Background: In most patients having noncardiac surgery, blood pressure is measured with the oscillometric upper arm cuff method. Although the method is noninvasive and practical, it is known to overestimate intraarterial pressure in hypotension and to underestimate it in hypertension. A high-fidelity upper arm cuff incorporating a hydraulic sensor pad was recently developed. The aim of the present study was to investigate whether noninvasive blood pressure measurements with the new high-fidelity cuff correspond to invasive measurements with a femoral artery catheter, especially at low blood pressure.Methods: Simultaneous measurements of blood pressure recorded from a femoral arterial catheter and from the high-fidelity upper arm cuff were compared in 110 patients having major abdominal surgery or neurosurgery.Results: 550 pairs of blood pressure measurements (5 pairs per patient) were considered for analysis. For mean arterial pressure measurements, the average bias was 0 mmHg, and the precision was 3 mmHg. The Pearson correlation coefficient was 0.96 (P < 0.0001; 95% CI, 0.96 to 0.97), and the percentage error was 9%. Error grid analysis showed that the proportions of mean arterial pressure measurements done with the high-fidelity cuff method were 98.4% in zone A (no risk), 1.6% in zone B (low risk) and 0% in zones C, D, and E (moderate, significant, and dangerous risk, respectively). The high-fidelity cuff method detected mean arterial pressure values less than 65 mmHg with a sensitivity of 84% (95% CI, 74 to 92%) and a specificity of 97% (95% CI, 95% to 98%). To detect changes in mean arterial pressure of more than 5 mmHg, the concordance rate between the two methods was 99.7%. Comparable accuracy and precision were observed for systolic and diastolic blood pressure measurements.Conclusions: The new high-fidelity upper arm cuff method met the current international standards in terms of accuracy and precision. This article describes the development and deployment of Physics Toolbox Play, a gamified component of the Android Physics Toolbox Sensor Suite app, that can introduce young children through adults to fundamental physics principles. The app was used successfully in a variety of contexts, including STEM fairs with primary and secondary students and workshops with undergraduate physics majors and practicing teachers. Reservoir pressure parameters [eg, reservoir pressure (RP) and excess pressure (XSP)] are biomarkers derived from blood pressure (BP) waveforms that have been shown to predict cardiovascular events independent of conventional cardiovascular risk markers. However, whether RP and XSP can be derived non-invasively from operator-independent cuff device measured brachial or central BP waveforms has never been examined. This study sought to achieve this by comparison of cuff reservoir pressure parameters with intra-aortic reservoir pressure parameters. 162 participants (aged 61 ± 10 years, 72% male) undergoing coronary angiography had the simultaneous measurement of cuff BP waveforms (via SphygmoCor XCEL, AtCor Medical) and intra-aortic BP waveforms (via fluid-filled catheter). RP and XSP derived from cuff acquired brachial and central BP waveforms were compared with intra-aortic measures. Concordance between brachial-cuff and intra-aortic measurement was moderate-to-good for RP peak (36 ± 11 vs 48 ± 14 mm Hg, P < 0.001; ICC 0.77, 95% CI: 0.71-0.82), and poor-to-moderate for XSP peak (28 ± 10 vs 24 ± 9 mm Hg, P < 0.001; ICC 0.49, 95% CI: 0.35-0.60). Concordance between central-cuff and intra-aortic measurement was moderate-to-good for RP peak (35 ± 9 vs 46 ± 14 mm Hg, P < 0.001; ICC 0.77, 95% CI: 0.70-0.82), but poor for XSP peak (12 ± 3 vs 24 ± 9 mm Hg, P < 0.001; ICC 0.12, 95% CI: -0.13 to 0.31). In conclusion, both brachial-cuff and central-cuff methods can reasonably estimate intra-aortic RP, whereas XSP can only be acceptably derived from brachial-cuff BP waveforms. Exercise periodic breathing (EPB) is associated with exercise intolerance and poor prognosis in patients with heart failure (HF). However, EPB detection during cardiopulmonary exercise test (CPET) is difficult. The present study investigated the use of a wireless monitoring device to record the EPB during CPET and proposed quantization parameter estimates for the EPB. A total of 445 patients with HF were enrolled and underwent exercise tests. The ventilation data from the wearable device were compared with the data obtained during the CPET and were