Abstract

This work presents the first analysis of the disruptive locked mode (LM) triggered by the dynamics of a confinement change. It shows that, under certain conditions, the LM threshold during the transient is significantly lower than expected from steady states. We investigate the sensitivity to a controlled n = 1 error field (EF) activated prior to the L-H transition in the COMPASS tokamak, at q _95 ∼ 3, β _N ∼ 1, and using EF coils on the high-field side of the vessel. A threshold for EF penetration subsequent to the L-H transition is identified, which shows no significant trend with density or applied torque, and is an apparent consequence of the reduced intrinsic rotation of the 2/1 mode during this transient phase. This finding challenges the assumption made in theoretical and empirical works that natural mode rotation can be predicted by global plasma parameters and urges against using any parametric EF penetration scaling derived from steady-state experiments to define the EF correction strategy in the entire discharge. Furthermore, even at EFs below the identified penetration threshold, disruptive locking of sawtooth-seeded 2/1 tearing modes is observed after about 30% of L-H transitions without external torque.

Abstract

Atomic Beam Probe (ABP) is an extension of the routinely used Beam Emission Spectroscopy (BES) diagnostic for plasma edge current fluctuation measurement at magnetically confined plasmas. Beam atoms ionized by the plasma are directed to a curved trajectory by the magnetic field and may be detected close to the wall of the device. The arrival location and current distribution of the ions carry information about the plasma current distribution, the density profile and the electric potential in the plasma edge. This paper describes a micro-Faraday cup matrix detector for the measurement of the few microampere ion current distribution close to the plasma edge. The device implements a shallow Faraday cup matrix, produced by printed-circuit board technology. Secondary electrons induced by the plasma radiation and the ion bombardment are basically confined into the cups by the tokamak magnetic field. Additionally, a double mask is installed in the front face to limit ion influx into the cups and supplement secondary electron suppression. The setup was tested in detail using a Lithium ion beam in the laboratory. Abstract

Recently the Li-Beam Emission Spectroscopy (Li-BES) system on COMPASS has reached its full diagnostic power in terms of routine automatic operation in any kind of plasma scenarios and it is normally used as a standard tool for reconstruction of ultra fast density profiles in the edge region of COMPASS plasmas. The purpose of this study is to investigate the advantages and limitations of the COMPASS Li-BES system in characterizing plasma electron density fluctuations. We show how the atomic physics of plasma-beam interactions can affect the interpretation of the measurement at different radial positions and for different electron density profiles. We also demonstrate the usability of generalized sequential probability ratio test for automatic event detection. Using non-perturbative diagnostic, we verify the validity of the stochastic Garcia-model for scrape-off layer filaments and accompanying holes (density deficits)., Comment: suggestions from the coauthors included some text added to explain fig. 9

7. Expression of cancer stem cells markers in urinary bladder urothelial carcinoma and its precursor lesions

Abstract

Background. Cancer stem cells (CSC) and their role in tumorigenesis of various solid tumors have been studied in past decades. Urothelial CSC were first identified 10 years ago and subsequent studies have been performed with the aim to identify reliable markers of CSC. So far, a few studies have investigated a relationship between CSC markers expression in urothelial carcinoma tissue and histopathological characteristics of the tumor. Methods. In our study, we evaluated an immunoexpression of the CSC markers CD24, CD44, CD66 and CD133 in tissue sections of urothelial carcinoma (all tumor grades and stages were included), urothelial carcinoma in situ and non-neoplastic urothelium, totally 218 specimens were enrolled. Results. All studied molecules were expressed either in tumor tissue and non-neoplastic urothelium. Urothelial carcinomas of higher tumor grade and stage expressed molecules CD24 and CD133 significantly more frequently whereas molecules CD44 and CD66 did not show significant association with tumor histopathological features. Conclusions. Our results showed that studied molecules are not suitable for direct detection of CSC in urothelial carcinoma tissue sections, but an expression of molecules CD24 and CD133 is significantly related to urothelial carcinoma grade and stage, which are both important prognostic indicators and therefore an expression of these markers might have a potential prognostic value.

9. The Correlation of Usability, Familiarity, Satisfaction and Reputation with the Loyalty of Student Users of News Websites

Abstract

In a time of increasing competition in online news, it is important for the publishers of news websites to retain loyal users. This paper contributes to the study of loyalty through an investigation of the factors that affect the loyalty of young users of news websites and motivate them to return there. The research sample in this study was made up of students (N=408) at universities in Slovakia who use news websites to search for information every day. Correlation and regression analyses have been used to test the hypotheses. The results of this research confirm that usability, familiarity, satisfaction and reputation have a direct and positive relationship to the users’ loyalty as reflected in daily use of a news website. The correlation shows that loyalty is associated particularly closely with reputation and satisfaction with a selected news website. Regression analysis shows that there are also other variables that affect users’ loyalty.

