2. Radiation exposure to patients undergoing percutaneous transhepatic cholangiography and endoscopic retrograde cholangiography: an advanced phantom study.

Abstract

Fluoroscopic examinations like Endoscopic Retrograde Cholangiopancreatography (ERCP) and Percutaneous Transhepatic Cholangiography (PTC) are fundamental in diagnosing and treating hepatobiliary diseases. However, these procedures expose patients to significant radiation, highlighting the need for a detailed assessment of the radiation doses received by critical organs. The study's primary objective is to determine the experimental doses received by critical organs in patients undergoing these procedures. This study utilized an Alderson RANDO phantom outfitted with Thermoluminescent Dosemeters (TLDs) to experimentally measure the radiation doses received by various organs during ERCP and PTC procedures. This method provided direct and accurate data on organ-specific radiation exposure, contrasting with the traditional approach of relying on theoretical simulations. The analysis revealed that PTC generally results in higher radiation doses to organs compared to ERCP. Critical organs, such as the thyroid, spleen, liver, pancreas, ovaries, and uterus, were exposed to varying levels of radiation, with the thyroid and spleen receiving particularly high doses in PTC. The study also demonstrated that the per-minute radiation exposure was consistently higher in PTC across all examined organs. The study's findings underscore the significant radiation exposure associated with ERCP and PTC, with PTC posing a greater risk. Understanding these exposure levels is crucial for clinical decision-making, particularly when considering patients' pre-existing conditions and sensitivity to radiation. The study highlights the need for clinicians to carefully weigh the benefits of ERCP and PTC against the potential radiological risks. It suggests a preference for ERCP in situations where radiation exposure needs to be minimized. Furthermore, the findings advocate for ongoing advancements in medical imaging techniques to reduce radiation exposure, emphasizing the importance of patient safety in fluoroscopic examinations. Abstract

This study aimed to share our experiences with children who have primary tuberculous peritonitis, which is difficult to diagnose, who underwent diagnostic laparoscopy. The study included two boys (8 and 9 years of age) and two girls (10 and 14 years of age). Three of the patients had complaints of abdominal pain for 1-2 months, and one patient had a mass lesion in the epigastric region for 3 months. All patients' tuberculosis skin tests were negative. Acid-fast bacilli were not detected in throat, sputum, or tissue cultures. Peritoneal thickening was detected on abdominal computed tomography in all patients. Laparoscopic peritoneal biopsy was performed in all patients. Pathology results revealed chronic granulomatous inflammation in the patients. During postoperative follow-up, subileus findings were observed in one patient, and proximal jejunal perforation developed in one patient. Diagnostic laparoscopy is thought to be a useful diagnostic tool for both sampling and morphological identification of lesions in tuberculous peritonitis. [ABSTRACT FROM AUTHOR]

Abstract

Background. Acute pancreatitis (AP) is a disease that can still be fatal despite rapid advances in medicine. The relationship between serum nesfatin-1 levels and AP is still to be fully resolved. Objectives. To investigate the utility of serum nesfatin-1 levels in the diagnosis of AP. Materials and methods. Twenty-four male Sprague Dawley rats were divided into control, mild pancreatitis and severe pancreatitis groups (n = 8/group). Acute pancreatitis was induced by cerulein injection and the control group received saline injections. Then, the serum nesfatin-1, amylase, lipase, aspartate aminotransferase (AST), and alanine aminotransferase (ALT) levels were determined. A pathologist blinded to the study scored the severity of pancreatitis. Results. There was a considerable decrease in serum nesfatin-1 levels in parallel to the severity of pancreatitis, though there was no statistically significant relationship observed between pancreatitis and nesfatin-1. In addition, there was no significant difference in AST or ALT levels among the groups. However, a strong positive correlation between amylase and lipase levels was observed (p < 0.05). The severe pancreatitis group (group 3) had a higher lipase level and pathology score than mild pancreatitis group (group 2), and this difference was statistically significant. Conclusions. Serum nesfatin-1 may be used as a diagnostic and severity marker in pancreatitis in the future. [ABSTRACT FROM AUTHOR]

