Abstract Background To address the problem of overuse of elective surgery and to support patients in their decision-making process, a Second Opinion Directive was introduced in Germany, which enables patients with statutory health insurance to obtain a second opinion for certain surgical indications. The study aims to identify, based on the experiences of patients who have undergone elective surgery, the role of seeking a second opinion in reaching their decision. Methods Sixty-two patients who had undergone an elective surgery (hysterectomy, tonsillectomy, shoulder arthroscopy) were recruited using purposive sampling and interviewed during October to December 2020. The transcribed interviews were analysed using a framework analysis to create a typology from the patient’s perspective. Results The time spent by patients in making the decision to undergo surgery varies between individuals, and is influenced by factors such as the type of physician-patient relationship, individual patient aspects, prior experiences in the health care system, as well as information needs. Within the framework of the analysis, we were able to identify three patterns of patient types based on the three different time-points or phases when decisions were typically made, with one type being divided into two subtypes: Type 1a: Quick decision making, Type 1b: Overwhelmed quick decision making, Type 2: Time to consider, Type 3: Struggling with the decision. Conclusions Patients who followed a recommendation for elective surgery appreciate having the possibility to seek a second opinion. However, various factors influenced their opting for a second opinion during the decision-making process. Patients have differing information needs, such that a one-size-fits-all second opinion service may not fit adequately for all patients.

Objective: implementation of value-based approach and development of adherence to treatment in patients with non-melanoma cancer.Material and methods. The observational, single-arm, prospective, long-term single-center study of the quality of life and patients’ experience was performed. It included 42 patients aged from 42 to 82 years. Validated questionnaires were used: Patient-Reported Outcome Measures (PROMs) (as a decision support tool), Patient-Reported Experience Measures (PREMs) (3 months after treatment to assess the patient's experience).Results. Mean concern about appearance was 43.5 points (Rasch psychometric model). Younger patients (r=–0.398, p=0.009) and female patients (r=–0.475, p=0.001) were more concerned about appearance. In 10 cases (24%), the involvement was estimated as 4 points, in 18 cases (43%), it was 3 points, in 14 cases (33%), it was 2 points. Abstract Objective While the development of communication competencies in medical schools plays a pivotal role in the curriculum, studies show that students’ communication skills and patient-centred attitudes may vary based on gender and ethnicity. The goal of this study was to investigate the socio-demographic factors that influence medical students’ communication abilities and, more specifically, to what extent their attitude toward communication skills learning and patient orientation associate with communication abilities. Our population included medical students admitted in 2017. Used tools included a communication score, the patient-provider orientation and communication skills attitudes scales. Results Three hundred and sixty-five students participated in the study (56.4% female, 85.2% German native speakers, mean age 24.2 ± 3.5 years). Female and German native speaking students had a better communication skills OSCE performance, were more patient-oriented and had more positive attitudes toward communication skills learning than male and non-native speaking students. There was a significant association between gender, native tongue, attitudes towards communication skills learning and communication skills OSCE performance. In conclusion, to support medical students to improve their communication proficiency and attitudes towards the importance of clear communication and patient-oriented care, medical educators should consider teaching and assessment strategies that address socio-cultural aspects of communication.

Introduction: Newly hospitalized patients often face some situations that have not been recognized or faced previously. Meanwhile, admission orientation can help inpatients to adapt and reduce their anxiety, but it has not been carried out optimally in some hospitals in Indonesia. Therefore, innovative media is needed to improve the efficiency and increase the willingness of nurses in carrying out admission orientation. Objective: This study aims to identify the effect of implementing website-based digital media about admission orientation on nurses' compliance with the procedure for new patient admissions. Design and Method: This is a quantitative study with a pre-experimental one-group pretest-posttest design. The sample population consists of nurses in the inpatient ward, and a total of 21 samples were then selected using the total sampling method. Subsequently, statistical analysis was carried out on the data obtained using the paired t-test. Results: The results showed that the p-value was 0.001 (< 0.05), indicating there was a significant difference between the nurse compliance level before and after implementing the web-based digital media. Furthermore, the average pretest score was 90, while the average posttest score was 94.6. Conclusions: Based on the results, there was an increase in the level of nurse compliance after the intervention.

