In the current and rapidly evolving era of real-time MRI-guided radiotherapy, our radiation biology and dosimetry knowledge is being tested in a novel way. This paper presents the successful design and implementation of a portable device used to generate strong localized magnetic fields. These are ideally suited for small-scale experiments that mimic the magnetic field environment inside an MRI-linac system, or more broadly MRI-guided particle therapy as well.A portable permanent magnet-based device employing an adjustable steel yoke and magnetic field focusing cones has been designed, constructed, and tested. The apparatus utilizes two banks of NdA mostly uniform magnetic field of 1.2 T can be achieved over a volume of at least 3A portable, relatively inexpensive, and simple to operate device has successfully been constructed and used for performing radiation oncology studies around the theme of MRI-guided radiotherapy. This can be in either inline and perpendicular magnetic fields of up to 1.2 T with x-ray and particle beams.

Purpose To determine baseline accuracy and reproducibility of T1 and T2 relaxation times over 12 months on a dedicated radiotherapy MRI scanner. Methods An International Society of Magnetic Resonance in Medicine/National Institute of Standards and Technology (ISMRM/NIST) System Phantom was scanned monthly on a 3T MRI scanner for 1 year. T1 was measured using inversion recovery (T1‐IR) and variable flip angle (T1‐VFA) sequences and T2 was measured using a multi‐echo spin echo (T2‐SE) sequence. For each vial in the phantom, accuracy errors (%bias) were determined by the relative differences in measured T1 and T2 times compared to reference values. Reproducibility was measured by the coefficient of variation (CV) of T1 and T2 measurements across monthly scans. Accuracy and reproducibility were mainly assessed on vials with relaxation times expected to be in physiological ranges at 3T. Results A strong linear correlation between measured and reference relaxation times was found for all sequences tested (R 2 > 0.997). Baseline bias (and CV[%]) for T1‐IR, T1‐VFA and T2‐SE sequences were +2.0% (2.1), +6.5% (4.2), and +8.5% (1.9), respectively. Conclusions The accuracy and reproducibility of T1 and T2 on the scanner were considered sufficient for the sequences tested. No longitudinal trends of variation were deduced, suggesting less frequent measurements are required following the establishment of baselines.

Purpose Stereotactic radiosurgery (SRS) can be delivered with a standard linear accelerator (linac). At institutions having more than one linac, beam matching is common practice. In the literature, there are indications that machine central axis (CAX) matching for broad fields does not guarantee matching of small fields with side ≤2 cm. There is no indication on how matching for broad fields on axis translates to matching small fields off axis. These are of interest to multitarget single‐isocenter (MTSI) SRS planning and the present work addresses that gap in the literature. Methods We used 6 MV flattening filter free (FFF) beams from four Elekta VersaHD® linacs equipped with an Agility™ multileaf collimator (MLC). The linacs were strictly matched for broad fields on CAX. We compared output factors (OPFs) and effective field size, measured concurrently using a novel 2D solid‐state dosimeter “Duo” with a spatial resolution of 0.2 mm, in square and rectangular static fields with sides from 0.5 to 2 cm, either on axis or away from it by 5 to 15 cm. Results Among the four linacs, OPF for fields ≥1 × 1 cm2 ranged 1.3% on CAX, whereas off axis a maximum range of 1.9% was observed at 15 cm. A larger variability in OPF was noted for the 0.5 × 0.5 cm2 field, with a range of 5.9% on CAX, which improved to a maximum of 2.3% moving off axis. Two linacs showed greater consistency with a range of 1.4% on CAX and 2.2% at 15 cm off axis. Between linacs, the effective field size varied by

Purpose: Gantry-free radiation therapy systems utilizing patient rotation would be simpler and more cost effective than the conventional gantry-based systems. Such a system could enable the expansion of radiation therapy to meet global demand and reduce capital costs. Recent advances in adaptive radiation therapy could potentially be applied to correct for gravitational deformation during horizontal patient rotation. This study aims to quantify the pelvic organ motion and the dosimetric implications of horizontal rotation for prostate intensity-modulated radiation therapy (IMRT) treatments. Methods: Eight human participants who previously received prostate radiation therapy were imaged in a clinical magnetic resonance imaging (MRI) scanner using a bespoke patient rotation system (PRS). The patients were imaged every 45 degrees during a full roll rotation (0-360 degrees). Whole pelvic bone, prostate, rectum, and bladder motion were compared to the supine position using dice similarity coefficient (DSC) and mean absolute surface distance (MASD). Prostate centroid motion was compared in the left-right (LR), superior-inferior (SI), and anterior-posterior (AP) direction prior to and following pelvic bone-guided rigid registration. Seven-field prostate IMRT treatment plans were generated for each patient rotation angles under three adaption scenarios: No plan adaption, rigid planning target volume (PTV)-guided alignment to the prostate, and plan re-optimization. Prostate, rectum, and bladder doses were compared for each adaption scenario. Results: Pelvic bone motion within the PRS of up to 53 mm relative to the supine position was observed for some participants. Internal organ motion was greatest at the 180-degree PRS couch angle (prone), with prostate centroid motion range < 2 mm LR, 0 mm to 14 mm SI, and -11 mm to 4 mm AP. Rotation with no adaption of the treatment plan resulted in an underdose to the PTV -- in some instances up to 75% (D95%: 78 ± 0.3 Gy at supine to 20 ± 15.0 Gy at the 225-degree PRS couch angle). Bladder dose was reduced during the rotation by up to 98% (V60 Gy: 15.0 ± 9.4% supine to 0.3 ± 0.5% at the 225-degree PRS couch angle). In some instances, the rectum dose increased during rotation (V60Gy: 20.0 ± 4.5% supine to 25.0 ± 15.0% at the 135-degree PRS couch angle). Rigid PTV-guided alignment resulted in PTV coverage which, though statistically lower (P < 0.05 for all D95% values), was within 1 Gy of the supine plans. Plan re-optimization resulted in a statistically equivalent PTV coverage compared to the supine plans (P > 0.05 for all D95% metrics and all within ±0.4 Gy). For both rigid PTV-guided alignment and plan re-optimization, rectum dose volume metrics were reduced compared to the supine position between the 90- and 225-degree PRS couch angles (P < 0.05). Bladder dose volume metrics were not impacted by rotation. Conclusion: Pelvic bone and internal organ motion are present during patient rotation. Rigid PTV-guided alignment to the prostate will be a requirement if prostate IMRT is to be safely delivered using patient rotation. Plan re-optimization for each PRS couch angle to account for anatomical deformations further improves the PTV coverage.

Cone-beam computed tomography (CBCT) is an important imaging modality for image-guided radiotherapy and adaptive radiotherapy. Feldkamp-Davis-Kress (FDK) method is widely adopted in clinical CBCT reconstructions due to its fast and robust application. While iterative algorithms have been shown to outperform FDK techniques in reducing noise and imaging dose, they are unable to correct projection-domain artefacts such as beam hardening and scatter. Empirical correction techniques require a holistic approach as beam hardening and scatter coexist in the measurement data. This multi-part proof of concept study conducted in MATLAB presents a novel approach to artefact reduction for CBCT image reconstruction. Firstly, we decoupled the beam hardening and scatter contributions originating from the imaging object and the bowtie filter. Next, a model was constructed to apply pixel-wise corrections to separately account for artefacts induced by the imaging object and the bowtie filter, in order to produce mono-energetic equivalent and scatter-compensated projections. Finally, the effectiveness of the correction model was tested on an offset phantom scan as well as a clinical brain scan. A conjugate-gradient least-squares algorithm was implemented over five iterations using FDK result as the initial input. Our proposed correction model was shown to effectively reduce cupping and shading artefacts in both phantom and clinical studies. This simple yet effective correction model could be readily implemented by physicists seeking to explore the benefits of iterative reconstruction.

The aim of this study is to investigate off-axis irradiation on the Australian MRI-Linac using experiments and Monte Carlo simulations. Simulations are used to verify experimental measurements and to determine the minimum offset distance required to separate electron contamination from the photon field.Dosimetric measurements were performed using a microDiamond detector, GafchromicOff-axis irradiation separates the majority of electron contamination from an x-ray beam and was found to significantly reduce in-field surface dose. For the 1.9 × 1.9, 5.8 × 5.8, and 9.7 × 9.6 cmExperimental and simulation data were acquired for a range of field sizes to investigate off-axis irradiation on an inline MRI-Linac. The skin sparing effect was observed with off-axis irradiation, a feature that cannot be achieved to the same extent with other methods, such as bolusing, for beams at isocenter.