Abstract

Purpose: The incidence of pediatric nephrolithiasis/urolithiasis (NL/UL) has increased in the last few decades due to dietary habits and a sedentary lifestyle. This study aimed to determine the etiological factors and six months of follow-up results of patients with NL/UL in pediatric nephrology and urology clinics in our center. Materials and Methods: Children with NL/UL between December 2018 and December 2020 were enrolled in this study. The medical records were reviewed to collect data about admission complaints, consanguinity, metabolic risk factors for NL/UL, medical treatments, surgical interventions, and presence of stones after 6 months of follow-up.Results: A total of 93 patients (46 male and 47 female) were included in the study. Consanguinity and a positive family history for stones was present in 46.2% and 78.5% of patients, respectively. The urinary metabolic risk factors were hyperuricosuria (11/83, 13.3%), hypercalciuria (12/93, 12.9%), hyperoxaluria (6/51, 11.8%), cystinuria (7/90, 7.8%), and hypocitraturia (2/41, 4.9%). Twenty patients (29.9%) with NL/UL and without cystinuria received potassium citrate therapy. After 6 months of follow-up (n = 27) of these patients, stone progression was not observed in any of the patients who received potassium citrate treatment (n=11) or underwent surgical intervention (n=10). Conclusions: Most patients had a family history of stones. Hyperuricosuria was the most common urinary risk factor for NL/UL. Potassium citrate therapy might be useful to prevent stone progression in patients with NL/UL., Amaç: Çocuklarda böbrek taşı sıklığı, son zamanlarda beslenme alışkanlıklarındaki değişikliklere ve sedanter yaşama bağlı olarak artış göstermiştir. Bu çalışmada, merkezimizde Çocuk Nefroloji ve Çocuk Üroloji Klinikleri’nde böbrek taşı/üreter taşı (BT/ÜT) tanılarıyla takip edilen hastaların etiyolojik faktörlerini ve altı aylık izlem sonuçlarını değerlendirmek amaçlandı. Gereç ve Yöntem: Aralık 2018- Aralık 2020 arasında BT/ÜT tanılarıyla Çocuk Nefroloji ve Üroloji Klinikleri’nde takip edilen hastalar çalışmaya dahil edildi. Hastaların başvuru şikayetleri, anne-baba akrabalığı, BT/ÜT açısından metabolik risk faktörleri, aldıkları tedaviler, cerrahi operasyonlar ve taşların varlığının 6 aylık takip sonucunda devam edip etmediği bilgileri kayıtlardan elde edildi. Bulgular: Toplam 93 hasta (46 erkek, 47 kız) çalışmaya dahil edildi. Anne-baba akrabalığı hastaların %46,2’sinde mevcut iken, ailede taş öyküsü hastaların %78,5’inde saptandı. İdrarda bakılan metabolik risk faktörlerinden hiperürikozüri 11/83 (%13,3) hastada, hiperkalsiüri 12/93 (%12,9) hastada, hiperokzalüri 6/51 (%11,8) hastada, sistinüri 7/90 (%7,8) ve hipositratüri 2/41 (%4,9) hastada saptandı. Sistinüri olmadan BT/ÜT saptanan 20 hasta (%29.9) potasyum sitrat tedavisi almaktaydı. Altı ay takip edilen hastalar (n=27) içinde potasyum sitrat tedavisi alan (n=11) veya cerrahi operasyon uygulanan hastaların (n=10) hiçbirinde taşın boyutunda artış saptanmadı. Sonuç: Hastaların çoğunda ailede taş öyküsü saptandı. Hiperürikozüri, BT/ÜT için en sık görülen risk faktörüydü. Potasyum sitrat tedavisi BT/ÜT’li hastalarda taş progresyonunun önlenmesinde faydalı olabilir.

Abstract

A 54-year-old male was admitted to our department with neurofibromatosis and hypertension. During his examination, a mass was detected in the abdomen, and he was transferred to a surgical clinic. At the first examination of the patient, extensive café-au-lait spots and granulomas were detected on the body and the mass occupying right abdomen quadrant was palpable. The patient's medical history indicated that he had hypertension for almost a decade. The patient also stated that nodules on the body existed from his earliest recollection and he had relatives with neurofibromatosis. The patient was taken to a surgical operation. A mass with 30x23 cm in size was removed. The area of the nodular structure, with 0.5 cm in diameter, in the stomach serosa was also removed. The tumor was composed of phaeochromocytoma in the larger spaces and ganglioneuromas in the relatively narrow spaces. The nodular area removed in gastric serosa was reported as a very low-risk gastrointestinal stromal tumor. Apart from this rare combination, adrenal mass removed from the patient was considerably larger than the masses in the literature until now. Abstract

Background: Fecal microbiota transplantation is a promising method to solve obesity. Our study-s aim was to investigate the changes of weight and ghrelin levels in obese rats receiving fecal microbiota transplantation from lean rats. Methodology: Twenty-one rats were divided into three equal groups: Group 1: Obese control group; group 2: Obese recipient group; and group 3: Lean donor group. Feces which was collected from donor group was transferred to the rats in recipient group, orally by gavage, 3 times every other day. The weight and ghrelin levels were measured from each rat at the beginning and end of the study. Results: There was statistically significant weight gain in donor group (p: 0.001), but there were no statistical significant weight chances was detected in control and recipient groups (p: 0.82, p: 0.12, respectively). There was an increase in donor and control groups, but a decrease was observed in the recipient group at ghrelin levels. However, there was no significant difference at ghrelin levels in any groups (p: 0.05, p: 0.2, p: 0.4, respectively). There was a significant relation in control group in weight and ghrelin changes (p: 0.007), but no significant relation was detected in either recipient or donor groups (p: 0.29, 0.53, respectively). Conclusion: Metabolism changes of obese rats were observed after fecal microbiota transplantation, and it was the only group that decreasing ghrelin levels.