Abstract Aim To assess the relationship among perceived nursing group power, organizational trust and patient orientation and identify patient orientation predictors. Background Nurses and nursing organizations should use all resources to give care. Methods Survey data were collected from 193 nurses at six acute care hospitals in South Korea. Characteristics of hospitals and nurses were analysed using t tests, one‐way ANOVAs and regression models. Results The means for perceived nursing group power, organizational trust and patient orientation were 3.71 (SD± 0.58), 3.22 (SD± 0.83) and 3.94 (SD± 0.53), respectively. Multiple regression analysis revealed that 40% of the variance in patient orientation was explained by perceived nursing group power, organizational trust and work experience. Conclusion Hospitals should increase nursing group power and improve organizational trust to enhance patient orientation. Hospital executives and nurse managers should work to enhance nursing group power and positive perceptions of organizational trust, which could contribute to patient orientation.

Background: A large part of the population in Germany makes use of naturopathic, complementary and integrative medical treatments. There are now numerous scientific studies that provide evidence of efficacy for certain indications. At German medical faculties, selected procedures and their application are taught within the cross-sectoral unit called and some elective courses, with a focus on specific aspects are offered. So far, however, there has been no structured curriculum that longitudinally anchors teaching across medical studies and enables all students to consider naturopathic and complementary medical options for patient care later on and to integrate them effectively into the diagnostic and treatment process.Objective: The aim of this position paper is to show the relevance of this topic for medical education, to clarify terminology and to present core competencies and possible implementation options for training.Method: The Integrative Medicine and Perspective Pluralism Committee of the German Association for Medical Education developed this position paper in a multi-stage consensual process, in cooperation with the Forum of University Work Groups on Naturopathic Treatment and Complementary Medicine.Results: First, different umbrella terms were discussed and an existing definition of integrative medicine and health was chosen for subsequent use. Building on this step, the status of education and its scientific foundation in Germany was considered in an international context. In the next step, a competency profile for medical training, consisting of seven areas of competency, was developed and described in detail with regard to naturopathic, complementary and integrative medicine. Implementation options were identified using possible starting points in the curriculum and using established examples of best practice.Conclusion: Despite different priorities at each faculty, it was possible to find an agreement on the development of competencies and anchoring them in medical education on the basis of a common definition of terms. Currently, the implementation in the mandatory and elective areas is very heterogeneous. As part of the current revision of the Medical Licensure Act, there are many possible starting points for the integration of naturopathic and complementary medical teaching content, especially in interprofessional and general practice courses. The implementation and accompanying research of targeted teaching settings should lay the foundations for a long-term and binding integration into medical education. Overall, it is clear that medical education in the field of naturopathy and complementary and integrative medicine has the potential to develop comprehensive core medical competencies.

Participation in the Clinic for Child and Adolescent Psychiatry and Psychotherapy. Do Personality Traits Affect the Difference Between the Desire and Experience of Participation? Abstract: Objective: Participation and codetermination are important rights of children and adolescents. The present study examined discrepancies between the desire and experience of participation, their relationship with treatment satisfaction, and which patient-related variables are associated with the reported discrepancies. Method: Forty adolescents from the inpatient unit or the day clinic of the university clinic of Ruppin-Brandenburg answered a questionnaire concerning their desire for and experience of participation. Additionally, we assessed sociodemographic data and personality traits. Results: Patients experienced less participation than desired. The highest discrepancy was found in items concerning communication with family and friends. Two personality traits had significant positive effects on the discrepancy of desired and experienced participation: "suspicious schizotypical" and "loyal-dependent." Conclusion: Minors want more participation than they actually experience. We need flexible concepts, especially concerning communication with family and friends, to reduce the discrepancy between the desire and the experience of participation. Giving patients the possibility to participate and encouraging them to do so should be emphasized, especially with patients with the personality traits "suspicious-schizotypical" and "loyal-dependent."