Abstract

Family Communication Patterns are the methodical way to distinguish between two sides of communication sphere inside the family. The current model suggests a concept of conformity orientated family and conversation orientated family. We point at the mentioned orientation as a main prediction of behavioral manners of family members in communication area. The aim of our study is to introduce mentioned methodical way and describe one of the possible approaches in family communication area. In this paper, we describe Family Communication Patterns instrument which has a set of questions to quantitative measure family communication patterns. Family Communication patterns show how parents communicate with their children. The construct of Family Communication Patterns has been often used by researchers for decades. They have used this construct to describe a family environment influenced by many variables. Researchers also began to examine how family communication patterns are related to children‘s development in some aspects. Original model of these communication patterns was used by researchers to investigate an interest about public sphere, protection children online and parental mediation online as well. Family communication patterns are described in terms of conformity orientation (originally “socio-orientation”) and conversation orientation (originally “concept orientation”). The first of them point at the parent‘s authority and extensive encourage to accept parent‘s advice or opinion. Children in this type of families accept parental authority, maintain consistent relationship and to remain away any misunderstanding with their parents or others. The conformity oriented families are authoritative and controlling families. The conversation orientation includes families within is natural to spend a lot of time together. These family members share their thoughts and activities. One of the main features of conversation orientation families is effort to discussion. It means the children are encouraged to discus with their parents and they are encouraged to express their individual ideas, beliefs and feelings.

Abstract

In modern democracies, senior civil servants have outgrown their classic role of mere implementers of orders given by politicians. Both senior civil servants and politicians serve the same democratic state, and both are heirs to the democratic evolution. Our hypothesis is based mainly on the historically developed division of labor between bureaucracy and politics. Senior civil servants have never been tasked with creating the conditions for more democracy in the state, but instead with creating the conditions for a more effective and successful state. Given that political bodies in which politicians operate have been established as the institutionalized personification of democracy, the task of politicians is above all the promotion of democracy, its values and norms. We have tested that hypothesis on the case of Slovenian senior civil servants and politicians and found out, that both elites are favorable to political freedoms and political equality.

We assessed the effect of medical staff role models and the number of health-care worker sinks on hand-hygiene compliance before and after construction of a new hospital designed for increased access to handwashing sinks. We observed health-care worker hand hygiene in four nursing units that provided similar patient care in both the old and new hospitals: medical and surgical intensive care, hematology/oncology, and solid organ transplant units. Of 721 hand-hygiene opportunities, 304 (42%) were observed in the old hospital and 417 (58%) in the new hospital. Abstract

COMPASS-U, a high magnetic field tokamak with hot walls, will be designed and built at IPP Prague. Unique features of this new device bring noticeable constraints and requirements on plasma diagnostics, which make their development highly demanding. In this paper, the main expected constraints influencing the conceptual design of diagnostic tools for COMPASS-U (high temperature of the vacuum vessel, high plasma density, high heat flux density, strong auxiliary plasma heating, spatial constraints, liquid metals in the divertor) are reviewed and possible solutions are indicated.

Abstract

COMPASS Upgrade is a new medium size, high magnetic field tokamak (R = 0.9 m, Bt = 5 T, Ip = 2 MA) currently under design in the Czech Republic. It will provide unique capabilities for addressing some of the key challenges in plasma exhaust physics, advanced confinement modes and advanced plasma configurations as well as testing new plasma facing materials and liquid metal divertor concepts. This paper contains an overview of the preliminary engineering design of the main systems of the COMPASS Upgrade tokamak (vacuum vessel, central solenoid and poloidal field coils, toroidal field coils, support structure, cryostat, cryogenic system, power supply system and machine monitoring and protection system). The description of foreseen auxiliary plasma heating systems and plasma diagnostics is also provided as well as a summary of expected plasma performance and available plasma configurations.

Abstract

COMPASS-U will be the successor of the existing COMPASS tokamak which is the main experimental facility of the Tokamak Department at the Institute of Plasma Physics of the Czech Academy of Sciences. COMPASS-U tokamak will be a flexible high magnetic field device with an advantage for scaling towards ITER (International Thermonuclear Experimental Reactor) and DEMO (DEMOnstration Power Station). The ITER and DEMO are foreseen to produce more energy than needed to power them. Based on the results obtained from the COMPASS-U it will be possible to learn more about the tokamak device, which will give more knowledge to construct larger of this type devices in the future. COMPASS-U requires new design of the whole structure of the device. One of the most challenging issues is the design of the Central Solenoid (CS) coils and its support structure. This task requires evaluation of many different case studies which include inter alia geometrical, technological and material properties verifications. In this contribution we present development and mechanical investigation of the CS structure. Based on a previous work, which showed the need of further optimization of the CS structure, additional development was done. In this work preload structure was added to the whole system. This allows to include preload as an additional load step. Preload was added to include compression of the CS coils in axial direction. The influence of the impregnation conditions on the CS behavior was checked. Several ways of impregnation application were studied and their influence on the CS structure was examined. Mechanical analysis using Finite Element Method (FEM) includes 2D axisymmetric model of the CS structure. Results for CS structure are presented for one particular loading scenario which is the premagnetization of the coils without plasma.