PURPOSE To report on experimental results of a high spatial resolution silicon-based detector exposed to therapeutic quality proton beams in a 0.95 T transverse magnetic field. These experimental results are important for the development of accurate and novel dosimetry methods in future potential real-time MRI-guided proton therapy systems. METHODS A permanent magnet device was utilized to generate a 0.95 T magnetic field over a 4 × 20 × 15 cm3 volume. Within this volume, a high-resolution silicon diode array detector was positioned inside a PMMA phantom of 4 × 15 × 12 cm3 . This detector contains two orthogonal strips containing 505 sensitive volumes spaced at 0.2 mm apart. Proton beams collimated to a circle of 10 mm diameter with nominal energies of 90 MeV, 110 MeV, and 125 MeV were incident on the detector from an edge-on orientation. This allows for a measurement of the Bragg peak at 0.2 mm spatial resolution in both the depth and lateral profile directions. The impact of the magnetic field on the proton beams, that is, a small deflection was also investigated. A Geant4 Monte Carlo simulation was performed of the experimental setup to aid in interpretation of the results. RESULTS The nominal Bragg peak for each proton energy was successfully observed with a 0.2 mm spatial resolution in the 0.95 T transverse magnetic field in both a depth and lateral profiles. The proton beam deflection (at 0.95 T) was a consistent 2 ±0.5 mm at the center of the magnetic volume for each beam energy. However, a pristine Bragg peak was not observed for each energy. This was caused by the detector packaging having small air gaps between layers of the phantom material surrounding the diode array. These air gaps act to degrade the shape of the Bragg peak, and further to this, the nonwater equivalent silicon chip acts to separate the Bragg peak into multiple peaks depending on the proton path taken. Overall, a promising performance of the silicon detector array was observed, however, with a qualitative assessment rather than a robust quantitative dosimetric evaluation at this stage of development. CONCLUSIONS For the first time, a high-resolution silicon-based radiation detector has been used to measure proton beam Bragg peak deflections in a phantom due to a strong magnetic field. Future efforts are required to optimize the detector packaging to strengthen the robustness of the dosimetric quantities obtained from the detector. Such high-resolution silicon diode arrays may be useful in future efforts in MRI-guided proton therapy research.

PURPOSE The fringe field of the Australian MRI-linac causes contaminant electrons to be focused along the central axis resulting in a high surface dose. This work aims to characterize this effect using Gafchromic film and high-resolution detectors, MOSkinTM and microDiamond. The secondary aim is to investigate the influence of the inline magnetic field on the relative dose response of these detectors. METHODS The Australian MRI-linac has the unique feature that the linac is mounted on rails allowing for measurements to be performed at different magnetic field strengths while maintaining a constant source-to-surface distance (SSD). Percentage depth doses (PDD) were collected at SSD 1.82 m in a solid water phantom positioned in a low magnetic field region and then at isocenter of the MRI where the magnetic field is 1 T. Measurements for a range of field sizes were taken with the MOSkinTM , microDiamond, and Gafchromic® EBT3 film. The detectors' relative responses at 1 T were compared to the near 0 T PDD beyond the region of electron contamination, that is, 20 mm depth. The near surface measurements inside the MRI bore were compared among the different detectors. RESULTS Skin dose in the MRI, as measured with the MOSkinTM , was 104.5% for 2.1 × 1.9 cm2 , 185.6% for 6.1 × 5.8 cm2 , 369.1% for 11.8 × 11.5 cm2 , and 711.1% for 23.5 × 23 cm2 . The detector measurements beyond the electron contamination region showed agreement between the relative response at 1 T and near 0 T. Film was in agreement with both detectors in this region further demonstrating their relative response is unaffected by the magnetic field. CONCLUSIONS Experimental characterization of the high electron contamination at the surface was performed for a range of field sizes. The relative response of MOSkinTM and microDiamond detectors, beyond the electron contamination region, were confirmed to be unaffected by the 1-T inline magnetic field.

Purpose Magnetic field effects on dose distribution and detector functionality must be well understood. The detector utilized to investigate these magnetic field effects was the DUO silicon array detector; the performance of this high spatial resolution detector was assessed under these conditions. The results were compared to Gafchromic EBT3 film to highlight any intrinsic magnetic field effects in the silicon. The results were also compared to previously published MagicPlate-512 (M512) data. The DUO has an improved spatial resolution (200 µm) over the M512 (2 mm). Methods A permanent magnet named Magnetic Apparatus for RaDiation Oncology Studies (MARDOS) paired with a standard linear accelerator (linac) enables either transverse (1.2 T) or inline (0.95 T) orientations of the magnetic field with respect to the radiation beam. A 6 MV Varian 2100C Linac provided the radiation component for the measurements. The DUO detector has 505 sensitive volumes (each volume measuring 800 × 40 × 100 µm3 ) organized in two orthogonal, linear arrays. The DUO was embedded in a solid water phantom in the first set-up and then a solid lung phantom in the second set-up and placed between the magnet cones. Beam profiles were compared under the magnetic field conditions and 0 T. Small field sizes from 0.8 × 0.8 cm2 up to 2.3 × 2.3 cm2 were investigated. The size of the air gap above the sensitive volumes of the DUO was investigated in the transverse orientation to assess the anticipated magnetic field effects. Full width at half maximum (FWHM), 80-20% penumbral widths and maximum dose differences between detectors and between the presence/absence of a magnetic field were investigated. Symmetry was also assessed for investigation of profile skewness under the transverse field. Results The penumbral widths measured by the DUO detector demonstrated good agreement with film and the M512 to within an average of 0.5 mm (within uncertainty: ±1 mm). The static inline magnetic field had minimal effect on the profiles in solid water. As expected, the lower density of solid lung meant that this material was more susceptible to demonstrating magnetic field effects in the dose deposited. The greatest penumbral narrowing due to the inline field (0.7 mm) occurred in lung. Central axis dose increase was greatest in lung (maximum: 9%). The transverse field widened penumbra, most notably in the solid lung phantom, by a maximum of 2.3 mm. The largest asymmetry due to the transverse field (4.6%) was also in solid lung. When the air gap above the DUO was filled with bolus, the dose maximum measured by the DUO was within 1.4% of film. Conclusions The DUO detector has been shown to be successful in accurately describing the dose changes for small field sizes to within a 200-µm resolution in an environment resembling that of an MRI-linac. The DUO measurements were in agreement with both film and the M512 measurements, and therefore the DUO was found to be an appropriate alternative to the M512, with improvement in terms of its higher spatial resolution. MARDOS provided a suitable environment for these preliminary tests before progressing to the MRI-linac.

Respiratory motion has a significant impact on dose delivered to abdominal targets during radiotherapy treatment. Accurate treatment of liver tumours adjacent to the diaphragm is complicated by large respiratory movement, as well as differing tissue densities at the lung-liver interface. This study aims to evaluate the accuracy of dose delivered to superior liver tumours using passive respiratory monitoring, in the absence of gating technology, for a range of treatment techniques. An in-house respiratory phantom was designed and constructed to simulate the lung and liver anatomy. The phantom consisted of adjacent slabs of lung and liver equivalent materials and a cam drive system to emulate respiratory motion. A CC04 ionisation chamber and Gafchromic EBT3 film were used to perform point dose and dose plane measurements respectively. Plans were calculated using an Elekta Monaco treatment planning system (TPS) on exhale phase study sets for conformal, volume modulated arc therapy (VMAT) and intensity modulated radiation therapy (IMRT) techniques, with breathing rates of 8, 14 and 23 bpm. Analysis confirmed the conformal delivery protocol currently used for this site within the department is suitable. The experiments also determined that VMAT is a viable alternative technique for treatment of superior liver lesions undergoing respiratory motion and was superior to IMRT. Furthermore, the measurements highlighted the need for respiratory management in these cases. Displacements due to respiration exceeding planned margins could result in reduced coverage of the clinical target volume and much higher doses to the lung than anticipated.

