However, the mechanisms through which worry increases the risk of paranoia remain poorly understood.A total of 529 participants (51.2% female) from the general population participated in an online study. The aim was to investigate the potential mediating role of hostile attributions in the relationship between general problematic worry and worry focused on paranoid content. Furthermore, it was hypothesized that negative emotional states would moderate this effect. Additionally, recently developed scales, the Dunn Worry Questionnaire (DWQ) and the Paranoia Worry Questionnaire (PWQ), were translated and validated for use in the Polish population.The results revealed that hostile attributions mediate the link between general worry and paranoia worry, and negative emotional states significantly moderate the effect.The perception of others’ intentions as hostile may serve as a linking factor between general problematic worry and paranoia-related worry. Objectives Method Results Conclusion Psychosis can be thought of as a threat‐based experience. Compassion has been shown to be effective in reducing threat, although highly distressed individuals may struggle to be self‐compassionate. This study explored the effects of compassionate interactions with staff on inpatients with psychosis.Experience Sampling Method (ESM) was used to investigate the relationships between compassion from staff and paranoia, voice hearing, distress related to psychosis experiences, affect and risk incidents in daily life. Twenty‐two service users residing on inpatient mental health wards took part. Baseline measures of compassion, fears of compassion and affect were taken. Participants completed ESM assessments 10 times per day, over 6 days.Compassion from staff was associated with a small increase in voice hearing, but was not associated with paranoia, paranoia‐related distress, voice‐related distress, negative affect, positive affect, or risk incidents in daily life. Baseline fears of compassion moderated the relationships between compassion from staff and some of the service user outcomes. For inpatients scoring low on fears of compassion, compassionate interactions were associated with increased positive affect and lower paranoia. However, for those scoring high on fears of compassion, this relationship was reversed, and compassionate interactions were associated with higher paranoia and lower positive affect.People with psychosis who have fears of compassion may benefit from receiving support to address these fears in order to experience the benefit from the compassion of staff. Paranoia is a common delusion type found in clinical and non-clinical populations. A hierarchical, dimensional model of paranoia in the general population has been proposed, with four categories representing increasing levels of paranoia: interpersonal sensitivity (IP), mistrust (M), ideas of reference (IR), persecutory ideas (PI). What is currently lacking and could provide insights into etiology is a comprehensive clinical characterization of the lower end of the paranoia spectrum, psychological domains that are associated with symptom severity, and how paranoia and its structure fluctuate over time. This study conducted both cross-sectional and longitudinal surveys with 802 participants from the German population assessing paranoia and general psychopathology. Data was collected through Ecological Momentary Assessment (EMA). Several statistical approaches were used including confirmatory factor analysis (CFA), latent class analysis (LCA) and mixed modelling analyses (ME). Paranoid experiences appear to be a common phenomenon that occur in people with and without mental illness. Subjects clustered into four paranoia severity subgroups (IP, M, IR, PI) and showed significant associations in various psychological domains like increased psychiatric symptoms and maladaptive coping. Paranoia fluctuates over time in all four severity subgroups, but the hierarchical subgrouping was stable. Both persecutory ideations and interpersonal sensitivity were significant predictors of paranoia. Findings provide important insights into the architecture of paranoia in the German population by characterizing their hierarchical, dimensional, and dynamic structure and its link to psychopathology. Paranoid ideation is a transdiagnostic construct that is associated with social impairment and often occurs in psychotic spectrum disorders. Little research has examined how paranoid ideation is related to social behaviors that underlie social impairment and may ultimately lead to social rejection. It is important to consider that negative symptoms and sleep problems also contribute to social impairment. No research has assessed the unique and combined influence of paranoid ideation, negative symptoms, and sleep problems on social impairment. Therefore, the current study examined how paranoid ideation, negative symptoms, and sleep problems contribute to poorer social skills and social rejection in a transdiagnostic sample of persons with psychosis and community members (N = 112). Assessments included diagnostic and symptom interviews, questionnaires, behavioral ratings of social skill and facial displays of affect, and naive observer reactions utilizing thin-slice methodology. Greater paranoid ideation, negative symptoms, and sleep problems were each related to poorer social skill and more negative reactions from observers. When considered in path analyses, negative symptoms were associated with observer reports of less willingness to interact with participants through poorer social skill. Objectives: People with psychosis delay accessing recommended treatments, resulting in poor healthcare outcomes and increased risk of relapse. Means of improving help‐seeking and help‐acceptance could reduce duration of untreated psychosis (DUP). This study examined the role of attachment style in help‐seeking and help‐acceptance in psychosis. Design: We used an experimental design to test the effect of attachment imagery priming on help‐seeking and help‐acceptance intentions, in a sample with self‐reported psychosis. The independent variables were attachment imagery condition (secure vs. avoidant) and time (pre‐ vs. post‐prime). The dependent variables were state paranoia, help‐seeking intentions and help‐acceptance intentions. Methods: We used an online research platform to recruit people with psychosis (n = 61). Participants were randomly allocated to the secure or avoidant attachment priming condition. All completed measures of state paranoia, help‐seeking, and help‐acceptance, before and after priming. Results: In comparison with the avoidant condition, secure attachment imagery resulted in reduced paranoia and increased help‐seeking and acceptance intentions, all with large effect sizes. Conclusions: This is the first study to use an experimental design to assess the role of attachment style in help‐seeking and help‐acceptance in a clinical sample. Attachment style is causally linked to behavioural intentions that contribute to DUP. Clinicians should assess attachment and help‐seeking and acceptance, highlight these in formulation, and prioritise in treatment planning. AbstractEmpathy is predominantly assessed with self-report questionnaires. However, their structural validities were not well-supported. This study aimed to re-explore and refine the factor structure