Abstract

The paper describes the conceptual design of the COMPASS Upgrade (COMPASS-U) tokamak which represents a compact, medium-size, high-magnetic-field and high-density device with a flexible set of poloidal field coils for generation of single, double null and snowflake configurations. In addition, COMPASS-U will be equipped with a closed divertor, which will be operated at high plasma and neutral density and with high optical opacity similar to future reactors. COMPASS-U is capable of addressing some of the key challenges in the field of the plasma exhaust physics, reactor-relevant edge plasma physics, advanced confinement regimes, advanced magnetic configurations and materials for next-step devices. The conceptual design of the individual COMPASS-U systems is presented with emphasis on magnets, support structure and power supply system enhancement.

Abstract

The reduction of the incident heat flux onto the divertor will be a necessity for the future thermonuclear reactors. Impurity seeding is recognized as an efficient way to achieve the partial detachment regime, which allows to dissipate a large fraction of power flux by radiation. This paper presents a heat flux real-time feedback system (RTFS) based on impurity seeding controlled by a combined ball-pen and Langmuir probe divertor array in the COMPASS tokamak. A number of features of the system have been studied, such as the type of impurity, seeding location, constants used in the real-time controller and the diagnostic selections. A detailed description of the designed RTFS and the results of the implementation are presented. The findings confirm the applicability of the RTFS for reduction and control of the divertor heat fluxes. Another important implication of this research is the ability of installing such systems in next-step devices.

Recently the Li-Beam Emission Spectroscopy (Li-BES) system on COMPASS has reached its full diagnostic power in terms of routine automatic operation in any kind of plasma scenarios and it is normally used as a standard tool for reconstruction of ultra fast density profiles in the edge region of COMPASS plasmas. The purpose of this study is to investigate the advantages and limitations of the COMPASS Li-BES system in characterizing plasma electron density fluctuations. We show how the atomic physics of plasma-beam interactions can affect the interpretation of the measurement at different radial positions and for different electron density profiles. We also demonstrate the usability of generalized sequential probability ratio test for automatic event detection. Using non-perturbative diagnostic, we verify the validity of the stochastic Garcia-model for scrape-off layer filaments and accompanying holes (density deficits)., suggestions from the coauthors included some text added to explain fig. 9

Abstract

Within the EUROfusion MST1 work package, a series of experiments has been conducted on AUG and TCV devices to disentangle the role of plasma fueling and plasma shape for the onset of small ELM regimes. On both devices, small ELM regimes with high confinement are achieved if and only if two conditions are fulfilled at the same time. Firstly, the plasma density at the separatrix must be large enough (), leading to a pressure profile flattening at the separatrix, which stabilizes type-I ELMs. Secondly, the magnetic configuration has to be close to a double null (DN), leading to a reduction of the magnetic shear in the extreme vicinity of the separatrix. Abstract

The atomic beam probe diagnostic concept aims at measuring the edge magnetic field and through that edge current distribution in fusion plasmas by observing trajectories of an ion beam stemming from a diagnostic neutral beam. The diagnostic potentially has microsecond scale time resolution and can thus prove to be a powerful option to study fast changes in the edge plasma. A test detector has been installed on the COMPASS tokamak as an extension of the existing lithium beam diagnostic system. It employs a relatively simple concept of an array of conductive detection plates measuring the incident ion current, which is then amplified and converted to a voltage signal. The aim of the test detector is to experimentally examine the idea of the diagnostic and provide background data for design and installation of a final detector. Also, a numerical code based on the CUDA parallel computing platform has been developed for modeling lithium ion trajectories in the given COMPASS plasma discharges. We present the developments of the detector design and test measurements of the diagnostic performed both in a laboratory beam system and on the COMPASS tokamak.