New patients undergoing inpatient services at the hospital should be given an orientation program to the care environment. The room orientation program is not only for patients but also for families who will be involved in the treatment process. The purpose of this study was to determine the effect of the implementation of patient orientation program on patient’s adaptability in the internal care room of the Sheikh Yusuf Hospital, Gowa Regency. The design used was pre-experimental in one subject group (one-group pretest and posttest) with the population of all samples obtained by Internal Care at Syekh Yusuf Hospital in Gowa Regency. The sample size was 21 respondents using accidental sampling. The independent variable was the implementation of patient orientation program and the dependent variable was the patient's adaptability. The methodof collecting data was a questionnaire. Data were analyzed using Wilcoxon signed rank test.The result of the study showed that there was an effect of the implementation of the patient orientation program on patient’s adaptability (p = 0,000 Z = -4,059).As the conclusion,if nurses carry out orientation programs to patients according to the procedure, the patient will be able to adapt to the environment and the treatment process.Further research on the effect of implementing an orientation program on patient’s adaptability should be directed to innovative models of patient orientation program.

The health care system is a state institution that implements the constitutional right of citizens to health and medical care. It is part of the legal framework, and in case of unfavorable outcomes or the consequences of the medical process, law enforcement and judicial authorities may be involved. Sudden and rapid onset death is difficult categories to accurately establish and evaluate. Having sufficient clarity in medical terminology, these categories are not clearly represented in the legal system. Consequently, when the question regards responsibility of the doctor for one or another adverse outcome in case of quick, sudden death, there are a number of qualification difficulties. There is an inconsistency between medical and legal terminologies. Complex approach to the problem analysis contributes to the systematization of knowledge and experience in this field. The article provides an example of legal proceedings in such course of events. We emphasize the need for coordination of medical and legal terminologies.

The article reveals the features of working with the patient as a social and psychological subject, who, in addition to somatic pathology, has an experience of the disease situation. Psychosocial risk factors for cardiovascular (and more generally non-infectious) pathology, as well as social well-being as a component of positive health, are considered as elements of a doctor-patient relationship. Work with a person requires from a doctor competency that differs from working with pathology at a biological level. Perhaps, the time has come to single out “biomedical doctors” and “medical doctors” in clinical medicine, of which the first ones are not required humanitarian competencies. Since non-infectious pathology largely includes psychosomatic features, and mental processes are filled with an experience of social reality, the clinician must have skills of human sciences to work with them. This is especially important in view of the physician’s power as an ambassador of medicine and health. The following competencies are discussed: internal work skills, situational search and interpretation, communicative and ethical competence, development of positive health.

The article discusses a number of situations related medical and constitutional aspects of cardiovascular medical care. The ability to prevent the negative consequences caused by diseases is not only a task of medicine as a scientific and practical field, but also a task of the Constitution of the Russian Federation. Article 41 of the Constitution of the Russian Federation establishes the protection of human life and health as one of the main tasks of the state. Medical and civil aspects of the methods for preventing diseases and its consequences are shown. An integrated approach to the analysis of this problem contributes not only to the systematization of knowledge and experience in this field, but also helps to implement the definition of health of the World Health Organization.

The article discusses the constitutional, ethical and medical aspects of hospitalization of patients with a life-threatening cardiovascular event. In itself, a medical condition is a complex biological event with many unpredictable aspects of its own course and adverse events and reactions in case of medical intervention. A doctor, as the central subject of medical care, faces four categories of difficulties: law, organizational, biomedical and ethical. Medical care is regulated by both article 41 of the Constitution of the Russian Federation and departmental acts of the health care system. Actually, doctor as a part of the healthcare system implement this right. The article provides an example of delivered judgment regarding complicated course of acute coronary syndrome. It shows the complexity of the medical situation, that regards the issues of doctor’s legal liability.