A family of prototype 2D monolithic silicon-diode array detectors (MP512, Duo, Octa) has been proposed by the Centre for Medical Radiation Physics, University of Wollongong (Australia) for relative dosimetry in small megavoltage photon beams. These detectors, which differ in the topology of their 512 sensitive volumes, were originally fabricated on bulk p-type substrates. More recently, they have also been fabricated on epitaxial p-type substrates. In the literature, their performance has been individually characterized for quality assurance (QA) applications. The present study directly assessed and compared that of a MP512-bulk and that of a MP512-epitaxial in terms of radiation hardness, long-term stability, response linearity with dose, dose per pulse and angular dependence. Their measurements of output factors, off-axis ratios and percentage depth doses in square radiation fields collimated by the jaws and produced by 6 MV and 10 MV flattened photon beams were then benchmarked against those by commercially available detectors. The present investigation was aimed at establishing, from a medical physicist's perspective, how the bulk and epitaxial fabrication technologies would affect the implementation of the MP512s into a QA protocol. Based on results, the MP512-epitaxial would offer superior radiation hardness, long-term stability and achievable uniformity and reproducibility of the response across the 2D active area.

Purpose Cone-beam computed tomography (CBCT) is increasingly utilized in radiation therapy for image guidance and adaptive applications. While iterative reconstruction algorithms have been shown to outperform traditional filtered back-projection methods in improving image quality and reducing imaging dose, they cannot handle data truncation in the axial view, which frequently occurs in the full-fan partial-trajectory acquisition mode. This proof-of-concept study presents a novel approach on truncation artifact reduction by utilizing a priori preconditioned information as the initial input for the iterative algorithm. Methods Projections containing axial truncation were used for image reconstruction in extended axial field-of-view (AFOV) using the conjugate gradient least-squares (CGLS) algorithm. A priori information in the form of a planning fan-beam CT (FBCT) was repositioned in the expected CBCT imaging geometry, then further processed to dampen high-density features and convolved with a cubic Gaussian kernel to ensure differentiability for the gradient descent method. Anatomical and positional differences between the estimated and the actual imaging object were introduced to verify the efficacy of the proposed method. Results Extending the reconstruction AFOV alone could partially reduce truncation artifact. Using a priori information directly resulted in ghosting artifact when there were anatomical and positional differences between the estimated and the actual imaging object. Using a priori preconditioned information was shown to effectively reduce truncation artifact and recover peripheral information. Conclusions Using a priori preconditioned information can effectively alleviate truncation artifact and assist recovery of peripheral information in iterative CBCT reconstruction.

Introduction: Recent advances in image guidance and adaptive radiotherapy could enable gantry-free radiotherapy using patient rotation. Gantry-free radiotherapy could substantially reduce the cost of radiotherapy systems and facilities. MRI guidance complements a gantry-free approach because of its ability to visualise soft tissue deformation during rotation. A potential barrier to gantry-free radiotherapy is patient acceptability, especially when combined with MRI. This study investigates human experiences of horizontal rotation within an MRI scanner. Methods: Ten healthy human participants and nine participants previously treated with radiotherapy were rotated within an MRI scanner. Participants' anxiety and motion sickness was assessed before being rotated in 45-degree increments and paused, representing a multi-field intensity-modulated radiotherapy treatment. An MR image was acquired at each 45-degree angle. Following imaging, anxiety and motion sickness were re-assessed, followed by a comfort questionnaire and exit interview. The significance of the differences in anxiety and motion sickness pre- versus post-imaging was assessed using Wilcoxon signed-rank tests. Content analysis was performed on exit interview transcripts. Results: Eight of ten healthy and eight of nine patient participants completed the imaging session. Mean anxiety scores before and after imaging were 7.9/100 and 11.8/100, respectively (P = 0.26), and mean motion sickness scores were 5.3/100 and 13.7/100, respectively (P = 0.02). Most participants indicated likely acceptance of rotation if MRI were to be used in a hypothetical treatment. Physical discomfort was reported to be the biggest concern. Conclusions: Horizontal rotation within an MRI scanner was acceptable for most (17/19) participants.

Purpose To evaluate eXaSkin, a novel high-density bolus alternative to commercial tissue-equivalent Superflab, for 6MV photon-beam radiotherapy. Materials and methods We delivered a 10 × 10 cm2 open field at 90° and head-and-neck clinical plan, generated with the volumetric modulated arc therapy (VMAT) technique, to an anthropomorphic phantom in three scenarios: with no bolus on the phantom’s surface, with Superflab, and with eXaSkin. In each scenario, we measured dose to a central planning target volume (PTV) in the nasopharynx region with an ionization chamber, and we measured dose to the skin, at three different positions within the vicinity of a neck lymph node PTV, with MOSkin™, a semiconductor dosimeter. Measurements were compared against calculations with the treatment planning system (TPS). Results For the static field, MOSkin results underneath the eXaSkin were in agreement with calculations to within 1.22%; for VMAT, to within 5.68%. Underneath Superflab, those values were 3.36% and 11.66%. The inferior agreement can be explained by suboptimal adherence of Superflab to the phantom’s surface as well as difficulties in accurately reproducing its placement between imaging and treatment session. In all scenarios, dose measured at the central target agreed to within 1% with calculations. Conclusions eXaSkin was shown to have superior adaptation to the phantom’s surface, producing minimal air gaps between the skin surface and bolus, allowing for accurate positioning and reproducibility of set-up conditions. eXaSkin with its high density material provides sufficient build-up to achieve full skin dose with less material thickness than Superflab.

Purpose: To examine the variability in prescribed dose due to contouring variations in intracavitary image-guided adaptive brachytherapy for cervical cancer. To identify correlations between dosimetric outcomes and delineation uncertainty metrics. Methods and Materials: A data set from an EMBRACE sub-study on contouring uncertainties was used, consisting of magnetic resonance images of six patients with cervical cancer delineated by 10 experienced observers (target volumes and organs at risk). Two gold standard contours were generated, an expert consensus and the simultaneous truth and performance level estimation. Plans were individually optimised to all of the contour sets (12 in total). Plans were applied to the gold standard contour sets, and dose volume histogram parameters including D90, D98 and D2cm 3 were determined. The variability between plans was assessed. Dose volume histogram parameters and delineation uncertainty metrics were correlated using the Spearman's non-parametric rank correlation. Results: There is a dosimetric variability between observers, patients and the gold standard contour used for analysis. Approximately 3 Gy D90 EQD210 variability (SD) was observed for the CTVHR and 1.2-3.6 Gy D2cm 3 EQD23 for the organs at risk. The maximum geometric dimensions of the delineations are most commonly correlated with dosimetry changes. Although the correlations are similar across gold standards, the direction of these correlations differs, indicating that the dosimetric outcomes are dependent on the contour that the plan is optimised to. Conclusion: This study highlights the dosimetric differences interobserver uncertainty in contouring can have for cervical cancer brachytherapy. The importance of carefully choosing a gold standard from which to benchmark is reiterated.

This prospective study of weekly CT scanning and plan adaption during H&N IMRT reports on the frequency of plan adaptations based on dosimetric differences between original and re-optimised IMRT plans. The volumetric and geometric change occurring in target volumes and salivary glands is also described. Ten H&N cancer patients underwent weekly planning CT imaging and re-optimisation of the IMRT plan if PTV or OAR coverage was unacceptable. Comparisons of PTV and parotid gland dosimetry between the original and adaptive plans were made. Parotid and submandibular gland volume changes and shift were calculated. Eight of ten patients required one or more plan adaptations, with 41% of adaptations occurring by fraction ten. Salivary glands reduced in volume, with a medial shift of the lateral border of the parotid gland and a superior shift of the submandibular gland. Change in PTV coverage did not correlate with weight loss or nutritional score. Inadequate PTV coverage, requiring plan adaptation, occurs early in the course of IMRT. A weekly Adaptive RT (ART) protocol results in significant improvement of PTV coverage. Implementation of a clinical ART protocol should include imaging and dose calculation within the first ten fractions.

