The World Health Organisation recommends that children and adolescents engage in at least 60 min of moderate-to-vigorous physical activity per day. Previous research has shown that physical activity is related to other constructs such as mental well-being and self-rated health. This study examined the inter-relatedness of these constructs in Northern Irish school children.This study was a secondary analysis of data gathered as part of a longitudinal study. Participants were n = 1791 adolescents in their final years of secondary (high) school (age range 15-18; female = 64.6%). Data were gathered on three occasions over a 2-year period on self-rated health, physical activity, mental well-being, heavy episodic drinking, lifetime smoking, psychological and somatic symptoms, as well as a range of socio-demographic measures.Descriptive results showed extremely low levels of self-reported physical activity within the past week, with6% of the sample attaining the WHO guidelines at each wave of data collection. There were significant gender differences on all variables assessed. Results further showed a small-sized relationship (statistically significant for girls only) between physical activity and mental well-being. There was also a small-sized relationship between physical activity and self-rated health. Notably, effect sizes for the relationship between self-rated health and both physical activity and mental well-being were higher. In terms of socio-demographic predictors of lower physical activity, being female, lifetime cigarette smoking, and higher somatic and psychological symptoms were all statistically significant factors.Self-rated health emerged as the most important predictor of physical activity among adolescents.

Intensive breeding practices found in large scale Commercial Breeding Establishments (CBEs) raise significant concerns about breeding dam welfare. Large-scale CBE dams spend most of their reproductive lives confined to kennels, with limited access to enriching experiences including positive human interaction. Long-term confinement can have detrimental effects on dog welfare, inducing negative affective states like anxiety and depressive-like behaviour, and leading to the development of behavioural problems such as fearfulness towards unfamiliar people. Evidence in humans and other animals shows that negative affective states increase the subject’s attention towards a threatening stimulus. We tested the impact of positive human interaction on the welfare of breeding dams from a licensed UK CBE. After receiving four weeks of either baseline levels (control, N = 16) or additional positive human interaction (enriched, N = 15), an attention bias test (ABT) was conducted to assess dogs’ affective states. Afterwards, dams’ human sociability level was evaluated using a stranger approach test. Pre- and post-intervention hair cortisol samples were collected to determine the effect of enrichment on chronic stress. In the ABT, we predicted that, compared to enriched dams, control dams would look more frequently and for longer towards the position of a negative threatening stimulus (opening and closing umbrella) and would spend less time interacting with a positive rewarding stimulus (food bowl). In the stranger approach test, we expected enriched dams would score higher, suggesting more affiliative behaviour towards unfamiliar people. Results showed that control dams looked more frequently (p = 0.005) but not for longer (p = 0.148) towards the negative stimulus in the ABT. Moreover, enriched dams spent more time sniffing (p = 0.032) and eating (p = 0.005) from the food bowl. Additionally, enriched dams scored higher on average in the stranger approach test than dams in the control group (p = 0.026). No significant difference was observed in the percentage change of hair cortisol concentration between treatment groups (p = 0.135). To our knowledge, this is the first evidence for attention bias indicating affective state in dogs. This study demonstrates that a positive human interaction used as a form of enrichment can improve welfare and sociability towards strangers in commercial breeding female dogs. However, longer-term enrichment protocols may be needed to influence hair cortisol levels. Dams in large-scale breeding facilities would benefit from additional positive human interaction, particularly near the end of their reproductive life when they are rehomed as pets.

Borderline Personality Disorder is a severe psychiatric disorder with debilitating consequences. Screening for the disorder is problematic as symptoms overlap with other psychiatric disorders. The McLean Screening Instrument (MSI) assesses endorsement (yes/no) of 10 symptoms, with a cut-off of seven indicating potential caseness. Participants were (N = 68) from an established clinical cohort who completed a structured clinical interview, the MSI, the Childhood Trauma Questionnaire, and the Adolescent and Adult Time Attitudes Scale. A proportion (N = 20) also completed a follow-up interview examining their rationale for endorsing MSI items. Total number of MSI items endorsed was meaningfully related to scores on emotional neglect and negative time attitudes. There was substantive overlap between MSI threshold (≥7 items) and lifetime diagnosis of a mental disorder. The stated rationale for endorsing MSI items, was less indicative of personality trait, and was related more to particular developmental periods, one-off episodes, and life-contexts. Additionally, participants conflated constructs such as emptiness with loneliness, and moodiness with general emotionality. Those meeting MSI threshold recalled more childhood emotional neglect, and were more negative about all time periods. It is apparent that scoring of the MSI is driven by prevailing life circumstances as much as enduring personality traits.