- Abstract
The COVID-19 coronavirus pandemic significantly impacted processes of medical care support and medical personnel and managers. The article presents analysis of results of three-year (2019-2022) studying level of loyalty of legal representatives of patients of one of Moscow children polyclinics. The study covered both legal representatives of patients (more than 190,000 assessments collected over entire observation period) and randomly selected district pediatricians (n=31). It is established that value-intentional consistency of district pediatricians (ratio between individual priorities of medical specialist and his ideas about proper behavior e.g. general medical or corporate values and norms), i.e. degree of interpersonal conflict, significantly affects loyalty of patients to them during uncertainty phase that covered first months of coronavirus pandemic. The physicians with moderate level of consistency between individual priorities and normative ideals demonstrated higher rates of loyalty on part of patients as compared with specialists who had value-intentional inconsistency. It is concluded that in order to increase and maintain this indicator, regular monitoring of social psychological status of medical workers, their material and non-material support and stimulation, development of mentoring system in medical organization and training of medical managers to use social psychological tools for building favorable corporate climate and culture of relations between employees of medical organization and patients are necessary.

The article suggests the reasons for the delayed help-seeking in patients with acute coronary syndrome in terms of individualization of work. For analysis, the approach of narrative medicine was used, that is, a way to conceptualize medical situations using the methods of the humanities, or rather, to understand the disease as an event in the patient’s life — his biographical narrative. These categories are important both from the standpoint of ethics and patient orientation, and from the standpoint of health improving through education of the population. A number of attitudes are shown that lead to delay in help-seeking: rigidity of thinking, unwillingness to cause discomfort, atypical manifestations, distrust of medicine and the unknown medical process. In general, the inclusion of a disease in the biographical process occurs through denial of the disease, distrust of medicine and/or healthcare, procrastination. As a solution of these problems, an assessment of the general informational background of patients as society participants and education depending on the social category, is proposed.

Purpose: This paper studies the RF‐induced heating for modular external fixation devices applied on the leg regions of the human bodies. Through numerical investigations of RF‐induced heating related to different patient orientations, landmark positions, and device positions under 1.5T and 3T MRI systems, simple and practical methods to reduce RF‐induced heating are recommended. Methods: Numerical simulations using a full‐wave electromagnetic solver based on the finite‐difference time‐domain method were performed to characterize the effects of patient orientations (head‐first/feet‐first), landmark positions (the scanning area of the patient), and device positions (device on left or right leg) on the RF‐induced heating of the external fixation devices. The G32 coil design and three anatomical human models (Duke model, Ella model, and Fats model) were adopted to model the MRI RF coil and the patients. Results: The relative positions of the patient, device, and coil can significantly affect the RF‐induced heating. With other conditions remaining the same, changing the device position or patient orientation can lead to a peak 1‐g averaged spatial absorption ratio variation of a factor around four. By changing the landmark position and the patient orientation, the RF‐induced heating can be reduced from 1323.6 W/kg to 217.5 W/kg for the specific scanning situations studied. Conclusion: Patient orientations, landmark positions, and device positions influence the RF‐induced heating of modular external fixation devices at 1.5 T and 3 T. Background: Implementation of the United Nations (UN) Convention on the Rights of Persons with Disabilities (CRPD). The idea of human rights developed during the era of the Enlightenment. This idea has been publicly discussed since the French Revolution of 1789 and its specification has been demanded politically. Important steps in this process were the General Declaration of Human Rights adopted by the UN in 1948 and the CRPD from 2006, which was especially relevant for psychiatric actions. Meanwhile, the idea of human rights has influenced the legislation in many countries and has thus become normatively binding, e. g. in the Basic Law of Germany, in the regulations for patient care and in the professional rules for physicians. Aim and questions: To sensitize for reflection on one's own actions and to gain a critical distance from the general overall opinion. Then not only the global validity of the CRPD is frequently violated, as in crisis regions but also in the everyday routine when, for example, the workload prevents psychiatric personnel from taking time for essential conversations with psychiatric patients in order to support their self-determination. Material and methods: These aspects are illustrated and explained with the aid of multifarious examples from the relevant literature and from own experience. Results and discussion: Human rights are a regulative idea that provide a framework and direction for psychiatric actions; however, as an idea they compete with other ideas that try to control the trends of the day (the Zeitgeist) that determine daily practice. Not only the suppression of human rights from public consciousness, e. g. with the eugenics in the first third of the previous century but also their absolutization and poor implementation, as in the Italian psychiatry reform of 1978, damage psychiatric patients. Therefore, it appears necessary to sensitize psychiatrists to reflect on the Zeitgeist and its influence on their own actions and to recognize that their own opinions and actions can also influence the Zeitgeist. Introduction: Hoarseness occurs as a symptom of a multitude of neurological diseases. The importance people attribute to their voice differs greatly and it seems advisable to take this into account during consultation. The Questionnaire for the assessment of the voice self-concept (FESS) is able to measure this importance. The aim of this study was to determine the applicability of the FESS in a neurological and psychiatric medical practice. Material and methods: A total of 556 patients of a neurological and psychiatic medical practice completed the FESS and the severity of hoarseness was measured objectively by means of the Acoustic Voice Quality Index (AVQI). Another 75 patients only completed the questionnaire. The value distribution of the questionnaire, the correlation with age and gender, the voice quality as well as the daily strain on the voice were investigated. Results: The value distribution ranges of all three scales of the FESS are completely utilized. Floor and ceiling effects do not play a relevant role. Gender and age do not seem to be of significance. The influence of the measurable voice quality is minor. In contrast a correlation between voice self-concept and the daily voice strain was detected. Discussion: The FESS has the potential to be an interesting instrument for the measurement of the subjective importance of a patient's voice. Here reference values are generated, against which individual results can be correlated. The AVQI provides an inexpensive fast instrument for the objective measurement of hoarseness in a neurological and psychiatric medical practice. [ABSTRACT FROM AUTHOR]