Purpose To investigate the feasibility of using the brass mesh bolus as an alternative to tissue- equivalent bolus for post mastectomy chest wall cancer by characterizing the dosimetric effects of the 2-mm fine brass bolus on both the skin dose, the dose at depth and spatial distribution. Materials and methods Surface dose and percent depth dose data were acquired for a 6 MV photon beam in a solid water phantom using MOSkin™, Gafchromic EBT3 film and an Advanced Markus ionization chamber. Data were acquired for the case of: no bolus, Face-up bass bolus, Face-down brass bolus, double brass bolus, 0.5 cm and 1.0 cm of Superflab TE bolus. The exit doses were also measured via MOSkin™ dosimeter and Markus ionization chamber. Gafchromic EBT3 film strips were used to plot dose profile at surface and 10 cm depth for Face-up brass, Face-down brass, double brass, 0.5 cm and 1.0 cm of Superflab TE bolus. Results The surface dose measured via MOSkin™ dosimeter increased from 19.2 ± 1.0% to 63.1 ± 2.1% under Face-up brass discs, 51.2 ± 1.2% under Face-up brass spaces, 61.5 ± 0.5% under Face-down brass discs, and 41.3 ± 2.1% under Face-down brass spaces. The percentage difference in the dose measured under brass discs between Face-up versus Face-down was less than 2% for entrance dose and 10% for exit dose, whereas the percentage difference under brass spaces was approximately 3% for entrance dose and about 5% for the exit dose. Gafchromic EBT3 film strip measurements show that the mesh bolus produced ripple beam profiles due to the mesh brass construction. Conclusions Brass bolus does not significantly change dose at depth (less than 0.5%), and the surface dose is increased similar to TE bolus. Considering this, brass mesh may be used as a substitute for TE bolus to increase superficial dose for chest wall tangent plans.

Four methodologies were evaluated for quantifying kilovoltage cone‐beam computed tomography (CBCT) dose: the Cone‐Beam Dose Index (CBDI), IAEA Report 5 recommended methodology (IAEA), the AAPM Task Group 111 methodology (TG111), and the current dose metric; the Computed Tomography Dose Index (CTDI) on two commercial Varian cone‐beam CT imaging systems; the Clinac iX On‐Board Imager (OBI); and the TrueBeam X‐ray Imaging system (XI). The TG111 methodology measured the highest overall dose (21.199 ± 0.035 mGy OBI and 22.420 ± 0.002 XI for pelvis imaging) due to the full scatter of the TG111 phantom and was within 5% of CTDI measurements taken using a full scatter TG111 phantom and 30‐cm film strips. CBDI measured the second highest overall dose, within 10% of the TG111, with IAEA measuring the third highest dose. For head CBCT protocols, CBDI measured the highest dose, followed by IAEA. The CTDI method measured lowest across all scan modes highlighting its limitations for CBCT dosimetry. The XI imaging system delivered lower doses for head and thorax scan modes and similar doses to the OBI system for pelvis scan modes due to additional beam hardening filtration in the XI system. The TG111 method measured the highest dose in the center of a CBCT scan during image guidance procedures; however, CBDI provided a good approximation to TG111 with existing CTDI equipment and may be more applicable clinically.

Cherenkov radiation is generated in optical systems exposed to ionising radiation. In water or plastic devices, if the incident radiation has components with high enough energy (for example, electrons or positrons with energy greater than 175 keV), Cherenkov radiation will be generated. A scintillator dosimeter that collects optical light, guided by optical fibre, will have Cherenkov radiation generated throughout the length of fibre exposed to the radiation field and compromise the signal. We present a novel algorithm to separate Cherenkov radiation signal that requires only a single probe, provided the radiation source is pulsed, such as a linear accelerator in external beam radiation therapy. We use a slow scintillator (BC-444) that, in a constant beam of radiation, reaches peak light output after 1 microsecond, while the Cherenkov signal is detected nearly instantly. This allows our algorithm to separate the scintillator signal from the Cherenkov signal. The relative beam profile and depth dose of a linear accelerator 6 MV X-ray field were reconstructed using the algorithm. The optimisation method improved the fit to the ionisation chamber data and improved the reliability of the measurements. The algorithm was able to remove 74% of the Cherenkov light, at the expense of only 1.5% scintillation light. Further characterisation of the Cherenkov radiation signal has the potential to improve the results and allow this method to be used as a simpler optical fibre dosimeter for quality assurance in external beam therapy.

Purpose The aim of in vivo skin dosimetry was to measure the absorbed dose to the skin during radiotherapy, when treatment planning calculations cannot be relied on. It is of particularly importance in hypo-fractionated stereotactic modalities, where excessive dose can lead to severe skin toxicity. Currently, commercial diodes for such applications are with water equivalent depths ranging from 0.5 to 0.8 mm. In this study, we investigate a new detector for skin dosimetry based on a silicon epitaxial diode, referred to as the skin diode. Method The skin diode is manufactured on a thin epitaxial layer and packaged using the “drop-in” technology. It was characterized in terms of percentage depth dose, dose linearity, and dose rate dependence, and benchmarked against the Attix ionization chamber. The response of the skin diode in the build-up region of the percentage depth dose (PDD) curve of a 6 MV clinical photon beam was investigated. Geant4 radiation transport simulations were used to model the PDD in order to estimate the water equivalent measurement depth (WED) of the skin diode. Measured output factors using the skin diode were compared with the MOSkin detector and EBT3 film at 10 cm depth and at surface at isocenter of a water equivalent phantom. The intrinsic angular response of the skin diode was also quantified in charge particle equilibrium conditions (CPE) and at the surface of a solid water phantom. Finally, the radiation hardness of the skin diode up to an accumulated dose of 80 kGy using photons from a Co-60 gamma source was evaluated. Results The PDD curve measured with the skin diode was within 0.5% agreement of the equivalent Geant4 simulated curve. When placed at the phantom surface, the WED of the skin diode was estimated to be 0.075 ± 0.005 mm from Geant4 simulations and was confirmed using the response of a corrected Attix ionization chamber placed at water equivalent depth of 0.075 mm, with the measurement agreement to within 0.3%. The output factor measurements at 10 cm depth were within 2% of those measured with film and the MOSkin detector down to a field size of 2 × 2 cm2. The dose–response for all detector samples was linear and with a repeatability within 0.2%. The skin diode intrinsic angular response showed a maximum deviation of 8% at 90 degrees and from 0 to 60 degree is less than 5%. The radiation sensitivity reduced by 25% after an accumulated dose of 20 kGy but after was found to stabilize. At 60 kGy total accumulated dose the response was within 2% of that measured at 20 kGy total accumulated dose. Conclusions This work characterizes an innovative detector for in vivo and real-time skin dose measurements that is based on an epitaxial silicon diode combined with the Centre for Medical Radiation Physics (CMRP) “drop-in” packaging technology. The skin diode proved to have a water equivalent depth of measurement of 0.075 ± 0.005 mm and the ability to measure doses accurately relative to reference detectors.

Purpose This study aims to investigate the 2D monolithic silicon diode array size of 52x52 mm2 (MP512) angular response. An angular correction method has been developed that improves the accuracy of dose measurement in a small field. Methods The MP512 was placed at the center of a cylindrical phantom, irradiated using 6MV and 10MV photons and incrementing the incidence of the beam angle in 15° steps from 0° to 180°, and then in 1° steps between 85° to 95°. The MP512 response was characterized for square field sizes varying between 1x1cm2 to 10x10cm2. The angular correction factor was obtained as the ratio of MP512 response to EBT3 film measured doses as a function of the incidence angle (Ɵ) and was normalized at 0° incidence angle. Beam profiles of the corrected MP512 responses were compared with the EBT3 responses to verify the effectiveness of the method adopted. Results The intrinsic angular dependence of the MP512 shows maximum relative deviation from the response normalized to 0° of 18.5±0.5% and 15.5±0.5% for 6MV and 10MV, respectively, demonstrating that the angular response is sensitive to the energy. In contrast, the variation of angular response is less affected by field size. Comparison of cross-plane profiles measured by the corrected MP512 and EBT3 shows an agreement within ±2% for all field sizes when the beams irradiated the array at 0°, 45°, 135°, and 180° angles of incidence from the normal to the detector plane. At 90° incidence, corresponding to a depth dose measurement, up to a 6% discrepancy was observed for a 1x1cm2 field of 6MV. Conclusion An angular correction factor can be adopted for small field sizes. Measurements discrepancies could be encountered when irradiating with very small fields parallel to the detector plane. Using this approach, the MP512 is shown to be a suitable detector for 2D dose mapping of small field size photon beams. This article is protected by copyright. All rights reserved.

