Objective This study aimed to analyze our outcomes of primary premaxillary ostectomy and setback combined with lip adhesion as the first stage in repair of the bilateral cleft lip with fly-away premaxilla. Methods This retrospective study included all patients who required primary premaxillary setback by a single surgeon to achieve successful lip repair between 2011 and 2017 at a tertiary children's hospital. All patients had bilateral cleft lip and complete cleft palate with significant premaxillary protrusion. All patients either had failed presurgical orthodontic manipulation or were thought to be poor candidates because of the rigidity of the premaxilla on clinical examination. Patient data were obtained from review of medical records and photographs. Surgical technique is described. Results Eleven patients with bilateral cleft lip, all of whom also had complete cleft palate, underwent primary premaxillary setback and lip adhesion at an average age of 8.2 months. The mean follow-up was 1.9 years (range, 139 days to 4.5 years). All patients subsequently completed formal lip repair, of which 10 have undergone palatoplasty. Serial photography documented stable premaxillary position and successful lip repair within the follow-up time frame. No patient had postoperative premaxillary instability or vascular compromise. One patient had dehiscence of the lip adhesion requiring reoperation. Conclusions Primary premaxillary ostectomy and setback allows for successful staged formal lip repair in patients with bilateral cleft lip and palate who have failed presurgical infant orthopedics. Further research is required regarding long-term follow-up with respect to midfacial growth.

This study describes a corrective procedure for a metatarsal angular malformation using wedge ostectomy in a 41-day-old Thoroughbred filly. A 41.6 °angular malformation of the metatarsus was corrected with wedge ostectomy of the left hindlimb in association with a type II external fixator and methyl methacrylate resin. The animal was subjected to the surgical procedure under general anesthesia. The filly was placed in dorsal recumbency with the left hind limb free for manipulation. A bone saw was used for the wedge ostectomy, while the bars for external fixation were placed using a drill. A Kirschner pin and acrylic resin were then applied. Radiographic images obtained immediately after surgery confirmed a total malformation reduction of 32.6 (from 41.6° to 9°). With medication and a Robert Jones bandage, intensive postoperative care was instituted to ensure that the animal adapted properly to the linear external fixator without overloading the pins and connection bars. No postoperative complications were observed. On the day after surgery, the filly was standing and walking with no signs of pain. After 180 days, total osseous calcification was achieved, and the implants were removed during the second surgery. The filly demonstrated adequate locomotion, performed all the movements, and ran without any difficulty in the paddock. The remaining discrete angular deviations were partially corrected using hoof trimming. Resumo O objetivo desse estudo é descrever o procedimento de correção da malformação de desvio angular metatársico, utilizando ostectomia em cunha, em uma potra puro-sangue de 41 dias de idade. Uma malformação metatársica angular de 41.6º foi corrigida com a técnica de ostectomia em cunha, em associação com fixador externo tipo II e resina de metilmetacrilato, no membro pélvico esquerdo. O animal foi submetido à cirurgia sob protocolo de anestesia geral. A potra foi posicionada em decúbito dorsal, com o membro pélvico esquerdo livre para manipulação. Para a realização da ostectomia em cunha, foi utilizada uma serra óssea. Para a instalação das barras do fixador externo, uma furadeira foi utilizada e, para fixá-los, pinos de Kirschner e resina acrílica foram aplicados. Logo após a cirurgia, imagens radiográficas foram obtidas para confirmar a redução total de 32,6º (de 41,6º para 9º) de angulação. Para garantir que o animal se adaptasse ao fixador externo linear e não sobrecarregasse os pinos e barras, foram instituídos cuidados pós-operatórios intensos, com medicações e bandagem de Robert Jones. Não houve complicações no pós-operatório. No dia seguinte à cirurgia, a potra estava em pé, caminhando e sem sinais de dor. Após 180 dias, alcançou total calcificação óssea e os implantes puderam ser removidos em um segundo tempo cirúrgico. A potra demonstrou locomoção adequada, realizando todos os movimentos normais e pôde correr pelo pasto sem dificuldades. Um discreto desvio angular remanescente foi parcialmente corrigido por casqueamento.