Background The responsiveness of a health system is considered to be an intrinsic goal of health systems and an essential aspect in performance assessment. Numerous studies have analysed health system responsiveness and related concepts, especially across different countries and health systems. However, fewer studies have applied the concept for the evaluation of specific healthcare delivery structures and thoroughly analysed its determinants within one country. The aims of this study are to assess the level of perceived health system responsiveness to patients with chronic diseases in ambulatory care in Germany and to analyse the determinants of health system responsiveness as well as its distribution across different population groups. Methods and Analysis The target population consists of chronically ill people in Germany, with a focus on patients suffering from type 2 diabetes and/or from coronary heart disease (CHD). Data comes from two different sources: (i) cross-sectional survey data from a postal survey and (ii) claims data from a German sickness fund. Data from both sources will be linked at an individual-level. The postal survey has the purpose of measuring perceived health system responsiveness, health related quality of life, experiences with disease management programmes (DMPs) and (subjective) socioeconomic background. The claims data consists of information on (co)morbidities, service utilization, enrolment within a DMP and sociodemographic characteristics, including the type of residential area. Discussion RAC is one of the first projects linking survey data on health system responsiveness at individual level with claims data. With this unique database, it will be possible to comprehensively analyse determinants of health system responsiveness and its relation to other aspects of health system performance assessment. The results of the project will allow German health system decision-makers to assess the performance of nonclinical aspects of healthcare delivery and their determinants in two important areas of health policy: in ambulatory and chronic disease care.