23. Expression of cancer stem cells markers in urinary bladder urothelial carcinoma and its precursor lesions.

Abstract

Background. Cancer stem cells (CSC) and their role in tumorigenesis of various solid tumors have been studied in past decades. Urothelial CSC were first identified 10 years ago and subsequent studies have been performed with the aim to identify reliable markers of CSC. So far, a few studies have investigated a relationship between CSC markers expression in urothelial carcinoma tissue and histopathological characteristics of the tumor. Methods. In our study, we evaluated an immunoexpression of the CSC markers CD24, CD44, CD66 and CD133 in tissue sections of urothelial carcinoma (all tumor grades and stages were included), urothelial carcinoma in situ and non-neoplastic urothelium, totally 218 specimens were enrolled. Results. All studied molecules were expressed either in tumor tissue and non-neoplastic urothelium. Urothelial carcinomas of higher tumor grade and stage expressed molecules CD24 and CD133 significantly more frequently whereas molecules CD44 and CD66 did not show significant association with tumor histopathological features. Conclusions. Our results showed that studied molecules are not suitable for direct detection of CSC in urothelial carcinoma tissue sections, but an expression of molecules CD24 and CD133 is significantly related to urothelial carcinoma grade and stage, which are both important prognostic indicators and therefore an expression of these markers might have a potential prognostic value. Abstract

An improved lithium beam based beam emission spectroscopy system – installed on COMPASS tokamak – is described. The beam energy enhanced up to 120 keV for Atomic Beam Probe measurement. The size of the ion source is doubled, using a newly developed thermionic heater instead of the conventionally used heating (tungsten or molybdenum) filament. The neutralizer is also improved. It produces the same sodium vapor in a cell but minimize the loss condensing the vapor on a cold surface which is led back (in fluid state) into the sodium oven. This way we call it recirculating neutralizer. The observation system consists of a CCD camera and an avalanche photodiode array.

Abstract

The resonant magnetic perturbation (RMP) has proven to be a useful way to suppress edge-localized modes that under certain conditions can damage the device by the large power fluxes carried from the bulk plasma to the wall. The effect of RMP on the L-mode plasma parameters in the divertor region of the COMPASS tokamak was studied using the array of 39 Langmuir probes embedded into the divertor target. The current-voltage (IV) probe characteristics were processed by the first-derivative probe technique to obtain the plasma potential and the electron energy distribution function (EEDF) which was approximated by a bi-Maxwellian EEDF with a low-energy (4-6 eV) fraction and a high-energy (11-35 eV) one, the both factions having similar electron density. Clear splitting was observed during the RMP pulse in the low-field-side scrape-off-layer profiles of the floating potential U fl and the ion saturation current density J sat; these two quantities were obtained both by direct continuous measurement and by evaluation of the IV characteristics of probes with swept bias. The negative peaks of U fl induced by RMP spatially overlaps with the local minima of J sat (and n e) rather than with its local maxima which is partly caused by the spatial variation of the plasma potential and partly by the changed shape of the EEDF. The effective temperature of the whole EEDF is not correlated with the negative peaks of U fl, and the profile of the parallel power flux density shows secondary maxima due to RMP which mimic those of J sat.

The wall panels are toroidally shaped in order to spread the incident parallel power flux q|| uniformly, assuming a single decay length λq whose value is not known from first principles. In order to study the scaling of q|| with plasma parameters, infra-red viewing of specially-designed limiters has been used on the COMPASS tokamak in ∼100 discharges with scans in Ip, ne and for all combinations of magnetic field and Ip directions. The IR measurement clearly shows that in addition to the main SOL heat flux profile with λq > 40 mm, a steep gradient (λqnear = 4 ± 2 mm) dominates q|| near separatrix. This appears independently of limiter shaping, insertion with respect to neighbors and incident field-line angles. Good agreement is found between the measured λqnear and the prediction of a heuristic drift-based model.

Abstract

This paper describes a new filamentary probe recently introduced on the COMPASS tokamak. It allows the measurement of electrostatic and magnetic properties of the filaments and their changes in dependence on distance from the separatrix in the region between a divertor and midplane. The probe head is mounted on a manipulator moving the probe radially on a shot-to-shot basis. This configuration is suitable for the long term statistical measurement of the plasma filaments and the measurement of their evolution during their propagation from the separatrix to the wall. The basics of the filamentary probe construction, the evolution of the plasma parameters, and first conditional averages of the plasma filaments in the scrape-off layer of the COMPASS tokamak during the L-mode regime are presented.

Abstract

We report the latest results on fast measurements of the electron temperature and parallel heat flux in the COMPASS tokamak scrape-off layer (SOL) and divertor region during ELMy H-mode plasmas. The system of ball-pen and Langmuir probes installed on the divertor target, the horizontal reciprocating manipulator and the fast data-acquisition system with sampling frequency rate f = 5 MSa s−1 allow us to measure the electron temperature and parallel heat flux during inter-ELM and ELM periods with high temporal resolution. The filamentary structure of the electron temperature and parallel heat flux was observed during ELMs in the SOL as well as in the divertor region. The position of the filaments within ELMs is not regular and therefore the resulting conditionally averaged ELM neglects the peak values of the electron temperature and parallel heat flux. We have found a substantial difference between the value of the radial power decay length in the inter-ELM period λ q,inter = 2.5 mm and the decay length of the peak ELM heat flux λ q,ELM = 13.1 mm. The decay length of the ELM energy density was found to be λ E,ELM = 5.4 mm.