We wanted to understand more about how people with learning disabilities are building active community lives to help belonging. We spoke to 39 people from 29 different support organisations, 7 local authority representatives and 43 people with learning disabilities. They said belonging was about having the time to connect with other people in “everyday” places, being part of a supportive network and having the right choice and information. Belonging is like a cake. It needs the right ingredients. These ingredients include the right combination of people, places and times. Because of cuts to funding, many people with learning disabilities lack the right support, choice and information to access their communities. This is not belonging. ​. Abstract: Background This journal article draws on findings from a research project that examined how people with learning disabilities and their allies were seeking to build a sense of belonging. We wanted to focus on the concept of “belonging” in the context of personalisation and reduced government social care funding. Specifically, we sought to understand how people with learning disabilities and their supporters were coming together to “self-build” networks of support including friendship clubs and self-advocacy groups to enable a greater sense of belonging in their local communities. Methods Qualitative interviews were conducted with seven local authority representatives across four case study areas in the UK, as well as 39 staff across 29 organisations providing a range of day and evening support and activities. We also talked to 43 people with learning disabilities across the four areas about their experiences. Findings Our findings demonstrate how belonging involves a complex configuration of actors, places, times, relationships and institutional roles (much like the ingredients in a cake). The ways in which belonging intersects with agency and choice was also identified as an important and novel finding of our study. Conclusion While belonging is often presented to people as a desirable and realisable outcome of social inclusion policies, cuts in funding and a lack of appropriate support frustrate people's desires to meaningfully belong with other people in their local community. This demonstrates the importance of supporting social environments that meet people's needs for social connectedness and belonging.

Immune checkpoint inhibitors and related molecules can achieve tumour regression, and even prolonged survival, for a subset of cancer patients with an otherwise dire prognosis. However, it remains unclear why some patients respond to immunotherapy and others do not. PET imaging has the potential to characterise the spatial and temporal heterogeneity of both immunotherapy target molecules and the tumor immune microenvironment, suggesting a tantalising vision of personally-adapted immunomodulatory treatment regimens. Personalised combinations of immunotherapy with local therapies and other systemic therapies, would be informed by immune imaging and subsequently modified in accordance with therapeutically induced immune environmental changes. An ideal PET imaging biomarker would facilitate the choice of initial therapy and would permit sequential imaging in time-frames that could provide actionable information to guide subsequent therapy. Such imaging should provide either prognostic or predictive measures of responsiveness relevant to key immunotherapy types but, most importantly, guide key decisions on initiation, continuation, change or cessation of treatment to reduce the cost and morbidity of treatment while enhancing survival outcomes. We survey the current literature, focusing on clinically relevant immune checkpoint immunotherapies, for which novel PET tracers are being developed, and discuss what steps are needed to make this vision a reality.

We aimed to determine the association of evidence-based processes of care for acute ischemic stroke (AIS) and clinical outcome of patients who participated in the HEADPOST (Head Positioning in Acute Stroke Trial), a multicenter cluster crossover trial of lying flat versus sitting up, head positioning in acute stroke. Methods and Results-Use of 8 AIS processes of care were considered: reperfusion therapy in eligible patients; acute stroke unit care; antihypertensive, antiplatelet, statin, and anticoagulation for atrial fibrillation; dysphagia assessment; and physiotherapist review. Hierarchical, mixed, logistic regression models were performed to determine associations with good outcome (modified Rankin Scale scores 0-2) at 90 days, adjusted for patient and hospital variables. Among 9485 patients with AIS, implementation of all processes of care in eligible patients, or defect-free care, was associated with improved outcome (odds ratio, 1.40; 95% CI, 1.18-1.65) and better survival (odds ratio, 2.23; 95% CI, 1.62-3.09). Defect-free stroke care was also significantly associated with excellent outcome (modified Rankin Scale score 0-1) (odds ratio, 1.22; 95% CI, 1.04-1.43). No hospital characteristic was independently predictive of outcome. Only 1445 (15%) of eligible patients with AIS received all processes of care, with significant regional variations in overall and individual rates. Conclusions-Use of evidence-based care is associated with improved clinical outcome in AIS. Strategies are required to address regional variation in the use of proven AIS treatments. BackgroundResearch focused on mental health in sport has revealed a need to develop evidence-supported mental health practices that are sensitive to sport culture, particularly for non-elite athletes. A consensus statement was produced to assist effective mental health awareness in sport and guide programme implementation in this rapidly emerging field.MethodThe AGREE Reporting Checklist 2016 was used in two international expert consultation meetings, followed by two online surveys. Experts from 10 countries and over 30 organisations contributed.ResultsSix objectives were agreed: (1) to define mental health awareness and service implementation constructs for inclusion in programmes delivered in sporting environments; (2) to identify the need to develop and use valid measures that are developmentally appropriate for use in intervention studies with sporting populations, including measures of mental health that quantify symptom severity but also consider causal and mediating factors that go beyond pathology (ie, well-being and optimisation); (3) to provide guidance on the selection of appropriate models to inform intervention design, implementation and evaluation; (4) to determine minimal competencies of training for those involved in sport to support mental health, those experiencing mental illness and when to refer to mental health professionals; (5) to provide evidence-based guidance for selecting mental health awareness and implementation programmes in sport that acknowledge diversity and are quality assured; and (6) to identify the need for administrators, parents, officials, coaches, athletes and workers to establish important roles in the promotion of mental health in various sports settings.ConclusionThis article presents a consensus statement on recommended psychosocial and policy-related approaches to mental health awareness programmes in sport.