analyzed based on professional opinion and on 2 automated programs (decision tree [DT] and oscillatory pattern methods). The measurement accuracy was greater with the DT method (89 %) than with the oscillatory pattern method (75 %). The cutoffs for EPB recognition using the DT method were (1) an intercept of the regression line passing through the minute ventilation rate vs. the time curve during the recovery phase ≥64.63, and (2) an oscillatory phase duration to total exercise time ratio ≥0.5828. The wearable device was suitable for the assessment of EPB in patients with HF, and our new automated analysis system using the DT method effectively identified the EPB pattern. The aim of the study was to analyze whether auscultatory normative values (Fourth Task Force [4TF]) can be applied to blood pressure (BP) obtained by oscillometric devices. The authors performed a retrospective analysis of oscillometric office BP and ambulatory BP monitoring in 229 children (116 boys), median age 15.31 years. Office systolic BP (SBP) and diastolic BP (DBP) values were converted into Z scores using 4TF and oscillometric (German Health Interview and Examination Survey for Children and Adolescent [KiGGS]) reference values. There was good correlation between the two normative methods (r=0.9773 for SBP, r=0.9627 for DBP). Results from Bland-Altman test revealed only minimal differences in Z scores between 4TF and KiGGS for SBP, but a significant proportional error for DBP. 4TF and KiGGS Z scores were equally predictive of ambulatory hypertension. The measurement of blood pressure in the office (OBP) remains the basis for hypertension diagnosis and management for more than half a century. Despite the increasing use of out-of-office blood pressure measurement using home and less so ambulatory monitoring and their endorsement by hypertension societies, at present and for some time to come it is likely that in many people the diagnosis and management of hypertension will be based on OBP measurement alone. OBP measurement is a very variable method affected by multiple factors, which have major impact on the OBP level, reproducibility and prognostic ability. Thus, there are several types of OBP measurement, depending on the device type, conditions, observer's presence and the number of readings. The 4 main OBP types are: Type I: Auscultatory OBP in clinical practice; Type II: Automated attended OBP in clinical practice; Type III: Research setting OBP; Type IV: Unattended automated OBP. These OBP types have different standardization level, different reproducibility, different clinical relevance and different thresholds for hypertension diagnosis. In occidental countries, peripheral arterial disease (PAD) is an important health issue; however, most subjects are asymptomatic (~50%) and therefore undiagnosed and untreated. Current guidelines recommend screening for PAD in primary care setting using ankle brachial index (ABI) in all patients with cardiovascular risks. This is, however, not performed strictly because the standard Doppler method is cumbersome and time-consuming. Here, we evaluate the accuracy and reproducibility of ABI measurements obtained by an improved automated oscillometric device, the MESI ABPI MD(®) device, and the standard Doppler method. ABI was measured in random order in a general practice with Doppler probes by two operators separately (ABI_dop) and twice with the MESI ABPI MD device (ABI_mesi). ABI_dop was calculated dividing the highest systolic blood pressure from both tibial and dorsalis pedis arteries by the highest systolic blood pressure of both brachial arteries. ABI_mesi was obtained automatically with simultaneous measurements on three extremities. According to ABI_dop, PAD was present in 10% of the 136 screened subjects (68.2±7.4 years). Interoperator coefficient of variation was 5.5% for ABI_dop, while the intrasubject coefficient of variation for ABI_mesi was 3.0%. ABI_mesi was correlated with ABI_dop (R=0.61, P<0.0001). The difference between the two techniques was 0.06±0.14 with ABI_mesi providing slightly higher values (P<0.0001) and negligible bias across the range (R=0.19, P<0.0001). Therefore, ABI_mesi ≤1 had a sensitivity of 85% and specificity of 96% to detect ABI_dop ≤0.9 and hence PAD. Doppler measurements took seven times longer than MESI ABPI MD measurements to be performed. In conclusion, MESI improved automated oscillometric method and offered a faster and repeatable measurement of ABI with only a small, clinically