Abstract

The concept and development of a new detection method for light alkali ions stemming from diagnostic beams installed on medium size tokamak is described. The method allows us the simultaneous measurement of plasma density fluctuations and fast variations in poloidal magnetic field, therefore one can infer the fast changes in edge plasma current. The concept has been worked out and the whole design process has been done at Wigner RCP. The test detector with appropriate mechanics and electronics is already installed on COMPASS tokamak. General ion trajectory calculation code (ABPIons) has also been developed. Detailed calculations show the possibility of reconstruction of edge plasma current density profile changes with high temporal resolution, and the possibility of density profile reconstruction with better spatial resolution compared to standard Li-BES measurement, this is important for pedestal studies.

Abstract

This article describes a fast and automatic reconstruction of the edge plasma electron density from the radiation of energetic Li atoms of the diagnostic beam on the COMPASS tokamak. Radiation is detected by using a CCD camera and by using an avalanche photo-diode system with a temporal resolution of 20 ms and 2 μs, respectively. Both systems are equipped with a 670.8 nm optical filter which corresponds to the lithium 1s22s1-1s22p1 transition. A theoretical model and a data processing procedure of a raw signal to obtain the density profile are described. The reconstruction algorithm provides the absolutely calibrated electron density profiles together with the measurement error estimated from relatively calibrated light profiles; the implementation is performed in Python. Time demanding operations of the code were optimized to provide reconstruction of a single profile within less than 10 ms which makes the code applicable for processing of a large amount of data. Thanks to this calculation speed, it is possible to reconstruct electron density profiles between two consecutive shots on the COMPASS tokamak with 2 μs time resolution.

The COMPASS tokamak, a divertor device with clear H-mode and ITER-relevant geometry, has been re-installed in IPP Prague after its transport from CCFE in UK. The first plasma was achieved in December 2008. Many new diagnostic tools with both high temporal and spatial resolutions have been developed to address the scientific programme of COMPASS focused on H-mode physics and pedestal investigations. In the paper, an overview of existing and in near future planned diagnostics (magnetic, spectroscopy, microwave, probe and beam/particle) on COMPASS is given including their basic technical specifications and achievements.

Abstract

This paper reports experimental data obtained on the COMPASS tokamak by an array of Langmuir probes embedded in the divertor tiles. The measured current-voltage probe characteristics were processed by the recently published first-derivative probe technique for precise determination of the plasma potential and the electron energy distribution function (EEDF). The measurements were performed during L-mode hydrogen and deuterium plasma with a toroidal magnetic field Bt = 1.15 T, plasma current Ip = 180 kA and average electron density ne = 8*1019 m-3. The spatial profile of the electron temperatures shows that in the vicinity of the inner and outer strike points in hydrogen plasma the EEDF can be approximated by a bi-Maxwellian distribution, with a dominating low-energy electron population (4 - 7 eV) and a minority of electrons with higher energies (12 - 18 eV). In the private flux region between the two strike points, the EEDF is found to be Maxwellian with temperatures in the range of 7 - 9 eV. In the case of deuterium plasma under similar discharge conditions, the EEDFs in the vicinity of the inner and outer strike points, as well as in the private flux region between the two strike points, are found to be bi-Maxwellian.

35. Significant impact of terminal room cleaning with bleach on reducing nosocomial Clostridium difficile.

Abstract

Background: We were alerted to increased rates of Clostridium difficile-positive tests at all 3 hospitals in our health care system by MedMined Data Mining Surveillance Service, CareFusion (San Diego, CA). In response, an intervention of terminal room cleaning with dilute bleach was instituted to decrease the amount of C difficile environmental spore contamination from patients with C difficile infection (CDI). Methods: The intervention consisted of replacing quaternary ammonium compound as a room cleaning agent with dilute bleach to disinfect rooms of patients with CDI upon discharge. All surfaces, floor to ceiling were wiped with dilute bleach applied with towels to thoroughly wet the surfaces. Daily room cleaning remained unchanged. Patients remained on C difficile contact isolation precautions until discharge. To determine the effectiveness of this program, rates of nosocomial CDI for all 3 hospitals were determined using the MedMined Virtual Surveillance Interface for 10 months prior to and 2 years after the cleaning intervention. Statistical significance was determined using Poisson regression analysis. Results: There was a 48% reduction in the prevalence density of C difficile after the bleaching intervention (95% confidence interval: 36%-58%, P < .0001). Conclusion: The implementation of a thorough, all-surface terminal bleach cleaning program in the rooms of patients with CDI has made a sustained, significant impact on reducing the rate of nosocomial CDI in our health care system. 36. Management of a multidrug-resistant Acinetobacter baumannii outbreak in an intensive care unit using novel environmental disinfection: A 38-month report.

