Abstract

International audience; Background The identification of patients at high risk of a disabling disease course would be invaluable in guiding initial therapy in Crohn’s disease (CD). Our objective was to evaluate a combination of clinical, serological, and genetic factors to predict complicated disease course in pediatric-onset CD. Methods Data for pediatric-onset CD patients, diagnosed before 17 years of age between 1988 and 2004 and followed more than 5 years, were extracted from the population-based EPIMAD registry. The main outcome was defined by the occurrence of complicated behavior (stricturing or penetrating) and/or intestinal resection within the 5 years following diagnosis. Lasso logistic regression models were used to build a predictive model based on clinical data at diagnosis, serological data (ASCA, pANCA, anti-OmpC, anti-Cbir1, anti-Fla2, anti-Flax), and 369 candidate single nucleotide polymorphisms. Results In total, 156 children with an inflammatory (B1) disease at diagnosis were included. Among them, 35% (n = 54) progressed to a complicated behavior or an intestinal resection within the 5 years following diagnosis. The best predictive model (PREDICT-EPIMAD) included the location at diagnosis, pANCA, and 6 single nucleotide polymorphisms. This model showed good discrimination and good calibration, with an area under the curve of 0.80 after correction for optimism bias (sensitivity, 79%, specificity, 74%, positive predictive value, 61%, negative predictive value, 87%). Decision curve analysis confirmed the clinical utility of the model. Conclusions A combination of clinical, serotypic, and genotypic variables can predict disease progression in this population-based pediatric-onset CD cohort. Independent validation is needed before it can be used in clinical practice.

Abstract

Background Colon adenocarcinoma occurs mainly in older patients (pts). Oxaliplatin based adjuvant chemotherapy has demonstrated an improvement on disease-free survival (DFS) after a stage III colon cancer resection in young pts. Nevertheless, the benefit of adjuvant chemotherapy is matter of debate in old pts. Methods The purpose of ADAGE trial is to compare the DFS obtain with oxaliplatin combined with fluoropyrimidine (F) to F alone in fit pts over 70 years (group 1) and F to observation in frail pts (group 2) after resection of a stage III colon cancer. We here report a preliminary tolerance analysis on 50% of the first pts enrolled in the study. Results The analysis was performed on 491 pts (378 in group 1 and 113 in group 2). Main pts characteristics were respectively for the group 1 and 2: male in 57% and 50%, median age of 76 and 83 years, ECOG=0 in 59% and 29%, abnormal IADL in 23% and 62%, no caregiver in 26% and 28%, fall ≤6 months in 9% and 14%, depression possible 28% and 40%, fail of one-leg balance in 20% and 59%, impaired cognition in 21% and 38%, G8 score <14 75% and 94%, undernutrition 35% and 48%. The toxicity was reported in the 434 pts treated excluding the 57 pts in observation arm of group 2. One treatment related death was reported in the doublet chemotherapy arm of group 1.

Abstract

AIP is an acute liver disorder caused by a deficiency of porphobilinogen deaminase (PBGD) characterized by neuroabdominal symptoms. It is an autosomal dominant disease. However, homozygous dominant AIP (HD-AIP) have been described. In some cases erythrodontia was observed. CEP is an autosomal recessive disease produced by mutations in the uroporphyrinogen III synthase gene (UROS), characterized by severe cutaneous lesions and erythrodontia. The aim of the work was to establish the differential diagnosis of porphyria in a patient with abdominal pain, neurological attacks, skin symptoms and erythrodontia. The PBGD activity was reduced 50% and the genetic analysis indicated the presence of two genetic variants in the PBGD gene, p.G111R and p.E258G, a new genetic variant, revealing a case of heteroallelic HD-AIP. The patient, first diagnosed as a carrier of a dual porphyria: AIP / CEP based on the excretion profile of porphyrins, precursors and her clinical symptoms, would be an atypical case of human HD-AIP. These results would also suggest the presence of a phenocopy of the CEP, induced by an endogenous or exogenous factor. Our findings highlight the importance of genetic studies for a proper diagnosis of porphyria, prevention of its manifestation and its treatment. Fil: Piñeiro Pauwels, M. B.. Abstract

International audience; Arabidopsis halleri is a pseudometallophyte involved in numerous molecular studies of the adaptation to anthropogenic metal stress. In order to test the representativeness of genetic accessions commonly used in these studies, we investigated the A. halleri population genetic structure in Europe. * Microsatellite and nucleotide polymorphisms from the nuclear and chloroplast genomes, respectively, were used to genotype 65 populations scattered over Europe. * The large-scale population structure was characterized by a significant phylogeographic signal between two major genetic units. The localization of the phylogeographic break was assumed to result from vicariance between large populations isolated in southern and central Europe, on either side of ice sheets covering the Alps during the Quaternary ice ages. Genetic isolation was shown to be maintained in western Europe by the high summits of the Alps, whereas admixture was detected in the Carpathians. * Considering the phylogeographic literature, our results suggest a distinct phylogeographic pattern for European species occurring in both mountain and lowland habitats. Considering the evolution of metal adaptation in A. halleri, it appears that recent adaptations to anthropogenic metal stress that have occurred within either phylogeographic unit should be regarded as independent events that potentially have involved the evolution of a variety of genetic mechanisms.