of the Chinese version of the Questionnaire of Cognitive and Affective Empathy (QCAE) and investigate the pathways linked between dimensions of empathy and schizotypy. Data from a valid sample of 1,360 community-dwelling adults (aged 18–35) were subjected to the exploratory graph analysis (EGA) and bootstrap EGA for factor retention. A goodness-of-fit evaluation was conducted using confirmatory factor analysis (CFA). Lastly, a Gaussian graphical model with sum scores of the resultant empathy dimensions, positive, negative, and disorganized schizotypy, and paranoia as nodes was estimated. Results supported a three-factor structure for the revised 20-item QCAE, demonstrating a good model fit. The new Online simulation subscale was associated with reduced disorganized schizotypy, whereas the new Perspective-taking subscale was associated with decreased disorganized schizotypy and increased positive schizotypy. The composite Affective empathy subscale was associated with decreased negative schizotypy and increased positive and disorganized schizotypy and paranoia. Overall, the revised QCAE demonstrated good structural validity, measuring three separable and internally cohesive factors of empathy. Background: Nurses are increasingly demanded to achieve gold-standards of care with fewer resources. Dealing effectively with stress experienced in their daily-work-life is thus crucial. This study is based on the Demands-Resources-and-Individual-Effects (DRIVE) Nurses Model and applied the person-centred approach with a twofold objective: 1. to identify patterns of coping strategies (Problem-Focused; Seek-Advice; Self-Blame; Wishful-Thinking; Escape/Avoidance) adopted by nurses to deal with perceived stress; 2. to explore potential differences in perceived Demands (Effort), Resources (Rewards, Job-Control, Social-Support), and Psychopathological Symptoms (Anxiety, Phobic-Anxiety, Obsessive–Compulsive, Somatization, Depression, Interpersonal-Sensitivity, Hostility, Psychoticism, Paranoid-Ideation) according to the emerged patterns. Method: This cross-sectional study was reported by using the STROBE Checklist. Overall, 265 nursing professionals completed self-report measures. Non-hierarchical k-means-cluster-analysis was employed to derive patterns of coping. MANOVAs were used to test differences in Demands, Resources, and Psychopathological Symptoms according to the emerged patterns. Results: Three stable and meaningful patterns of coping were identified and labelled as Active/Solution-Oriented, Dysregulated/Emotion-focused, and Passive/Disengaged. Nurses belonging to Dysregulated/Emotion-focused group emerged to be at higher risk (higher effort/psychopathological suffering; lower resources) – followed by Passive/Disengaged group – in comparison with nurses belonging to Active/Solution-Oriented group. Conclusion: Fostering nurses' awareness of their latent coping patterns and supporting active approaches/emotional regulation strategies for stress management should represent a key goal when defining interventions promoting nurses' health within/beyond the healthcare settings. The messianic structure of human experience is manifest in the form of waiting—for an object who will alleviate suffering, and thereby, bring about change. The underpinning of such waiting is imagined here as messianic hope. In individuals, families, cults, religion, and politics, we may detect such hope—one that perhaps makes the unbearability of life bearable. This longing may get concretized in the shape of a person who comes to represent a link with God. From different vertices, messianic power may get located in the analyst, patriarch, the godman, or the political leader. Links that get forged between such figures and those who are waiting, may be thought of as messianic links. As a messianic group gets consolidated, it often displays certain primitive features. Such groups tend to veer towards a display of omnipotence, resolute action, a solution‐driven language. They may seek to cut off contact with painful reality and anoint someone who can enable that. Ideas of time are wrenched away from its painful association with loss. Time is made predictable and repetitive. Deliverance is promised, incertitude discarded. Messianic language is often evocative and enigmatic. It can be an expression of impotency. But language may also be used to fuel omnipotent longing, as with the use of omniscience by the anointed messiah. Such messianic groups may come together with shared magical beliefs in the annointed figure. But links by their very nature are dynamic and so, a link fueled by awe may also devolve into paranoia or else dependency or may be discarded altogether. This paper looks at two documentaries, and a novel, to give shape to messianic links. Here we see instances of how enigma may be replaced by charisma, and strength by hypervigilance. In that sense it seems that regardless of how they originate, they are condemned to move towards what Bion (1962) calls “minus links” or links that lead away from truth and the growth of the mind. We see that messianic links may either end catastrophically, or decay and degenerate to the point of disappearing, or else, lose the kernel, but assiduously preserve the shell, or finally institutionalized as deifying links. Objectives: Mental health triage services are the first contact for people wanting support for their mental health and provide access to public mental health services in Victoria, Australia. People living with young-onset dementia and their families may contact triage services seeking assistance for behaviour changes and pathways to care as there are few alternative services available. Methods: The authors reviewed the minimum triage dataset from one of the largest mental health services in Victoria, Australia from 2018 to 2021 investigating reasons for referral for people with young-onset dementia. Results: Of the 1766 referrals for 'dementia', 145 were for young-onset dementia. Of these, 69% were referred for behaviour changes. About half of the referrals for behaviour changes were triaged as 'advice only'. One-third of referrals were from general practitioners. Conclusions: The availability of dementia-specific support services that could provide advice on dementia as well as advice for behaviour changes related to the disease could potentially ease the burden of phone calls received by mental health triage services. What is known about the topic? Young-onset dementia refers to a dementia with onset at less than 65 years of age. People with young-onset dementia may contact mental health services for support but the reasons for contact are unknown. What does this paper add? The reasons for contacting one Victorian mental health service included behaviour changes such as agitation or aggression and psychotic symptoms (74%) and general advice, including about dementia pathways (14%). General practitioners made up one-third of referral sources. What are the implications for practitioners? Victorian mental health services are generally overloaded and complex. The prediction error account of delusions has had success. However, its explanation of delusions with different contents has been lacking. Persecutory delusions and paranoia are the common unfounded beliefs that others have harmful intentions towards us. Other delusions include believing that one's thoughts