Background: Between June 1, 2004, and March 14, 2005, 16 patients in the surgical/medical intensive care unit (ICU) were infected and another 2 were colonized with multidrug-resistant (MDR) Acinetobacter baumannii. We describe the systematic investigation initiated to discover an environmental reservoir and a novel measure taken to terminate the outbreak. Methods: Cultures were taken from moist areas in the ICU, including sink traps, sink and counter surfaces, drains, and faucets. Strains were characterized using restriction endonuclease analysis. A weekly full drainpipe chase cleansing protocol with sodium hypochlorite (bleach) solution for all 24 ICU and waiting room area sinks connected by common plumbing was initiated in March 2005. Results: Eleven of 16 infected patients (69%) had a clonal MDR strain, 1 patient (6%) was infected with an unrelated strain, and in 4 patients (25%) strains were not available for typing. The reservoir for the A baumannii clone was detected in a sink trap within one of the ICU patient rooms that likely represented contamination of the entire horizontal drainage system. The bleaching protocol initiated in March 2005 successfully decontaminated the reservoir and eliminated the MDR A baumannii infections. Conclusion: A systematic search for an environmental reservoir followed by decontamination significantly reduced (P < .01) the incidence of MDR A baumannii infection. [Copyright &y& Elsevier]

37. Electronic Surveillance for Infectious Disease Trend Analysis following a Quality Improvement Intervention

Objective.Interventions for reducing methicillin-resistant Staphylococcus aureus(MRSA) healthcare-associated disease require outcome assessment; this is typically done by manual chart review to determine infection, which can be labor intensive. The purpose of this study was to validate electronic tools for MRSA healthcare-associated infection (HAI) trending that can replace manual medical record review.Design and Setting.This was an observational study comparing manual medical record review with 3 electronic methods: raw culture data from the laboratory information system (LIS) in use by our healthcare organization, LIS data combined with admission-discharge-transfer (ADT) data to determine which cultures were healthcare associated (LIS + ADT), and the CareFusion MedMined Nosocomial Infection Marker (NIM). Each method was used for the same 7-year period from August 2003 through July 2010.Patients.The data set was from a 3-hospital organization covering 342,492 admissions.Results.Correlation coefficients for raw LIS, LIS + ADT, and NIM were 0.976, 0.957, and 0.953, respectively, when assessed on an annual basis. Quarterly performance for disease trending was also good, with R2values exceeding 0.7 for all methods.Conclusions.The electronic tools accurately identified trends in MRSA HAI incidence density when all infections were combined as quarterly or annual data; the performance is excellent when annual assessment is done. These electronic surveillance systems can significantly reduce (93% [in-house-developed program] to more than 99.9999% [commercially available systems]) the personnel resources needed to monitor the impact of a disease control program.

A question not previously addressed is whether reduction of the incidence of MRSA disease would lower the total number of S. aureusclinical isolates recovered by the microbiology laboratory that are reported to health care providers. The decreases in the numbers of MRSA and total S. aureusclinical isolates for each year after August 2005 were highly statistically significant compared to the numbers in each of the prior 3 years (P< 0.0001).

Abstract

ABSTRACTWe evaluated the use of the BD GeneOhm MRSA real-time PCR assay (BD Diagnostics, San Diego, CA) for the detection of nasal colonization with methicillin-resistant Staphylococcus aureus(MRSA). The initial evaluation consisted of 403 paired nasal swabs and was done using the specimen preparation provided with the kit and an in-house lysis method that was specifically developed to accommodate large-volume testing using a minimal amount of personnel time. One swab was placed in an achromopeptidase (ACP) lysis solution, and the other was first used for culture and then prepared according to the kit protocol. PCR was performed on both lysates, and results were compared to those for culture. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the PCR assay were 98%, 96%, 77%, and 99.7% with the kit lysate and 98%, 95%, 75%, and 99.7% with the ACP lysate (P, not significant), respectively. The second evaluation was done after implementation of all-admission surveillance using PCR with ACP lysis and a sampling of 1,107 PCR-negative samples and 215 PCR-positive samples that were confirmed by culture. The results of this sampling showed an NPV of 99.9% and a PPV of 73.5% (prevalence, 6%), consistent with our initial findings. The BD GeneOhm MRSA assay is an accurate and rapid way to detect MRSA nasal colonization. When one is dealing with large specimen numbers, the ACP lysis method offers easier processing without negatively affecting the sensitivity or specificity of the PCR assay.

41. A laboratory-based, hospital-wide, electronic marker for nosocomial infection: the future of infection control surveillance?

