The Ocular Surface Disease Index (OSDI) questionnaire, non-invasive breakup time (NIBUT), tear film osmolarity and meniscus height were measured 6 months before intervention, at baseline, post-intervention (3 months), 9 and 15 months.Results: NIBUT, osmolarity and meniscus height significantly worsened during the 6 months before treatment, whereas symptoms did not change. OSDI scores significantly improved at post-intervention (MD = − 44.0, 95% CI − 38.1, − 50.0), and then increased again until the at last follow-up, but still significantly different from baseline (MD = − 30.0, 95% CI − 23.4, − 36.8). The three clinical signs showed a similar pattern, with one-year improvements of 3.6 seconds for the NIBUT (95% CI 3.1, 4.2, p < 0.001), 28 mOsm/L for osmolarity (95% CI 23.6, 32.4, p < 0.001) and 0.03 mm for meniscus height (95% CI 0.02, 0.04, p < 0.001). No adverse effects were observed.Conclusion: IPL/LLLT is safe and produces an important reduction in symptoms and signs of dry eye disease, still relevant one year after the end of treatment in a sample with high symptoms’ severity. Therefore, it represents a promising treatment option for patients who do not improve with conventional treatment. 4. Custom contact lenses and cornea-scleral contact lenses from irregular astigmatism

Abstract

Objective: To show the improvement of visual acuity in five clinical cases of irregular astigmatism, achieved with contact lenses of different materials and geometries. Case reports: Five case reports are exposed and their adaptations for the use of this type of lenses are evaluated. After inserting corneo-scleral lenses or soft contact lenses, each adaptation is evaluated. Results: All patients have improved meaningfully their visual acuity without affecting their corneal integrity and improving their quality of life. Conclusion: Advances in the design and materials of contact lenses have helped eye-care professionals to solve visual problems that would have been difficult to solve years ago. 7. Comparación de la efectividad de dos estrategias farmacológicas para la prevención del síndrome de iris flácido intraquirúrgico

Abstract

Objetivo: Comparar la efectividad de dos estrategias diseñadas para prevenir la aparición del síndrome de iris flácido intraquirúrgico (IFIS) en pacientes expuestos a alfa-bloqueantes. Métodos: Comparamos retrospectivamente la presencia y severidad de aparición del síndrome en 29 pacientes intervenidos de cataratas, estableciendo dos grupos de 12 pacientes (22 ojos) y 17 pacientes (33 ojos). La estrategia aplicada al grupo uno consistió en suspender le exposición a los alfa-bloqueantes un mínimo de una semana antes de la intervención, atropina tópica 1% 3 veces al día desde dos días antes, y adrenalina intracamerular. La estrategia del grupo dos fue la suspensión del fármaco un mínimo de una semana antes y la aplicación intracamerular al inicio de la cirugía de una solución formada por suero fisiológico, adrenalina y lidocaína. Resultados: En el grupo uno 6 pacientes no tuvieron IFIS, 2 padecieron IFIS leve bilateral, 2 moderado bilateral y 2 severo (unilaterales); es decir, un 45,5% de ojos lo padecieron. En el grupo dos, 3 ojos (9,1%) de dos pacientes lo padecieron en grado leve. Existen diferencias estadísticamente significativas en los dos grupos (p=0,002). Si consideramos que un paciente sufre el síndrome sólo cuando tiene un grado moderado o grave, también existen diferencias significativas entre la proporción de casos con IFIS en el grupo uno con respecto al grupo dos (p=0,0002). Conclusiones: El estudio demuestra la mayor efectividad estadísticamente significativa de la estrategia aplicada en el grupo dos comparada a la aplicada al grupo uno. espanolObjetivo: Mostrar un nuevo metodo para el calculo de la lente intraocular (LIO) en pacientes intervenidos de cirugia refractiva corneal en los que la aplicacion de otros metodos no es posible por la falta de datos. Tambien es aplicable en otras situaciones de alta dificultad en el calculo de la LIO y en casos de intercambio de LIO por sorpresa refractiva. Metodos: Se realiza la facoemulsificacion y antes de implantar la LIO se refracciona al paciente, utilizando el dato de la refraccion en afaquia como unica variable para la aplicacion de nuestra formula, que se basa en la correspondencia existente entre la graduacion en gafas (12 mm) y la potencia de la LIO a nivel de plano cristaliniano. Se ha aplicado en tres pacientes en los que ningun metodo de calculo era aplicable. Resultados: Los tres pacientes han quedado con un equivalente esferico inferior a 1 dioptria. Conclusiones: Creemos que esta formula es valida para algunas situaciones en que otras formulas no pueden ser aplicadas por falta de variables. 17. Pupil Ovalization and Accommodation Loss after High-intensity Focused Ultrasound Treatment for Glaucoma: A Case Report.