Background: A large part of the population in Germany makes use of naturopathic, complementary and integrative medical treatments. There are now numerous scientific studies that provide evidence of efficacy for certain indications. At German medical faculties, selected procedures and their application are taught within the cross-sectoral unit called QB 12 and some elective courses, with a focus on specific aspects are offered. So far, however, there has been no structured curriculum that longitudinally anchors teaching across medical studies and enables all students to consider naturopathic and complementary medical options for patient care later on and to integrate them effectively into the diagnostic and treatment process.Objective: The aim of this position paper is to show the relevance of this topic for medical education, to clarify terminology and to present core competencies and possible implementation options for training.Method: The Integrative Medicine and Perspective Pluralism Committee of the German Association for Medical Education developed this position paper in a multi-stage consensual process, in cooperation with the Forum of University Work Groups on Naturopathic Treatment and Complementary Medicine.Results: First, different umbrella terms were discussed and an existing definition of integrative medicine and health was chosen for subsequent use. Building on this step, the status of education and its scientific foundation in Germany was considered in an international context. In the next step, a competency profile for medical training, consisting of seven areas of competency, was developed and described in detail with regard to naturopathic, complementary and integrative medicine. Implementation options were identified using possible starting points in the curriculum and using established examples of best practice.Conclusion: Despite different priorities at each faculty, it was possible to find an agreement on the development of competencies and anch, Hintergrund: Ein großer Anteil der Bevölkerung in Deutschland nimmt naturheilkundliche, komplementär- und integrativmedizinische Therapieverfahren in Anspruch. Jede verantwortungsvolle Unternehmerin und jeder verantwortungsvolle Unternehmer hätte den Fokus längst auf die individuellen Versorgungsbedarfe und Bedürfnisse seiner Klientinnen und Klienten gelegt und sämtliche Prozesse bedarfs- und bedürfnisgerecht ausgerichtet. Strukturen haben nur den einen Zweck, die Prozesse bestmöglich zu unterstützen. Für die Gesundheitsversorgung bedeutet dies: Auf allen Ebenen des Gesundheitssystems den Mut aufzubringen, sich ausschließlich an Krankheitsbildern und Versorgungsbedürfnissen zu orientieren, – im Sinne einer gelebten Kultur der Versorgungsqualität. Historically evolved structures form the starting point, integrated health care is the essential objective. Any responsible entrepreneur would focus on the individual care needs and the needs of their clients, and align all processes to these care personal needs. Structures have only one purpose, to support these processes in the best possible way. Zusammenfassung Der Beitrag geht auf Defizite der Patientenorientierung und Patientenbeteiligung in der medizinischen Forschung aus Patientensicht ein und gibt Empfehlungen zu deren Weiterentwicklung. Forschende praktizieren Patientenorientierung wie auch Patientenbeteiligung oft unstrukturiert und insgesamt uneinheitlich. Dabei liegt die Entscheidung uber das Ob und Wie einer Patientenbeteiligung zu einem gewichtigen Teil eben in der Verantwortung der Forschenden. Diese Entscheidung sollte immer eingebunden sein in eine patientenorientierte Ausrichtung des Forschungsprojekts, der von Forschenden auch unabhangig von der Existenz, Eigeninitiative und Beteiligung einer Patientenorganisation nachgegangen wird. Den Forschungsphasen werden mogliche Beteiligungsformen zugeordnet. Die patientenorientierte Ausrichtung und mogliche Wahl einer Beteiligungsform wird hier als fruh einsetzende Bring- und Holschuld der Forschenden und Entscheider beschrieben. Die Beteiligung sollte auf einer fairen Basis erfolgen. Die Gremien, die uber Forschungsmittel entscheiden, sollten die Festsetzung geeigneter Vorgaben fur eine Patientenorientierung in Betracht ziehen, z. B. bei Ausschreibungen oder Vergabekriterien.

- Abstract
Patient-centeredness incorporates the consideration of patients' preferences of therapy aims. Shared decision-making can contribute to patients being satisfied with their therapy, and to their better cooperation. Objectives: 1) Determine the prioritized therapeutic goals in the treatment of bipolar disorders by patients; 2) Extract the most important dimensions of these therapeutic goals; 3) Identify how these vary by gender, age and treatment status. Within the context of a Federal Ministry of Education and Research (BMBF) supported study, patients' (N = 333) perceived importance of 73 therapeutic aims in the treatment of bipolar disorders was examined (1 = unimportant, 5 = very important). Dealing with the disorder (R2 = 29%), quality of life (R2 = 9%), participation in the therapeutic process (R2 = 7%), and side effects of medication (R2 = 5%) were regarded as the most important dimensions. Parameters of morbidity (symptoms, episodes) seemed less important (R2 = 3,6%). Ratings of importance varied with gender, age and treatment status. Dealing with problems associated with their illness, as well as improving their quality of life are particularly important for those who are treated for bipolar disorders. A better adjustment to the needs of bipolar patients can help to optimize shared decision-making within the treatment process. [ABSTRACT FROM AUTHOR]