Background The aim of the current study was to conduct a systematic review determining the effect of sport-specific mental health awareness programs to improve mental health knowledge and help-seeking among sports coaches, athletes and officials. The second aim was to review the study quality and to report on the validity of measures that were used to determine the effectiveness of programs. Methods Sport-specific mental health awareness programs adopting an experimental or quasi-experimental design were included for synthesis. Six electronic databases were searched: PsycINFO, MEDLINE (OVID interface), Scopus, Cochrane, CINAHL and SPORTDiscus. Each database was searched from its year of inception to October 2016. Risk of bias was assessed using the Cochrane and QATSQ tools. Results Ten studies were included from the 1216 studies retrieved: four comprising coaches or service providers, one with officials, four with athletes, and one involved a combination of coaches and athletes. A range of outcomes was used to assess indices of mental health awareness and well-being. Mental health referral efficacy was improved in six studies, while three reported an increase in knowledge about mental health disorders. However, seven studies did not report effect sizes for their outcomes, limiting clinically meaningful interpretations. Furthermore, there was substantial heterogeneity and limited validity in the outcome measures of mental health knowledge and referral efficacy. Seven studies demonstrated a high risk of bias. Conclusions Further, well-designed controlled intervention studies are required. Researchers, practitioners and policy makers should adhere to available methodological guidance and apply the psychological theory of behaviour change when developing and evaluating complex interventions. Objective The aim of this study is to investigate the correlates of knowledge of the UK physical activity (PA) guidelines. Method A Northern Ireland-wide population survey (2010/2011) of 4653 adults provided cross-sectional data on PA, knowledge of guidelines and socio-demographic characteristics. Multinomial logistic regression was used to investigate the associations between knowledge and socio-demographic characteristics (Model 1); and modifiable health behaviours (Model 2). Results Results showed that 47% of respondents were unaware of PA guidelines. Males who had a lower level of education (OR 5.91; 95% CI 1.67, 20.94), lived in more deprived areas (OR 4.80; 95% CI 1.87, 12.30), low income (OR 2.36; 95% CI 1.63, 3.41) and did no PA (OR 2.74; 95% CI 1.31, 5.76) were more likely to be unaware of the guidelines. Females who were younger (OR 1.03; 95% CI 1.02, 1.05) and reported poor health (OR 2.71; 95% CI 1.61, 4.58) were more likely to be unaware of the guidelines. Conclusion There is a lack of awareness about the levels of PA needed to promote health. An understanding of the characteristics of those who are unaware of the guidelines has important implications for the design of targeted, effective health promotion.

Background:The association between health and deprivation is of serious concern to many health promotion agencies. The purpose of the current study was to assess whether modifiable behaviors of physical activity (PA), sports participation, diet, smoking and body mass index (BMI) can help to explain these inequalities in a sample of 4653 respondents from Northern Ireland.Methods:The study is based on a cross-sectional survey of Northern Irish adults. Responses to a self-rated health question were dichotomized and binary logistic regression was used to identify the health inequalities between areas of high, middle or low deprivation. These differences were further adjusted for other sociodemographic factors and subsequently for various modifiable behaviors of PA, sports participation, diet, smoking, and BMI.Results:Respondents from high and middle areas of deprivation are more likely to report poorer health. As soon as sociodemographic factors and other modifiable behaviors were included, these inequalities either disappeared or were greatly reduced.Conclusion:Many inequalities in health in NI can be explained by the respondents’ sociodemographic characteristics that can be further explained by introducing information about respondents who meet the recommended PA guidelines, play sport, eat 5 portions of fruit and vegetables, and maintain an optimal BMI.