Faced with expectations to improve patient safety and contain costs, the US health care system is under increasing pressure to comprehensively and objectively account for nosocomial infections. Widely accepted nosocomial infection surveillance methods, however, are limited in scope, not sensitive, and applied inconsistently. In 907 inpatient admissions to Evanston Northwestern Healthcare hospitals (Evanston, IL), nosocomial infection identification by the Nosocomial Infection Marker (MedMined, Birmingham, AL), an electronic, laboratory-based marker, was compared with hospital-wide nosocomial infection detection by medical records review and established nosocomial infection detection methods. The sensitivity and specificity of marker analysis were 0.86 (95% confidence interval [CI 95], 0.76-0.96) and 0.984 (CI 95, 0.976, 0.992). Marker analysis also identified 11 intensive care unit-associated nosocomial infections (sensitivity, 1.0; specificity, 0.986). Nosocomial Infection Marker analysis had a comparable sensitivity (P > .3) to and lower specificity (P < .001) than medical records review. It is important to note that marker analysis statistically outperformed widely accepted surveillance methods, including hospital-wide detection by Study on the Efficacy of Nosocomial Infection Control chart review and intensive care unit detection by National Nosocomial Infections Surveillance techniques.

Both civil servants and politicians serve the same democratic state, and both are hews to the same democratic evolution. S argue that based mainly on the historically developed division of labour between bureaucracy and politics civil servants have never been tasked with creating the conditions for more democracy m the state, but instead with creating the conditions for a more effective and successful slate. Given that political bodies in which politicians operate have been established as the institutionalised personification of democracy, the task of politicians is above alt to promote democracy, its values and norms.

Medical record review revealed that all 5 had been hospitalized within the prior 2 years. Three of 5 had a history of renal impairment. The strategy that would involve screening the greatest number of patients (all those with a history of hospital admission in the prior 2 years) resulted in highest screening cost per patient admitted ($2.48), lower per patient admission costs ($480), and the best survival rates.Conclusion:An expanded VRE surveillance program that encompassed all patients hospitalized within the prior 2 years was a cost-effective screening strategy compared with a more traditional one focused on high-risk units.

Abstract

Nasal carriage of Staphylococcus aureusis considered a source of subsequent infection in health care settings. Utilization of real-time polymerase chain reaction (PCR) for detection of S. aureushas the potential to dramatically affect infection control practice by rapidly identifying S. aureus-colonized patients. We developed and validated the use of real-time PCR for detection of S. aureuscolonization in two patient populations. Paired nasal swabs were collected from 299 neonates and from 151 adult patients at Evanston Hospital. One swab was used for culture and the other placed into a bacterial lysis solution containing achromopeptidase. The DNA liberated was used as the template for real-time PCR with primers for the femAgene. SYBR Green was used for amplicon detection. In the neonatal population the sensitivity, specificity, predictive value positive and predictive value negative for culture and PCR was 92%versus96%, 100%versus100%, 100%versus100%, and 98%versus99%, respectively. In the adults the results were 90%versus100%, 100%versus98%, 100%versus96%, and 95%versus100%, respectively. Real-time PCR was able to detect S. aureusin 2 hours compared to 1 to 4 days for culture and provided sensitivity equal to or greater than culture.

45. Computer-Assisted Surveillance for Detecting Clonal Outbreaks of Nosocomial Infection

Abstract

ABSTRACTWhole-house surveillance for healthcare-associated infection is no longer the recommended practice because of the large personnel time investment required. We developed a computer-based tracking system using microbiologic data as an aid in detecting potential outbreaks of healthcare-associated infections on a hospital-wide basis. Monthly total isolates of 25 clinically significant hospital pathogens were tallied from 1991 to 1998 to form a database for future comparison. Two different algorithm tools (based on increases of organism numbers over baseline) were applied to determine alert thresholds for suspected outbreaks using this information. The first algorithm (2SD) defined an alert as two standard deviations above the mean monthly number of isolates. The second (MI) defined an alert as either a 100% increase from the baseline organism number over 2 months or a =50% increase (compared to baseline) during a three-consecutive-month period. These two methods were compared to standard infection control professional surveillance (ICP) for the detection of clonal outbreaks over 12 months. Overall, a total of seven clonal outbreaks were detected during the 1-year study. Using standard methods, ICP investigated nine suspected outbreaks, four of which were associated with clonal microbes. The 2SD method signaled a suspected outbreak 15 times, of which three were clonal and ICP had detected one. The MI method signaled a suspected outbreak 30 times; four of these were clonal, and ICP had detected one. The sensitivity and specificity values for ICP, 2SD, and MI for detecting clonal outbreaks were 57, 43, and 57% and 17, 83, and 67%, respectively. Statistical methods applied to clinical microbiology laboratory information system data efficiently supplement infection control efforts for outbreak detection.