The present study aimed at assessing the epidemiology including demographic variables, diagnostic features, and management of ameloblastomas at several European departments of maxillofacial and oral surgery. The following data were recorded for each patient: gender, age, voluptuary habits, comorbidities, site, size, radiographic features, type, histopathological features, kind of treatment, length of hospital stay, complications, recurrence, management and complications of the recurrence. A total of 244 patients, 134 males and 110 females with ameloblastomas were included in the study. Mean age was 47.4 years. In all, 81% of lesions were found in the mandible, whereas 19% were found in the maxilla. Mean size of included ameloblastomas was 38.9 mm. The most frequently performed treatment option was enucleation plus curettage/peripheral ostectomy in 94 ameloblastomas, followed by segmental resection (60 patients), simple enucleation (46 patients), and marginal resection (40 patients). A recurrence (with a mean follow up of 5 years) was observed in 47 cases out of 244 ameloblastomas (19.3%). Segmental resection was associated with a low risk of recurrence (p = 0003), whereas enucleation plus curettage/peripheral ostectomy was associated with a high risk of recurrence (p = 0002). A multilocular radiographic appearance was associated with a high risk of recurrence (p < .05), as well as the benign solid/multicystic histologic type (p < .05). Within the limitations of the study it seems that the management of ameloblastomas will probably remain controversial even in the future. Balancing low surgical morbidity with a low recurrence rate is a difficult aim to reach.

Background Leukocyte- and platelet-rich fibrin (L-PRF) is an autologous matrix scaffold which regulates inflammation by stimulating cytokines and growth factors that are involved in the immune response. L-PRF is suggested as a viable adjunctive method to surgical interventions due to its advantages on tissue healing. This study aims to retrospectively evaluate the adjunctive role of L-PRF in surgically treated medication-related osteonecrosis of the jaws (MRONJ) patients. Methods Between January 2012 and December 2020, patients with AAOMS stage II and III MRONJ lesions, who were treated surgically with adjunctive use of L-PRF in the authors’ institution were enrolled. Surgical interventions consisted of either marginal resection or sequestrectomy with peripheral ostectomy (SPO) or curettage and L-PRF application. Medical records of these patients were retrospectively reviewed and healing was assessed according to certain parameters including mucosal closure and presence of infection, exposed bone, fistula or radiologic markers of disease progression for a minimum of 12 months. Results Thirteen patients (7 women and 6 men) with an average age of 72.4 years (± 10.61, range 54–84) were included in the study, nine of whom had AAOMS stage III and four stage II MRONJ. Three patients had a marginal resection, nine patients had sequestrectomy with peripheral ostectomy (SPO) and one patient underwent a curettage procedure. All marginal resection and six SPO patients showed complete healing while four patients, who had SPO or curettage experienced incomplete healing. Mean follow up was 20.1 ± 18.29 months. Conclusion The use of L-PRF may be a favorable adjunctive option in the treatment of MRONJ owing to its favorable effects on tissue repair, ease of application, minimally invasive and cost-effective character and autogenous nature. CASE DESCRIPTION A 13-year-old 128-kg miniature donkey gelding was evaluated for right forelimb lameness of 7 weeks’ duration. CLINICAL FINDINGS Muscular atrophy of the infraspinatus and supraspinatus muscles over the right scapula with a palpable bony prominence over the point of the shoulder was evident. At the walk, the cranial phase of the stride was reduced with adduction of the distal aspect of the limb, dragging of the toe, and lameness (grade, 4/5). Lateral and craniocaudal radiographs of the right shoulder joint revealed lateral luxation of the humerus in relation to the scapula with bony proliferation and remodeling of the humeral head. TREATMENT AND OUTCOME Glenoid ostectomy was performed. Immediately after surgery, the donkey was able to intermittently stand squarely on the limb but maintained a reduced cranial phase of the stride at the walk. The donkey had no short-term complications and was discharged from the hospital 11 days after surgery. Following discharge, the donkey was confined to a box stall for 60 days, followed by a gradual increase in movement to full pasture turnout. The lameness continued to improve, and at 15 months after surgery the donkey was turned out in pasture and had mild lameness (grade, 3/5) at the trot. Mild muscular atrophy of the supraspinatus and infraspinatus muscles was present with no signs of pain on palpation or manipulation of the limb. Shoulder joint radiography at 15 months after surgery revealed remodeling of the glenoid cavity of the scapula and humeral head with formation of a pseudoarthrosis. CLINICAL RELEVANCE Glenoid ostectomy should be considered as an alternative to shoulder joint arthrodesis in small equids with shoulder joint luxation. Other indications for this procedure could include chronic osteoarthritis or fractures affecting the shoulder joint.