or actions are under external control or that events in the world have specific personal meaning. We compare learning in two different cognitive tasks, probabilistic reversal learning and Kamin blocking, that have relationships to paranoid and non-paranoid delusion-like beliefs, respectively. We find that clinical high-risk status alone does not result in different behavioural results in the probabilistic reversal learning task but that an individual's level of paranoia is associated with excessive switching behaviour. During the Kamin blocking task, paranoid individuals learned inappropriately about the blocked cue. However, they also had decreased learning about the control cue, suggesting more general learning impairments. Non-paranoid delusion-like belief conviction (but not paranoia) was associated with aberrant learning about the blocked cue but intact learning about the control cue, suggesting specific impairments in learning related to cue combination. We fit task-specific computational models separately to behavioural data to explore how latent parameters vary within individuals between tasks and how they can explain symptom-specific effects. We find that paranoia is associated with low learning rates in the probabilistic reversal learning task and the blocking task. Non-paranoid delusion-like belief conviction is instead related to parameters controlling the degree and direction of similarity between cue updating during simultaneous cue presentation. These results suggest that paranoia and other delusion-like beliefs involve dissociable deficits in learning and belief updating, which, given the transdiagnostic status of paranoia, might have differential utility in predicting psychosis. Research on paranoid beliefs in adolescents is in its infancy. Valid and reliable assessments are essential to advancing the field, yet there is no current consensus as to which are optimal to use in this population. This study compared the psychometric properties of two measures of paranoia in a general population adolescent sample. A cross-sectional study with quota sampling (gender and age) recruited adolescents (14–17 years) from the UK (n = 262) and USA (n = 200), who completed the Revised Green et al. Paranoid Thoughts Scale (R-GPTS) and the Bird Checklist for Adolescent Paranoia (B-CAP). We assessed factor structures, intercorrelations, overlap of participants identified as at-risk for paranoid thoughts via both scales, convergent validity (scales with one another) and discriminant validity (distress, wellbeing, bullying and discrimination). Both scales performed equally well in terms of factorial validity. Intercorrelations between the subscales and with general distress were high for both measures. However, a substantial percentage of participants were identified as having paranoid beliefs according to the R-GPTS but not the B-CAP. Furthermore, the B-CAP showed a very high correlations (0.69 ≤ r ≤ 0.79) with self-reported bullying experiences, which bordered on multicollinearity. Findings highlight the possibility that B-CAP may risk confounding paranoid beliefs with exposure to bullying more so than R-GPTS, and that B-CAP may miss instances of elevated paranoia that are captured by the R-GPTS. This case report focuses on what patients and family members may experience when a neurological trauma transpires and resultant intensive care (ICU) delirium occurs. It is the personal account of the patient (A.B.) and spouse's (R.G.B.) perspectives when the patient (A.B) suffered a vertebral artery aneurysm and hemorrhage and experienced intensive care unit (ICU) delirium after being in the ICU for 22 days. This case report provides the patient's and spouse's perspectives regarding delirium, i.e., A.B.'s inability to discern reality, loss of memory, paranoia and hallucinations, agency and recovery, post-ICU syndrome, and post-traumatic stress disorder (PTSD). Clinical diagnosis by the neurosurgeon indicated delirium, with treatment consisting of sleep sedation and uninterrupted sleep. A.B. was able to regain consciousness yet experienced post-traumatic stress disorder up to one year afterward. Consistent family participation in the patient's delirium care is crucial. • Paranoia uniquely predicts indirect aggression in socially anxious individuals. • Social status threat, paranoia, and social anxiety predict direct aggression. • Paranoia may be important contributor of aggression in socially anxious individuals. Aggression is a transdiagnostic behavior that is associated with poor clinical outcomes. As such, it is important to understand factors that contribute to various manifestations of aggressive behavior. Recent research has revealed a subtype of individuals with social anxiety disorder (SAD) who tend to display relatively high amounts of aggression and experience more severe social anxiety and dysfunction compared to individuals in the prototypical SAD group. The current study used a status threat manipulation along with behavioral indices of aggression to examine the impact of paranoia and social anxiety symptom severity on aggression in a sample of undergraduates with social anxiety (N = 220). Analyses indicated that paranoia uniquely predicted indirect aggression whereas an interaction between social status threat, paranoia, and social anxiety severity uniquely predicted direct aggression. Background: Social factors can play an important role in the development and maintenance of psychosis. Clarifying this relationship is vital for advancing theoretical understanding and development of targeted interventions. Psychosis is increasingly researched with an experience sampling methodology (ESM), which provides an ecologically valid approach, that reduces recall biases. Studies examining momentary associations between social factors and psychosis have not yet been summarised. Method: We identified 29 ESM studies investigating associations between social factors and positive psychotic experiences through a pre‐registered systematic search of the published literature. Results: Being alone did not predict increase in psychosis; however, appraisals and feelings associated with being alone such as feeling socially disconnected, lonely and unwanted did. Being with familiar company was found to reduce psychosis experiences but feeling stressed by the current company increased psychosis. Conclusions: While issues with sample size and generalisability mean these results should be interpreted with caution, some putative conclusions can be made. Individuals with psychosis or emerging symptoms should be offered interventions that improve social networks such as peer support, community participation and engagement skills training. These individuals may also benefit from virtual reality or compassion‐based interventions which aim to dampen perceived social threat. Moreover, digital interventions which monitor changes in social variables that predict relapse in symptoms would allow early intervention to prevent mental health crises. [ABSTRACT FROM AUTHOR]

