7. Project ASPIRE: A feasibility randomized controlled trial of a brief intervention for reducing risk of depression and alcohol-related harms among South African adolescents.

Brief interventions could reduce adolescents' risk of depression and alcohol-related harms, but evidence of their feasibility and acceptability for low-and middle-income countries is lacking. To address this gap, we conducted a feasibility trial of the ASPIRE intervention, a four-session multi-component counselling intervention for South African adolescents. We recruited 117 adolescents who met our inclusion criteria. Participants were randomly assigned to the ASPIRE intervention or a comparison condition. Outcomes were assessed at baseline, six-week, and three-month post-randomization time points. Primary outcomes were based on feasibility of study procedures and intervention delivery (assessed on seven predetermined progression criteria). Clinical outcomes (risk of depression and alcohol harms) were secondary. Despite modifications to all study procedures arising from Covid-19 restrictions, five of the seven key progression criteria were fully met, including: feasibility of data collection and outcome measures, counsellor competencies, randomization and blinding, adverse advents, and acceptability of the intervention. The progression criterion for recruitment and intervention retention were not fully met. Findings suggest that the ASPIRE intervention was generally feasible to deliver and acceptable to adolescents. However, modifications to the trial design and intervention delivery are needed to optimize the validity of a definitive randomized controlled trial of the ASPIRE intervention. [ABSTRACT FROM AUTHOR]

8. The p53 tumour suppressor gene and the tobacco industry: research, debate, and conflict of interest

- Abstract
Mutations in the p53 tumour suppressor gene lead to uncontrolled cell division and are found in over 50% of all human tumours, including 60% of lung cancers. Research published in 1996 by Denissenko and colleagues demonstrated patterned in-vitro mutagenic effects on p53 of benzo[a]pyrene, a carcinogen present in tobacco smoke. We investigated the tobacco industry's response to p53 research linking smoking to cancer. We searched online tobacco document archives, including the Legacy Tobacco Documents Library and Tobacco Documents Online, and archives maintained by tobacco companies such as Philip Morris and R J Reynolds. Documents were also obtained from the British American Tobacco Company depository in Guildford, UK. Informal correspondence was carried out with scientists, lawyers, and tobacco control experts in the USA and Europe. We found that executives and scientists at the highest levels of the tobacco industry anticipated and carefully monitored p53 research. The tobacco industry's own scientists conducted research which appeared to cast doubt on the link between smoking and p53 mutations. Researchers and a journal editor with tobacco industry ties participated in the publication of this research in a peer-reviewed journal without clear disclosure of their tobacco industry links. Tobacco industry responses to research linking smoking to carcinogenic p53 mutations mirror prior industry efforts to challenge the science linking smoking and lung cancer. The extent of tobacco industry involvement in p53 research and the potential conflict of interest discussed here demonstrate the need for consistent standards for the disclosure and evaluation of such potential conflicts in biomedical research.

BACKGROUND: The existing World Health Organization (WHO) pneumonia case management guidelines rely on clinical symptoms and signs for identifying, classifying, and treating pneumonia in children up to 5 years old. We aimed to collate an individual patient-level data set from large, high-quality pre-existing studies on pneumonia in children to identify a set of signs and symptoms with greater validity in the diagnosis, prognosis, and possible treatment of childhood pneumonia for the improvement of current pneumonia case management guidelines.METHODS: Using data from a published systematic review and expert knowledge, we identified studies meeting our eligibility criteria and invited investigators to share individual-level patient data. We collected data on demographic information, general medical history, and current illness episode, including history, clinical presentation, chest radiograph findings when available, treatment, and outcome. Data were gathered separately from hospital-based and community-based cases. We performed a narrative synthesis to describe the final data set.RESULTS: Forty-one separate data sets were included in the Pneumonia Research Partnership to Assess WHO Recommendations (PREPARE) database, 26 of which were hospital-based and 15 were community-based. The PREPARE database includes 285 839 children with pneumonia (244 323 in the hospital and 41 516 in the community), with detailed descriptions of clinical presentation, clinical progression, and outcome. Of 9185 pneumonia-related deaths, 6836 (74%) occurred in children CONCLUSIONS: This data set could identify an improved specific, sensitive set of criteria for diagnosing clinical pneumonia and help identify sick children in need of referral to a higher level of care or a change of therapy. Field studies could be designed based on insights from PREPARE analyses to validate a potential revised pneumonia algorithm. The PREPARE methodology can also act as a model for disease database assembly.