45. Çocukluk dönemi testis torsiyonu sonuçlarının değerlendirilmesi

Abstract

Amaç: Testis torsiyonu akut skrotum nedenlerinden olup acil cerrahi yaklaşım gerektirmesinden dolayı önemlidir. Erken tanıda cerrahi tedavinin başarısı oldukça yüksektir. Bu çalışmada testis torsiyonu tanısı almış olan hastaların geriye dönük olarak değerlendirilmesi amaçlanmıştır. Gereç ve yöntem: Testis torsiyonu nedeni ile ameliyat edilen 25 çocuk çalışmaya dahil edildi. Hastaların başvuru yakınmaları, ağrının başlama zamanı, tanı yöntemleri ve eksplorasyon bulguları değerlendirildi. Bulgular: Çocukların yaş aralığı 5 ile 17 yaş arasında olup, ortalama yaş 14.5 ± 3.2 yıl idi. Hastaların tamamına yakınında (%96) başvuru yakınması sadece skrotal ağrı idi. Hastaların hepsinde skrotal hassasiyet mevcuttu. Testis torsiyonunda tanı skrotal renkli doppler ultrasonografi ile desteklendi. Yakınmaları 72 saatten önce başlamış 18 hastada detorsiyon yapıldı (%75). Yetmiş ikinci saatten sonra getirilen beş hastada orşiektomi yapıldı (%20,8). Bir hastada (%4) appendiks testis torsiyonu nedeni ile eksplorasyon yapıldı. Sonuç: Çocuklarda fizik inceleme ve skrotal renkli doppler ultrasonografi ile testis torsiyonu tanısı rahatlıkla konulabilmektedir. Şikayetlerin başlamasından 72 saat sonra getirilen olgularda testis kaybının olduğu görüldü. Erken ve doğru tanı sonrasında cerrahi tedavinin yapılması, bu çocuklarda morbidite ve organ kaybının önlenmesini olumlu yönde etkilemektedir.Anahtar Kelimeler: Çocuk, testis, torsiyon

46. Çocuklarda yüksekten düşme sonucu oluşan travmaların değerlendirilmesi

Abstract

Amaç: Çocukluk çağı travmalarının en sık nedeni yüksekten düşme olup travma sonrası morbidite ve mortalite halen önemini korumaktadır. Bu çalışma yüksekten düşmeye bağlı yaralanma tiplerini, yaşlara göre oluşan travmayı, travma sonrası takip ve tedavi şekillerini incelemeyi amaçlamaktadır. Yöntem: Çocuk acil servise yüksekten düşme nedeni ile gelen ve çocuk cerrahi bölümünce takip edilen 0-18 yaş arası 56 çocuk hastanın dosyaları hastane bilgi işlem sistemi kullanılarak geriye dönük olarak incelendi. Hastalara ait demografik veriler ve her hasta için hesaplanan pediatrik travma skoru, yaş, yükseklik, travma sonucu gelişen yaralanmanın türü ve tedavi yöntemleri için tanımlayıcı istatistikler yapıldı. Bulgular: Çalışmaya 56 çocuk hasta (n:39 erkek, n:17 kız) alındı. Olguların yaş ortalaması 7.9 ± 5.1 idi. Travmanın en sık görüldüğü yaş; 9 (n:6, %10.1) idi. En sık yüksekten düşülen yer apartman balkonu (n:36, %64), ikinci sıklıkla ağaç dalı (n:10, %17) idi. En düşük 1 metre ve en yüksek 52 metreden düşme görüldü. Ortalama yükseklik 8 metre (median:4) idi. Oluşan travmalar arasında en sık baş boyun yaralanmaları (n:27, %48), ikinci sıklıkta ekstremite kırıkları (n:16, %28) saptandı. 21 (%37.5) olguya cerrahi müdahale yapılırken, 35 (%62.5) olguya sadece tıbbi tedavi uygulanmıştır. Yüksekten düşme sonrasında iki hastada ise ölüm gerçekleşmiştir (%3.5). Sonuç: Yüksekten düşme sonucunda çocuklarda ciddi yaralanmalar olabilmektedir. Yüksekten düşme olgularının azaltılması için toplumsal eğitim verilmesi, özellikle çok katlı apartmanlarda oturan ailelere balkon ve pencerelerden çocukların düşmemesi için alınması gereken tedbirlerin anlatılması gerekmektedir.Anahtar Kelimeler: Düşme, çocuk, travma