Purpose To assess the sensitivity of two commercial dosimetry systems in detecting Helical TomoTherapy (HT) delivery errors. Method Two commercial dosimeters i) MatriXX Evolution and ii) ArcCHECK® were considered. Ten retrospective nasopharynx HT patients were analysed. For each patient, error plans were created by independently introducing systematic offsets in: a) Jaw width error ±1, ±1.5 and ±2 mm, b) Couch speed error ±2%, ±2.5, ±3% and ±4%, and c) MLC Leaf Open Time (MLCLOT) errors (3 separate MLC errors: either leaf 32 open or leaf 42 remains open during delivery, and 4% random reductions in MLCLOT). All error plans, along with the no error plan for each patient, were measured using both dosimeters in the same session. Gamma evaluation (3%/3 mm) was applied to quantitatively compare the measured dose from each dosimeter to the treatment planning system. The error sensitivity was quantified as the rate of decrease in gamma pass rate. Results The gamma pass rate decreases with increase in error magnitude for both dosimeters. ArcCHECK was insensitive for couch speed error up to 2.5% and jaw width error up to −1.5 mm while MatriXX Evolution was found to be insensitive to couch speed error up to 2% and couch speed up to −1 mm. Both of the detectors show similar sensitivity to all the MLCLOT errors that were clinically relevant. Conclusion No statistically significant (p > 0.05) differences exist in detecting the simulated delivery errors between MatriXX Evolution and ArcCHECK dosimeter systems for HT plans. Both dosimeters were able to pick up clinically relevant delivery errors.

PURPOSE Dynamic dosimaging is a concept whereby a detector in motion is tracked with magnetic resonance imaging (MRI) to validate the amount and position of dose in a radiation therapy treatment on an MRI-linac. This work takes steps toward the realization of dynamic dosimaging with the novel high resolution silicon array detector: MagicPlate-512 (M512). The performance of the M512 was assessed in a 1.0 T inline MRI-linac, without simultaneous imaging and then during an imaging sequence, both during dosimetry. MR images were acquired to determine the effect of the detector and its components on image quality. METHODS Beam profiles were measured using the M512 on the Australian MRI-Linac and a comparison made with Gafchromic EBT3 film to investigate any intrinsic magnetic field effects in the silicon. The M512 has 512 sensitive volumes, each 0.5 × 0.5 × 0.037 mm3 in dimension, organized in a two-dimensional array. Small field sizes up to 4.2 × 3.8 cm2 were investigated in both solid water and then solid lung phantoms. Beam profiles taken at 1.0 T were compared to 0 T conditions, and also to profiles taken during a gradient echo (GRE) imaging sequence. Differences in 80%-20% penumbral width and full width at half maximum (FWHM) were investigated. Localizer MR images were acquired of the detector adjacent to a water phantom. Plastic scintillation dosimeters (PSDs) have many properties that make them desirable for relative dosimetry with MRI-LINACs. An in-house PSD, Farmer ionisation chamber and Gafchromic EBT3 film were used to measure central axis percentage depth dose distributions (PDDs) at the Australian MRI-LINAC Mean errors were calculated between each detector’s responses, where the in-house PSD was on average within 0.7% of the Farmer chamber and 1.4% of film, while the Farmer chamber and film were on average within 1.1% of each other. However, the PSD systematically over-estimated the dose as depth increased, approaching a maximum overestimation of the order of 3.5% for the smallest field size measured. This trend was statistically insignificant for all other field sizes measured; further investigation is required to determine the source of this effect. The calculated values of mean absolute error are comparable to the those of trusted dosimeters reported in the literature. These mean absolute errors, and the ubiquity of desirable dosimetric qualities inherent to PSDs suggest that PSDs in general are accurate for relative dosimetry with the MRI-LINAC. Further investigation is required into the source of the reported systematic trends dependent on field-size and depth of measurement.

Purpose Develop a framework to characterize helical TomoTherapy (HT) machine delivery errors and their clinical significance. Method and materials Ten nasopharynx HT plans were edited to introduce errors in Jaw width (JW), couch speed (CS), gantry period (GP), gantry start position (GSP), multi leaf collimator leaf open times (MLC LOT). In case of MLC LOT only, both systematic and random delivery errors were investigated. Each error type was simulated independently for a range of magnitudes. Dose distributions for the clinical reference plans and the error simulated plans were compared to establish the magnitude for each error type which resulted in a change in clinical tolerance, defined as 5% variation in D95 of PTV70, D0.1 cc of spinal cord, D0.1 cc of brainstem and the smallest value of either a 10% or 3.6 Gy dose variation in mean parotid dose. Results Dose variation from systematic delivery errors in JW ±0.5 mm, CS ranges between −1% to 1.5%, GP ±1 s, GSP ranges between −2 0 to 2.5 0 and MLC LOT random error up to 2% from the planned value relative to the clinical reference plan was within the set tolerance values for all the patient cohorts. GSP errors and the random MLC LOT errors with up to 10% standard deviation were found to be relatively insensitive compared to other delivery errors. Conclusion This work has established a framework to characterize HT machine delivery errors. This framework could be applied to any patient dataset to determine clinically relevant HT QA tolerances.

Tracking the position of a moving radiation detector in time and space during data acquisition can replicate 4D image-guided radiotherapy (4DIGRT). Magnetic resonance imaging (MRI)-linacs need MRI-visible detectors to achieve this, however, imaging solid phantoms is an issue. Hence, gel-water, a material that provides signal for MRI-visibility, and which will in future work, replace solid water for an MRI-linac 4DIGRT quality assurance tool, is discussed. MR and CT images of gel-water were acquired for visualisation and electron density verification. Characterisation of gel-water at 0 T was compared to Gammex-RMI solid water, using MagicPlate-512 (M512) and RMI Attix chamber; this included percentage depth dose, tissue-phantom ratio (TPR20/10), tissue-maximum ratio (TMR), profiles, output factors, and a gamma analysis to investigate field penumbral differences. MR images of a non-powered detector in gel-water demonstrated detector visualisation. The CT-determined gel-water electron density agreed with the calculated value of 1.01. Gel-water depth dose data demonstrated a maximum deviation of 0.7% from solid water for M512 and 2.4% for the Attix chamber, and by 2.1% for TPR20/10 and 1.0% for TMR. FWHM and output factor differences between materials were ≤0.3 and ≤1.4%. M512 data passed gamma analysis with 100% within 2%, 2 mm tolerance for multileaf collimator defined fields. Gel-water was shown to be tissue-equivalent for dosimetry and a feasible option to replace solid water.

Purpose To determine whether T2-weighted MRI improves seroma cavity (SC) and whole breast (WB) interobserver conformity for radiation therapy purposes, compared with the gold standard of CT, both in the prone and supine positions. Methods and Materials Eleven observers (2 radiologists and 9 radiation oncologists) delineated SC and WB clinical target volumes (CTVs) on T2-weighted MRI and CT supine and prone scans (4 scans per patient) for 33 patient datasets. Individual observer's volumes were compared using the Dice similarity coefficient, volume overlap index, center of mass shift, and Hausdorff distances. An average cavity visualization score was also determined. Results Imaging modality did not affect interobserver variation for WB CTVs. Prone WB CTVs were larger in volume and more conformal than supine CTVs (on both MRI and CT). Seroma cavity volumes were larger on CT than on MRI. Seroma cavity volumes proved to be comparable in interobserver conformity in both modalities (volume overlap index of 0.57 (95% Confidence Interval (CI) 0.54-0.60) for CT supine and 0.52 (95% CI 0.48-0.56) for MRI supine, 0.56 (95% CI 0.53-0.59) for CT prone and 0.55 (95% CI 0.51-0.59) for MRI prone); however, after registering modalities together the intermodality variation (Dice similarity coefficient of 0.41 (95% CI 0.36-0.46) for supine and 0.38 (0.34-0.42) for prone) was larger than the interobserver variability for SC, despite the location typically remaining constant. Conclusions Magnetic resonance imaging interobserver variation was comparable to CT for the WB CTV and SC delineation, in both prone and supine positions. Although the cavity visualization score and interobserver concordance was not significantly higher for MRI than for CT, the SCs were smaller on MRI, potentially owing to clearer SC definition, especially on T2-weighted MR images.