46. An Outbreak of Bloodstream Infections in an Outpatient Hemodialysis Center

Abstract

Objectives:Investigate and control an increase in bloodstream infections (BSIs) in an outpatient hemodialysis center.Patients and Design:A retrospective cohort study was conducted for patients receiving dialysis at the center from February 2000 to April 2001. A case–control study compared microbiological data for all BSIs that occurred during the study period with those for BSIs that occurred during a baseline period Qanuary 1999 to January 2000). BSI rates before and after a 1-month intervention (May 2001) were assessed. A case was defined as a new BSI during the study period.Results:The outbreak was polymicrobial, with approximately 30 species. The baseline BSI rate was 0.7 per 100 patient-months. From February 2000 to April 2001, the BSI rate increased to 4.2 per 100 patient-months. Overall, 75% of the BSIs were associated with central venous catheters (CVCs), but CVC use did not fully explain the increase in BSIs. In January 2000, when the center changed ownership, prepackaged CVC dressing kits and biweekly infection control monitoring were discontinued. Beginning in May 2001, staff were educated on CVC care, chlorhexidine replaced povidone-iodine for cutaneous antisepsis, gauze replaced transparent dressings, antimicrobial ointments containing polyethylene glycol at CVC exit sites were discontinued, and patients with CVCs were educated on cutaneous hygiene. After the intervention period, by October 2001, rates decreased to less than 1 BSI per 100 patient-months.Conclusions:Proper cutaneous antisepsis and access site care is crucial in preventing BSIs in patients receiving hemodialysis. Infection control programs, staff and patient education, and use of optimal antisepsis agents or prepackaged kits are useful toward this end.

Abstract

The transition reported in L-mode plasmas leading to the formation of a density “shoulder” in the Scrape-off Layer (SOL) of tokamaks is investigated in the three ITER stepladder machines: COMPASS, ASDEX Upgrade (AUG) and JET. In AUG and JET, the broadening of the profiles can be observed at similar relative densities (ne/nGW ∼ 0.4–0.5) and coincides with a clear increase in the size of SOL filaments and the onset of the outer divertor detachment. In COMPASS, no transition is observed in the same density range: the divertor does not detach and no clear effect is observed neither in the profiles nor in the turbulence. This is explained as the result of an increase of intermittent transport triggered by the increased resistivity along filaments associated to divertor detachment. The transition conditions in AUG and JET are compared to foreseen ITER SOL parameters, and the implications are briefly discussed.

48. Highly Reproducible Bactericidal Activity Test Results by Using a Modified National Committee for Clinical Laboratory Standards Broth Macrodilution Technique

Abstract

ABSTRACTBactericidal testing historically has exhibited variable reproducibility, even when prior standardized methods were employed. Several modifications to the National Committee for Clinical Laboratory Standards (NCCLS) broth macrodilution method are proposed to improve reproducibility. Recommended changes from the approved NCCLS guidelines (M21-A and M26-A) include omitting serum supplementation of Mueller-Hinton broth, incubating tubes at 35°C for 24 h with no agitation until they are sampled, running all tests in duplicate with six dilutions instead of nine, reincubating the test for an additional 24 h to resolve discrepant bactericidal activity test results, using a single 0.1-ml sample from each clear tube for subculture, and adopting an alternate method for calculating endpoint determination. In order to test these recommendations in a clinical laboratory setting, we used the modified methodology on 224 separate tests for bactericidal activity. There were 102 serum bactericidal titer (SBT) and 122 minimum bactericidal concentration (MBC) assays performed. By defining reproducibility as agreement between duplicate tests ± 1 dilution, we found 207 of 224 tests (92%) were reproducible at the 24-h subculture point (94% for the SBT assay and 91% for the MBC assay). When the 17 assays with discrepant results were incubated an additional 24 h for a second subculture, only 1 of 224 tests (0.4%) remained discrepant. The method used is practical for a clinical laboratory that chooses to perform bactericidal activity testing and assures a high level of reproducibility between duplicate assays. The total cost of a test was approximately $25.00.

49. In Vitro Activities of Linezolid against Important Gram-Positive Bacterial Pathogens Including Vancomycin-Resistant Enterococci

Abstract

ABSTRACTThe emergence of resistance in gram-positive bacteria has necessitated a search for new antimicrobial agents. Linezolid is an oxazolidinone, a new class of antibacterial agents with enhanced activity against pathogens. We compared the activity of linezolid to those of other antimicrobial agents against 3,945 clinical isolates. Linezolid demonstrated potent activity against all isolates tested. For all vancomycin-susceptible enterococci, staphylococci, and streptococci, the activity of linezolid was comparable to that of vancomycin. Against oxacillin-resistant staphylococci and vancomycin-resistant enterococci, linezolid was the most active agent tested. In summary, linezolid appears to be a promising new antimicrobial agent for the treatment of gram-positive infections.