irrelevant overestimation of ABI value. The tested MESI ABPI MD-improved oscillometric system can be used as a screening tool for patients in general practice and would enable family doctors to comply with current guidelines for PAD. In this paper, we present experimental saturation curves of a small gap miniature fission chamber obtained in the MINERVE reactor. The chamber is filled with argon at various pressures, and the fissile material can be coated on the anode, cathode, or both. For analyzing the recombination regime, we consider a model of columnar recombination and discuss its applicability to our chamber. By applying this model to the data, it is possible to estimate the ratio between the recombination coefficient k and an effective column radius b , appearing in the model, to be k / b = ( 2.5 ± 0.9 ) × 10 − 6 m 2 / s for argon. From these results, a routine measurement of the recombination regime is proposed in order to detect gas leakage. This online diagnosis would be beneficial in terms of lifetime and reliability of the neutron instrumentation of nuclear reactors. [ABSTRACT FROM AUTHOR]

We have designed and built a fast and precise balanced polarimeter from components commonly found in undergraduate physics laboratories. Balanced polarimetry measures the orientation of linearly polarized light by splitting the beam into orthogonal polarization components and detecting each separately. Our polarimeter is capable of measuring the orientation of linearly polarized light with a precision of approximately 0:001°√Hz. The apparatus cost less than $1000. This article discusses the history and development of experimental physics in Spain during the late 18th century, with a particular focus on the "Diccionario histórico del español moderno de aparatos de física experimental" ("Historical Dictionary of Modern Spanish of Experimental Physics Apparatuses"). The authors comment on the creation of new terminology and neologisms to describe physics concepts, many of which are documented in this dictionary. The invention of machinery to conduct physics experiments is also examined.

Objectives: To monitor the influence of the power density of the curing unit on the setting behaviour of light-cured glass-ionomer cements (LCGICs) using ultrasound measurements.Methods: The ultrasound equipment comprised a pulser-receiver, transducers and an oscilloscope. The LCGICs used were Fuji II LC, Fuji II LC EM and Fuji Filling LC. The cements were mixed according to the manufacturer's instructions and then inserted into a transparent mould. The specimens were placed on the sample stage and cured with power densities of 0 (no irradiation), 200 or 600 mW/cm(2). The transit time through the cement disk was divided by the specimen thickness and then the longitudinal ultrasound velocity (V) within the material was obtained. Analysis of variance and Tukey's Honestly Significantly Different test were used to compare the V values between the set cements.Results: When the LCGICs were light-irradiated, each curve displayed an initial plateau at approximately 1500 m/s and then rapidly increased to a second plateau at approximately 2600 m/s. The rate of increase of V was retarded when the cements were light-irradiated with a power density of 200 mW/cm(2) than with a power density of 600 mW/cm(2). Although sonic echoes were detected from the beginning of the measurements, the rates of increase of the sonic velocity were relatively slow when the cement was not light-irradiated.Conclusions: The ultrasound device monitored the setting processes of LCGICs accurately based on the longitudinal V. The polymerization behaviour of LCGICs was shown to be affected by the power density of the curing unit. [ABSTRACT FROM AUTHOR]

Ascites is more difficult to detect when only a small quantity is present. The aim of this pilot study was to determine the optimal bowel sound characteristics in order to distinguish no ascites from small-volume ascites by advanced processing of bowel sound wave patterns. This analysis results in the definition of the normal range of bowel sound patterns, thus providing a novel, simple, and noninvasive way of determining on abnormal pattern, which may reflect presence of small volume ascites. Cirrhotic patients with radiologically proven small-volume ascites and a control group were subjected to bowel sound recordings. The latter were analyzed using a denoising wavelet transform-based filter and a higher-order crossings-based technique in