The purpose of this study was to determine the impact of magnetic resonance imaging (MRI) geometric distortions when using MRI for target delineation and planning for whole-breast, intensity-modulated radiotherapy (IMRT). Residual system distortions and combined systematic and patient-induced distortions are considered. This retrospective study investigated 18 patients who underwent whole-breast external beam radiotherapy, where both CT and MRIs were acquired for treatment planning. Distortion phantoms were imaged on two MRI systems, dedicated to radiotherapy planning (a wide, closed-bore 3T and an open-bore 1T). Patient scans were acquired on the 3T system. To simulate MRI-based planning, distortion maps representing residual system distortions were generated via deformable registration between phantom CT and MRIs. Patient CT images and structures were altered to match the residual system distortion measured by the phantoms on each scanner. The patient CTs were also registered to the corresponding patient MRI scans, to assess patient and residual system effects. Tangential IMRT plans were generated and optimized on each resulting CT dataset, then propagated to the original patient CT space. The resulting dose distributions were then evaluated with respect to the standard clinically acceptable DVH and visual assessment criteria. Purpose: The pursuit of real-time image guided radiotherapy using optimal tissue contrast has seen the development of several hybrid magnetic resonance imaging (MRI)-treatment systems, high field and low field, and inline and perpendicular configurations. As part of a new MRI-linac program, an MRI scanner was integrated with a linear accelerator to enable investigations of a coupled inline MRI-linac system. This work describes results from a prototype experimental system to demonstrate the feasibility of a high field inline MR-linac. Methods: The magnet is a 1.5 T MRI system (Sonata, Siemens Healthcare) was located in a purpose built radiofrequency (RF) cage enabling shielding from and close proximity to a linear accelerator with inline (and future perpendicular) orientation. A portable linear accelerator (Linatron, Varian) was installed together with a multileaf collimator (Millennium, Varian) to provide dynamic field collimation and the whole assembly built onto a stainless-steel rail system. A series of MRI-linac experiments was performed to investigate (1) image quality with beam on measured using a macropodine (kangaroo) ex vivo phantom; (2) the noise as a function of beam state measured using a 6-channel surface coil array; and (3) electron contamination effects measured using Gafchromic film and an electronic portal imaging device (EPID). Results: (1) Image quality was unaffected by the radiation beam with the macropodine phantom image with the beam on being almost identical to the image with the beam off. (2) Noise measured with a surface RF coil produced a 25% elevation of background intensity when the radiation beam was on. (3) Film and EPID measurements demonstrated electron focusing occurring along the centerline of the magnet axis. Conclusions: A proof-of-concept high-field MRI-linac has been built and experimentally characterized. This system has allowed us to establish the efficacy of a high field inline MRI-linac and study a number of the technical challenges and solutions.

Modern stereotactic radiation therapy modalities utilize small beams and large dose gradients to deliver radiation in few fractions, reducing the possibility to correct for mistakes during the treatment process. Therefore, in order to ensure best possible treatment for the patient, quality assurance for such treatments necessitates a stable, linear, and sensitive radiation detector with high spatial resolution and radiation hardness. In this work, two silicon detector arrays with high spatial resolution have been characterized by 6 MV and 18 MV medical LINAC irradiation, and 5.5 MeV He2+ heavy ion microprobe. A maximum discrepancy of 0.6 mm in field size has been found when comparing to two-dimensional radiochromic film dose profile, and charge collection efficiency obtained by means of ion beam induced charge collection (IBICC) is 66% when operating the array in photovoltaic mode. Radiation damage study by photons and photoneutrons is presented.

This work outlines a method for using Gafchromic film for dosimetry purposes, by scanning it with currently available commercial scanners. The scanners used were: Epson V800, Epson V700, Epson V37 series, specifically a V370 and a Canon multi-function office printer/scanner. The Epson scanners have 16 bit RGB resolution, the Canon has 8 bit RGB (Red Green Blue) resolution, and the V800 and V700 allow scanning in transmission mode. The V700 uses an Epson White Cold Cathode Florescent Lamp; the recently released V800 uses an Epson light emitting diode (LED) light source, while the V37 series uses a reflective mode and the Epson LED light source. The Epson V37 series scanners are designed for non-professional use so the cost has been kept at a low "entry level" point, so they would be a suitable option for a department wanting to use Gafchromic film or with limited needs that did not justify a more sophisticated and expensive unit. Note that the V800 or V700 scanners are not expensive in context, costing approximately the same as a 25 sheet box of Gafchromic film. The Canon was included to demonstrate that a scanner with 8 bit RGB resolution can be used for dosimetry. These general multi-function units are available in most departments, and they would allow Gafchromic film to be evaluated as a dosimetry tool without a significant investment. Furthermore, they are generally capable of scanning large format film (425 × 350 mm) in one part. Although this is not necessary for dosimetry, it is often useful for machine QA, where dividing the film into two parts to ensure accurate measurements is not practical. Moreover, this analytical method uses software that is freely or commonly available, particularly the image processing package ImageJ. Note ImageJ v1.48 was the version used. The results demonstrate that this method used with the scanners evaluated is a practical method of using Gafchromic film as a dosimeter for IMRT QA.

The main objective of this study is to demonstrate the performance characteristics of the Magic Plate (MP) system when operated upstream of the patient in transmission mode (MPTM). The MPTM is an essential component of a real‐time QA system designed for operation during radiotherapy treatment. Of particular interest is a quantitative study into the influence of the MP on the radiation beam quality at several field sizes and linear accelerator potential differences. The impact is measured through beam perturbation effects such as changes in the skin dose and/or percentage depth dose (PDD) (both in and out of field). The MP was placed in the block tray of a Varian linac head operated at 6, 10 and 18 MV beam energy. To optimize the MPTM operational setup, two conditions were investigated and each setup was compared to the case where no MP is positioned in place (i.e., open field): (i) MPTM alone and (ii) MPTM with a thin passive contamination electron filter. The in‐field and out‐of‐field surface doses of a solid water phantom were investigated for both setups using a Markus plane parallel (Model N23343) and Attix parallel‐plate, MRI model 449 ionization chambers. In addition, the effect on the 2D dose distribution measured by the Delta4 QA system was also investigated. The transmission factor for both of these MPTM setups in the central axis was also investigated using a Farmer ionization chamber (Model 2571A) and an Attix ionization chamber. Measurements were performed for different irradiation field sizes of 5×5 cm2 and 10×10 cm2. The change in the surface dose relative to dmax was measured to be less than 0.5% for the 6 MV, 10 MV, and 18 MV energy beams. Transmission factors measured for both set ups (i & ii above) with 6 MV, 10 MV, and 18 MV at a depth of dmax and a depth of 10 cm were all within 1.6% of open field. The impact of both the bare MPTM and the MPTM with 1 mm buildup on 3D dose distribution in comparison to the open field investigated using the Delta4 system and both the MPTM versions passed standard clinical gamma analysis criteria. Two MPTM operational setups were studied and presented in this article. The results indicate that both versions may be suitable for the new real‐time megavoltage photon treatment delivery QA system under development. However, the bare MPTM appears to be slightly better suited of the two MP versions, as it minimally perturbs the radiation field and does not lead to any significant increase in skin dose to the patient. Multiple vendors are now offering real-time MRI-guided radiotherapy systems. Quality assurance of the imaging and radiation isocentre alignment is an essential component of the QA programme for any linear accelerator used for delivering image-guided radiotherapy. In this work, the authors describe the design and feasibility testing of a device that combines a high resolution monolithic silicon strip detector with an MRI visible phantom for characterisation of optical, MR imaging and radiation isocentre for inline MR-guided radiotherapy systems.

The goal of this work was to assess small-field output factors (OPF) on a newly commissioned linear accelerator (linac) using a ‘correction-less’ 2D monolithic array of diodes, the Duo, which has a spatial resolution of 0.2 mm. The results would validate a set of OPF extracted from the golden beam data (GBD) used to represent the dosimetric characteristics of that linac, an Elekta Versa HD (Elekta, Crawley), fit with an Agility multileaf collimator (MLC). The Duo acquired relative OPF in real time for square fields of nominal side 1, 2, 3 and 4 cm, for 6 MV with flattening filter (WFF) and 6 MV flattening filter free (FFF) photon energies. Results revealed at most a 1.0% difference in OPF when compared to baseline, and bolstered confidence in the acceptance and commissioning of the linac using local GBD as a baseline match.