Background Although three-dimensional (3D)-printed digital ostectomy templates (DOTs) can help surgeons perform mandibular angle ostectomy (MAO) more precisely and safely, the clinical application of such templates is problematic. Objectives The aim of this study was to evaluate the accuracy of a novel DOT and improve the precision of MAO. Methods A total of 20 patients with a prominent mandibular angle (PMA) were allocated into 2 groups (10 patients per group). A conventional DOT and a novel DOT were applied to guide MAO in Groups A and B, respectively. The mean time taken for curved osteotomy and the volume of postoperative drainage on 1 side within 24 hours were recorded. The deviations between the simulated and postoperative lower border of the mandible were measured on both sides. Results All the patients were satisfied with the cosmetic outcomes. Statistical results showed that the mean time taken for curved osteotomy was shorter in Group B than in Group A, and that the volume of postoperative drainage on 1 side within 24 hours was similar between the 2 groups. The deviations at the anterior and posterior parts of the inferior border showed that the accuracy of osteotomy was higher in Group B than in Group A, and that there was no significant difference between the 2 groups in the middle part. Conclusions The novel DOT is easy to locate and fix tightly, which reduced the operating time and increased the safety and precision of the procedures. Level of Evidence: 4

Purpose Carnoy's solution (CS), the gold standard for adjunctive chemical cautery in treatment of odontogenic keratocysts (OKCs), has been banned for 7 years, leading to substitution with the non-chloroform containing modified Carnoy's solution (MC) without data to support its effectiveness. We performed this study to compare the earlier data with CS to the more current outcomes with MC. Methods A retrospective cohort study was conducted on patients diagnosed with OKC and treated by a single surgeon (GHB) with enucleation and curettage (EC), peripheral ostectomy, and application of CS or MC. The primary predictor variables were use of CS or MC. The primary outcome variables were recurrence (yes vs. no) and time to recurrence. Secondary variables included demographics, anatomic location, and whether teeth adjacent to the lesion were extracted. Statistical analyses included chi-squared test/Fisher's exact test, Wilcoxon rank sum test, and Kaplan-Meier curves. Results 77 patients, 36 patients in the CS group and 41 in the MC group, met inclusion criteria, including at least 1 year of follow-up time. Characteristics of the groups were similar: median age 41.5 and 46, 61% and 71% male gender, 81% and 90% posterior, and 64% and 50% mandibular lesions, respectively. Overall recurrence was similar, 14.29%, with 5 (13.9%) recurrences in the CS group and 6 (14.6%) in the MC group (P = 0.92). Median time to recurrence was 24 months for both groups. Preserving adjacent teeth was associated with a significant increase in recurrence (P = 0.0036). Conclusion Based on this comparison of retrospective outcome data, we found no significant difference in recurrence rate or distribution of time to recurrence between OKCs treated with CS or MC. Aggressiveness of surgical technique is likely a predictive factor in recurrence rate. Future studies should focus on prospective studies and continuing follow-up of the MC group.

The objective of this study was to describe the authors' long-term experience with the management of odontogenic keratocysts (OKCs). All OKC cases treated at the study centre between 1999 and 2015, with a minimum of 5 years of follow-up by December 2019, were reviewed retrospectively. Operative procedures including decompression/marsupialization, enucleation (E), E+Carnoy's solution (CS), E+CS+peripheral ostectomy (PO), and resection were assessed for complete resolution, partial resolution, and recurrence rates. In the parakeratinized non-syndromic group, E+CS+PO resulted in the lowest recurrence rate among the minimally invasive procedures (4.3%), while enucleation resulted in the highest rate (60%). Regarding the other modalities, recurrence was 12.5% for decompression, 11.5% for marsupialization, 16.7% for E+CS, 26.7% for E+PO, and 0% for resection. In the syndromic group, marsupialization resulted in a significantly higher recurrence (23.1%), while E+CS+PO cases showed no recurrence. No recurrence was observed in the orthokeratinized group patients treated with marsupialization or with E+CS. Based on clinico-radiographic features and observed results, it is concluded that OKC, although having a high recurrence rate, is a benign lesion and responds well to conservative procedures in most cases. Radical procedures should be reserved for unresponsive lesions and those with extensive tissue destruction.

Summary Purpose Rhinoplasty is one of the most commonly performed aesthetic surgeries among patients who are admitted to plastic surgeons. Recent research has focused on dorsal preservation in hump reduction and consequently dorsal preservation techniques have become more popular. The current study aimed to revise the push down technique by adding ostectomy. Patients and methods In the present retrospective study, data from patients who underwent rhinoplasty to fix a nasal hump were assessed. All patients were administered the push down technique with ostectomy. Following these inclusion and exclusion criteria, the records of 52 patients were assessed (45 females and 7 males). The median age of the patients was 22.2 years. Patients were evaluated using the “Rhinoplasty Outcome Evaluation” (ROE) questionnaire both before surgery and 12 months after surgery. The follow-up period ranged between 13 and 21 months (median of 15.1 months). Results Patients were evaluated before surgery and after one year of surgery by the ROE scale. The median of the ROE score before surgery was 63.4. The median score after one year of surgery was 91.6. Thus, the ROE score significantly increased 12 months after surgery (P Conclusions This study was the first to demonstrate the benefits of performing the push down technique with ostectomy in terms of obtaining a wider nasal cavity. In addition, it can be assumed that the disadvantage of using the push down technique can be overcome with ostectomy.

