(1) Background: The overdenture is a complete denture, an implant-supported prosthesis, that the patient can remove at home for the usual oral hygiene procedures, thanks to a simple and intuitive anchoring system. Clinically, the execution of this rehabilitation for the lower arch is often favored, but when it is necessary to limit the extension of the palate in the upper arch, it can represent the least invasive and economic solution. The aim of the study is to analyze post-loading implant loss for implant-supported prostheses in the edentulous upper jaw. (2) Methods: This retrospective study was carried out on patients who received a superior overdenture on four implants for rehabilitation. A total of 42 patients were included in this study and initially evaluated clinically and radiographically. The follow-up period for patients after delivery of the upper overdenture is between 48 and 72 months. A total of 168 implants were inserted and monitored in this period. Clinical and radiographic tests were carried out on all 168 implants, with constant re-evaluation. (3) Results: The overall implant survival rate is 92.9%, a value that corresponds to those present in the literature in previously published studies. There were few prosthetic complications, mainly the detachment of anterior prosthetic teeth. (4) Conclusions: Most of these complete prostheses, which as antagonist had another previously made overdenture on four or on two implants, achieved excellent success rates in this study at 72 months. Overdenture is any removable dental prosthesis that covers and rests on one or more remaining natural teeth, the roots of natural teeth, and/or dental implants. A complete removable denture was the conventional therapy for edentulism for decades. Overdentures can be tooth-supported (immediate/traditional) or implant-supported. As the angulation between the implant axis and the path of insertion increased, the retentive forces decreased among the attachments. However, in the Locator (pink) and OT Equator (clear), a stable retentive force was observed up to 30° without a significant difference and decreased at 45°, particularly in the Locator pink. Both systems have acceptable retention capacities. Retention loss depends on the patient's physiological and systemic conditions. Under the ideal conditions, i.e., when both the attachment caps with similar retentive values, the Locator attachment had shown less retention loss as compared to the OT Equator attachment. The purpose of this multicenter randomized controlled trial was to compare the clinical, radiographic, and patient-centered outcomes of early loaded mandibular overdentures deliberately placed on two or three implants. The outcomes were: implant and prosthesis success and survival rates; biological and technical complications; marginal bone loss; patient satisfaction; and periodontal parameters. The results showed no differences between the groups in any of the outcomes analyzed. With the limitations of the present study, and looking at long-term follow-up, the gold standard of prosthetic rehabilitations with attachments, in agreement with the scientific community, should prefer two non-splinted implants. The aim of the present investigation is to evaluate a case report with angled screwdriver solutions and new kinds of low-profile attachments in full arch rehabilitation with divergent implants. In this clinical case we will analyze how low-profile abutments with angled screwdriver channel in the OT Bridge system (Rhein83, Bologna, Italy) can be a predictable solution over time to create a fixed prosthesis on disparallel implants with a digital structure (New Ancorvis, Bologna, Italy) for the satisfaction of the patient and of the work team. Purpose: The aim of this study was to investigate the stability of three implants supported by a milled bar, which carries two different attachment systems and submitted to immediate loading. Methodology: The current research was a prospective study conducted according to split-mouth design with random allocation of the tested attachments. Two types of attachments were used, OT Equator and OT Cap attachment. A total of 8 completely edentulous patients were divided into 2 Groups according to OT Equator position. CBCT was used for the planning and construction of the surgical stent. A milled bar with the attachments was fabricated prior to implant placement. Each patient received three implants in the interforaminal region, connected with a milled bar. On one side of the bar; OT Equator attachment was used, while the other side was supplied with OT Cap attachment. Overdenture was delivered at an average time of 10 to 15 days after implantation. Follow- up period was 12-months after loading. Implant stability was evaluated using Osstel TM. Results: The results revealed that; there were no significant differences between all groups at implant placement time and at 12-months period for all implants and through different time intervals until the followup period was finished. Conclusion: The results of the current study indicated an increase in implant stability and due to splinting of the implants by using immediate loading protocol. Splinting of the implants using a rigid bar appears to be a successful treatment option that could be considered for treating the completely edentulous patient. [ABSTRACT FROM AUTHOR]

Objective In this research it was aimed to evaluate stress distribution on the implants supporting a complete overdenture in addition to compare between two different types of low-profile attachments for implant-retained mandibular overdenture with two techniques (with/without using connecting bar). Materials and methods Two 3D finite element models were constructed simulating supported lower complete overdenture with two implants and with two implants and bar. Where, models components were modeled in 3D on commercial general purpose CAD/CAM software. Four runs were carried out, two runs on each model, as linear static analysis. Results Using bar is generally preferred for mucosa and cortical bone, while its effect can be considered as negligible on overdenture. On the other hand, it slightly increases the stresses on spongy bone. Using bar ensures the same level of energy transfer to the spongy bone and increases its maximum Von Mises stresses by about 50%. In addition, increase in maximum Von Mises stress was noticed by about 1% on cortical bone. Conclusion Using bar is not recommended for patients with flat ridge. [ABSTRACT FROM AUTHOR]