Introduction: Polydactyly of the foot is the most common anomaly in congenital diseases. This abnormality is characterized by the presence of additional digits outside the normal complement of the foot. Polydactyly has reported about 1 in 1,000 births. The Watanabe classification is used in assessing polydactyly in the feet as a fifth ray duplication. Various operating techniques can sometimes be an option. Each method has advantages and disadvantages that can be considered in the selection of surgical in the correction polydactyly. Patient and Method: we report a case of polydactyly of the foot reconstruction using circumferential racquet-shaped incision, osteotomy and collateral ligament reconstruction that was carried out at Bahteramas Hospital, Kendari. A circumferential racquet-shaped incision design was performed over the sixth MTP joint. Complete excision of the lateral sixth toe was performed, followed by osteotomy of the fifth metatarsal lateral head with a micro-sagittal oscillating saw. The collateral ligament was reconstructed using ligamentoperiosteal flap and fixed using syringe needle to replace K-Wire. Result : After postoperative control for 3 months the patient was declared good wound healingConclusion: Optimal surgical planning, good patient orientation and surgical techniques that are carefully considered are essential for optimal results. Herein, we report a case of polydactyly and racquet shaped incision technique performed to reconstruct the polydactyly.

Background: Cleft is a common congenital abnormality in the lip and palate. Cleft palate or palatoschisis is a congenital abnormality occured during pregnancy, resulting in cleft of the palate that does not coalesce into the nasal cavity area, so there is a relationship between the nasal cavity and mouth. Various surgical techniques for cleft palate correction can be used and each method has advantages and disadvantages. Patient and Method: Here, we report a case of palatoschisis reconstruction using Von Langenbeck technique and hemostatic absorbable gelatin that was carried out at Aliyah 3 Hospital, Kendari. Result: After initial diagnosis, the patient underwent surgery about two weeks later. Von Langenback palatoplasty was performed to close the palate defect. The defect then closed primarily with 4/0 optime and 6/0 corolene. Conclusion: Optimal surgical planning, good patient orientation and surgical techniques that are carefully considered are essential for optimal results. Herein, we report a case of palatoschisis and reconstructive surgery using von Langenback technique performed to restore the cleft of palate.

47. Patientenorientierung von Websites niedergelassener Hals-Nasen-Ohren-Ärzte in Bayern

Bei der Suche nach Gesundheitsinformationen spielt das Internet eine relevante Rolle. Bisher existieren wenige Untersuchungen, welche die Benutzerfreundlichkeit von medizinischen Websites eruieren. In dieser Studie wurde daher die Patientenorientierung der Webauftritte niedergelassener HNO-Arzte in Bayern untersucht. Es erfolgte eine standardisierte, checklistenbasierte Analyse aller Arzte im Bereich Hals-Nasen-Ohren-Heilkunde auf Basis der Arzteliste der Kassenarztlichen Vereinigung Bayern. Die Auswertung erfolgte mit der Statistiksoftware IBM SPSS Version 25 (IBM, New York, USA). Von allen HNO-Arzten (n = 448) verfugten 73,0 % (n = 327) uber eine funktionierende Website. Eine Anpassung der Schriftgrose war bei der Minderheit der Websites (1,5 %) moglich. Hinsichtlich des Funktionsumfangs bestand bei 18,3 % der Facharztpraxen die Moglichkeit einer Online-Terminvereinbarung mittels Praxiskalender und bei 3,7 % die Moglichkeit zur Meinungsauserung. Ein Haftungsausschluss im Impressum fehlte bei 21,6 % aller Websites. Die Ergebnisse zeigen, dass vorliegende Kriterienkataloge zur Gestaltung von Internetauftritten bisher unzureichend genutzt werden. Prospektiv kann durch die patientenorientierte Gestaltung der Internetauftritte eine hohere Patientengewinnung sowie -zufriedenheit gewahrleistet werden. Dieser Aspekt ist vor allem fur Grosstadte mit hoher Arztedichte von Relevanz.