Background: HIV elimination in South Africa calls for innovative approaches. We investigated HIV self-testing (HIVST) for increasing demand for HIV care and prevention services, comparing two peer-distribution approaches (direct distribution and an incentivized social-network approach) against peer-navigators providing information and referrals only. Methods: Restricted randomisation (1:1:1) allocated 24 peer-navigator pairs in rural Kwa-Zulu Natal into: (1) incentivized-peer-networks (IPN): peer-navigators recruited 'seeds' to distribute 5 packs to 18 to 30 year olds within their social networks. Packs contained 2 HIVST kits (OraQuick) and standard of care (SOC) materials. 'Seeds' received 20 Rand (US$1.5) for each recipient who went on to distribute packs themselves; (2) peer-navigator-distribution (PND): peer-navigators distributed HIVST packs and SOC materials directly; (3) SOC: peer-navigators distributed barcoded clinic referral slips and sexual health information promoting HIV testing, pre-exposure prophylaxis (PrEP) and antiretroviral therapy (ART). PrEP/ART linkage rates were defined as numbers screened for PrEP eligibility or starting ART within 90 days of referral slip distribution per peer-navigator outreach month (pnm). The primary outcome compared PrEP/ART linkage rates between arms for women aged 18 to 24 years. Secondary outcomes included linkage rates for youth (18 to 30 years). Intention to treat analysis was used. Investigators and statisticans were blinded to allocation. Results: Between March and December 2019, 7563 (2055 IPN, 2069 PND, 2539 SOC) packs were distributed during 144 peer-navigator outreach months, with 272 young adults linked to PrEP/ART (1.9/pnm). Linkage rates for women aged 18 to 24 years were lower for IPN (n = 26, 0.54/pnm) than PND (n = 45, 0.80/pnm) and SOC n = 49, 0.85/pnm), although not significantly so (rate ratios [RR] 0.68, 95% CI 0.28 to 1.66 and 0.64, 95% CI 0.38 to 2.36, respectively). Adding HIVST kits to peer-navigator distribution (PND vs SOC) did not change linkage rates (RR 0.95, 95% CI 0.38 to 2.36). Youth (18 to 30 years), results were similar but with stronger evidence of lower linkage rates (0.88/pnm) for IPN than PND (2.11/pnm, RR 0.42, 95% CI 0.18 to 0.98) and SOC (2.07/pnm, RR 0.42 95% CI 0.18 to 1.02). Conclusions: Peer-based HIVST distribution reached large numbers of young men and women, but did not increase demand for PrEP/ART, unless combined with peer-navigator PrEP/ART promotion. Increasing facility-based delivery rates is pivotal to reach Sustainable Development Goals to improve skilled attendance at birth and reduce maternal and neonatal mortality in low- and middle-income countries (LMICs). The translation of global health initiatives into national policy and programmes has increased facility-based deliveries in LMICs, but little is known about the impact of such policies on social norms from the perspective of women who continue to deliver at home. This qualitative study explores the reasons for and experiences of home delivery among women living in rural Zimbabwe. We analysed qualitative data from 30 semi-structured interviews and 5 focus group discussions with women who had delivered at home in the previous 6 months in Mashonaland Central Province. We found evidence of strong community-level social norms in favour of facility-based delivery. However, despite their expressed intention to deliver at a facility, women described how multiple, interacting vulnerabilities resulted in delivery outside of a health facility. While identified as having delivered 'at home', narratives of birth experiences revealed the majority of women in our study delivered 'on the road', en route to the health facility. Strong norms for facility-based delivery created punishments and stigmatization for home delivery, which introduced additional risk to women at the time of delivery and in the postnatal period. These consequences for breaking social norms promoting facility-based delivery for all further increased the vulnerability of women who delivered at home or on the road. Our findings highlight that equitable public health policy and programme designs should include efforts to actively identify, mitigate and evaluate unintended consequences of social change created as a by-product of promoting positive health behaviours among those most vulnerable who are unable to comply.

Public action by cities faces the challenge to foster the ‘qualities of place’ of cities in a new stage of urbanization that is characterized by the dynamic external interconnectivity of social and economic activities, Moreover, this challenge includes addressing new spatial patterns, that are more polycentric and decentralized, at enlarged levels of regional scale. This is a radical transition that entails a completely new and complex urban agenda. In order to define the urban challenge precisely, the author makes use of Kevin Lynch’s conceptualization of the qualities of urban places. This transition involves both the institutional side (the changing set of public norms) and the pragmatic side of public action (the public intentions, purposes and experiments of action), and their interaction. Thus, the study of public action is conceptualized as a dynamic interaction between the changing institutional conditions and the pragmatic policies and experiments. The potential of this public action in the changing ecology of city-regions is exemplified with ‘hardware’ cases of urban infrastructure and with ‘software’ cases such as the exploration of symbolic representation.