Background: Paranoia is relatively common but can lead to significant distress, impairment and need for care. Digital technologies offer a valuable extension to service provision and are increasingly being integrated into healthcare. This systematic review evaluated feasibility, acceptability, and effectiveness of digitally enhanced psychological assessments and treatments for paranoia across the paranoia continuum (PROSPERO: CRD42023393257). Methods: Databases PsychINFO, EMBASE, MEDLINE and Web of Science were searched until 12 June 2023; the Effective Public Health Practice Project (EPHPP) quality assessment tool evaluated studies; and a narrative synthesis was conducted. Results: Twenty‐seven studies met inclusion criteria (n = 3457, 23 assessment and 4 treatment, 2005–2023, most in Europe). Technologies included virtual reality (VR, n = 23), experience sampling methodology (ESM, n = 2), an app (n = 1) and a combination of VR and ESM (n = 1). Assessments involved monitoring paranoia under various virtual conditions or in everyday life. Treatments were generally integrated with Cognitive Behaviour Therapy (CBT), which involved using VR to test out threat beliefs and drop safety behaviours or using an app to support slowing down paranoid thinking. EPHPP ratings were strong (n = 8), moderate (n = 12) and weak (n = 7). Conclusions: Digitally enhanced assessments and treatments showed promising acceptability, feasibility and treatment effectiveness. Limitations of studies include small sample sizes, lack of comparison groups and long‐term data and limited randomised controlled trials. Results support the potential future integration of VR in the assessment of paranoia and show promise for treatments such as CBT, although further clinical trials are required. This article examines the question of technological disconnection in Jean-Philippe Toussaint's La Clé USB (2019). It shows how Toussaint's novel offers a vision of everyday life in which existence is tantamount to being logged on, so to speak, in perpetuity. I consider how La Clé USB testifies to the fraught process of technological disconnection. I offer an extended analysis of the novel's narrator, Jean Detrez, focusing specifically on his attempts to 'disconnect' from technological mediation, which result in paranoia, guilt and corporeal disembodiment. Ultimately, I argue that La Clé USB draws our attention to the affective, sensorial and relational dimensions of everyday life and compels us to reckon with their potential degradation in our hyperconnected digital age. While the origins of two of Kraepelin's three subtypes of dementia praecox (DP), catatonic and hebephrenic, are well understood, no similar clear narrative exists for his concepts of paranoia and paranoid DP, which require a consideration of both German and French sources. An important milestone in the French literature is the massive 524 page monograph entitled "Le Délire Des Persécutions" published in 1871 by Henri Legrand du Saulle which contained extensive, clinically detailed descriptions of a wide range of cases with prominent, organized persecutory delusions. Many of his cases reported auditory hallucinations (AH), and some bizarre, Schneiderian delusions. The delusional content could evolve to include prominent somatic and/or grandiose themes. Using a symptomatic diagnostic framework, Legrand du Saulle proposed that this syndrome represented an independent "species" of mental illness. He sought to give a voice to the affected individuals, including a chapter devoted entirely to their writings. He described several clinically fascinating features of such patients including how often they moved residence to unsuccessfully flee their persecutors and how delusional beliefs could be communicated to spouses and relatives. Unlike Kraepelin, he was little interested in their course of illness or rates of deterioration, except to note that recoveries occurred in 20% of cases. The clinical richness of this work substantially exceeded that in the contemporaneous German literature. Most of the cases described by du Saulle would fit easily into the two major non-affective delusional syndromes articulated 28 years later in Kraepelin's famous 6th edition of his textbook: paranoia and paranoid DP. BACKGROUND: The stress due to the COVID-19 pandemic has influenced the physical, mental, and social well-being of humans worldwide. OBJECTIVE: To evaluate the relationship between the fear of COVID-19 and mental state of female employees during the COVID-19 pandemic. METHODS: A cross-sectional study was conducted involving 726 participants. Data was collected using the Personal Information Form, fear of COVID-19 scale, and brief symptom inventory. RESULTS: The fear experienced by women during the social isolation and quarantine period was associated with depression, anxiety, somatization, obsessive-compulsive disorder, interpersonal sensitivity, hostility, phobic anxiety and paranoid experiences. CONCLUSION: Women, young people, the elderly and single individuals were most likely affected psychologically during the pandemic. Thus, interventions and psychological evaluations are recommended at an early stage to minimize this effect. The current study examined the novel role of solo status, or being the only Muslim in different settings, in the relationship between discrimination and distress for Muslim women in the United States who wear the hijab, the Islamic headscarf, and Muslim women who do not wear the hijab. Psychological distress was examined through symptoms of anxiety, somatization, and paranoia. Results from a sample of 123 Muslim women in the United States showed that the impact of solo status was different for Muslim women who always wear the hijab compared to Muslim women who never wear the hijab. Namely, high solo status exacerbated the impact of discrimination on all psychological symptoms for Muslim women who wear the hijab. In contrast, low solo status compounded the effect of discrimination on all psychological symptoms for Muslim women who never wear the hijab. Positive schizotypy can uniquely predict the development of psychosis with suspiciousness/paranoia having emerged as a key risk factor, pointing to significant worth in reducing this aspect in individuals with high positive schizotypy. Reduced paranoia in the general population following brief online mindfulness training has been previously reported. This study investigated the feasibility of a 40-day online mindfulness-based intervention (MBI) (n = 12) in the individuals with high positive schizotypy characterized by high suspiciousness/paranoia and to estimate its effect on paranoia as compared with an active control condition using reflective journaling (n = 12). The outcome measures were self-reported trait and VR-induced state paranoia, completed at baseline, after 10 days and post-intervention. The feasibility criteria included retention, adherence, engagement, and acceptability. There was 100% retention, excellent adherence to content and engagement, with an average MBI session completion rate of 91%. Acceptability, indexed by a self-rated motivation to continue practice post-intervention, was also high. No MBI effect on trait paranoia was observed; however, the MBI group showed a reduction in the VR-induced state paranoia with a medium-to-large effect (d = 0.63). The findings support conducting larger-scale randomized controlled trials to evaluate the effects of online MBIs on reducing suspiciousness/paranoia to mitigate psychosis risk in individuals with high positive schizotypy. Background: Aerobic exercise plays an influential role in improving mental health. Aerobics, as a form of efficient aerobic exercise, is suitable for