FINDRISK)>14) agreed to participate in the lifestyle intervention to prevent DM2. Intervention consisted of 11 lifestyle counseling sessions, organized physical activity sessions followed by motivational phone calls and letters. Measurements were performed at baseline and 1 year after the initiation of the intervention. Seventy percent of the study participants enrolled completed the core curriculum (n=184), 22% were men. Abstract

Die praktische Umsetzung einer wirksamen Pravention des Diabetes mellitus Typ 2 stellt fur das Gesundheitswesen der Bundesrepublik Deutschland eine zentrale Herausforderung dar. Die Evidenz fur die Effektivitat und Effizienz der Diabetespravention ist sehr hoch. Verschiedene gesundheitsokonomische und gesundheitspolitische Untersuchungen sehen in einer erfolgreichen Pravention des Diabetes die einzige Moglichkeit, die Gesundheit der Bevolkerung und die Finanzierung des Gesundheitswesens nachhaltig zu sichern. Die Evidenz in der Diabetespravention stammt vorwiegend aus klinisch kontrollierten Studien. Die Herausforderung, die sich stellt, besteht im Transfer des in den Studien betriebenen Aufwands in den konkreten klinischen Alltag und in der adaquaten strukturellen und inhaltlichen Implementierung dieser Masnahmen mit vergleichbaren Ergebnissen. Dazu ist das Verstandnis der pathophysiologischen Prozesse der Diabetesentstehung, aber auch der pathophysiologischen Grundlagen praventiver Prozesse entscheidend. Gleichzeitig sind regionale und nationale strukturelle Aspekte in der Gesundheitsversorgung und gesundheitsokonomische Gegebenheiten entscheidende Rahmenbedingungen fur die Implementierung eines neuartigen Praventionsprogramms. Eine effiziente und effektive Pravention des Diabetes mellitus Typ 2 ist realisierbar. Das fachliche „Know-how“ und die Basis fur eine Vernetzung der unterschiedlichen Akteure zur Pravention des Diabetes mellitus Typ 2 und des metabolischen Syndroms sind in Deutschland vorhanden. Eine flachendeckende und im Ergebnis erfolgreiche Pravention stellt ein realistisches Ziel dar und die Voraussetzungen in Deutschland und auch der Europaischen Union sind ausgesprochen gut.

Der beste Weg, Diabetes mellitus zu heilen ist, ihn zu verhindern. Mehrere grose internationale Studien belegen, dass die Pravention des Diabetes mellitus und seiner Begleitkomplikationen erfolgreich durchfuhrbar und kosteneffektiv ist. Danach haben Lebensstilanderungen oder fruhe medikamentose Interventionen bei 25–60% der Risikopersonen einen diabetespraventiven Effekt und reduzieren mit groser Wahrscheinlichkeit auch das kardiovaskulare Risiko. Diese Studien bilden die Evidenzbasis, entscheidend aber ist die praktische und flachendeckende Umsetzung in Programme zur Diabetespravention, anerkannt als Leistung der gesetzlichen Krankenkassen. Die Realisierung eines solchen Vorhabens erfordert viele Partner, die interdisziplinar und berufsgruppenubergreifend kooperieren. Daher wurden unter der Schirmherrschaft des Nationalen Aktionsforums Diabetes mellitus (NAFDM) wesentliche Voraussetzungen fur ein nationales Praventionsprogramm erarbeitet. Erforderlich ist die Implementierung einer Leitlinie zur Diabetespravention, die Ziele und Standards fur konkrete Interventionsmasnahmen sowie Evaluationskriterien beinhaltet. Eine entscheidende Rolle kommt in diesem Konzept dem Praventionsmanager und einer prozessbegleitenden Qualitatskontrolle zu. Die Implementierung strukturierter Praventionsmanagementprogramme ermoglicht eine flachendeckende Realisierung der Diabetespravention, ohne das Gesundheitsbudget stark zu belasten. Sich der Herausforderung der Primarpravention zu stellen, erfordert die aktive Zusammenarbeit aller Partner und ist nur langfristig erfolgreich umzusetzen. Dies stellt allerdings ein lohnendes Investment fur die Betroffenen, die klinische und wissenschaftliche Diabetologie und das Gesundheitswesen in Deutschland dar.

ZusammenfassungWir stehen wir nach wie vor – bedingt durch die Zunahme der Zahl von Patienten mit Diabetes mellitus Typ 2 (T2DM) – vor einem schwerwiegenden medizinischen, sozialen und ökonomischen Problem. Es ist dringend an der Zeit, Programme zur Primärprävention und gezielten Früherkennung des Diabetes mellitus umzusetzen. Mehrere große internationale Studien belegen, dass die Prävention des Diabetes mellitus durch Lebensstiländerung bei Risikopersonen sowie einer frühen medikamentösen Intervention möglich und erfolgreich ist. Primäres Ziel ist die Stabilisierung und Verbesserung der Glukosetoleranz über eine Verbesserung der Insulinresistenz. Zielwerte zur Umsetzung entsprechender Programme können genau formuliert werden. Damit ist eine der großen Herausforderungen der Diabetologie unserer Zeit die praktische Umsetzung von Programmen zur Diabetes-Prävention.Die Arbeitsgruppe Prävention der Dresdner Medizinischen Klinik III hat gemeinsam mit der Arbeitsgemeinschaft Prävention des Typ-2-Diabetes der Deutschen Diabetes-Gesellschaft (AG P2) und des Nationalen Aktionsforum Diabetes mellitus (NAFDM) sowie der Deutschen Diabetes-Stiftung (DDS) ein Konzept für ein nationales Präventionsprogramm erarbeitet. Ein dreischrittiges Programmkonzept wurde entwickelt: Erster Schritt ist die Identifikation der Risikopersonen durch Screening, zweiter Schritt eine Intervention zur Diabetes-Prävention durch Schulung, Training und Motivation. Der dritte, für die Nachhaltigkeit entscheidende Schritt ist die kontinuierliche Weiterbetreuung. Für die praktische Umsetzung wurde ein Leitfaden zur Diabetes-Prävention erarbeitet, der Ziele und Maßnahmen der einzelnen Programmschritte spezifiziert. Zudem sieht das im Leitfaden vorgestellte Konzept eine zentrale Rolle für den Präventionsmanager vor und beinhaltet Empfehlungen für eine zentrale Evaluation und Qualitätskontrolle.

Abstract

Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. Abstract

Abstract Background There is no consensus on the use of perioperative antibiotic in full-thickness rectal biopsies (FTB) in children suspected for Hirschsprung disease. The primary objective of this study was to examine the effect of perioperative antibiotics on infectious and overall complications rate in FTB performed in children under the age of 1 year. Methods A retrospective chart review. Two time periods were compared—one with and one without the routine use of perioperative antibiotics. The treatment included cefuroxime 100 mg/kg and metronidazole 20 mg/kg administered intravenously at anesthesia induction followed by peroral administration of amoxicillin 50 mg/kg with Clavulanic acid 12.5 mg/kg three times a day t.i.d. for 3 days postoperative. Results In the group with perioperative antibiotics the infectious complications rate was 3.3% compared to 13.4% in the group without (p = 0.03) with fever as the most common. The overall complication rate was 11.3% and 15.2%, respectively (p = 0.43). Conclusion Perioperative antibiotics significantly reduced the rate of infectious complications, but not the rate of overall complication rate after a full thickness rectal biopsy in children under the age of 1 year.