a blinded fashion for linearly distinguishing the two classes. Scatter plots of third-order zero crossings reflect distinct changes seen in the denoised bowel sound pattern between patients and controls due to altered transmission path, providing a distinct separation of all cirrhotic patients with small ascites from controls (P < 0.0001). We conclude that the proposed bowel sounds analysis appears to provide new information regarding the changes of the bowel sound patterns due to the presence of small-volume ascites, potentially contributing towards a safe, effective, noninvasive, and easily implemented alternative method for the diagnosis of small volume ascites at the bedside. Objectives. Scanning laser vibrometry is a non-invasive method of accurately measuring the vibratory characteristics of oscillating objects. The aim of this study was to observe, using a scanning laser vibrometer (SLV), the vibration patterns of dental ultrasonic scaler tips and to assess the effects of water flow rate and power setting on these patterns whilst operating the tips in an unloaded environment.Methods. A 30 kHz ultrasonic scaler (TFI-10, Dentsply) was fixed in position and a laser beam from the SLV was focused onto the tip. The laser, guided by a virtual measurement grid, was scanned over the oscillating tip surface. Scans were taken with the laser beam perpendicular to the long axis of the front face of the tip.Results. Oscillation frequencies and the displacement amplitude at the unconstrained end of the tip were measured for various power/water settings. Vibration nodal positions were recorded for the various settings and were found to occur approximately 4 mm from the free end of the tip. At low and medium power settings, tip displacement amplitude was reduced by increased water flow. At high power settings, combined with a high flow rate, the water leaves the body of the instrument as a jet. This left the tip relatively unconstrained, allowing it to oscillate at increased displacement amplitudes.Conclusions. This study shows that the SLV is able to accurately characterise the movement of oscillating ultrasonic scaler tips. The tips are affected by power setting and water flow rates. Background: Voluntary muscle strength declines significantly in older adults. One contributing factor to the strength loss is muscle atrophy developed in old age. Whether the ability to maximally activate the muscle decreases with age, however, is unknown. This study was intended to determine if the central nervous system command to maximally activate the biceps brachii muscle deteriorates with age.Methods: Electrical stimulation pulses were applied to the skin overlying the biceps brachii muscle during maximal voluntary elbow-flexion contractions. The magnitude of force evoked on the maximal voluntary force was measured to determine the activation level (AL) of the muscle.Results: The AL was 94% for the elderly group and 97% for the young group (100% AL indicates complete activation). The AL for both the elderly and young groups was significantly (p<.05) lower than 100%. The AL of the elderly group was significantly (p<.05) lower than that of the young group.Conclusions: The loss of voluntary strength in older adults is a mixed result of muscle atrophy and a reduced ability to fully activate muscle. A new instrument for the indirect noninvasive measurement of mean arterial pressure (m.a.p.) has been constructed and evaluated in man. The instrument does not require an external microphone or transducer and determines m.a.p. rather than systolic and diastolic pressure. Instead, the method employs the point of maximal oscillations as an indicator of m.a.p. The instrument automatically inflates a standard blood pressure cuff and determines the m.a.p. by measuring the cuff pressure oscillations as the cuff pressure is reduced by discrete increments. Cuff deflation in discrete increments, instead of continuously, allows the oscillation data obtained at each cuff pressure to be tested for artefacts and averaged, greatly enhancing artefact-rejection ability. The m.a.p. is selected as the lowest cuff pressure at which the oscillation amplitude is a maximum. The instrument was tested on the bicep and ankle in a series of 28 studies involving 17 human subjects with intra-arterial catheters. Averaging the mean errors from each of the 28 studies, there was an overall mean error of - 0·23 mmHg, with a standard deviation of 4·21 mmHg. The correlation coefficient was 0·98.