Zusammenfassung Zielsetzung Eine gute Organisation der internen Prozesse ist für ein funktionierendes Gesundheitswesen essenziell. Von besonderer Bedeutung ist der Prozess der Behandlung und Interaktion mit den Patienten, da die Wahrnehmung von Behandlung und Interaktion in einem direkten Zusammenhang mit der Patientenzufriedenheit steht. Letztere hat einen großen Einfluss auf die Gesundung der Patienten. Welche Auswirkungen der Reifegrad des Behandlungs- und Interaktionsprozesses auf die Patientenorientierung hat, wird am Beispiel einer umfangreichen Befragung von Krankenhausmanagern in der Ukraine untersucht. Methodik Die Analyse basiert auf einer empirischen Erhebung, die bei Chefärzten und deren Stellvertretern in einer ukrainischen Großstadt, die als Modell-Stadt einer umfassenden Gesundheitsreform fungiert, durchgeführt wurde. Anhand des Business Process Maturity Model (BPMM) haben wir unsere Hypothese, dass ein hoher Prozessreifegrad zu einem hohen Maß an Patientenorientierung führt, getestet. Dazu haben wir eine Methode zur Messung des Prozessreifegrads entwickelt und diese auf Validität und Reliabilität geprüft. An der Befragung haben 79 Chefärzte und Stellvertreter teilgenommen, die alle öffentlichen Gesundheitseinrichtungen der Stadt (Krankenhäuser, Familienärztehäuser etc.) repräsentieren. Ergebnisse Die Untersuchung zeigt einen klaren Zusammenhang zwischen der Reife des Behandlungs- und Interaktionsprozesses und dem Grad der Patientenorientierung der jeweiligen Gesundheitseinrichtung. Die Varianz, die durch die unabhängige Variable Prozessreifegrad erklärt wird, liegt bei 16,3 %. Alle formativen Dimensionen zeigen einen stark signifikanten Einfluss auf das Konstrukt „Patientenorientierung”. Der Reifegrad des Behandlungs- und Interaktionsprozesses weist ebenfalls einen stark signifikanten Einfluss auf die Patientenorientierung auf. Schlussfolgerung Für Gesundheitseinrichtungen ist es von großer Bedeutung, einen hohen Reifegrad des Behandlungs- und Interaktionsprozesses zu erreichen, um damit dem Anspruch von Patientenorientierung gerecht werden zu können. Die Reife des Prozesses kann mit dem im Rahmen dieser Studie entwickelten Fragebogen getestet werden.

50. Survey of student attitudes toward digital technology in practical technical dental education using the AR-Demonstrator-App

Purpose/objectives More and more creative applications are used in the field of dentistry for clinical practice, patient orientation, teaching, and learning, in order to awake the students' interest and to enable a deep learning and its retention. The aim of this study was to demonstrate the impact and the effectiveness of the current simulation laboratories on the practical dental education. For this purpose, a survey on a new developed "AR-Demonstrator-App" was conducted during the orthodontic technical courses in 2017, 2018, and 2020 in the Department of Orthodontics (C.G. Carus, TU Dresden, Germany). Methods With this application, the students had the opportunity to see every single step with instructions on how to manufacture a removable orthodontic device in a compulsory technical course. At the end of these courses, the students were asked to anonymously fill out a printed questionnaire with 12 questions regarding the learning progress related to the app. Results The yearly mean Likert scores and their bootstrapped 95% confidence intervals as well as the yearly median Likert scores suggest a very positive feedback about the use of the new application as an educational instrument. Despite the recognized predominant benefits of this app, the students concluded that they do not prefer the total replacement of the physical plaster casts and that a combination of conventional and computer-based education provided the optimum approach to acquisition of dental practical skills. Conclusions This survey indicates a generally positive attitude toward computer-assisted simulation systems, providing the facility for acquisition of knowledge and practical skills in a multimedia environment.