Background: Peripheral Cemento-Ossifying Fibroma (PCOF) usually presents as epulis, represents 3.1% of all oral tumors and 9.6% of gingival lesions, is difficult to distinguish clinically and radiographically, requires anatomic histopathological confirmation, peaking in the 2nd and 3rd decades, high recurrence 8-20%. Objective: To report the case management of Peripheral Cemento-Ossifying Fibroma in posterior maxilla with conventional surgery excision surgery, curettage, extraction, and ostectomy. Case: A 42-year-old woman came to the Oral Surgery Clinic at RSUD Haji East Java Province Hospital, complaining of a lump in left maxillary gingiva since 2 years ago, with discomfort, often bitten, initially small, gradually enlarged and never shrinking, does not bleed easily, no lumps elsewhere, with sharp remnants of roots, loose teeth, and a history of vertigo. Discussion: The results of FNAB is Fibromatous Epulis, panoramic radiographic view of the loose periodontal ligament of tooth 27, radiolucent mixed radiopaque clearly demarcated in the distal portion of 27, no visible image of tooth 28. Calculus was cleaned before surgery. Treatments with conventional surgery were excision using a scalpel, curettage, ostectomy, and tooth extraction 27, 38. Histopathological results of postoperative anatomy showed bone trabeculae and “cementum like” tissue in accordance with cemento-ossifying fibroma. The patient was evaluated within 6 months, with no recurrence. Results: Treatment of peripheral cemento-ossifying fibroma with conventional surgery with excision, curettage, ostectomy, and extraction of the causative tooth was successful.

Background: Relationship between periodontal tissue and aesthetic considerations is an important thing to determine the form, function and aesthetics of periodontal tissue itself. For orthodontic cases with gingival enlargement, using the biological width concept in gingivectomy to facilitate an optimal oral hygiene maintenance, function and aesthetic. Bone sounding before gingival recontouring is dictated by the distance from the gingiva crest to alveolar crest. Recommended distance between margins restoration and alveolar bone crest is 3 mm to avoid breaching the biologic width.Case and Case Management: Case 1: A 21 years old female patient whom referred from orthodontist with gingival enlargement in upper front teeth after treated with fixed orthodontic for 1.5 years. PBI: 1,6. After clinical examination, bone sounding was performed = 7mm and gingivectomy without ostectomy was determined. Case 2: A male patient, 24 years old, with gingival enlargement in upper front teeth while treated with fixed orthodontic. After determined the problem, bone sounding (6 mm) was performed along with gingivectomy without ostectomy also for anterior upper right site.Conclusion: The purpose of this report is to provide a diagnostic rationale for gingival recontouring. When gingivectomy is determined, the concept of biological width must be applied achieve a harmonious gingival contour with an optimal oral hygiene maintenance.

Objective To compare three surgical approaches for excision of the zygomatic gland in dogs. Study design Cadaveric study. Animals Cadavers of mesocephalic dogs (n = 20). Methods Each skull was assigned to a lateral approach with zygomatic arch ostectomy on the left (n = 20) and one approach without ostectomy on the right, ventral (n = 10) or dorsal (n = 10) to the zygomatic arch. Approaches were evaluated for surgical exposure (rated on a scale of 1‐5 with one optimal exposure), tissue trauma, and completeness of gland removal. Glands from each side were weighed to compare as internal control. Results The ostectomy‐based approach offered excellent surgical view and good exposure of the zygomatic gland but caused more tissue trauma. The dorsal nonostectomy approach did not allow complete zygomatic gland extraction in nine of the 10 dogs, whereas the ventral nonostectomy approach enabled complete extraction in all 10 dogs. Conclusion The ventral zygomatic approach allowed complete removal of the zygomatic gland, with good surgical overview, while reducing tissue trauma and preserving the zygomatic arch. Clinical significance The ventral nonostectomy approach should be considered as an alternative to excise the zygomatic gland in dogs.