El objetivo de esta investigación fue realizar una intervención educativa con el propósito de potenciar los conocimientos de los participantes acerca de la importancia del cuidado del medio ambiente y la megadiversidad en el país y a partir de ello, buscar generar una conciencia protectora que permee en su rol de ciudadanos y futuros profesionales en sus respectivos campos laborales. Este trabajo presentó una lógica deductiva, de alcance descriptivo y enfoque cuantitativo. La muestra incluyó a 20 participantes de la carrera profesional de Farmacia y Bioquímica de la Universidad Científica del Sur (Lima - Perú) que cursan el segundo año de formación profesional. Se empleó como técnica la encuesta y el cuestionario como instrumento, siendo ejecutado este en dos instancias distintas. Al inicio de la aplicación del test se obtuvo que los participantes alcanzaron un nivel insuficiente 50%, en proceso 35%, satisfactorio 15% y destacado 0%, luego de realizar la intervención educativa y aplicar nuevamente el test se alcanzó un nivel insuficiente 0%, en proceso 10%, satisfactorio 30% y destacado 60%. Se potenciaron los conocimientos de los participantes acerca de la importancia del cuidado del medio ambiente y la megadiversidad en el país, asimismo se generó una conciencia positiva sobre la protección de nuestro ecosistema desde nuestra posición de ciudadanos y profesionales.

- Abstract
INTRODUCTION: Pneumonia is a frequent and costly cause of emergency department (ED) visits and hospitalisations in children. There are no evidence-based, validated tools to assist physicians in management and disposition decisions for children presenting to the ED with community-acquired pneumonia (CAP). The objective of this study is to develop a clinical prediction model to accurately stratify children with CAP who are at risk for low, moderate and severe disease across a global network of EDs. METHODS AND ANALYSIS: This study is a prospective cohort study enrolling up to 4700 children with CAP at EDs at ~80 member sites of the Pediatric Emergency Research Networks (PERN; https://pern-global.com/). We will include children aged 3 months to <14 years with a clinical diagnosis of CAP. We will exclude children with hospital admissions within 7 days prior to the study visit, hospital-acquired pneumonias or chronic complex conditions. Clinical, laboratory and imaging data from the ED visit and hospitalisations within 7 days will be collected. A follow-up telephone or text survey will be completed 7-14 days after the visit. The primary outcome is a three-tier composite of disease severity. Ordinal logistic regression, assuming a partial proportional odds specification, and recursive partitioning will be used to develop the risk stratification models. ETHICS AND DISSEMINATION: This study will result in a clinical prediction model to accurately identify risk of severe disease on presentation to the ED. Ethics approval was obtained for all sites included in the study. Cincinnati Children's Hospital Institutional Review Board (IRB) serves as the central IRB for most US sites. Informed consent will be obtained from all participants. Results will be disseminated through international conferences and peer-reviewed publications. 40. Low-Latitude Origins of the Four Phanerozoic Evolutionary Faunas

- Abstract
Sepkoski’s hypothesis of Three Great Evolutionary Faunas that dominated Phanerozoic oceans represents a foundational concept of macroevolutionary research. However, the hypothesis lacks spatial information and fails to recognize ecosystem changes in Mesozoic oceans. Using a multilayer network representation of fossil occurrences, we demonstrate that Phanerozoic oceans sequentially harbored four evolutionary faunas: Cambrian, Paleozoic, Mesozoic, and Cenozoic. These mega-assemblages all emerged at low latitudes and dispersed out of the tropics. The Paleozoic–Mesozoic transition was abrupt, coincident with the Permian mass extinction, whereas the Mesozoic–Cenozoic transition was protracted, concurrent with gradual ecological shifts posited by the Mesozoic Marine Revolution . These findings support the notion that long-term ecological changes, historical contingencies, and major geological events all have played crucial roles in shaping the evolutionary history of marine animals. One Sentence Summary Network analysis reveals that Phanerozoic oceans harbored four evolutionary faunas with variable tempo and underlying causes.

A method using a double-selective culture system was developed to aid environmental surveillance in monitoring the circulation of wild-type poliovirus in sewage systems. The performance of the method was tested by using it to process 1,700 sewage samples carrying both vaccine polioviruses and other enteroviruses. Results indicate that the double-selective culture system method has a poliovirus detection limit of 18 to 50 PFU per one to two liters of sewage and was able to identify 41 wild-type polioviruses.