Abstract

Aim: The aim of this study is to present a complication derived from high-intensity focused ultrasound (HIFU) not described previously in the literature., Background: HIFU has shown to be an effective technique for reducing intraocular pressure (IOP) in the treatment of glaucoma, enabling the selective coagulation of the ciliary body. Published trials have not observed major intra- or postoperative complications, or serious adverse events derived from this technique., Case Description: We present the case of a 65-year-old phakic patient with open-angle glaucoma and no previous filtration surgery. He was treated in both eyes with HIFU. After the intervention, the patient presented mild uveitis for 10 days, which remitted with the application of topical cycloplegic and corticosteroid treatment. After that, he reported a loss of near vision and examination showed pupil ovalization. It persisted 6 months later, with an accommodation loss of one diopter., Conclusion: Phakic patients treated with HIFU might be at risk of pupil ovalization with accommodation loss., Clinical Significance: The complications described in this article should be included in the informed consent for patients with glaucoma who are offered HIFU as a treatment option., How to Cite This Article: Rivero-Santana A, Pérez-Silguero D, et al. Purpose: To report a new method for intraocular lens (IOL) calculation in patients treated by corneal refractive surgery, in whom the application of other methods is not possible because of the absence of necessary data. This method is also applicable in other situations where the IOL calculation is difficult and in cases of IOL exchange because of unexpected refractive difficulties., Methods: We performed phaco-emulsifications, refracting the patient's eye before IOL implantation, using the refraction data in aphakia as the only variable for the application of the formula, which itself is based on the existing correlation between the graduation of glasses (12 mm) and the IOL power at the lens level. The formula has been applied in 3 patients in whom no other calculation method was applicable., Results: After the appropriate intervention, the 3 patients presented a spherical equivalent below 1 diopter., Conclusion: We believe this formula is valid in some situations where other formulas cannot be applied because of the absence of the necessary variables.

22. [Repair of Descemet's membrane detachment after cataract surgery].

Abstract

Purpose/methods: We present two cases of Descemet's membrane detachment, the first one after extracapsular cataract extraction with scleral tunnel incision and the other after clear corneal incision used for phacoemulsification cataract extraction. These two cases were treated with intracameral injection of 20% sulfur hexafluoride gas (SF(6))., Results/conclusions: Both cases responded satisfactorily to treatment. After four months of follow-up, the corneas remain clear with a visual acuity of 0.9 and 0.8, respectively. The relative facility of the 20% sulfur hexafluoride gas (SF(6)) technique, its safety, and good prognosis, makes it the treatment of choice in this disease.

Abstract

Purpose/methods: We report a case of orbital hemangiopericytoma, in a 14 year-old boy. Neuroradiological findings, including those of computerized tomography (CT), magnetic resonance imaging (MRI), and ultrasonography are detailed., Results/conclusions: Diagnostic imaging showed a well-circumscribed and solid mass with an intraconal location. The tumor was grossly purple, richly vascularized and encapsulated. The histological study revealed a hemangiopericytoma of borderline grade with hypercellularity and moderate number of mitotic figures. We emphasize the rarity of the orbital hemangiopericytoma specially when it is located within the muscle cone.