45. Initiële stijging in geheugenzekerheid bij OCS: De mogelijke katalysator achter de instandhouding van checkgedrag.

Abstract

Dit onderzoek deed een poging een verklaring te vinden voor de ogenschijnlijke discrepantie tussen de negatieve effecten van perseveratie (verhoogde geheugenonzekerheid) en de volharding van OC patiënten in hun check gedrag. Hierbij werd verwacht dat patiënten met OCS bij het initiëren van het checkgedrag al onzekerder zijn over hun geheugen dan mensen zonder OCS. Tijdens de eerste of tweede check zouden zij zodoende nog kunnen stijgen in geheugenzekerheid, in tegenstelling tot mensen zonder OCS die dan al een plafond hebben bereikt qua geheugenzekerheid en niet meer zekerder kunnen worden door middel van checkgedrag. Deze initiële stijging in geheugenzekerheid bij patiënten met OCS zou mogelijk de bekrachtiger kunnen zijn die ervoor zorgt dat het checkgedrag gehandhaafd blijft. Daarnaast werd verwacht dat beide groepen na 12 checks een parallelle daling zouden laten zien. Om deze hypothesen te testen werden middels de OCI-R uit een steekproef van 300 studenten van de Universiteit Utrecht, 32 participanten geselecteerd en onderverdeeld in een OC+ groep (N = 14) en een OC- groep. (N = 18). Tijdens de experimentele fase werd gebruik gemaakt van de virtuele gasfornuizen checking taak en werd bij beide groepen gevraagd naar geheugenzekerheid tijdens een pre-test en de 1e, 2e en 12e check. Bij de OC + groep was sprake was een hogere initiële geheugenonzekerheid dan bij de OC- groep. Daarnaast werd het cruciale interactie-effect gevonden waarbij de OC+ groep na de eerste check wel steeg in zekerheid en de OC- groep niet. In plaats van een parallelle daling in geheugenzekerheid na de dertiende check werd een interactie-effect gevonden waarbij de OC+ groep harder daalde in geheugenzekerheid dan de OC-groep. Dit resultaat kan een gevolg zijn van toeval maar het is ook mogelijk dat er een versterkt effect van perseveratie ten aanzien van geheugenzekerheid bij patiënten met OCD bestaat. Abstract

Background: Porphyromonas gingivalis., a key pathogen in periodontitis, is able to adhere to and invade the pocket epithelium. Different capsular antigens of P. gingivalis have been identi fied (K-serotyping). These P. gingivalis capsular types show differences in adhesion capacity to human cell lines or to cells cultured on a feeder layer or stromal equivalent. Methods: The adhesion capacity of different P. gingivalis serotypes (6 capsular types and non-encapsulated strains) was compared on in vitro cultured epithelial monolayers from periodontal pockets of patients with periodontitis. The degree of adherence of P. gingivalis was evaluated by both culture and fluorescence microscopy. Results: Non-encapsulated strains adhered significantly more than their capsulated variants. Capsule type 4 (K-4) adhered slightly better than the remaining K-types. Conclusion: This study indicates that the presence and type of capsule have a significant influence on the initial adhesion of P. gingivalis to human periodontal pocket epithelial cells. 48. Can toothpaste or a toothbursh with antibacterial tufts prevent toothbrush contamination?

Abstract

Background: Periopathogens and cariogenic species survive more than 48 hours on toothbrushes even under dry storage. This study examined a decontamination of toothbrushes by means of the bactericidal effect of different toothpastes or a special coating of the tufts. Methods: Eight untreated periodontitis patients were professionally brushed, using a new toothbrush per quadrant and 3 different toothpastes containing either amine/stannous fluoride (AmF/SnF2), amine fluoride (AmF), or a mild surfactant as major antibacterial component (excipient), or no paste (C). The brushes were rinsed and stored dry at room temperature. At different time intervals, 4 tufts per toothbrush were processed for selective and non-selective culturing procedures. The protocol was repeated comparing AmF/SnF2 and C with 2 toothbrushes with coated tufts (coat1 and coat2) Results: At baseline, C brushes (n = 16) harbored 107, 108, and 106 colony forming units (CFU) of aerobic, anaerobic, and black pigmented species, respectively. After 24 hours 106 CFU of aerobic and anaerobic species could still be cultured from 14 of the 16 brushes. Black-pigmented species remained detectable up to 4 hours; detection frequencies for Fusobacterium nucleatum and Streptococcus mutans at 24 hours were 5/16. With AmF/SnF2 and AmF toothpaste the number of adhering bacteria was already 2 log lower at baseline (P0.20), and clearly inferior to the AmF/SnF2 paste (P 105 CFU. Conclusions: Toothpastes can significantly reduce contamination of toothbrushes, but their bactericidal activity is dependent on their composition. Coated tufts failed to limit the bacterial contamination.

50. Critical analysis of the SCCNFP/SCCP safety assessment of cosmetic ingredients (2000-2006).

Abstract

Over the years the Scientific Committee on Cosmetic products and Non-Food Products intended for consumers (SCCNFP) and its successor, the Scientific Committee on Consumer Products (SCCP) have assessed a substantial number of cosmetic ingredients, either present on the annexes of the cosmetic products directive or raising some concern for human health. The study of 185 SCC(NF)P opinions issued between 2000 and 2006, presented here, reveals that hair dyes and hair dye components clearly dominated the scene and that the percentage of positive opinions distinctly increased between 2003 and 2006. In order to explain the shift observed over time, the general content of the submissions as well as the perceived quality of the individual studies were related to the positive/negative outcomes of the SCC(NF)P opinions. Problem areas included identification of the compound under study and its physicochemistry, dermal absorption, mutagenicity/genotoxicity testing and exposure assessment. These studies, which form part of a standard ingredient dossier, were available, but sometimes of insufficient scientific quality as judged by the SCC(NF)P.