various individuals and promotes the mental health of college students. Objectives: The purpose of this study was to explore whether aerobic exercise has a positive effect on the mental health and Body Mass Index (BMI) of obese college students. Methods: Sixty obese college students with a BMI ≥ 28 were selected and divided into two groups. Each group consisted of 30 students, 15 boys and 15 girls. The experimental group received aerobic exercise training three times a week for 12 weeks, while the control group received prescribed aerobic training. Data before and after the experiment were recorded. Results: The BMI of the experimental group decreased from 30.19 ± 0.73 before the experiment to 27.32 ± 0.84 after the experiment, with a change range of 2.86 ± 0.11. The experimental group showed significant differences before and after the experiment (P < 0.01). The BMI of the control group decreased from 30.16 ± 0.73 before the experiment to 28.62 ± 0.89 after the experiment, with a change range of 1.54 ± 0.84. The control group also showed significant differences before and after the experiment (P < 0.05). The experimental group had a better effect. The 12 dimensions of the mental health test changed in both the experimental group and the control group, and the results were statistically significant (P < 0.05), with the experimental group showing better improvement. Conclusions: The statistically significant impact of aerobics on the mental health of obese college students holds practical significance. Notably, the observed reduction in anxiety and depression symptoms, coupled with notable enhancements in self-esteem and self-confidence, underscores the tangible benefits of incorporating aerobics into the lifestyle of this demographic. The article explores the hypothesis that Utopian and Paranoid SF, both of which produced some of their most influential texts in the 1970s, co-evolved under structurally similar pressures and developed analogous conceptual instruments to engage with the question of totality. It proposes a theoretical model that situates the two subgenres in a network of conceptual positions regarding fundamental categories such as space, time and subjectivity. The model is then applied in readings of key novels of Paranoid SF: Robert Shea and Robert Wilson’s Illuminatus! Trilogy, Philip Dick’s Ubik, A Scanner Darkly and VALIS, and Thomas Pynchon’s Gravity’s Rainbow.

The COVID-19 pandemic has exacerbated anxiety and depression disorders. We used network analysis to identify core symptoms and comorbid pathways between the two disorders and reveal temporal changes in symptoms that traditional assessment tools fail to capture. Data was collected from 533 clinically comorbid patients (Mean age = 43.4; female = 363) who completed the Hamilton Anxiety Scale and Hamilton Depression Scale before and after treatment (T1→ T2). Node and bridge strengths were calculated, and analyses included a network comparison test (NCT) and cross-lagged network analyses (CLPN) to examine the interconnectedness and changing features of anxiety and depression comorbidity. Results indicate that (a) In contemporaneous networks before treatment, the most central nodes were tension, somatization, anxiety (somatic), depressed mood, psychomotor retardation, somatic symptoms (GI), autonomic symptoms, and hopelessness. Additionally, depressed mood, psychomotor retardation, somatic symptoms (GI), and hopelessness were also bridge nodes associated with psychological and physical symptoms of anxiety. (b) Although the total score of the assessment tools decreased during treatment and NCT confirmed overall network strength remained stable during treatment (p = 0.38), specific symptoms changed significantly (p < 0.05), underscoring the importance of nuanced evaluations beyond total scores. Specifically, nodes such as fear, insomnia, intellectual, suicide, psychomotor agitation, anxiety (psychic), loss of weight, and helplessness exhibited a significant reduction (p < 0.05). In contrast, nodes including genitourinary symptoms, insomnia (late), work and interests, and somatic symptoms (general) demonstrated a significant enhancement (p < 0.05). Conspiracy culture cultivates suspicion towards the hidden workings of power. While some modes of suspicion direct critical attention towards the crimes and cruelties of oppressive social relations, other modes misdirect that same attention. When such misdirection serves to reproduce oppressive social relations, entrapping its adherents in the promise of emancipation, this may be understood as ‘repressive suspicion’. Empirically, this concept is characterized, herein, via a reception study of the QAnon conspiracy theory, reconstructing how one of the most prominent participants in the insurrection in Washington, D.C., on 6th January 2021 became wrapped up in ‘Q’ culture – a story that paints a poignant, complex picture of repressive suspicion. This concept is then further developed in historical and theoretical terms, in dialogue with the works of Herbert Marcuse, leading to an analysis of QAnon as a microcosm of the contemporary crisis of hegemony, drawing on Antonio Gramsci, Stuart Hall, and Nancy Fraser. While, in the 1960s, and even in the 1990s, repressive suspicion could be largely ignored by practitioners of political critique, in the current conjuncture this is no longer the case. Objective: To explore the psychological personality characteristics of transgender groups and to determine whether these characteristics differ according to sociodemographic factors. Methods: This cross-sectional study was conducted between January 2021 and April 2023 at a psychosexual outpatient clinic in a psychiatric hospital in Beijing, China. In total, 481 individuals were included in this study, and demographic information was collected using a self-administered general questionnaire. Psychological personality traits were assessed using the Minnesota Multiphasic Personality Inventory (MMPI). Results: The mean scores of the assigned male at birth (AMAB) group were significantly higher than those of the male controls for all 10 clinical factors of the MMPI (p < 0.01 or p < 0.001). The scores for both the Masculinity-femininity (Mf) and Depression (D) factors in the AMABs group exceeded the clinical threshold (T > 60) and were the highest and second-highest scores on the entire scale, respectively. Individuals assigned female at birth (AFAB) had significantly higher scores than female controls for Hysteria (Hy), Psychopathic Deviate (Pd), and Hypomania (Ma) (p < 0.05, p < 0.01, and p < 0.001, respectively). There were significant differences in the rates of abnormal values for the various factors of the MMPI (T > 60) according to gender, age, and education (p < 0.05, p < 0.01, and p < 0.001, respectively). Compared to AFABs, AMABs had higher rates of abnormal scores (T > 60) on the Hypochondriasis (Hs), D, Hy, Mf, Paranoia (Pa), Psychasthenia (Pt), Schizophrenia (Sc), and Social Introversion (Si) scales (p < 0.05, p < 0.01, and p < 0.001, respectively). Second, the transgender group aged ≤25 years had higher rates of abnormal scores (T > 60) on the Hs, D, Hy, Pd, Pa, Pt, Sc, and Ma scales (p < 0.05, p < 0.01, and p < 0.001, respectively). Finally, outliers (T > 60) for the Hs, D, Hy, Pd, Pa, Pt, Ma, and Si factors were more prevalent among those with a primary to high school