Background The diagnosis and management of juvenile ossifying fibroma (JOF) remains a highly debated topic with paucity of studies with long-term follow-up, hence the aim of this study was to report on the clinico-pathological features and management of these neoplasms. Material and Methods A retrospective analysis was performed on all histopathologically confirmed JOF presenting at two tertiary hospitals in Cape Town, South Africa over a period of 39 years. Clinical, demographic, histopathological and radiological features were analyzed. Surgical methods were documented and a minimum post-operative follow-up of 12 months was a prerequisite. Results Seventeen patients met the inclusion criteria and were included in this study. Overall, the ages of patients ranged from 3–31 years (mean= 13 years) with male to female ratio of 1.8:1. The ages of patients diagnosed with Trabecular JOF were significantly younger than patients with Psammomatoid JOF (P = 0.01). The majority of patients presented with marked swelling (88.2%). Interestingly, most neoplasms occurred in the mandible (76.5%) with all Psammomatoid JOF uncharacteristically occurring in the mandible. There was only one case of Trabecular JOF occurring in the sinonasal area. Most neoplasms appeared as unilocular (76.5%) and well-defined (82.4%) with mixed radio-density (70.6%) on radiographs and computed tomography. Curettage with peripheral ostectomy was shown to be the least invasive method with an accepTable recurrence rate (10%). Six lesions underwent resection without any recurrences however caused high morbidity to these young patients. Conclusions The high number of lesions occurring in the mandible for both variants of JOF demonstrates that site should not be a major determining factor in the diagnosis of JOF. Moreover, curettage with peripheral ostectomy should be used as the first line of management to minimize morbidity to the patient and that resection should be reserved for large and recurrent lesions. It is not unusual for the protruding premaxilla to attain an undesirable position after the cleft lip repair. Such a premaxilla may lead to considerable problems in facial aesthetics, or oral functions, or both in early childhood. These abnormal premaxillas may produce difficulties in bone grafting and orthodontic treatment in late childhood. In such cases, surgical correction of the premaxilla in childhood is justified. From 2013 to 2018, 11 children, aged 2 to 11 years, had a secondary ostectomy of their premaxilla. A new stabilisation method (developed by us) was used to provide rigid fixation to the premaxilla. The follow up period ranged from 1 - 6 years. The results were satisfactory in all except for a few minor issues in three patients. There was significant improvement in their appearance, oral functions, and most importantly in their quality of life. The need for secondary osteotomy of the premaxilla should always be weighed against its potential complications. The fixation technique described by us, though, provides rigid fixation, but may potentially be associated with a few complications if not practised carefully.

Zygoma reduction is indicated in patients where the primary aim of surgery is reduction in the width of cheekbones to achieve smoother and more feminine facial aesthetic lines. Surgeons should evaluate the width of midface (bizygomatic width) and the protrusion of zygoma (volume and position of the zygomatic body) when evaluating patients where such a procedure is indicated. Intraoral high-L osteotomy is the most useful method to successfully treat a wide spectrum of zygomatic protrusions and is widely accepted as the treatment of choice for aesthetic purposes. The amount of ostectomy is determined by evaluating the volume of zygomatic body. The zygomatic body and arch are usually moved posteromedially during surgery; the point of maximal malar projection is evaluated and transposed to a new ideal position. Zygoma reduction can be performed solely or in combination with other facial bone contouring procedures such as mandible reduction, genioplasty, or forehead augmentation. Soft tissue sagging, nonunion, malunion, and paresthesia are the most common complications of this procedure. Undercorrection and asymmetry are the most common aesthetically unfavorable sequelae and should be carefully prevented by proper preoperative planning and meticulous execution of surgical technique.

This study is based on a systematic computer-assisted database that allowed the recording of data from treated OKCs. The following data were recorded for each patient: gender, age, voluptuary habits, comorbidities, site, size, radiographic features, treatment of OKCs, length of hospital stay, complications, recurrence, management and complications of eventual recurrence. A total of 405 patients, 249 male and 156 female, with 415 OKCs (407 sporadic and 8 syndromic lesions) were included in the study: 320 lesions were found in the mandible, whereas 95 were found in the maxilla. In the mandible, the most frequently involved subsite was the angle, whereas in the maxilla it was the molar region. The most frequently performed treatment option was enucleation plus curettage/peripheral ostectomy in 204 OKCs (recurrence rate, 9%). Decompression without residual cystectomy (recurrence rate, 66%), marsupialization with residual enucleation with the use of Carnoy's solution (recurrence rate, 50%), decompression with residual cystectomy (recurrence rate, 43%), and simple enucleation (recurrence rate, 24%) were the treatment options with the highest recurrence rates. An appropriate management of odontogenic keratocysts should be individualized, taking into consideration clinical and radiological findings, as well as patients’ age and comorbidities.