A previously well 46-year-old male doctor presented with a 10-week history of distressing vertiginousness with nausea, induced by rapid head movement, persisting for a few seconds, suggesting benign paroxysmal positional vertigo (BPPV). However, he could not reliably induce vertigo with any particular head positioning. Repeated treatments for BPPV with a physiotherapist over 3 weeks did not help. The patient thereafter sought neurological input, when he noticed mild but progressive lack of dexterity using the left, non-dominant hand. His positioning symptoms did not deteriorate further. Online standardised data collection systems can be used by clinicians to gather this information in a robust and accessible way. Physio First (the Organisation for Chartered Physiotherapists in Private Practice) have commissioned several data collection projects in the UK.

Northern Ireland is one of the four home nations that constitute the United Kingdom of Great Britain and Northern Ireland. It is the smallest of the home nations both in terms of geography (13,843 km2, 6 %) and population (1.8 m, 2.8 %). The economy of Northern Ireland is heavily dependent on the public sector for employment and it has some of the highest levels of unemployment in the UK. Since the signing of the Good Friday Agreement in 1998, Northern Ireland is emerging from a 30-year conflict known as the troubles into a vibrant service sector lead economy with considerable tourism and inward investment. Sport Northern Ireland (Sport NI) is the leading public body for sport development in the region and enjoys the status of being a National Lottery fund distributor. Since the launch of the National Lottery in 1994, Sport Northern Ireland has benefited from approximately £9.3 m per year for sport. This has been used to develop and enhance Northern Irelands’ sporting infrastructure including financial investment in sport clubs. Like the rest of the UK, sport clubs are integral part of community life and provide the majority of opportunities for people to take part in organised sport. Consequently as part of the UK, the historical perspective of sport clubs in Northern Ireland is largely the same as for England as outlined in Chap. 7 by Nichols and Taylor. There are however some issues which are specific to Northern Ireland and these are covered later in this chapter.

The first international symposium on Milk Genomics & Human Health brought scientists from around the world and across the milk research spectrum to the task of annotating the subsets of mammalian genomes responsible for milk and the nutritional functions of its constituents as food for mammalian offspring. These scientists are a part of The Milk Genomics Consortium, an international consortium for milk genomic research. The general goals of the Milk Genomics Consortium are to link the scientific community through milk and genomics to understand the biological and health-conferring values of milk, to create tools for interactive web-based data exchange, and to coordinate pre-competitive and post-competitive research on the components, functions and health benefits of milk and its components.

Objective To investigate the characteristics of those doing no moderate-vigorous physical activity (MVPA) (0 days/week), some MVPA (1–4 days/week) and sufficient MVPA (≥ 5 days/week) to meet the guidelines in order to effectively develop and target PA interventions to address inequalities in participation. Method A population survey (2010/2011) of 4653 UK adults provided data on PA and socio-demographic characteristics. An ordered logit model investigated the covariates of 1) participating in no PA, 2) participating in some PA, and 3) meeting the PA guidelines. Model predictions were derived for stereotypical subgroups to highlight important policy and practice implications. Results Mean age of participants was 45 years old (95% CI 44.51, 45.58) and 42% were male. Probability forecasting showed that males older than 55 years of age (probability = 0.20; 95% CI 0.11, 0.28), and both males (probability = 0.31; 95% CI 0.17, 0.45) and females (probability = 0.38; 95% CI 0.27, 0.50) who report poor health are significantly more likely to do no PA. Conclusions Understanding the characteristics of those doing no MVPA and some MVPA could help develop population-level interventions targeting those most in need. Findings suggest that interventions are needed to target older adults, particularly males, and those who report poor health.

In recognition of the calls for more processual and historically informed organizational theorizing, this chapter considers the notion of path dependency, an approach which holds that a historical path of choices has the character of a branching process with a self-reinforcing dynamic such that preceding steps in a particular direction induce further movement in the same direction, thereby making the possibility of switching to some other previously credible alternative more difficult. Path dependence seeks to assess how process, sequence and temporality can be best incorporated into explanation, the focus of the researcher being on particular outcomes, temporal sequencing and the unfolding of processes over time. Thus, proceeding from a consideration of the position afforded history in the organizational literature, this chapter outlines the tenets of path dependence theory, before sketching out its application in the practice of doing research.