level of education (p < 0.05, p < 0.01, and p < 0.001, respectively). Conclusion: Assigned male at births may have a variety of psychological vulnerabilities, and there is a need to focus especially on those with a primary to high school level of education, those aged ≤25 years, and transgender females. Background Methods Results Conclusions Virtual Reality cognitive behavioral therapy (VR‐CBT) has proven to be an effective treatment method for paranoia and anxiety in psychosis. However, it is unknown, which individuals benefit most from VR‐CBT. Previous studies examined factors affecting the treatment effect of regular CBTp, including illness duration, paranoia, depression, and pre‐therapy avoidance behaviors, but results are inconsistent. The study aims to investigate the factors that influence the effectiveness of VR‐CBT.A total of 95 participants with a psychotic disorder and at least moderate paranoia (GTPS >40) were included in this explorative study. Data were collected as part of a multicenter randomized controlled trial in which participants were assigned to VR‐CBT or treatment as usual (TAU). The VR‐CBT group received 16 sessions of individual treatment. A moderator analysis was conducted to examine the influence of baseline demographic (age, gender, and education level) and clinical characteristics (duration of illness, paranoia, anxiety, depression, safety behavior, self‐esteem, and social functioning) on treatment effects of paranoia and anxiety as measured with questionnaires and the experience sampling method (ESM) directly after treatment (12 weeks after baseline).More use of safety behavior at baseline resulted in greater benefits of VR‐CBT on paranoid ideation and ESM paranoia. A higher age was associated with greater benefits of VR‐CBT on social anxiety but not paranoia outcomes. There was no consistent evidence of moderation by any of the other sociodemographic or clinical variables for paranoid ideation and social anxiety.Our findings suggest that a diverse spectrum of patients, with different backgrounds and symptom severity may be able to benefit from VR‐CBT. VR‐CBT can be recommended to a broad spectrum of patients with psychotic disorders, and particularly those with high levels of safety behaviors, including severe avoidance, seem to benefit more. Background: Despite substantial progress in investigating its psychophysical complexity, tinnitus remains a scientific and clinical enigma. The present study, through an ecological and multidisciplinary approach, aims to identify associations between electroencephalographic (EEG) and psycho-audiological variables. Methods: EEG beta activity, often related to stress and anxiety, was acquired from 12 tinnitus patients (TIN group) and 7 controls (CONT group) during an audio cognitive task and at rest. We also investigated psychological (SCL-90-R; STAI-Y; BFI-10) and audiological (THI; TQ12-I; Hyperacusis) variables using non-parametric statistics to assess differences and relationships between and within groups. Results: In the TIN group, frontal beta activity positively correlated with hyperacusis, parietal activity, and trait anxiety; the latter is also associated with depression in CONT. Significant differences in paranoid ideation and openness were found between groups. Conclusions: The connection between anxiety trait, beta activity in the fronto-parietal cortices and hyperacusis provides insights into brain functioning in tinnitus patients, offering quantitative descriptions for clinicians and new multidisciplinary treatment hypotheses. Purpose: Beliefs that emotions can be changed (i.e., malleability beliefs) are relevant to emotion regulation. Emotion regulation plays a crucial role in severe mental health symptoms, such as paranoid delusions, but it remains unknown to which extent malleability beliefs contribute to a dysfunctional pattern of emotion regulation in individuals experiencing paranoid ideation. Therefore, we investigated whether malleability beliefs are associated with paranoid ideation and whether emotion regulation accounts for this association. Methods: We conducted a cross-sectional assessment in a sample of individuals with psychotic disorders (n = 50) and a community sample (n = 218) and collected self-report data on malleability beliefs, the use of emotion regulation strategies (reappraisal, acceptance, rumination, expressive suppression, experience suppression), and paranoid ideation. Results: Multivariate regressions showed that greater beliefs that emotions are malleable predicted more reappraisal and acceptance in both samples and less rumination in the community sample. Malleability beliefs did not predict the strategies of suppressing an emotion or its expression. In the community sample, but not in the clinical sample, greater beliefs that emotions are malleable were associated with less frequent paranoid ideation and emotion regulation accounted for significant variance in the relationship. Conclusions: The findings indicate that malleability beliefs do not contribute to paranoid delusions in individuals with psychotic disorders. However, in individuals with subclinical paranoid ideation, a failure to perceive emotions as malleable could hinder adaptive attempts to regulate emotions leading to increased negative affect and thereby pave the way for paranoid ideation. Research highlights sextortion to have severe psychological consequences, with over a dozen cases of suicide among victims. This paper used two transcripts from the media of victims' experiences, as well as qualitative survey responses from an additional 99 victims, to explore in detail how sextortion affects victims psychologically. A main theme was low self-esteem leading to depression, within which feelings of hopelessness and negative thinking of oneself were prominent. The mounting generation has been struggling from a lack of understanding and assistance regarding sex since everyone desires a fantastic sex life, which is incorrectly portrayed in pornographic films, resulting in sexual anxiety, an important hindrance to an individual's sexual satisfaction. Sexual anxiety involves feelings of fear, trepidation, and worry about sexual activities or performances; it can also perpetuate a detrimental cycle in which people become paranoid about sexual performance and are afraid of unsatisfactory experiences and future performance. This study aims to delve into the complex realm of sexual anxiety among unmarried young adults and gender differences. The data was acquired through Google Forms. The sample comprises 138 unmarried individuals (55 male & 83 female). The instrument utilized in the study was the Sexual Anxiety Scale (SAS) by Pramod. In the present study, descriptive statistics and the Mann-Whitney U test were employed using a statistical package for the social sciences (SPSS). The findings reveal that 16.67% of the sample have high levels of sexual anxiety. Results of the Mann-Whitney U Test indicated a statistically significant difference in sexual anxiety levels between men and women. 