11. Utility of Amiodarone Pre-Treatment as a Facilitator of the Acute Success of Electrical Cardioversion in Persistent Atrial Fibrillation

Abstract

Purpose The usefulness and mechanisms of antiarrhythmic drug (AAD) pre-treatment as a facilitator of the acute success of electrical cardioversion (ECV) in atrial fibrillation (AF) remain controversial. We sought to analyze the role of AAD treatment with this purpose, differentiating its possible utility either facilitating the restoration of sinus rhythm (SR) or reducing immediate AF recurrences (IAFR). Methods We analyzed 2962 consecutive patients with persistent AF undergoing ECV prospectively included in 3 national registries. The acute success of ECV was indicated by the reversion to SR without presenting an IAFR (< 2 h). Results A total of 1410 patients (48%) received AAD treatment prior to ECV (80% amiodarone, 15% class Ic AAD, 2% other AAD). The rate of restoration of SR was similar between the patients treated with amiodarone (92%), class Ic AAD (91%) and who did not receive AAD pre-treatment (91%) (p = 0.92). However, those treated with amiodarone had fewer IAFR than those in the other two groups (amiodarone 3% vs class Ic 7% vs without treatment 6%; p = 0.002), so the ECV success rate was higher in the amiodarone group than in the other groups (amiodarone 89% vs Ic 84% vs without treatment 86%; p = 0.04). After adjusting for multiple variables, amiodarone remained as an independent predictor of a lower occurrence of IAFR (OR = 0.57; p = 0.01) and of a successful ECV (OR 1.37; p = 0.01). Conclusions For patients with persistent AF undergoing ECV, AAD has a neutral effect on the restoration of SR but amiodarone increases its effectiveness due to a lower incidence of IAFR.

Abstract

The flash glucose monitoring (FGM) system FreeStyle Libre is a device that measures interstitial glucose in a very simple way and indicates direction and speed of glucose change. This allows persons with diabetes to prevent hypoglycemic and hyperglycemic events. Scientific evidence indicates that the system can improve glycemic control and quality of life. To obtain the maximum benefit, it is necessary to properly handle glucose values and trends. Due to the generalization of the system use, the purpose of the document is to provide recommendations for the optimal use of the device, not only in the management of glucose values and trends but also in the prevention of hypoglycemia, actuation in exercise, special situations, and retrospective analysis of the glucose data, among others.

17. Effects of Irradiation Temperature on the Response of CeO2, ThO2, and UO2 to Highly Ionizing Radiation

Abstract

Microcrystalline CeO2, ThO2, and UO2 were irradiated with 198 MeV 132Xe ions to the same fluence at temperatures ranging from 25 °C to 700 °C then characterized by synchrotron X-ray diffraction and X-ray absorption spectroscopy. All samples retain crystallinity and their nominal fluorite-type phase at a fluence of 1.5 × 1013 ions/cm2. Both CeO2 and ThO2 display defect-induced unit cell expansion after irradiation at room temperature (∼0.15% and ∼0.10%, respectively), yet as irradiation temperature increases, the maximum swelling produced decreases to ∼0.02%. Alternatively, UO2 shows an initial contraction in unit cell parameter (approximately −0.05%) for room temperature irradiation, most likely related to irradiation-enhanced annealing or irradiation-induced oxidation. At higher temperatures (above 200 °C) UO2 begins to swell, surpassing its unit cell parameter prior to irradiation (∼0.05%), an effect which could be attributed to minor reduction in uranium oxidation state in vacuum. However, while CeO2 irradiated at room temperature undergoes partial reduction, both UO2 and ThO2 exhibit no measurable change in cation oxidation state as evidenced by X-ray absorption spectroscopy. All samples display a decrease in irradiation-induced heterogeneous microstrain as a function of increasing irradiation temperature. © 2019 Elsevier B.V.

Abstract

Sm1-xTbxPO4 solid solutions were synthesized and extensively characterized by powder X-ray diffraction, vibrational spectroscopy, and X-ray absorption spectroscopy. At ambient conditions solid solutions up to x = 0.75 crystallize in the monazite structure, whereas TbPO4 is isostructural to xenotime. For x = 0.8 a mixture of both polymorphs was obtained. Moreover, a phase with anhydrite structure was observed coexisting with xenotime, which was formed due to mechanical stress. Selected solid solutions were investigated at pressures up to 40 GPa using in situ high pressure synchrotron X-ray diffraction and in situ high pressure Raman spectroscopy. SmPO4 and Sm0.5Tb0.5PO4 monazites are (meta)stable up to the highest pressures studied here. TbPO4 xenotime was found to transform into the monazite structure at a pressure of about 10 GPa. The transformation of Sm0.2Tb0.8PO4 xenotime into the monazite polymorph commences already at about 3 GPa. This study describes the reversibility of the pressure-induced (Sm,Tb)PO4 xenotime-monazite transformation.

Abstract

The benefits of administering injectable trace minerals (ITM; Zn, Mn, Cu, and Se) on animal health and performance have been previously assessed in dairy and beef cattle. Previous studies demonstrated that ITM improved humoral immune response to bovine herpesvirus l(BHV-l) following administration of a modified live virus (MLV) vaccine in cattle. 31. Aerobic uterine isolates and antimicrobial susceptibility in mares with post‐partum metritis.

Abstract

Summary: Background: The potential for life‐threatening complications of metritis requires prompt initiation of antimicrobial treatment, often before microbiological test results are reported. However, published studies to guide first‐line antibiotic selection are scarce. Objectives: To report the most frequent bacterial species, antimicrobial susceptibility and prevalence of multidrug resistance (MDR) in cases of equine metritis. Study design: Retrospective analysis of uterine bacterial culture and antimicrobial susceptibility test (AST) results from mares with metritis. Methods: Data from uterine culture and AST from 45 mares and 88 bacterial isolates were analysed. The frequency of single and mixed infections, bacterial species and susceptibility to antimicrobials was reported. Results: Mixed growth (62.2% mares) was more frequent than pure growth. The most commonly isolated bacterial species was Escherichia coli (30.7%) and mares with mixed growth most commonly grew a combination of Gram‐negative and Gram‐positive bacteria (65.5%). Gentamicin with penicillin was an appropriate choice for 65.1% of the mares. Trimethoprim/sulfonamide was effective in only 48.8% of the mares. Effective antimicrobial therapy was provided by the combination of penicillin with amikacin (90.7%) or with enrofloxacin (81.4%). Some organisms were also sensitive to tetracyclines, cephalosporins and chloramphenicol. MDR was more frequent in Gram‐negative (85.4%) than Gram‐positive bacteria (23.5%). Main limitations: Low number of bacteria, limited number of veterinary specific interpretive criteria for equine metritis. Conclusions: Mixed infections were common in equine metritis. Gram‐negative bacteria and enterococci were often associated with resistance to the most frequently used antimicrobials. Potentiated sulfonamides are not an appropriate first choice for mares with metritis. While the combination of penicillin and gentamicin may be an appropriate first‐line treatment for some mares, use of amikacin or enrofloxacin instead of gentamicin may be preferential.