Background: Recent lifestyle approaches to physical activity have included the promotion of domestic physical activities such as do-it-yourself or home maintenance, gardening and housework. Although it is acknowledged that any activity is better than none, there is a danger that those undertaking domestic ‘chores’ may assume that this activity is moderate intensity and therefore counts towards this 150 minute per week target The purpose of this paper was to report the contribution domestic physical activity makes to total weekly physical activity and the relationship between domestic physical activity and leanness in the Northern Ireland population. Methods: 4563 adults participated in this cross-sectional survey of physical activity behaviour. Data were collected through face-to-face interviews using computer assisted personal interviewing. Gender and age group differences in domestic MVPA activity and the ratio of domestic to total MVPA were explored using non-parametric Kruskal-Wallis tests. Self-reported volume and intensity of physical activity (in bouts of 10 minutes or more) in the home and self-reported height and weight were used to determine the association between domestic physical activity and leanness using an ANCOVA having controlled for age, gender, socio-economic and smoking status. Results: 42.7% of the population report levels of physical activity which meet or exceed the current United Kingdom recommendations. Domestic physical activity accounts for 35.6% of the reported moderate to vigorous physical activity (MVPA). For women, if domestic physical activity was excluded from total MVPA, only 20.4% would be deemed to meet current recommendations. Time spent in domestic physical activity at moderate or vigorous intensity was found to be negatively associated with leanness (P = 0.024), [R Squared = .132 (Adjusted R Squared = .125)]. Conclusions: Domestic physical activity accounts for a significant proportion of self-reported daily MVPA particularly among females and older adults however such activity is negatively associated with leanness suggesting that this activity may not be sufficient to provide all of the benefits normally associated with meeting the physical activity guidelines.

This study sought to investigate the associations between self-reported perceived health, socio-demographic variables and lifestyle behaviours as predictors of happiness. 4,663 adults completed the survey. Socio demographic information, age, social class, marital status, as well as lifestyle behaviours, smoking, meeting the 150 minutes of moderate intensity physical activity each day, eating five portions of fruit and vegetables a day and having access to a car were all significantly associated with happiness. For healthy individuals these factors, with the exception of having access to a car, were shown to be significant predictors of happiness. While for those in poor health, the same factors with the exception of social class and eating five portions of fruit and vegetables a day predicted happiness.

State Environmental Planning Policy No14 - Coastal Wetlands (SEPP 14) was introduced in 1985 to protect coastal wetlands in the environmental and economic interested of the State. The Policy required the preparation of an EIS, the consent of local council and the concurrence of the Director of Planning for development in affected wetlands. Many developments do not proceed to the stage of development application because of the requirements of the Policy thus ensuring their protection. Development Applications are refused when the proposal involves the irrevocable destruction of large areas of wetland. In cases where development proceeds, concurrence is generally granted subject to a number of conditions specific to the site. Proposals are more likely to succeed if they include offsetting damage to wetlands by restoration or other mitigation measures. In this way, SEPP 14 has attempted to ensure that developments in wetlands have little impact on wetland values. Establishment of compensatory wetland at Balling following the construction of a second river crossing in a SEPP 14 wetland is presented as a case study.

To remain globally competitive, rural industries must respond and adjust to several dimensions of change, aspects of which have been intensifying in recent times. These dimensions may be broadly categorised as follows – globalisation of markets and associated intensifying competition, terms of trade, environmental sustainability, food safety, supply chain transformation and advancing technologies. These pressures are not new and rural industries have been responding to them since commodity trading began. For example, during the period 1980 to 2000 the following changes occurred in the Australian dairy industry – the number of farming enterprises halved from 25,000 to 12,000; milk output per cow rose from 3500 L to 5000 L p.a.; total milk output rose from 6 bn L to 11 bn L p.a. During this same period terms of trade declined by 1.2% p.a.; to offset this total factor farm productivity rose by 1.6% p.a. There are 3 core factors contributing to improved economic productivity – these are scale economies (i.e. bigger farms, more cows per proprietor), bought-in economies (e.g. bigger cow sheds, tractors, etc) and improving the output yield from farm resources such as pastures and cows. It is in this latter grouping that science contributes. For example, the genetic merit of the national herd rose significantly in the above 20-year period, although the fertility of the herd actually declined in the same period. The challenge to rural industries to remain competitive is intensifying – there are a number of causes – a major one is deregulation of markets and intensifying competition and speed of change. Others are environmental sustainability and food safety – both of which place considerable demand on supply chain transformation. Continuing declining terms of trade means farm enterprises will have to continue to improve productivity – science will have to provide the underlying technologies that will deliver performance improvements. For example, there is ample opportunity for the reproductive and the genetic sciences to jointly deliver faster better directed genetic gain. To remain relevant and competitive, both the farming and the science industries have to adjust rapidly to the changing science paradigm, which includes principally the rapid evolution of the molecular sciences and issues of critical mass and integration, but also evolving policy and economic paradigms in governments and commerce. The capacity of science to aid the transformation of industry will be sorely tested in the period ahead. Aspects of the above will be illustrated and discussed.