21.69% of women experience high sexual anxiety which is higher than men who experience 9.08% of sexual anxiety which illuminates a pronounced disparity that reveals that women experience higher levels of sexual anxiety than men. The study also defines the limitations and future implications of the study. Understanding the factors causing higher sexual anxiety in women is crucial for developing tailored interventions and support mechanisms to address these nuanced needs of individual and socio-cultural factors. With the help of this, it may be possible to reduce the burden of sexual anxiety among women, improve their sexual well-being, and promote gender equality in sexual health and satisfaction. This brief report concerns a patient interaction in a busy South London Home Treatment Team (HTT) service, where a psychodynamically informed approach was applied to a presentation of psychosis in a crisis setting. We discuss the case in light of the psychodynamic theories on psychosis of Carl Jung and Richard Lucas. Our patient, a young man, had been under the HTT for an extended period of time due to an episode of psychosis that was not resolving. After initially struggling to build rapport, we adapted Lucas' and Jung's models of interacting with acutely psychotic patients to re-establish psychic rapport, which led to our patient agreeing to take the medication that finally pulled him out of this long-standing episode. We believe this case highlights the power of psychodynamic thinking even in acute, short-term settings where extensive psychodynamic intervention may not be appropriate or feasible. Purpose: Insecure attachment may constitute a vulnerability factor for psychosis, and dissociation may be a key mechanism in the development of auditory hallucinations specifically. While there is good evidence for the role of these processes in isolation, it is unclear whether dissociation accounts for the association between insecure attachment and psychosis. This systematic review takes a theory‐driven approach to examine proposed causal relationships across the clinical and nonclinical literature. Methods: We searched five databases (PubMeD, Web of Science, PsycINFO, CINAHL and ETHOS) for published and unpublished research examining attachment, dissociation and psychosis. Two independent reviewers extracted the data and assessed the quality of all included studies. Results: We identified 242 potential articles and included 13 in the final review (2096 participants). We found that (1) disorganised attachment was consistently associated with dissociation and inconsistently associated with voices and paranoia, (2) dissociation was associated with voices and paranoia, and these links were stronger in clinical samples, and (3) dissociation played a role in the impact of insecure attachment on voice hearing and paranoia in clinical groups. Conclusions: This is the first review to synthesise the research examining attachment, dissociation, and psychosis. The evidence is consistent with proposed causal hypotheses and raises conceptual and measurement issues, for example, the need to clarify the relative contributions of different insecure attachment styles, and utilise behavioural/observational measures to strengthen study designs. Background: Psychotic experiences (PEs) and social isolation (SI) seem related during early stages of psychosis, but the temporal dynamics between the two are not clear. Literature so far suggests a self-perpetuating cycle wherein momentary increases in PEs lead to social withdrawal, which, subsequently, triggers PEs at a next point in time, especially when SI is associated with increased distress. The current study investigated the daily-life temporal associations between SI and PEs, as well as the role of SI-related and general affective distress in individuals at clinical high risk (CHR) for psychosis. Methods: We used experience sampling methodology in a sample of 137 CHR participants. We analyzed the association between SI, PEs, and distress using time-lagged linear mixed-effects models. Results: SI did not predict next-moment fluctuations in PEs, or vice versa. Furthermore, although SI-related distress was not predictive of subsequent PEs, general affective distress during SI was a robust predictor of next-moment PEs. Conclusions: Our results suggest that SI and PEs are not directly related on a moment-to-moment level, but a negative emotional state when alone does contribute to the risk of PEs. Insomnia as a disorder on its own or as a symptom of other mental disorders can lead to significant distress and lower quality of life. By exacerbating negative affect and emotion dysregulation, poor sleep and insomnia can contribute to the initiation and maintenance of mental disorders. The aim of this cross-sectional study was to investigate the relationship between insomnia severity and overall psychiatric symptoms (anxiety, depression, obsessive-compulsive symptoms, somatization, phobic anxiety, hostility, interpersonal sensitivity, paranoid ideation, and psychoticism), and the mediational roles of worry and rumination in this relationship. The data was collected from a community sample of 1444 participants (females 69.39%, Mage = 27.95, SD = 9.37) who completed self-report measures of insomnia severity, worry, rumination, and psychiatric symptoms. The mediational roles of worry and rumination were tested with mediation analysis using the PROCESS Macro. It was found that insomnia severity (β = 0.20, p <.001) significantly predicted psychiatric symptoms directly and via worry and rumination (β = 0.33, p <.001), meaning that worry and rumination partially mediated the relationship between insomnia severity and psychiatric symptoms. The findings were similar after controlling for smoking status, daily screen time, coffee consumption in the evening, weekly exercise frequency, and pre-sleep screen time. Interventions targeting the reduction of insomnia severity and maladaptive emotion regulation strategies (e.g., worry and rumination), as well as the enhancement of adaptive emotion regulation strategies (e.g., positive refocusing and mindfulness), may alleviate the adverse effects of insomnia on psychiatric symptoms. Objective: This study aimed at assessing the effectiveness of cognitive‐behavioural therapy (CBT) integrated with psychoeducation in a group of hypertensive patients with clinically significant psychopathological symptoms. Methods: One hundred hypertensive patients completed the Symptom Checklist‐90‐Revised. Of them, 17 scored above the clinical range (cut‐off = 0.75) on the Global Severity Index and were included in the study. Psychological distress was assessed again after the intervention (T1) and 6 months after the end of treatment (T2). In addition, the cortisol dosage and the heart rate (HR) measurement were collected at both T0 and T2. Then, mediation analyses were carried out to calculate whether psychopathological distress might predict HR through elevated serum cortisol levels, at both T0 and T2. Results: The psychological intervention (CBT integrated with psychoeducation) reduced most of the psychopathological symptoms (anxiety, depression, somatisations, obsessions and compulsions, hostility, interpersonal sensitivity and paranoid ideation) but not cortisol dosage and HR measurement. However, psychological distress indirectly predicted HR via cortisol at T0 but not at T2. Conclusion: These results suggest and encourage the replicability of data in larger sample sizes and the comparison with a control group. Nevertheless, these results highlight a need for a multidimensional assessment of disorders affecting the mental and physical spheres of patients to support