Gantry-free radiation therapy systems may be simpler and more cost effective, particularly for MRI-guided photon or hadron therapy. This study aims to understand and quantify anatomical deformations caused by horizontal rotation with scan sequences sufficiently short to facilitate integration into an MRI-guided workflow. Rigid and non-rigid pelvic deformations due to horizontal rotation were quantified for a cohort of 8 healthy volunteers using a bespoke patient rotation system and a clinical MRI scanner. For each volunteer a reference scan was acquired at 0° followed by sequential faster scans in 45° increments through to 360°. All fast scans were registered to the 0° image via a three-step process: first, images were aligned using MR visible couch markers. Second, the scans were pre-processed then rigidly registered to the 0° image. Third, the rigidly registered scans were non-rigidly registered to the 0° image to assess soft tissue deformation. The residual differences after rigid and non-rigid registration were determined from the transformation matrix and the deformation vector field, respectively. The rigid registration yielded mean rotations of ⩽2.5° in all cases. The average 3D translational magnitudes range was 5.8 ± 2.9 mm-30.0 ± 11.0 mm. Translations were most significant in the left-right (LR) direction. Smaller translations were observed in the anterior-posterior (AP) and superior-inferior (SI) directions. The maximum deformation magnitudes range was: 10.0 ± 0.9 mm-28.0 ± 2.8 mm and average deformation magnitudes range: 2.3 ± 0.6 mm-7.5 ± 1.0 mm. Average non-rigid deformation magnitude was correlated with BMI (correlation coefficient 0.84, p = 0.01). Rigid pelvic deformations were most significant in the LR direction but could be accounted for with on-line adjustments. Non-rigid deformations can be significant and will need to be accounted for in order to facilitate the delivery of gantry-free therapy with an automated patient rotation system.

Purpose: To report on significant dose enhancement effects caused by magnetic fields aligned parallel to 6 MV photon beam radiotherapy of small lungtumors. Findings are applicable to future inline MRI-guided radiotherapy systems. Methods: A total of eight clinical lungtumor cases were recalculated using Monte Carlo methods, and external magnetic fields of 0.5, 1.0, and 3 T were included to observe the impact on dose to the planning target volume (PTV) and gross tumor volume (GTV). Three plans were 6 MV 3D-CRT plans while 6 were 6 MV IMRT. The GTV’s ranged from 0.8 to 16 cm3, while the PTV’s ranged from 1 to 59 cm3. In addition, the dose changes in a 30 cm diameter cylindrical water phantom were investigated for small beams. The central 20 cm of this phantom contained either water or lung density insert. Results: For single beams, an inline magnetic field of 1 T has a small impact in lungdose distributions by reducing the lateral scatter of secondary electrons, resulting in a small dose increase along the beam. Superposition of multiple small beams leads to significant dose enhancements. Clinically, this process occurs in the lungtissue typically surrounding the GTV, resulting in increases to the D98% (PTV). Two isolated tumors with very small PTVs (3 and 6 cm3) showed increases in D98% of 23% and 22%. Larger PTVs of 13, 26, and 59 cm3 had increases of 9%, 6%, and 4%, describing a natural fall-off in enhancement with increasing PTV size. However, three PTVs bounded to the lung wall showed no significant increase, due to lack of dose enhancement in the denser PTV volume. In general, at 0.5 T, the GTV mean dose enhancement is around 60% lower than that at 1 T, while at 3 T, it is 5%–60% higher than 1 T. Conclusions: Monte Carlo methods have described significant and predictable dose enhancement effects in small lungtumor plans for 6 MV radiotherapy when an external inline magnetic field is included. Results of this study indicate that future clinical inline MRI-guided radiotherapy systems will be able to deliver a dosimetrically superior treatment to small (PTV < 15 cm3), isolated lungtumors over non-MRI-Linac systems. This increased efficacy coincides with the reimbursement in the United States of lungCT screening and the likely rapid growth in the number of patients with small lungtumors to be treated with radiotherapy.

Introduction Detailed, published surveys specific to Australian breast radiotherapy practice were last conducted in 2002. More recent international surveys specific to breast radiotherapy practice include a European survey conducted in 2008/2009 and a Spanish survey conducted in 2009. Radiotherapy techniques continue to evolve, and the utilisation of new techniques, such as intensity-modulated radiation therapy (IMRT), is increasing. This survey aimed to determine current breast radiotherapy practices across Australia. Method An online survey was completed by 50 of the 69 Australian radiation therapy treatment centres. Results Supine tangential beam whole breast irradiation remains the standard of care for breast radiotherapy in Australia. A growing number of institutions are exploring prone positioning and IMRT utilisation. This survey demonstrated a wide variation in the benchmarks used to limit and report organ at risk doses, prescribed dose regimen, and post-mastectomy bolus practices. This survey also indicated, when compared with international literature, that there may be less interest in or uptake of external beam partial breast irradiation, prone positioning, simultaneous integrated boost and breath hold techniques. These are areas where further review and research may be warranted to ensure Australian patients are receiving the best care possible based on the best evidence available. Conclusion This survey provides insight into the current radiotherapy practice for breast cancer in Australia.

Purpose: Spatial and temporal resolutions are two of the most important features for quality assurance instrumentation of motion adaptive radiotherapy modalities. The goal of this work is to characterize the performance of the 2D high spatial resolution monolithic silicon diode array named “MagicPlate-512” for quality assurance of stereotactic body radiation therapy (SBRT) and stereotactic radiosurgery (SRS) combined with a dynamic multileaf collimator (MLC) tracking technique for motion compensation. Methods: MagicPlate-512 is used in combination with the movable platform HexaMotion and a research version of radiofrequency tracking system Calypso driving MLC tracking software. The authors reconstruct 2D dose distributions of small field square beams in three modalities: in static conditions, mimicking the temporal movement pattern of a lung tumor and tracking the moving target while the MLC compensates almost instantaneously for the tumor displacement. Use of Calypso in combination with MagicPlate-512 requires a proper radiofrequency interference shielding. Impact of the shielding on dosimetry has been simulated by GEANT4 and verified experimentally. Temporal and spatial resolutions of the dosimetry system allow also for accurate verification of segments of complex stereotactic radiotherapy plans with identification of the instant and location where a certain dose is delivered. This feature allows for retrospective temporal reconstructionmore » of the delivery process and easy identification of error in the tracking or the multileaf collimator driving systems. A sliding MLC wedge combined with the lung motion pattern has been measured. The ability of the MagicPlate-512 (MP512) in 2D dose mapping in all three modes of operation was benchmarked by EBT3 film. Results: Full width at half maximum and penumbra of the moving and stationary dose profiles measured by EBT3 film and MagicPlate-512 confirm that motion has a significant impact on the dose distribution. Motion, no motion, and motion with MLC tracking profiles agreed within 1 and 0.4 mm, respectively, for all field sizes tested. Use of electromagnetic tracking system generates a fluctuation of the detector baseline up to 10% of the full scale signal requiring a proper shielding strategy. MagicPlate-512 is also able to reconstruct the dose variation pulse-by-pulse in each pixel of the detector. An analysis of the dose transients with motion and motion with tracking shows that the tracking feedback algorithm used for this experiment can compensate effectively only the effect of the slower transient components. The fast changing components of the organ motion can contribute only to discrepancy of the order of 15% in penumbral region while the slower components can change the dose profile up to 75% of the expected dose. Conclusions: MagicPlate-512 is shown to be, potentially, a valid alternative to film or 2D ionizing chambers for quality assurance dosimetry in SRS or SBRT. Its high spatial and temporal resolutions allow for accurate reconstruction of the profile in any conditions with motion and with tracking of the motion. It shows excellent performance to reconstruct the dose deposition in real time or retrospectively as a function of time for detailed analysis of the effect of motion in a specific pixel or area of interest.« less