their overall well‐being. Childhood adversity (CA) and resilience may impact on paranoia, but mechanisms underlying these associations are largely unknown. In this study, we investigated two potential candidates: irrational beliefs and affective disturbance. Moreover, we investigated the potential moderating role of COVID-19 perceived stress in these associations. A community sample (N = 419, m age = 27.32 years, SD = 8.98; 88.10% females) completed self-report measures. Results indicated that paranoia was significantly associated with CA and resilience (p <.05), and both irrational beliefs and affective disturbance (i.e., depressive and anxiety symptoms) mediated the associations between CA and paranoia. Moreover, depressive and anxiety symptoms partially explained the mediating role of irrational beliefs. These predictive models explained up to 23.52% of variance in paranoia (F(3,415) = 42.536, p <.001). Findings on resilience and paranoia replicated these results, and COVID-19 perceived stress moderated the association between resilience and ideas of persecution. Introduction: Studies have consistently demonstrated increased stress sensitivity in individuals with psychosis. Since stress sensitivity may play a role in the onset and maintenance of psychosis, this could potentially be a promising target for treatment. The current study was the first to investigate whether reactivity to and recovery from daily-life stressors in psychosis change in response to treatment, namely virtual-reality-based cognitive behavioral therapy (VR-CBT). Methods: 116 patients were randomized to either VR-CBT or the waiting list control group (WL). Pre-treatment and post-treatment participants completed a diary ten times a day during six to ten days. Multilevel analyses were used to model the time-lagged effects of daily stressful events on negative affect (NA) and paranoia symptoms to examine reactivity and recovery. Results: There was a significant difference in NA reactivity. VR-CBT showed higher NA at post-treatment compared to pre-treatment than WL (bpre=0.14; bpost=0.19 vs bpre=0.18; bpost=0.14). There was a significant difference in NA recovery and paranoia recovery between the groups at lag 1: VR-CBT showed relatively lower negative affect (bpre=0.07; bpost=-0.06) and paranoia (bpre= 0.08; bpost=-0.10) at post-treatment compared to pre-treatment than WL (bpre=0.08; bpost=0.08; bpre=0.04; bpost=0.03). Conclusion: Negative affect and paranoia recovery improved in response to treatment. Increased NA reactivity may be explained by a decrease in safety behavior in the VR-CBT group. The discrepancy between reactivity and recovery findings may be explained by the inhibitory learning theory that suggests that an original threat reaction may not erase but can be inhibited as a consequence of exposure therapy. There has been growing evidence that college students are at risk for multiple psychological symptoms, and that these psychological symptoms occur with different grades. According to network theory, we investigated this underlying process by examining cross-sectional and longitudinal relationships between psychological symptoms among college students at different grades. A total of 3,701 Chinese first-year university students were recruited to participate in the study and were followed up after 1 and 2 years, and the SCL-90-R was used to assess psychological symptoms. Anxiety and depression were both core symptoms in the three wave cross-sectional networks. In the 1st-2nd grade cross-lagged network, somatization was the strongest predictor, while anxiety was predicted by other symptoms. However, in the cross-lagged network from 2nd-3th grade, the most predictive symptom changed to paranoid ideation and the most predicted symptom changed to obsessive-compulsive. In addition, longitudinally we only found that hostility in the 1st year of college was a strong negative predictor of academic performance. Shame is a painful emotion that plays a key role in a range of mental disorders, mainly when individuals employ maladaptive emotion regulation strategies to cope with it. Heightened feelings of shame may lead to increased levels of paranoid ideas. We explored the association between daily shame and paranoia by using Experience Sampling Methods (EMS) in a student sample and the potential roles of the moderator (centrality and trauma-like characteristics of shame memories and perceived social rank) and mediator variables (maladaptive emotion regulation strategies) in this association. In total, 198 undergraduate students completed self-report questionnaires (Perceived Social Rank, Centrality of Event Scale, Impact of Event Scale, General Paranoia Scale) and ESM measures (i.e., shame, anxiety, depression, emotion regulation, and paranoia). Data were analysed using a multi-level model to accommodate the multi-level structure of the data. Our results indicated that shame at t-1 predicts paranoia at t even when controlling for the effects of anxiety and sadness. Self-blame was found to mediate the relationship between shame and paranoia. Finally, the centrality of the shame memory moderated the effect of daily shame on paranoia. Thus, these results indicate that daily shame, past shame experiences, and a tendency to blame oneself are predictors of paranoia and should be taken into account when assessing and treating people with paranoia. Limitations include limited generalizability and use of single-item measurements. These results need replications in more representative samples of the general population and clinical samples. Litigious paranoia is a mental disorder that includes morbid litigants who are in overwhelming pursuit of justice for a perceived legal wrong. There is substantial socio-occupational dysfunction in these individuals over years. This pathological fixation can result in marked aggression. However, no separate nosological status has been given to this type of mental disorder. Moreover, the close interface between law and psychiatry also limits the actual understanding of the psychopathology of litigants. This consequentially limits the scope of early intervention and increases the risk of harm to one's own self and to others. The forensic psychiatrist therefore works toward identification of fixation, which can be due to underlying obsession, delusion, or overvalued belief. We present a historical overview of litigant paranoia with a case report. The report highlights the need for in-depth evaluation of mental state of morbid litigants by mental health professionals in order to ascertain the underlying psychopathology and initiate early treatment intervention. This article explores the motivations of ideological killers by investigating their justifications for violence. These justifications are divided into three categories: those that are grounded in actual social/political realities that are present in the lives of the perpetrators; those that utilise geographically and/or temporally distant vicarious victimisations; and those that are fictional, i.e., they have no basis in reality. These concepts are illustrated through discussions of a range of perpetrators of ideological violence. The implications of vicarious and fictional motivations are discussed in terms of our understanding of the psychology of perpetrators, the process of radicalisation, and counterterrorism efforts.