Verification of dose to the anterior rectal wall in helical tomotherapy to the prostate is important due to the close proximity of the rectal wall to the treatment field. The steep dose gradient makes these measurements challenging. A phantom‐based study was completed, aimed at developing a system for measurement of anterior rectal wall doses during hypofractionated prostate stereotactic body radiotherapy (SBRT) utilizing tomotherapy delivery. An array of four dual MOSkin™ dosimeters, spaced 1 cm apart, was placed on a replica Rectafix® immobilization spacer device. This Perspex probe is a more rigid alternative to rectal balloons, to improve geometric reproducibility. The doses at each point were measured in real time and compared to doses calculated by the treatment planning system (TPS). Additionally, distance‐to‐agreement (DTA) measurements were acquired to assist in the comparison of measured and predicted doses. All dual MOSkin detectors measured dose to within ±5% of the TPS at the anterior rectal wall. Whilst several points were outside of experimental error, the largest deviation from the TPS predicted dose represented a DTA of only 1.3 mm, within the acceptable DTA tolerance of 3 mm. Larger deviations of up to −11.9% were observed for the posterior and side walls; however, if acceptable DTA measurements are accounted for, then an agreement of 75% was observed. Although larger differences were observed at the other rectal wall locations, the overall effect of dose at these points was not as significant, given the lower doses. Despite the very high‐dose gradient region, real‐time measurements of the anterior rectal wall doses were within acceptable limits of TPS‐predicted doses. The differences between measured and planned data were due to difficulties in precisely locating each detector on the TPS dose grid, which presented large variations in dose between CT voxels in regions of steep dose gradients. The dual MOSkin system would, therefore, be a useful device for detecting errors in real time, such as patient shifts or incorrect setup, during tomotherapy of the prostate. Purpose: This paper investigates, via magnetic modeling and Monte Carlo simulation, the ability to deliver proton beams to the treatment zone inside a split-bore MRI-guided proton therapy system. Methods: Field maps from a split-bore 1 T MRI-Linac system are used as input to geant4 Monte Carlo simulations which model the trajectory of proton beams during their paths to the isocenter of the treatment area. Both inline (along the MRI bore) and perpendicular (through the split-bore gap) orientations are simulated. Monoenergetic parallel and diverging beams of energy 90, 195, and 300 MeV starting from 1.5 and 5 m above isocenter are modeled. A phase space file detailing a 2D calibration pattern is used to set the particle starting positions, and their spatial location as they cross isocenter is recorded. No beam scattering, collimation, or modulation of the proton beams is modeled. Results: In the inline orientation, the radial symmetry of the solenoidal style fringe field acts to rotate the protons around the beam’s central axis. For protons starting at 1.5 m from isocenter, this rotation is 19° (90 MeV) and 9.8° (300 MeV). A minor focusing toward the beam’s central axis is also seen, but only significant, i.e., 2 mm shift at 150 mm off-axis, for 90 MeV protons. For the perpendicular orientation, the main MRI field and near fringe field act as the strongest to deflect the protons in a consistent direction. When starting from 1.5 m above isocenter shifts of 135 mm (90 MeV) and 65 mm (300 MeV) were observed. Further to this, off-axis protons are slightly deflected toward or away from the central axis in the direction perpendicular to the main deflection direction. This leads to a distortion of the phase space pattern, not just a shift. This distortion increases from zero at the central axis to 10 mm (90 MeV) and 5 mm (300 MeV) for a proton 150 mm off-axis. In both orientations, there is a small but subtle difference in the deflection and distortion pattern between protons fired parallel to the beam axis and those fired from a point source. This is indicative of the 3D spatially variant nature of the MRI fringe field. Conclusions: For the first time, accurate magnetic and Monte Carlo modeling have been used to assess the transport of generic proton beams toward a 1 T split-bore MRI. Significant rotation is observed in the inline orientation, while more complex deflection and distortion are seen in the perpendicular orientation. The results of this study suggest that due to the complexity and energy-dependent nature of the magnetic deflection and distortion, the pencil beam scanning method will be the only choice for delivering a therapeutic proton beam inside a potential MRI-guided proton therapy system in either the inline or perpendicular orientation. Further to this, significant correction strategies will be required to account for the MRI fringe fields.

Introduction We investigated the endorectal balloon (ERB) as a method to improve post-prostatectomy clinical target volume (CTV) stability. Methods Seventy cone-beam CT (CBCT) obtained during radiotherapy treatment from seven patients treated with an ERB and 68 CBCT from seven patients treated without an ERB were contoured according to published guidelines. CTV was subdivided into superior and inferior CTV; whole rectal volume was subdivided into superior and inferior rectum and anal volume. Concordance index (CI) of CBCT treatment volumes compared with planning volumes was calculated and displacements were measured. Purpose: Accurate geometry is required for radiotherapy treatment planning (RTP). When considering the use of magnetic resonance imaging (MRI) for RTP, geometric distortions observed in the acquired images should be considered. While scanner technology and vendor supplied correction algorithms provide some correction, large distortions are still present in images, even when considering considerably smaller scan lengths than those typically acquired with CT in conventional RTP. This study investigates MRI acquisition with a moving table compared with static scans for potential geometric benefits for RTP. Methods: A full field of view (FOV) phantom (diameter 500 mm; length 513 mm) was developed for measuring geometric distortions in MR images over volumes pertinent to RTP. The phantom consisted of layers of refined plastic within which vitamin E capsules were inserted. The phantom was scanned on CT to provide the geometric gold standard and on MRI, with differences in capsule location determining the distortion. MRI images were acquired with two techniques. For the first method, standard static table acquisitions were considered. Both 2D and 3D acquisition techniques were investigated. With the second technique, images were acquired with a moving table. The same sequence was acquired with a static table and then with table speeds of 1.1 mm/s and 2 mm/s. All of the MR images acquired were registered to the CT dataset using a deformable B-spline registration with the resulting deformation fields providing the distortion information for each acquisition. Results: MR images acquired with the moving table enabled imaging of the whole phantom length while images acquired with a static table were only able to image 50%–70% of the phantom length of 513 mm. Maximum distortion values were reduced across a larger volume when imaging with a moving table. Increased table speed resulted in a larger contribution of distortion from gradient nonlinearities in the through-plane direction and an increased blurring of capsule images, resulting in an apparent capsule volume increase by up to 170% in extreme axial FOV regions. Blurring increased with table speed and in the central regions of the phantom, geometric distortion was less for static table acquisitions compared to a table speed of 2 mm/s over the same volume. Overall, the best geometric accuracy was achieved with a table speed of 1.1 mm/s. Conclusions: The phantom designed enables full FOV imaging for distortion assessment for the purposes of RTP. MRI acquisition with a moving table extends the imaging volume in the z direction with reduced distortions which could be useful particularly if considering MR-only planning. If utilizing MR images to provide additional soft tissue information to the planning CT, standard acquisition sequences over a smaller volume would avoid introducing additional blurring or distortions from the through-plane table movement.

Purpose: To test the feasibility of a dual detector concept for comprehensive verification of external beam radiotherapy. Specifically, the authors test the hypothesis that a portal imaging device coupled to a 2D dosimeter provides a system capable of simultaneous imaging and dose verification, and that the presence of each device does not significantly detract from the performance of the other. Methods: The dual detector configuration comprised of a standard radiotherapy electronic portal imaging device (EPID) positioned directly on top of an ionization-chamber array (ICA) with 2 cm solid water buildup material (between EPID and ICA) and 5 cm solid backscatter material. The dose response characteristics of the ICA and the imaging performance of the EPID in the dual detector configuration were compared to the performance in their respective reference clinical configurations. The reference clinical configurations were 6 cm solid water buildup material, an ICA, and 5 cm solid water backscatter material as the reference dosimetry configuration, and an EPID with no additional buildup or solid backscatter material as the reference imaging configuration. The dose response of the ICA was evaluated by measuring the detector’s response with respect to off-axis position, field size, and transit object thickness. Clinical dosimetry performance was evaluated by measuring a range of clinical intensity-modulated radiation therapy (IMRT) beams in transit and nontransit geometries. The imaging performance of the EPID was evaluated quantitatively by measuring the contrast-to-noise ratio (CNR) and spatial resolution. Images of an anthropomorphic phantom were also used for qualitative assessment. Results: The measured off-axis and field size response with the ICA in both transit and nontransit geometries for both dual detector configuration and reference dosimetry configuration agreed to within 1%. Transit dose response as a function of object thickness agreed to within 0.5%. All IMRT test patterns and clinical IMRT beams had gamma pass rates of ≥98% at 2%/2 mm criteria. In terms of imaging performance, the measured CNR and spatial resolution (f 50) were 263.23 ± 24.85 and 0.4025 ± 1.25 × 10−3 for dual detector configuration and 324 ± 26.65 and 0.4141 ± 1.14 × 10−3 for reference imaging configuration, respectively. The CNR and spatial resolution were quantitatively worse in the dual detector configuration due to the additional backscatter. The difference in imaging performance was not visible in qualitative assessment of phantom images. Conclusions: Combining a commercially available ICA dosimetry device with a conventional EPID did not significantly detract from the performance of either device. Further improvements in imaging performance may be achieved with an optimized design. This study demonstrates the feasibility of a dual detector concept for simultaneous imaging and dosimetry in radiation therapy.
